Abstract

© Author(s) 2015. A comparison exercise on thermal-optical elemental carbon/organic carbon (ECOC) analysers was carried out among 17 European laboratories. Contrary to previous comparison exercises, the 17 participants made use of an identical instrument set-up, after correcting for temperature offsets with the application of a recently developed temperature calibration kit (Sunset Laboratory Inc, OR, US). Temperature offsets reported by participants ranged from -93 to +100 °C per temperature step. Five filter samples and two sucrose solutions were analysed with both the EUSAAR2 and NIOSH870 thermal protocols. z scores were calculated for total carbon (TC); nine outliers and three stragglers were identified. Three outliers and eight stragglers were found for EC. Overall, the participants provided results between the warning levels with the exception of two laboratories that showed poor performance, the causes of which were identified and corrected through the course of the comparison exercise. The TC repeatability and reproducibility (expressed as relative standard deviations) were 11 and 15% for EUSAAR2 and 9.2 and 12% for NIOSH870; the standard deviations for EC were 15 and 20% for EUSAAR2 and 20 and 26% for NIOSH870. TC was in good agreement between the two protocols, TCNIOSH870 =0.98 × TCEUSAAR2 (R2 = 1.00, robust means). Transmittance (TOT) calculated EC for NIOSH870 was found to be 20% lower than for EUSAAR2, ECNIOSH870 = 0.80 × ECEUSAAR2 (R2 = 0.96, robust means). The thermograms and laser signal values were compared and similar peak patterns were observed per sample and protocol for most participants. Notable deviations from the typical patterns indicated either the absence or inaccurate application of the temperature calibration procedure and/or pre-oxidation during the inert phase of the analysis. Abstract

The performance and the uncertainty of receptor models (RMs) were assessed in intercomparison exercises employing real-world and synthetic input datasets. To that end, the results obtained by different practitioners using ten different RMs were compared with a reference. In order to explain the differences in the performances and uncertainties of the different approaches, the apportioned mass, the number of sources, the chemical profiles, the contribution-to-species and the time trends of the sources were all evaluated using the methodology described in Belis et al. (2015). In this study, 87% of the 344 source contribution estimates (SCEs) reported by participants in 47 different source apportionment model results met the 50% standard uncertainty quality objective established for the performance test. In addition, 68% of the SCE uncertainties reported in the results were coherent with the analytical uncertainties in the input data. The most used models, EPA-PMF v.3, PMF2 and EPA-CMB 8.2, presented quite satisfactory performances in the estimation of SCEs while unconstrained models, that do not account for the uncertainty in the input data (e.g. APCS and FA-MLRA), showed below average performance. Sources with well-defined chemical profiles and seasonal time trends, that make appreciable contributions (>10%), were those better quantified by the models while those with contributions to the PM mass close to 1% represented a challenge. The results of the assessment indicate that RMs are capable of estimating the contribution of the major pollution source categories over a given time window with a level of accuracy that is in line with the needs of air quality management.

Abstract

The identification of sources is one of the prerequisites for the implementation of the Air Quality Directive (AQD). It provides scientific support to the development and periodic revision of air quality plans and short term action plans and to the quantification of categories with special status like long range transport, natural sources and winter road salting and sanding. The suitability of receptor models (RM) for the apportionment of pollutant sources in the implementa-tion of the AQD is testified by the amount of published studies in 2005 and 2010 in correspondence with the entry into force of new provisions for PM10 and PM2,5, respectively. In recent years, Member States were requested to provide official estimations of source contribution to the Commission like the quantification of natural sources in 2006 and PM10 time extension reports in 2009 (Fragkou et al., 2011). These experiences have shown that al-though this kind of methodology is used by about 60 % of the European experts involved in source apportion-ment there is a considerable variability in the methodo-logical approaches and adopted tools. Furthermore, there are critical steps that require strict quality assurance standards and skilled practitioners (e.g. identification of the appropriate number of sources). In order to foster harmonization in this field, the JRC has promoted a number of interconnected initiatives linked to FAIRMODE. One of those was to set up a group of experts with skills in different areas to assess RM methodologies and propose common criteria and procedures for source apportionment studies. The infor-mation collected was summarized in a document which is intended to serve as a basis for a common Receptor Model Technical Protocol (RMTP). The RMTP is addressed to different kinds of us-ers: Policy makers and managers interested in the output of RMs for development of mitigation measures, air quality experts and scientists unfamiliar with these tech-niques, and RM practitioners involved in the model ex-ecution and interpretation of results. In order to address such heterogeneous readership the document was designed to be accessible at different levels. The RMTP is organized in three sections : an introduction to present the methodology to the unskilled reader, illustrating its capabilities and recom-mending when and how to use it ; a core section concerning the most common RM methodologies for source apportionment with in-depth analysis boxes for more experienced readers. The section is structured in 13 chapters following, as a check list, the logical steps to accomplish a source apportionment study. The first chapters deal with preliminary activities like the evaluation of the study area, collection of exist-ing information and experimental design. In the follow-ing chapters data collection and data treatment are dis-cussed. The section includes also chapters regarding spe-cific aspects of widely used methodologies like CMB, Factor Analysis, and PMF. The evaluation of test per-formance and reporting are discussed in the last part of this section ; the third section of the document was conceived to give an insight on the capabilities and the possible future trends in RM methodology. It consists of four chapters dealing with advanced, innovative techniques for which ready- to-use tools are already available or under development : trajectory analysis combined with RM, constrained and expanded models, AMS data processing, and the aethalometer model. The document includes a number of annexes to provide additional and practical information on specific topics, and examples.

