OBJECTIVE: Smoking is a serious problem that has a devastating impact on health. The objective of this study was to describe the prevalence of and factors influencing smoking among medical and non-medical students in Tbilisi, Georgia, as well as to determine whether medical education has an impact on smoking. METHODS: A cross-sectional study was carried out at Tbilisi State Medical University and Tbilisi State University, both of which are located in Tbilisi, Georgia. A total of 400 4th-year students (200 students at each university) were asked to complete standardized questionnaires. RESULTS: Of the sample as a whole, 48.75% were identified as smokers and 51.25% were identified as nonsmokers. The mean age was 20.24 years among smokers and 20.26 years among nonsmokers. Of the medical students, 49.5% were smokers, as were 48.0% of the non-medical students. The male-to-female ratio in the study population was 0.9:1.1. Smoking was found to have a strong relationship with gender, males accounting for 65% of all smokers. Of the smokers, 56.9% stated that they would like to quit smoking (for health or financial reasons). Of the medical students, 59.5% expressed a willingness to quit smoking, as did 54.2% of the non-medical students. CONCLUSIONS: There is a need to improve smoking education for undergraduate students. Special attention should be given to the inclusion of anti-smoking education in undergraduate curricula, as well as to the implementation of smoking prevention campaigns at institutions of higher education. However, such measures will be effective only if tobacco control policies are strictly enforced on the national level as well. OBJETIVO: O tabagismo é um problema sério, cujo impacto na saúde é devastador. O objetivo deste estudo foi descrever a prevalência do tabagismo e os fatores que o influenciam em estudantes de medicina e outros universitários em Tbilisi, Geórgia, bem como determinar se a educação médica tem impacto no tabagismo. MÉTODOS: Foi realizado um estudo transversal na Universidade Médica Estadual de Tbilisi e na Universidade Estadual de Tbilisi, ambas em Tbilisi, Geórgia. Pedimos a 400 alunos de quarto ano (200 em cada universidade) que respondessem a questionários padronizados. RESULTADOS: Do total da amostra, 48,75% eram fumantes e 51,25% eram não fumantes. A média de idade dos fumantes foi de 20,24 anos e a dos não fumantes foi de 20,26 anos. Dos estudantes de medicina, 49,5% eram fumantes, assim como o eram 48,0% dos universitários que não estudavam medicina. A razão entre os gêneros masculino e feminino foi de 0,9:1,1. O tabagismo apresentou forte relação com o gênero; 65% dos fumantes eram do sexo masculino. Dos fumantes, 56,9% disseram que gostariam de parar de fumar (por motivos de saúde ou financeiros). Dos estudantes de medicina, 59,5% disseram que estavam dispostos a parar de fumar, assim como o disseram 54,2% dos universitários que não estudavam medicina. CONCLUSÕES: É preciso melhorar a educação sobre o tabagismo nas universidades. Deve-se dar atenção especial à inclusão de educação antitabagismo no currículo das universidades e à implantação de campanhas de prevenção ao tabagismo em instituições de ensino superior. Entretanto, essas medidas só serão eficazes se as políticas de controle do tabaco forem rigorosamente observadas também em nível nacional.

OBJECTIVE: To compare lung function between patients with post-poliomyelitis syndrome and those with sequelae of paralytic poliomyelitis (without any signs or symptoms of post-poliomyelitis syndrome), as well as between patients with post-poliomyelitis syndrome and healthy controls. METHODS: Twenty-nine male participants were assigned to one of three groups: control; poliomyelitis (comprising patients who had had paralytic poliomyelitis but had not developed post-poliomyelitis syndrome); and post-poliomyelitis syndrome. Volunteers underwent lung function measurements (spirometry and respiratory muscle strength assessment). RESULTS: The results of the spirometric assessment revealed no significant differences among the groups except for an approximately 27% lower mean maximal voluntary ventilation in the post-poliomyelitis syndrome group when compared with the control group (p = 0.0127). Nevertheless, the maximal voluntary ventilation values for the post-poliomyelitis group were compared with those for the Brazilian population and were found to be normal. No significant differences were observed in respiratory muscle strength among the groups. CONCLUSIONS: With the exception of lower maximal voluntary ventilation, there was no significant lung function impairment in outpatients diagnosed with post-poliomyelitis syndrome when compared with healthy subjects and with patients with sequelae of poliomyelitis without post-poliomyelitis syndrome. This is an important clinical finding because it shows that patients with post-poliomyelitis syndrome can have preserved lung function. OBJETIVO: Comparar a função pulmonar de pacientes com síndrome pós-poliomielite à de pacientes com sequelas de poliomielite paralítica (sem quaisquer sinais ou sintomas de síndrome pós-poliomielite) e à de sujeitos saudáveis. MÉTODOS: Vinte e nove sujeitos do sexo masculino foram divididos em três grupos: controle, poliomielite (pacientes que sofreram de poliomielite paralítica, mas que não apresentaram síndrome pós-poliomielite) e síndrome pós-poliomielite. Os voluntários foram submetidos a avaliações da função pulmonar (espirometria e avaliação da força muscular respiratória). RESULTADOS: Os resultados da espirometria não revelaram diferenças significantes entre os grupos, à exceção da ventilação voluntária máxima, cuja média no grupo síndrome pós-poliomielite foi aproximadamente 27% mais baixa que no grupo controle (p = 0,0127). No entanto, os valores de ventilação voluntária máxima observados no grupo pós-poliomielite foram comparados aos da população brasileira e se apresentaram dentro da faixa normal. Não foram observadas diferenças significantes entre os grupos no tocante à força muscular respiratória. CONCLUSÕES: À exceção da ventilação voluntária máxima mais baixa, não houve comprometimento significante da função pulmonar em pacientes ambulatoriais com diagnóstico de síndrome pós-poliomielite quando comparados a pacientes com sequelas de poliomielite, mas sem a síndrome pós-poliomielite e a sujeitos saudáveis. Trata-se de um importante achado clínico, pois mostra que pacientes com síndrome pós-poliomielite podem apresentar função pulmonar preservada.

OBJECTIVE: To identify which noninvasive ventilation (NIV) masks are most commonly used and the problems related to the adaptation to such masks in critically ill patients admitted to a hospital in the city of São Paulo, Brazil. METHODS: An observational study involving patients ≥ 18 years of age admitted to intensive care units and submitted to NIV. The reason for NIV use, type of mask, NIV regimen, adaptation to the mask, and reasons for non-adaptation to the mask were investigated. RESULTS: We evaluated 245 patients, with a median age of 82 years. Acute respiratory failure was the most common reason for NIV use (in 71.3%). Total face masks were the most commonly used (in 74.7%), followed by full face masks and near-total face masks (in 24.5% and 0.8%, respectively). Intermittent NIV was used in 82.4% of the patients. Adequate adaptation to the mask was found in 76% of the patients. Masks had to be replaced by another type of mask in 24% of the patients. Adequate adaptation to total face masks and full face masks was found in 75.5% and 80.0% of the patients, respectively. Non-adaptation occurred in the 2 patients using near-total facial masks. The most common reason for non-adaptation was the shape of the face, in 30.5% of the patients. CONCLUSIONS: In our sample, acute respiratory failure was the most common reason for NIV use, and total face masks were the most commonly used. The most common reason for non-adaptation to the mask was the shape of the face, which was resolved by changing the type of mask employed. OBJETIVO: Identificar as interfaces de ventilação não invasiva (VNI) mais utilizadas e os eventuais problemas relacionados a sua adaptação em pacientes críticos internados em um hospital na cidade de São Paulo, SP. MÉTODOS: Estudo observacional, com pacientes acima de 18 anos admitidos nas unidades de pacientes graves que usaram VNI. Foram coletados a causa de uso da VNI, interface utilizada, esquema de uso da VNI, adaptação e motivos para a falta de adaptação à interface. RESULTADOS: Avaliamos 245 pacientes, com média de idade de 82 anos. A insuficiência respiratória aguda foi a causa mais frequente de uso da VNI (71,3%). A interface mais utilizada foi a máscara facial total (74,7%), seguida por máscara facial e máscara facial quase total (24,5% e 0,8%, respectivamente). Em 82,4% dos pacientes, a VNI foi utilizada por períodos. A adaptação adequada à interface ocorreu em 76% dos pacientes. As máscaras tiveram de ser trocadas em 24% dos pacientes. Houve adaptação adequada à máscara facial total e à máscara facial em 75,5% e em 80,0% dos pacientes, respectivamente. A adaptação foi inadequada nos 2 pacientes que utilizaram a máscara facial quase total. A causa mais frequente da falta de adaptação foi o formato da face, em 30,5% dos pacientes. CONCLUSÕES: Nesta amostra, a insuficiência respiratória aguda foi a causa mais frequente de uso da VNI, e a máscara facial total foi a interface mais utilizada. A causa mais comum da falta de adaptação à máscara foi o formato da face, que foi resolvida após a troca da interface.

OBJECTIVE: Lung cancer (LC) is the leading cause of cancer-related death and represents a major public health problem worldwide. Another major cause of morbidity and mortality, especially in developing countries, is tuberculosis. The simultaneous or sequential occurrence of pulmonary tuberculosis and LC in the same patient has been reported in various case series and case-control studies. The objective of this study was to describe the characteristics of patients developing tuberculosis and LC, either simultaneously or sequentially. METHODS: This was a cross-sectional study based on the review of medical charts. RESULTS: The study involved 24 patients diagnosed with tuberculosis and LC between 2009 and 2012. The diagnoses of tuberculosis and LC occurred simultaneously in 10 patients, whereas tuberculosis was diagnosed prior to LC in 14. The median time between the two diagnoses was 5 years (interquartile range: 1-30 years). Fourteen patients (58.3%) were male, 20 (83.3%) were White, and 22 (91.7%) were smokers or former smokers. The most common histological type was adenocarcinoma, identified in 14 cases (58.3%), followed by epidermoid carcinoma, identified in 6 (25.0%). Seven patients (29.2%) presented with distant metastases at diagnosis; of those 7 patients, 5 (71%) were diagnosed with LC and tuberculosis simultaneously. CONCLUSIONS: In the present study, most of the patients with tuberculosis and LC were smokers or former smokers, and tuberculosis was diagnosed either before or simultaneously with LC. Non-small cell lung cancer, especially adenocarcinoma, was the most common histological type. OBJETIVO: O câncer de pulmão (CP) é o tipo mais letal de câncer na população mundial e representa um importante problema de saúde pública. A tuberculose é outra causa significativa de morbidade e mortalidade, especialmente em países em desenvolvimento. A ocorrência de tuberculose pulmonar e CP no mesmo paciente, simultaneamente ou não, tem sido descrita em diversas séries de casos e estudos de caso-controle. O objetivo deste estudo foi descrever as características de pacientes com tuberculose e CP, com apresentação simultânea ou sequencial. MÉTODOS: Estudo transversal, realizado através de revisão de prontuários. RESULTADOS: Foram incluídos no estudo 24 pacientes com diagnósticos de tuberculose e CP no período entre 2009 e 2012. Os diagnósticos de tuberculose e CP ocorreram simultaneamente em 10 pacientes, enquanto a tuberculose foi diagnosticada antes do CP em 14. A mediana do tempo entre os diagnósticos foi de 5 anos (variação interquartil: 1-30 anos). Quatorze pacientes (58,3%) eram do sexo masculino, 20 (83,3%) eram brancos, e 22 (91,7%) eram tabagistas ativos ou ex-tabagistas. O tipo histológico mais comum foi o adenocarcinoma, em 14 casos (58,3%), seguido de carcinoma epidermoide, em 6 (25,0%). Sete pacientes (29,2%) já apresentavam metástases à distância no momento do diagnóstico; desses 7, 5 (71%) tiveram o diagnóstico de CP e TB simultaneamente. CONCLUSÕES: Neste estudo, a maioria dos pacientes com tuberculose e CP eram tabagistas ou ex-fumantes e a tuberculose foi diagnosticada tanto antes quanto simultaneamente ao diagnóstico de CP. O carcinoma brônquico não pequenas células, em especial o adenocarcinoma, foi o tipo histológico mais comum.

To evaluate the association between fraction of exhaled nitric oxide (FeNO) values and forced expiratory volume in the first second (FEV1) and the level of asthma control, as proposed by the Global Initiative for Asthma (GINA), in asthmatic children and adolescents attended at the National Institute of Women, Children and Adolescents Health Fernandes Figueira of Fundação Oswaldo Cruz (IFF/FIOCRUZ).This was a cross-sectional study, with a review of medical records of 90 asthmatics between 7 and 17 years old, who were followed up at the IFF/FIOCRUZ Asthma Outpatient Clinic and were referred to perform respiratory function tests (RFT)between March 2013 and September 2014. After classification according to GINA, patients performed complete spirometry and FeNO measurement. Subsequently, they were separated into two groups: regular and non-regular inhaled corticosteroid (ICS) use, regardless of the ventilatory pattern in spirometry.The association between FEV1 values and the degree of asthma control according to GINA (p=0.001) was observed in all patients, regardless of ICS use, but there was no association between FEV1 and levels of FeNO.The correlation observed between GINA and FEV1 reinforces the importance of spirometry in the clinical follow-up of these patients. Although no association was found between the value of FeNO and the degree of asthma control and FEV1, FeNO may be an early method to detect airway inflammation, even before the symptoms and spirometric changes.Avaliar a associação entre os valores da fração exalada de óxido nítrico (FeNO) e do volume expiratório forçado no primeiro segundo (VEF1) e o grau de controle da asma, segundo proposta do Global Initiative for Asthma (GINA), em crianças e adolescentes asmáticos atendidos no Instituto Nacional de Saúde da Mulher, da Criança e do Adolescente Fernandes Figueira da Fundação Oswaldo Cruz(IFF/FIOCRUZ).Estudo transversal, com revisão de prontuários de 90 asmáticos entre 7 e17 anos de idade, acompanhados no Ambulatório de Asma do IFF/FIOCRUZ e encaminhados ao setor de Prova de Função Respiratória (PFR) entre março de 2013 e setembro de 2014. Após classificação segundo o GINA, os pacientes realizaram espirometria completa e medida da FeNO. Posteriormente, foram separados em dois grupos: em uso regular e não regular de corticosteroide inalatório (CI), independentemente do padrão ventilatório na espirometria.Observou-se associação entre os valores de VEF1 e o grau de controle da asma segundo o GINA (p=0,001) em todos os pacientes analisados, independentemente do uso de CI, mas não houve associação entre os valores de VEF1 e os níveis da FeNO.A correlação observada entre o GINA e o VEF1 reforça a importância da espirometria no seguimento clínico desses pacientes. Embora não tenha sido detectada associação entre o valor da FeNO e o grau de controle da asma e o VEF1,a FeNO pode constituir um método precoce para detectar inflamação nas vias aéreas, antes mesmo dos sintomas e das alterações espirométricas.

