Unilateral condylar hyperplasia (UCH) is an uncommon disease involving progressive facial asymmetry. The aim of this research was to perform an analysis of the diagnosis and treatment of patients with UCH in a clinical series. An observational retrospective study was performed on subjects with progressive facial asymmetry in the lower third of the face; all the subjects were under treatment with condylectomy and orthodontics to improve occlusion and face balance. Variables such as age, sex, clinical type, SPECT (single photon emission computed tomography) intensity and a requirement for secondary surgery were included; the Shapiro Wilk test was performed to analyze the normality of the data and nonparametric analysis and the Kruskal-Wallis or Mann-Whitney tests were used to assess the association between the SPECT difference and the variables, where 2-tailed p values < 0.05 were considered to be statistically significant. Forty-nine patients between 10 and 45 y.o. (average age: 19.1 ± 7.4 y.o.) were included in the study. There were 41 female (83.6%) and 8 male (16.4%) subjects. The SPECT analysis comparing the right and left condyles with more than 10% in caption of the isotope was present in 46 subjects; the results obtained using SPECT were not statistically related to the age or sex of the sample (p = 0.277). The patients were classified into clinical types I, II and III, and no correlations could be confirmed between the clinical type and other variables. High condylectomy was conducted on all patients, among which 14 patients underwent a secondary surgery for orthognathic or cosmetic treatment, and was not related to the initial variables used in diagnosis (p = 0.98); interestingly, the second surgical treatment was more present in the clinical type I and in subjects under 16 years old with no statistical differences. The aim of the study was to describe an approach where condylar resection with condylar neck preservation was combined with Le Fort I osteotomy and unilateral mandibular sagittal split ramus osteotomy (SSRO). Patients with a unilateral condylar osteochondroma combined with dentofacial deformity and facial asymmetry who underwent surgery between January 2020 and December 2020 were enrolled. The operation included condylar resection, Le Fort I osteotomy and contralateral mandibular sagittal split ramus osteotomy (SSRO). Simplant Pro 11.04 software was used to reconstruct and measure the preoperative and postoperative craniomaxillofacial CT images. The deviation and rotation of the mandible, change in the occlusal plane, position of the "new condyle" and facial symmetry were compared and evaluated during follow-up. Three patients were included in the present study. The patients were followed up for 9.6 months on average (range, 8–12). Immediate postoperative CT images showed that the mandible deviation and rotation and occlusion plane canting decreased significantly postoperatively; facial symmetry was improved but still compromised. During the follow-up, the mandible gradually rotated to the affected side, the position of the "new condyle" moved further inside toward the fossa, and both the mandible rotation and facial symmetry were more significantly improved. Within the limitations of the study it seems that for some patients a combination of condylectomy with condylar neck preservation and unilateral mandibular SSRO can be effective in achieving facial symmetry. The aim of this study was to generate a quantitative dynamic assessment of facial movement symmetry changes after orthognathic surgery. Twenty-five patients diagnosed with skeletal class III malocclusion with facial asymmetry who underwent bimaxillary surgery were recruited. The patients were asked to perform a maximum smile that was recorded using a three-dimensional facial motion capture system preoperatively (T0), 6 months postoperatively (T1), and 12 months postoperatively (T2). Eleven facial landmarks were selected to analyse the cumulative distance and average speed during smiling. The absolute differences for the paired landmarks between the sides were analysed to reflect the symmetry changes. The results showed that the asymmetry index of the cheilions at T2 was significantly lower than that at T0 (P = 0.004), as was the index of the mid-lateral lower lips (P = 0.006). The mean difference in cheilions was 2.13 ± 1.41 mm at T0, 1.33 ± 1.09 mm at T1, and 1.00 ± 0.98 mm at T2. The facial total mobility at T1 was significantly lower than that at T0 (P < 0.001), while the total mobility at T2 was significantly higher than that at T1 (P = 0.012). Background: Condylar hyperplasia is an abnormal growth affecting the temporomandibular joint caused by overactive cells in mandibular condyle. It is relatively rare, and its exact frequency is unknown. Purpose: To describe the clinical, radiographic, and histological characteristics from pathology of two patients with active condylar hyperplasia, whose purpose is to focus on specific surgical treatment of condylar hyperplasia without the need for additional management with orthognathic surgery and its satisfactory evolution. Methods: A systematic review of the literature was carried out through of search in different databases such as LiLACS, PubMed, ScienceDirect and Scopus considering articles published between 2008 and 2023, a period of 15 years. 41 articles corresponding to literature reviews, clinical cases and systematic reviews were selected. Results: surgical management of condylectomy versus combined with orthognathic surgery, it was found that 63,64 % about the articles analyzed were treated only with high condylectomy and subsequent orthodontia, while 36,36 % of these were managed with high condylectomy plus orthognathic surgery in the same surgical act or in a second surgical procedure. Presentation of cases: The presentation of two clinical cases of condylar hyperplasia who report aesthetic and functional changes, treated with condylectomy on the affected side is made. Conclusions: Condylar hyperplasia is a pathological disorder characterized by generating asymmetries and facial deformations, deviations, malocclusions, functional, aesthetic and psychological alterations, condylectomy being the procedure of choice which can be complemented with orthognathic surgery and orthodontics. Aim: To present a case of Sagliker syndrome (SS), a rare syndrome caused by longterm heavy tertiary hyperparathyroidism (HPT) in end-stage renal disease (ESRD). Case report: In the year 2000, the 38-year-old man was diagnosed with ESRD. He is currently of low height, paraplegic, pigeon and barrel chested, with elongated upper extremities, deformed fingers, mandibular and maxillary asymmetric deformities with teeth malformations. Due to extremely high serum parathormone (PTH), resistant to pharmacological treatment, subtotal parathyroidectomy (PTx) was performed in 2007. However, he underwent parathyroid resurgery in 2010 for persistently high PTH of more than 2500 pg/mL (upper normal limit 69). In 2012, imaging found two