Background Bloodstream infection due to beta-lactamase and carbapenemase-producing gram-negative bacteria poses a substantial challenge to the effectiveness of antimicrobial treatments. Therefore, this study aimed to investigate the magnitude of beta-lactamase, carbapenemase-producing gram-negative bacteria, and associated risk factors of bloodstream infections in patients at a tertiary care hospital, in Addis Ababa, Ethiopia. Methods An institutional-based cross-sectional study was conducted with convenience sampling techniques from September 2018 to March 2019. Blood cultures were analyzed from 1486 bloodstream infection suspected patients across all age groups. The blood sample was collected using two BacT/ALERT blood culture bottles for each patient. Gram stain, colony characteristics, and conventional biochemical tests were used to classify the gram-negative bacteria at the species level. Antimicrobial susceptibility testing was carried out to screen beta-lactam and carbapenem drug-resistant bacteria. The E-test was conducted for extended-spectrum-beta-lactamase and AmpC-beta-lactamase-producers. A modified and EDTA-modified carbapenem inactivation method was conducted for carbapenemase and metallo-beta-lactamases producers. Data collected using structured questionnaires and medical records were reviewed, encoded, and cleaned using EpiData V3.1. software. The cleaned data were exported and analyzed using SPSS version 24 software. Descriptive statistics and multivariate logistic registration models were used to describe and assess factors associated with acquiring drug-resistant bacteria infection. A p-value Result Among 1486 samples, 231 gram-negative bacteria were identified; of these, 195(84.4%) produce drug-hydrolyzing enzymes, and 31(13.4%) produce more than one drug-hydrolyzing enzyme. We found 54.0% and 25.7% of the gram-negative bacteria to be extended-spectrum-beta-lactamase and carbapenemase-producing, respectively. The extended-spectrum-beta-lactamase plus AmpC-beta-lactamase-producing bacteria account for 6.9%. Among the different isolates Klebsiella pneumonia 83(36.7%) was the highest drug-hydrolyzing enzyme-producing bacteria. Acinetobacter spp 25(53.2%) was the most carbapenemase producer. Extended-spectrum-beta-lactamase and carbapenemase-producing bacteria were high in this study. A significant association between age groups and extended-spectrum-beta-lactamase producer bacterial infection was seen, with a high prevalence in neonates (p = Conclusion Gram-negative bacteria were the main pathogens responsible for drug-resistant bloodstream infections. A high percentage of extended-spectrum-beta-lactamase and carbapenemase-producer bacteria were found in this study. Neonates were more susceptible to extended-spectrum-beta-lactamase and AmpC-beta-lactamase-producer bacteria. Patients in general surgery, caesarean section delivery, and intensive care unit were more susceptible to carbapenemase-producer bacteria. The suction machines, intravenous lines, and drainage tubes play an important role in the transmission of carbapenemase and metallo-beta-lactamase-producing bacteria. The hospital management and other stakeholders should work on infection prevention protocol implementation. Moreover, special attention should be given to all types of Klebsiella pneumoniae and pan-drug resistance Acinetobacter spp transmission dynamics, drug resistance genes, and virulence factors.

BackgroundHigh penetration of smartphones and ownership among the young generation increased the utilization of mobile health applications for health management among university students more than ever. Compared to other health service provision mechanisms mobile health service has higher advantages in promoting a healthy lifestyle since it is not limited to time and space. Even though there are numerous studies conducted in the area of mobile health utilization, this phenomenon is not well studied among university students in Ethiopia, Hence this study aims to determine the level of mobile health applications utilization among health science students.MethodA cross-sectional study was carried out at Debre Markos University from April 05 to June 25, 2022 among health science students. For this study, 423 students were recruited from health Science College using a systematic sampling method. The data was gathered using a self-administered questionnaire. Using SPSS version 26 software descriptive analysis, bivariate and multivariable logistic regressions were performed. A P-value of .05 at a 95% confidence interval was considered statistically significant. The questionnaire's validity was determined by expert opinion, and by calculating its reliability using cronbach alpha (α = .78).ResultThis study revealed that more than half 59% (144/244) of the study participants have installed and utilized 1-3 health-related apps. Among the students who have mobile health applications 38.6% utilize mobile health application several times a day while only 2.1% utilized rarely. Usage history shows that 40% of the study participants utilized the applications for a year, while 15.4% of the students utilized them for more than 2 years. Easiness of applications (AOR = 4.8, 95%CI: [2.1-11.2]), skill AOR = 4.2, 95%CI: [3.8-12]) and trust (AOR = 2.8, 95%CI: [1.6-4.8]) were among the factors that were associated with mobile health applications utilization. Students were asked about the barriers that limit the use mobile health applications and self-reported barriers includes Lack of knowledge and awareness of application benefits, and Lack of adequate time to use applications.ConclusionThe study shows that mobile health applications utilization is moderate relative to previous studies. Mobile health application developers for the young generation should consider healthy lifestyle related applications like fitness and weight loss apps.