To analyze if there is variation in the results of spirometry performed by healthy schoolchildren on different days.Cross-sectional study with healthy schoolchildren aged 7 to 12 years old, in Florianópolis, Santa Catarina, Southern Brazil. Prepubescent children, non-athletes, born at full term, without cardiorespiratory, rheumatic, musculoskeletal, neurological and visual or hearing deficit were included. The child's health was assessed by a health questionnaire and by the survey International Study of Asthma and Allergies in Childhood. The spirometric test was performed on 3 different days, not exceeding the interval of 15 days between the first and last exams, following the guidelines of the American Thoracic Society. The absolute values and percentages of predicted values of forced vital capacity, forced volume in the first second and peak expiratory flow were analyzed. Statistical analysis was performed using the Shapiro-Wilk normality test followed by one-way analysis of variance or Friedman test and by Bonferroni's post-hoc test for multiple parametric comparisons. Also, the intraclass correlation coefficient was applied to compare results on the same patients on the different tested days.Twenty-two children were analyzed, with a mean age of 9.3±1.1 years. All the spirometric parameters showed a reduction in absolute value after the first exam, but without significant difference. Regarding reproducibility of spirometry results, the tests presented intraclass correlation coefficient0.70.There was no change in the results of spirometry performed by healthy schoolchildren on three different days.Analisar se há alteração no resultado do exame espirométrico executado por escolares saudáveis, quando realizado em dias diferentes.Estudo transversal com escolares saudáveis com 7 a 12 anos, provenientes de escolas da Grande Florianópolis, Santa Catarina, Brasil. Foram incluídas crianças pré-púberes, não atletas, nascidas a termo, sem qualquer doença cardiorrespiratória, reumática, musculoesquelética, neurológica e déficits visuais ou auditivos. A higidez da criança foi avaliada por meio de um questionário de saúde e do questionário International Study of Asthma and Allergies in Childhood. O exame espirométrico foi realizado em 3 dias diferentes, não excedendo o intervalo de 15 dias entre o primeiro e o último exame, seguindo as diretrizes da American Thoracic Society. Foram analisados os valores absolutos e os percentuais dos valores preditos de capacidade vital forçada, volume forçado no primeiro segundo e pico de fluxo expiratório. A análise estatística foi realizada com o teste de normalidade de Shapiro-Wilk, seguida por análise de variância de uma via ou teste de Friedman e pelo teste post-hoc de Bonferroni para comparações múltiplas paramétricas. Foi aplicado o coeficiente de correlação intraclasse (ICC) para comparar os resultados dos mesmos pacientes entre os diferentes dias.Vinte e duas crianças foram analisadas, com média de idade de 9,3±1,1 anos. Todos os parâmetros espirométricos apresentaram redução numérica no decorrer dos dias avaliados, no entanto, sem diferença significante. Na análise da reprodutibilidade da espirometria, os testes apresentaram coeficiente de correlação intraclasse0,70.Não houve alteração do resultado da espirometria executada por escolares saudáveis em três dias distintos.

OBJECTIVE: To estimate self-reported discrimination due to sexual orientation among men who have sex with men (MSM) in Brazil and to analyze associated factors. METHODS: A cross-sectional study of 3,859 MSM recruited in 2008–2009 with respondent driven sampling. Data collection conducted in health centers in 10 Brazilian cities. A face-to-face questionnaire was used and rapid HIV and syphilis tests conducted. Aggregated data were weighted and adjusted odds ratio estimated to measure the association between selected factors and self-reported discrimination due to sexual orientation. RESULTS: The sample was predominantly young, eight plus years of schooling, pardo (brown), single, low-income, and identified themselves as gay or homosexual. The prevalence of self-reported discrimination due to sexual orientation was 27.7% (95%CI 26.2–29.1). Discrimination was independently associated with: age < 30 years, more years of schooling,community involvement and support, history of sexual and physical violence, suicidal thoughts, and unprotected receptive anal intercourse. CONCLUSIONS: The prevalence of self-reported discrimination among MSM in Brazil is high. These results challenge the assumptions that MSM-specific prevention and support programs are not required or that health professionals do not need special training to address MSM needs.

OBJECTIVE To investigate the association between institutional violence in obstetrics and postpartum depression (PP depression) and the potential effect of race, age, and educational level in this outcome. METHODS This is a cross-sectional study about the health care conditions for the maternal and child population of the Federal District, Brazil, carried out in 2011. The study has used a probabilistic sample of 432 women, whose children were aged up to three months, stratified by clusters. Indicators of institutional violence and demographic characteristics have been used in a logistic regression model to estimate the probability of occurrence of postpartum depression. RESULTS The model has identified a high prevalence of postpartum depression, being it higher among non-white women and adolescent females, besides having a strong positive association between the several indicators of obstetric violence and postpartum depression. Positive interactions on a multiplicative scale have also been observed between: violence by negligence by health care professionals and race and age; physical violence from health care professionals and age; and, verbal violence from health care professionals and race. CONCLUSIONS The indicators adopted to reflect institutional violence in obstetric care are positively associated with postpartum depression, which calls for a reflection on the need to make the health care protocols adequate to the precepts of the Brazilian humanization of childbirth care policies and changes in the obstetric care model.

OBJECTIVE To describe the fifth-year medical students' self-evaluation based on the reflexive discourse of the Activities Diaries (portfolio) from the Pediatric Internship I and Child Care Rotations. METHODS Cross sectional, qualitative and descriptive study using the collective subject discourse of the diaries used during the internship of the Medical School, in Catanduva, São Paulo, from January to November, 2011. The registered students' testimonials in the portfolio sections called self-assessment and students' impression were assessed according to their central ideas (discipline organization, breastfeeding outpatient clinic, number of admissions in the pediatric hospital ward and satisfaction with the Child Health training ), related to the teaching of Pediatrics and Child Care. The portfolios with incomplete registers were excluded. RESULTS The testimonials of 47 interns (75% of the students) were analyzed, and 21.3% of them expressed satisfaction with the discipline organization and 27.7% praised the inclusion of the breastfeeding outpatient clinics in the course. For 25.5% of the academics, the number of admissions in the pediatric wards was insufficient for an ideal learning; however, 70.2% were satisfied with the Child Health training. CONCLUSIONS This critical analysis allowed a summary of the reflections, suggestions and critics registered by the interns and can be used as a tool for improvement of the professional cycle. OBJETIVO Describir la autoevaluación de estudiantes del quinto año de Medicina, a partir del discurso reflexivo de los Diarios de Actividades (cartera) del internato de Pediatría I y Cuidado del Niño. MÉTODOS Estudio transversal, cualitativo de tipo descriptivo, por medio del discurso del sujeto colectivo de los Diarios de Actividades utilizados por la coordinación de internato del referido curso, ubicado en Catanduva, São Paulo (Brasil), de enero a noviembre de 2011. Se incluyeron en el estudio todos los testimonios de los alumnos, registrados en los apartados de la cartera, nombrados autoevaluación e impresión del alumno, los que contienen las ideas centrales (organización de la asignatura, ambulatorio de lactancia materna, número de internaciones en las enfermerías pediátricas y satisfacción con la pasantía en Salud del Niño) relacionadas a la enseñanza de Pediatría y Cuidado del Niño. Se excluyeron los diarios con registros incompletos. RESULTADOS Se analizaron los testigos de 47 internos del quinto año (75% de los alumnos), siendo que el 21,3% manifestaron satisfacción respecto a la organización de la asignatura y el 27,7% elogiaron la inclusión del Ambulatorio de Lactancia Materna en la carrera. Para el 25,5% de los académicos, el número de internaciones en las enfermerías de Pediatría fue insuficiente para un aprendizaje ideal; sin embargo, el 70,2% resultaron satisfechos con la pasantía en Salud del Niño. CONCLUSIONES Este análisis crítico permitió una tabulación de las reflexiones, sugerencias y críticas registradas por los internos en el Diario de Actividades y se puede utilizarlo como instrumento para perfeccionamiento del ciclo profesionalizante. MÉTODOS Estudo transversal, qualitativo do tipo descritivo, por meio do discurso do sujeito coletivo dos Diários de Atividades utilizados pela coordenadoria de internato do referido curso, localizado em Catanduva, São Paulo, de janeiro a novembro de 2011. Incluíram-se no estudo todos os depoimentos dos alunos, registrados nas seções do portfólio, denominadas autoavaliação e impressão do aluno, as quais contêm as ideias centrais (organização da disciplina, ambulatório de aleitamento materno, número de internações nas enfermarias pediátricas e satisfação com o estágio em Saúde da Criança) relacionadas ao ensino de Pediatria e Puericultura. Excluíram-se os diários com registros incompletos. RESULTADOS Analisaram-se os depoimentos de 47 internos do quinto ano (75% dos discentes), sendo que 21,3% manifestaram satisfação quanto à organização da disciplina e 27,7% elogiaram a inclusão do Ambulatório de Aleitamento Materno na graduação. Para 25,5% dos acadêmicos, o número de internações nas enfermarias de Pediatria foi insuficiente para um aprendizado ideal; entretanto, 70,2% ficaram satisfeitos com o estágio em Saúde da Criança. CONCLUSÕES Esta análise crítica permitiu uma organização das reflexões, sugestões e críticas registradas pelos internos e pode ser utilizada como instrumento para aprimoramento do ciclo profissionalizante.

OBJECTIVE: To evaluate the impact of manual vibrocompression and nasotracheal suctioning on heart (hr) and respiratory (rr) rates, peripheral oxygen saturation (SpO2), pain and respiratory distress in infants in the postoperative period of a cardiac surgery. METHODS: Randomized controlled trial, in which the assessments were performed by the same physiotherapist in two moments: before and after the procedure. The infants were randomly divided into two groups: Intervention (IG), with manual chest vibrocompression, nasotracheal suctioning and resting; and Control CG), with 30 minutes of rest. Cardiorespiratory data (SpO2; hr; rr) were monitored and the following scales were used: Neonatal Infant Pain Scale (NIPS), for pain evaluation, and Bulletin of Silverman-Andersen (BSA), for respiratory distress assessment. OBJECTIVE: To assess vitamin E levels in the breast milk, analyzing the prematurity and the birth weight influence in α-tocopherol concentration of colostrum milk. METHODS: Cross-sectional study, in which the colostrum was collected from 93 nursing mothers in a public maternity of Natal, Rio Grande do Norte, Northeast Brazil. The newborns were classified based on gestational age and birth weight. The analysis of α-tocopherol in the milk was carried out by high performance liquid chromatography. RESULTS: The α-tocopherol concentration in the colostrum of lactating women whose children were born at term was 1,093.6±532.4µg/dL; for preterm infants, the concentration was 1,321.6±708.5µg/dL (p=0.109). In the preterm group, the α-tocopherol concentration in the colostrum of lactating women whose children were born with low and normal birth weight was 1,316.0±790.7 and 1,327.2±655.0µg/dL, respectively (p=0.971). In the term group, the α-tocopherol levels were higher in mothers of children with birth weight >4000g, being 1,821.0±575.4µg/dL, compared to 869.5±532.1µg/dL and 1,039.6±477.5µg/dL with low and adequate birth weight, respectively (p>0.05). CONCLUSIONS: Prematurity did not influence α-tocopherol levels in the colostrum milk. Mothers who had macrossomic term neonates presented increased α-tocopherol levels. These results indicate that birth weight can influence α-tocopherol leves in the colostrum milk. MÉTODOS: Estudio transversal, con colecta de leche calostro de 93 mujeres atendidas en una maternidad pública de Rio Grande do Norte (Brasil). Los recién nacidos fueron clasificados con base en la edad gestacional y en el peso al nacer. El α-tocoferol fue determinado por cromatografía líquida de alta eficiencia. RESULTADOS: La concentración de α-tocoferol en el calostro de lactantes cuyos hijos nacieron a término fue de 1.093,6±532,4µg/dL; en el caso de recién nacidos pretérmino, la concentración fue de 1.321,6±708,5µg/dL (p=0,109). En el grupo pretérmino, las mujeres con neonatos de bajo peso y de peso adecuado presentaron valores de α-tocoferol de 1.316,0±790,7 y 1.327,2±655,0µg/dL (p=0,971), respectivamente. En el grupo a término, hubo valores superiores de α-tocoferol en mujeres con niños de peso al nacer >4.000g, siendo 1.821,0±575,4µg/dL en comparación a 869,5±532,1µg/dL y 1.039,6±477,5µg/dL con bajo peso y peso adecuado, respectivamente (p>0,05). CONCLUSIONES: A pesar de presentar tendencia de aumento respecto al grupo a término, la prematuridad no influenció el α-tocoferol en el calostro. Sin embargo, lactantes que tuvieron recién nacidos a término con macrosomía presentaron niveles aumentados de α-tocoferol. Esos resultados indican que el peso al nacer puede influenciar el α-tocoferol de la leche, sugiriendo que neonatos con bajo peso pueden ingerir menos vitamina E cuando amamantados. OBJECTIVE: To describe demographic characteristics, surgical results, postoperative complications, and overall survival rates in surgically treated patients with lung metastases.METHODS: This was a retrospective analysis of 119 patients who underwent a total of 154 lung metastasis resections between 1997 and 2011.RESULTS: Among the 119 patients, 68 (57.1%) were male and 108 (90.8%) were White. The median age was 52 years (range, 15-75 years). In this sample, 63 patients (52.9%) presented with comorbidities, the most common being systemic arterial hypertension (69.8%) and diabetes (19.0%). Primary colorectal tumors (47.9%) and musculoskeletal tumors (21.8%) were the main sites of origin of the metastases. Approximately 24% of the patients underwent more than one resection of the lesions, and 71% had adjuvant treatment prior to metastasectomy. The rate of lung metastasis recurrence was 19.3%, and the median disease-free interval was 23 months. The main surgical access used was thoracotomy (78%), and the most common approach was wedge resection with segmentectomy (51%). The rate of postoperative complications was 22%, and perioperative mortality was 1.9%. The overall survival rates at 12, 36, 60, and 120 months were 96%, 77%, 56%, and 39%, respectively. A Cox analysis confirmed that complications within the first 30 postoperative days were associated with poor prognosis (hazard ratio = 1.81; 95% CI: 1.09-3.06; p = 0.02).CONCLUSIONS: Surgical treatment of lung metastases is safe and effective, with good overall survival, especially in patients with fewer metastases. OBJETIVO: Descrever características demográficas, resultados operatórios, complicações pós-operatórias e taxa de sobrevida global em pacientes com metástases pulmonares tratados cirurgicamente.MÉTODOS: Análise retrospectiva de 119 pacientes submetidos a um total de 154 cirurgias de ressecção de metástase pulmonar entre 1997 e 2011.RESULTADOS: Do total de 119 pacientes, 68 (57,1%) eram do sexo masculino, e 108 (90,8%) eram brancos. A mediana de idade foi de 52 anos (variação, 15-75 anos). Nessa amostra, 63 pacientes (52,9%) apresentaram comorbidades, sendo as mais frequentes hipertensão arterial sistêmica (69,8%) e diabetes (19,0%). Tumores primários colorretais (47,9%) e musculoesqueléticos (21,8%) foram os principais sítios de origem das metástases. Aproximadamente 24% dos pacientes foram submetidos a mais de uma ressecção das lesões, e 71% fizeram tratamento adjuvante prévio à metastasectomia. A taxa de recidiva de metástase pulmonar foi de 19,3%. A mediana do intervalo livre de doença foi de 23 meses. A principal via de acesso usada foi toracotomia (78%), e o tipo de ressecção mais frequente foi em cunha e segmentectomia (51%). O índice de complicações pós-operatórias foi de 22% e o de mortalidade perioperatória foi de 1,9%. As taxas de sobrevida global em 12, 36, 60 e 120 meses foram, respectivamente, de 96%, 77%, 56% e 39%. A análise de Cox confirmou que complicações nos primeiros 30 dias pós-operatórios associaram-se a pior prognóstico (hazard ratio = 1,81; IC95%: 1,09-3,06; p = 0,02).CONCLUSÕES: O tratamento cirúrgico das metástases pulmonares oriundas de diferentes sítios tumorais é efetivo e seguro, com boa sobrevida global, especialmente nos casos com um menor número de lesões pulmonares.

