151. Clinical value of the determination of serum guanase activity. Studies on patients and experimental data from mongrel dogs and cultured rat hepatocytes

Abstract

Serum guanase activity was measured by a new method and compared with serum glutamic oxalacetic transaminase and glutamic pyruvic transaminase levels in 150 patients with various disorders, 21 dogs with experimental myocardial infarction, and 2 CCl4-treated dogs. Additionally, studies of the effect of CCl4 on enzyme release were undertaken using cultured rat hepatocytes. Glutamic oxalacetic transaminase, glutamic pyruvic transaminase, and guanase activities were found to be significantly elevated in patients with various liver disorders, those with acute myocardial infarction with prominent congestion of the liver, and also in CCl4-treated dogs. However, serum guanase activity was normal in patients with a variety of non-liver-related diseases including acute myocardial infarction, and in dogs with experimental myocardial infarction without liver damage, even when the serum glutamic oxalacetic transaminase and glutamic pyruvic transaminase activities were increased. The glutamic oxalacetic transaminase, glutamic pyruvic transaminase, and guanase activities in the culture medium of rat hepatocytes indicated in the presence of 0.5 mM CCl4 were elevated. These findings indicate that serum guanase activity is a more specific indicator of liver damage than serum glutamic oxalacetic transaminase and glutamic pyruvic transaminase.

Abstract

A 50-year-old man was admitted to our hospital because of chest pain. Twenty-four-hour ECG recording demonstrated ST-segment depression and elevation at the time of spontaneous angina. During treadmill exercise test, the patient developed chest pain with ST-segment depression in leads V4 to V6. After the administration of nifedipine (10 mg), the patient was able to reach up to the maximum predicted heart rate without anginal symptoms and ST-T changes. Coronary arteriogram demonstrated 50% stenosis at the proximal portion of the left anterior descending artery (LAD) and two small fistulas originated from LAD to pulmonary artery. Spasm was induced at the proximal portion of LAD by the hyperventilation. If patients with coronary arteriovenous fistula (CAVF) have symptoms, elective CAVF ligation has been recommended. However, this case suggests that coronary spasm could be one of the cause of angina pectoris in patients with CAVF. Elective CAVF ligation must be carefully indicated in CAVF patients with angina pectoris.

Abstract

The isomagnetic maps of normal subjects and patients with right and left atrial overloading were recorded to determine the characteristic features of the magnetic field of atrial depolarization. The isomagnetic maps examined in this study indicated the instantaneous current source, which specifically localizes the current sources due to the right and left atria, respectively. The magnetic field recorded with a second derivative gradiometer clearly detected the cardiac current source from the right atrium, which is located close to the anterior chest wall, thus this method improved the diagnostic sensitivity for right atrial overloading. In patients with left atrial overloading, the isomagnetic map showed multiple dipoles due to the right and left atria, respectively, which are difficult to be detected by the electrocardiogram or isopotential map. These results suggest that the magnetocardiogram provides useful information on the current source to supplement information obtained by the conventional electrocardiogram.

Abstract

Polarcardiograms using the posteroanterior axis as the polar axis were recorded in 112 cases of the clinically healthy subjects. QRS wave with small notch or slur both in front and behind of the main triangular configuration with narrow base were most frequently observed in the frontal magnitude ECG. T wave showed simple monophasic upright wave with the mildly sloping ascending limb and the relatively rapidly sloping descending limb. The magnitude of the maximum QRS vector and the maximum QRS vector time in frontal plane were very similar with those of the spatial QRS vector. The quantitative values obtained by the measurement of the polarcardiograms agreed well with those obtained by the vectorcardiograms reported by the various authors. Polarcardiography made possible not only to know the polar coordinates of the cardiac vector at any instant very easily, but also to express the time-related curve of the polar coordinates, which vary as a function of time, in easily understandable analogue pattern. QRS and T waves of the magnitude, the alpha-longitude and P-A latitude ECG in the normal subjects were classified into the several definite patterns.