BackgroundAccording to the World health organization, neonatal mortality is defined as the death of babies within the first 28 days of their lives. The newborn period is the most vulnerable period for a child's survival, with the bulk of neonatal deaths occurring on the first day and week. According to a recent study, about a third of all newborn deaths occur within the first day of life, and nearly three-quarters occur within the first week. This study aimed to assess the determinants of neonatal mortality among neonates admitted to the neonatal intensive care unit in Dessie comprehensive and specialized hospital, northeast Ethiopia.MethodologyHealth institution-based unmatched case-control study was conducted among neonates admitted to Dessie comprehensive and specialized hospital, Ethiopia from February 01 up to March 30, 2020. After keeping cases and controls in separate frames, study participants were chosen using a simple random sampling procedure until the sample size was met. Epi data version 7.0 and SPSS version 25 were used for data entry and analysis respectively. P ≤ 0.05 was used as a cut point of statistical significance in multivariable binary logistic regression.ResultsA total of 698 (233 cases and 465 controls) participated in the study. Pregnancy induced hypertension (AOR = 3.02; 95% CI; 1.47–6.17), public hospital delivery (AOR = 3.44; 95% CI; 1.84–6.42), prematurity (AOR = 2.06; 95% CI; 1.43–2.96), being referred (AOR = 4.71; 95% CI; 3.01–7.39), and hypothermia (AOR = 2.44; 95% CI; 1.56–3.82) were determinant factors of neonatal mortality.ConclusionPregnancy-induced hypertension, public hospital delivery, prematurity, referral, and hypothermia were found to be the determinant factors of neonatal mortality. It would be important to give due attention to neonates delivered from mothers with a history of hypertensive disorder. Besides better to give due attention to neonates delivered in public health institutions, prematurely delivered, referred, and hypothermic neonates. Lastly, further research should be conducted to investigate the additional determinants of neonatal mortality.

BackgroundMenstrual irregularity can occur at any age, but it is most common among women under the age of 23 years. Menstrual irregularity is a foremost gynecological problem and a cause of anxiety to students and those close to them. These students experience monthly absenteeism, premenstrual symptoms, and a lack of concentration due to menstrual problems, all of which interfere with their education. Therefore, this study aimed to assess the magnitude of menstrual irregularity and associated factors among college students in Debre Berhan Town, North Shewa, Amhara Regional State, Ethiopia, in 2021.MethodsAn institution-based cross-sectional study was conducted from June to July 2021 in Debre Berhan town. Data was collected using self-administered questionnaires in 420 eligible female college students by systematic random sampling technique. Weight and height were measured and Body Mass Index (BMI) was calculated after data collection. Each questionnaire was checked for completeness, cleaned, coded, entered into EPI-DATA, and then transported to SPSS software. Bi-variable and multivariable logistic regression analyses were employed to determine the association of each independent variable with the dependent variable. P ≤ 0.05 were used to declare association and select predictors.ResultsIn the current study, 395 students participated with a response rate of 93.6%. Of all the total respondents, the magnitude of menstrual cycle irregularity was 33.4% (95% CI 28.6–38.2). Age < 20 years old [AOR = 3.88, 95% CI (1.25–12.18)], age of menarche ≤ 12 years [AOR = 4, 95% CI (1.18–13.9), sleeping hours ≤ 5 h [AOR= 2.26, 95% CI (1.04–4.93)], perceived stress [AOR = 2, 95% CI (1.53–3.23)] and being overweight [AOR = 2, 95% CI (1.13–3.23) were the variables significantly associated with the magnitude of menstrual irregularity.Conclusion and recommendationThis study shows that more than one-third of the college students in Debre Berhan town have experienced menstrual irregularity. Being less than 20 years old, having a history of early menarche, being overweight, and perceived stress were a variable significantly associated with menstrual irregularity. To control menstrual irregularity, girls should control their weight and lead a healthy lifestyle, including getting adequate sleep which could be aided by training on time management.