To build a fuzzy computational model to estimate the number of hospitalizations of children aged up to 10 years due to respiratory conditions based on pollutants and climatic factors in the city of São José do Rio Preto, Brazil.A computational model was constructed using the fuzzy logic. The model has 4 inputs, each with 2 membership functions generating 16 rules, and the output with 5 pertinence functions, based on the Mamdani's method, to estimate the association between the pollutants and the number of hospitalizations. Data from hospitalizations, from 2011-2013, were obtained in DATASUS - and the pollutants Particulate Matter (PM10) and Nitrogen Dioxide (NO2), wind speed and temperature were obtained by the Environmental Company of São Paulo State (Cetesb).A total of 1,161 children were hospitalized in the period and the mean of pollutants was 36 and 51 µg/m3 - PM10 and NO2, respectively. The best values of the Pearson correlation (0.34) and accuracy measured by the Receiver Operating Characteristic (ROC) curve (NO2 - 96.7% and PM10 - 90.4%) were for hospitalizations on the same day of exposure.The model was effective in predicting the number of hospitalizations of children and could be used as a tool in the hospital management of the studied region.Construir um modelo computacional fuzzy para estimar o número de internações de crianças até 10 anos por doenças respiratórias, com base nos dados de poluentes e fatores climáticos da cidade de São José do Rio Preto, Brasil.Foi construído modelo computacional utilizando a lógica fuzzy. O modelo tem 4 entradas, cada uma com 2 funções de pertinência gerando 16 regras, e a saída com 5 funções de pertinência, baseado no método de Mamdani, para estimar a associação entre os poluentes e o número de internações. Os dados de internações, de 2011-2013, foram obtidos no Departamento de Informática do Sistema de Saúde (DATASUS) e os poluentes material particulado (PM10) e dióxido de nitrogênio (NO2), a velocidade do vento e a temperatura foram obtidos pela Companhia Ambiental do Estado de São Paulo (Cetesb).Foram internadas 1.161 crianças no período analisado, e a média dos poluentes foi 36 e 51 µg/m3 - PM10 e NO2, respectivamente. Os melhores valores da correlação de Pearson (0,34) e da acurácia avaliada pela curva Receiver Operating Characteristic - ROC (NO2 - 96,7% e PM10 - 90,4%) foram para internações no mesmo dia da exposição.O modelo mostrou-se eficaz na predição do número de internações de crianças, podendo ser utilizado como ferramenta na gestão hospitalar da região estudada.

Background: Heart failure (HF) is a highly prevalent syndrome. Although the long-term prognostic factors have been identified in chronic HF, this information is scarcer with respect to patients with acute HF. despite available data in the literature on long-term prognostic factors in chronic HF, data on acute HF patients are more scarce. Objectives: To develop a predictor of unfavorable prognostic events in patients hospitalized for acute HF syndromes, and to characterize a group at higher risk regarding their clinical characteristics, treatment and outcomes. Methods: cohort study of 600 patients admitted for acute HF, defined according to the European Society of Cardiology criteria. Primary endpoint for score derivation was defined as all-cause mortality and / or rehospitalization for HF at 12 months. For score validation, the following endpoints were used: all-cause mortality and / or readmission for HF at 6, 12 and 24 months. The exclusion criteria were: high output HF; patients with acute myocardial infraction, acute myocarditis, infectious endocarditis, pulmonary infection, pulmonary artery hypertension and severe mitral stenosis. Results: 505 patients were included, and prognostic predicting factors at 12 months were identified. One or two points were assigned according to the odds ratio (OR) obtained (p < 0.05). After the total score value was determined, a 4-point cut-off was determined for each ROC curve at 12 months. Two groups were formed according to the number of points, group A < 4 points, and group B = 4 points. Group B was composed of older patients, with higher number of comorbidities and predictors of the combined endpoint at 6, 12 and 24 months, as linearly represented in the survival curves (Log rank). Conclusions: This risk score enabled the identification of a group with worse prognosis at 12 months. Resumo Fundamento: A insuficiência cardíaca (IC) é uma síndrome de elevada prevalência. Apesar de existir na literatura informação relativa aos fatores prognósticos a longo prazo na IC crônica, esta é mais escassa no que diz respeito aos pacientes com IC aguda. Objetivos: Desenvolver um score preditor de eventos prognósticos desfavoráveis em doentes admitidos com síndromes de IC aguda e caracterizar um grupo de maior risco quanto às suas características clínicas, terapêutica e resultados. Métodos: Estudo de coorte de 600 doentes internados com IC aguda, definida de acordo com os critérios da Sociedade Europeia de Cardiologia. O endpoint primário para a derivação do score foi definido como mortalidade de qualquer causa e/ou reinternação por IC aos 12 meses. Para a validação do score, foram utilizados como endpoints: mortalidade de qualquer causa e/ou reinternação por IC aos 6, 12 e 24 meses. Os critérios de exclusão foram: IC de alto débito, pacientes com infarto agudo do miocárdio, miocardite aguda, endocardite infeciosa, infeção pulmonar, hipertensão arterial pulmonar e estenose mitral grave. Resultados: Foram incluídos 505 doentes e identificados preditores prognósticos aos 12 meses. Atribuíram-se 1 ou 2 pontos (p.) de acordo com os odds ratio (OR) obtidos (p < 0,05). Após a determinação do valor de score total, foi estabelecido um cut-off de 4 pontos por curva ROC. Constituíram-se 2 grupos de acordo com a pontuação, grupo A < 4 p. versus grupo B = 4 p. O grupo B era constituído por idosos, com maior número de comorbidades e preditor de endpoint combinado aos 6, 12 e 24 meses traduzido linearmente nas curvas de sobrevida (Log rank). Conclusões: Este score de risco permitiu identificar um grupo com pior prognóstico aos 12 meses.

To analyze the association between postural balance and anthropometric indicators in elementary school students.This cross-sectional, descriptive and quantitative study included children enrolled in the first year of elementary school, of both sexes, in the age group of 6 to 7 years. Children with any physical or cognitive impairment, children who did not participate in all stages of the study evaluation, or those who failed to perform postural balance assessments were excluded. The children underwent a balance evaluation through a computerized dynamic posturograph, with sensory organization tests (TOSs) in six different sensory conditions. In order to verify the anthropometric indicators, the body weight and height measurements were evaluated for later calculation of the body mass index (BMI), which was categorized into four groups: low weight, normal, overweight and obesity.Eighty children with a mean age of 6.2±0.8 years were included, being 47 girls (58.8%). The analysis of the anthropometric indicators identified that 26.3% of students were overweight and 15% were obese. The children had averages below the reference values considered for their age range in conditions III and VI. There was a negative association between condition V and BMI and a positive association between values below normal in condition VI with overweight and obesity.There were associations between excess body weight and values below normal in some balance conditions, indicating that the anthropometric indicators interfered in the children's postural balance.Analisar a associação entre o equilíbrio postural e indicadores antropométricos em escolares do primeiro ano do ensino fundamental.Estudo transversal, descritivo e quantitativo que incluiu crianças regularmente matriculadas no primeiro ano do ensino fundamental, de ambos os sexos, na faixa etária de 6 a 7 anos. Foram excluídas as crianças com qualquer incapacidade física ou cognitiva, as que não participaram de todas as etapas de avaliação do estudo ou as que não conseguiram realizar a avaliação do equilíbrio postural. As crianças realizaram uma avaliação do equilíbrio por meio da posturografia dinâmica computadorizada (PDC), por meio dos testes de organização sensorial (TOSs) em seis diferentes condições sensoriais. Para verificar os indicadores antropométricos, foram avaliadas as medidas de peso corporal e estatura, para posterior cálculo do índice de massa corporal (IMC), sendo este categorizado em quatro grupos: baixo peso, normal, sobrepeso e obesidade.Foram incluídas 80 crianças (6,2±0,8 anos), sendo 47 meninas (58,8%). A análise dos indicadores antropométricos identificou 26,3% de escolares com sobrepeso e 15% com obesidade. As crianças apresentaram médias abaixo dos valores de referência considerados para sua faixa etária nas condições III e VI. Foi verificada associação negativa entre a condição V com o IMC e uma associação positiva entre os valores abaixo da normalidade na condição VI com o sobrepeso e a obesidade.Foram verificadas associações entre o excesso de peso corporal e valores abaixo da normalidade em algumas condições do equilíbrio, indicando que os indicadores antropométricos interferiram no equilíbrio postural das crianças.

To study the applicability of the Strength and Difficulties Questionnaire (SDQ 2,4-p) as a screening tool for mental health problems in preschoolers, in the context of Primary Health Care; to evaluate the mental health problems of the sample, comparing data from SDQ (2,4-p) and from the Child Behavior Check List (CBCL 1½-5 years).Cross-sectional observational study with a convenience sample of 31-50-month-old children, whose caretakers provided informational reports. In the first stage, professionals from the primary care health unit have applied the SDQ (2,4-p) during routine appointments. Subsequently, the CBCL (1½-5) was applied by a professional experienced in infant mental health. The SDQ and CBCL results were compared and the correlation between the scales was tested.Among 280 questionnaires available to the health professionals, 48 were filled out and the CBCL was applied to 40 of the participants. Among the problems found with the SDQ, 18 cases (37.6% out of 48) have shown abnormal score in the "Total Difficulties" and 38 (80.9% out of 48) have shown normal score in the "Impact of Difficulty". Behavioral issues were highlighted by the percentage of abnormal scores (47.9%). The correlation between SQD and CBCL was positive for all scales, except for the pro-social behavior.Clinically important mental health problems were found in preschool children. Variables of the SDQ discriminate normal and abnormal scores according to the CBCL parameters, thus functioning as a good screening tool.Estudar a aplicabilidade do Questionário de Capacidades e Dificuldades - Strength and Difficulties Questionnaire (SDQ 2,4-p) - como instrumento de rastreamento, na Atenção Básica à Saúde (ABS), de problemas de saúde mental em crianças pré-escolares; caracterizar os problemas de saúde mental da amostra, comparando dados do SDQ (2,4-p) e do Inventário de Comportamentos da Criança - Child Behavior Check List (CBCL 1½-5 anos).Estudo observacional transversal com amostra de conveniência composta por crianças de 31 a 50 meses, cujos responsáveis forneceram relato das informações. Na primeira etapa, profissionais da unidade básica de saúde (UBS) aplicaram o SDQ (2,4-p) durante consultas de rotina. Na sequência, o CBCL (1½-5) foi aplicado aos responsáveis por profissional experiente especializado em saúde mental infantil. Resultados do SDQ e do CBCL foram comparados; correlação entre as escalas foi analisada.Dos 280 questionários disponibilizados aos profissionais da UBS, 48 foram preenchidos e houve aplicação do CBCL em 40 dos participantes. Entre os problemas rastreados com o SDQ, 18 casos (37,6% de 48) apresentam escore anormal no “Total de Dificuldades” e 38 (80,9% de 48), escore normal no “Impacto da Dificuldade”. Problemas de conduta se destacaram pela porcentagem de escores anormais (47,9%). A correlação entre SDQ e CBCL foi positiva em todas as escalas, exceto no comportamento pró-social.Problemas de saúde mental clinicamente importantes foram encontrados em crianças pré-escolares. Variáveis do SDQ discriminam escores normais e anormais, conforme parâmetros do CBCL, funcionando como um bom instrumento de triagem.

To determine if the treatment of stunted children offered at a specialized center influences their cognitive performance.Two groups of children from vulnerable families were selected, one consisting of stunted children being treated at the Nutrition Education and Recovery Center (CREN), and the other group of eutrophic children from a local, public day care center. At CREN, children are treated in a day-hospital system (9 hours/day, 5 days/week), receiving medical, nutritional and psycho-pedagogical support. All children were submitted to the Denver-II Development Screening Test and had their development and the height-for-age index assessed at 3 moments: at the beginning of the follow-up, and after 6 and 12 months. The socioeconomic status, according to the Brazilian Economic Classification Criteria, was assessed at the beginning of the follow-up. Data were treated by prevalence ratios for cross-sectional baseline analysis, using the Poisson regression, and by pooled prevalence ratios for longitudinal analysis, using a generalized equation estimation model, both adjusted by age, sex and economic status.Seventy-four children were included, 37 for each group. There were no differences in age, sex and socioeconomic status between groups. In the longitudinal analysis, the CREN group showed better performance in the personal-social domain (pooled prevalence ratio: 0.89; 95% confidence interval - 95%IC 0.82-0.95), with no differences in the other domains.The treatment offered at CREN satisfactorily improved the social skills of the treated children, without changing other domains.Determinar se o tratamento de crianças com baixa estatura, ofertado em um centro especializado, influencia seu desempenho cognitivo.Foram analisados dois grupos de crianças advindas de famílias vulneráveis, um com crianças com baixa estatura em tratamento no Centro de Recuperação e Educação Nutricional (CREN) e outro de crianças eutróficas de uma creche municipal localizada na mesma região do CREN. No CREN, as crianças são tratadas em semi-internato (9 horas/dia, 5 dias/semana), recebendo suporte médico, nutricional e psicopedagógico. Todas foram submetidas ao Teste de Triagem do Desenvolvimento de Denver-II e avaliadas quanto ao índice de estatura-para-idade em 3 momentos distintos: no início do acompanhamento e após 6 e 12 meses. A classificação socioeconômica das crianças, de acordo com os Critérios de Classificação Econômica Brasil, foi feita no início do acompanhamento. Calcularam-se razões de prevalência para a análise transversal da linha de base, por meio de uma regressão de Poisson, e razões de prevalência agrupadas para a análise longitudinal, por meio de um modelo de estimativas de equações generalizadas, ambas ajustadas por idade, sexo e classe socioeconômica.Ao todo, 74 crianças foram analisadas, 37 em cada grupo. Não houve diferenças de idade, sexo e classe socioeconômica entre os grupos. Na análise longitudinal, o grupo CREN apresentou melhor desempenho no domínio pessoal-social (razão de prevalência agrupada: 0,89; intervalo de confiança de 95% - IC95%: 0,82-0,95), sem diferenças significativas para os demais domínios.O tratamento ofertado pelo CREN melhorou satisfatoriamente as habilidades sociais das crianças tratadas, sem alterar os demais domínios.

