Striking evidence associates cancer stem cells (CSCs) to the high recurrence rates and poor survival of patients with muscle-invasive bladder cancer (BC). However, the prognostic implication of those cells in risk stratification is not firmly established, mainly due to the functional and phenotypic heterogeneity of CSCs populations, as well as, to the conflicting data regarding their identification based on a single specific marker. This emphasizes the need to exploit putative CSC-related molecular markers with potential prognostic significance in BC patients. This study aimed to isolate and characterize bladder CSCs making use of different functional and molecular approaches. The data obtained provide strong evidence that muscle-invasive BC is enriched with a heterogeneous stem-like population characterized by enhanced chemoresistance and tumor initiating properties, able to recapitulate the heterogeneity of the original tumor. Additionally, a logistic regression analysis identified a 2-gene stem-like signature (SOX2 and ALDH2) that allows a 93% accurate discrimination between non-muscle-invasive and invasive tumors. Our findings suggest that a stemness-related gene signature, combined with a cluster of markers to more narrowly refine the CSC phenotype, could better identify BC patients that would benefit from a more aggressive therapeutic intervention targeting CSCs population.

Lung cancer is one of the most common cancers in the world with a high mortality rate. We analyzed 45 surgical samples of the adenocarcinoma, 13 with lymph node metastasis. APC, BCL2, chromogranin A, CK 5/6/18 (LP34), CK20, CK7, cyclin D1, EGFR, ERCC1, HER2, Ki67, LRP, MRP, P53, RB and TTF1 expressions were evaluated by immunohistochemistry (IHC).Higher Ki67, APC, ERCC1 expressions and lower TTF1 expression were identified in advanced stages (IIA and IIIA) of adenocarcinomas, which reflect a more aggressive, less differentiated, possibly a non-TRU adenocarcinoma.Acinar, micropapillary and BA/lepidic adenocarcinoma patterns were the most similar patterns and papillary was the most different pattern followed by solid pattern, according to expression of these markers. Different adenocarcinoma patterns are engaged with different molecular pathways for carcinogenesis, based on the differences of expression. Acinar, BA/lepidic and micropapillary showed higher TTF1 expression (type TRU), and papillary and solid patterns revealed less TTF1 expression, exhibiting a non-TRU/bronchial phenotype. Solid pattern revealed lower HER2 and higher EGFR and ERCC1 (this compared to papillary) expression; papillary higher HER2 and lower ERCC1 expressions; micropapillary higher RB expression; and acinar lower ERCC1 and higher EGFR expressions. Ciclin D1 seems to have more importance in acinar and BA/lepidic patterns than in micropapillary. ERCC1 protein expression in micropapillary, solid and BA/lepidic patterns may indicate DNA repair activation. Inhibition of apoptosis could be explained by BCL2 overexpression, present in all adenocarcinoma patterns. MRP-1 and LRP were overexpressed in all patterns, which may have implications for drug resistance.Further studies are needed to interpret these data regarding to therapy response in advanced staged bronchial-pulmonary carcinomas. During the last decades, there has been a growing awareness about occupational safety and health risks by the various interested parties in the construction industry. However, despite the substantial improvements achieved, the rate of accidents is still significantly higher than in most of the other industries. Two major reasons have been used to explain this high rate of accidents in the construction industry: (i) the intrinsic riskiness due to the nature of the activities and the particular characteristics of constructions projects and organizations; and (ii) the financial and economic issues regarding the implementation of additional safety measures in a growing competitive market. This companion paper is presented in two parts. The present document refers to Part 2 and makes use of the background knowledge and existing initiatives reviewed in Part 1 to propose and detail the Occupational Safety and Health Potential Risk Model (OSH-PRM). The proposed model was conceived to assist in conducting cost-benefit analysis for occupational safety and health risk management. The OSH-PRM enables an enhanced management of the resources available to improve safety and health conditions in the various activities and for different groups of workers involved in the execution stage of a construction project.

