Decision making in organizational purchases is subject to the influence of several variables, considering that many members can participate of the process of selecting a product. Purchase centers in companies generally include several participants with different interests, levels of authority, status, and persuasiveness. In view of the above, the objective of this work was to identify, in a company in the forestry sector, the actors' perception of their influence on the purchase process and / or replacement of forest harvesting machines. For this, a structured questionnaire was applied where the members of the company, from the strategic, tactical and operational levels, should select which role they played in the decision making to buy the machines, in order to identify if they are in line with their functions. The results showed that, for the company studied, there is a gap between the role perceived by the collaborator and his actual role, and this is more evident for initiator and internal filters, where most of the respondents did not identify with this classification. CRITICAL thinking , *DECISION making , *DIGITAL libraries , *HYPERTEXT literature , *LITERATURE reviews - Abstract
The importance of ethical decision making in health sciences has promoted the generation of schemes that guide this process. The objective is to propose a theoretical model of bioethical decision making. We searched and reviewed the literature in databases and electronic libraries with keywords, eleven models of ethical decision making were identified and analyzed according to established parameters and results of studies on the subject were included. The schemes analyzed propose a decision making process based on a variable number of cognitive steps and, in general, provide few elements for their practical application. The Reason-Emotion Integration Model for Bioethical Decision Making in Health Sciences was proposed. This model synthesizes the cognitive steps to make appropriate decisions, by integrating reason and emotion, the possibility of applying various ethical frameworks and implementing an ethical competence and critical thinking skills. This study aims to integrate the evidence about the uncertainty about the disease in the patient with chronic illness and their family caretaker, guided by the theory of uncertainty about the disease. For this, an integrative synthesis of the literature was carried out during the years 2007 to 2017. The Medline, Science Direct, Ovid Nursing, Scielo, Scopus, CINAHL and Psycinfo databases were used in the English and Spanish languages. To carry out the integrative synthesis, 46 publications were included, with 21 qualitative, 19 quantitative, 2 mixed and 4 reviews. The studies were mostly carried out in patients with cancer, neurodegenerative diseases, organic failures, heart failure, COPD and in palliative care of different diseases. Among the main conclusions of the study, it is suggested that the person with chronic illness and their family caretaker develop uncertainty about the chronic illness due to shortcomings in education about the disease and care, as well as the social support they receive from the health team and its support networks. The purpose of this article is to identify the relationships between vulnerability and autonomy that academic literature has developed in relation to homeless. For the fulfillment of the objective, a bibliographic search was developed in different databases under the criteria of homeless, autonomy, decision making, vulnerability, bioethics. As a conclusion, it is clear that the mental problems that this population group can present demonstrate the difficulties that exist to resolve the false bioethical dichotomy that exists between autonomy and vulnerability. A new task for bioethics is identified, such as overcoming the presented dichotomy that can provide substantial elements that help to overcome the difficulties that arise for the attention of homeless. The paper presents a proposal for a mathematical and management model for a business decision-making approach, which is based on the analysis of the market segmentation theory, but linked to the specific calculation of the different existing market levels values, this through the use of a market equation based on cost, return or market coefficient, and its corresponding segmentation level, data that is managed in a single equation, which when used with the mathematical derivative, allows to establish the economic value of a product or service in a certain market condition, model that allows the correct decision-making regarding a particular segmentation strategy choice. During the lifecycle of a construction project, decisions are made by stakeholders in all project stages that will define the path for the development of the building asset. Due to the absence of controlled and centralised information to be accessible for decision-making processes, many of these decisions are not supported with data, but based on the experience and intuition of leaders without a tangible fundament that supports the decisions process. Based on quantity information obtained through digitalization and BIM processes, it is possible to generate structured databases to create automatised analytics to be implemented for a better assessment of the variables involved in a decision and be able to make data driven decisions to optimise the outcome of these resolutions. Quantity information can be the link between the technical solution and the construction strategy and associated production costs. This study is focused on the required structure of information needed to implement a complete BIM-based Quantity Take-off (QTO) that can be used to query relevant data and apply BIM Analytics based on quantity information, to support stakeholders to make data-driven decisions. Main challenges, existing standardisation initiatives and solutions offered by software frame a State of the Art presented in this document that intends to give a notion on the level of implementation BIM-based QTO nowadays. Five professionals involved in decision-making processes for the AECO Industry supported this study with their professional