The goal of this project was to enable the solution of hard formal verification problems - and thereby reduce the cost and improve the quality of advanced hardware designs - by making it possible to combine different verification techniques in flexible ways. This was be accomplished by developing an open software environment that allows various techniques to be combined easily. Various prototype tools were developed to support and use this environment, and results were evaluated on some example designs of interest. There were significant accomplishments in several areas. We developed a better understanding of design and implementation issues in building a federated environment. SRJ's widely-used PVS theorem prover was modified to provide APIs (application program interfaces) enabling it to be used in a variety of other tools. The enhanced version of PVS was used in the successful verification of hardware and safety-critical systems by other groups. We implemented a general-purpose system of cooperating decision procedures for quantifier-free first-order logic formulas, called SVC. This system integrated several decision procedures into a single framework, and was also used as a software component in several different verification tools, so it was "federated" at two different levels. The source code for the SVC system is freely available, and the system has been extensively used inside and outside of Stanford. Later, work was begun on re-engineering SVC to be more flexible, powerful, and efficient. We developed and prototyped new techniques for checking table-based specifications of embedded software systems, in the RSML description language. We analyzed and found some problems in an RSML specification of the safety-critical TCAS collision avoidance system. A tool for doing approximate symbolic model checking was prototyped and applied to an example design from another group at Stanford.

Quality Control Review Objective: As the cognizant Federal agency for SRI International, the Office of the Inspector General (OIG), DoD, performed a quality control review to determine whether the FY 1999 report that SRI International submitted to the Single Audit Clearinghouse met the applicable reporting standards and OMB Circular A-133; and whether KPMG LLP and DCAA conducted the audit as required by applicable standards and OMB Circular A-133.

The U.S. Census Bureau is conducting the 2000 decennial census. The accuracy of this census depends in part on the proper functioning of 10 interrelated information systems, one of which is the Bureau's headquarters (HQ) processing system. Given the criticality of this system, the General Accounting Office (GAO) was asked to (1) identify the nature and status of the HQ processing system and (2) assess the quality of the system and the risks facing the Bureau if effective quality controls are not in place. GAO performed its work from July through September 2000 in accordance with generally accepted government auditing standards. The HQ processing system consists of 48 applications, all developed internally by the Bureau, that support a variety of census operations such as updating address files, creating a file of census responses, and preparing data for tabulation and dissemination. The Bureau reported that, as of July 20, 2000, 31 of the 48 applications were complete and supported or are supporting census 2000 operations. The Bureau lacks effective, mature software and system development processes to control development of its HQ processing system applications. It relies instead on the efforts of individuals to deliver applications on time and within budget-an approach that increases the risk that the applications will not be available when needed and/or perform as intended. As a result, the Bureau lacks adequate assurance that the functions performed by the HQ processing system applications-such as ensuring accurate and complete address files and identifying the correct households for enumerators to contact are properly executed., Report to the Subcommittee on the Census, Committee on Government Reform, House of Representatives.

This Special Publication (SP) introduces a brief overview of concrete repair guidance offered by the American Concrete Institute (ACI) International, the International Concrete Repair Institute (ICRI), the American Concrete Pavement Association (ACPA), the Aberdeen Group, the Portland Cement Association (PCA), and work by authors considered experts in the field of concrete repair. The publication is divided into two parts: Part I, "Condition Assessment," and Part II, "Concrete Repair." Part I details condition survey, concrete evaluation, evaluation of structures, and additional surface evaluation; Part II introduces concrete removal, surface preparation, repair materials, placement methods, curing, quality control, and repair guidance.

In the Appraiser Program of the Software Engineering Institute (SEI), authorized Lead Assessors lead Capability Maturity Model-Based Appraisals for Internal Process Improvement (CBA IPI). At the conclusion of each assessment, they are required to submit certain artifacts to the SEI. Data from assessments is recorded to provide the community with information on the state of the software community's process maturity, as related to the Capability Maturity Model (CMM) for Software Version 1.1. These data can be viewed on the SEI Web site: . Additional feedback data are required of a Lead Assessor in order to monitor the consistency of use of the assessment method for quality control purposes. Data are collected from Lead Assessors, assessment team members, and sponsors of the assessments. The results reported in this document reflect information sent to the SE by Lead Assessors through a Lead Assessor's Requirements Checklist. The checklist aids the Lead Assessors in keeping track of their implementation of each of the method's requirements. The checklist also provides information back to the community regarding metrics being reported by Lead Assessors; this helps in more effective planning for future assessments. In addition, the checklist acts as a quality control mechanism to monitor the consistency of use of each of the method's activities.