suspected parathyroid glands and one of them was surgically removed. Postoperatively, the expected decrease in calcium and PTH serum concentrations did not occur. Ten years after the diagnosis of ESRD, the patient began to notice more pronounced skeletal deformities (upper, lower jaw, extremities, deformities of fingers, kyphoscoliosis) along with depressive disorder. Laboratory findings still show extremely high PTH (1994 pg/mL), low calcium, 1.89 mmol/L (normal range 2.14–2.53), and high alkaline phosphatase, 837 U/L (normal range 60–120), despite continuous pharmacological treatment. Conclusion: SS was firstly recognized by Yahya Sagliker in 2004 and it has not been described in Croatia to date. Pervasiveness and knowledge of the syndrome is still poor. The most efficient way of treating/preventing SS is early total PTx in severe secondary HPT related to ESRD. However, it can only stop progress of the disease, but cannot return skeletal deformities. Investigations on mate choice in humans frequently report preferences, but there is little knowledge about what is important for rejection by a potential mate. The present study aims to verify if facial asymmetry and facial disharmony have an influence on mate rejection by men. We hypothesized that more asymmetric and disharmonious faces would be more rejected. For this purpose, photographs of women's faces were presented in pairs by self-declared heterosexual men. It was requested they reject one of the faces as a potential mate for a long-term relationship. Women's faces were also analyzed to measure facial asymmetry and facial disharmony. We used a linear mixed model to evaluate the effect of the cited metrics on each face's number of rejections. We found that the female metrics influenced mate rejection only if associated with male age and income. The older participants rejected female partners with asymmetric faces. We suggest that aging makes men more demanding in mate choices, at least considering facial asymmetry. We concluded that rejection could be a key variable in mate choice studies, but further research is needed to clarify its effects. [ABSTRACT FROM AUTHOR]

Introduction: facial deformities are a set of residual volumetric alterations of genetic malformations, traumas and postoncologic scars considered as a devastating disease. Their reconstruction is a challenge for the plastic surgery specialist due to the complexity of facial aesthetics in which a set of structures that guarantee the psychosocial integrity of the patient and his validism are harmonized. Lipotransfer is presented as a treatment alternative, which has evolved with the purpose of adjusting to the needs of usefulness and feasibility Objective: to analyze the scientific-social impact of lipotransfer in the treatment of facial deformities as an example of scientific progress in Plastic Surgery in Villa Clara Province. Method: the medical literature published in Spanish and English languages was reviewed, Google Scholar and PubMed search engines were used, SciELO and Oxford digital libraries and Medline database were consulted and appropriate key words were used. Conclusions: lipotransfer with platelet-rich plasma in the treatment of facial deformities constitutes a viable, safe, economical treatment alternative with satisfactory results that satisfies the patient and restores the anatomical and functional integrity of the damaged structures, which allows its validation and is an example of scientific progress in plastic surgery in the province. Background The greatest challenge of a free-muscle transfer in facial reanimation surgery is anchoring muscle to perioral soft tissue. An additional incision provides a better way to anchor the transferred muscle but leaves a visible scar. Herein, we compared the functional and aesthetic outcomes in a conventional facelift incision approach with and without the addition of a red line incision. Methods This study included patients experiencing unilateral facial paralysis who underwent free gracilis transfer between December 2016 and December 2018. Anchoring through facelift incision with extended neck incision was performed in the control group, while a red line incision was added in the red line group by avoiding extended neck incision. Oral commissure excursion, upper lip and commissure height differences, and symmetry were measured. Results A total of 24 patients met the criteria and were included in this study. Of these, 10 patients were assigned to the red line group and 14 to the control group. The red line and control groups demonstrated a similar symmetry index in both resting (0.96 ± 0.18 and 0.92 ± 0.10, respectively; p = 0.435) and smiling (0.94 ± 0.12 and 0.91 ± 0.10, respectively; p = 0.314). However, the upper lip height difference demonstrated greater postoperative improvement (resting: 2.12 ± 1.13 vs. 3.92 ± 1.41 mm, p = 0.002, and smiling: 1.68 ± 0.88 vs. 3.41 ± 1.69 mm, p = 0.004, respectively). A scar could be easily concealed on the mucocutaneous line of the lip. Conclusion This novel method for muscle anchoring in facial reanimation surgery allows surgeons to approach perioral muscle more directly. This leads to improved surgical results by securely anchoring the transferred muscles using a fixation point that is more medially located than in other approaches. Fibrous dysplasia is a developmental abnormality characterized by the replacement of normal bone tissue by fibrous connective tissue with poorly organized bone trabeculae. This disorder rarely occurs in the craniofacial region, but in such cases it causes facial asymmetries and has severe clinical implications for the patient. This case report describes the treatment of an 18-year-old man who presented with complaints of facial deformity and decreased visual acuity. Cone beam computed tomography revealed a diffuse bone lesion affecting the region of the maxillary, frontal, and nasal bones on the left side of the face. After microscopic examination, the diagnosis of craniofacial fibrous dysplasia was made. The patient underwent a bilateral temporal craniotomy to perform decompression of the orbital apices and correct the loss of visual acuity. In addition, surgical cosmetic contouring of the facial bones was performed. The patient has been followed up by a multidisciplinary team; at his most recent examination, 18 months after the last surgical intervention, his clinical condition remained stable. Recently, the demand for correcting facial asymmetry has been increasing, even when the extent of asymmetry is small. This case report describes nonsurgical orthodontic treatment for facial asymmetry in a 13-year-old female patient, facilitated by moving the deviant mandible to the nondeviated side after correcting for relevant dental compensation using microimplants. Mandibular repositioning was attempted using intermaxillary elastics between the microimplants placed into each jaw and guided by resin that was bonded on the maxillary first molar of the deviated side. To enhance mandibular movement, correction