knowledge to identify how quantity information can be used to support data-driven decisions and with proposals to overcome the main challenges in the generation of the structured information. The application of four case studies explains the process to link modelled-based QTO and BIM Analytics based on quantity information focused on four types of decision-making processes: Progress Tracking, Value Engineering, Change Management and E Procurement. Information requirements, levels of information need, required classification of data and main considerations were framed based on literature review, on the implementation of case studies and on the knowledge of participant professionals to guide stakeholders for the required structurization of information, generation of databases and BIM Analytics to support decision-making and make data driven decisions. During this dissertation, the development of an assessment tool was achieved. Objective. Describe users' experience with participation in clinical decision-making at Family Health Centers (CESFAM) in the Primary Health Care (PHC) system. Methods. Qualitative descriptive study. Focus groups made up of CESFAM users were held in southeastern Santiago, Chile, and the information was thematically analyzed using Atlas.ti version 6® software. Results. Five focus groups were held (n = 41). The main themes that emerged from the discussions were the passive role of users in decisions and delegated decisionmaking, based on their trust in the health professional. Users' passive role limits their opportunities for participation in clinical decision-making, ceding power to the health professional. However, establishing a therapeutic partnership allows users to feel that they are being treated as unique individuals whom the professional listens to and respects, ensuring that the decisions of the health team can be trusted, since they look out for the users' real interests. Conclusion. Users' participation in clinical encounters is still limited in Chile. However, bolstering that participation is essential for increasing user satisfaction and promoting people-centered care. [ABSTRACT FROM AUTHOR]

Objective. The Perinatal Information System (SIP) represents a milestone in the use of systematized information in the Region of the Americas. What has been learned from the system has contributed to the development of a model based on a set of core indicators (SCI). The objective of the study was to describe the historical and methodological process involved in the development, implementation, and territorial scaling- up of an SCI to monitor and evaluate women's and perinatal health policies, programs, and services geared to management (SIP-GESTIÓN). Methods. The study was conducted in two stages: 1) a four-phase validation of an SCI in a hospital network: a) construction of the theoretical indicator model, b) operations research, c) final selection of the indicators, and d) the definition of reference standards; and 2) territorial scaling-up. Results. A total of 17 indicator models were identified. The initial model included 177 indicators divided into seven dimensions (context, habits, accessibility, use of services, quality of care, maternal and fetal impact, and maternal and neonatal impact), with 21 indicators remaining after three Delphi rounds. The final model (SIPGESTIÓN), which included 40 indicators, was then used to study 240,021 (79.1%) of the 303,559 deliveries attended in 122 selected maternity facilities in 24 jurisdictions (100%) in Argentina. The information is presented in national terms and by health region, province, and hospital. Conclusions. This model has made it possible to achieve high levels of information coverage and quality and territorial scaling-up and is useful for management, research, and the reorientation of programs and policies. Objective. To analyze the value judgments behind cost--benefit tradeoffs made by health stakeholders in deciding whether or not to incorporate new health technologies and how they should be financed and allocated in limited-resource settings in Brazil. Method. From June 2009 to January 2010, a sample of stakeholders in the public and private health sector was identified and invited to complete an online survey consisting of two questionnaires: one collecting socio-demographic/professional information and one capturing resource allocation preferences in four hypothetical scenarios for the incorporation of new health technologies. Results. A total of 193 respondents completed the survey; more than half were male (53.9%) and the most common age group was 31--40 years (36.8%). Scenario 1 (incorporation of a new drug treatment for chronic disease, by reducing/eliminating resources for existing programs) was rejected by 49.2% of the survey sample, who preferred to maintain the status quo for existing programs. Scenario 2 (incorporation of the same new treatment, but financed by a new tax) was rejected by 58.0%. Scenario 3 (incorporation of a new treatment for a highly lethal disease, by age group--20--75 years versus 75+ years--by reducing/eliminating resources for existing programs), was rejected by 42.0%, while 20.7% supported allocations for both groups, 34.2% supported allocations exclusively for the 20--75-year age group, and 3.1% supported allocations exclusively for the 75+ year age group. For Scenario 4, which consisted of five different resource allocations for prevention and treatment programs for another highly lethal disease, the most preferred option (chosen by 50.8% of respondents) was 75%:25% ( prevention versus treatment). Conclusions. When incorporating a new health technology requires reducing/eliminating other health programs, financing it through a tax, or having to choose certain age groups (e.g., younger, working people versus older people), respondents are likely to reject it. When offered the choice of limiting the scope of the program (e.g., prevention versus treatment), respondents are likely to favor prevention. This was the first study in Brazil to capture value judgments that affect stakeholder decision-making on various resource allocations for different scenarios for health technology introduction in limited-resource settings. Future research should investigate the perspective of society as a whole to determine the best approach for decision-making based on common values and consensus within a particular health care system. - Abstract
