The study focuses on the design and evaluation of inflatable abdominal binders for managing Orthostatic Hypotension. Orthostatic hypotension is a condition characterized by a significant drop in blood pressure when a person stands up, leading to symptoms such as dizziness, lightheadedness, and even fainting. The management of orthostatic hypotension typically involves a combination of pharmacological and non-pharmacological strategies. In the context of this research, an inflatable abdominal binder was designed, leveraging components that are not only economically viable but also easily obtainable. The evaluation of this device was conducted using a medical education manikin, specifically the CAE iStan manikin. The results demonstrated a correlation between the inflation values of the belt and the resulting pressure values exerted on the body. The general recommendation for an abdominal binder is to exert a pressure of 20–40 mmHg. Contrary to this, the study found that to maintain safe external pressure on the abdomen, the binder should not be inflated over 25 mmHg. This safety threshold was used as a reference point in the study, suggesting a potential need to revisit the standard recommendations for abdominal binder pressure. Further research is needed to assess the device’s effectiveness in human subjects and to potentially redefine the safe and effective pressure range for abdominal binders.

Autism rates have been reported to be increasing rapidly in industrialized societies. The pathology most often combines neurological symptoms associated with language and social impairments with gastrointestinal symptoms. This study aimed to measure differences in oral metatranscriptome and mitochondrial health between ASD children and neurotypical USA and Colombia (“Blue Zone”) children. In addition, this study aimed to determine whether using prebiotics and probiotics would change the oral microbiome and mitochondrial health of ASD children. Buccal swabs and saliva samples were obtained from 30 autistic individuals (USA) at three intervals: prior to intervention, post-prebiotic, and post-probiotic. In addition, a subject component who were neurotypical, which included individuals from the USA (30) and Colombia (30), had buccal swabbing and salivary sampling performed for metatranscriptomic and mitochondrial comparison. Significant differences were observed in the temporal data, demonstrating shifts that interventions with probiotics and polyols may have precipitated. Particular bacterial strains were significantly more prevalent in the autism group, including a strain that reduced neurotransmitter levels via enzymatic degradation. This supports the hypothesis that the microbiome may influence the occurrence and degree of autism. Verbal skills increased in six of the 30 ASD subjects following xylitol and three more after probiotic supplementation, according to both parental reports and the subjects’ healthcare providers.

The purpose of this research is to explore the biomechanical consequences of maternal injuries on fetal movements. Additionally, the research aims to comprehend the relationship between these injuries and fetal movement within the amniotic sac and to understand the extent to which the amniotic fluid can provide protection during severe injuries. The focus is on the potential impact these injuries could have on surgical procedures and preventative strategies. Using advanced computational simulations, the study investigates how various maternal injuries can influence the behavior of amniotic fluid and the subsequent stress exerted on fetal development. The findings suggest that maternal injuries can induce stress, primarily affecting the posterior regions of the fetus and the umbilical cord, depending on the boundary and initial conditions. This stress is associated with fetal displacement within the amniotic sac. While the amniotic fluid provides a certain level of protection, its limitations become apparent during severe injuries. These insights have implications for the field of surgery, particularly fetal procedures. They underscore the need for improved protective measures and the development of personalized obstetric and neonatal care strategies. Moreover, the study highlights the potential of computational simulations in aiding surgeons. These simulations can provide a more accurate understanding of the critical areas to focus on during surgical procedures, thereby enhancing the precision and safety of these operations.

Mental health today includes much more than the treatment of psychiatric disorders. More and more interventions aim to bring mental health support closer to people and psychotherapeutic interventions to people with somatic conditions. Since the treatment of people with metabolic syndrome and diabetes mellitus type 2 also requires a change in lifestyle, mental health has a prominent role. This overview paper wants to offer a solution after recognizing the given patterns where psychotherapy certainly has a significant and irreplaceable role. Precisely because of this phenomenon, psychological eating patterns associated with diabetes mellitus and hence metabolic syndrome should be studied, and attempts should be made to uncover patterns in occurrence. The aim of this study is to review the literature and consider the connection among diabetes mellitus, metabolic syndrome, and psychological eating patterns such as emotional and compulsive eating, as well as through the lens of food addiction. Furthermore, we have attempted to uncover the role of psychiatry and psychotherapy in the treatment of diabetes mellitus and metabolic syndrome and delve into the complexity of recognizing these patterns and emphasize the importance of a multidisciplinary approach in the treatment of diabetes mellitus and metabolic syndrome.