To evaluate the association between nutritional status, lung function and morbidity in a 36-month cohort in children and adolescents with cystic fibrosis.Prospective cohort of children and adolescents with cystic fibrosis aged 1-15 years. At the baseline, the nutritional status was determined by weight-for-height and body mass index-for-age for children2 years and ≥2 years, respectively, and classified as: nutritional failure, nutritional risk and acceptable; and by the 50th percentile, according to the World Health Organization (WHO) growth charts. Lung function was assessed by forced expiratory volume in one second (FEV1). Morbidity was determined by the presence of infection and hospitalization by pulmonary exacerbation. Risk ratio and 95% confidence interval (95%CI) were calculated, being significant when p0.05.We evaluated 38 children and adolescents (median age 3.8 years). Patients that were classified as having nutritional failure at baseline had a RR of 5.00 (95%CI 1.49; 16.76) to present impaired lung function after 36 months. Those classified bellow the 50th percentile had a RR of 4.61 (95%CI 0.89; 23.81) to present the same outcome. Nutritional status was not a risk factor for morbidity in this cohort.Nutritional deficit was associated with impaired lung function, but not with morbidity in children and adolescents with cystic fibrosis.Avaliar a associação entre estado nutricional, função pulmonar e morbidade em coorte de 36 meses de crianças e adolescentes com fibrose cística (FC).Coorte prospectiva de 36 meses, com crianças e adolescentes com FC e idade entre 1 e 15 anos. No tempo inicial, o estado nutricional foi determinado a partir dos indicadores: peso-para-estatura e índice de massa corporal-para-idade, para crianças2 anos e ≥2 anos, respectivamente, e classificado em: falência nutricional, risco nutricional e estado nutricional aceitável; também foi determinado por meio do percentil 50°, de acordo com a curva de crescimento da World Health Organization (WHO). A função pulmonar foi avaliada pelo volume expiratório forçado no primeiro segundo (VEF1). A morbidade foi determinada pela presença de infecção e hospitalização por exacerbação pulmonar. Foi calculado risco relativo (RR) e intervalo de confiança (IC95%), considerando significante p0,05.Foram avaliadas 38 crianças e adolescentes (mediana de idade 3,8 anos). Os pacientes classificados em falência nutricional no início do estudo mostraram um RR de 5,00 (IC95% 1,49; 16,76) para o comprometimento da função pulmonar após 36 meses. Aqueles classificados abaixo do percentil 50° apresentaram RR de 4,61 (IC95% 0,89; 23,81) para o desfecho. O estado nutricional não foi fator de risco para morbidade.O déficit nutricional esteve associado ao comprometimento da função pulmonar, mas não com a morbidade em crianças e adolescentes com FC.

To identify the prevalence of feeding difficulties in preschoolers, its association with epidemiological factors and previous eating habits, and repercussion on nutritional status.Cross-sectional study with a questionnaire given to the mothers of 301 children aged 2-6 years enrolled in public and private kindergartens in Natal, Northeast Brazil, conducted in 2014-2015. Feeding difficulty was assessed according to Kerzner's criteria, resulting in the profiles "highly selective intake", "active child with small appetite", "fear of feeding", and "child with psychological disorder or neglected". Association with the following independent variables was analyzed by logistic regression: breastfeeding time, age of cows' milk and complementary feeding introduction, age range, family income, type of school, mothers' profile (responsive or nonresponsive), and body mass index (BMI).Feeding difficulty was found in 37.2% of cases, with predominance of "highly selective intake" (25.4%). It was not associated with infancy feeding practices, family income or type of school. There were no differences between the BMI Z score means for the groups with and without feeding difficulty (1.0±1.5 SD and 1.1±1.4 SD, respectively). The five-to-six age range had more occurrences (OR 1.8; 95%CI 1.1-2.9). Children of responsive mothers were less likely to have feeding difficulties (OR 0.4; 95%CI 0.2-0.8).Feeding difficulties were very frequent. Nutritional status was not impacted by it, and infancy eating habits were not associated with it. Responsive mothers' profile is a protective factor against eating difficulties and reinforces the importance of behavioral factors and mother-child interaction.Identificar a prevalência de dificuldade alimentar (DA) em pré-escolares, sua associação com fatores epidemiológicos e práticas alimentares pregressas, bem como sua repercussão sobre o estado nutricional.Estudo transversal com aplicação de questionário às mães de 301 crianças de dois a seis anos de creches públicas e privadas em Natal, Rio Grande do Norte, em 2014 e 2015. Identificou-se DA segundo critérios de Kerzner, incluindo os perfis de “ingestão altamente seletiva”, “criança agitada com baixo apetite”, “fobia alimentar” e “criança com distúrbio psicológico ou negligenciada”. As variáveis de associação analisadas por regressão logística foram: tempo de aleitamento materno, idade de introdução de leite de vaca e da alimentação complementar, faixa etária, renda familiar, tipo de escola, perfil das mães (responsivas ou não responsivas) e índice de massa corpórea (IMC).DA foi encontrada em 37,2% dos casos analisados, com predomínio de “ingestão altamente seletiva” (25,4%). Não houve associação entre DA e práticas alimentares na fase de lactente, renda familiar e tipo de escola. Não houve diferença entre as médias de escore Z IMC para os grupos com e sem DA (1,0±1,5DP e 1,1±1,4DP, respectivamente). A faixa etária de cinco a seis anos apresentou maior ocorrência de DA (OR 1,8; IC95% 1,1-2,9) e filhos de mães com perfil responsivo tiveram menores chances de apresentar DAs (OR 0,4; IC95% 0,2-0,8).DA foi de alta prevalência. Não houve repercussão sobre o estado nutricional nem associação às práticas alimentares pregressas. O perfil responsivo das mães é fator protetor para as DAs e reforça a importância da natureza comportamental e da interação mãe-filho.

To identify a low-cost abdominal adiposity index that has a higher accuracy in predicting excess weight in children aged four to seven years old.A cross-sectional study with a sample of 257 children aged 4 to 7 years old. Indicators of abdominal adiposity assessed were: waist circumference (WC), waist-to-height ratio (WHR) and central fat percentage (measured by dual energy X-ray absorptiometry - DEXA). Overweight children were classified using body mass index by age (BMI/age). In the analysis, the prevalence ratio (PR) using Poisson regression with a robust variance was estimated, and a receiver operating characteristic (ROC) curve was built, with a statistical significance of p0.05.The prevalence of overweight children was 24.9% and a higher median of all abdominal adiposity indicators was observed in the overweight group. Children with increased values of WC (PR=4.1; 95%CI 2.86-5.86), WHR (PR=5.76; 95%CI 4.14-8.02) and a central fat percentage (PR=2.48; 95%CI 1.65-3.73) had a higher prevalence of being overweight. Using the ROC curve analysis, the WHR index showed a higher area under the curve, when compared to the WC and to the central fat percentage estimated by DEXA for predicting the classification of being overweight.Given the results, WHR is suggested for the screening of overweight children.Identificar um indicador de adiposidade abdominal de baixo custo e com maior acurácia para predizer o excesso de peso em crianças de quatro a sete anos idade.Estudo transversal com amostra de 257 crianças de 4 a 7 anos. Os indicadores de adiposidade abdominal avaliados foram: perímetro da cintura (PC), relação cintura-estatura (RCE) e percentual de gordura central (avaliado pela técnica dual energy X-ray absorptiometry - DEXA). O excesso de peso foi classificado pelo índice de massa corporal por idade (IMC/I). Nas análises, estimou-se a razão de prevalência (RP) pela regressão de Poisson com variância robusta e utilizou-se a curva (receiver operating characteristics ­- ROC), considerando como significância estatística p0,05.A prevalência de excesso de peso foi de 24,9%, e observou-se maior mediana dos indicadores de adiposidade abdominal no grupo de crianças com excesso de peso. As crianças com valores aumentados de PC (RP=4,1; IC95% 2,86-5,86), RCE (RP=5,76; IC95% 4,14-8,02) e percentual de gordura central (RP=2,48; IC95% 1,65-3,73) apresentaram maior prevalência de excesso de peso. Verificou-se, na análise de curva ROC, que o índice RCE apresentou maior área sob a curva, comparado ao PC e ao percentual de gordura central estimada pelo DEXA, na predição do excesso de peso.Diante dos resultados, sugere-se a utilização da RCE para triagem de crianças com excesso de peso.

To investigate the influence of the first postpartum visit, family income, pacifier habit, number of siblings and birth weight on the maintenance of exclusive breastfeeding in infants aged one week up to six months.In this cross-sectional study, data were collected through a survey, which included social and demographic characteristics of the families and the breastfeeding practice in children aged one week to six months, who received care at family health units in the municipality of Vitória de Santo Antão, Pernambuco, Northeast Brazil, between December 2014 and February 2015. Prevalence ratio was used to indicate how many times the outcome prevalence was increased by the influence of the studied variables, as well as a binary logistic regression model for the analysis and reliability of the results.The prevalence of exclusive breastfeeding was 41.7%. Family income, pacifier habit, number of siblings and birth weight did not show statistical association with the maintenance of exclusive breastfeeding. However, the absence of postpartum home visits adversely influenced the outcome (p=0.009). The children who received home visits had a higher chance of being exclusively breastfed for six months or more (PR 2.28, 95%CI 1.17-4.42). In the logistic regression, only the visit showed significance to estimate the probability of exclusive breastfeeding.The absence of postpartum home visits negatively influenced the duration of exclusive breastfeeding. This finding fills a gap in the knowledge of determinants of exclusive breastfeeding and may guide the planning of local strategies and actions to promote, protect and support exclusive breastfeeding.Averiguar a influência da primeira visita puerperal, da renda familiar, do hábito de chupeta, do número de irmãos e do peso ao nascer na manutenção do aleitamento materno exclusivo (AME) em lactentes com uma semana de vida até seis meses de idade no município de Vitória de Santo Antão, Pernambuco.Neste estudo transversal, coletaram-se dados por inquérito que abrangiam características sociais e demográficas das famílias e prática de amamentação em crianças com uma semana até seis meses de vida, que compareceram às unidades de saúde da família de Vitória de Santo Antão nos dias de puericultura, entre dezembro de 2014 e fevereiro de 2015. Para indicar quantas vezes a prevalência do desfecho foi aumentada por influência dos fatores analisados, utilizou-se a Razão de Prevalência, bem como um modelo de regressão logística binária para análise e confiabilidade dos resultados.A prevalência de amamentação exclusiva foi de 41,7%. A renda familiar, o hábito de chupeta, o número de irmãos e o peso ao nascer não demonstraram significância estatística sobre a manutenção do AME. Em contrapartida, a ausência da visita puerperal (p=0,009) influenciou negativamente a sua permanência. As crianças que receberam visita mostraram maior possibilidade de estarem em AME (RP 2,28, IC95% 1,17-4,42). Na regressão logística apenas a visita apontou significância para estimar a probabilidade de ocorrer AME.A ausência da visita puerperal influenciou negativamente a manutenção do AME. Esse achado preenche a lacuna referente ao conhecimento dos fatores determinantes sobre essa prática e norteia o planejamento de ações e estratégias locais para promoção, proteção e apoio à amamentação exclusiva.

to verify the association among the knowledge attitude and practice of women in relation to the smear test and the age range.a cross-sectional research was undertaken, associated with the knowledge, attitude and practice survey at a Primary Health Care service. The sample consisted of 775 women, distributed in three age ranges: adolescent, young and elderly.although high rates of inappropriate knowledge were found in all age ranges, it was significantly higher among the adolescents (p=0.000). A similar trend was found in the attitude component, with percentages of inappropriateness in adolescence that drop as age advances (p=0.000). develop the content and face validation of an educative manual for patients with head and neck cancer submitted to radiation therapy.descriptive methodological research. The Theory of Psychometrics was used for the validation process, developed by 15 experts in the theme area of the educative manual and by two language and publicity professionals. A minimum agreement level of 80% was considered to guarantee the validity of the material.the items addressed in the assessment tool of the educative manual were divided in three blocks: objectives, structure and format, and relevance. Only one item, related to the sociocultural level of the target public, obtained an agreement rate80%, and was reformulated based on the participants' suggestions. All other items were considered appropriate and/or complete appropriate in the three blocks proposed: objectives - 92.38%, structure and form - 89.74%, and relevance - 94.44%.the face and content validation of the educative manual proposed were attended to. This can contribute to the understanding of the therapeutic process the head and neck cancer patient is submitted to during the radiation therapy, besides supporting clinical practice through the nursing consultation.validar o conteúdo e a aparência de manual educativo direcionado aos pacientes com câncer de cabeça e pescoço, submetidos à radioterapia.pesquisa metodológica, de caráter descritivo. Utilizou-se a Teoria da Psicometria para o processo de validação, o qual foi realizado por 15 peritos na área temática do manual educativo e por dois profissionais de letras e publicidade. estimate the prevalence of anxiety in laryngectomy patients in the pre and postoperative periods and its relation with the self-care level.observational research of 40 patients with stage IV laryngeal cancer. Three observations took place: in the preoperative phase, at seven and at 14 days after the surgery; between June 2010 and December 2012. Two self-care levels were defined: self-sufficient and needing help for activities of daily living and treatment-related activities. To assess the anxiety levels, Zigmond's hospital anxiety scale (1983) was used.in the preoperative and postoperative phases, the patients presented high levels of anxiety. Concerning self-care, on average, self-sufficient patients presented lower levels of anxiety than patients who needed help to accomplish activities of daily living and activities deriving from the surgery, without significant differences.anxiety is present at all times in laryngectomy patients and the reduction of the self-care deficit seems to decrease it, without putting a permanent end to it.estimar a prevalência de ansiedade do paciente laringectomizado no pré-operatório e pós-operatório e sua relação com o nível de autocuidado.pesquisa observacional de 40 pacientes com câncer da laringe estágio IV. En relación a los autocuidados, los pacientes autosuficientes presentaron en media niveles más bajos de ansiedad que los pacientes que necesitaban ayuda para realizar las actividades de la vida diaria y las derivadas de la cirugía, sin llegar a ser significativas estas diferencias.la ansiedad está presente en todo momento en el paciente con laringectomia y la disminución del déficit de autocuidados parece disminuirla sin terminar definitivamente con ella.