Corporate and academic research management involves the identification of research lines which lead funds and technical efforts of groups that are devoted to research, development and innovation activities. In the process of decision making which conduct this management, there is a high prevalence of individual experiences and interest of researchers over the objective consultation of different sources which provide information and optimize such process. This paper aims at validating a methodology that links two different perspectives about decision making in research, development and innovation, showing partial results from five actions proposed to identify and strengthen research lines of corporate and academic groups. Such validation demonstrates a sense of belonging, clarity and applicability resulting from the proposed actions which make them useful tools for decision making in research, development and innovation and for the creation of new research lines. Total time of prehospital care (PHC) is the time elapsing from the inbound call up to the moment when the ambulance is available for serve in another emergency event. This research aims to show that selecting the destination hospital impacts significantly the total PHC time, which influences the survival of the patient being transported as well as the time to make the resource available again (the ambulance). Consequently, a technique for selecting the destination hospital is proposed herein including some dimensions related both to the patient (diagnosis, specialty and insurance company) and to the hospital (occupancy and closeness). The performance was evaluated based on a simulation of discrete events. It is concluded that the proposed technique provides a better PHC time in 73% of the studied cases, with a mean decrease between 40 and 80 minutes as compared to the most commonly used technique (selecting the closest hospital). This work aims to map, throughout the Catalonian (Spain) public health system and based on the normative documents, the modifications that took place from 2009. It is intended to identify the progress and/or setback in the way how the social control is being institutionalized. This endeavor prioritized the following aspects: organizational design of the public health system and the participation channels; the types and forms of the information enabling the citizen's ability to exercise control; the citizen's right of free access to public information; and the mechanisms for the citizen to be heard. Significant advancements driven towards the social control were confirmed but it is still necessary to provide the citizen with better conditions to control and evaluate the public action so that the relationship State/society be modified, to allow a qualified selection among the different aspects related to the issues of health, and to use the citizen collective action as a tool. Marine conservation has become one of the main factors in the transformation of oceans, reshaping the relations of power which define the future of such zones, with the participation of diverse agents in decision-making. This paper analyzes the creation of Marine Protected Areas in the Corcovado Gulf in Chilean Northern Patagonia. It reveals that these Marine Protected Areas are i) a response to a controversial process of territorial transformation, aggravated by the expansion of the salmon industry and calls for the conservation of biodiversity; ii) the emergence of a conservationist assemblage, based on the idea of collective efforts, and its involvement in a multi-agent network of governance. Introduction. The steady dynamics of science, technology and innovation plus the volume of information that these subjects have, represent a challenge to the research groups endeavor and to the commitment of the later ones to be knowledge developing units that give significant outputs to its social and academic settings. Therefore, there must be a planning alternative that supports the decisionmaking processes in order to give it a better knowledge founding, to reduce the possible risks and to think ahead the possible changes. This alternative must be supported by an organized, selective and systematic procedure, such as the technological surveillance. Objective. To create a descriptive literature review regarding technological surveillance and a proposal for the technological surveillance method applied to the management of scientific activity of the Estudio Genetico de Enfermedades Complejas (EGEC - Genetic Study for Complex Diseases) research group. Methodology. Descriptive literature review. The following academic databases were consulted: Medline, SciELO, Ebsco e IEEE and Google Scholar search engine. The combination of keywords used when consulting the academic databases, was "vigilancia tecnológica/technological surveillance", "salud/health", "enfermedad/disease", "ejemplos en salud", "universidad" and "investigación". Articles of the last five years are included, while articles that only had to deal with the results dissemination of technological surveillance exercises, were excluded. Based on the literature review analysis of results, a technological surveillance model is proposed which its objective is to manage the EGEC research group's scientific activities. In order to do so, a stages, inputs and funds diagrams that integrated the surveillance model, were created. Thus, the already mentioned diagrams included a cancer pilot test as a validation strategy, and with the test results, the model was subjected to additional adjustments. Results. The Spanish standard UNE 166006:2011 created by the Spanish Association for Standarization and Certification (AENOR), was taken as an international reference whilst the program España y sus Regiones Intercambian Conocimiento con Antioquia (ERICA) and the methodology for technological surveillance Innovaciones a partir de la Vigilancia Tecnológica (InnoViTech), were taken as national referents. Additionally, conceptual stages, good practices for processes execution and defined budget for the development of technological surveillance, are established. The main components for the proposed model include needs identification, plus the planning, collection, auditing, analysis, documentation, and information spreading processes. Conclusions. The proposed model works as a foundation for developing a knowledge management strategy of a research group. Also, the model highlights the importance of climbing towards the usage of competitive intelligence systems within itself. [ABSTRACT FROM AUTHOR]

