Abstract

New mothers who want to nurse their babies should avoid taking the antidepressant drug fluoxetine. A 32-year-old woman with manic-depression took fluoxetine, carbamazepine and buspirone throughout her pregnancy. When the child was born, her doctor advised her not to breastfeed her baby. She began breastfeeding while continuing to take the drugs. The child experienced several seizures according to the mother, but neurologic evaluations were normal. Blood samples from mother and baby as well as breast milk samples were analyzed and revealed significant amounts of fluoxetine.

Abstract

To assess the feasibility of comparing the rates of positive depression screens at 6 weeks and 3 months postpartum in women using immediate postpartum etonogestrel implant (ENG-implant) and women using non-hormonal contraception or sterilisation. This was a pilot prospective cohort study performed to test the design adequacy of comparing the rates of positive postpartum PHQ-9 screens (≥10) in women using immediate postpartum ENG-implant and women using non-hormonal contraception or sterilisation. Participants were recruited during the third trimester of pregnancy or during delivery hospitalisation. They self-allocated to one of the two comparison groups. PHQ-9 surveys were administered during the third trimester of pregnancy, immediately postpartum, and at 6 weeks and 3 months postpartum. Between June 2017 and March 2018, 91 patients were recruited. Of these patients, 11 were excluded and the remaining 80 were split evenly into each cohort. The women in the ENG-implant group were younger, less educated, and more often publicly insured. The percentage of participants with positive PHQ-9 screens were: 3% during the postpartum hospitalisation, 6.2% at 6 weeks postpartum, and 10.2% at 3 months postpartum. PHQ-9 scores were similar between groups at both postpartum time points. The rates of positive PHQ-9 screens at 6 weeks postpartum were similar between groups. These preliminary data suggest that immediate postpartum placement of the ENG-implant does not negatively impact the risk for a positive depression screen. 199. Serum nortriptyline levels in nursing mothers and their infants

Abstract

About 10 to 15 percent of postpartum women suffer from depression. However, there have been few reports in the literature of drugs evaluated for treating postpartum depression. One potential complication is that the infant will be adversely affected by the drug, if the mother is breast feeding her child. Also, the mother may not absorb the drug as she normally would because of the physiological changes in her body due to childbirth and nursing. In the present paper, seven postpartum women suffering from depression were treated with nortriptyline, an antidepressant medication. The healthy infants were aged six months or less. After a minimum of 15 days at a stable dose of the drug, blood samples were taken from mothers within three hours of nursing their infants; blood samples were also taken from the infants at this time. Even though the blood levels in the mothers were clinically effective, no infant had a detectible level of nortriptyline in his blood. This suggests that nortriptyline was not passed to the baby in breast feeding. In the blood of two of the youngest infants, a less active byproduct of nortriptyline, 10-hydroxynortriptyline, was found. However, no adverse effects in any infant were observed.
