Abstract

Purpose: The primary purpose of this paper is to discuss the impact of urban design on community well-being and the integration of smart cities to emphasise long-term community planning. Design/methodology/approach: A comprehensive literature search was conducted to identify publications addressing smart city integration's effects on community well-being concerning urban design. SpringerLink, ScienceDirect, Google Scholar, and ScienceDirect were accessed for relevant information, and fifty publications were evaluated in total. The publications contain various kinds of literature on the impacts of urban design on community well-being with smart city integration. The period of the review is from 2019 to 2023. After a thorough review, we will filter out literature with redundancy and deemed unsuitable. Findings: The findings revealed that community well-being focuses on urban people's overall quality of life. In contrast, urban design emphasises urban areas' physical layout and organisation to increase livability, and smart cities use technology to improve urban services and sustainability. Research limitations/implications: Through a rigorous, comprehensive review, this study depicts the impact of urban design on community well-being and the integration of smart cities to emphasise long-term community planning. Practical implications: This study offers several components used in community well-being, the impact of urban design and the integration of smart cities in the community. Originality/value: To the best of the author's knowledge, little effort has been devoted to the impact of urban design on community well-being with the smart city integration. This research helps to close the gap. Abstract

Abstract Aims Heart failure (HF) is a growing health problem, yet there are limited data on patients with HF in Malaysia. The Malaysian Heart Failure (MY‐HF) Registry aims to gain insights into the epidemiology, aetiology, management, and outcome of Malaysian patients with HF and identify areas for improvement within the national HF services. Methods and results The MY‐HF Registry is a 3‐year prospective, observational study comprising 2717 Malaysian patients admitted for acute HF. We report the description of baseline data at admission and outcomes of index hospitalization of these patients. The mean age was 60.2 ± 13.6 years, 66.8% were male, and 34.3% had de novo HF. Collectively, 55.7% of patients presented with New York Heart Association (NYHA) Class III or IV; ischaemic heart disease was the most frequent aetiology (63.2%). Most admissions (87.3%) occurred via the emergency department, with 13.7% of patients requiring intensive care, and of these, 21.8% needed intubation. The proportion of patients receiving guideline‐directed medical therapy increased at discharge (84.2% vs. 93.6%). The median length of stay (LOS) was 5 days, and in‐hospital mortality was 2.9%. Predictors of LOS and/or in‐hospital mortality were age, NYHA class, estimated glomerular filtration rate, and comorbid anaemia. LOS and in‐hospital mortality were similar regardless of ejection fraction. Conclusions The MY‐HF Registry showed that the HF population in Malaysia is younger, predominantly male, and ischaemic‐driven and has good prospects with hospitalization for optimization of treatment. These findings suggest a need to reassess current clinical practice and guide resource allocation to improve patient outcomes.

Abstract

Objective: Double-J (DJ) ureteric stents are commonly placed perioperatively for semirigid or flexible ureteroscopic renal surgery. It is believed that lesser stent material within the bladder mitigates stent-related symptoms. This study aimed to evaluate the J-Fil ureteral stent, a single pigtail suture stent compared with conventional DJ stent in relation to stent symptoms in an Asian population undergoing ureterorenal intervention. Methods: Based on internal audit committee recommendation approval, the records of 50 patients retrieved, available data of 41 patients who were prospectively enrolled into two groups (Group 1 [J-Fil stent group], n=21 and Group 2 [DJ stent group], n=20) between August 2020 to January 2021, were analysed. Parameters compared were nature of procedure, stone location and size, ease of deployment or removal, and complications. A modified universal stent symptom questionnaire was used to assess morbidity of stent symptoms within 48 h of insertion and at removal. Results: Both groups had similar median age, distribution in male to female ratio, and stone size. The overall median universal stent symptom questionnaire score at insertion was similar for bladder pain, flank or loin pain, and quality of life between Group 1 and 2; however, at removal Group 1 fared significantly better than Group 2, especially for flank or loin pain and pain at voiding. Both groups had similar ease in insertion with no hospital readmissions. Conclusion: Our audit favoured the single pigtail suture stent in Asian ureters in mitigating stent-related issues. It showed a good safety profile with easy deployment and removal. It promises a new standard in stenting.