Objective: To estimate the entrance surface air kerma (Ka,e) and air kerma in the region of radiosensitive organs in radiographs of pediatric paranasal sinuses. Materials and Methods: Patient data and irradiation parameters were collected in examinations of the paranasal sinuses in children from 0 to 15 years of age at two children's hospitals in the city of Recife, PE, Brazil. We estimated the Ka,e using the X-ray tube outputs and selected parameters. To estimate the air kerma values in the regions of the eyes and thyroid, we used thermoluminescent dosimeters. Results: The Ka,e values ranged from 0.065 to 1.446 mGy in cavum radiographs, from 0.104 to 7.298 mGy in Caldwell views, and from 0.113 to 7.824 mGy in Waters views. Air kerma values in the region of the eyes ranged from 0.001 to 0.968 mGy in cavum radiographs and from 0.011 to 0.422 mGy in Caldwell and Waters views . In the thyroid region, air kerma values ranged from 0.005 to 0.932 mGy in cavum radiographs and from 0.002 to 0.972 mGy in Caldwell and Waters views. Conclusion: The radiation levels used at the institutions under study were higher than those recommended in international protocols. We recommend that interventions be initiated in order to reduce patient exposure to radiation and therefore the risks associated with radiological examination of the paranasal sinuses. Resumo Objetivo: Estimar o kerma no ar na superfície de entrada (Ka,e) e o kerma no ar na região de órgãos radiossensíveis em radiografias pediátricas dos seios paranasais. Materiais e Métodos: Foram coletados os dados dos pacientes e parâmetros de irradiação em exames de seios paranasais de crianças com 0 a 15 anos, em dois hospitais infantis na cidade do Recife, PE. O Ka,e foi estimado utilizando os rendimentos dos tubos de raios X e os parâmetros selecionados. Os valores de kerma no ar nas regiões dos olhos e tireoide foram estimados utilizando dosímetros termoluminescentes. Resultados: Os valores de Ka,e variaram de 0,065 a 1,446 mGy para radiografias cavum, 0,104 a 7,298 mGy para Caldwell e 0,113 a 7,824 mGy para Waters. Os valores de kerma no ar na região dos olhos variaram de 0,001 a 0,968 mGy para cavum e 0,011 a 0,422 mGy para Caldwell e Waters. Na região tireóidea, os valores de kerma no ar variaram de 0,005 a 0,932 mGy para cavum e 0,002 a 0,972 mGy para Caldwell e Waters. Conclusão: Os parâmetros de irradiação utilizados pelas instituições foram mais elevados do que os recomendados em protocolos britânicos. Ações de otimização são recomendadas de modo a contribuir para a redução da radiação nos pacientes e, portanto, os riscos.

Objective: to analyze whether the tutor's use of reflexive strategies encourages the students to reflect. The goal is to discover what type of strategies can help to achieve this and how tutors and students behave in the practical context. Method: a qualitative and ethnographic focus was adopted. Twenty-seven students and 15 tutors from three health centers participated. The latter had received specific training on reflexive clinical tutoring. The analysis was developed through constant comparisons of the categories. Results: the results demonstrate that the tutors' use of reflexive strategies such as didactic questioning, didactic empathy and pedagogical silence contributes to encourage the students' reflection and significant learning. Conclusions: reflexive practice is key to tutors' training and students' learning. Objetivo: analizar si el uso de estrategias reflexivas por parte de la tutora de prácticas clínicas fomenta la reflexión en los estudiantes. Se trata de conocer qué tipo de estrategias pueden ayudar a hacerlo y cómo se comportan tutoras y estudiantes en el contexto práctico. Método: se ha utilizado un enfoque cualitativo de corte etnográfico en el que participaron 27 estudiantes y 15 tutores de tres centros sanitarios que habían recibido formación específica sobre tutoría clínica reflexiva. El análisis se ha realizado mediante las comparaciones constantes de las categorías. Resultados: los resultados demuestran que el uso de estrategias reflexivas como la interrogación didáctica, la empatía didáctica y el silencio pedagógico por parte de las tutoras contribuye a fomentar la reflexión del estudiante y su aprendizaje significativo. Conclusiones: la práctica reflexiva es la clave para la formación de los tutores y para el aprendizaje de los estudiantes. Objective: to construct an instrument for the assessment of graduates of undergraduate nursing courses and to validate this instrument through the consensus of specialists. Method: methodological study. In order to elaborate the instrument, documental analysis and a literature review were undertaken. Validation took place through use of the Delphi Conference, between September 2012 and September 2013, in which 36 specialists from Brazilian Nursing participated. In order to analyze reliability, the Cronbach alpha coefficient, the item/total correlation, and the Pearson correlation coefficient were calculated. Results: the instrument was constructed with the participation of specialist nurses representing all regions of Brazil, with experience in lecturing and research. The first Delphi round led to changes in the first instrument, which was restructured and submitted to another round, with a response rate of 94.44%. In the second round, the instrument was validated with a Cronbach alpha of 0.75. Conclusion: the final instrument possessed three dimensions related to the characterization of the graduate, insertion in the job market, and evaluation of the professional training process. This instrument may be used across the territory of Brazil as it is based on the curricular guidelines and contributes to the process of regulation of the quality of the undergraduate courses in Nursing. Resumo Objetivo: construir um instrumento para a avaliação de egressos de cursos de graduação em enfermagem e validar esse instrumento pelo consenso de especialistas. Método: estudo metodológico. Para a elaboração do instrumento, realizou-se análise documental e revisão de literatura. A validação ocorreu por Conferência Delphi, entre setembro de 2012 e setembro de 2013, da qual participaram 36 especialistas da Enfermagem brasileira. Para a análise de confiabilidade, calculou-se o coeficiente alfa de Cronbach, a correlação item/total e o coeficiente de correlação de Pearson. Resultados: o instrumento foi construído com a participação de especialistas enfermeiros, representantes de todas as regiões do país, com experiência em docência e pesquisa. A primeira rodada Delphi gerou alterações no primeiro instrumento, que foi reestruturado e submetido a nova rodada, com taxa de resposta de 94,44%. Conclusão: o instrumento final possui três dimensões relacionadas à caracterização do egresso, inserção no mercado de trabalho e avaliação do processo de formação profissional. Esse instrumento pode ser utilizado em território nacional por basear-se nas diretrizes curriculares e contribuir com o processo de regulação da qualidade dos cursos de graduação em enfermagem. Resultados: el instrumento fue construido con la participación de especialistas enfermeros, representantes de todas las regiones del país, con experiencia en docencia e investigación. La primera rodada Delphi generó alteraciones en el primer instrumento, el que fue reestructurado y sometido a una nueva rodada, con tasa de respuesta de 94,44%. En la segunda rodada, se validó el instrumento con alfa de Cronbach de 0,75. Conclusión: el instrumento final posee tres dimensiones: caracterización del egresado, introducción en el mercado de trabajo y evaluación del proceso de formación profesional. El instrumento podrá ser utilizado en el territorio nacional por basarse en las directrices curriculares y contribuir con el proceso de regulación de la calidad de los cursos de graduación en enfermería.

Objective: to validate the content of the new nursing diagnosis, termed risk for pressure ulcer. Method: the content validation with a sample made up of 24 nurses who were specialists in skin care from six different hospitals in the South and Southeast of Brazil. Data collection took place electronically, through an instrument constructed using the SurveyMonkey program, containing a title, definition, and 19 risk factors for the nursing diagnosis. The data were analyzed using Fehring's method and descriptive statistics. The project was approved by a Research Ethics Committee. Results: title, definition and seven risk factors were validated as "very important": physical immobilization, pressure, surface friction, shearing forces, skin moisture, alteration in sensation and malnutrition. Among the other risk factors, 11 were validated as "important": dehydration, obesity, anemia, decrease in serum albumin level, prematurity, aging, smoking, edema, impaired circulation, and decrease in oxygenation and in tissue perfusion. The risk factor of hyperthermia was discarded. Conclusion: the content validation of these components of the nursing diagnosis corroborated the importance of the same, being able to facilitate the nurse's clinical reasoning and guiding clinical practice in the preventive care for pressure ulcers. Objetivo: validar o conteúdo do novo diagnóstico de enfermagem, denominado risco de úlcera por pressão. Método: trata-se de uma validação de conteúdo, com amostra composta por 24 enfermeiros especialistas no cuidado à pele, procedentes de seis diferentes hospitais do Sul e Sudeste brasileiro. A coleta de dados ocorreu por meio eletrônico, em instrumento construído pelo programa SurveyMonkey, que continha título, definição e 19 fatores de risco para o diagnóstico de enfermagem. Os dados foram analisados pelo método de Fehring e pela estatística descritiva. O projeto foi aprovado em Comitê de Ética em Pesquisa. Resultados: título, definição e sete fatores de risco foram validados como "muito importante", sendo: imobilidade, pressão, fricção, cisalhamento, umidade, sensações prejudicadas e desnutrição. Dentre os demais fatores de risco, onze foram validados como "importante": desidratação, obesidade, anemia, baixo nível de albumina, prematuridade, envelhecimento, tabagismo, edema, circulação prejudicada, diminuição da oxigenação e da perfusão tissular. O fator de risco hipertermia foi descartado. Conclusão: a validação de conteúdo desses componentes do diagnóstico de enfermagem corroborou a importância dos mesmos, podendo facilitar o raciocínio clínico do enfermeiro, norteando a prática clínica no cuidado preventivo à úlcera por pressão. Objetivo: validar el contenido del nuevo diagnóstico de enfermería denominado riesgo de úlcera por presión. Método: se trata de una validación de contenido, con muestra compuesta por 24 enfermeros especialistas en el cuidado de la piel, procedentes de seis diferentes hospitales del Sur y Sureste brasileño. La recolección de datos se realizó por medio electrónico, con un instrumento construido por el programa SurveyMonkey, que contenía: título, definición y 19 factores de riesgo para el diagnóstico de enfermería. Los datos fueron analizados por el método de Fehring y con la estadística descriptiva. El proyecto fue aprobado en el Comité de Ética en Investigación. Resultados: título, definición y siete factores de riesgo fueron validados como "muy importante"; los factores fueron: inmovilidad, presión, fricción, cizallamiento, humedad, sensaciones perjudicadas y desnutrición. Entre los demás factores de riesgo, once fueron validados como "importante": deshidratación, obesidad, anemia, bajo nivel de albumina sérica, prematuridad, envejecimiento, tabaquismo, edema, circulación perjudicada, disminución de la oxigenación y perfusión tisular. El factor de riesgo hipertermia fue descartado. Conclusión: la validación de contenido, de esos componentes del diagnóstico de enfermería, confirmó la importancia de los mismos, lo que puede facilitar el raciocinio clínico del enfermero, orientando la práctica clínica en el cuidado preventivo de la úlcera por presión.

OBJECTIVE: To provide insights for nutrition and public health practitioners on how to engage with other sectors to achieve public health goals. Specifically, this study provides lessons from the example of integrating family farming and a nutrition into a legal framework in Brazil on how to successfully shift other sectors toward nutrition goals.\ud \ud METHODS: The study analyzed policy processes that led to a Brazilian law linking family farming with the National School Feeding Program. Main actors involved with the development of the law were interviewed and their narratives were analyzed using a well-established theoretical framework.\ud \ud RESULTS: The study provides five key lessons for promoting intersectorality. First, nutrition and health practitioners can afford to embrace bold ideas when working with other sectors. Second, they should engage with more powerful sectors (or subsectors) and position nutrition goals as providing solutions that meet the interests of these sector. Third is the need to focus on a common goal - which may not be explicitly nutrition-related - as the focus of the intersectoral action. Fourth, philosophical, political, and governance spaces are needed to bring together different sectors. Fifth, evidence on the success of the intersectoral approach increases the acceptance of the process.\ud \ud CONCLUSIONS: This study on policy processes shows how a convergence of factors enabled a link between family farming and school feeding in Brazil. It highlights that there are strategies to engage other sectors toward nutrition goals which provides benefits for all sectors involved.

OBJECTIVE : To evaluate the validity and reliability of the Questionnaire for Compliance with Standard Precaution for nurses. METHODS : This methodological study was conducted with 121 nurses from health care facilities in Sao Paulo's countryside, who were represented by two high-complexity and by three average-complexity health care facilities. Internal consistency was calculated using Cronbach's alpha and stability was calculated by the intraclass correlation coefficient, through test-retest. Convergent, discriminant, and known-groups construct validity techniques were conducted. RESULTS : The questionnaire was found to be reliable (Cronbach's alpha: 0.80; intraclass correlation coefficient: (0.97) In regards to the convergent and discriminant construct validity, strong correlation was found between compliance to standard precautions, the perception of a safe environment, and the smaller perception of obstacles to follow such precautions (r = 0.614 and r = 0.537, respectively). The nurses who were trained on the standard precautions and worked on the health care facilities of higher complexity were shown to comply more (p = 0.028 and p = 0.006, respectively). CONCLUSIONS : The Brazilian version of the Questionnaire for Compliance with Standard Precaution was shown to be valid and reliable. Further investigation must be conducted with nurse samples that are more representative of the Brazilian reality. The use of the questionnaire may support the creation of educational measures considering the possible gaps that can be identified, focusing on the workers' health and on the patients' safety.

Objective: To evaluate the clinical evolution and the association between nutritional status and severity of asthma in children and adolescents enrolled in Primary Health Care. Methods: A retrospective cohort study of 219 asthmatic patients (3-17 years old) enrolled in Primary Care Services (PCSs) in Embu das Artes (SP), from 2007 to 2011. Secondary data: gender, age, diagnosis of asthma severity, other atopic diseases, family history of atopy, and body mass index. To evaluate the clinical outcome of asthma, data were collected on number of asthma exacerbations, number of emergency room consultations and doses of inhaled corticosteroids at follow-up visits in the 6th and 12th months. The statistical analysis included chi-square and Kappa agreement index, with 5% set as the significance level. Results: 50.5% of patients started wheezing before the age of 2 years, 99.5% had allergic rhinitis and 65.2% had a positive family history of atopy. Regarding severity, intermittent asthma was more frequent (51.6%) and, in relation to nutritional status, 65.8% of patients had normal weight. There was no association between nutritional status and asthma severity (p=0.409). After 1 year of follow-up, 25.2% of patients showed reduction in exacerbations and emergency room consultations, and 16.2% reduced the amount of inhaled corticosteroids. Conclusions: The monitoring of asthmatic patients in Primary Care Services showed improvement in clinical outcome, with a decreased number of exacerbations, emergency room consultations and doses of inhaled corticosteroids. No association between nutritional status and asthma severity was observed in this study.