This study reports the dominant linguistic errors that occur in the written productions of Arba Minch University (hereafter AMU) students. A sample of paragraphs was collected for two years from students ranging from freshmen to graduating level. The sampled compositions were then coded, described, and explained using error analysis method. Both quantitative and qualitative analyses showed that almost all components of the English language (such as orthography, morphology, syntax, mechanics, and semantics) in learners' compositions have been affected by the errors. On the basis of surface structures affected by the errors, the following kinds of errors have been identified: addition of an auxiliary (*I was read by gass light), omission of a verb (*Sex before marriage ^ many disadvantages), misformation in word class (*riskable for risky) and misordering of major constituents in utterances (*I joined in 2003 Arba minch university). The study also identified two causes which triggered learners' errors: intralingual and interlingual. The majority of the errors, however, attributed to intralingual causes, which mainly resulted from the lack of full mastery on the basics of the English language.

It includes undernutrition (wasting, stunting, underweight, and mineral and vitamin-related malnutrition) and overnutrition.Purpose: To estimate the prevalence and identify the risk factors for undernutrition among under-five children in Debre Berhan Town, North Shewa, Ethiopia.Methods: A community-based cross-sectional study was conducted in Debre Berhan Town, from October 07, 2019 to January 24, 2020. Three hundred and eighty-five under-five children who were selected using systematic random sampling technique were included in this study. To collect data, a structured questionnaire and anthropometrical measurements were used. Data entry was done through Epi data 4.21, and for data analysis statistical package for social sciences version 20.0 was employed. Bivariate and multivariable logistic regression analysis was used to identify the factors associated with malnutrition. The statistical significance was stated at p value < 0.05 with 95% confidence intervals.Results: In the overall sample the total prevalence of undernutrition in below age-5 children was 61 (15.8%), the corresponding figures for underweight, stunted, and wasting were 26%, 41%, and 33%, respectively. Factors that contributed to under-five undernutrition were maternal illiteracy, not breastfeeding exclusively, preterm birth, absence of antenatal care, exposure to infectious diseases and diarrhea.Conclusion: There was a higher prevalence of stunting (41%), wasting (33%), and being underweight (26%) in Debre Berhan town than the national (Ethiopia) or regional (Amhara) malnutrition prevalence. Mothers’ educational status should be improved by teaching them that proper nutrition is important for their child’s growth and development. Antenatal care for all pregnant women, education on child care, infection prevention, and child feeding should be provided and further strengthened.Keywords: malnutrition, under nutrition, stunting, underweight, wasting, Debre Berhan