Abstract

We assessed the influence of clinically significant mitral regurgitation (MR) on clinical-echocardiographic response and outcome in heart failure (HF) patients treated with a biventricular defibrillator (cardiac resynchronization therapy defibrillator [CRT-D]). METHODS AND RESULTS: A total of 659 HF patients underwent successful implantation of CRT-D and were enrolled in a multicenter prospective registry (median follow-up of 15 months). Following baseline echocardiographic evaluation, patients were stratified into two groups according to the severity of MR: 232 patients with more than mild MR (Group MR+: grade 2, 3, and 4 MR) versus 427 patients with mild (grade 1) or no functional MR (Group MR-). On 6- and 12-month echocardiographic evaluation, MR was seen to have improved in the vast majority of MR+ patients, while it remained unchanged in most MR- patients. On 12-month follow-up evaluation, a comparable response to CRT was observed in the two groups, in terms of the extent of left ventricular reverse remodeling and combined clinical and echocardiographic response. During long-term follow-up, event-free survival did not differ between MR+ and MR- patients, even when subpopulations of patients with ischemic heart disease and with dilated cardiomyopathy were analyzed separately. On multivariate analysis, the only independent predictor of death from any cause was the lack of β-blocker use. CONCLUSIONS: This observational analysis supports the use of CRT-D in HF patients with clinically significant MR; MR had no major influence on patient outcome.

Abstract

Aims To analyse the effectiveness of cardiac resynchronization therapy (CRT) in patients with valvular heart disease (a subset not specifically investigated in randomized controlled trials) in comparison with ischaemic heart disease or dilated cardiomyopathy patients. Methods and results Patients enrolled in a national registry were evaluated during a median follow-up of 16 months after CRT implant. Patients with valvular heart disease treated with CRT ( n = 108) in comparison with ischaemic heart disease ( n = 737) and dilated cardiomyopathy ( n = 635) patients presented: (i) a higher prevalence of chronic atrial fibrillation, with atrioventricular node ablation performed in around half of the cases; (ii) a similar clinical and echocardiographic profile at baseline; (iii) a similar improvement of LVEF and a similar reduction in ventricular volumes at 6–12 months; (iv) a favourable clinical response at 12 months with an improvement of the clinical composite score similar to that occurring in patients with dilated cardiomyopathy and more pronounced than that observed in patients with ischaemic heart disease; (v) a long-term outcome, in term of freedom from death or heart transplantation, similar to patients affected by ischaemic heart disease and basically more severe than that of patients affected by dilated cardiomyopathy. Conclusion In ‘real world’ clinical practice, CRT appears to be effective also in patients with valvular heart disease. However, in this group of patients the outcome after CRT does not precisely overlap any of the two other groups of patients, for which much more data are currently available.

Abstract

Background: Few data exist on the long-term changes and the prognostic value of heart rate variability (HRV) assessed by implanted devices in heart failure (HF) patients treated with resynchronization therapy (CRT). Aims: To analyze the long-term changes in the standard deviation of 5-minute median atrial–atrial sensed intervals (SDANN), and assess its role in predicting CRT efficacy and major cardiovascular events. Methods and results: We included 509 consecutive patients implanted with CRT devices. At 12-month follow-up, 44 patients had died and 86 patients had at least one HF hospitalisation. Abstract

The wall paintings of the Michelozzo’s Courtyard (XVI century) at the main entrance of Palazzo Vecchio in Florence are in an advanced state of degradation. With the aim of understanding the environmental conditions in the Courtyard a wide research project has been instigated. In this study, the fine and coarse fractions of the PM10 have been collected by a ‘streaker’ sampler with an hourly resolution and then analysed by PIXE at the 3 MV tandetron accelerator in Florence; the first results will be presented here.