During its existence, the retail industry underwent profound transformations. Over the past three decades the retail industry has incorporated new technologies, namely Artificial Intelligence, Internet of Things and Big Data Analytics. All these advancements deeply intensified competition in global retailing. The COVID-19 pandemic had a significant impact on the businesses of global retailers. The purpose of the paper is to present and briefly analyse the evolution of the largest global retailers in terms of their retail revenues in the COVID-19 pandemic period 2020-2021. In this respect, a qualitative research method was used. The results of the paper show that global retailers are dominating the retail industry. They obtained impressive retail revenues even during the COVID-pandemic period 2020-2021. Walmart, the American giant, was by far the largest global retailer in that period.

Today’s dynamic and changing business environment raises numerous challenges for any business organization, irrespective of its size and industry. The Fourth Industrial Revolution has brought numerous advancements in the information and communications technologies which require continuous innovation. This is why corporations, especially multinational and transnational, have heavily invested in research and development activities in order to increase their efficiency. The rapid spread of digital technologies has determined corporate management to re-evaluate its approach towards innovation and, therefore, corporations began to establish innovation ecosystems. The goal of the paper is to illustrate and analyse the hierarchy of America’s most innovative corporations in the period 2022-2023. The author applied a qualitative research method in order to achieve the aim of the paper. The paper demonstrates that innovation, in general, and corporate innovation, in particular, represent key factors in ensuring business success. The hierarchy of the first ten American most innovative corporations in the period 2022- 2023 was clearly dominated by the corporations from the IT&C sector.

Abstract In this work, we address the impact of a small lepton number violation on charged lepton electric dipole moments — EDMs. Low-scale seesaw models protected by lepton number symmetry and leading to pseudo-Dirac pairs in the neutrino heavy spectrum provide a natural explanation for the smallness of neutrino masses with potentially testable consequences. Among which, it was thought that the small mass gap in each pair of pseudo-Dirac neutrinos may induce important contribution to the charged lepton EDMs. Recently, it has been shown that the contribution from some of the Feynman diagrams to charged lepton EDMs exactly cancel by virtue of the Ward-Takahashi identity in quantum electrodynamics. We thus consider here the Standard Model minimally extended with pairs of pseudo-Dirac sterile fermions and derive the complete analytical formula at two loops for the charged lepton EDMs. In addition, we numerically evaluate the order of the predicted EDMs consistent with the experimental bounds and constraints such as neutrino oscillation data, charged lepton flavour violating processes, sterile neutrino direct searches, meson decays, sterile neutrino decays, and cosmological and astrophysical observations. We find that, in the minimal setup accommodating neutrino data (masses and mixings) with only two pseudo-Dirac pairs, the predicted electron EDM is O 10 − 36 $$ \mathcal{O}\left({10}^{-36}\right) $$ e cm, at most, which is much smaller than the current experimental bound and even future sensitivity. Hopefully, the electron EDM might reach future sensitivity, once extra pseudo-Dirac neutrinos are taken into account. The analytical formulae we derive are generic to any model involving pseudo-Dirac pairs in the heavy neutrino spectrum.

Abstract Voltage-sensing phosphatase (VSP) exhibits voltage-dependent phosphatase activity toward phosphoinositides. VSP generates a specialized phosphoinositide environment in mammalian sperm flagellum. However, the voltage-sensing mechanism of VSP in spermatozoa is not yet characterized. Here, we found that VSP is activated during sperm maturation, indicating that electric signals in immature spermatozoa are essential. Using a heterologous expression system, we show the voltage-sensing property of mouse VSP (mVSP). The voltage-sensing threshold of mVSP is approximately −30 mV, which is sensitive enough to activate mVSP in immature spermatozoa. We also report several knock-in mice in which we manipulate the voltage-sensitivity or electrochemical coupling of mVSP. Notably, the V312R mutant, with a minor voltage-sensitivity change, exhibits abnormal sperm motility after, but not before, capacitation. Additionally, the V312R mutant shows a significant change in the acyl-chain profile of phosphoinositide. Our findings suggest that electrical signals during sperm maturation are crucial for establishing the optimal phosphoinositide environment in spermatozoa.