Because water is a key at-risk resource, improved water management is essential. In terms of quantity, the two major alternatives in water management can be grouped into: (i) improving efficiency in water use; and (ii) exploring alternative water sources. Rainwater harvesting (RWH) is one of the most promising alternative water sources, since rainwater can easily be collected and used without significant treatment for non-potable purposes. However, the economical viability of these systems is not always assured. This paper examines the most relevant technical and economical issues in designing domestic RWH systems, evaluating the technical feasibility and economical viability of this technology in the particular weather and water use in Portugal. The evaluation is performed for a single-family residence, where the water use pattern was monitored. The precipitation pattern is characterized for two locations in Portugal, Porto and Almada, since they represent two different scenarios for RWH. The RWH and water savings efficiency were assessed and payback period estimated for both cities. It was found that, for an optimum rainwater tank, the water savings potential are similar for both locations, despite the differences in the average annual precipitation. A simple rule for estimating the optimum tank capacity for single-family households in Portugal is proposed. A sensitivity analysis shows an important influence of water fees on the economical viability of RWH systems in single-family houses in Portugal, namely when compared to changes in the consumption pattern.

Birt-Hogg-Dube (BHD) is a rare syndrome of inherited renal cell carcinomas, characterised by cutaneous lesions and pulmonary cysts and pneumothorax in a vast majority of the patients. Awareness of this syndrome is important in order to refer patients for genetic counselling and personalised follow-up as soon as possible. We describe a case of a 30-year-old female referred to our institution due to incidental discovery of solid bilateral renal masses. Renal biopsies were consistent with chromophobe tumour, and bilateral nephrectomy was performed. Gross examination revealed deformed kidneys with 28 brown and solid lesions, size variable between 0.1 and 6 cm, histologically corresponding to renal cell carcinomas, chromophobe type. Genetic test was required that showed a c.573delGAinsT frameshift mutation in heterozigosity at the folliculin gene, consistent with BHD diagnosis.

NTRODUCTION: Collapsing glomerulopathy (CG) is a rare form of glomerular injury. Although commonly associated with human immunodeficiency virus (HIV) infection, it can occur in association with systemic lupus erythematosus (SLE). CASE REPORT: We present the case of a 50-year-old man, with chronic kidney disease secondary to focal and segmental glomerulosclerosis, who received a cadaveric kidney transplant in 2007. There were no relevant intercurrences until May 2015, when he presented with nephrotic range proteinuria (± 4 g/d). A graft biopsy was performed and it did not show any significant pathological changes. In September, he developed a full nephrotic syndrome (proteinuria 19 g/d) and a graft biopsy was repeated. CG features were evident with a rich immunofluorescence. Antinuclear antibodies (ANA) and anti-double-stranded DNA (anti-dsDNA) antibodies were positive; the remaining immunologic study was normal. Viral markers for HIV, hepatitis C virus (HCV), and hepatitis B virus (HBV) were negative. The patient was treated with corticosteroid pulses and plasmapheresis (seven treatments). A rapid deterioration of kidney function was seen and he became dialysis dependent. He was discharged with a low-dose immunosuppressive treatment. In October, he was hospitalized with diffuse alveolar hemorrhage (DAH). The auto-immune study was repeated, revealing complement consumption and positive titers of ANA and Anti-dsDNA antibodies. Anti-neutrophil cytoplasmic antibodies (ANCAs) and antiglomerular basement membrane antibody (anti-GBM) were negative. Treatment with intravenous corticosteroids, plasmapheresis, and human immunoglobulin was ineffective and the outcome was fatal. CONCLUSION: This case report highlights the possible association of CG and SLE. To our knowledge, it is the first case of SLE presenting with CG and DAH, with the singularity of occurring in a kidney transplant recipient receiving immunosuppression. info:eu-repo/semantics/publishedVersion