of the transverse occlusal cant and buccolingual inclination of the teeth were also performed. After 65 months of gradual treatment, facial symmetry, with favorable occlusion and jaw function, was achieved. These satisfactory results, including a well-balanced face and good occlusal interdigitation, were well maintained at the 53-month follow-up. Among the craniofacial structures, the mandible is the only bony structure with movable joints. Each part (including condyle process, coronoid process, and ramus) of mandible would interaction with the muscles and proceed different osteogenesis progress. The objective of this study was to evaluate the mandibles with symmetric and asymmetric skeletal Class III jaw relations by quantifying differences in the condyle process, coronoid process and ramus on CBCT (Cone-beam computer tomography) images. Our hypothesis was that CBCT would reveal no voluminal differences between deviated and non-deviated mandibular segments in asymmetric skeletal Class III. CBCT imagines were collected from dental department, KMUH and then divided into symmetric Class III group (Menton deviation < 4mm) and asymmetric Class III group (Menton deviation≧4mm). The mandibular structure would be segmented to ramus, condylar and coronoid process. Each volume was measured. Independent t test was used for comparison between groups, and paired t test was applied for comparison between both segmented parts within each group. Significant differences between deviation and non-deviation sides in the asymmetric group were found in condylar and ramus segments for volumetric quantitative measurements. There has no significant difference in ramus parts between groups. Significant greater condylar volume was found in non-deviation side of asymmetric group. The results demonstrated that in the side with greater mandible growth potential, the condylar and the ramus volume would be greater as well. Orthodontics is the study of malpositioned teeth and jaws. Some commonly found orthodontic issues are malocclusions, mandibular prognathism, retrognathism, and facial asymmetry. These issues are associated with several genetic factors. For instance, mandibular prognathism is related to gene variations, such as insulin-like growth factor 1 and the HoxC gene cluster. Facial asymmetry is related to NFATC1 and NBAS genes. Dental agenesis is related to several genes, including paired box 9 (PAX9), homeobox 1 (MSX1), and ectodysplasin A. Apart from these genes, several other genes, such as MYO1H gene, ACTN3 gene, ENPP1, ESR1 genes, PITX1 and PITX2 genes, IL1 gene, and PAX9 gene, have been studied in the orthodontic issues faced by people. Some of these genes, such as ENPP1 and ESR1 genes, are directly related to orthodontic issues, whereas some of the genes, such as ACTN3 genes, are associated with issues in the muscle tone that can eventually lead to orthodontic issues. Background: To study the dentoskeletal characteristics and the degree of compensations in skeletal Class I adults with unilateral posterior crossbite (UPCB). Methods: A sample of 40 adults was chosen for this cross-sectional study. 20 skeletal Class I adults with UPCB (mean age: 22.20 ± 2.88 years), were compared to 20 skeletal Class I adults with normal occlusion (mean age: 27.56 ± 5.76 years). The respective dentoskeletal measurements were made on cross-sectional images from cone-beam computed tomography scans. Results: Skeletally, both groups showed significant differences (P < 0.05) in mandibular corpus length and menton deviation with the UPCB group showing the greatest displacement. Maxillomandibular vertical asymmetry and condylar positional asymmetry were not significant in both groups (P > 0.05). For dental variables on the second premolar and first molar, the UPCB group showed greater linear and angular differences when compared to the control group (P < 0.05). On the crossbite side, maxillary posterior teeth were more buccally inclined, and mandibular posterior teeth were more lingually inclined. However, on the non-crossbite side, both maxillary and mandibular posterior teeth were lingually inclined. Conclusion: Adults with UPCB showed distinct transverse dentoskeletal asymmetry. An important parameter in diagnostic analysis and treatment planning of different biological areas is facial symmetry, and several etiological factors have been attributed to skeletal facial asymmetry. Although causality cannot be determined, previous studies have reported a relationship between the anatomical deviation of the nasal septum and facial development. Diagnosis is critical for patients in growth stages due to the association between nasal septum deviation (NSD) and abnormal growth of the nasomaxillary complex. To understand this relationship, this study aimed to investigate the influence of nasal septum deviation on fluctuating asymmetries (FA) of the nasomaxillary complex at different stages of skeletal maturation. Another goal was to determine whether an association exists between the degrees of septal deviation severity and asymmetry of the nasomaxillary complex. This was a retrospective, cross‐sectional observational study comprising 60 selected cone‐beam computed tomography (CBCT) scans that were divided into four groups (n = 15) according to the degree of septal deviation and skeletal maturation: mild deviation (MD; <10°), moderate to severe deviation (MSD; ≥10°), early group (EG), and late group (LG). The angle and area of deviation were measured for the greatest NSD, and a geometric morphometric approach was used to evaluate the nasal septum (NS) shape. The morphology of the nasomaxillary complex and the presence of fluctuating asymmetries were evaluated using 23 two‐dimensional landmarks on the nasomaxillary complex (nasal, lateral, and palatal regions) with Procrustes ANOVA and Mann–Whitney test. Additionally, Spearman's correlation and multivariate regression were used to correlate the NSD with asymmetries in these regions. No significant differences were observed in the Procrustes FA scores of the nasomaxillary complex between the MD‐EG × MSD‐EG and MD‐LG × MSD‐LG (p > 0.05). However, the results of the multivariate regression revealed more specific aspects of asymmetry (asymmetry component), there was a positive correlation between the NSD angle and the palatal regions (p = 0.035 and p = 0.047, middle and posterior, respectively), and the nasal septum shape and anterior palatal regions (p = 0.039). The nasal and lateral regions did not correlate with the NSD angle in the multivariate regression analysis (p > 0.05). The results of this study indicate that there were no significant differences in the fluctuating asymmetry of the nasomaxillary complex between the mild and moderate to severe nasal septum deviation groups, in both early and late skeletal maturation stages. However, a positive correlation was observed in the degree of nasal septum deviation angle and asymmetry components of the middle and posterior palatal regions, likewise between the nasal septum shape and asymmetry components of the anterior palatal region. The diagnosis of nasal septum deviation by both physicians and dentists is important, as a relationship was observed with fluctuating asymmetry component of the palatal region. This information can guide the decision of the treatment planning for these individuals, and should be considered, especially in cases of severe septum deviation, due to the great anatomical proximity of these structures. This study aimed to review the surgical outcomes, complications, and long-term relapses in patients with unilateral or bilateral coronal craniosynostosis, who underwent one-piece fronto-orbital distraction osteogenesis (FODO) without bandeau. The cephalic index, frontal angle, and supraorbital symmetry ratio were measured on the initial and follow-up computed tomography images. Esthetic outcomes were evaluated using the Whitaker classification. 