Foram aplicados, o teste de Mantoux e uma prova intradérmica dupla de tuberculina (PID), em 131 pacientes hospitalizados para elucidação de diagnóstico, sendo os resultados comparados estatisticamente com o diagnóstico final obtido por métodos clínicos, cirúrgicos e laboratoriais. O teste de Mantoux lido às 72 h seguido de nova aplicação de PPD no mesmo local, sendo lida a nova reação após 24 h constitui a prova intradérmica dupla (PID), sendo o resaltado final obtido da diferença em mm entre a primeira e a segunda leitura. A análise estatística mostrou que a PID é melhor que o teste de Mantoux, com menos erros, menos reações falsas-positivas, probabilidade nula de dar falsos-negativos, maior sensibilidade e maior especificidade. Concluiu-se que a PID pode ser usada em elucidação de diagnóstico e em inquéritos de saúde pública em tuberculose.The Mantoux test and a double intradermal tuberculin test (PID), were carried out on 131 patients hospitalized for diagnostic elucidation. Comparison was made between the results of the two intradermal tests and the final diagnosis, obtained by clinical, surgical and laboratory methods, and the data were submitted to statistical analysis. Measuring Mantoux reaction after 72 h, and injecting a new intradermal dose of PPD at exactly the same point and repeating the reading after another 24 h, the difference in size (mm) between the first and second readings may be interpreted in three different ways: 1) A difference of less than 3 mm between the two readings: nill statistical probability of patient with tuberculosis. 2) A difference of 3 mm or more, with maximal reaction diameter of 9 mm: indecisive result; patient probably without tuberculosis, however it is suggested that a new PID should be carried out after another 90 days. 3) A difference of 3 mm or more, with maximal reaction of 10 mm or more: patient probably has tuberculosis. The statistical analysis showed that the PID test is better than the Mantoux test, presenting fewer errors, less false-positive results, nill probability of false-negative results, better sensibility and specificity. The conclusion was drawn that the PID is a test that can be used in diagnostic elucidation and publici health surveys on the incidence of tuberculosis.

A 5 year-old boy experienced anaphylaxis after eating a jelly product for diet supplement containing erythritol as a major component. Prick test with the jelly product was negative, but the second oral ingestion of the jelly product at home caused another allergic reaction. Prick test with erythritol was negative even at 300 mg/ml, which was almost the solubility limit. Intradermal test was marginally positive at 0.1 mg/ml, and clearly positive at 1 mg/ml or higher concentration. We found subtle dose-response reaction utilizing basophil activation test, examined with 24 hour incubation at the concentration of 40-4000 μg/ml. At the oral challenge test in the hospital, 3 g of erythritol induced remarkable coughing, urticaria, edema, wheezing and hypoxemia. Erythritol is a natural sugar alcohol, with the molecular weight of 122.12, which is recently being widely used for diet supplements, beverages, or drug medicines due to its properties of calorie-free and good-tasting, with easy-to-use physical characteristics. We now have to recognize erythritol as a candidate for food allergen, and to be careful about negative result of prick test.

Vaccination of cattle with a commercial human tuberculosis (TB) vaccine, Mycobacterium bovis bacillus Calmette-Guérin (BCG) Danish, at a dose equivalent to 5 human doses of BCG has protected these animals against TB in field and experimental trials. There is interest in determining whether a 10-fold-lower dose could still protect cattle but not induce a tuberculin intradermal test response. Two groups of calves (n = 9/group) were vaccinated subcutaneously with a lyophilized BCG Danish vaccine containing either 0.5 (1 × 10(5) to 4 × 10(5) CFU) or 5 (1 × 10(6) to 4 × 10(6) CFU) human doses of BCG Danish, with an additional group of 10 calves serving as nonvaccinated controls. Fifteen weeks after vaccination, these animals were challenged intratracheally with 5 × 10(3) CFU of virulent M. bovis and another 15 weeks later were slaughtered and examined for the presence of tuberculous lesions. Vaccination of the calves with either 0.5 or 5 equivalent human doses of BCG Danish induced similar levels of protection against challenge with M. bovis, with both groups showing significant reductions in the pathological and microbiological parameters compared to those for the the control group (P < 0.05). Vaccination with either of the two BCG doses induced similar numbers of animals responding to the tuberculin intradermal test at 11 weeks postvaccination. Vaccination with a 0.5 equivalent human dose of a commercial lyophilized BCG vaccine can protect cattle against challenge with M. bovis.