Objective: To investigate the prevalence of oral language, orofacial motor skill and auditory processing disorders in children aged 4-10 years and verify their association with age and gender. Methods: Cross-sectional study with stratified, random sample consisting of 539 students. The evaluation consisted of three protocols: orofacial motor skill protocol, adapted from the Myofunctional Evaluation Guidelines; the Child Language Test ABFW - Phonology; and a simplified auditory processing evaluation. Descriptive and associative statistical analyses were performed using Epi Info software, release 6.04. Chi-square test was applied to compare proportion of events and analysis of variance was used to compare mean values. Significance was set at p≤0.05. Results: Of the studied subjects, 50.1% had at least one of the assessed disorders; of those, 33.6% had oral language disorder, 17.1% had orofacial motor skill impairment, and 27.3% had auditory processing disorder. There were significant associations between auditory processing skills’ impairment, oral language impairment and age, suggesting a decrease in the number of disorders with increasing age. Similarly, the variable "one or more speech, language and hearing disorders" was also associated with age. Conclusions: The prevalence of speech, language and hearing disorders in children was high, indicating the need for research and public health efforts to cope with this problem. Objetivo: Investigar a prevalência de alterações de linguagem oral, motricidade orofacial e processamento auditivo em crianças de 4-10 anos e verificar a sua associação com a idade e o gênero. Métodos: Estudo transversal com amostra aleatória e estratificada, composta por 539 crianças. Foram usados para avaliação protocolo de motricidade orofacial, adaptado do Roteiro para Avaliação Miofuncional; prova de Fonologia do Teste de Linguagem Infantil ABFW; e avaliação simplificada do processamento auditivo. Foram feitas análises estatísticas descritivas e de associação com o software Epi Info, versão 6.04. Para comparar as proporções foi empregado o qui-quadrado e para comparar médias foi empregada a análise de variância. Foi considerado significativo p≤0,05. Resultados: Das crianças avaliadas, 50,1% apresentaram pelo menos uma das alterações estudadas, 33,6% mostraram alteração de linguagem oral, 17,1% de motricidade orofacial e 27,3% do processamento auditivo. Observou-se associação significativa entre alterações fonoaudiológicas de processamento auditivo, linguagem oral e a faixa etária, o que sugere diminuição do número de crianças com alterações fonoaudiológicas com o aumento da idade. A variável "uma ou mais alterações fonoaudiológicas" também se associou à faixa etária, de maneira similar à acima descrita. Conclusões: A prevalência de alterações fonoaudiológicas na população estudada foi considerada alta, o que evidencia a necessidade de pesquisas e ações em saúde para o enfrentamento do problema.

OBJECTIVE To assess the impact of implementing long-stay beds for patients of low complexity and high dependency in small hospitals on the performance of an emergency referral tertiary hospital. METHODS For this longitudinal study, we identified hospitals in three municipalities of a regional department of health covered by tertiary care that supplied 10 long-stay beds each. Patients were transferred to hospitals in those municipalities based on a specific protocol. The outcome of transferred patients was obtained by daily monitoring. Confounding factors were adjusted by Cox logistic and semiparametric regression. RESULTS Between September 1, 2013 and September 30, 2014, 97 patients were transferred, 72.1% male, with a mean age of 60.5 years (SD = 1.9), for which 108 transfers were performed. Of these patients, 41.7% died, 33.3% were discharged, 15.7% returned to tertiary care, and only 9.3% tertiary remained hospitalized until the end of the analysis period. We estimated the Charlson comorbidity index – 0 (n = 28 [25.9%]), 1 (n = 31 [56.5%]) and ≥ 2 (n = 19 [17.5%]) – the only variable that increased the chance of death or return to the tertiary hospital (Odds Ratio = 2.4; 95%CI 1.3;4.4). The length of stay in long-stay beds was 4,253 patient days, which would represent 607 patients at the tertiary hospital, considering the average hospital stay of seven days. The tertiary hospital increased the number of patients treated in 50.0% for Intensive Care, 66.0% for Neurology and 9.3% in total. Patients stayed in long-stay beds mainly in the first 30 (50.0%) and 60 (75.0%) days. CONCLUSIONS Implementing long-stay beds increased the number of patients treated in tertiary care, both in general and in system bottleneck areas such as Neurology and Intensive Care. The Charlson index of comorbidity is associated with the chance of patient death or return to tertiary care, even when adjusted for possible confounding factors. RESUMO OBJETIVO Avaliar o impacto da implantação de leitos de longa permanência para pacientes de baixa complexidade e alta dependência em hospitais de pequeno porte sobre o desempenho de hospital terciário de referência em emergência. MÉTODOS Para este estudo longitudinal, foram identificados hospitais em três municípios no departamento regional de saúde coberto pela instância terciária e que forneciam 10 leitos de longa permanência cada. Os pacientes foram transferidos para os hospitais desses municípios com base em protocolo específico. Obteve-se o desfecho dos pacientes transferidos por acompanhamento diário. Fatores de confusão foram ajustados por regressão logística e semiparamétrica de Cox. RESULTADOS Entre 1 de setembro de 2013 e 30 de setembro de 2014, foram transferidos 97 pacientes, sendo 72,1% homens, com idade média de 60,5 anos (DP = 1,9), para os quais foram realizadas 108 transferências. Desses pacientes, 41,7% evoluíram ao óbito, 33,3% receberam alta, 15,7% retornaram à instância terciária, e apenas 9,3% permaneceram internados até o final do período analisado. Foi calculado o índice de comorbidade de Charlson – 0 (n = 28 [25,9%]), 1 (n = 31 [56,5%]) e ≥ 2 (n = 19 [17,5%]) – a única variável que aumentou a chance de óbito ou retorno ao hospital terciário (Razão de Chances = 2,4; IC95% 1,3;4,4). O hospital terciário aumentou o número de vagas em 50,0% para terapia intensiva, 66,0% para neurologia e 9,3% para as vagas totais. A permanência dos pacientes nos leitos de longa permanência limitou-se em grande parte aos primeiros 30 (50,0%) e 60 (75,0%) dias. CONCLUSÕES A implantação de leitos de longa permanência teve impacto no aumento de vagas novas oferecidas pela instância terciária tanto gerais como para áreas de estrangulamento do sistema, como a Neurologia e Terapia Intensiva. O índice de comorbidade de Charlson está associado à chance de o paciente evoluir ao óbito ou retornar para a instância terciária, mesmo quando ajustado por possíveis fatores de confusão.

OBJECTIVE To evaluate the physical inactivity-related inpatient costs of chronic non-communicable diseases. METHODS This study used data from 2013, from Brazilian Unified Health System, regarding inpatient numbers and costs due to malignant colon and breast neoplasms, cerebrovascular diseases, ischemic heart diseases, hypertension, diabetes, and osteoporosis. In order to calculate the share physical inactivity represents in that, the physical inactivity-related risks, which apply to each disease, were considered, and physical inactivity prevalence during leisure activities was obtained from Pesquisa Nacional por Amostra de Domicílio (Brazil's National Household Sample Survey). The analysis was stratified by genders and residing country regions of subjects who were 40 years or older. The physical inactivity-related hospitalization cost regarding each cause was multiplied by the respective share it regarded to. RESULTS In 2013, 974,641 patients were admitted due to seven different causes in Brazil, which represented a high cost. South region was found to have the highest patient admission rate in most studied causes. The highest prevalences for physical inactivity were observed in North and Northeast regions. The highest inactivity-related share in men was found for osteoporosis in all regions (≈ 35.0%), whereas diabetes was found to have a higher share regarding inactivity in women (33.0% to 37.0% variation in the regions). Ischemic heart diseases accounted for the highest total costs that could be linked to physical inactivity in all regions and for both genders, being followed by cerebrovascular diseases. Approximately 15.0% of inpatient costs from Brazilian Unified Health System were connected to physical inactivity. CONCLUSIONS Physical inactivity significantly impacts the number of patient admissions due to the evaluated causes and through their resulting costs, with different genders and country regions representing different shares. OBJETIVO Avaliar o custo de internações por doenças crônicas não transmissíveis atribuível à inatividade física. MÉTODOS Este estudo utilizou dados de 2013, do Sistema Único de Saúde, referentes ao número e respectivo custo das internações por neoplasia maligna de cólon e mama, doenças cerebrovasculares, doenças isquêmicas do coração, hipertensão, diabetes e osteoporose. Para o cálculo da fração atribuível à inatividade física foram considerados os riscos relativos da inatividade física a cada doença e a prevalência de inatividade física no lazer foi obtida da Pesquisa Nacional por Amostra de Domicílio. A análise foi estratificada por sexo e região do País de indivíduos com idade igual ou superior a 40 anos. O custo das internações de cada causa atribuível à inatividade física foi multiplicado pela respectiva fração a ela atribuível. RESULTADOS Foram realizadas 974.641 internações hospitalares por sete causas de internações no Brasil, em 2013, o que representou custo alto. A região Sul apresentou a maior taxa de internação na maioria das causas estudadas. A maior prevalência de inatividade física ocorreu nas regiões Norte e Nordeste. A maior fração atribuível à inatividade em homens foi encontrada para a osteoporose em todas as regiões (≈ 35,0%), enquanto o diabetes apresentou maior fração atribuível à inatividade em mulheres (variação de 33,0% a 37,0% nas regiões). As doenças isquêmicas do coração foram responsáveis pelos mais altos custos totais e atribuíveis à inatividade física em todas as regiões e em ambos os sexos, seguidas das doenças cerebrovasculares. Aproximadamente 15,0% dos custos ao Sistema Único de Saúde das internações foi atribuível à inatividade física. CONCLUSÕES A inatividade física impacta significativamente o número de internações hospitalares pelas causas avaliadas e nos custos resultantes, com diferenças na ocorrência dependendo do sexo e região do País.

OBJECTIVE To describe the migration flows of demand for public and private hospital care among the health regions of the state of Sao Paulo, Brazil. METHODS Study based on a database of hospitalizations in the public and private systems of the state of Sao Paulo, Southeastern Brazil, in 2006. We analyzed data from 17 health regions of the state, considering people hospitalized in their own health region and those who migrated outwards (emigration) or came from other regions (immigration). The index of migration effectiveness of patients from both systems was estimated. The coverage (hospitalization coefficient) was analyzed in relation to the number of inpatient beds per population and the indexes of migration effectiveness. RESULTS The index of migration effectiveness applied to the hospital care demand flow allowed characterizing health regions with flow balance, with high emigration of public and private patients, and with high attraction of public and private patients. CONCLUSIONS There are differences in hospital care access and opportunities among health regions in the state of Sao Paulo, Brazil. OBJETIVO Descrever os fluxos migratórios de demanda por assistência hospitalar pública e privada entre as regiões de saúde no estado de São Paulo, Brasil. MÉTODOS Estudo com base em banco de dados de hospitalizações nos sistemas público e privado, no estado de São Paulo, em 2006. Foram analisados dados de 17 regiões de saúde do estado, considerando aqueles hospitalizados na própria região de saúde e os que migraram para fora (evasão) ou vieram de outras regiões (invasão). Calculou-se o índice de eficácia migratória de pacientes de ambos os sistemas. Foi analisada a cobertura (coeficiente de hospitalização) em relação ao número de leitos por habitantes e os índices de eficácia migratória. RESULTADOS O índice de eficácia migratória aplicado ao fluxo de demanda da assistência hospitalar permitiu caracterizar as regiões de saúde com equilíbrio de fluxos, com alta evasão de pacientes públicos e privados, e com alta atração de pacientes públicos e privados. CONCLUSÕES Há diferenças de acesso e oportunidades à assistência hospitalar entre as regiões de saúde no estado de São Paulo.

OBJECTIVE To estimate differential associations between the exposure to violence in the family of origin and victimization and perpetration of intimate partner violence in pregnancy. METHODS A nested case-control study was carried out within a cohort study with 1,120 pregnant women aged 18–49 years old, who were registered in the Family Health Strategy of the city of Recife, State of Pernambuco, Brazil, between 2005 and 2006. The cases were the 233 women who reported intimate partner violence in pregnancy and the controls were the 499 women who did not report it. Partner violence in pregnancy and previous experiences of violence committed by parents or other family members were assessed with a standardized questionnaire. Multivariate logistic regression analyses were modeled to identify differential associations between the exposure to violence in the family of origin and victimization and perpetration of intimate partner violence in pregnancy. RESULTS Having seen the mother suffer intimate partner violence was associated with physical violence in childhood (OR = 2.62; 95%CI 1.89–3.63) and in adolescence (OR = 1.47; 95%CI 1.01–2.13), sexual violence in childhood (OR = 3.28; 95%CI 1.68–6.38) and intimate partner violence during pregnancy (OR = 1.47; 95% CI 1.01 – 2.12). The intimate partner violence during pregnancy was frequent in women who reported more episodes of physical violence in childhood (OR = 2.08; 95%CI 1.43–3.02) and adolescence (OR = 1.63; 95%CI 1.07–2.47), who suffered sexual violence in childhood (OR = 3.92; 95%CI 1.86–8.27), and who perpetrated violence against the partner (OR = 8.67; 95%CI 4.57–16.45). CONCLUSIONS Experiences of violence committed by parents or other family members emerge as strong risk factors for intimate partner violence in pregnancy. Identifying and understanding protective and risk factors for the emergence of intimate partner violence in pregnancy and its maintenance may help policymakers and health service managers to develop intervention strategies.

OBJECTIVE To compare the effectiveness of two speech therapy interventions, vocal warm-up and breathing training, focusing on teachers’ voice quality.METHODS A single-blind, randomized, parallel clinical trial was conducted. The research included 31 20 to 60-year old teachers from a public school in Salvador, BA, Northeasatern Brazil, with minimum workloads of 20 hours a week, who have or have not reported having vocal alterations. The exclusion criteria were the following: being a smoker, excessive alcohol consumption, receiving additional speech therapy assistance while taking part in the study, being affected by upper respiratory tract infections, professional use of the voice in another activity, neurological disorders, and history of cardiopulmonary pathologies. The subjects were distributed through simple randomization in groups vocal warm-up (n = 14) and breathing training (n = 17). The teachers’ voice quality was subjectively evaluated through the Voice Handicap Index (Índice de Desvantagem Vocal, in the Brazilian version) and computerized voice analysis (average fundamental frequency, jitter, shimmer, noise, and glottal-to-noise excitation ratio) by speech therapists.RESULTS Before the interventions, the groups were similar regarding sociodemographic characteristics, teaching activities, and vocal quality. The variations before and after the intervention in self-assessment and acoustic voice indicators have not significantly differed between the groups. In the comparison between groups before and after the six-week interventions, significant reductions in the Voice Handicap Index of subjects in both groups were observed, as wells as reduced average fundamental frequencies in the vocal warm-up group and increased shimmer in the breathing training group. Subjects from the vocal warm-up group reported speaking more easily and having their voices more improved in a general way as compared to the breathing training group.CONCLUSIONS Both interventions were similar regarding their effects on the teachers’ voice quality. OBJETIVO Comparar a efetividade de duas intervenções fonoaudiológicas, aquecimento vocal e treino respiratório, na qualidade vocal de professores.MÉTODOS Realizou-se ensaio clínico randomizado, cego ao avaliador, com delineamento em paralelo 1:1. Participaram da pesquisa 31 professores da rede estadual de ensino de Salvador, BA, entre 20 e 60 anos de idade, carga laboral mínima de 20h semanais, com ou sem alteração vocal autorreferida. Os critérios de exclusão foram: tabagismo, elevada frequência de consumo de álcool, acompanhamento fonoaudiológico simultâneo à participação no estudo, estado de infecções de trato respiratório superior, uso profissional da voz em outra atividade, distúrbios neurológicos e histórico de patologias cardiopulmonares. Os indivíduos foram alocados por randomização simples nos grupos aquecimento vocal (n = 14) e treino respiratório (n = 17). A qualidade vocal dos professores foi avaliada subjetivamente pelo Índice de Desvantagem Vocal e pela análise acústica computadorizada da voz (frequência fundamental média, jitter, shimmer, ruído e proporção glottal-to-noise excitation), realizada por fonoaudiólogos.RESULTADOS Antes das intervenções, os grupos eram semelhantes quanto às características sociodemográficas, atividade docente e qualidade vocal. As variações pré e pós-intervenção nos indicadores de autoavaliação e acústicos da voz não diferiram significantemente entre os grupos. Na comparação intragrupos, pré e pós-intervenções de seis semanas, observou-se redução significativa do Índice de Desvantagem Vocal nos participantes dos dois grupos, redução da frequência fundamental média no grupo aquecimento vocal e aumento doshimmer no grupo treino respiratório. Indivíduos do grupo aquecimento vocal referiram maior facilidade para falar e maior benefício em relação à melhoria geral da voz, comparado ao grupo treino respiratório.CONCLUSÕES Ambas intervenções foram similares quanto a seus efeitos sobre a qualidade vocal dos professores. Entretanto, cada intervenção isoladamente, em especial o aquecimento vocal, contribuiu para melhorar a qualidade vocal dos professores.REGISTRO DO ENSAIO NCT02102399, “Vocal Warm-up and Respiratory Muscle Training in Teachers”.