Abstract

The composition dependence of gravitational constant G is measured at the millimeter scale to test the weak equivalence principle, which may be violated at short range through new Yukawa interactions such as the dilaton exchange force. A torsion balance on a turning table with two identical tungsten targets surrounded by two different attractor materials (copper and aluminum) is used to measure gravitational torque by means of digital measurements of a position sensor. Values of the ratios and were and , respectively; these were obtained at a center to center separation of 1.7 cm and surface to surface separation of 4.5 mm between target and attractor, which is consistent with the universality of G. A weak equivalence principle (WEP) violation parameter of at the shortest range of around 1 cm was also obtained. [ABSTRACT FROM AUTHOR]

Abstract

We report a measurement of the $\nu_\mu$ inclusive charged current cross sections on iron and hydrocarbon in the T2K on-axis neutrino beam. The measured inclusive charged current cross sections on iron and hydrocarbon averaged over the T2K on-axis flux with a mean neutrino energy of 1.51 GeV are $(1.444\pm0.002(stat.)_{-0.157}^{+0.189}(syst.))\times 10^{-38}\mathrm{cm}^2/\mathrm{nucleon}$, and $(1.379\pm0.009(stat.)_{-0.147}^{+0.178}(syst.))\times 10^{-38}\mathrm{cm}^2/\mathrm{nucleon}$, respectively, and their cross section ratio is $1.047\pm0.007(stat.)\pm0.035(syst.)$. Abstract

We report the first measurement of the neutrino-oxygen neutral-current quasielastic (NCQE) cross section. It is obtained by observing nuclear deexcitation $\gamma$-rays which follow neutrino-oxygen interactions at the Super-Kamiokande water Cherenkov detector. We use T2K data corresponding to $3.01 \times 10^{20}$ protons on target. By selecting only events during the T2K beam window and with well-reconstructed vertices in the fiducial volume, the large background rate from natural radioactivity is dramatically reduced. We observe 43 events in the $4-30$ MeV reconstructed energy window, compared with an expectation of 51.0, which includes an estimated 16.2 background events. The background is primarily nonquasielastic neutral-current interactions and has only 1.2 events from natural radioactivity. The flux-averaged NCQE cross section we measure is $1.55 \times 10^{-38}$ cm$^2$ with a 68\% confidence interval of $(1.22, 2.20) \times 10^{-38}$ cm$^2$ at a median neutrino energy of 630 MeV, compared with the theoretical prediction of $2.01 \times 10^{-38}$ cm$^2$., Comment: 10 pages, 6 figures

Abstract

New data from the T2K neutrino oscillation experiment produce the most precise measurement of the neutrino mixing parameter theta_{23}. Using an off-axis neutrino beam with a peak energy of 0.6 GeV and a data set corresponding to 6.57 x 10^{20} protons on target, T2K has fit the energy-dependent nu_mu oscillation probability to determine oscillation parameters. Marginalizing over the values of other oscillation parameters yields sin^2 (theta_{23}) = 0.514 +0.055/-0.056 (0.511 +- 0.055), assuming normal (inverted) mass hierarchy. The best-fit mass-squared splitting for normal hierarchy is Delta m^2_{32} = (2.51 +- 0.10) x 10^{-3} eV^2/c^4 (inverted hierarchy: Delta m^2_{13} = (2.48 +- 0.10) x 10^{-3} eV^2/c^4). Adding a model of multinucleon interactions that affect neutrino energy reconstruction is found to produce only small biases in neutrino oscillation parameter extraction at current levels of statistical uncertainty., Comment: 8 pages, 4 figures. 183. Adult Case of Pontocerebellar Hypoplasia without the Claustrum.

Abstract

We describe the case of a 63-year-old man with pontocerebellar hypoplasia without the claustrum (CL). The patient had a history of cerebral palsy, intelligent disability, cerebellar atrophy, and seizures since birth. At age 61, brain computed tomography (CT) revealed significant cerebellar and brainstem atrophy. At age 63, he was admitted to our hospital for aspiration pneumonia. Although he was treated with medications, including antibiotics, he died one month after admission. The autopsy revealed a total brain weight of 815 g, with the small-sized frontal lobe, cerebellum, and pons. The cross-section of the fourth ventricle had a slit-like appearance, rather than the typical diamond shape. In addition, bilateral CLs were not observed. Apart from CL, no other missing brain tissue or cells could be identified. Microscopic examinations disclosed neurofibrillary tangles in the hippocampus but not in the cortex; however, neither senile plaques nor Lewy bodies were detected. No acquired lesions, including cerebral infarction, hemorrhage, or necrosis, were noted. We pathologically diagnosed the patient with pontocerebellar hypoplasia without CL. As there have been no prior reports of pontocerebellar hypoplasia lacking CL in adults, this case may represent a new subtype. Congenital CL deficiency is likely associated with abnormalities in brain development. CL may play a role in seizure activity, and the loss of bilateral CLs does not necessarily result in immediate death. Further studies are needed to clarify the functions of CL.

