Abstract

The association between scandium status and risk of acute myocardial infarction (MI) was examined in a multicentre case control study in 10 centres from Europe and Israel. Scandium in toenails was assessed in 684 cases and 724 controls less than 70 years of age. Mean concentrations of toenail scandium were 6.74 μg/kg in cases and 7.75 μg/kg in controls. Scandium among controls, adjusted for age and centre was positively associated with concentrations of lycopene and oleic acid in adipose tissue (P = 0.002 for both nutrients). Pearson correlations adjusted for age and centre were significant (P < 0.05) between scandium and lycopene (r = 0.08), zinc (r = 0.08), mercury (r = 0.18) and oleic acid (r = 0.21). Overall, cases had lower levels of scandium than controls after adjustment for age and centre (case control ratio, 0.87; 95% CI 0.79-0.96). This association persisted after adjustment for other cardiovascular risk factors (case-control ratio 0.88; 95% CI, 0.79-0.98). The risk of MI at high scandium levels was reduced after adjustment for age and centre (P-trend = 0.04). Further adjustments for BMI, history of hypertension, smoking, alcohol intake, diabetes, family history of CHD, alpha-tocopherol, beta-carotene, lycopene, selenium and mercury slightly attenuated this trend (P = 0.055). Our results suggest that toenail scandium level is associated with a reduced risk of acute MI, but we are uncertain whether this element can really play a protective role in the development of CHD. Without an identified plausible mechanism, these results should be regarded as preliminary and should be tested in future studies.

Abstract

We report on a study of tinnitus in more than 400 patients 20 to 80 years old who had suffered from tinnitus for more than one year. Quality of tinnitus, localization, frequency range, severity, masking and residual inhibition effects show no age dependence. The possible causes of tinnitus, is onset and duration and factors that increase or reduce tinnitus also show no age dependence. Regarding the grade of annoyance judged from a subjective scaling, younger patients seen to suffer more from tinnitus than do older subjects, who seem to compensate their tinnitus more often. With regard to the different therapeutic methods used in this study (transtympanal electrostimulation, iontophoresis, biofeedback, tinnitus masker, hearing aid), the stable benefit over more than one year experienced by those patients fitted with a hearing aid was encouraging for young and old subjects. In both groups, about 3% of the patients could be successfully equipped with a tinnitus masker. Independent of age, the improvement by the other methods is less than 1%. Our results show that sufficient prognostic data on the efficiency of a therapeutic method cannot be perceived from the different tinnitus measurements for either the young or the old subjects.

Abstract

Transition and gap detection experiments with synthetic speech stimuli in normal and pathological hearing were carried out to investigate the feasibility of some form of feature-analysing mechanism in speech perception. Identification experiments were performed before the hearing aid fitting, directly thereafter and half a year later to evaluate the important individual cues of speech perception with regard to perceptual training. Possible distortions in the temporal structure of the speech signals by the hearing aids were considered. For our experiments we generated a series of synthetic speech stimuli such as the ba-da-ga sequence, which are identical in their acoustic cues except for the transition of the second formant. The start frequency was changed in nine 200-Hz steps from 800 Hz to 2,400 Hz thus producing a ba at one end of the series and a ga at the other. The phenomenon of categorical perception could be investigated with this set of synthesized speech stimuli. In a second experiment the influence of silent intervals on the identification of plosives was analysed increasing the artificial silent interval in 10 ms steps from 0 ms to 120 ms in speech stimuli like schal thus producing stahl for the 120 ms silent interval. The results of patients with different types of hearing loss fitted with hearing aids are compared with normal hearing. Our investigations show that it will be possible to achieve an integration of fine-structure analysing mechanisms in speech perception into the management and rehabilitation of people with impaired hearing using special speech processors in modified hearing aids and developing special speech and hearing training programs.

193. The large-scale placebo-controlled beta-blocker studies in systolic heart failure revisited: results from CIBIS-II, COPERNICUS and SENIORS-SHF compared with stratified subsets from MERIT-HF.

Abstract

AIMS: The four pivotal beta-blocker trials in heart failure (HF) had different inclusion criteria, making comparison difficult without patient stratifying. The aim of this study was to compare, in similar patients, the effects of bisoprolol, metoprolol controlled release/extended release (CR/XL), carvedilol and nebivolol on (i) total mortality, (ii) all-cause mortality or hospitalization due to cardiovascular causes (time to first event), (iii) all-cause mortality or hospitalization because of HF and (iv) tolerability, defined as discontinuation of randomized treatment. METHODS: We compared stratified (s ) subsets in MERIT-HF with patients in CIBIS-II [New York Heart Association (NYHA) class III/IV and ejection fraction (EF) <=35%] and COPERNICUS (NYHA III/IV and EF <25%) and in patients with systolic HF in SENIORS-SHF (age >=70 years and EF <=35%). RESULTS: The annual mortality rates in the placebo and beta-blocker arms were: (i) CIBIS-II (n = 2647), 13.2% vs. 8.8% (relative risk reduction 34%, 95% CI: 19-46, P < 0.0001) and MERIT-HFs (n = 2002), 14.8% vs. 8.6% (relative risk reduction 42%, 95% CI: 24-56, P < 0.0001); (ii) COPERNICUS (n = 2289), 19.7% vs. 12.8% (relative risk reduction 35%, 95% CI: 19-48, P = 0.0014) and MERIT-HFs (n = 795), 19.1% vs. 11.7% (relative risk reduction 39%; 95% CI: 11-58, P = 0.0086); (iii) SENIORS-SHF (n = 1359), 11.3% vs. 9.7% (relative risk reduction 16%, NS) and MERIT-HFs (n = 985), 14.8% vs. 10.1% (relative risk reduction 32%, 95% CI: 2-53, P = 0.038). The effects on the other outcomes assessed were similar. Analyses indicated fewer discontinuations from randomized treatment on beta-blockers compared with placebo in COPERNICUS and the MERIT-HFs subsets. CONCLUSION: The efficacy and tolerability of bisoprolol, carvedilol and metoprolol CR/XL are similar in patients with systolic HF, irrespective of NYHA class or ejection fraction.