45 patients were included in this study. The average follow-up interval was 5.4 ± 1.1 years (range 2.5–8.5 years). In patients with bilateral coronal craniosynostosis, the frontal angle decreased with a relapse ratio of −2.8 ± 4.3% during long-term follow-up compared with that during short-term follow-up (p = 0.028). In patients with unilateral coronal craniosynostosis, the supraorbital symmetry ratio decreased slightly, with a relapse ratio of −3.8 ± 2.6% during long-term follow-up (p = 0.017). Complications included dural tears during osteotomy (n = 2), early distractor removal (n = 2), and wound problems (n = 3). Within the limitations of the study it seems that one-piece fronto-orbital distraction osteogenesis (FODO) generates successful and sustainable results even in the long term. Featured Application: Better surgical planning of the mandibular angle region. The surgical correction of dento-maxillofacial deformities for aesthetic reasons is increasingly required. In order to obtain the results that our patients expect, it is necessary to very thoroughly evaluate every aesthetic detail of the face, because an optimal result comes from the correction of all parameters. The region of the mandibular angles is very important in determining the final aesthetics but is at the same time difficult to predict. Traditional programming based on model surgery is inadequate in this regard compared to that based on CT (Computed Tomography) reconstruction. Introduction: Surgical correction of facial asymmetry is commonly performed in at least two stages. Recently, because of the long duration of a two-step procedure, the demand for a one-step procedure has increased. Our study aims to present a fully digitalized workflow for one-stage mandibular contouring (MC) and bimaxillary surgery to correct severe facial asymmetry using 3D technology. Materials and Methods: A retrospective monocentric study was conducted for all patients affected by severe facial asymmetry who had undergone MC and orthognathic surgery between January 2018 and June 2020 at the Face Surgery Center, in Parma, Italy. Results: The final study sample included 20 patients (12 women and 8 men). The mean age of the patients at the time of surgery was 20.8 years (range: 18–25 years). At the one-year follow-up, all patients had stable occlusion with a symmetric face. Mandibular angle degree (Ar–Go–Me) increased significantly from 113. 6° to 122.7° at the left side and from 113.3° to 122.7° at the right side (p < 0.05) (Table 1). The mandibular width (Go–Go) decreased from 116.5 to 106.4 mm (p < 0.05). Conclusion: A fully digitalized workflow for one-stage MC and bimaxillary surgery is a safe and valid option to correct facial asymmetry. Several studies have demonstrated accelerated brain aging in Alzheimer’s dementia (AD). Previous studies have also reported that facial asymmetry increases with age. Because obtaining facial images is much easier than obtaining brain images, the aim of this work was to investigate whether AD exhibits accelerated aging patterns in facial asymmetry. We developed new facial asymmetry measures to compare Alzheimer’s patients with healthy controls. A three-dimensional camera was used to capture facial images, and 68 facial landmarks were identified using an open-source machine-learning algorithm called OpenFace. A standard image registration method was used to align the three-dimensional original and mirrored facial images. This study used the registration error, representing landmark superimposition asymmetry distances, to examine 29 pairs of landmarks to characterize facial asymmetry. After comparing the facial images of 150 patients with AD with those of 150 age- and sex-matched non-demented controls, we found that the asymmetry of 20 landmarks was significantly different in AD than in the controls (p < 0.05). The AD-linked asymmetry was concentrated in the face edge, eyebrows, eyes, nostrils, and mouth. Facial asymmetry evaluation may thus serve as a tool for the detection of AD.

Hemifacial hyperplasia (HH) is a rare congenital condition involving enlargement of one or more tissues of the face. The treatment is surgically challenging and requires expertise. This manuscript aims to report two similar appearing HH but warranting different surgical treatment. A 19-year-old female and a 14-year-old boy presented with right facial asymmetry since birth and sought correction of the same. Surgical treatment was planned. Based on clinical history, diagnosis and imaging, HH was diagnosed. The first case was entirely a soft tissue abnormality that was treated with debulking while the second case had involvement of facial bones, necessitating surgical recontouring. The facial asymmetry was addressed. Healing was uneventful. Though the aesthetical concern and appearance of the two cases of HH were same, the treatment vastly differed. This was based on the source of asymmetry. Proper diagnosis and informed decision are a key for successful surgical outcome.

Combining double-jaw surgery and orthodontic treatment to correct dentofacial asymmetry is a standard procedure but time consuming and costly. In order to provide alternative solutions, previewing the possible results with precise virtual surgical planning (VSP) system can help clinicians select patients suited for correcting facial asymmetry surgically without orthodontic treatment. In our experiences, after rigid or semirigid fixation of the maxilla, using intermaxillary fixation (IMF) without mandibular bony fixation is another key. This kind of fixation can minimize the chances of changing the condylar position and occlusion after the rotation of the maxillomandibular complex. If the original occlusion can be maintained, then orthodontic treatment can be avoided. Here we presented two facial asymmetry cases who requested surgical correction and refused orthodontic treatment. With careful preoperative planning using VSP, double-jaw surgery with maxillary bony fixation and IMF was conducted and both patients were satisfied by their aesthetic outcomes. With minimal dental compensation, the long-term results were relatively stable.