Abstract. Background. Emphysematous pyelonephritis (EPN) is a necrotizing infection of the kidney and the surrounding tissues associated with considerable mortality. We aimed to formulate a score that classifies the risk of mortality in patients with EPN at hospital admission. Materials and methods. Patients diagnosed with EPN between 2013 and 2020 were retrospectively included. Data from 15 centers (70%) were used to develop the scoring system, and data from 7 centers (30%) were used to validate it. Univariable and multivariable logistic regression analyses were performed to identify independent factors related to mortality. Receiver operating characteristic curve analysis was performed to construct the scoring system and calculate the risk of mortality. A standardized regression coefficient was used to quantify the discriminating power of each factor to convert the individual coefficients into points. The area under the curve was used to quantify the scoring system performance. An 8-point scoring system for the mortality risk was created (range, 0–7). Results. In total, 570 patients were included (400 in the test group and 170 in the validation group). Independent predictors of mortality in the multivariable logistic regression were included in the scoring system: quick Sepsis-related Organ Failure Assessment score ≥2 (2 points), anemia, paranephric gas extension, leukocyte count >22,000/μL, thrombocytopenia, and hyperglycemia (1 point each). This research aims to quantify the spatial pattern of urban land use/land cover (LULC) change while considering environmental effects. This paper integrates historical Landsat imagery, The Environment for Visualizing Images (ENVI), geographical information system (GIS), and socioeconomic data to determine the spatial–temporal urban LULC dynamics and the conversion of LULC in response to the rapid urbanization from 1992 to 2022. Principle component analysis and multiple linear regression are used to determine and model the relationship between the socioeconomic factors and the changes for identifying the driving forces. The results indicate that impervious surfaces have exponentially increased, expanding more than two times from 2,348 to 4,795 km2, in contrast to bare lands, which drastically declined by 95%, from 1,888 to 87 km2. Water bodies have always been relatively fewer, at approximately 100 km2. In addition, the majority of farmland in Jinan City is concentrated in the northern region with a steady area in the range of 2,100–2,900 km2, while the majority of woodland located in the southern region declined from 3,774.52 km2 (37%) to 3,088.28 km2 (30%). Economic development, population growth, and climate change are the primary factors that have an obvious impact on LULC changes. Abstract

In this work, the Solar Cell Capacitance Simulator (SCAPS-1D) is employed to evaluate the characteristics of CdTe thin films with ZnTe as the Back Surface Field (BSF) layer and estimate the effective copper doping ratio at both the atomic scale and the device operational response perspective. The electrical characteristics of ZnTe, at varying levels of copper doping, were derived using density functional theory (DFT) by applying the generalized gradient approximation (GGA) and Hubbard U corrections (DFT+U). The performance of ZnTe with different Cu concentrations as a BSF layer was evaluated by analysing the values of four key parameters that are open circuit voltage (VOC), short circuit current density (JSC), fill factor (FF), and conversion efficiency (η). The results indicate that an increase in Cu concentration from 0% to 3%, 6%, 10%, and 12% resulted in a reduction of the energy band gap. Specifically, the energy band gap decreased from 2.24 eV to 2.10 eV, 1.98 eV, 1.92 eV, and 1.88 eV, respectively. Optimal Cu doping promotes the favourable shift in the valence band maxima (VBM) and formation of p + -ZnTe, lowering thermionic emission and improving carrier lifetime, which results in an improved ohmic contact, η = 18.73% for 10% of Cu content. Excessive doping in contrast degraded the overall device performance by forming an unmatched carrier band offset at the front interface with CdS, increasing the acceptor type defect and CdTe compensation rate. Overall, the findings suggest that incorporating a controlled level of Cu, which in this case is around 10%, promotes the efficiency and stability of the proposed CdTe device configuration to a certain extent.

