Abstract

Nasopharyngeal carcinoma (NPC) is a particularly prevalent carcinoma in southern China. Central nervous system (CNS) metastasis is an infrequent lesion of NPC, although direct intracranial invasion is not uncommon with locally advanced stage. The pathway of CNS metastasis is postulated to be either through hematogenous route or cerebral spinal fluid (CSF) spread. Even though application of surgical management and radiotherapy may contribute to improved survival and neurologic function of some patients, the overall survival in these patients remains poor.

Abstract

Background: As a result of the one‐child policy of 1979, today there are numerous one‐child families with adolescents in Mainland China. Little is known about the experiences of parents of such adolescents diagnosed with cancer. Objectives: This study explored the experiences of parents whose adolescent child was diagnosed with cancer in one‐child families in China. Methods: A qualitative methodology based on hermeneutic phenomenology was employed. The participants were parents of adolescent cancer patients in one‐child families. Data were collected using in‐depth semi‐structured interviews and analysed using van Manen's hermeneutic phenomenological approach. The COREQ checklist was used for this study. Results: Fourteen parents (eleven mothers, three fathers) participated in the study. One overarching theme emerged: feeling like the sky is falling down. In addition, there were five related themes: self‐condemnation; 'white lies' – that is, difficulty in telling the truth; struggling with a sense of collapse; fear of losing the child and hopelessness. Almost all the participants experienced insomnia after learning about the diagnosis. In a few cases, hopelessness‐induced suicidal ideation or even suicidal behaviour. Conclusions: Parents of adolescents diagnosed with cancer in one‐child families in China experienced extremely painful emotions. The truth about their child's condition not only caused immense psychological trauma but also induced pessimism about their own future. Implications for clinical practice: The experiences of parents in one‐child families where an adolescent child has been diagnosed with cancer should be taken seriously. Abstract

Crowd flow analysis is a important part of urban computing, which is playing an vital role in urban plan and management. This paper addresses the challenges in current population flow analysis. We propose a user space dynamic discovery method based on graph signal model. Taking the base station as a spatial node, the spatial dependence relationship between the nodes is modeled as a spatial network map. The spectral wavelet operator is applied to the spatial map signal to generate wavelet coefficients on different wavelet scales. The simulation results, show that our scheme can be used for valuable information such as the origin, spread and span of population mobility.

Abstract

T helper 1 (Th1) immunity is typically viewed as a critical adaptation by vertebrates against intracellular pathogens. Identifying novel targets to enhance Th1 cell differentiation and function is increasingly important for anti-infection immunity. Here, through small-molecule screening focusing on epigenetic modifiers during the in vitro Th1 cell differentiation process, we identified that the selective histone deacetylase 6 (HDAC6) inhibitors ricolinostat and nexturastat A (Nex A) promoted Th1 cell differentiation. HDAC6-depleted mice exhibit elevation of Th1 cell differentiation, and decreased severity of Listeria monocytogenesinfection. Mechanistically, HDAC6 directly deacetylated CBP-catalyzed acetylation of signal transducer and activator of transcription 4 (STAT4)-lysine (K) 667 via its enzymatic activity. Acetylation of STAT4-K667 is required for JAK2-mediated phosphorylation and activation of STAT4. Stat4K667Rmutant mice lost the ability to normally differentiate into Th1 cells and developed severe Listeriainfection. Our study identifies acetylation of STAT4-K667 as an essential signaling event for Th1 cell differentiation and defense against intracellular pathogen infections, and highlights the therapeutic potential of HDAC6 inhibitors for controlling intracellular pathogen infections.