Background/purpose: Studies have reported the advantages of digital imaging-assisted orthognathic surgery planning, but there is scarce information about a full digital planning modality. This study evaluated the 3D cephalometric-based and patient-reported outcomes of a full digital workflow for orthognathic surgery planning in the treatment of asymmetric maxillomandibular disharmony. Methods: A postoperative 3D image dataset of 30 Taiwanese Chinese patients with asymmetric skeletal Class III deformities who underwent full digital planning for two-jaw surgery were retrieved from the authors’ database. The 3D cephalometric data (dental, skeletal, and soft tissue evaluations) were compared to the ethnicity-matched 3D cephalometric normative values. Patient-reported outcome measure tools regarding postoperative overall appearance and satisfaction with facial areas (ranging from 0 to 100 and 0 to 10, respectively) were administered. The nasal septum has been postulated to have an intrinsic growth power and act as a pacemaker for facial development, its interactions with local craniofacial structures likely to influence facial anatomy and morphology. Recent studies have begun to investigate the link between nasal septum deviation and facial asymmetry; however, the magnitude and mechanisms of this relation are still unclear. This study aimed to analyse the degree of nasal septum deviation in a sample of infants and children (males and females from 0 to 8 years old) and its correlation with the three-dimensional structure of the facial skeleton. The scope was to test whether septal deviation is linked, and might cause, the development of a more asymmetric face. For this aim, 41 3D landmarks (homologous points) were collected on the nasal septum and cranial surface of 46 specimens extracted from medical CT-scans and were analysed using Geometric Morphometrics, Multiple Linear regressions, Multivariate ANOVAs, and Principal Component Analysis (PCA). Results showed no significant correlation between magnitude of septal deviation and the ontogeny (changes in age) or sex of the sample, but a significant association was found between side of deviation and septal deviation magnitude and frequency. The asymmetric PCA reveals that most of the asymmetry identified is fluctuating, and that changes in the asymmetric morphology of the face are not associated to a specific side of septal deviation. In addition, a series of Multivariate ANOVAs showed that age, sex, and septal deviation have no impact on facial asymmetry, with only age impacting the symmetric development of the facial morphology. When looking at factors impacting the general morphology of the face, age is again the only major driving component, with fluctuating asymmetry and sex only approaching significance. These results could imply a certain degree of dissociation between the mechanisms of facial and septal growth and development; however, an investigation of other key developmental stages in facial morphology is needed to further understand the relation between septal deviation and facial growth. Unilateral posterior crossbite (UXB) is a common, severely asymmetric malocclusion, characterized by maxillary hypoplasia and masticatory dysfunction. The aim of this research is to evaluate the asymmetry of mandibular condyles and rami in children with UXB. This comparative cross-sectional study included 33 children with UXB (girls = 15, boys = 18; mean age ± SD = 8.0 ± 1.3 years.months]) and 33 age- and gender-matched controls (girls = 15, boys = 18; mean age ± SD = 8.4 ± 1.3 years.months]). Pre-treatment OPGs were analyzed by comparing the height of condyles and rami between the sides using the method by Habets et al. (1988); the result was considered significant if the degree of asymmetry was >6%. Children with UXB showed a significantly increased asymmetry of mandibular condyles (mean ± SD = 10.7% ± 9, p < 0.001), but not of rami (mean ± SD = 1.9% ± 2.3), compared to controls. The rami did not show significant asymmetry in either group. The presence of an increased condylar asymmetry index in a developing patient with unilateral posterior crossbite is a sign of altered skeletal growth and should be considered in the diagnostic process and treatment plan. Purpose: The aim of this study was to investigate the presence of depressive symptoms and self-esteem, and their association with facial asymmetry in adults who were operated on in early childhood due to metopic and sagittal craniosynostosis. Methods: The study population consisted of 49 non-syndromic patients of whom 41 had premature fusion of the sagittal and 8 of metopic suture. There were 64 controls from the Finnish National Register. Self-esteem, depressive symptoms, and subjective satisfaction with one's appearance were evaluated by using the Rosenberg self-esteem questionnaire (RSE), the short form of Beck Depression Inventory (R-BDI), and a purpose-designed questionnaire on satisfaction with facial and overall appearance. Aesthetic evaluation was done from standard photographs using panels. The facial symmetry was calculated by using 3D photogrammetric methods. Results: Patients did not have a lower self-esteem or experience significantly more moderate or severe depressive symptoms. However, 20% of the patients and 6% of the controls (p = 0.041) experienced mild depressive symptoms. There was no difference between self-assessed evaluation of appearance between the groups. Only weak correlation was found between facial asymmetry and RSE or R-BDI results (cc = 0.27–0.42, p < 0.05). The self-assessed evaluation of facial and overall appearance correlated strongly with the R-BDI results. Conclusion: Non-syndromic craniosynostosis patients are as satisfied with their appearance in adulthood as the control group and do not experience a lower self-esteem or more depressive symptoms. Facial asymmetry does not correlate with low self-esteem or clinically significant depressive symptoms in adulthood. Subjective evaluation of one's appearance correlated with depressive symptoms. Congenital syngnathia is a very rare condition of unknown etiology with multiple clinical manifestations due to different combinations of bony or soft tissue adhesions between the mandible and maxilla. It shares possible association with other congenital anomalies in the head and neck region, or with other syndromes. The aims of the present work were: (1) to perform a literature review on bony syngnathia in order to obtain a general framework on epidemiology and management protocol; (2) to describe a case of a two-year-old boy with a left unilateral bony fusion treated with a distraction protocol of three months. Original articles were searched through PubMed, Cochrane Central database and Embase with a cut-off date of June 2022. In total, 94 articles were identified through database searching. After 39 exclusions, 55 articles were included in the review process. Eighty-seven cases of bony syngnathia have been reported from 1936 to 2022, and different classifications have been suggested in terms of location, extension, functional outcomes and management protocol. Complications may range from feeding difficulty to aspiration pneumonia and respiratory arrest. Early surgery is generally required to release the ankylosis and to establish good mandible function. Objective: During the process of neural recovery after facial palsy, distressing sequelae of static and dynamic facial asymmetry develop in patients with facial palsy. A pronounced nasolabial fold is mainly responsible for static facial asymmetry, which leads to many psychological and social problems in patients. Objective and qualitative assessment of facial appearance is critical to determine the severity of sequelae of static facial asymmetry and whether an intervention is effective for treatment. In the present study, an attempt was made to develop three-dimensional analysis method to assess sequelae of static facial asymmetry after facial palsy.Methods: Eight patients with sequelae of facial