Abstract Background The COVID-19 pandemic has fueled the widespread adoption of telemedicine in healthcare, particularly in Sarawak, Malaysia. This study investigates the use and acceptance of Sarawak’s inaugural multidisciplinary geriatric telemedicine service, TELEG. Methods This cross-sectional study took place at the Sarawak Heart Centre’s geriatric department from July 1, 2021, to April 30, 2022. Convenient sampling included all TELEG-enrolled patients during this period, to achieve minimum sample size of 148. TELEG’s utilization was assessed in terms of medication therapy and treatment plan optimization, as well as enhanced healthcare accessibility. Participants’ acceptance of TELEG was measured using the Service User Technology Acceptability Questionnaire (SUTAQ) administered through Google Forms. Descriptive statistics percentages illustrated the proportion of participants who found TELEG moderately to highly acceptable. Associations between baseline characteristics and overall acceptance were explored through bivariate analyses, including Pearson’s correlation test, independent t-test, and ANOVA. The influence of six SUTAQ dimensions on overall acceptance, multivariable linear regression using enter method was employed. Statistical significance was determined by p-values less than 0.5. Results Among 180 geriatric patients enrolled in TELEG during the study period, 149 agreed to participate. TELEG led to medication therapy optimization for 88.6% of participants, primarily involving dose adjustment (44.7%), de-prescribing (31.8%), and prescribing (15.9%). Additionally, 53.8% received treatment plan optimization, predominantly in the form of self-care education (56.3%), referrals for further treatment (33.8%), additional laboratory investigations (29.6%), and increased monitoring (26.8%). Among those educated in self-care (n = 40), dietary intake (27.5%), lower limb exercise (25.0%), and COVID-19 vaccination (12.5%) were the most common topics. Abstract

Recent advances in deep learning and computer vision offer an excellent opportunity to investigate high-level visual analysis tasks such as human localization and human pose estimation. Although the performance of human localization and human pose estimation has significantly improved in recent reports, they are not perfect and erroneous localization and pose estimation can be expected among video frames. Studies on the integration of these techniques into a generic pipeline that is robust to noise introduced from those errors are still lacking. This paper fills the missing study. We explored and developed two working pipelines that suited the visual-based positioning and pose estimation tasks. Analyses of the proposed pipelines were conducted on a badminton game. We showed that the concept of tracking by detection could work well, and errors in position and pose could be effectively handled by a linear interpolation technique using information from nearby frames. Abstract

Deep neural networks with lower precision weights and operations at inference time have advantages in terms of the cost of memory space and accelerator power. The main challenge associated with the quantization algorithm is maintaining accuracy at low bit-widths. We propose learned gradient linear symmetric quantization (LG-LSQ) as a method for quantizing weights and activation functions to low bit-widths with high accuracy in integer neural network processors. First, we introduce the scaling simulated gradient (SSG) method for determining the appropriate gradient for the scaling factor of the linear quantizer during the training process. Second, we introduce the arctangent soft round (ASR) method, which differs from the straight-through estimator (STE) method in its ability to prevent the gradient from becoming zero, thereby solving the discrete problem caused by the rounding process. Finally, to bridge the gap between full-precision and low-bit quantization networks, we propose the minimize discretization error (MDE) method to determine an accurate gradient in backpropagation. The ASR+MDE method is a simple alternative to the STE method and is practical for use in different uniform quantization methods. In our evaluation, the proposed quantizer achieved full-precision baseline accuracy in various 3-bit networks, including ResNet18, ResNet34, and ResNet50, and an accuracy drop of less than 1% in the quantization of 4-bit weights and 4-bit activations in lightweight models such as MobileNetV2 and ShuffleNetV2.

Abstract

Singapore’s national myopia prevention efforts have largely focused on school vision screening and public education on outdoor activities in the past two decades. Given the emergence of evidence-based myopia interventions, this policy review and analysis investigates the potential benefits and drawbacks of optometrist prescribing privileges as it has been proposed to reduce the barriers to access effective interventions, such as combined therapy (e.g., orthokeratology treatment and low-dose atropine therapy). In this policy analysis, two policy options were identified to be feasible based on evidence from a systematic literature search and they were analysed along with status quo using the Centers for Disease Control and Prevention (CDC) Policy Analysis Framework. This includes independent prescribing and supplementary prescribing, where the former entails autonomous clinical decision making, and the latter entails co-management with ophthalmological supervision. The policy review and analysis found independent prescribing the most favourable and concluded that this should be implemented in view of its benefits for the community. Public health impact is expected to be substantial due to increased patient access, reduced treatment costs, early interventions, improved treatment compliance, and reduced wait times and inconvenience. It is feasible because treatment processes can be streamlined, and it can be implemented based on existing collaborative prescribing frameworks. Economical and budgetary impact is also substantial given the direct savings generated, which can consequently help to reduce the disease burden.