The ability to use ciclosporin (Atopica®: Novartis Animal Health, Greensboro, NC, USA) prior to intradermal testing (IDT) would help avoid exacerbation of clinical disease that can be associated with drug withdrawal. This study evaluated the effects of 30 days of administration of ciclosporin at a dose of 5 mg/kg once daily on IDT reactivity (immediate phase reactions) in a group of dogs with atopic dermatitis (AD) with initial positive IDT reactions. 16 dogs diagnosed with AD were included in the study. Eight dogs (group A) were treated with ciclosporin orally at 5 mg/kg once daily for 30 days. Eight dogs (group P) were treated with a placebo orally once daily for 30 days. IDT was performed at day 0 and day 30 on all dogs enrolled using a standardized panel of 45 aqueous allergens (Greer Laboratories, Lenoir, NC, USA) appropriate to our geographical region. IDT reactivity was assessed by both subjective and objective methods at 15 min post-intradermal injection. Serum for allergen-specific immunoglobulin (IgE) serology was obtained at day 0 and day 30. The study was designed as a double-blinded, placebo-controlled, cross-over study. Data were analysed using a split-plot analysis of variance with the grouping factor of treatment and the repeat factor of time (SAS System for Windows). At week 4, ciclosporin did not have a statistically significant effect on IDT reactivity or serology results. The neutralising antibody response after the yellow fever vaccine (YF-17D) skin test was measured in 7 egg allergic persons in whom further vaccination was abandoned because of a strong local urticarial reaction to the YF-17D vaccine test dose. We found that this test dose of 0.1mL of YF-17D vaccine was sufficient to induce a protective antibody response in all 7 subjects. Intradermal injection of 1/5th dose of the yellow fever vaccine appears to be sufficient, in non-allergic as well as allergic persons, and non-inferior to the subcutaneous full dose.

Background/aims: The diagnosis of clonorchiasis is based on demonstrating eggs in stool or bile. It is believed that bile examination is the most precise method for detecting eggs. We evaluated diagnostic usefulness of intradermal test (IDT) by comparing it with the result of bile examination., Methods: For 88 patients with pancreatobiliary diseases, we examined Clonorchis sinensis eggs in bile and performed IDT for clonorchiasis. The bile was obtained from endoscopic nasobiliary or percutaneous transhepatic biliary drainage tubes., Results: We calculated ROC curve to decide the cut-off value of IDT in determining diagnostic accuracy on the basis of bile examination. We chose a value of 40 mm2, which significantly improved the sensitivity, without reducing the specificity. With a cut-off value of 40 mm2, the sensitivity, specificity, positive and negative predictive values of IDT were 81.5%, 67.2%, 52.4%, and 89.1%, respectively. The value of IDT was not affected by age and showed no difference between benign and malignant diseases. However, in egg-positive patients, the mean value was lower in malignant diseases than in benign diseases., Conclusions: In patients with pancreatobiliary diseases, IDT with a cut-off value of 40 mm2 seems to be a valuable supplementary diagnostic test for clonorchiasis in view of its high sensitivity.

Nineteen healthy beagles, eight healthy basset hounds and 17 basset hounds with Malassezia dermatitis were tested intradermally with two extracts of M pachydermatis. One healthy beagle and two affected basset hounds showed wheal and flare reactions 15 minutes after the injection. Delayed reactions, consisting of erythematous macules and plaques, were commonly observed 24 hours after the injection in both the healthy and affected basset hounds, but occurred infrequently in the beagles. At 24 hours the diameters of the lesions in the healthy and affected basset hounds were significantly (P<0.01) greater than those in the healthy beagles, but the diameters in the healthy and affected basset hounds did not vary significantly. Delayed reactions in six of the basset hounds with Malassezia dermatitis were characterised histologically by superficial perivascular and periadnexal infiltrates of neutrophils and lymphocytes.

Objective: To explore the relationship between the reactive intensity of allergen intradermal test and the concentration of specific IgE (sIgE) in serum., Method: Intradermal test of six kinds of allergens was done in 192 cases of allergic rhinitis, including house dust, mite in powder and dust, poly mould, spring pollen, summer autumn pollen and Artemisia pollen. The serumic study of these kinds of allergens was also done with Pharmacia CAP system., Result: The total accordance rates of these six kinds of allergens with IgE in serum respectively are: house dust 65.93%, mite in powder and dust 68.1%, poly mould 55.6%, spring pollen 66.7%, summer autumn pollen 60.0% and Artemisia pollen 74.4%. The correlation index of them respectively are: house dust 0.4235 (P < 0.01), mite in powder and dust 0.4029(P < 0.01), poly mould 0.4932, spring pollen 0.3277 (P < 0.05), summer autumn pollen 0.2412 (P > 0.05) and Artemisia pollen 0.5000 (P < 0.01)., Conclusion: The intradermal test of five kinds of allergens except for summer autumn pollen correlates with the concentration of IgE in serum.