184. Polymyalgia Rheumatica Following Influenza B Infection: A Case Report.

Abstract

We describe a case of polymyalgia rheumatica (PMR) following an influenza B infection. The patient was a 71-year-old woman who developed a fever above 38.0°C and was diagnosed with influenza B, confirmed by a rapid antigen test. Although the fever resolved after three days, she continued to experience neck pain, back pain, and joint pain, particularly in both shoulders and hips. She also reported morning joint stiffness lasting for more than an hour and occasional low-grade fever. She presented to our hospital 25 days after the onset of symptoms. Blood tests revealed elevated C-reactive protein and erythrocyte sedimentation rate, but levels of creatine phosphokinase, rheumatoid factor, and anti-cyclic citrullinated peptide were not elevated. She was diagnosed with PMR and treated with prednisolone (PSL) 15 mg/day. The response to steroids was remarkably good, and PSL was tapered over six months. PMR is believed to result from an immune-mediated process and may be associated with certain human leukocyte antigen haplotypes. Additionally, PMR is sometimes preceded by an infection. 185. Haploidentical transplantation with post-transplant cyclophosphamide versus single cord blood transplantation in adults with relapsed/refractory non-Hodgkin lymphoma.

Abstract

Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a curative treatment for relapsed or refractory non-Hodgkin lymphoma (R/R NHL). Allo-HSCT using post-transplant cyclophosphamide (PTCY-haplo) and umbilical cord blood transplantation (uCBT) are important donor options in the absence of matched related siblings. However, the data comparing these two donor sources in R/R NHL are limited. Using the Japanese nationwide transplantation registry data, we identified 857 patients with R/R NHL, including 169 patients who received PTCY-haplo and 688 who received uCBT for their first allo-HSCT between January 2013 and December 2021; 514 patients (60%) had B-cell lymphoma. More PTCY-haplo recipients received allo-HSCT using a reduced-intensity conditioning regimen in recent years. The 3-year overall survival (OS), progression-free survival (PFS), and graft-versus-host disease (GVHD)-free/relapse-free survival (GRFS) rates in the PTCY-haplo and uCBT groups were 44% versus 39% (P = 0.326), 34% versus 33% (P = 0.660), and 19% versus 23% (P = 0.910), respectively; the adjusted hazard ratios for OS, PFS, and GRFS were 0.89 (95% confidence interval: 0.69-1.15, P = 0.373), 0.98 (0.78-1.22, P = 0.852), and 0.92 (0.83-1.21, P = 0.920), respectively. The PTCY-haplo group showed faster neutrophil and platelet engraftment and a lower incidence of grade III-IV acute GVHD. 186. Cytotoxic Lesions of the Corpus Callosum (CLOCCs) Related to Hypoglycemia: Clinical Insights and Pathogenesis.

Abstract

We describe the case of an 88-year-old man with cytotoxic lesions of the corpus callosum (CLOCCs) related to hypoglycemia. The patient developed a disturbance of consciousness following excessive alcohol consumption and anorexia. In the emergency room, his blood sugar level was 9 mg/dL, and he was immediately treated with a rapid infusion of glucose. Brain magnetic resonance imaging (MRI) revealed hyperintensity in the corpus callosum and bilateral deep white matter on diffusion-weighted imaging, along with decreased apparent diffusion coefficient values. The following day, these findings were attenuated, and he was diagnosed with CLOCCs. 187. Migraine With Aura Accompanied by Myoclonus: A Case Report.