Abstract

The growing risk of contracting viral infections due to high-density populations and ecological disruptions, such as climate change and increased population mobility, has highlighted the necessity for effective antiviral treatment and preventive measures against Dengue virus (DENV), Chikungunya virus (CHIKV), and Zika virus (ZIKV). Recently, there has been increasing attention on the use of probiotics as a potential antiviral option to reduce virus infections. The present study aimed to assess the immunomodulatory effects of heat-killed Lactococcus lactis strain plasma (LC-Plasma) on peripheral blood mononuclear cells (PBMCs) and its subsequent antiviral response against DENV, CHIKV, and ZIKV. To evaluate the immunomodulatory effects of LC-Plasma on PBMCs isolated from healthy individuals, PBMCs were cultured at a density of 2 × 105 cells/well and stimulated with 10 µg/mL of LC-Plasma. LC-plasma-stimulated PBMCs demonstrated elevated interferon-alpha (IFN-α) production and cluster of differentiation 86 (CD86) and human leukocyte antigen-DR isotype (HLA-DR) upregulation, potentially linked to plasmacytoid dendritic cell (pDC) activation. The replication of DENV, CHIKV, and ZIKV was dose-dependently inhibited when Huh-7 cells were stimulated with LC-Plasma-stimulated PBMC supernatant (LCP Sup). IFN-stimulated gene (ISG) expression, including IFN-stimulated gene 15 (ISG15), IFN-stimulated exonuclease gene 20 (ISG20), IFN-induced transmembrane protein 1 (IFITM-1), myxovirus resistance protein A (MxA), and radical S-adenosyl methionine domain-containing protein 2 (RSAD2), was significantly upregulated in LCP Sup-stimulated Huh-7 cells. Findings from this study indicate that LC-Plasma has the potential to induce IFN-α production, leading to an enhancement in the expression of ISGs and contributing to a broad-spectrum antiviral response. Thus, LC-Plasma may serve as a rational adjunctive treatment to ameliorate viral diseases, warranting future clinical trials.

Abstract

This paper advances our understanding of how schools can become change agents capable of transforming local practice to address the challenges arising from the COVID-19 pandemic. It presents a novel application of cultural-historical activity theory to reinterpret evidence on widespread learning loss and increasing educational inequities resulting from the pandemic, and to identify scalable transformative learning opportunities through reframing the crisis as a double stimulation. By reviewing evidence of the emerging educational landscape, we first develop a picture of the new 'problem space' upon which schools must act. We develop a problem space map to serve as the first stimulus to articulate local challenges. Integrating this problem space with research on professional change, we identify conceptual tools to capture learning gaps and implement pedagogic interventions at scale, in order to enhance schools' agency in directly addressing the crisis. These tools can act as the second stimulus, enabling educators to address local challenges. We conclude by discussing the COVID-19 educational crisis as a unique stimulus for professional learning and outline the potential for durable shifts in educational thinking and practice beyond the pandemic. We argue that this unprecedented historic disruption can be harnessed as a transformative professional learning opportunity. In particular, we consider how research on professional change offers local, scalable interventions and tools that can support educators in preventing the new insights from 'slipping away' post-pandemic. Utilising the notions of boundaries and tool-mediated professional change, we examine the ways in which this disruption generates opportunities to envision alternative futures for equitable learning in school.

Abstract

Variants in RAC3, encoding a small GTPase RAC3 which is critical for the regulation of actin cytoskeleton and intracellular signal transduction, are associated with a rare neurodevelopmental disorder with structural brain anomalies and facial dysmorphism. We investigated a cohort of 10 unrelated participants presenting with global psychomotor delay, hypotonia, behavioural disturbances, stereotyped movements, dysmorphic features, seizures and musculoskeletal abnormalities. MRI of brain revealed a complex pattern of variable brain malformations, including callosal abnormalities, white matter thinning, grey matter heterotopia, polymicrogyria/dysgyria, brainstem anomalies and cerebellar dysplasia. These patients harboured eight distinct de novo RAC3 variants, including six novel variants (NM_005052.3): c.34G > C p.G12R, c.179G > A p.G60D, c.186_188delGGA p.E62del, c.187G > A p.D63N, c.191A > G p.Y64C and c.348G > C p.K116N. We then examined the pathophysiological significance of these novel and previously reported pathogenic variants p.P29L, p.P34R, p.A59G, p.Q61L and p.E62K. In vitro analyses revealed that all tested RAC3 variants were biochemically and biologically active to variable extent, and exhibited a spectrum of different affinities to downstream effectors including p21-activated kinase 1. We then focused on the four variants p.Q61L, p.E62del, p.D63N and p.Y64C in the Switch II region, which is essential for the biochemical activity of small GTPases and also a variation hot spot common to other Rho family genes, RAC1 and CDC42. Acute expression of the four variants in embryonic mouse brain using in utero electroporation caused defects in cortical neuron morphology and migration ending up with cluster formation during corticogenesis. Notably, defective migration by p.E62del, p.D63N and p.Y64C were rescued by a dominant negative version of p21-activated kinase 1. Our results indicate that RAC3 variants result in morphological and functional defects in cortical neurons during brain development through variant-specific mechanisms, eventually leading to heterogeneous neurodevelopmental phenotypes.

