Forest age distribution and occurrence of traces of past fires was studied in a natural Picea abies -dominated landscape in the Onega peninsula in north-west Russia. Forest age (maximum tree age) was determined and charcoal and fire scars were searched for in 43 randomly located study plots. In 70% of the study plots (30/43) trees older than 200 years existed. The largest 50-year age class consisted of plots with 251â300 year old forests. Traces of fires were found in all types of study plots, in forests on mineral soil as well as on peatlands. However, fire has been a rare disturbance factor, as traces of fires could not be found in 35% of the study plots (15/43). Estimated from the forest age class distribution, the fire rotation time for the whole area has been at least 300 years, but possibly considerably longer. This fire rotation time is much longer than fire history studies (largely based on examination of fire scars) commonly have reported for the average time between successive fires in Fennoscandia and Northwest Russia. The results suggest that the often stated generalisations about the importance and natural frequency of fire disturbance in boreal forests do not apply in landscapes dominated by Picea abies.

Two near natural old-growth forests, one dominated by Picea abies and the other by Pinus sylvestris, were studied for their fire history, and spatial patterns of trees and tree ages. The spatial tree age structure and the disturbance history of the forests were examined by drawing age class maps based on mapped and aged trees and by dating fires based on fire scars, and by using spatial analyses at tree scale. The tree age structures of the Picea and Pinus dominated forests were different, mainly due to differences in fire history and sensitivity of the dominant tree species to fire. Fire histories and tree age structures of both sites have probably been affected by human in the ancient past. However, in the Picea dominated site, the fires had been severe, killing most of the trees, whereas in the Pinus dominated site the severity of fires had been more variable, leaving some Pinus and even Picea trees alive. In the Pinus dominated site, the tree age distribution was multimodal, consisting of two Pinus cohorts, which were established after fires and a later Picea regeneration. The Picea dominated site was composed of four patches of different disturbance history. In the oldest patch, the tree age distribution was unimodal, with no distinct cohorts, while a single cohort that regenerated after severe fire disturbances dominated the three other patches. In both sites the overall spatial patterns of living and dead trees were random and the proportion of spatially autocorrelated variance of tree age was low. This means that trees of different age grew more or less mixed in the forest without forming spatially distinct regeneration patches, even in the oldest patch of Picea dominated Liimatanvaara, well over 200 years after a fire. The results show that detail knowledge of disturbance history is essential for understanding the development of tree age structures and their spatial patterns.

Abstract Background Musculoskeletal symptoms among adolescents are related to the time spent using a computer, but little is known about the seriousness of the symptoms or how much they affect everyday life. The purpose of the present study was to examine the intensity of musculoskeletal pain and level of inconvenience to everyday life, in relation to time spent using a computer. Methods In a survey, 436 school children (12 to 13 and 15 to 16 years of age), answered a questionnaire on musculoskeletal and computer-associated musculoskeletal symptoms in neck-shoulder, low back, head, eyes, hands, and fingers or wrists. Pain intensity (computer-associated symptoms) and inconvenience to everyday life (musculoskeletal symptoms) were measured using a visual analogue scale. Based on the frequency and intensity, three categories were formed to classify pain at each anatomic site: none, mild, and moderate/severe. The association with time spent using the computer was analyzed by multinomial logistic regression. Results Moderate/severe pain intensity was most often reported in the neck-shoulders (21%); head (20%); and eyes (14%); and moderate/severe inconvenience to everyday life was most often reported due to head (29%), neck-shoulders (21%), and low back (16%) pain. Compared with those using the computer less than 3.6 hours/week, computer use of ≥ 14 hours/week, was associated with moderate/severe increase in computer-associated musculoskeletal pain at all anatomic sites (odds ratio [OR] = 2.9-4.4), and moderate/severe inconvenience to everyday life due to low back (OR = 2.5) and head (OR = 2.0) pain. Conclusions Musculoskeletal symptoms causing moderate/severe pain and inconvenience to everyday life are common among adolescent computer users. Daily computer use of 2 hours or more increases the risk for pain at most anatomic sites.