The histoplasmosis in Spain is an imported disease presenting in most of case diagnostic difficulties. In this paper, the intradermal skin test with Histoplasma capsulatum antigen as diagnostic method in immunocompetent patients with clinical and radiological signs compatible with histoplasmosis after being visited Central and South American endemic counties, in which this mycosis is endemic. Nine Spanish patients coming from different countries of Latin America with fever and acute respiratory symptoms compatible with histoplasmosis were studied. Other nine accompanying subjects and five controls were also evaluated. Patients underwent mycological cultures and and serological tests for H. capsulatum. Intradermal test with 1% histoplasmine were done in all patients. Serology and skin tests tests were also performed in accompanying people. Intradermal were done in healthy controls. Skin test with histoplasmine were positive in seven of the nine patients. Six of these showed precipitating antibodies against the same antigen. H. capsulatum was only isolated from bone marrow biopsy samples in one patient. The seven patients were given itraconazole by oral route and all symptoms improved after 2 and 4 weeks. In five accompanying subjects the skin test were also positive so that a subclinical histoplasmosis was diagnosed. In the remaining patients and healthy accompanying subjects histoplasmosis infection was excluded. In non endemic geographical areas of histoplasmosis intradermal skin test with histoplasmin when used in immunocompetent individuals is an easy and reliable method for the diagnosis of this mycosis as well as for epidemiological studies.

Among many diagnostic tests for allergic rhinitis, the intradermal (ID) test is practical and reliable. However, there are several factors affecting compliance, practicability and interpretation, and also problems on hypersensitivity of the ID. For these reasons, we evaluated other tests which have been thought to have high reliability as diagnostic and/or screening assays, namely, skin prick test and specific IgE detection in seventy-four perennial rhinitis patients (51 males and 23 females whose ages were between 15-60 years). In this study, Dermatophagoides pteronyssinus and D. farinae extracts, known to be the most common aeroallergens in Thailand, were used as the allergens/antigens. Compared to the standard ID test, sensitivities to D. pteronyssinus and D. farinae of the studied patients tested by skin prick test were 90.4% and 86.4%, and specificities were 99.5% and 93.1%, respectively. Sensitivities to D. pteronyssinus and D. farinae using specific IgE assay were 96.3% and 88.9%, and specificities were 96.2% and 88.9%, respectively. It was concluded that the skin prick test can be used as a screening method for patients with allergic rhinitis, while the specific IgE detection can be used as an alternative for diagnosis of patients who are susceptible to the ID test or for those who are severely susceptible to allergic rhinitis such that medication can not be withdrawn for the ID test.

Objective: Identification of allergens by intradermal test in patients with asthma, allergic rhinitis, and eczema. Materials and Methods: Intradermal test was performed in 100 patients of Ambala over an 8-year period to identify the common allergens. A total of 197 allergens including 50 types of pollen, 19 fungi, 17 insects, 14 types of dust, 6 animal dander, 7 types of fabric and feather and 82 types of foods, dust mite, and parthenium were tested. Results: In this study, the major allergens were pollen (51%) followed by foods (28.9%), insects (26.9%), fungus (12.6%), dusts (6.7%). Among pollen allergens, Brassica campestris (8%) was a major allergen followed by Ageratum conyzoides (7%) and Artemisia scoparia (6%) Cannabis sativa, Cynodon dactylon and Maerua arenaria (5%). Among the fungal group, Alternaria tenuis, Aspergillus flavus, Aspergillus fumigates, Candida albicans, Penicillium sp., Rhizopus nigricans (3%), Fusarium solani (2%) were found. In the insect group, mosquito (7%), ant (6%), grasshopper (5%), locust (male), moth, and house fly (4%) were the major allergens. Among the dust allergens, grain dust rice (3%), straw dust, house dust, and grain dust bajra (2%) were found. Among the food allergens, prawn (5%), almonds, Baker's yeast, Bengal gram (3%) and mushroom, mango ripe, rajma, cinnamon, chocolate, beans fresh, and areca nut (2%) were found. Conclusion: It can be said that the knowledge may help to create a mapping of allergens in this area and help to treat patients by immunotherapy or avoidance strategy.

From August to October 1992 a study of prevalence of cutaneous positivity to histoplasmin and paracoccidioidin was carried out in the cities of Cachoeira do Sul (Valley of Jacuí River) and Santo Angelo (western hillside of the PlateAn), Rio Grande do Sul, Brazil. Skin tests were made in 193 soldiers from Cachoeira do Sul and 161 soldiers from Santo Angelo, both groups of 17 to 19 years of age. In Cachoeira do Sul the prevalence of positive tests to histoplasmin was 89% and to paracoccidioidin 82%. In Santo Angelo 48% of the intradermical reactions were positive to histoplasmin and 39% to paracoccidioidin.