Abstract Background Lymphatic vessels (LVs) play a crucial role in immune reactions by serving as the principal conduits for immune cells. However, to date, no study has analyzed the morphological changes in the LVs of patients with biliary atresia (BA). In this study, we aimed to determine the morphological changes in the LVs irrigating the liver in patients with BA, elucidate their correlations with the morphology of the portal vein (PV) branches, and discuss their etiopathogenetic significance. Methods Morphometric analyses of liver biopsy specimens from patients treated between 1986 and 2016 were performed. The parameters measured were as follows: the whole liver area of the specimen, fibrotic area, number of LVs, LVs without patent lumen (designated as Ly0) and PV branches, and diameters of the LVs with patent lumen and the PVs. Results The numbers of LVs, Ly0, and PV branches per unit area of the whole liver specimen were significantly higher in patients with BA than in control participants with liver disease and those with normal livers. However, no correlation was observed between the fibrotic area and the average diameter of LVs or PVs, and between the fibrotic area and the number of LVs or PV branches. Furthermore, no correlation was observed between the total number of LVs and the number of PV branches. Conclusions The present study showed a significant increase in the number of total LVs and Ly0, characterized by a high Ly0 to total LVs ratio, suggesting that lymphangiogenesis occurs in the liver of patients with BA.

Background Large numbers of young people worldwide, especially in the Global South, wish to migrate but lack the capacity to do so, with potentially detrimental consequences for their well-being and mental health. Termed ‘involuntary immobility’, this phenomenon is numerically larger than migration, but remains for now a largely underexplored area of research. Focusing on young Senegalese living in Casamance, this paper contributes to the limited literature on the implications of immobility for subjective well-being. It does so by (i) considering different degrees and types of involuntary immobility and their intersections, (ii) laying out the implications of (interacting) social and spatial immobilities for well-being with particular attention to youth’s agency in navigating a lack of capabilities, and (iii) by accounting for heterogenous experiences taking a gendered approach. Methods For this study, 35 semi-structured interviews were conducted with 18- to 39-year-olds in Ziguinchor region, recruited through purposive and snowball sampling. Thematic analysis was employed on verbatim transcriptions of audio-recorded interviews. Results Our study reveals a nuanced reality where aspirations to (temporarily) move abroad coexist with aspirations to stay in Casamance. Participants’ life aspirations and overarching projects clash, however, both with a lack of capabilities to move abroad and to enact these locally. This pervasive immobility decreases life satisfaction and generates negative emotions, such as stress, anxiety, discouragement, and distress. Despite these obstacles, our findings also underscore the agency and resourcefulness displayed by the youth as they navigate their limited control over life choices and paths. Conclusions Involuntary spatial immobility exacerbates the dominant experience of social immobility, magnifying its effect on youth’s well-being, revealing a previously unacknowledged phenomenon. Our findings further emphasize the pressing need for a more cohesive alignment between migration policies and information campaigns on one hand and the real experiences and challenges encountered by their intended audience on the other.

Abstract Background Communication is a multifaceted process, ranging from linear, one-way approaches, such as transmitting a simple message, to continuous exchanges and feedback loops among stakeholders. In particular the COVID-19 pandemic underscored the critical need for timely, effective and credible evidence communication to increase awareness, levels of trust, and evidence uptake in policy and practice. However, whether to improve policy responses in crises or address more commonplace societal challenges, comprehensive guidance on evidence communication to decision-makers in health policies and systems remains limited. Our objective was to identify and systematize the global evidence on frameworks, guidance and tools supporting effective communication of research evidence to facilitate knowledge translation and evidence-informed policy-making processes, while also addressing barriers and facilitators. Methods We conducted a rapid scoping review following the Joanna Briggs Manual. Literature searches were performed across eight indexed databases and two sources of grey literature, without language or time restrictions. The methodological quality of included studies was assessed, and a narrative-interpretative synthesis was applied to present the findings. Results We identified 16 documents presenting either complete frameworks or framework components, including guidance and tools, aimed at supporting evidence communication for policy development. These frameworks outlined strategies, theoretical models, barriers and facilitators, as well as insights into policy-makers’ perspectives, communication needs, and preferences. Three primary evidence communication strategies, comprising eleven sub-strategies, emerged: “Health information packaging”, “Targeting and tailoring messages to the audience”, and “Combined communication strategies”. Based on the documented barriers and facilitators at micro, meso and macro levels, critical factors for successful communication of evidence to policy-makers were identified. Conclusions Effective communication is indispensable for facilitating knowledge translation and evidence-informed policy-making. Nonetheless gaps persist in frameworks designed to enhance research communication to policy-makers, particularly regarding the effectiveness of multiple communication strategies. To advance in this field, the development of comprehensive frameworks incorporating implementation strategies is warranted. Additionally, barriers and facilitators to implementing effective communication must be recognized and addressed taking diverse contexts into consideration. Competitive ice climbing involves ascending ice and natural rock/manmade features using specialized equipment. Despite its growing popularity, there is limited knowledge regarding the relationship between ice climbers’ biomechanics and performance. The purpose of this study was to analyze spatiotemporal variables and upper-extremity joint kinematics during an elite lead ice-climbing competition. A total of 24 (16 male, 8 female) competitors participated. Video data was recorded during the ice climbing competition, and biomechanical analysis software was used to measure kinematic variables (shoulder and elbow angles) and spatiotemporal (time climbing/resting and number of moves/rests) throughout a section of the competition route. Independent t-tests examined differences between the top and bottom 50% of competitors, and correlations assessed the strength of the relationship between the measured variables and competition rank. We found a strong correlation between elbow and shoulder angles at weight bearing on the ice tool, indicating that ice climbers rely on more extended arm positions, which may decrease muscle fatigue, maintain optimal muscle fiber lengths, and keep the trunk close to the wall with lower contact forces. Additionally, we found that higher-performing ice climbers moved faster with fewer moves, which is likely due to their ability to identify specific holds as affordances to guide their movement.