Abstract

Migraine is a condition characterized by pulsating headaches, often accompanied by photophobia, phonophobia, and/or gastrointestinal symptoms such as nausea and vomiting. Approximately 15% to one-third of migraine patients experience an aura either before or during the headache. To the best of our knowledge, the occurrence of migraine with myoclonus is extremely rare. This report describes a rare case of migraine with aura accompanied by myoclonus. The patient is a 46-year-old man who developed a visual aura followed by vomiting and a throbbing headache on the right side. As the headache intensified, involuntary movements of the left lower extremity appeared. Brain magnetic resonance imaging (MRI) revealed no structural abnormalities or stroke lesions; however, arterial spin labeling MRI showed hypoperfusion in the right cerebral hemisphere. An ophthalmological evaluation was unremarkable. He was diagnosed with migraine with myoclonus, and the intravenous administration of diazepam and sumatriptan resulted in the cessation of the myoclonus and mild relief of the headache. By the day after admission, the myoclonus and visual symptoms had completely disappeared. 188. Rumination Swallowing in Limb-Girdle Muscular Dystrophy.

Abstract

Patients with limb-girdle muscular dystrophy (LGMD) may develop swallowing disorders as the disease progresses. We detected a novel swallowing pattern in patients with LGMD, characterized by unique findings such as a rumination-like behavior, where food reaches the vallecula and piriform sinuses and then is regurgitated back into the oral cavity, re-chewed, and swallowed again. We termed this swallowing pattern "rumination swallowing." Abstract

Robot-assisted rehabilitation is becoming an important option in rehabilitation medicine. We utilized one such device, the elbow joint hybrid assistive limb (HAL), for a patient with Guillain-Barré syndrome (GBS). The patient was a 64-year-old man, 16 months post-onset of GBS. Due to severe neuropathy, he was completely dependent on others and unable to eat independently. After approximately two weeks of intensive rehabilitation and self-training following his hospital discharge, he regained the ability to feed himself. 190. Verbal-Cue Rehabilitation Exercises Are Effective for the Mixed Callosal-Frontal Variant of Alien Hand Syndrome Following Stroke.

Abstract

Alien hand syndrome (AHS) is a rare condition in which a brain-injured patient develops involuntary movements, particularly in the upper limbs, along with difficulty in controlling them and a loss of ownership of the affected hand. AHS is traditionally classified into frontal, callosal, and occipital types. Recently, mixed types have also been reported. In this report, we describe a case of mixed callosal-frontal type AHS following stroke, in which symptoms improved with a verbal-cue rehabilitation exercise. The patient was a 62-year-old woman diagnosed with cerebral infarction in the right frontal lobe and corpus callosum due to disruption of the right anterior cerebral artery. She exhibited left hemiplegia, left deep tendon hyperreflexia, and AHS in the left upper extremity. She received verbal-cue rehabilitation exercises for AHS in addition to regular rehabilitation therapy and medications. Abstract

Context: Patients with cervical compressive myelopathy (CCM) often complain of body balance problems, such as fear of falling and bodily unsteadiness. However, no accepted patient-reported outcome measures (PROMs) for this symptomatology exist. The Falls Efficacy Scale-International (FES-I) is one of the most widely used PROMs for evaluating impaired body balance in various clinical fields., Objective: To examine reliability, validity, and minimum clinically important difference (MCID) of the FES-I for the evaluation of impaired body balance in patients with CCM., Methods: Patients who underwent surgery for CCM were retrospectively reviewed. The FES-I was administered preoperatively and at 1 year postoperatively. Further, cJOA-LE score (subscore for lower extremities in the Japanese Orthopaedic Association score for cervical myelopathy) and stabilometric data, obtained at the same time points of the FES-I administration, were analyzed. Reliability was examined through internal consistency with Cronbach's alpha. Convergent validity was studied using correlation analysis. The MCID was estimated using anchor- and distribution-based methods., Results: Overall, 151 patients were included for analysis. Cronbach's alpha coefficient was the acceptable value of 0.97 at both baseline and 1 year postoperatively. As for convergent validity, the FES-I had significant correlations with the cJOA-LE score and stabilometric parameters both at baseline and 1 year postoperatively. The MCID calculated using anchor- and distribution-based methods was 5.5 and 10, respectively., Conclusion: FES-I is a reliable and valid PROM to evaluate body balance problems for the CCM population. The established thresholds of MCID can help clinicians recognize the clinical significance of changes in patient status.