- Abstract
The nurse should adopt the managerial administrative practice during the decision-making processes, aiming to achieve better health care results, with the use of permanent education in health so that knowledge production occurs among the team, based on the problems experienced, qualifying and forming the team. The objective was to analyze the nurses' perception about the use of Permanent Education in Health in the management of care. This is an exploratory, descriptive, qualitative approach, in which 15 supervising nurses from a public hospital in the South of Brazil participated. Data were collected from May to July 2014, through the Culture Circle. From the results of the analysis emerged two thematic axes: Permanent Education in Health as a strategy for the improvement of the management of the care indicated that the Permanent Education consists of a valuable strategy to the improvement of the management of the care and; Possibilities and weaknesses in the operationalization of the Permanent Education in Health, in which the intention of the nurses was verified in implanting the logic of the Permanent Education in the institution. However, centralized management was pointed out as a factor that hinders this initiative. It is concluded that despite the institutional and professional fragilities, nurses perceive Permanent Education as a potent strategy to improve the management of care and seek to implement it in the service. Introduction: Decision making (DM) is among the most important abilities for everyday functioning. However, the most widely used measures of DM come from behavioral paradigms, whose ecological validity and standalone use has been criticized in the literature. Though these issues could be addressed by the use of DM questionnaires as a complementary assessment method, no such instruments have been validated for use in Brazilian Portuguese. Therefore, the aim of this study was to conduct the translation and validation of the Melbourne Decision Making Questionnaire (MDMQ) for use in a Brazilian population. Methods: The adaptation of the MDMQ involved the following steps: translation, back-translation, expert review and pilot study. These steps were followed by factor analysis and internal consistency measurements, which led to the exclusion of 4 items from the scale. The 18-item version of the MDMQ was then administered to a validation sample consisting of healthy adults, as well as patients with bipolar disorder (BD) and major depressive disorder (MDD). Results: The instrument displayed good internal consistency, with the hypervigilance subscale showing the lowest, though still acceptable, Cronbach's alpha value. Its factor structure was comparable to that of the original MDMQ according to confirmatory factor analysis. Nevertheless, the MDMQ was sensitive to both depression severity and the presence of MDD and BD, both of which are known to have an impact on DM ability. Conclusion: The version of the MDMQ produced in the present study may be an important addition to neuropsychological assessment batteries with a focus on DM and related abilities. Objective: To investigate clinical and laboratory characteristics associated with referral of acutely ill older adults to exclusive palliative care. Methods: A retrospective cohort study based on 572 admissions of acutely ill patients aged 60 years or over to a university hospital located in São Paulo, Brazil, from 2009 to 2013. The primary outcome was the clinical indication for exclusive palliative care. Comprehensive geriatric assessments were used to measure target predictors, such as sociodemographic, clinical, cognitive, functional and laboratory data. Stepwise logistic regression was used to identify independent predictors of palliative care. Results: Exclusive palliative care was indicated in 152 (27%) cases. In the palliative care group, in-hospital mortality and 12 month cumulative mortality amounted to 50% and 66%, respectively. Major conditions prompting referral to palliative care were advanced dementia (45%), cancer (38%), congestive heart failure (25%), stage IV and V renal dysfunction (24%), chronic obstructive pulmonary disease (8%) and cirrhosis (4%). Major complications observed in the palliative care group included delirium (p<0.001), infections (p<0.001) and pressure ulcers (p<0.001). Following multivariate analysis, male sex (OR=2.12; 95%CI: 1.32-3.40), cancer (OR=7.36; 95%CI: 4.26-13.03), advanced dementia (OR=12.6; 95%CI: 7.5-21.2), and albumin levels (OR=0.25; 95%CI: 0.17-0.38) were identified as independent predictors of referral to exclusive palliative care. Conclusion: Advanced dementia and cancer were the major clinical conditions associated with referral of hospitalized older adults to exclusive palliative care. High short-term mortality suggests prognosis should be better assessed and discussed with patients and families in primary care settings [ABSTRACT FROM AUTHOR]

This essay analyzes three elections that had unexpected results: the exit of the United Kingdom from the European Union, the rejection of the peace agreement in Colombia and the election of Trump as president of the United States. It emphasizes three aspects: the predictions were made in a situation of uncertainty based on rules or conventions, the campaigns involved strong emotional elements and the results were affected by cultural factors (identity and religious) and not only economic ones. Standard economics omits such factors although it should take them into account. [ABSTRACT FROM AUTHOR]