Abstract Background Rapeseed oil is the principal dietary source of monounsaturated and n-3 polyunsaturated fatty acids in the Northern Europe. However, the effect of rapeseed oil on the markers of subclinical atherosclerosis is not known. The purpose of this study was to compare the effects of dietary intake of cold-pressed turnip rapeseed oil (CPTRO) and butter on serum lipids, oxidized LDL and arterial elasticity in men with metabolic syndrome. Methods Thirty-seven men with metabolic syndrome completed an open and balanced crossover study. Treatment periods lasted for 6 to 8 weeks and they were separated from each other with an eight-week washout period. Subjects maintained their normal dietary habits and physical activity without major variations. The daily fat adjunct consisted either of 37.5 grams of butter or 35 mL of VirginoR CPTRO. Participants were asked to spread butter on bread on the butter period and to drink CPTRO on the oil period. The fat adjunct was used as such without heating or frying. Results Compared to butter, administration of CPTRO was followed by a reduction of total cholesterol by 8% (p < 0.001) and LDL cholesterol by 11% (p < 0.001). The level of oxidized LDL was 16% lower after oil period (p = 0.024). Minimal differences in arterial elasticity were not statistically significant. Conclusion Cold-pressed turnip rapeseed oil had favourable effects on circulating LDL cholesterol and oxidized LDL, which may be important in the management of patients at high cardiovascular risk. Obesity is a world-wide epidemic and it is a risk factor for type 2 diabetes (T2D). Few randomized controlled studies have compared the 2 most common surgical procedures, Roux-en-Y gastric bypass (RYGB) and sleeve gastrectomy (SG) in the treatment of obese patients with T2D. To compare diabetes remission rates (glycosylated hemoglobin ≤6.0%, without diabetes medications) in obese T2D patients (body mass index, 35–50) undergoing RYGB or SG. Three University Hospital clinics and 1 Regional Hospital in Sweden. Forty-nine patients with T2D were included. Twenty-five were randomized to RYGB and 24 to SG. There was no difference between groups regarding patient characteristics, duration of T2D, overall usage of antidiabetic medications, or glycosylated hemoglobin levels. All patients (100%) completed 1-year follow-up and 47 (95.9%) 2-year follow-up. Remission of T2D was not significantly different between the RYGB and SG, reaching 44% and 46% (n = 25 and n = 24, respectively, P =.897, power =.80) at 1 year, and 48% and 55% (n = 25 and n = 22, respectively, P =.654) at 2 years of follow-up. Similarly, mean glycosylated hemoglobin was improved in both groups at 1 and 2 years, with no significant differences between the groups (RYGB baseline versus 1 yr; mean ± standard deviation: 7.9 ± 1.5 versus 5.8 ±.6%, P <.0001; versus 2 yr: 5.9 ±.7%, P <.0001; SG baseline versus 1 yr: 8.2 ± 1.9 versus 5.9 ±.7%, P <.0001; versus 2 yr: 5.9 ± 1.1%, P <.0001). Total weight loss was not different but percentage excess weight loss was higher after RYGB compared with SG both at 1 and 2 years; mean ± standard deviation: 78 ± 22 versus 60 ± 22%, and 76 ± 24 versus 54 ± 21%, respectively (P <.01 for both). Waist circumference also decreased significantly more in the RYGB group. Despite superior excess weight loss after RYGB, T2D remission rates did not differ significantly between RYGB and SG after 2 years. Anal dysplasia can lead to anal cancer, which affects persons living with HIV (PLWH) more than people in the general population. Screening for anal dysplasia is recommended to detect anal cancer at an early stage. The aim of our process improvement project was to improve compliance and consistency in implementing anal dysplasia screening for PLWH receiving care at a Ryan White facility covering 18 counties in western North Carolina. There were 291 PLWH screened for anal dysplasia during the 9-month data-gathering period. The compliance rate significantly increased from a preintervention rate of 31.3% to 57.5% (p <.001). There were 109 (37.5%) abnormal screening results. PLWH who had abnormal screening results were more likely to be White. Gender and age were not significantly associated with abnormal screening results. Anal dysplasia screening is a simple procedure to detect precursors to cancer that can be integrated into the primary care of PLWH. [ABSTRACT FROM AUTHOR]

Objective Skeletal muscles serve several important roles in maintaining good health. Insulin-like growth factor-1 (IGF-1) is a promoter of protein synthesis in skeletal muscle. Its binding protein, Insulin-like growth factor-binding protein-1 (IGFBP-1) can be one determinant of IGF-1 activity. In the present study we investigate the association between serum IGFBP-1 and muscle mass. Design Cross-sectional analysis of 4908 women, between 55 and 85 years old, participating in the Swedish Mammography Cohort-Clinical. Methods We defined low relative muscle mass (LRMM) as an appendicular lean mass divided by height squared of less than 5.45 (kg/m 2 ), assessed by dual energy x-ray absorptiometry. IGFBP-1 was measured by radioimmunoassay. Logistic regression was used to calculate odds-ratios of LRMM across quartiles of IGFBP-1. Results The odds of LRMM increased across quartiles of IGFBP-1. In the age-adjusted model the odds-ratio (OR) of LRMM was 3.41 (95% CI: 2.55–4.56), comparing the highest to the lowest quartile. This estimate was attenuated in multivariate models (OR: 1.84, 95% CI: 1.34–2.53), mainly due to inclusion of fat mass index. Conclusion Women with higher IGFBP-1 were more likely to have a low relative muscle mass. High IGFBP-1 may be a marker of a catabolic state. [ABSTRACT FROM AUTHOR]