Abstract

Fatty acid synthase (FASN) catalyzes the rate-limiting step of cellular lipogenesis. FASN expression is upregulated in various types of cancer cells, implying that FASN is a potential target for cancer therapy. 2-Deoxy-D-glucose (2-DG) specifically targets cancer cells by inhibiting glycolysis and glucose metabolism, resulting in multiple anticancer effects. However, whether the effects of 2-DG involve lipogenic metabolism remains to be elucidated. We investigated the effect of 2-DG administration on FASN expression in HeLa human cervical cancer cells. 2-DG treatment for 24 h decreased FASN mRNA and protein levels and suppressed the activity of an exogenous rat Fasn promoter. The use of a chemical activator or inhibitors or of a mammalian expression plasmid showed that neither AMPK nor the Sp1 transcription factor is responsible for the inhibitory effect of 2-DG on FASN expression. Administration of thapsigargin, an endoplasmic reticulum (ER) stress inducer, or 4-(2-aminoethyl) benzenesulfonyl fluoride (AEBSF), a site 1 protease inhibitor, mimicked the inhibitory effect of 2-DG on FASN expression. 2-DG did not further decrease FASN expression in the presence of thapsigargin or AEBSF. Site 1 protease mediates activation of ATF6, an ER stress mediator, as well as sterol regulatory element-binding protein 1 (SREBP1), a robust transcription factor for FASN. Administration of 2-DG or thapsigargin for 24 h suppressed activation of ATF6 and SREBP1, as did AEBSF. We speculated that these effects of 2-DG or thapsigargin are due to feedback inhibition via increased GRP78 expression following ER stress. Supporting this, exogenous overexpression of GRP78 in HeLa cells suppressed SREBP1 activation and Fasn promoter activity. These results suggest that 2-DG suppresses FASN expression via an ER stress-dependent pathway, providing new insight into the molecular basis of FASN regulation in cancer., (© 2024. 193. Influence of cranium orientation on cervical sagittal alignment during radiographic examination: a radiographic analysis.

Background Context: During the radiographic examination, the cranium orientation varies not only individually but also within the same subject, in different imaging sessions. Knowing how changes in the orientation of the cranium influences cervical sagittal alignment during the radiographic examination of the cervical spine can aid clinicians in the accurate evaluation for cervical sagittal alignment in clinical practice., Purpose: To radiographically examine the influence of cranium orientation on cervical sagittal alignment during radiographic examination in an asymptomatic cohort., Study Design: A prospective radiographic study., Patient Sample: Eighty asymptomatic volunteers (mean age, 40.4 years; 50.0% male) were enrolled., Outcome Measures: Cervical sagittal parameters including the regional slope (C1 slope, C2 slope, C5 slope, C7 slope, and T1 slope), Cobb angle (O-C1 angle, C1-C2 angle, C2-C5 angle, C5-C7 angle, and C7-T1 angle), and cranial/cervical offset (sella turcica tilt [ST tilt] and C2 tilt)., Methods: In all participants, standing lateral radiographs of the cervical spine were taken in 3 forward-gazing positions: anteverted-cranium (AC) position; neutral-cranium (NC) position; and retroverted-cranium (RC) position. Cervical sagittal parameters, including the regional slope, Cobb angle, and cranial/cervical offset, in these 3 positions were statistically compared., Results: The C1 and C2 slopes were anteverted and retroverted in the AC and RC positions, respectively, compared to those in the NC position. The C5 slope, C7 slope, and T1 slope were constant among the 3 positions. In O-C2 and C2-C5, statistically significant differences in the regional Cobb angles were identified among the 3 positions; however, there were no significant differences in the C5-C7 or C7-T1 segments. Cranial and cervical offsets of ST tilt and C2 tilt increased and decreased when the cranium was anteverted and retroverted, respectively., Conclusions: The current study suggests that the adjustment of the cranium orientation when taking cervical spine radiographs is mainly controlled at the upper cervical spine of the O-C2 segment in an asymptomatic cohort. On radiograph, alignment in the upper cervical segment of O-C2 changes; accordingly, the middle cervical segment of C2-C5 can change during the adjustment of cranium orientation. However, alignment in the lower cervical segment of C5-C7 and the cervicothoracic junction of C7-T1 remains constant. Further, cranial/cervical offset increases and decreases when the cranium is anteverted and retroverted, respectively. 194. Supernumerary Phantom Limb After Stroke in the Left Hemisphere: A Case Report.

