152. Expanding restrictive transfusion evidence in surgical practice: a multicentre, prospective cohort study

Abstract

BACKGROUND: Findings of observational studies investigating the impact of transfusions are at odds with those of randomised controlled trials, raising concern that observational studies may be inappropriate to inform transfusion decisions. We examined whether observational data could replicate evidence from randomised controlled trials on restrictive transfusion in cardiac and orthopaedic surgery, and be generalised to broader specialties as well as to a lower haemoglobin transfusion threshold (7 g/dL). MATERIAL AND METHODS: A multicentre, prospective cohort study was performed at three representative regional hospitals in China between 2015 and 2016. Participants were surgical inpatients (≥18 years; hospital stay ≥24 h) in six specialties: cardiac, cerebral, vascular (CCV), and orthopaedic, general, thoracic (non-CCV). Patients with a stable haemoglobin (7–10 g/dL) constituted the primary analytic sample, while patients with ≥500 mL intra-operative bleeding were analysed separately to avoid haemoglobin instability. The association of transfusion with surgical outcomes (death, in-hospital complications) was evaluated. RESULTS: The transfusion rate was 10.7% in 36,607 patients (mean age, 52.5±14.3 years; 52.3% female). After restriction, stratification, and propensity score matching to reduce patients’ heterogeneity, transfusion was unrelated to death (CCV: odds ratio [OR]=0.74, 95% confidence interval [CI]: 0.16–3.39; non-CCV: OR 0.83, 95% CI: 0.36–1.94) and the composite complication (CCV: OR 1.31, 95% CI: 0.63–2.72; non-CCV: OR=1.24, 95% CI: 0.81–1.90). The results were consistent in subgroups (elderly, coronary heart disease, malignant tumour, severe illness) and applicable to patients with significant bleeding after restoration of a stable haemoglobin. DISCUSSION: Transfusion at a stable haemoglobin concentration of 7–10 g/dL did not alter surgical outcomes. Our results show the feasibility of observational data to expand restrictive transfusion to broader specialties and a lower transfusion threshold in surgical practice.

Abstract

MXene‐based supercapacitors are promising electrochemical energy‐storage devices due to their ultrahigh volumetric capacitance, high‐power characteristics, and excellent cyclability. However, they suffer from severe self‐discharging behavior while the underlying self‐discharging mechanism is still unclear. Here, the self‐discharge behavior of MXene‐based supercapacitors from surface electronic structure of MXenes is disclosed, and a novel method to mitigate it is proposed. A superficial engineering strategy based on bio‐thermal treatment is developed to effectively tailor surface electronic structure of Ti3C2Tx MXenes by eliminating hydroxyl terminations. With the evolution of surface electronic structure, as revealed by Kelvin probe force microscope and synchrotron radiation X‐ray absorption fine structure analysis, MXene‐based supercapacitors with common aqueous electrolytes show >20% decline in self‐discharge rate. This decline mechanism originates from the increased work function that induces higher zero‐charge potential after the removal of hydroxyl groups in MXenes. Meanwhile, the strengthened surface dipole leads to higher surface free energy between MXene and electrolytes. These two positive effects endow MXenes with weaker self‐discharge kinetics. Specifically, the activation‐controlled self‐discharge process is greatly suppressed. Illuminating the relevance between electronic structure and self‐discharge accompanying superficial engineering suppression strategy can guide to development of high‐performance energy storage devices. Abstract

In this work, a new red phosphor with high color purity, Eu3+ ions doped Ba(Mg1/3Nb2/3)O3 phosphor has been prepared by wet chemical method. The structure analysis suggests BMN:x%Eu phosphors have a hexagonal phase and Ba2+ ions are replaced by Eu3+ ions in BMN. Upon excitation of NUV light, the BMN:x%Eu phosphors emit strong red light around 615 nm, derived from the 5D0-7F2 transition of Eu3+ ions. The relationship between luminescent properties and structure of BMN:x%Eu was discussed. The Judd-Ofelt intensity parameters (Ω2, Ω4) were calculated to analyze the asymmetry of the Eu3+ ions site occupancy further, and the quantum efficiency of BMN:3%Eu was found to be 77.26%. In addition, the decay curve indicates the decay time(τ) of BMN:3%Eu is determined to be 1.34 ms and Eu3+ ions occupy only one type of site. The CIE chromaticity coordinate (0.656,0.344) of BMN:3%Eu is quite close to the red phosphors standard value (0.670, 0.330), which indicates BMN:x%Eu can be a suitable red phosphor used in NUV-based white LEDs.