Background: Severe neonatal jaundice remains common in Nigeria. Phototherapy is the most commonly used inhospital treatment for neonatal hyperbilirubinaemia. Objectives: To describe the availability and distribution of phototherapy services as an essential neonatal service in hospitals in Jos, North-Central Nigeria. Methods: This was a crosssectional study of all secondary and tertiary level health facilities in Jos-North, Jos-South and Jos- East Local government Areas (LGAs) between January and March 2015. Results: There were 30 (90.9%) secondary and three (9.1%) tertiary health facilities. Twenty – eight (84.8%) were privately owned. Fourteen (42.4%) of the 33 hospitals provided phototherapy services, of which 11 (78.6%) were private. Phototherapy services were available in all the tertiary facilities and in only 11 (36.7%) of the secondary level health facilities. Most (10; 71.4%) of the hospitals with phototherapy were located in Jos-North LGA. There were 38 phototherapy units in all the 14 hospitals that offered phototherapy, consisting of 8 (21.1%) light emitting diodes (LED) and 30(78.9%) conventional units of which 25(83.3%) were locally fabricated. In all the hospitals that provided phototherapy, jaundiced neonates were managed by physicians of whom only 12% were fulltime paediatricians. All hospitals that offered phototherapy had laboratory facilities to assay bilirubin. Conclusions: Phototherapy services in Jos, Nigeria are inadequate and are concentrated in the urban private sector. An increase in phototherapy services especially in secondary public facilities is needed.

Abstract Indigenous goats are important to many livelihoods. Despite this, they are subjected to indiscriminate crossbreeding. This affects their genetic variability which is needed to survive in current regime of climate change. The study assessed population structure and genetic diversity of Galla and Small East African goats (SEA) using pedigree information. A total of 7384 animals, 5222 of the Galla and 2162 of the SEA breeds, born between the years 1983 and 2022, were utilized. Individuals with known parents were defined as reference population. From the results, the maximum generation traced for Galla and SEA populations was 14.6 and 14.5, respectively. However, only 6 and 5 generations for Galla and SEA were complete. Pedigree completeness increased with the increasing number of generations. The average generation interval (GI) for Galla and SEA was 3.84 ± 0.04 and 4.4 ± 0.13 years. The average increase in the rate of inbreeding per generation for Galla and SEA was 0.04 and 0.05, with the effective ancestors and founders (fa/fe) ratio being same (1.00) for both breeds. Fifty percent (50%) of the genetic variability in the populations was contributed by 3 and 1 ancestor for Galla SEA, respectively. The effective population size (Ne) was 5.19 and 4.77 for Galla and SEA. Therefore, the current breeding programs should be changed to avoid future genetic bottlenecks in this population. These findings offer an opportunity to enhance the current genetic status and management of Kenyan native goats and other regions with similar production systems.

Abstract Enteric glia have been recently recognized as key components of the colonic tumor microenvironment indicating their potential role in colorectal cancer pathogenesis. Although enteric glia modulate immune responses in other intestinal diseases, their interaction with the colorectal cancer immune cell compartment remains unclear. Through a combination of single-cell and bulk RNA-sequencing, both in murine models and patients, here we find that enteric glia acquire an immunomodulatory phenotype by bi-directional communication with tumor-infiltrating monocytes. The latter direct a reactive enteric glial cell phenotypic and functional switch via glial IL-1R signaling. In turn, tumor glia promote monocyte differentiation towards pro-tumorigenic SPP1+ tumor-associated macrophages by IL-6 release. Enteric glia cell abundancy correlates with worse disease outcomes in preclinical models and colorectal cancer patients. Thereby, our study reveals a neuroimmune interaction between enteric glia and tumor-associated macrophages in the colorectal tumor microenvironment, providing insights into colorectal cancer pathogenesis.