The information contained in this report is intended to assist both Navy and shipyard/contractor personnel engaged in the welding and inspection of commercially pure titanium seawater piping systems for U.S. Navy surface ships. This report provides recommendations, requirements and guidance information on quality control, materials, welding and inspection for fabrication of titanium pipe welds. The fabrication requirements contained in this report are taken from NAVSEA technical publication S9074-AR-GIB-010/278 Requirements for Fabrication Welding and Inspection, and Casting Inspection and Repair for Machinery, Piping and Pressure Vessels. The mandatory requirements, as indicated by the word "shall," are considered to be the minimum acceptable for first quality workmanship. Recommendations and guidance information is not mandatory. The contents of this report are drawn from many sources and have been selected to complement numerous NAVSEA publications without in any way modifying those documents.

Research under this grant has dealt with the general area of reliability theory and quality control, with emphasis on problems emanating from software engineering, software reliability and testing. In all, 6 papers have been published or accepted for publication in the open literature An innovation has been the development of the topic of warranties which seems to have growing importance especially with regard to military hardware and software systems.

This report is an introduction to the concepts which must be understood and the issues which must be resolved in order to produce quality software. Quality software is defined. Motivation for studying software quality, in the form of examples of past software fiascos, is presented. Various approaches for producing software quality, including better evaluation techniques, better quality measurement, better development processes, and better development tools, are each addressed in separate chapters. An annotated bibliography is provided.

This audit was performed in response to a DoD Hotline complaint regarding DoD procurement of aircraft fuel cells. The complainant alleged that foreign-manufactured fuel cells were of better quality and had a longer useful life than domestically manufactured fuel cells. The complainant also alleged that domestic manufacturers were attempting to eliminate foreign competition to enable domestic manufacturers to dictate prices. This report was based on our evaluation of the quality and pricing of foreign and domestic fuel cells procured by DoD for two types of Navy and Air Force helicopters. We evaluated the quality and pricing of Army helicopter fuel cells in Inspector General, DoD, Report No. 94-001, "Aircraft Fuel Cell Procurements,'? October 13, 1993. A mechanical testing program was developed for evaluation of the weakening effects of different types of commonly encountered flaws on the strength of composite boat hulls. A specification was developed for production of test coupons containing realistic and consistently reproducible simulated defects. Tensile and flexural testing procedures were specified, including development of a technique for tensile testing of cored specimens. A statistically-based test plan was developed. A mechanical testing program was conducted in which approximately 400 specimens were subjected to either tensile or flexural testing. this program was conducted to verify specimen production techniques, to validate mechanical testing procedures, and to acquire information about the statistical variability of test results, which is necessary for planning future experiments. The data from the testing also supported a limited number of conclusions about the effects of defects on the strength of composite specimens of solid, balsa-cored, and plastic foam-cored construction.... Previous studies of spray formed material at the Carderock Division, Naval Surface Warfare Center, Annapolis Detachment, have indicated that porosity is one of the few component characteristics affected by changes in process parameters. As a result, porosity has often been directly linked to quality. In this paper, the relationship between porosity and mechanical properties is examined in an effort to prove that porosity does indeed indicate quality. In general, increases in porosity lead to decreases in strength and hardness. This allows the generalization that higher porosity levels indicate lower quality material.

Equations that provide approximate but highly accurate lower confidence limits on process capability indices are provided. These equations yield the same value to the nearest hundredth or better than the values provided in tables in the literature. These equations can be used for a much more extensive set of data than those of the existing set of tables. This quarterly report contains further studies on the development of a liposomal drug delivery system for WR6026.2HC1. These studies include further investigations on improving entrapment efficiency and reproducibility, release of entrapped WR6026.2HC1 from negative and neutral liposomes and scale-up of liposome batch size.

Pyridoxal 5' -phosphate hemoglobin (PLP-Hb), prepared from hemoglobin and a four-fold excess of pyridoxal 5' -phosphate by the method of De Venuto and Zegna, has been chromatographically resolved into six components via a quaternary ammonium monobead support. On an analytical scale, the separations have been found to be rapid (ca. 50 min) and highly reproducible. The results also indicate that the preparation of PLP-Hb yields a reproducible product ratio. The potential of the analytical method for the routine quality control of blood substitutes derived from PLP-Hb is discussed. All five of the PLP derivatives (components II-VI), isolated and purified via a combination of conventional and preparative monobead anion-exchange chromatography, gave single peaks when analyzed by high-performance liquid chromatography. It was the purpose of this effort to use certain basic research results which indicated that it was possible to produce from phase separated glasses optical waveguide fibers with attenuations below 20 dB/Km and numerical apertures greater than available by chemical vapor deposition methods. Before the specific NELC applications for this fiber could be realized in commercial production, certain process engineering steps had to be solved. These production problems and the effort on each are discussed in the report.

A weld monitoring system has been developed based on conditioning the primary signals from the welding machine and comparing them with preset limits. A light is activated when a welding parameter such as voltage, current, or heat input is outside the control limits. The circuitry has been tested with simulated primary signals. The voltage channel has also been tested using primary signals from an automatic gas metal-arc welding machine.