asymmetry after facial palsy and ten healthy volunteers were enrolled. We used three-dimensional scanning analysis with a portable non-contact optical scanner to obtain three-dimensional surface data from a patient's face and produced a three-dimensional digital model of the face. We then identified a reference plane fixed with the patient's face, and measured the depth of the nasolabial fold of the face.Results: The nasolabial fold of the face on the affected side was significantly deeper than that on the unaffected side in patients with sequelae of static facial asymmetry after facial palsy. However, the depth of the facial nasolabial fold on the right side was not different from that on the left side in healthy volunteers. Affected-unaffected side differences in the depth of the nasolabial fold in patients with sequelae of static facial asymmetry after facial palsy were significantly larger than left-right differences in the depth of the nasolabial fold in healthy volunteers. Two weeks after treatment with botulinum toxin injection to the affected zygomaticus muscles, affected-unaffected side differences in the depth of the nasolabial fold were significantly decreased in the patients. In the patients who received botulinum toxin, the absolute values of affected-unaffected side differences in the depth of the nasolabial fold measured using the three-dimensional scanning analysis showed a significant positive correlation with the visual analog scale scores of facial asymmetry marked by independent doctors.Conclusion: The present findings suggest that affected-unaffected side differences in the depth of the nasolabial fold of the measured using three-dimensional scanning analysis can be used as an index of sequelae of static facial asymmetry. Facial symmetry affects the perception of facial beauty. Overall, facial harmony with an appropriate facial proportion and satisfactory midline facial symmetry are crucial factors for facial attractiveness. The role of orthodontists in correcting facial asymmetry begins with providing suitable diagnosis of the condition and identifying patients' expectations. Through a thorough, subjective clinical evaluation of dentofacial asymmetry, the procedures for the surgical orthodontic management of facial asymmetry are discussed. The aim of this clinical review is to provide information on surgical indications for facial asymmetry, optimal treatment goals, presurgical orthodontic preparation, the surgery-first approach, three-dimensional (3D) surgical simulation, postsurgical orthodontic finishing, and treatment outcome evaluation. A comprehensive 3D diagnosis with appropriate planning, accurate transference of surgical simulation to real surgery, slight overcorrection, periodic assessment of treatment outcomes, and awareness of treatment limitations are essential to improve treatment outcomes of facial asymmetry. Background: Unilateral posterior crossbite, one of the most frequent malocclusions, is often associated with functional lateral shift of the mandible. Although the effects of functional lateral shift on the mandible and temporomandibular joint have been examined in various animal experiments, cranial and maxillary changes have received less attention. Objective: The aim of this study was to investigate the effects of functional lateral shift on the craniofacial complex in growing rats. Methods: Eighty 5‐week‐old male Sprague–Dawley rats were randomly divided into an experimental group (n = 40), which received an oblique guide appliance that shifted the mandible to the left during closure, and a control group (n = 40). The rats were scanned by cone‐beam computed tomography at 3 days and 1, 2, 4 and 8 weeks. The dimensions of the mandibular bone, condyle, maxilla and cranium were measured. Results: The mandibles of rats in the experimental group were smaller than those of the rats in the control group and were asymmetrical. The condyles of the rats in the experimental group were thinner than those of the control rats. The condylar length on the ipsilateral side was shorter and wider than that on the contralateral side from 4 to 8 weeks. No significant differences in cranial length or height were observed between the experimental and control groups. The height of the upper first molar and alveolar bone on the contralateral side was significantly smaller than that on the ipsilateral side and in the controls from 4 to 8 weeks. Background: Craniofacial fibrous dysplasia (CFD) affecting the jaws can result in facial asymmetry, posing a unique challenge for dental implant placement in edentulous areas. This case report introduces an innovative approach for rehabilitating the posterior maxilla affected by CFD using a computer-guided technique for maxillary recontouring and substitution bone grafting at implant sites., Case Presentation: A 58-year-old edentulous female with a craniofacial fibrous dysplasia bone lesion affecting the right maxilla was referred for correction of asymmetry and dental implant placement. A computer guide was devised for shaving and contouring the right maxilla. Following virtual maxillary reduction, a second guide was created over the osteomized 3D model to plan the location of future dental implants and guide bone ostectomies for allograft substitution. Particulate bone substitute was applied to ostectomy sites in the affected maxillary alveolar ridge. Four implants were placed successfully four months post-operation, with integration confirmed by clinical examination after one year of follow-up., Conclusion: This treatment approach emerges as a reliable and effective modality for simultaneous dental implant rehabilitation in edentulous alveolar bone and jaw contouring in CFD cases.

Symmetry is present in various aspects of everyday life. A symmetrical face is considered attractive, whereas a lack of facial symmetry is regarded as a source of functional and aesthetic problems. Most of the people exhibit slight asymmetries, but some of them reveal severe asymmetries. Among patients presenting with severe facial asymmetries, there may be those with congenital defects. Congenital defects may manifest at the time of birth or be a result of birth trauma.One of the most prevalent asymmetrical birth defects is cleft lip and/or cleft palate. Other congenital defects include craniofacial syndromes, such as Treacher Collins syndrome (TCS) and Goldenhar syndrome. Among the rare syndromes with facial asymmetries, Klippel-Feil syndrome (KFS), PHACE (posterior fossa brain malformation, hemangiomas, arterial anomalies, cardiac anomalies, and eye abnormalities) syndrome, plagiocephaly, and Parry-Romberg syndrome are worth noticing. The majority of craniofacial asymmetries require surgery to improve the patient's facial appearance. The treatment is multidisciplinary and long, and the most common procedures involve reparative and regenerative surgeries. The aim of this review was to present the most common congenital defects with facial asymmetry.

Objective: To investigate the highest opportunity skin puncture point and needle orientation according to facial asymmetry and classification., Methods: Computed tomography of 136 patients was analyzed. Horizontal and vertical angles and distances from the canthal-tragal line were investigated to determine the puncture point and needle's angle., Result: All patients' average points were 7.39 (±2.85) mm anterior to the tragus and 3.44 (±4.18) mm below the canthal-tragal line with an angle of 8.53 (±6.90)° anteriorly and 32.26 (±7.23)° superiorly. Regarding asymmetry, there was a statistical difference in horizontal angle, depth, and canthal-tragal distance between the deviated and non-deviated sides. Especially, vertical distances were 4.44 (±4.66) mm and 2.59 (±4.11) mm in the deviated and non-deviated sides, respectively ( p < 0.001)., Conclusion: In closed-mouth, the puncture point was closer to the tragus and lower than the conventional point. The point in the deviated side should be considered lower than the non-deviated side.