OBJECTIVE To analyze the influence from context characteristics in the control of tuberculosis in prisons, and the influence from the program implementation degrees in observed effects.METHODS A multiple case study, with a qualitative approach, conducted in the prison systems of two Brazilian states in 2011 and 2012. Two prisons were analyzed in each state, and a prison hospital was analyzed in one of them. The data were submitted to a content analysis, which was based on external, political-organizational, implementation, and effect dimensions. Contextual factors and the ones in the program organization were correlated. The independent variable was the program implementation degree and the dependent one, the effects from the Tuberculosis Control Program in prisons.RESULTS The context with the highest sociodemographic vulnerability, the highest incidence rate of tuberculosis, and the smallest amount of available resources were associated with the low implementation degree of the program. The results from tuberculosis treatment in the prison system were better where the program had already been partially implemented than in the case with low implementation degree in both cases.CONCLUSIONS The implementation degree and its contexts – external and political-organizational dimensions – simultaneously contribute to the effects that are observed in the control of tuberculosis in analyzed prisons. OBJETIVO Analisar a influência das características contextuais do controle da tuberculose nas prisões e a influência dos graus de implantação do programa nos efeitos observados.MÉTODOS Estudo de casos múltiplos, com abordagem qualitativa, realizado no sistema penitenciário de dois estados brasileiros em 2011 e 2012. As unidades de análise foram duas unidades prisionais em cada estado e um hospital penal em um deles. Os dados foram objeto de análise de conteúdo, que teve como eixo condutor as dimensões externa, político-organizacional, implantação e efeitos. Foi realizada correlação entre os fatores contextuais e da organização do programa que influenciaram a implantação das ações. A variável independente foi o grau de implantação do programa e a dependente, os efeitos do Programa de Controle da Tuberculose nas unidades prisionais.RESULTADOS O contexto com maior vulnerabilidade sociodemográfica, maior taxa de incidência de tuberculose e menor disponibilidade de recursos associou-se ao baixo nível de implantação do programa. Os desfechos do tratamento da tuberculose no sistema prisional, nos dois casos, foram melhores onde o grau de implantação verificado foi parcialmente implantado, do que no caso com baixo nível de implantação.CONCLUSÕES O grau de implantação e os contextos – dimensões externa e político-organizacional – contribuem simultaneamente para os efeitos observados no controle da tuberculose nas prisões analisadas.

OBJECTIVE To analyze the spatial distribution of risk for tuberculosis and its socioeconomic determinants in the city of Rio de Janeiro, Brazil.METHODS An ecological study on the association between the mean incidence rate of tuberculosis from 2004 to 2006 and socioeconomic indicators of the Censo Demográfico (Demographic Census) of 2000. The unit of analysis was the home district registered in the Sistema de Informação de Agravos de Notificação (Notifiable Diseases Information System) of Rio de Janeiro, Southeastern Brazil. The rates were standardized by sex and age group, and smoothed by the empirical Bayes method. Spatial autocorrelation was evaluated by Moran’s I. Multiple linear regression models were studied and the appropriateness of incorporating the spatial component in modeling was evaluated.RESULTS We observed a higher risk of the disease in some neighborhoods of the port and north regions, as well as a high incidence in the slums of Rocinha and Vidigal, in the south region, and Cidade de Deus, in the west. The final model identified a positive association for the variables: percentage of permanent private households in which the head of the house earns three to five minimum wages; percentage of individual residents in the neighborhood; and percentage of people living in homes with more than two people per bedroom.CONCLUSIONS The spatial analysis identified areas of risk of tuberculosis incidence in the neighborhoods of the city of Rio de Janeiro and also found spatial dependence for the incidence of tuberculosis and some socioeconomic variables. However, the inclusion of the space component in the final model was not required during the modeling process. OBJETIVO Analisar a distribuição espacial de sobrerrisco para tuberculose e seus determinantes socioeconômicos no município do Rio de Janeiro.MÉTODOS Estudo ecológico sobre a associação entre a média da taxa de incidência da tuberculose de 2004 a 2006 e indicadores socioeconômicos do Censo Demográfico do ano 2000. A unidade de análise foi o bairro de residência registrado no Sistema de Informação de Agravos de Notificação do município do Rio de Janeiro, RJ. As taxas foram padronizadas por sexo e faixa etária e suavizadas pelo método bayesiano empírico. A autocorrelação espacial foi avaliada pelo índice de Moran. Foram estudados modelos de regressão linear múltipla e avaliada a pertinência de incorporar o componente espacial na modelagem.RESULTADOS Observou-se risco maior de adoecimento em alguns bairros da zona portuária e zona norte, além de grande incidência nas favelas da Rocinha e Vidigal, na zona sul e Cidade de Deus, na zona oeste. O modelo final identificou associação positiva para as variáveis: percentual de domicílios particulares permanentes em que o responsável recebia de três a cinco salários mínimos; percentual de moradores individuais no bairro; e percentual de pessoas que viviam em domicílio com mais de duas pessoas por dormitório.CONCLUSÕES A análise espacial identificou áreas de sobrerrisco de incidência da tuberculose nos bairros do município do Rio de Janeiro e encontrou dependência espacial para a incidência de tuberculose e algumas variáveis socioeconômicas. Entretanto, a inclusão do componente espacial, no modelo final, não se mostrou necessária durante o processo de modelagem.

OBJECTIVE To examine whether religiousness mediates the relationship between sociodemographic factors, multimorbidity and health-related quality of life of older adults. METHODS This population-based cross-sectional study is part of the Survey on Health, Well-Being, and Aging (SABE). The sample was composed by 911 older adults from Sao Paulo, SP, Southeastern Brazil. Structural equation modeling was performed to assess the mediator effect of religiousness on the relationship between selected variables and health-related quality of life of older adults, with models for men and women. The independent variables were: age, education, family functioning and multimorbidity. The outcome variable was health-related quality of life of older adults, measured by SF-12 (physical and mental components). The mediator variables were organizational, non-organizational and intrinsic religiousness. Cronbach’s alpha values were: physical component = 0.85; mental component = 0.80; intrinsic religiousness = 0.89 and family APGAR (Adaptability, Partnership, Growth, Affection, and Resolve) = 0.91. RESULTS Higher levels of organizational and intrinsic religiousness were associated with better physical and mental components. Higher education, better family functioning and fewer diseases contributed directly to improved performance in physical and mental components, regardless of religiousness. For women, organizational religiousness mediated the relationship between age and physical (β = 2.401, p < 0.01) and mental (β = 1.663, p < 0.01) components. For men, intrinsic religiousness mediated the relationship between education and mental component (β = 7.158, p < 0.01). CONCLUSIONS Organizational and intrinsic religiousness had a beneficial effect on the relationship between age, education and health-related quality of life of these older adults. OBJETIVO : Analisar se a religiosidade exerce efeito mediador na relação entre fatores sociodemográficos, multimorbidade e qualidade de vida relacionada à saúde em idosos. MÉTODOS : Este estudo transversal, de base populacional, é parte do estudo Saúde, Bem-Estar e Envelhecimento (SABE). A amostra foi composta por 911 idosos da cidade de São Paulo, residentes na comunidade. A modelagem de equações estruturais foi realizada para verificar o efeito mediador da religiosidade na relação entre variáveis selecionadas e a qualidade de vida relacionada à saúde dos idosos, com modelos para homens e mulheres. As variáveis independentes foram: idade, escolaridade, funcionalidade familiar e multimorbidade. A variável de desfecho foi a qualidade de vida relacionada à saúde, medida pelo SF-12 (componentes físico e mental). As variáveis mediadoras foram a religiosidade organizacional, a não organizacional e a intrínseca. Os coeficientes alfa de Cronbach foram: componente físico = 0,85; componente mental = 0,80; religiosidade intrínseca = 0,89 e Apgar familiar = 0,91. RESULTADOS : Maiores níveis de religiosidade organizacional e intrínseca estiveram associados a melhor componente físico e mental para os idosos. Maior escolaridade, melhor funcionalidade familiar e menor número de doenças contribuíram diretamente para melhor desempenho nos componentes físico e mental, independente da religiosidade. Para as mulheres, a religiosidade organizacional mediou a relação entre a idade e os componentes físico (β = 2,401; p < 0,01) e mental (β = 1,663; p < 0,01). Para os homens, a religiosidade intrínseca mediou a relação entre a escolaridade e o componente mental (β = 7,158; p < 0,01). CONCLUSÕES : A religiosidade organizacional e intrínseca exercem efeito benéfico sobre a relação entre idade, escolaridade e a qualidade de vida relacionada à saúde desses idosos.

OBJECTIVE To evaluate the impact of consuming ultra-processed foods on the micronutrient content of the Brazilian population’s diet. METHODS This cross-sectional study was performed using data on individual food consumption from a module of the 2008-2009 Brazilian Household Budget Survey. A representative sample of the Brazilian population aged 10 years or over was assessed (n = 32,898). Food consumption data were collected through two 24-hour food records. Linear regression models were used to assess the association between the nutrient content of the diet and the quintiles of ultra-processed food consumption – crude and adjusted for family income per capita. RESULTS Mean daily energy intake per capita was 1,866 kcal, with 69.5% coming from natural or minimally processed foods, 9.0% from processed foods and 21.5% from ultra-processed foods. For sixteen out of the seventeen evaluated micronutrients, their content was lower in the fraction of the diet composed of ultra-processed foods compared with the fraction of the diet composed of natural or minimally processed foods. The content of 10 micronutrients in ultra-processed foods did not reach half the content level observed in the natural or minimally processed foods. The higher consumption of ultra-processed foods was inversely and significantly associated with the content of vitamins B12, vitamin D, vitamin E, niacin, pyridoxine, copper, iron, phosphorus, magnesium, selenium and zinc. The reverse situation was only observed for calcium, thiamin and riboflavin. CONCLUSIONS The findings of this study highlight that reducing the consumption of ultra-processed foods is a natural way to promote healthy eating in Brazil and, therefore, is in line with the recommendations made by the Guia Alimentar para a População Brasileira (Dietary Guidelines for the Brazilian Population) to avoid these foods.

OBJECTIVE: To determine the prevalence of metabolic syndrome (MS) and the hypertriglyceridemic waist phenotype (HW) in a representative adolescent sample; as well as to establish which anthropometric indicator better identifies MS and HW, according to gender and adolescent age. METHODS: This cross sectional study had the participation of 800 adolescents (414 girls) from 10-19 years old. Anthropometric indicators (body mass index, waist perimeter, waist/stature ratio, waist/hip ratio, and central/peripheral skinfolds) were determined by standard protocols. For diagnosis of MS, the criteria proposed by de Ferranti et al. (2004) were used. HW was defined by the simultaneous presence of increased waist perimeter (>75th percentile for age and sex) and high triglycerides (>100 mg/dL). The ability of anthropometric indicators was evaluated by Receiver Operating Characteristic curve. RESULTS: The prevalence of MS was identical to HW (6.4%), without differences between genders and the adolescence phases. The waist perimeter showed higher area under the curve for the diagnosis of MS, except for boys with 17-19 years old, for whom the waist/stature ratio exhibited better performance. For diagnosing HW, waist perimeter also showed higher area under the curve, except for boys in initial and final phases, in which the waist/stature ratio obtained larger area under the curve. The central/peripheral skinfolds had the lowest area under the curve for the presence of both MS and HW phenotype. CONCLUSIONS: The waist perimeter and the waist/stature showed a better performance to identify MS and HW in both genders and in all three phases of adolescence. OBJETIVO: Determinar a ocorrência de síndrome metabólica (SM) e do fenótipo cintura hipertrigliceridêmica (CH) em amostra de adolescentes e estabelecer qual indicador antropométrico melhor identifica SM e CH, de acordo com gênero e fase da adolescência. MÉTODOS: Estudo transversal com 800 adolescentes (414 meninas) de 10-19 anos. Indicadores antropométricos (índice de massa corporal, perímetro da cintura, relação cintura/estatura, relação cintura/quadril e relação pregas cutâneas centrais/periféricas) foram determinados por protocolos padronizados. Para diagnóstico da SM, foi usada a proposta de Ferranti et al. (2004). A CH foi definida pela presença simultânea de perímetro da cintura aumentado (>75 percentil por idade e sexo) e triglicerídeos elevados (>100 mg/dL). O desempenho dos indicadores antropométricos foi avaliado por meio da curva Receiver Operating Characteristic. RESULTADOS: A prevalência de SM foi idêntica à de CH (6,4%), sem diferenças entre os gêneros e entre as fases da adolescência. O perímetro da cintura apresentou maior área abaixo da curva no diagnóstico da SM, exceto para meninos entre 17-19 anos, para os quais a relação cintura/estatura exibiu melhor desempenho. No diagnóstico da CH, a cintura isolada apresentou maior área abaixo da curva, exceto para os meninos nas fases inicial e final da adolescência, nos quais a relação cintura/estatura obteve maior área. A relação entre as pregas cutâneas apresentou pior desempenho para identificar SM e CH. CONCLUSÕES: O perímetro da cintura e a relação cintura/estatura mostraram o melhor desempenho para identificar SM e CH em ambos os sexos e nas três fases da adolescência.

