This resource is a comprehensive tutorial module for intravenous fluid resuscitation and blood product replacement during the management of postpartum hemorrhage. The module was prepared for use in residencies with significant obstetrical training but is appropriate for any individual seeking detailed information on fluid and blood product resuscitation in the management of postpartum hemorrhage. It can be used in conjunction with postpartum hemorrhage simulation, in support of didactic sessions, or as a stand-alone discussion or review of the subject matter. The presentation contains a narrated PowerPoint slide set with embedded summary tables. There is a transcription of all audio narration so that the resource is usable by the hearing impaired. The module was developed as part of a comprehensive blended learning curriculum for teaching the management of postpartum hemorrhage. The full curriculum included seven online modules followed by face-to-face teaching and learning with task trainers and a multidisciplinary simulation case. Project-associated pre- and posttesting demonstrated clear improvement in mastery of material as a result of using this module. All trainees completing the module reported that the module increased their knowledge “moderately” or “to a great extent.” Additionally, all trainees reported that they “agreed” or “strongly agreed” that they would recommend the module to another learner.

To assess women's knowledge, concerns, and willingness for adjunct high-risk human papillomavirus (HR-HPV) testing before and after an educational intervention.At the time of their annual gynecologic examination, women aged 30 years and older received an educational intervention about HR-HPV. Subjects completed preintervention and postintervention questionnaires. Demographic characteristics were summarized using frequency measures. Comparisons between the pre-education and posteducation questionnaires were performed using Fisher exact test.Fifty women completed the study. After the educational intervention, 77% of women were willing to be tested for HR-HPV. Sixty-seven percent of women would be likely to return for their annual gynecologic examination even if a Pap smear was not required for 3 years. Education statistically reduced concern regarding a positive HR-HPV result with 60% pre-education and 27% posteducation very concerned (p =.002). When surveyed about what their concerns would be if tested positive for HR-HPV, women associate future cervical cancer diagnosis (38% pre-education vs 48% posteducation, p =.903) but not partner infidelity (0%) with testing positive for HR-HPV. Knowledge concerning HPV, cervical cancer, and cervical cancer screening was statistically improved after the educational intervention in all but 2 questions.Women 30 years and older are willing to have adjunct HR-HPV testing, with education reducing their degree of concern about testing positive. Women who test positive would be most concerned about getting cervical cancer. Women would be willing to return for yearly gynecologic examinations, even if a Pap smear was not needed for 3 years. Education improves women's knowledge of HPV, cervical cancer, and cervical cancer screening, but did not allay the concern for getting cervical cancer.

The primary composite maternal outcome included: death, pulmonary edema, blood transfusion, ARDS, CVA, ATN, DIC, or liver rupture. RESULTS: Of 9960 evaluated pregnancies, 114 delivered 20 weeks and 101 had major fetal birth defects. Overall 28.2% had PAH 19.2% mild GTHN, 2.0% severe GHTN, 3.8% mild PrE, and 3.1% severe PrE. Outcome risks by PAH type (all p-values 0.001) and adjusted ORs (with no PAH as referent) are given (Table). Maternal and perinatal outcomes were elevated the most for severe PrE (2.6 to 9-fold) and least for mild GHTN (0.7 to 2.4-fold). CONCLUSION: The risks of adverse outcomes increase with severity of PAH sub-type. Mild PrE has similar perinatal risks to severe GHTN but maternal risks are greater.

Objective Several conditions are associated with increased preeclampsia (PE) risk. Whether altered maternal angiogenic factor levels contribute to risk in these conditions is unknown. Our objective was to compare angiogenic biomarker patterns in high-risk pregnancies and low-risk controls. Study Design We conducted a planned secondary analysis of a 2-center observational study of angiogenic biomarkers in high-risk women. A total of 156 pregnant women with a PE risk factor and 59 low-risk controls were studied. Serial maternal serum samples were collected during 3 gestational windows: 23-27 weeks, 28-31 weeks, and 32-35 weeks. Soluble fms-like tyrosine kinase 1 (sFlt1), soluble endoglin (sEng), and placental growth factor (PlGF) were measured by enzyme-linked immunosorbent assay. Geometric mean angiogenic biomarker levels and angiogenic ratio (sFlt1 + sEng):PlGF were compared with low-risk controls for each risk group, at each gestational window. Results Gestational biomarker patterns differed in PE risk groups as compared with low-risk controls. Women with multiple gestations had markedly higher sFlt1 and sEng at all gestational windows. Women with prior PE had higher sFlt1 and angiogenic ratio, and lower PlGF, from 28 weeks onward. Women with chronic hypertension had significantly higher angiogenic ratio for all 3 gestational windows, but differences disappeared when women with PE were excluded. Obese and nulliparous women had significantly lower PlGF, but no differences in the angiogenic ratio. Conclusion High-risk groups have altered angiogenic biomarker patterns compared with controls, suggesting that altered production or metabolism of these factors may contribute to PE risk, particularly in women with multiple gestations and prior PE.

Objective Placental soluble fms-like tyrosine kinase-1 may contribute to the pathogenesis of preeclampsia. Here we describe alterations in serum angiogenic factor levels in women with multiple gestation pregnancies, a major preeclampsia risk factor. Study Design We collected serial serum specimens from 101 pregnant women at high preeclampsia risk between 22 and 36 weeks' gestation. Soluble fms-like tyrosine kinase-1 and placental growth factor were measured by enzyme-linked immunosorbent assay. Women who had preeclampsia or gestational hypertension develop were excluded. Results Maternal soluble fms-like tyrosine kinase-1 was higher in multiple gestation (n = 20) compared with high-risk singleton (n = 81) pregnancies for each gestational age range examined. Maternal placental growth factor was significantly higher in multiple vs high-risk singletons before 31 weeks' gestation, whereas the soluble fms-like tyrosine kinase-1/placental growth factor ratio was higher in multiple vs high-risk singletons after 27 weeks. Conclusion Alterations in circulating angiogenic factors are present in women with multiple gestations and may contribute to higher preeclampsia risk in this population.