187. High performance Ag2Se films by a one-pot method for a flexible thermoelectric generator.

Abstract

Herein, a novel and facile method to fabricate a flexible Ag2Se thermoelectric (TE) film on a nylon substrate is developed. The Ag2Se film was prepared by vacuum-assisted filtration of the Ag2Se powder synthesized by a one-pot method followed by hot pressing. An optimal Ag2Se film shows a maximum power factor of ∼2043 μW m−1 K−2 at 300 K, which is one of the highest values reported for Ag2Se films on flexible substrates. The excellent performance of the film is mainly due to the high density and good crystallinity of Ag2Se grains with semi-coherent and coherent grain boundaries. Besides, the film also shows good flexibility. Furthermore, a 6-leg flexible TE generator (f-TEG) assembled from the optimal film produces a maximum power of ∼9.14 μW (corresponding power density of ∼30.46 W m−2) at a temperature gradient of 30.3 K, which proves that the film has exceptionally high TE properties. Abstract

In order to ensure the reliability of the furnace tube hook of the tube heating furnace, based on the on-site monitoring data, a three-dimensional steady state thermal structural coupling analysis model of the furnace tube hook is established, and the appropriate grid density is selected. The thermal structural coupling analysis of the hook was carried out using the finite element multi physical field method. ZG40Cr25Ni20 was selected as the hook material, and the temperature, deformation and equivalent stress of the hook under three different thermal load conditions were mainly analyzed. The results show that the maximum surface temperature of the hook is located at the tip of the web, and decreases with the decrease of the heat load of the heating furnace; Under the same stress and constraint conditions, the maximum deformation values under the three heat loads are 8.72, 8.11 and 7.07 mm respectively, and they are all located at the front end of the web, indicating that the higher the temperature is, the greater the deformation coordination is required; The maximum equivalent stress of the hook is located at the connection between the furnace wall and the hook. The strength of the dangerous section of the hook is checked according to the first strength theory. The check results show that the structural strength meets the design requirements. 195. High Thermoelectric Performance and Ultrahigh Flexibility Ag2S1–xSexfilm on a Nylon Membrane

Abstract

Flexible thermoelectric (TE) generators have recently attracted increasing attention as they have the potential to power wearable devices using the temperature difference between the human body and the environment. Ag2S is recently reported to have plasticity near room temperature; however, it has very low electrical conductivity, leading to its poor TE property. Here, to improve the TE property, different amounts of Se (Se/Ag2S molar ratios being 0.4, 0.5, and 0.6) solid solution-substituted Ag2S films on a nylon membrane are prepared by combing wet-chemical synthesis, vacuum filtration, and hot-pressing. The film (Se/Ag2S molar ratio = 0.6) exhibits a better TE performance with a power factor of 477.4 ± 15.20 μW m–1K–2at room temperature, which is comparable to that of bulk Ag2S1–xSex. In addition, the film possesses excellent flexibility (only ∼5.4% decrease in electrical conductivity after 2000 times bending along a rod with a radius of 4 mm). The power density of a 6-leg TE generator assembled with the film is 6.6 W/m2under a temperature difference of 28.8 K. This work provides a facile new route to Ag2S-based TE films with low cost, high TE performance, and ultrahigh flexibility.

196. Smoking-related cancer death among men and women in an ageing society (China 2020–2040): a population-based modelling study

Abstract

BackgroundChina is experiencing a postpeak smoking epidemic with accelerating population ageing. Understanding the impacts of these factors on the future cancer burden has widespread implications.MethodsWe developed predictive models to estimate smoking-related cancer deaths among men and women aged ≥35 years in China during 2020–2040. Data sources for model parameters included the United Nations World Population Prospects, China Death Surveillance Database, national adult tobacco surveys and the largest national survey of smoking and all causes of death to date. The main assumptions included stable sex-specific and age-specific cancer mortality rates and carcinogenic risks of smoking over time.ResultsIn a base-case scenario of continuing trends in current smoking prevalence (men: 57.4%–50.5%; women: 2.6%–2.1% during 2002–2018), the smoking-related cancer mortality rate with population ageing during 2020–2040 would rise by 44.0% (from 337.2/100 000 to 485.6/100 000) among men and 52.8% (from 157.3/100 000 to 240.4/100 000) among women; over 20 years, there would be 8.6 million excess deaths (0.5 million more considering former smoking), and a total of 117.3 million smoking-attributable years of life lost (110.3 million (94.0%) in men; 54.1 million (46.1%) in working-age (35–64 years) adults). An inflection point may occur in 2030 if smoking prevalence were reduced to 20% (Healthy China 2030 goal), and 1.4 million deaths would be averted relative to the base-case scenario if the trend were maintained through 2040.ConclusionsCoordinated efforts are urgently needed to curtail a rising tide of cancer deaths in China, with intensified tobacco control being key.