BACKGROUND: Polyomavirus nephropathy (BKVN) is an important cause of chronic allograft dysfunction (CAD). Recipient determinants (male sex, white race, and older age), deceased donation, high-dose immunosuppression, diabetes, delayed graft function (DGF), cytomegalovirus infection, and acute rejection (AR) are risk factors. Reducing immunosuppression is the best strategy in BKVN. The objective of our study was to evaluate CAD progression after therapeutic strategies in BKVN and risk factors for graft loss (GL). METHODS: Retrospective analysis of 23 biopsies, from patients with CAD and histological evidence of BKVN, conducted over a period of 10 years. Glomerular filtration rate was 60 years), and 22 had a deceased donor. Diabetes affected 4 patients, DGF occurred in 3, cytomegalovirus infection in 2, and AR in 15. All patients were medicated with calcineurin inhibitors (CNI) (95.7% tacrolimus) and corticoids, and 16 also received an antimetabolite. One year after antimetabolite reduction/discontinuation and/or CNI reduction/switching and/or antiviral agents, graft function was decreased in 11 patients, increased/stabilized in 10, and unknown in 2. GL occurred in 9 patients. Older age (hazard ratio, 1.76; 95% confidence interval, 0.94-3.28) and DGF (hazard ratio, 2.60; 95% confidence interval, 0.54-12.64) were the main risk factors for GL. The lower GFR at the time of the BKVN diagnosis was associated with an increased risk of initiation of dialysis. CONCLUSIONS: GL occurred in 39.1% of patients with BKVN and DGF; older age and lower GFR at the time of diagnosis were important risk factors. Early diagnosis of BKVN is essential to prevent GL. info:eu-repo/semantics/publishedVersion

INTRODUCTION: There is a high incidence of nephrotic proteinuria in renal transplant recipients, which is an accurate predictor of graft loss. Despite this, its histologic correlates and prognostic implications are still not well characterized. We assessed the clinical and histological correlates of kidney transplantation patients with nephrotic range proteinuria. METHODS: We have retrospectively analyzed clinical and histological data from 50 kidney transplantation biopsy specimens from 44 renal transplant recipients with nephrotic range proteinuria between 2006 and 2015. The median follow-up time was 93 months (range, 14 months to 190 months). RESULTS: The mean age of the patients was 45.2 ± 13.7 years and our cohort included 86% recipients of deceased-donor grafts. The maintenance immunosuppressive regimen included calcineurin inhibitors in 68% and mammalian target of rapamycin inhibitors in 32% of patients. The average proteinuria was 6.9 ± 3.8 g/d and 52% of patients presented with nephrotic syndrome. The main histological findings were transplant glomerulopathy (22%), de novo glomerular disease (22%), and recurrence of primary disease (22%). Tubular atrophy and interstitial fibrosis was present in 78% of the biopsy specimens. Thirty-one patients (62%) lost the graft at follow-up. There was no statistically significant difference between the histologic diagnosis nor the proteinuria levels and the outcome of the graft. CONCLUSIONS: The main causes of nephrotic range proteinuria in patients undergoing biopsy were transplant glomerulopathy, recurrence of the underlying disease, and de novo glomerulonephritis. Nephrotic range proteinuria was related to a high rate of graft loss. info:eu-repo/semantics/publishedVersion

The use of elements that help the project designer to make the environments more comfortable and efficient comes from an analysis that identifies the best solutions for each reality. Studies show that the use of light redirection panels in side openings reduces the incidence of direct sunlight in the area near the window, increases diffuse light in the first third of the room closest to the window, and improves the overall distribution of light in the room. The present work studies the laser cut panel application in an urban verticalization scenario in Maceió. The methodology used is divided into two parts: the first one consists in the elaboration of an additional algorithm for the TropLux software that allows to simulate computationally the material chosen, and it’s performance was verified after a comparison of the reflectance and transmittance directional obtained, and the values found in literature; The second part aims to analyze the advanced element utilization in an denser urban environment using the developed algorithm, in which the models were analyzed using parameters of average illuminance and it’s uniformity, as well as the statistics analysis of the data. The validation carried out indicates that the operation of the module corresponds to other results obtained in literature, attesting its effectiveness. The urban models analysis results indicate that the panel positioned vertically, at 0 °, works well as sunscreen against the direct rays of the sun. For light redirection function the element presented better results for clear skies partially covered when applied at 30°, and at 45° of inclination for clear skies, independent of the urban canyon orientation. However, scenarios of reduced vision of sky, less than 20%, presented different behavior, this need complementary analysis.