The macroeconomic announcements and their effects on stock markets are considered to be a measure of stock market integration. Earlier studies show that integrated stock markets exhibit immediate reaction to international macroeconomic news, whereas partially integrated or segmented markets mostly do not react to such announcements. This paper investigates the effect of surprises disguised in the macroeconomic announcements made by the European Monetary Union on CIVETS (Colombia, Indonesia, Vietnam, Egypt, Turkey, and South Africa) stock markets. Daily stock market data starting from January 1, 2007 to December 31, 2012 is analysed. The impact of macroeconomic announcements is estimated by using EGARCH model. The results show that the returns of four out of six CIVETS stock markets significantly react on the day of macroeconomic announcements, whereas the market volatility of all markets is affected due to the EMU's announcements. The results also show that not all types of announcements have significant impact on returns and volatilities in CIVETS, highlighting the importance of the contents of macroeconomic surprises. Although the peak onset of rheumatoid arthritis (RA) is after 35 years of age, the disease also occurs during the childbearing years and may interfere with reproduction. Several studies have shown that women with RA need a longer time to conceive and have fewer children than expected. The prolonged time to conceive may be related to very active disease and its treatment which requires postponing a pregnancy. A major proportion of patients experience improvement of symptoms during pregnancy; however, patients positive for rheumatoid factor and/or citrullinated protein antibodies have less chance to ameliorate during pregnancy. About 20% of RA patients will remain active during pregnancy and need drug therapy. Pregnancy outcome is, in general, uncomplicated when the patient was in remission at conception and remained quiescent during pregnancy. By contrast, high disease activity and treatment with >10 mg of prednisone/day increase adverse outcomes, particularly premature delivery. Several studies found birth weight within normal range but lower than in children of healthy women. Pregnancy complications are more frequently encountered in a first pregnancy than in subsequent pregnancies. Disease activity relapses in 50%-65% of pregnancies within 3 months after birth necessitating drug therapy during lactation. The management of a patient with a desire of children should include preconception counseling, adjustment of therapy to medications compatible with pregnancy and lactation, and monitoring disease activity throughout. The best condition for a successful outcome is a planned pregnancy in a period of remission or at low disease activity. [ABSTRACT FROM AUTHOR]

Background Increased permeability and uptake of proinflammatory bacterial endotoxins from gut microbiota has been suggested as a mechanism for obesity-associated chronic inflammation that causes obesity-associated insulin resistance. We hypothesized that intestinal barrier function may be restored after Roux-en-Y gastric bypass (RYGB) surgery and thereby contribute to decreased inflammation. The objective of this study was to investigate levels of the permeability-regulating tight-junction proteins in human small intestinal mucosa before and after RYGB surgery. Methods Paired intraindividual jejunal mucosa samples were retrieved at the time of surgery and 6 to 8 months after surgery. Mucosal cell surface area was calculated by histomorphometry. Mucosal samples were analyzed by proteomics to find patterns of protein regulations. Based on these findings further analyses were performed by Western blotting. Ussing chambers were used to analyze permeability in the retrieved mucosal samples. Results Mucosal surface area was significantly decreased after surgery. Global protein expression analysis showed a significant increase in the cytokeratin-8 (Ck8), which was confirmed by Western blotting. Further analyses showed a significant increase in claudin-3 and -4 expression after surgery, whereas occludin and zonula occludens-1 levels were decreased. Expressions of claudin-1, -2, -5 and vinculin were unchanged. Ussing chamber experiments revealed a linear correlation between the epithelial electrical resistance and claudin-3 protein expression. Conclusion Several alterations were found in the rerouted small intestine after surgery, indicating a decreased jejunal mucosal surface area and decreased paracellular permeability. These changes could contribute to decreased uptake of luminal microbiota-derived inflammatory mediators such as endotoxins after RYGB. [ABSTRACT FROM AUTHOR]