Métodos: Estudo transversal que selecionou 120 escolares (idade média 11,7±0,7 anos), sem consumo de medicamentos. Prática esportiva fora do ambiente escolar e educação física escolar foram avaliadas por entrevista face a face, enquanto a atividade física habitual foi avaliada por pedometria. Peso corporal, estatura e altura troncocefálica foram usados para estimar a maturação biológica. Foram avaliados: gordura corporal, pressão arterial, frequência cardíaca durante o repouso, velocidade do fluxo sanguíneo, espessura mediointimal das artérias (carótida e femoral), variabilidade da frequência cardíaca (média entre batimentos cardíacos consecutivos e o índice estatístico no domínio do tempo que representa atividade do sistema nervoso autônomo parassimpático por meio da raiz quadrada da média das diferenças sucessivas ao quadrado entre intervalos R-R consecutivos). Correlação de Spearman verificou relação entre as variáveis. Relacionamentos significativos foram ajustados por: sexo, etnia, idade, gordura corporal e maturação biológica. Resultados: Prática esportiva, independentemente dos ajustes, apresentou correlação positiva com atividade do sistema nervoso autônomo parassimpático (=0,039 [0,01; 0,76]). Por outro lado, a relação entre tal engajamento e a média entre os intervalos R-R (=0,031 [---0,01; 0,07]) foi mediada pela maturação biológica. Conclusões: A prática esportiva foi relacionada a uma maior variabilidade da frequência cardíaca durante o repouso. Objective: To analyze the relationship among sports practice, physical education class, habitual physical activity and cardiovascular risk in adolescents. Methods: Cross-sectional study with 120 schoolchildren (mean: 11.7±0.7 years old), with no regular use of medicines. Sports practice and physical education classes were assessed through face-to-face interview, while habitual physical activity was assessed by pedometers. Bodyweight, height and height-cephalic trunk were used to estimate maturation. The following variables were measured: body fatness, blood pressure, resting heart rate, blood flow velocity, intima-media thickness (carotid and femoral) and heart rate variability (mean between consecutive heartbeats and statistical index in the time domain that show the autonomic parasympathetic nervous system activity root-mean by the square of differences between adjacent normal R-R intervals in a time interval). Statistical treatment used Spearman correlation adjusted by sex, ethnicity, age, body fatness and maturation. Results: Independently of potential confounders, sports practice was positively related to autonomic parasympathetic nervous system activity (b=0.039 [0.01; 0.76]). On the other hand, the relationship between sport practice and mean between consecutive heartbeats (b=0,031 [–0.01; 0.07]) was significantly mediated by biological maturation. Conclusions: Sport practice was related to higher heart rate variability at rest. OBJECTIVE: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a minimally invasive diagnostic test with a high diagnostic yield for suspicious central pulmonary lesions and for mediastinal lymph node staging. The main objective of this study was to describe the diagnostic yield of EBUS-TBNA for mediastinal lymph node staging in patients with suspected lung cancer. METHODS: Prospective study of patients undergoing EBUS-TBNA for diagnosis. Patients ≥ 18 years of age were recruited between July of 2010 and August of 2013. We recorded demographic variables, radiological characteristics provided by axial CT of the chest, location of the lesion in the mediastinum as per the International Association for the Study of Lung Cancer classification, and definitive diagnostic result (EBUS with a diagnostic biopsy or a definitive diagnostic method). RESULTS: Our analysis included 354 biopsies, from 145 patients. Of those 145 patients, 54.48% were male. The mean age was 63.75 years. The mean lymph node size was 15.03 mm, and 90 lymph nodes were smaller than 10.0 mm. The EBUS-TBNA method showed a sensitivity of 91.17%, a specificity of 100.0%, and a negative predictive value of 92.9%. The most common histological diagnosis was adenocarcinoma. CONCLUSIONS: EBUS-TBNA is a diagnostic tool that yields satisfactory results in the staging of neoplastic mediastinal lesions. OBJETIVO: La ultrasonografía endobronquial con aspiración transbronquial por aguja fina (EBUS-TBNA, por sus siglas en inglés) es una alternativa mínimamente invasiva con un alto rendimiento diagnóstico para lesiones pulmonares centrales sospechosas de cáncer o para etapificación de linfonodos mediastínicos. El objetivo principal de este trabajo es describir el rendimiento de EBUS-TBNA como método de etapificación de linfonodos mediastínicos en pacientes con sospecha de cáncer pulmonar. MÉTODOS: Estudio prospectivo de pacientes sometidos a EBUS-TBNA como método diagnóstico. Se seleccionaron pacientes mayores de 18 años entre julio del 2010 y agosto del 2013. Se registraron variables demográficas, características radiológicas mediante TC axial de tórax, localización mediastínica según clasificación de la International Association for the Study of Lung Cancer y resultado diagnóstico definitivo (EBUS con biopsia diagnóstica o método diagnóstico definitivo). RESULTADOS: Se incluyeron 145 pacientes con un total de 354 biopsias. El 54,48% de los pacientes eran hombres, con edad promedio de 63,75 años. El tamaño promedio de los linfonodos fue de 15,03 mm, y 90 fueron menores de 10,0 mm. El rendimiento diagnóstico fue: sensibilidad, 91,17%; especificidad, 100,0%; y valor predictivo negativo, 92,9%. El diagnóstico histológico más frecuente fue adenocarcinoma. CONCLUSIONES: EBUS-TBNA es una herramienta diagnóstica con buenos resultados en el estudio de etapificación de lesiones neoplásicas en mediastino.

Objective: To identify risk factors for respiratory complications after adenotonsillectomy in children ≤ 12 years of age with obstructive sleep apnea who were referred to the pediatric ICU (PICU). Methods: A cross-sectional historical cohort study analyzing 53 children after adenotonsillectomy who met predetermined criteria for PICU referral in a tertiary level teaching hospital. The Student's t-test, Mann-Whitney test, and chi-square test were used to identify risk factors. Results: Of the 805 children undergoing adenotonsillectomy between January of 2006 and December of 2012 in the teaching hospital, 53 were referred to the PICU. Twenty-one children (2.6% of all those undergoing adenotonsillectomy and 39.6% of those who were referred to the PICU) had respiratory complications. Of those 21, 12 were male. The mean age was 5.3 ± 2.6 years. A high apnea-hypopnea index (AHI; p = 0.0269), a high oxygen desaturation index (ODI; p = 0.0082), a low SpO2 nadir (p = 0.0055), prolonged orotracheal intubation (p = 0.0011), and rhinitis (p = 0.0426) were found to be independent predictors of respiratory complications. Some of the complications observed were minor (SpO2 90-80%), whereas others were major (SpO2 ≤ 80%, laryngospasm, bronchospasm, acute pulmonary edema, pneumonia, and apnea). Conclusions: Among children up to 12 years of age with OSA, those who have a high AHI, a high ODI, a low SpO2 nadir, or rhinitis are more likely to develop respiratory complications after adenotonsillectomy than are those without such characteristics.

OBJECTIVE Analyze the implementation of drug price regulation policy by the Drug Market Regulation Chamber.METHODS This is an interview-based study, which was undertaken in 2012, using semi-structured questionnaires with social actors from the pharmaceutical market, the pharmaceuticals industry, consumers and the regulatory agency. In addition, drug prices were compiled based on surveys conducted in the state of Sao Paulo, at the point of sale, between February 2009 and May 2012.RESULTS The mean drug prices charged at the point of sale (pharmacies) were well below the maximum price to the consumer, compared with many drugs sold in Brazil. Between 2009 and 2012, 44 of the 129 prices, corresponding to 99 drugs listed in the database of compiled prices, showed a variation of more than 20.0% in the mean prices at the point of sale and the maximum price to the consumer. In addition, many laboratories have refused to apply the price adequacy coefficient in their sales to government agencies.CONCLUSIONS The regulation implemented by the pharmaceutical market regulator was unable to significantly control prices of marketed drugs, without succeeding to push them to levels lower than those determined by the pharmaceutical industry and failing, therefore, in its objective to promote pharmaceutical support for the public. It is necessary reconstruct the regulatory law to allow market prices to be reduced by the regulator as well as institutional strengthen this government body. OBJETIVO Analisar a implementação da política de regulação de preços de medicamentos pela Câmara de Regulação do Mercado de Medicamentos.MÉTODOS Estudo baseado na realização de entrevistas, em 2012, usando questionários semiestruturados, com atores sociais do mercado de medicamentos, representantes da indústria farmacêutica, de consumidores e do órgão regulador. Foram também compilados preços de medicamentos obtidos em pesquisas realizadas no Estado de São Paulo, nos pontos de venda, entre fevereiro de 2009 e maio de 2012.RESULTADOS As médias dos preços de medicamentos praticadas nos pontos de venda (farmácias e drogarias) estiveram muito abaixo do preço máximo ao consumidor, em relação à grande parte dos medicamentos comercializados no Brasil. Entre 2009 e 2012, 44 dos 129 preços praticados, correspondentes a 99 medicamentos constantes do banco de preços compilados, apresentaram variação superior a 20,0% entre a média de preços praticados nos pontos de venda e o preço máximo ao consumidor. Ademais, muitos laboratórios se recusaram a aplicar o coeficiente de adequação de preços nas vendas a órgãos públicos.CONCLUSÕES A regulação implementada pelo órgão regulador do mercado de medicamentos foi incapaz de controlar os preços de quantidade significativa dos medicamentos comercializados, pressionando-os a patamares inferiores àqueles determinados pela indústria farmacêutica e falhando, portanto, no seu objetivo de promover assistência farmacêutica. É necessária reforma da lei regulamentadora para permitir a redução de preços praticados no mercado pelo órgão regulador, bem como fortalecimento institucional desse órgão.

OBJECTIVE: To determine the frequency of HRCT findings and their distribution in the lung parenchyma of patients with organizing pneumonia. METHODS: This was a retrospective review of the HRCT scans of 36 adult patients (26 females and 10 males) with biopsy-proven organizing pneumonia. The patients were between 19 and 82 years of age (mean age, 56.2 years). The HRCT images were evaluated by two independent observers, discordant interpretations being resolved by consensus. RESULTS: The most common HRCT finding was that of ground-glass opacities, which were seen in 88.9% of the cases. The second most common finding was consolidation (in 83.3% of cases), followed by peribronchovascular opacities (in 52.8%), reticulation (in 38.9%), bronchiectasis (in 33.3%), interstitial nodules (in 27.8%), interlobular septal thickening (in 27.8%), perilobular pattern (in 22.2%), the reversed halo sign (in 16.7%), airspace nodules (in 11.1%), and the halo sign (in 8.3%). The lesions were predominantly bilateral, the middle and lower lung fields being the areas most commonly affected. CONCLUSIONS: Ground-glass opacities and consolidation were the most common findings, with a predominantly random distribution, although they were more common in the middle and lower thirds of the lungs. OBJETIVO: Determinar a frequência dos achados tomográficos e sua distribuição no parênquima pulmonar de pacientes com pneumonia em organização. MÉTODOS: Estudo retrospectivo das imagens de TCAR de 36 pacientes adultos (26 mulheres e 10 homens) com pneumonia em organização confirmada por exame histopatológico. A faixa etária dos pacientes incluídos foi de 19 a 82 anos (média: 56,2 anos). As imagens foram avaliadas por dois observadores, de forma independente, e os casos discordantes foram resolvidos por consenso. RESULTADOS: O achado tomográfico mais comum foi o de opacidades em vidro fosco, presentes em 88,9% dos casos. O segundo achado mais comum foi o de consolidação (em 83,3% dos casos), seguido de opacidades peribroncovasculares (em 52,8%), reticulação (em 38,9%), bronquiectasias (em 33,3%), nódulos intersticiais (em 27,8%), espessamento de septos interlobulares (em 27,8%), padrão perilobular (em 22,2%), sinal do halo invertido (em 16,7%), nódulos do espaço aéreo (em 11,1%) e sinal do halo (em 8,3%). As lesões foram predominantemente bilaterais, e os terços médios e inferiores dos pulmões foram as regiões mais afetadas. CONCLUSÕES: Os achados mais frequentes foram opacidades em vidro fosco e consolidações, com distribuição predominantemente aleatória, embora tenham sido mais comuns nos terços médios e inferiores dos pulmões.

OBJECTIVE: To identify environment factors predicting sensory profile of infants between 4 and 18 months old. METHODS: This cross-sectional study evaluated 97 infants (40 females e 57 males), with a mean age of 1.05±0.32 years with the Test of Sensory Functions in Infants (TSFI) and also asked 97 parents and 11 kindergarten teachers of seven daycare centers to answer the Affordances in the Home Environment for Motor Development-Infant Scale (AHEMD-IS). The AHEMD-IS is a questionnaire that characterizes the opportunities in the home environment for infants between 3 and 18 months of age. We tested the association between affordances and the sensory profile of infants. Significant variables were entered into a regression model to determine predictors of sensory profile. RESULTS: The majority of infants (66%) had a normal sensory profile and 34% were at risk or deficit. Affordances in the home were classified as adequate and they were good in the studied daycare centers. The results of the regression revealed that only daily hours in daycare center and daycare outside space influenced the sensory profile of infants, in particular the Ocular-Motor Control component. CONCLUSIONS: The sensory profile of infants was between normal and at risk. While the family home offered adequate affordances for motor development, the daycare centers of the infants involved demonstrated a good quantity and quality of affordances. Overall, we conclude that daily hours in the daycare center and daycare outside space were predictors of the sensory profile, particular on Ocular-Motor Control component.

OBJECTIVE To analyze the factors associated with stiffness of the great arteries in prepubertal children.METHODS This study with convenience sample of 231 schoolchildren aged 9-10 years enrolled in public and private schools in Vitória, ES, Southeastern Brazil, in 2010-2011. Anthropometric and hemodynamic data, blood pressure, and pulse wave velocity in the carotid-femoral segment were obtained. Data on current and previous health conditions were obtained by questionnaire and notes on the child’s health card. Multiple linear regression was applied to identify the partial and total contribution of the factors in determining the pulse wave velocity values.RESULTS Among the students, 50.2% were female and 55.4% were 10 years old. Among those classified in the last tertile of pulse wave velocity, 60.0% were overweight, with higher mean blood pressure, waist circumference, and waist-to-height ratio. Birth weight was not associated with pulse wave velocity. After multiple linear regression analysis, body mass index (BMI) and diastolic blood pressure remained in the model.CONCLUSIONS BMI was the most important factor in determining arterial stiffness in children aged 9-10 years. OBJETIVO Analisar fatores associados à rigidez arterial dos grandes vasos arteriais em crianças pré-púberes.MÉTODOS Estudo com amostra de conveniência de 231 escolares de nove e 10 anos de idade matriculados em instituições públicas e privadas de Vitória, ES, 2010-2011. Foram obtidos dados antropométricos e hemodinâmicos, pressão arterial e velocidade de onda de pulso no segmento carotídeo-femoral. Dados sobre condições de saúde atuais e pregressas das crianças foram obtidas por meio de questionário e anotações no cartão de saúde da criança. Foi aplicada regressão linear múltipla para identificar a contribuição parcial e total dos fatores na determinação dos valores da velocidade de onda de pulso.RESULTADOS Dentre os escolares, 50,2% eram meninas e 55,4% tinham 10 anos de idade. Entre as classificadas no último tercil de velocidade de onda de pulso, 60,0% apresentavam excesso de peso, maiores médias de pressão arterial, circunferência da cintura e relação cintura/estatura. O peso ao nascer não foi associado à velocidade de onda de pulso. Após análise de regressão linear múltipla, permaneceram no modelo as variáveis índice de massa corporal e pressão arterial diastólica.CONCLUSÕES Excesso de peso foi o mais importante fator na determinação da rigidez arterial de crianças.