They are significant in planning and examine integrated governance systems that encourage quality of care and accountability. So, the current research was planned to identify the level of engagement of the board members in health care quality and factors associated.Methods: A facility-based cross-sectional study was implemented among 250 board members and data were collected by self-administered questionnaire at selected governing health centers in Addis Ababa, Ethiopia from May 1 to 30, 2018. A simple random sampling technique was used to reach the study participants. Data were entered using EpiData 3.1 software and analysis was done using SPSS 23. Adjusted odds ratio with 95% confidence interval and p-value < 0.05 was used to declare statistical significance. The level of board members’ engagement was found to be low which implies that the board members do not give appropriate attention to the quality of care.Results: Good level of engagement of board members was (50.9%) [AOR=7.11, 95% CI (3.07– 16.47)]. Most of the governing board members did not engage in the quality of health care activities. Uses quality data as a basis for recognition [AOR=7.11, 95% CI (3.07– 16.47)], review a quality scorecard or dashboard [AOR=10.83, 95% CI (3.75– 31.29)], establishing goals relating to staff satisfaction [AOR=15.42, 95% CI (6.14– 38.75)] and receiving formal training [AOR=3.42, 95% CI (1.35 − 8.62)], having a strategy relating to communication with clients [AOR= 4.95, 95% CI (2.02– 12.15)] and spending more than 20% [AOR=11.96, 95% CI (3.27– 43.83)], received training on healthcare disparities [AOR=3.81, 95% CI (1.40– 10.36)], and having a plan on quality [AOR=16.38 95% CI (5.39– 49.72)] were found to be significant predictors of level of board member engagement.Conclusion: Collectively, most of the governing board members did not engage in the quality of health care activities. Stakeholders should work on capacity building for board members using training and further follow-up. Introduction: Early diagnosis of cervical cancer at the precancerous stage is critical for effective treatment and improved patient outcomes. Objective: This study aims to explore the use of SWIN Transformer and Convolutional Neural Network (CNN) hybrid models combined with transfer learning to classify precancerous colposcopy images. Methods: Out of 913 images from 200 cases obtained from the Colposcopy Image Bank of the International Agency for Research on Cancer, 898 met quality standards and were classified as normal, precancerous, or cancerous based on colposcopy and histopathological findings. The cases corresponding to the 360 precancerous images, along with an equal number of normal cases, were divided into a 70/30 train–test split. The SWIN Transformer and CNN hybrid model combines the advantages of local feature extraction by CNNs with the global context modeling by SWIN Transformers, resulting in superior classification performance and a more automated process. The hybrid model approach involves enhancing image quality through preprocessing, extracting local features with CNNs, capturing the global context with the SWIN Transformer, integrating these features for classification, and refining the training process by tuning hyperparameters. Results: The trained model achieved the following classification performances on fivefold cross-validation data: a 94% Area Under the Curve (AUC), an 88% F1 score, and 87% accuracy. On two completely independent test sets, which were never seen by the model during training, the model achieved an 80% AUC, a 75% F1 score, and 75% accuracy on the first test set (precancerous vs. normal) and an 82% AUC, a 78% F1 score, and 75% accuracy on the second test set (cancer vs. normal). Conclusions: These high-performance metrics demonstrate the models' effectiveness in distinguishing precancerous from normal colposcopy images, even with modest datasets, limited data augmentation, and the smaller effect size of precancerous images compared to malignant lesions. The findings suggest that these techniques can significantly aid in the early detection of cervical cancer at the precancerous stage. [ABSTRACT FROM AUTHOR]

Background: Urologic emergencies are common and complications from their management are high. Simulation‐based education (SBE) is a highly effective training method, allowing trainees to learn both technical and nontechnical skills in a safe environment. Training in the management of urological emergencies is limited in many healthcare settings, so we performed a needs assessment for a urological emergencies SBE course in Ethiopia. Methods: This study presents data from a descriptive cross‐sectional, survey‐based survey of consultants and residents in the Ethiopian surgical community. The survey was disseminated using online Google Forms, through social media (WhatsApp), and to colleagues in the College of Surgeons of East, Central and Southern Africa (COSECSA) via email and social media. Results: One hundred‐seven results were received; two were discarded due to incomplete data. Fifty three of the respondents were general surgeons and 38 of the respondents were urologists. Sixty nine respondents strongly agreed that simulation‐based training was important for first‐year surgical residents, whereas twenty‐five respondents agreed and nine respondents strongly disagreed; one respondent disagreed and one was neutral. Eighty seven respondents suggested a 3‐day training course, whereas 17 respondents suggested a two‐day course. More than 80 of the respondents rated training in the management of acute urinary retention, acute scrotum, urethral, and suprapubic catheterization as extreme or very important and 79 respondents wanted education about urologic trauma and Fournier's gangrene. Conclusion: Surgical and urology residents in Ethiopia have expressed a need for, and a strong interest in, simulation‐based urological emergency training. [ABSTRACT FROM AUTHOR]