Background and Aims Self-expandable metallic stents are considered the best palliative treatment of dysphagia for patients with advanced esophageal cancer. Adverse events (AEs) are a major concern, especially in patients with better prognosis and longer survival. The present study aimed to evaluate the AEs of patients who survived longer than 6 months with esophageal stents in place. Methods This is a retrospective analysis of a prospectively collected database including all patients submitted to esophageal stent placement for the palliation of malignant diseases during the period from February 2009 to February 2014 at a tertiary care academic center who had stents longer than 6 months. Results Sixty-three patients were included. Mean follow-up was 10.7 months. Clinical success was achieved in all patients, and the median stent patency was 7.1 months. AEs occurred in 40 patients (63.5%), totaling 62 AEs (mean, 1.5 AEs per patient). Endoscopic management of AEs was successful in 84.5% of cases, with a mean of 1.6 reinterventions per patient. The univariate analysis revealed that performance status, age, and post-stent radiotherapy presented a trend to higher risk of AEs. The multivariate analysis revealed that only performance status was associated with AEs ( P = .025; hazard ratio, 4.1). Conclusions AEs are common in patients with long-term esophageal stenting for malignancy. However, AEs were not related to higher mortality rate, and most AEs could be successfully managed by endoscopy. Only performance status was a risk factor for AEs. Our data suggest that metallic stenting is a valid option for the treatment of malignant esophageal conditions, even when survival longer than 6 months is expected.

The implementation of a risk-informed asset management system by a wastewater infrastructure utility requires information regarding the probability and the consequences of component failures. This paper focuses on the former, evaluating the performance of artificial intelligence tools, namely artificial neural networks (ANNs) and support vector machines (SVMs), in predicting the structural condition of sewers. The performance of these tools is compared with that of logistic regression on the case study of the wastewater infrastructures of SANEST — Sistema de Saneamento da Costa do Estoril (Costa do Estoril Wastewater System). The uncertainty associated to ANNs and SVMs is quantified and the results of a trial and error approach and the use of optimization algorithms to develop SVMs are compared. The results highlight the need to account for both the performance and the uncertainty in the process of choosing the best model to estimate the sewer condition, since the ANNs present the highest average performance (78.5% correct predictions in the test sample) but also the highest dispersion of performance results (73% to 81% correct predictions in the test sample), whereas the SVMs have lower average performance (71.1% without optimization and 72.6% with the parameters optimized using the Covariance Matrix Adaptation Evolution Strategy) but little variability.