Abstract

Supernumerary phantom limb (SPL) is a rare sensation of additional limbs that are perceived to exist alongside existing limbs. SPL can occur in various neural regions, but it is rare for SPL to be caused by left hemisphere cerebral infarction. In this report, we describe a case of a 64-year-old woman with SPL after a stroke. She had a history of handedness change. A neurological examination revealed that she had right hemiplegia, unilateral spatial neglect (USN), anosognosia, and pan-sensory loss on the right side of her body. Brain magnetic resonance (MR) imaging disclosed cerebral infarction in the left corona radiata region. She suffered from SPL in the right upper limb. Abstract

ABSTRACT In wastewater treatment by the activated sludge (AS) process, settleability and dewaterability of AS are key issues that are directly related to the treated water quality and sludge treatment costs. Several studies investigated the relationship between the shape of AS flocs and their settling/dewatering property. To quantify the floc morphology, it is imperative to attach a camera to a microscope or move the stage manually. Hence, labour and equipment costs may increase. In this study, by combining a digital microscope and an automatic stage, more than 100 magnified floc images were rapidly obtained from one AS sample dropped on a slide glass, and shape parameters were collectively calculated using an analysis software. During 1-year monitoring of four wastewater treatment plants in Sapporo City (Hokkaido, Japan), the morphological parameters and extracellular polymeric substances (EPS) quantity/quality of AS were analyzed on the basis of their correlation to the time to filtration (TTF) and sludge volume index (SVI), which are indicators for describing the dewatering and settling properties of AS, respectively. In one plant, larger, denser, and smoother flocs tended to contain less EPS and exhibited better sludge dewaterability. In another plant, larger, denser, and smoother flocs were considered to contribute to better settlement. Especially, an equivalent high-density floc diameter and the ratio of mixed liquor suspended solids (MLSS) concentration to the total floc area were commonly suggested to explain AS dewaterability and settleability.

196. High-entropy intermetallics: emerging inorganic materials for designing high-performance catalysts.

Abstract

Alloy materials have been used as promising platforms to upgrade catalytic performance that cannot be achieved with conventional monometallic materials. As a result of numerous efforts, the recent progress in the field of alloy catalysis has been remarkable, and a wide range of new advanced alloys have been considered as potential electro/thermal catalysts. Among advanced alloy materials, high-entropy intermetallics are novel materials, and their excellent catalytic performance has recently been reported. High-entropy intermetallics have several advantages over disordered solid-solution high-entropy alloys, that is, greater structural/thermal stability, more facile site isolation, more precise control of electronic structures, tunability, and multifunctionality. A multidimensional compositional space is indeed limitless, but such a compositional space also provides a well-designed surface configuration because of its ordered nature. In this review, we will provide fundamental insights into high-entropy intermetallics, including thermodynamic properties, synthesis requirements, characterization techniques, roles in catalysis, and reaction examples. 197. Establishment of a hepatitis B virus reporter system harboring a HiBiT-tag in the PreS2 region.

Abstract

Background: Approximately 296 million people suffer from chronic hepatitis B (CHB) caused by hepatitis B virus (HBV). Current standard treatment, nucleos(t)ide analogs, are not efficient enough to eradicate HBV from the hepatocytes. Thus, developing new drugs for CHB is desired to achieve complete cure., Methods: Here we established a novel HBV reporter system, HBV-HiBiT-PS2, to screen new drugs for CHB. HBV-HiBiT-PS2 was constructed by introducing a HiBiT-tag at the 5'-end of PreS2 and introduced into HepG2-NTCP cells. Culture supernatant containing HBV-HiBiT-PS2 virions was fractionated by a sucrose density gradient ultracentrifugation to characterize their components. Replication kinetics and reporter function of HBV-HiBiT-PS2 were determined by analyzing the parameters for HBV replication in the presence or absence of HBV inhibitors., Results: HBV-HiBiT-PS2 could be used for monitoring most of the replication cycle of HBV. The effects of well-characterized HBV inhibitors could be evaluated by the HiBiT activity. HBV-HiBiT-PS2 could be specialized for screening secretion inhibitors for hepatitis B surface antigen (HBsAg) because most of the HiBiT activity was derived from subviral particles which are the multimers of HBsAg., Conclusions: We demonstrated that HBV-HiBiT-PS2 would be a robust tool for screening novel drugs, especially HBsAg secretion inhibitors, targeted for CHB., (© The Author(s) 2024. Abstract