This study presents an antimicrobial polymeric material comprising cellulose nanocrystals (CNCs) grafted with an antimicrobial oligomer, polyhexamethylene guanidine hydrochloride (PHGH). A one‐pot reaction is implemented to graft PHGH onto CNCs, creating a non‐leaching and nano‐sized antimicrobial additive (mCNC). The mCNC is subsequently incorporated into a model polymer, polylactic acid (PLA), with concentrations of 2.5 to 10 wt.% and tested for its antimicrobial activity during dynamic and static contact with Escherichia coli and Bacillus subtilis bacteria. The grafting of PHGH onto CNC is confirmed with Fourier transform infrared spectroscopy (FTIR), X‐ray spectroscopy (XPS) and elemental analysis. The effect of mCNC incorporation at various loading levels on the morphology and physicomechanical properties of PLA is investigated with polarized optical microscope (POM), scanning electron microscope (SEM), dynamic mechanical analysis (DMA), tensile testing, and differential scanning calorimetry (DSC). In terms of the antimicrobial action, the films exhibited potent efficacy against Gram‐positive bacteria (B. subtilis), with growth inhibition of 3.97 to 4.66‐log reduction. However, 10 wt.% of mCNC loading is needed to achieve a significant bacterial inhibition (>6.24‐log) of the Gram‐negative bacteria (E. coli). Overall, the incorporation of PHGH grafted CNCs in polymers provided a non‐leaching antimicrobial film that has potential application in food packaging. [ABSTRACT FROM AUTHOR]

Aging is a multifaceted process impacting cells, tissues, organs, and organ systems of the body. Like other systems, aging affects both the adaptive and the innate components of the immune system, a phenomenon known as immunosenescence. The deregulation of the immune system puts elderly individuals at higher risk of infection, lower response to vaccines, and increased incidence of cancer. In the Western world, overnutrition has increased the incidence of obesity (linked with chronic inflammation) which increases the risk of metabolic syndrome, cardiovascular disease, and cancer. Aging is also associated with inflammaging a sterile chronic inflammation that predisposes individuals to age‐associated disease. Genetic manipulation of the nutrient‐sensing pathway, fasting, and calorie restriction (CR) has been shown to increase the lifespan of model organisms. As well in humans, fasting and CR have also been shown to improve different health parameters. Yet the direct effect of fasting and CR on the aging immune system needs to be further explored. Identifying the effect of fasting and CR on the immune system and how it modulates different parameters of immunosenescence could be important in designing pharmacological or nutritional interventions that slow or revert immunosenescence and strengthen the immune system of elderly individuals. Furthermore, clinical intervention can also be planned, by incorporating fasting or CR with medication, chemotherapy, and vaccination regimes. This review discusses age‐associated changes in the immune system and how these changes are modified by fasting and CR which add information on interventions that promote healthy aging and longevity in the growing aging population. In this investigation, the impact of reducing agent concentration on the synthesis of zinc oxide nanoparticles (ZnO NPs) was examined. During the synthesis, an assessment of ionic conductivity was carried out, revealing a significant increase in conductivity prior to the introduction of the reducing agent, followed by a sharp decrease upon its addition. Characterization of the ZnO NPs involved ultraviolet‐visible spectroscopy, scanning electron microscopy, Fourier‐transform infrared spectroscopy, and, X‐ray diffraction analysis. The outcomes suggest that the characteristics of the ZnO NPs are influenced by the concentration of the reducing agent during the synthesis process. Notably, the ZnO NPs synthesized with a higher concentration of reducing agent exhibited a narrower optical band gap and increased surface energy. Furthermore, employing a concentration of 0.5 v/v resulted in the rapid production of NPs with relatively uniform sizes. Conversely, concentrations below 0.5 v/v lead to slow formation, while concentrations exceeding 0.5 v/v yielded non‐uniform NPs. [ABSTRACT FROM AUTHOR]