Five years survival of lung cancer is 16%, significantly lower than in prostate (99.9%), breast (88.5%) and colon (64.1%) carcinomas. When diagnosed in the surgical stage it increases to 50% but this group only comprises 14â16% of the cases. DNA methylation has emerged as a potential cancer-specific biomarker. Hypermethylation of CpG islands located in the promoter regions of tumour suppressor genes is now firmly established as an important mechanism for gene inactivation. This retrospective study included 40 squamous cell carcinomas and 40 adenocarcinomas in various surgical TNM stages to define methylation profile and possible silencing of DNA repair genes â MLH1 and MSH2 â using Methylation-Specific PCR and protein expression by immunohistochemistry in tumoural tissue, preneoplastic lesions and respiratory epithelium with normal histological features. The protein expression of MLH1 and MSH2 genes, in the available preneoplastic lesions and in normal cylindrical respiratory epithelium appeared reduced. The frequency of promoter hypermethylation found on these DNA repair genes was elevated, with a higher prevalence of methylation of MLH1 gene in 72% of squamous cell carcinoma. The differences are not so obvious for MSH2 promoter hypermethylation. No correlation was found among the status of methylation, the protein expression and the clinicopathological characteristics. With a larger study, a better characterization of the hypermethylation status of neoplastic and preneoplastic lesions in small biopsies would be achieved, inherent to tumour histology, heterogeneity and preservation, and finally differences in the study population to elucidate other possible mechanisms of altered expression of the hMLH1 and hMSH. Parvovirus infection is usually asymptomatic especially in immunocompetent adults. When symptomatic it can range from mild to life threatening depending on the patient’s age and comorbidities. We report a case of a 40-year-old male patient with parvovirus infection who presented a purpuric rash in distal extremities, acute kidney injury, type II mixed cryoglobulinaemia and hypocomplementaemia. His renal biopsy showed a mesangioproliferative glomerulonephritis with positive immunoreactivity to C3, IgM and C1q. Parvovirus B19 was detected in the biopsy tissue by PCR. He was treated with prednisolone with total remission after 1 month. We discuss the diagnosis of kidney lesion due to parvovirus in an immunocompetent person, which is a very rare condition and its association with the cryoglobulinaemia diagnosis.

Wastewater drainage systems asset management decisions, in particular regarding rehabilitation interventions, are largely dependent on close-circuit television (CCTV) inspection results. However, the results of CCTV inspections are affected by several sources of uncertainty. Within the present communication, the inspector's uncertainty is quantified by comparing periodic inspection reports from three trunk sewers of a Portuguese sewer system. The inspections were carried out by the same experienced inspector using the same equipment. Therefore, the uncertainties from the lack of experience and the difference of the inspector and equipment were ruled out. The protocol uncertainty is also quantified comparing the results obtained with the Water Research Center (WRc) and the National Research Council of Canada (NRC) protocols condition ratings. Both operational and structural condition rating were analysed, but emphasis was given to the later since it dictates the repair and replacement interventions.

Operation, maintenance and rehabilitation comprise the main concerns of wastewater infrastructure asset management. Given the nature of the service provided by a wastewater system and the characteristics of the supporting infrastructure, technical issues are relevant to support asset management decisions. In particular, in densely urbanized areas served by large, complex and aging sewer networks, the sustainability of the infrastructures largely depends on the implementation of an efficient asset management system. The efficiency of such a system may be enhanced with technical decision support tools. This paper describes the role of artificial intelligence tools such as artificial neural networks and support vector machines for assisting the planning of operation and maintenance activities of wastewater infrastructures. A case study of the application of this type of tool to the wastewater infrastructures of Sistema de Saneamento da Costa do Estoril is presented.

Lecithin:cholesterol acyltransferase (LCAT) is the enzyme responsible for cholesterol esterification in plasma. Mutations in the LCAT gene leads to two rare disorders, familial LCAT deficiency and fish-eye disease, both characterized by severe hypoalphalipoproteinemia associated with several lipoprotein abnormalities. No specific treatment is presently available for genetic LCAT deficiency. In the present study, recombinant human LCAT was expressed and tested for its ability to correct the lipoprotein profile in LCAT deficient plasma. The results show that rhLCAT efficiently reduces the amount of unesterified cholesterol (−30%) and promotes the production of plasma cholesteryl esters (+210%) in LCAT deficient plasma. rhLCAT induces a marked increase in HDL-C levels (+89%) and induces the maturation of small preβ-HDL into alpha-migrating particles. Moreover, the abnormal phospholipid-rich particles migrating in the LDL region were converted in normally sized LDL.

