Samples of Barrett metaplastic specialized epithelium (SE), low-grade dysplasia (LGD), high-grade dysplasia (HGD), and invasive adenocarcinoma (CA) derived from 36 esophagectomy specimens were studied for loss of heterozygosity (LOH) in APC and MCC and for expression of APC protein. Of 18 cases that were heterozygous (informative) for APC, LOH was found in none of 14 SE samples, 2 of 8 LGD samples, 3 of 11 HGD samples, and 5 of 17 CA samples. Immunohistochemically, markedly reduced expression of APC protein (< 50% positive cells) was found in 3 of 19 HGD samples and 4 of 35 CA samples but not in SE or LGD samples. Of 17 cases informative for the MCC gene, LOH was detectable in 1 of 14 SE samples, none of 7 LGD samples, none of 9 HGD samples, and 4 of 16 CA samples. Allelic loss of APC and/or loss of APC protein expression occurs earlier in the metaplasia-dysplasia-carcinoma sequence in Barrett esophagus than LOH in the MCC gene. The determination of alterations at APC or MCC would be of limited importance for the surveillance of patients with Barrett esophagus.

Zusammenfassung: Mit den Fortschritten in der Computertechnologie und der neuroradiologischen Bildgebung wurde die Entwicklung der Neuronavigation möglich. Diese wird seither immer häufiger angewendet und hat die rahmenbasierte Stereotaxie zur intraoperativen Führung bei mikrochirurgischen, intrakraniellen Operationen weitgehend abgelöst. Mit Hilfe von dreidimensionalen Digitalisierinstrumenten werden dabei die präoperativ angefertigten CT- oder MRT- Bilder eines Patienten währen der Operation auf den Operationssitus übertragen. So wird eine genaue Lokalisation von Läsionen wie Tumoren oder anatomischen Strukturen ermöglicht. Die verwendeten Techniken wie positionsfühlende Gelenkarme, Infrarot- oder Robotersysteme werden ebenso wie die Anwendung in der klinischen Routine dargestellt. In 200 Eingriffen, die mit Neuronavigation durchgeführt wurden, hat sich die Methode als sehr hilfreich zur minimal invasiven Eingriffsplanung und -durchführung erwiesen. Mit Abweichungen zwischen 2 und 4 mm ist die Genauigkeit nur geringfügig schlechter als die Werte, wie sie für die rahmenbasierte Stereotaxie angegeben werden. Als besonders vorteilhaft wurde die Navigation von den Operateuren bei Eingriffen an der Schädelbasis sowie kleinen, tiefliegenden oder in eloquenten Arealen gelegenen Läsionen eingestuft. Hauptnachteil der Methode ist die Verwendung von präoperativem Bildmaterial, was zu Ungenauigkeiten führt, wenn es im Verlauf einer Operation durch zunehmende Tumorresektion zu Veränderungen der Anatomie kommt. Hier könnten Verfahren der intraoperativen Bildgebung wie der MRT eine Lösung darstellen. So ist es uns bereits gelungen, durch Aktualisierung der Navigationsdaten mit intraoperativer MRT Verschiebungen auszugleichen. Diese Entwicklungen werden zu einer zunehmenden Bedeutung der Neuronavigation in der operativen Routine führen, was durch eine Senkung der Operationsmorbidität auch erhebliche Auswirkungen auf die Indikationsstellung zu neurochirurgischen Eingriffen haben wird.

The effect of thiazide and loop diuretics on the tubular uptake of [3H]para-aminohippurate ([3H]PAH), a marker of the organic anion transport system, was tested in vitro on single S2 segments of the proximal tubule of rabbit kidneys. Because the tubules were not perfused, and hence were collapsed, the tubular [3H] PAH uptake reflects [3H]PAH transport across the contraluminal membrane. The diuretics were added to the bath solution in at least six different concentrations. The concentration of [3H]PAH in the bath solution amounted to 1.3 mumol/l, and was 12 times lower than the PAH concentration (16 mumol/l) at half-maximum PAH uptake. The results show that all of the nine diuretics tested inhibited [3H]PAH uptake. The IC50 values of the individual drugs ranged from 3 to 400 mumol/l. In general, thiazide diuretics had a lower affinity to the PAH transporter than loop diuretics. This probably results from differences in the pKa values which range from 3.6 to 4.1 for the loop diuretics and from 9.2 to 10 for the thiazide diuretics. Furthermore, there was a highly significant dependence of affinity on the hydrophobic properties of the diuretics. At concentrations 10- to 100-fold lower than the IC50 values all diuretics stimulated [3H]PAH uptake which may indicate a positive cooperative interaction of these drugs with the PAH transporter. Within the group of loop diuretics the order of the affinity to the PAH transporter and to the Na, 2 Cl, K cotransporter in the thick ascending limb of Henle's loop was identical.(ABSTRACT TRUNCATED AT 250 WORDS)