197. Impacts of traffic accessibility on ecosystem services: An integrated spatial approach

Abstract

The continuous degradation of ecosystem services is an important challenge faced by the world. Improvements in transportation infrastructure have had substantial impacts on economic development and ecosystem services. Exploring the influence of traffic accessibility on ecosystem services can delay or stop their deterioration; however, studies on its impact are lacking. This study addresses this gap by analysing the impact of traffic accessibility on ecosystem services using an integrated spatial regression approach based on an evaluation of the ecosystem services value (ESV) and traffic accessibility in the Middle Reaches of the Yangtze River Urban Agglomeration (MRYRUA) in China. The results indicated that the ESV in the MRYRUA continuously decreased during the study period, and the average ESV in plain areas, areas surrounding the core cities, and areas along the main traffic routes was significantly lower than that in areas along the Yangtze River and the surrounding mountainous areas. Traffic accessibility continued to increase during the study period, and the high-value areas centred on Wuhan, Changsha, Nanchang, and Yichang were radially distributed. The global bivariate spatial autocorrelation coefficient between the average ESV and traffic accessibility was negative. The average ESV and traffic accessibility exhibited significant spatial dependence and spatial heterogeneity. Spatial regression also proved that there was a negative association between the average ESV and traffic accessibility, and scale effects were evident. The findings of this study have important policy implications for future ecological protection and transportation planning.

Abstract

With the frequent occurrence of extreme weather events in the world, the ability of power system to resist disasters has attracted more and more attention. In this paper, a method to evaluate the resilience of distribution networks by focusing on the impact of critical loads under extreme weather events is proposed, and typhoon is taken as the representative to formulate a vulnerability curve of components. Furthermore, the Monte Carlo method is used to simulate the whole process of extreme weather disaster and to generate the fault scenario. Different weights are assigned to different levels of load according to the importance of load, and the weighted loss of load is selected as the evaluation index of resilience. Finally, the method is verified by the IEEE-33 bus system. The results show that this method is effective to quantify the resilience of distribution networks under extreme weather events.

199. CA125: An Increasingly Promising Biomarker of Heart Failure

Abstract

CA125 is a well-known tumor marker for diagnosis, monitoring, and risk stratification in ovarian cancer. It is not specific for malignant tumors and may be elevated in benign disease. In the past two decades, increasing evidence has emerged suggesting that the plasma level of CA125 can serve as a novel surrogate of heart failure (HF). CA125 in patients with HF is synthesized by serous epithelial cells in response to both mechanical and inflammatory stimuli. In patients with HF, regardless of etiology, CA125 levels correlate with the severity of clinical, hemodynamic, and echocardiographic parameters and with other biomarkers. Elevated CA125 can identify patients at high risk of rehospitalization and mortality, whether short- or long-term. Serial measurements and combination with different pathophysiology biomarkers can provide a more accurate prognosis value. It also can guide treatment as a robust biomarker of fluid overload and inflammation, particularly for diuretic dose optimization. These properties make it a very promising candidate for risk stratification and treatment guidance of HF.

Abstract

Background: Acquired epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI) resistance limits the long-term clinical efficacy of tyrosine kinase-targeting drugs. Although most of the mechanisms of acquired EGFR-TKI resistance have been revealed, the mechanism of ~ 15% of cases has not yet been elucidated. Methods: Cell viability was analysed using the Cell Counting Kit-8 (CCK-8) assay. Proteome profiler array analysis was performed to find proteins contributing to acquired EGFR-TKI resistance. Secreted OPN was detected by ELISA. Immunohistochemical analysis was conducted to detect expression of integrin αV in NSCLC tissue. The effect of VS-6063 on apoptosis and proliferation of PC9 gefitinib-resistant cells was detected by fluorescence-activated cell sorting (FACS) and clonogenic assays. A mouse xenograft model was used to assess the effect of VS-6063 on the sensitivity of PC9 gefitinib-resistant cells to gefitinib. Results: OPN was overexpressed in acquired EGFR-TKI-resistant NSCLCs. Secreted OPN contributed to acquired EGFR-TKI resistance by activating the integrin αVβ3/FAK pathway. Inhibition of FAK signalling increased sensitivity to EGFR-TKIs in PC9 gefitinib-resistant cells both in vitro and in vivo. Conclusions: OPN contributes to acquired EGFR-TKI resistance by up-regulating expression of integrin αVβ3, which activates the downstream FAK/AKT and ERK signalling pathways to promote cell proliferation in NSCLC.