Bronchial schwannomas are very rare pulmonary lesions, but its awareness is important to reach correct diagnosis and decide proper intervention. Clinical and radiological characteristics are mainly unspecific and pathological examination usually provides the definite diagnosis. In small lesions, endoscopic approach may be sufficient, but in large lesions associated with organising pneumonia surgical intervention may be required. Prognosis is typically favourable. We describe a case of a woman, aged 66 years, with productive cough and sporadic haemoptysis, dyspnoea, anorexia, excessive sweating and weight loss with 2 months evolution. CT scan showed a soft tissue dense lesion on the left hilum with 3.75 cm with 18-Fludeoxyglucose uptake. Left upper lobectomy was performed. Gross examination revealed a polypoid mass without necrosis, histologically showing cellular dense (Antoni A) and less dense (Antoni B) areas with Verocay bodies, slightly pleomorphic spindle cells, without mitotic activity and positive for S100 protein on immunohistochemistry.

The present work aims to make a diagnosis of the scientific-academic productions, starting from thematic axes such as: Teaching Knowledge, Monitoring and Teacher Training for Higher Education. In order to do so, a bibliographical research of the productions of theses and dissertations elaborated in the period from 2006 to 2014 was systematized, in order to establish the State of the Art of these productions and, thus, to balance them according to their temporal, spatial and Theoretical. The body of work is divided into two main chapters; The first one deals with the conceptual and formative aspects of the Teaching Knowledge according to renowned authors in the area, and then presents the conceptual, historical and legal aspects of Academic Monitoring, alluding to the trajectory and transformations of monitoring throughout Brazilian history; The second chapter presents a diagnosis of the productions of theses and dissertations elaborated in Brazil, followed by their analysis. The results evidenced that there are few methodological and reference variants in the productions, but they present a great gap in the production works with the aggregation of the mentioned themes, since only one paper presents in its research the three thematic axes. In the face of everything, it is concluded that there are few productions with the aforementioned themes, perhaps due to lack of interest or information, pointing to the non-recognition of the educational potential of the monitoring activities.

Modeling the service life of building components is a key requirement for several purposes, including the implementation of an effective and efficient asset-management system, life-cycle assessments, or life-cycle cost calculations. Because the phenomena underlying the degradation of building components is highly complex, it is common to use statistical approaches for modeling purposes. This paper is focused on the statistical modeling of the service life of the wood frame doors and windows of the National Palace of Sintra. A review of the main factors of degradation of traditional linseed-based painted wood frame doors and windows and of the approaches that have been used to model the service life of building materials and components is presented. The influence of the exposure to weather agents on the degradation rate of external wood frame doors and windows is investigated and a model to estimate the average condition level is proposed. The proposed model takes into account radiation and wind-dr...

The interaction between ecology, culture and genome evolution remains poorly understood. Analysing population genomic data from killer whale ecotypes, which we estimate have globally radiated within less than 250,000 years, we show that genetic structuring including the segregation of potentially functional alleles is associated with socially inherited ecological niche. Reconstruction of ancestral demographic history revealed bottlenecks during founder events, likely promoting ecological divergence and genetic drift resulting in a wide range of genome-wide differentiation between pairs of allopatric and sympatric ecotypes. Functional enrichment analyses provided evidence for regional genomic divergence associated with habitat, dietary preferences and postzygotic reproductive isolation. Our findings are consistent with expansion of small founder groups into novel niches by an initial plastic behavioural response, perpetuated by social learning imposing an altered natural selection regime. The study constitutes an important step toward an understanding of the complex interaction between demographic history, culture, ecological adaptation and evolution at the genomic level.

Engineering courses can benefit of using remote labs to support teaching and online learning. Online experimentation represents also a very important support in engineering teaching and can be used to improve the students learning process, for example in Electrical or Mechanical Engineering courses, on several topics. A remote lab based on a vibrating bean apparatus could be an effective tool to be used in practical classes and to enhance the students' experimental skills. This paper describes the demonstration of an online experiment, supported by a remote lab system, to carry out some experiments using a vibrating beam. An experimental setup is used to interact with the remote lab through a Web platform, where students can perform different experiments, visualizing and obtaining data in real time from the remote system.