Subtle craniofacial dysmorphology has been reported in schizophrenia patients. This dysmorphology includes midline facial elongation, frontonasal anomalies and a sexually dimorphic deviation from normal directional asymmetry of the face, with male patients showing reduced and female patients showing enhanced facial asymmetry relative to healthy control subjects. GFAP.HMOX1 0-12m transgenic mice (Mus musculus) that overexpress heme oxygenase-1 in astrocytes recapitulate many schizophrenia-relevant neurochemical, neuropathological and behavioral features. As morphogenesis of the brain, skull and face are highly interrelated, we hypothesized that GFAP.HMOX1 0-12m mice may exhibit craniofacial anomalies similar to those reported in persons with schizophrenia. We examined craniofacial anatomy in male GFAP.HMOX1 0-12m mice and wild-type control mice at the early adulthood age of 6-8 months. We used computer vision techniques for the extraction and analysis of mouse head shape parameters from systematically acquired 2D digital images, and confirmed our results with landmark-based geometric morphometrics. We performed skull bone morphometry using digital calipers to take linear distance measurements between known landmarks. Relative to controls, adult male GFAP.HMOX1 0-12m mice manifested craniofacial dysmorphology including elongation of the nasal bones, alteration of head shape anisotropy and reduction of directional asymmetry in facial shape features. These findings demonstrate that GFAP.HMOX1 0-12m mice exhibit craniofacial anomalies resembling those described in schizophrenia patients, implicating heme oxygenase-1 in their development. As a preclinical mouse model, GFAP.HMOX1 0-12m mice provide a novel opportunity for the study of the etiopathogenesis of craniofacial and other anomalies in schizophrenia and related disorders., (© 2024 The Authors. Objectives: Unilateral condylar hyperplasia (UCH), marked by progressive condylar growth, occurs between ages 11-30 with unclear etiology. This retrospective study aims to clarify the clinical, radiographic, and demographic features of UCH to improve its diagnosis and treatment., Material and Methods: Data for all patients included in this study were retrieved from the archive. Inclusion criteria were the diagnosis of UCH supported by a positive bone scan above 10% of condylar uptake in SPECT/CT, the patient's history of progressive facial asymmetry, symptoms confirmed by the orthodontist during follow-up, and clinical and radiological evaluation. Demographic characteristics, along with volumetric and linear measurements, were statistically evaluated., Results: Forty-one patients were included in this study with 70.73% female predominance. Patients were classified as transverse and vertical type condylar hyperplasia (CH) according to their prominent asymmetry characteristics. Eighteen patients were classified as vertical type CH and 23 patients were classified as transverse type CH. Twenty had left-side condylar overgrowth, and 21 had right-side overgrowth. Condylar volume difference was significantly higher in vertical type CH patients compared to transverse type CH (p = 0.004). Background: Objective assessment of facial movements remains pivotal for diagnosis, treatment, and long-term monitoring of patients with facial palsy (FP). This study aims to utilize curvature analysis in pediatric patients with and without FP to define facial contours and to quantify three-dimensional (3D) excursion during smile., Methods: Pediatric patients with and without FP had 3D motion capture acquired from rest to maximum smile positions. 3D curvature analysis was performed with a custom MATLAB algorithm. A 40-vertex "smile curve" along the nasolabial fold-cheek-orbit contour was extracted from each patient-specific 3D mesh hemiface. This allowed the quantification of excursion and asymmetry for affected vs. unaffected sides in FP patients, and comparison with controls., Results: Twelve FP (mean 12.5 yr, range 5-18) and 12 control (mean 12.1 yr, range 5-18) patients were analyzed. Differences in facial contour are intensified during animation, especially in the nasolabial fold, cheek, and periorbital regions of interest and the smile curve that links them. Mean excursion differed significantly between the FP and control groups in all regions of interest (p < 0.0001)., Conclusions: Curvature analysis provides insight into the unexplored relationship of the 3D morphological dynamics of the concavity and convexity of the face, to add to the understanding of this complex condition. Trabecular juvenile ossifying fibroma (TrJOF) is a rare, benign, fibro-osseous lesion that predominantly affects children and adolescents. The lesion is locally aggressive, has a high recurrence rate, and is often misdiagnosed due to its clinical and radiographic similarities to other lesions. A seven-year-old female presented with a history of swelling and pain on the right side of her face for the last month. Extraoral examination revealed facial asymmetry and exophthalmos of the right eye. Intraoral examination showed obliteration of the buccal vestibule, though the mucosal lining and teeth were intact. Cone-beam computed tomography (CBCT) demonstrated a large, expansile, osteolytic lesion occupying the right maxillary sinus and adjacent structures, suggesting a fibro-osseous lesion. The enucleated specimen showed a highly cellular connective tissue stroma with irregular bony trabeculae rimmed by osteoblasts, numerous multinucleated giant cells, and basophilic cementum-like calcifications. The histopathological evaluation confirmed the diagnosis of trabecular juvenile ossifying fibroma. This case highlights the diagnostic challenges of TrJOF due to its overlapping features with other fibro-osseous and giant cell lesions. Study Design: Retrospective with follow-up., Objective: To evaluate the long-term satisfaction of surgically treated patients with zygomaticomaxillary complex (ZMC) fractures in relation to the use of internal fixation, number of fixation points, and orbital floor (OF) reconstruction. Secondary objectives were to describe the use of antibiotics and post-operative infections., Methods: Patients with unilateral ZMC fractures between 2007-2018 and treatment with either open reduction and internal fixation (ORIF) or closed reduction (CR) were identified from medical records and invited to follow-ups between 2018-2020. Patients were examined, photographed, and completed a questionnaire. A review panel of 3 experienced surgeons evaluated photographs and computed tomography (CT) scans pre- and post-surgery., Results: The study sample consisted of 136 patients (108 ORIF, 28 CR) with a median follow-up time of 76 months. Patient satisfaction of surgical outcome was high (97.8%), with no significant differences in relation to the use of internal fixation, number of fixation points, or OF reconstruction. Dissatisfaction was primarily related to hypoesthesia. On post-operative CT scans, malar asymmetry was more often predicted in patients with 1-point fixations. On questionnaires and photographs, malar asymmetry was more common in patients with 3-point fixations. Prophylactic antibiotics had no effect on the rate of post-operative infections., Conclusions: Patient satisfaction was not influenced by internal fixation, number of fixation points, or OF reconstruction. Selected ZMC fractures can be treated with less invasive approaches. Caution should be observed when predicting long-term malar asymmetry on post-operative CT scans. Objective: The aim of this study was to find out how the preferred chewing side (PCS) affects facial asymmetry, what kind of factors affect PCS, and whether there are differences in facial asymmetry between symmetrical and asymmetrical masticators., Material and Methods: The study included 748 subjects (females n=452, males n=296) born in 1985-1986 in Northern Finland (Northern Finland Birth Cohort 1986, NFBC 1986). Subjects' faces were captured in facial 3D images with stereophotogrammetry technology, and they filled in a questionnaire concerning oral health. A comprehensive dental examination was done by a dentist. Subject´s chewing side preference was studied by chewing a piece of paraffin, cotton roll or parafilm. Asymmetry was measured from 3D images with different asymmetry measurements and facial landmarks., Results: Reduced number of teeth on contralateral side affects PCS (OR = 2.44 in the case of one tooth is missing). Being female increased the whole face and lower face symmetry (p values <0.001-0.824). Self-reported TMD pain has an effect on the sidedness of the chin; there is more pain in the larger side of the chin (OR = 9.45). Different chewing materials had no significant effect on the proportion of chewing sides., Conclusions: Females have a more symmetrical face compared to males. PCS does not have a statistically significant effect on facial asymmetry, but the variable affecting PCS itself is extracted teeth.