Kyphotic deformity following the loss of cervical lordosis can lead to unfavourable neurological recovery after cervical laminoplasty (CLP); therefore, it is essential to identify its risk factors. Recent studies have demonstrated that the dynamic parameters of the cervical spine, based on baseline flexion/extension radiographs, are highly useful to estimate the loss of cervical lordosis after CLP. However, it remains unclear whether such dynamic parameters can predict kyphotic deformity development after CLP. Hence, the present study aimed to investigate whether the dynamic parameters could predict kyphotic deformity in patients with cervical spondylotic myelopathy (CSM) after CLP. This retrospective study included 165 patients, consisting of 10 and 155 patients with and without cervical kyphosis of C2-C7 angle ≤ -10° at the final follow-up period, respectively. Among the static and dynamic parameters of the cervical spine, greater cervical kyphosis during flexion (fC2-C7 angle) demonstrated the best discrimination between these two cohorts, with an optimal cutoff value of -27.5°. Meanwhile, greater gap range of motion (gROM = flexion ROM - extension ROM ) had the highest ability to predict a loss of ≥ 10° in C2-C7 angle after CLP, with an optimal cutoff value of 28.5°. This study suggests that in patients with CSM, fC2-C7 angle ≤ -25° on baseline radiographs is a potential risk for kyphotic deformity after CLP. Abstract

In this study, we aimed to establish high-rate biological treatment of purified terephthalic acid (PTA) and dimethyl terephthalate (DMT) wastewater that minimizes the inhibitory effects of high concentration benzoate and acetate. To achieve this, we developed a novel bioreactor system and biostimulation strategy. An internal two-stage upflow anaerobic (ITUA) reactor was operated with (i) a packed bed containing green tuff medium underlying (ii) a compartment seeded with anaerobic granular sludge. Ethylene glycol was amended to stimulate syntrophic interactions. Continuous operation of the system for 1,026 days achieve an organic removal rate of 11.0 ± 0.6 kg COD/m 3 /d. The abundance of aromatic degraders significantly increased during operation. Abstract

An 80-year-old woman with a history of rheumatoid arthritis, hypertension, and hyperlipidemia, and no family history of hyperkinesis developed suddenly involuntary movement and visited our hospital two-day after onset. Neuro-examination revealed hemichorea-hemiballismus in the right side of the body, including the face (Suppl. video). Blood tests revealed neither hyperglycemia nor acanthocyte. Brain MRI showed acute microbleeding in the left subthalamic nucleus (Figure 1A-C). Although she was treated with haloperidol (max. 4.5 mg/day), hemichorea-hemiballismus did not subside. Repetitive transcranial magnetic stimulation (rTMS) with a low-frequency protocol (LFP) (1 Hz, 1200 pulses, with a stimulus intensity of 90% of the resting motor threshold, 3 days/week for 2-week) was applied to the left precentral knob (Figure 1D). Its effect was drastic, as the symptoms disappeared for half-hour after rTMS. Hemichorea-hemiballism then reappeared but was attenuated by repeated rTMS. The symptoms disappeared after one-month. Subthalamic nucleus lesions can develop hemichorea-hemiballism (1). According to a study on Huntington's disease and diabetic hemichorea-hemiballism, increased thalamocortical drive may increase the excitability of excitatory and inhibitory circuits of the frontal cortex as the etiologies of hyperkinesia (2-3). However, the target points of rTMS in treating hemichorea-hemiballism have not been consistent in literature. Moreover, rTMS with a LFP on the bilateral supplementary motor areas is effective in treating chorea in Huntington's disease (2). Additionally, rTMS on the ipsilateral precentral knob (primary motor cortex) with continuous θ burst stimulation (cTBS), which decreases the excitability and inhibitory cortical circuits, was effective in treating contralateral hemichorea caused by midbrain hemorrhage (3). Similar to cTBS, LFP can suppress cortical excitation (4); therefore, we applied rTMS with LFP on the primary motor cortex to treat hemichorea-hemiballism. Our results were drastic for both short- and long-term efficiency. This is the first report of the efficacy of rTMS with LFP in treating hemichorea-hemiballism caused by encephalorrhagia.