This paper examines, from a managerial viewpoint, the effects of programming group organization on both the people in the group and on the software thus produced. The purpose of this effort was to investigate and develop techniques to reduce the procurement cost and increase the reliability and timeliness of complex computer software. Specific goals of the program were to study and define a Software-First Machine (SOFM), determine to what extent the technique of Structured Programming is beneficial in the testing of software and contributes to the reliability of software, and provide the Air Force with initial guideance in the area of software reliability. This report presents an overview of the technological background common to the six tasks of project 522A, a part of MITRE Project 5220. The goals of the program were to: (1) Correlate the KSC magnetically driven flyer plate facility test techniques with those of another facility; (2) define AGT test techniques which successfully duplicate UGT test results; (3) expand the 3DQP material data base; and (4) establish 3D quality control procedures (3DQC) through analytics, NDM, NDT, resonance test techniques, and flyer plate loading procedures. KSC participation in the program touched on all four program goals. KSC impulsive load testing was most heavily concentrated in goals under the facility Correlation Study and the UGT Simulation Study. KSC analytical capability was concentrated in areas (3) and (4), expanding the 3DQP data base and establishing 3DQC procedures.

The objective of this program was to investigate the possibility of radically improving the productivity of sheet metal forming processes through innovative mechanistic and control system designs. Two prototype forming machines were designed, constructed and evaluated. An adaptive brakeforming system was designed and constructed. This computer-controlled brakeforming system uses direct measurement of the sheet angle to automatically compensate for springback of material. Potential benefits from adaptive brakeforming include reduced setup time, improved quality control, insensitivity to material property variations and improved operator safety. Cost reductions from reduced setup time alone were estimated at $500,000 per year or 18%. A prototype discrete die surface (DDS) was also designed and constructed. The device consists of an array of die elements or rods which are free to slide in a housing. The die elements can be positioned to preprogrammed locations under computer control. The device can also measure the positions of individual die elements. A computer program was developed to calculate pin locations which would produce a desired unloaded shape. Forming experiments on 6061 T3 aluminum were performed in which specimens were formed to circular shape using the discrete die surface.

The following Quality Assurance Plan and Standard Operating Procedures will be used by Texas Instruments Incorporated (TI), Ecological Services to provide services contracted by the Maryland Department of Natural Resources Energy Administration, Power Plant Siting Program to conduct finfish and macroinvertebrate studies in the R.P. Smith Plant vicinity during summer/fall 1980. This SOP was designed to standardize collection and analysis methods and to establish program specific Quality Assurance/Quality Control (QA/QC) procedures. Procedures outlined in this manual are controlled at various levels by the supervisors whose signature appears on the Ecological Services approval sheet accompanying this manual.

Theoretical and experimental studies related to the development of a thickness design procedure for airfield pavements are discussed. The pavement is treated as an elastic layered half-space and the moduli of elasticity of the paving materials as the primary design factors. Equations and influence charts derived from the application of elastic theory to two- and three-layered systems are used to interpret the experimental plate-bearing data. Values of the modulus of elasticity are presented for three base materials. It is shown that the modulus of elasticity depends upon the quality of the base, its depth and density, and the stiffness of the contiguous subgrade. The principal failure modes observed in flexible pavements and the suitability of the triaxial test as an adjunct to pavement design are discussed. A suggested revision in the current Navy pavement design procedure is presented. Recommendations for continued research related to layered theory pavement design are also included. A thickness design criterion for portland-cement-stabilized sand bases is presented as part of this study. (Author)

The report describes a new instrument to measure many photometric, radiometric, and colorimetric quantities. It has particular application in the characterization of colored signals and visual displays because a single instrument can measure both the luminance and the chromaticity of an illuminated indicating device or display screen. It can also be made part of an inspection system or an industrial 'on-line' quality control process for dyed or painted products, alternately measuring tristimulus color values and total reflectance if desired. The instrument can also be programmed to process spectral information gathered from a target according to two (or more) differeent response functions. This would enable it to measure more complicated radiometric and photometric quantities such as cone-to-rod (dark adaptation) ratios. (Author)

The Systems Research and Development Service of the FAA is conducting a research program for upgrading airport pavement technology by a coordinated contractor/in-house/industry effort. Three areas of action are design criteria, materials and construction, and test and evaluation. By the end of CY 1973 most of the current design, field test and evaluation efforts based on existing design technology will be completed and results of the study will be handed off to Airports Service for implementation by the regional authorities. To a great extent this information will supplement our existing technology which was derived from extrapolation of military and highway flexible and rigid pavement designs and post-World War 2 experience. A nominal 12.5 second stab initiated, delay, detonator has been developed for the Mk 347 Mod 0 Bomb Fuze. This detonator has been subjected to the standard Navy rough handling and surveillance tests. A limited number of field tests were successful. Manufacture of a lot of detonators by private industry resulted in no fabrication problems. The detonator is completely documented with drawings and specifications.

The NCEL mechanical subgrade is a device which simulates the action of a natural subgrade. This apparatus is intended for use in evaluating materials for airfield pavement construction. The adoption of a test procedure is a preliminary step in the evaluation of all types of airfield pavement materials using the mechanical subgrade and the concepts of the layered pavement design method.