Bell's palsy, an acute, idiopathic, and typically unilateral facial nerve paralysis, represents a common cause of sudden facial weakness. The aetiology is often attributed to viral infections. This case report discusses the presentation, diagnosis, and management of a rare case of idiopathic bilateral Bell's palsy. We present a case of a 31-year-old male who presented to the emergency department with a one-week history of progressive bilateral facial weakness following initial neck and jaw pain. Despite the resolution of pain, the patient experienced complete facial paralysis on both sides, including the inability to raise eyebrows, close eyes fully, and numbness over the lips. The patient presented with no complaints of headache, trauma, vision changes, or recent travel history. Examination and routine blood tests yielded normal results, and a head CT scan showed no abnormalities. As a result, the diagnosis of idiopathic bilateral Bell's palsy was confidently confirmed. Purpose: This retrospective study aimed to evaluate sequential changes in soft tissue thickness after bilateral sagittal split ramus osteotomy (BSSRO) in skeletal class III patients with facial asymmetry and to explore their correlation with surgical movements for optimal postoperative facial symmetry and esthetic outcomes., Material and Methods: This study included 37 patients with class III malocclusion and > 4 mm Menton (Me) deviation who underwent BSSRO. Posteroanterior cephalograms were captured at preoperative (T0), 6 weeks (T1), 6 months (T2), and 1 year (T3) postoperative intervals to analyze changes in Me deviation, fronto-ramal inclination (FRI), and soft tissue thickness. Statistical analysis was conducted to assess the changes in soft tissue thickness over time and the effects of surgical correction., Results: Significant improvements in facial asymmetry were noted after surgery, with reductions in Me deviation and FRI on both the deviated side (DS) and non-deviated side (NDS). An increase in soft tissue thickness was observed on both the DS and NDS after surgery, with the NDS showing a continued increase between 6 months and 1 year, indicating an ongoing compensation for symmetry restoration. The study also identified a positive correlation between the surgical movement of the FRI and the increase in soft tissue width on the NDS after 6 months., Conclusion: This study established that soft tissue thickness continues to adapt and change up to 1 year after BSSRO, underscoring the need for a long-term evaluative approach in orthognathic surgery for patients with facial asymmetry., Competing Interests: Declaration of competing interest The authors declare no competing interests., (Copyright © 2024 The Authors. Total joint reconstruction (TJR) has become the most preferred method of reconstruction in recent years for hemifacial microsomia (HFM). This requires meticulous planning for the glenoid fossa and ramus prosthesis along with the arch in certain indications. TJR in a type V HFM is extremely challenging due to agenesis/hypolasia of the zygoma and arch which compromises anchorage of the glenoid prosthesis. Conventional options used for such indications incorporate extended designs for the fossa. However, the two designs used commonly are associated with limitations; (i) the temporal extension is overtly large and cannot be anchored in patients with thin temporal bone and (ii) the glenoid fossa incorporated into the arch prosthesis, where the fossa is not positioned on stable bone and occlusal loading is non-physiological; transmitted onto the arch prosthesis rather than the skull base. The authors propose the use of the mastoid process as an alternative for anchorage of fossa prosthesis. This technique offers stable anchorage while facilitating ideal positioning on the skull base for optimal masticatory load transmission. Abstract Background There is currently no information on how different mandibular segments are affected by juvenile idiopathic arthritis. The aim of this paper is to assess volumetric differences of different mandibular segments in subjects with unilateral and bilateral JIA and to compare them with non-JIA control volumes. Materials and methods Forty subjects with unilateral TMJ involvement and 48 with bilateral TMJ involvement were selected for the case group and 45 subjects with no known rheumatic comorbidities for the control group. The mandible of each subject was divided according to a validated method into different paired volumes (hemimandible, condyle, ramus and hemibody). Results The ANOVA test revealed a statistically significant difference in all the groups for condylar and ramus volumes, and the pairwise comparison evidenced a statistically significant higher condylar and ramus volume in the control group (1444.47 mm3; 5715.44 mm3) than in the affected side in the unilateral JIA group (929.46 mm3; 4776.31 mm3) and the bilateral JIA group (1068.54 mm3; 5715.44 mm3). Moreover, there was also a higher condylar volume in the unaffected side in the unilateral JIA group (1419.39 mm3; 5566.24 mm3) than in the bilateral JIA group and the affected side in the unilateral JIA group. Conclusions The affected side of unilateral JIA patients showed statistically significant lower volumes in the hemimandible, in the condyle and in the ramus. The largest total mandibular volume was observed in the control group, followed by the unilateral JIA group and, lastly, by the bilateral JIA group.

Abstract Objectives The purpose of this study was to compare differences in facial soft tissue thickness in three-dimensional (3D) images before and after orthognathic surgery in patients with skeletal Class III malocclusion and to obtain a better understanding of the relationship between hard and soft tissue changes after surgery. Materials and method The present retrospective study included 31 patients with skeletal Class III malocclusion with mandibular chin deviation greater than 4 mm who had undergone cone-beam computed tomography before and 6 months after surgery. Seven bilateral points were established. Measurements were taken from software-generated multiplanar reconstructions. The predictor variables were timing (pre- and postoperatively) and side (deviated vs. nondedicated). A regression model and correlation analysis were conducted for statistical analysis. Results The difference of bilateral facial soft tissue thickness was statistically significantly different between deviated and nondeviated sides (P < 0.05), with lower values observed on the deviated side. The soft tissue thickness has become nearly symmetric at local regions of the lower thirds of the face after orthognathic surgery. However, most measurements showed a negative correlation between changes in soft tissue thickness and changes in bone tissues. Conclusions Skeletal Class III malocclusion with facial asymmetry is accompanied by differences in soft tissue thickness when comparing Dev and N-Dev sides of the posterior region of the mandible, where soft tissues are thinner on the Dev side. Soft tissue thickness can compensate for or camouflage the underlying asymmetric mandible. In addition, the asymmetric soft tissue thickness on the lower third of the face can be partially improved by orthognathic surgery, but the amount of soft tissue thickness change is not consistent with that of hard tissue positional change.