Abstract Background Plant-based mosquito control methods may use as a supplementary malaria vector control strategy. This study aimed to evaluate the effect of smoking ethno-medicinal plants on indoor density and feeding activity of malaria vectors at early hours of the night and its residual effect after midnight in southern Ethiopia. Methods Both field and tent trials were conducted to evaluate the impact of smoking Juniperus procera leaves, Eucalyptus globulus seeds and Olea europaea leaves in Kolla Shara Village from July 2016 to February 2017. For the field trial, five grass-thatched traditional huts (three for ethno-medicinal plants and two as control [only charcoal smoking and non-charcoal smoking]) were used. Indoor host-seeking mosquitoes were collected by CDC light traps. A Latin square design was employed to minimize the bias due to the variation in house location and different sampling nights. For the tent experiment, 25 3–5-day-old starved wild female Anopheles mosquitoes reared from the larvae were released into the tents where a calf was tethered at the mid-point of each tent. Results A total of 614 Anopheles mosquitoes belonging to 5 species were collected from 5 huts, of which 93.4% was An. arabiensis; O. europaea, E. globulus and J. procera reduced the indoor density of An. arabiensis, with the mean percentage drop of 80%, 73% and 70%, respectively. BackgroundPost-abortion contraceptive utilization is the initiation and use of family planning methods at the time of management of abortion or before fertility returns. A significant discrepancy was reported regarding the prevalence and its associated factors of post-abortion contraceptive utilization in Ethiopia. So, this systematic review and meta-analysis aimed to estimate the pooled prevalence of post-abortion contraceptive utilization and its associated factors in Ethiopia.MethodsPreferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guideline was used. The databases such as PubMed, Google Scholar, Science Direct, Cochrane library, Scopus, CINAHL, Web of Science, and additional searches by using direct Google search, libraries, and preprint were searched. Joanna Briggs Institute Meta-Analysis of Statistics Assessment and Review Instrument (JBI-MAStARI) was used for critical appraisal.ResultsA total of 17 studies with 13,075 individuals were included. Of these, 14 studies with 5,719 individuals were used to estimate the prevalence. The pooled prevalence of post-abortion contraceptive utilization in Ethiopia was 63.64% (95% CI: 57.75–69.53). The subgroup analysis reported the highest prevalence of post-abortion contraceptive utilization in a study conducted in Addis Ababa (77.40%), a study published in 2015–2021 (66.15%), and among studies with a sample size >400 (66.84%). The pooled odds ratio (OR) of post-abortion contraceptive utilization for the mothers who had post-abortion family planning counseling was 4.15 (95% CI = 1.30, 13.2), and history of family planning utilization was 4.28 (95% CI = 2.66, 6.89).ConclusionsThe pooled prevalence of utilization of post-abortion contraceptives in this meta-analysis remains low. Post-abortion family planning counseling and the history of the utilization of modern family planning methods were significantly associated with the practice of post-abortion contraceptives. The Ministry of Health should encourage post-abortion family planning utilization, making more efforts on post-abortion contraceptive counseling. Health facilities should work hard to strengthen the family planning counseling service, especially focusing on those who had no previous family planning utilization.

Even though there are positive inclinations towards the continuum of maternity care, the problem is still significant. So, the purpose of this study was to assess the utilization of continuum maternity care and associated factors among women who gave birth in the last 12 months before the data collection period in Debre Berhan town.Methods: A community-based cross-sectional study was conducted from February 17 to March 15/2020. The respondents were selected by using the cluster sampling technique. Face-to-face interview was used for data collection. EpiData software version 3.1 was used for data entry and exported to SPSS version 21 for further analysis. In multivariable logistic regression, a statistically significant association has declared a p-value < 0.05.Results: In this study, the proportion of women who completed the continuum of maternity care was 37.2% (95% CI: 33.4– 41.1). Completing primary education (AOR: 2.73, 95% CI: 1.17– 6.38), secondary education (AOR: 3.11, 95% CI: 1.32– 7.31), college and above educational level (AOR: 4.15, 95% CI: 1.79– 9.57), initiation of first ANC visit ≤ 16wks (AOR: 2.57, 95% CI: 1.41– 4.68), knowing key pregnancy danger signs (AOR: 1.91, 95% CI: 1.15– 3.19), and well prepared on birth and complication readiness (AOR: 1.59, 95% CI: 1.10– 2.32) were found to positively increase the chance of completing maternity care in our study area.Conclusion: Even if a higher proportion of mothers completed the continuum of maternity care in the study area than the finding at national level of 9.1% basing EDHS 2016 data, further interventions are mandatory to reach the acceptable level. Therefore, health promotion programs targeting mothers with no education and lower educational level are important to increase their awareness about the importance of completing all levels of maternity care, health education, and counseling regarding early initiation of ANC, pregnancy danger signs, and birth preparedness and complication readiness plan.Keywords: completion, continuum, maternity care, Ethiopia