189. Student Values and Wellbeing in Mathematics Education: Perspectives of Chinese Primary Students

Abstract

Around the world the prevalence of mathematics anxiety and disengagement point to a poor sense of student 'mathematical wellbeing'. Mathematical wellbeing is defined here as the fulfilment of one's ultimate or core values, accompanied by positive feelings and functioning in mathematics education. Yet student wellbeing and how to support it in specific school subjects including mathematics is vastly under-researched. Also, despite outperforming other countries in international mathematics assessments, East Asian students generally appear to experience poor affect and high academic burden in mathematics education, which are indicative of poor mathematical wellbeing. In this context, this explorative and descriptive study investigates the values that are associated with the mathematical wellbeing of a cohort of Chinese primary school students. 258 Grade three students in Chengdu, China responded to a questionnaire which explored teacher-nominated mathematics learning moments that students valued most for their mathematical wellbeing. Findings pointed to students valuing all the learning moments to some extent, with good grades, fun and interest, mathematics-themed videos, and teacher praise rated most commonly for wellbeing. The values underlying these learning moments are similar to the seven mathematical wellbeing dimensions found in Australian classrooms. The findings also highlight the cultural subjectivities involved, in which the same classroom actions can reflect different underlying values across cultures, and in which a particular value can be espoused in different ways in different places. We also look ahead to suggest how teachers might support students' mathematical wellbeing, by recognising and then fulfilling their students' values in the mathematics classroom.

194. Online Assessment Misconduct Detection using Internet Protocol and Behavioural Classification

Abstract

With the recent prevalence of remote education, academic assessments are often conducted online, leading to further concerns surrounding assessment misconducts. This paper investigates the potentials of online assessment misconduct (e-cheating) and proposes practical countermeasures against them. The mechanism for detecting the practices of online cheating is presented in the form of an e-cheating intelligent agent, comprising of an internet protocol (IP) detector and a behavioural monitor. The IP detector is an auxiliary detector which assigns randomised and unique assessment sets as an early procedure to reduce potential misconducts. The behavioural monitor scans for irregularities in assessment responses from the candidates, further reducing any misconduct attempts. This is highlighted through the proposal of the DenseLSTM using a deep learning approach. Additionally, a new PT Behavioural Database is presented and made publicly available. Experiments conducted on this dataset confirm the effectiveness of the DenseLSTM, resulting in classification accuracies of up to 90.7%.

Abstract

The inner operations of the human brain as a biological processing system remain largely a mystery. Inspired by the function of the human brain and based on the analysis of simple neural network systems in other species, such as Drosophila, neuromorphic computing systems have attracted considerable interest. In cellular-level connectomics research, we can identify the characteristics of biological neural network, called population, which constitute not only recurrent fullyconnection in network, also an external-stimulus and selfconnection in each neuron. Relying on low data bandwidth of spike transmission in network and input data, Spiking Neural Networks exhibit low-latency and low-power design. In this study, we proposed a configurable population-based digital spiking neuromorphic processor in 180nm process technology with two configurable hierarchy populations. Also, these neurons in the processor can be configured as novel models, integer quadratic integrate-and-fire neuron models, which contain an unsigned 8-bit membrane potential value. The processor can implement intelligent decision making for avoidance in real-time. Moreover, the proposed approach enables the developments of biomimetic neuromorphic system and various low-power, and low-latency inference processing applications.