Objective The objective of the study was to evaluate angiogenic factors for the prediction of preeclampsia in high-risk women. Study Design We collected serial serum specimens from 94 women at high preeclampsia risk between 22 and 36 weeks' gestation. Soluble fms-like tyrosine kinase-1 (sFlt1) and placental growth factor (PlGF) were measured by enzyme-linked immunosorbent assay. Results Mean serum sFlt1 and the sFlt1/PlGF ratio were higher in subjects who developed early-onset (less than 34 weeks) preeclampsia, as compared with subjects without preeclampsia, from 22 weeks gestation onward. In subjects who developed late-onset (34 weeks or later) preeclampsia, sFlt1 was significantly increased after 31 weeks' gestation. The sFlt1/PlGF ratio at 22-26 weeks was highly predictive of early-onset preeclampsia. The within-woman rate of change of the sFlt1/PlGF ratio was predictive of overall preeclampsia risk. Conclusions In high-risk women, serum sFlt1 and the sFlt1:PlGF ratio are altered prior to preeclampsia onset and may be predictive of preeclampsia. Larger studies are needed to confirm these findings.

Aims/hypothesis During pregnancy, adipose tissue (AT) must expand to support the growing fetus and the future nutritional needs of the offspring. Limited expandability of AT is associated with insulin resistance, attributed to ectopic lipid deposition. This study aimed to investigate human AT expandability during pregnancy and its role in the pathogenesis of gestational diabetes mellitus (GDM). Methods This cross-sectional study of omental (OM) and subcutaneous (SQ) AT collected at Caesarean delivery included 11 pregnant and three non-pregnant women with normal glucose tolerance (NGT), five with GDM, three with type 2 diabetes mellitus. Adipocyte size, capillary density, collagen content and capillary growth were measured. Affymetrix arrays and real-time PCR studies of gene expression were performed. Results Mean OM adipocyte size was greater in women with GDM than in those with NGT (p = 0.004). Mean OM and SQ capillary density was lower in GDM compared with NGT (p = 0.015). Capillary growth did not differ significantly between groups. The most differentially expressed AT transcript when comparing non-pregnant and pregnant women corresponded to the IGF binding protein (IGFBP)-5, the expression levels of which was found by subsequent quantitative real-time PCR to be lower in women with GDM vs women with NGT (p

To examine a wide range of study designs and outcomes to estimate the extent to which interventions in outpatient perinatal care settings are associated with an increase in the uptake of depression care.PubMed, CINAHL, PsycINFO, ClinicalTrials.gov, and Scopus (EMBASE) were searched for studies published between 1999 and 2014 that evaluated mental health care use after screening for depression in perinatal care settings.Inclusion criteria were: 1) English language; 2) pregnant and postpartum women who screened positive for depression; 3) exposure (validated depression screening in outpatient perinatal care setting); and, 4) outcome (mental health care use). Searches yielded 392 articles, 42 met criteria for full-text review, and 17 met inclusion criteria. Study quality was assessed using a modified Downs and Black scale.Articles were independently reviewed by two abstractors and consensus reached. Study design, intervention components, and mental health care use were defined and categorized. Seventeen articles representing a range of study designs, including one randomized controlled trial and one cluster randomized controlled trial, were included. The average quality rating was 61% (31.0-90.0%). When no intervention was in place, an average of 22% (13.8-33.0%) of women who screened positive for depression had at least one mental health visit. The average rate of mental health care use was associated with a doubling of this rate with patient engagement strategies (44%, 29.0-90.0%), on-site assessments (49%, 25.2-90.0%), and perinatal care provider training (54%, 1.0-90.0%). High rates of mental health care use (81%, 72.0-90.0%) were associated with implementation of additional interventions, including resource provision to women, perinatal care provider training, on-site assessment, and access to mental health consultation for perinatal care providers.Screening alone was associated with 22% mental health care use among women who screened positive for depression; however, implementation of additional interventions was associated with a two to fourfold increased use of mental health care. Although definitive studies are still needed, screening done in conjunction with interventions that target patient, health care provider, and practice-level barriers is associated with increased improved rates of depression detection, assessment, referral, and treatment in perinatal care settings.

Background Maternal and offspring immediate and long-term health are affected by pregnancy weight gain and maternal weight. This study was designed to determine feasibility of: 1) recruiting a socio-economically and racially/ethnically diverse sample of pregnant women into a longitudinal observational study, including consenting the women for serial biologic specimen evaluations; 2) implementing comprehensive assessments (including biologic, anthropometric, behavioral, cognitive/psychosocial and socio-demographic, and cultural measures) at multiple time points over the study period, including collecting biologic specimens at planned and unplanned pregnancy delivery times; and 3) retaining the sample for one year into the postpartum period. Additionally, the study will provide preliminary data of associations among hypothesized predictors, mediators and moderators of pregnancy and post-partum maternal and infant weight trajectories. The study was conceptualized under a Biopsychosocial Model using a lifespan approach. Study protocol and baseline characteristics are described. Methods/Design We sought to recruit a sample of 100 healthy women age 18–45 years, between 28–34 weeks gestation, with singleton pregnancies, enrolled in care prior to 17 weeks gestation. Women provide written consent for face-to-face (medical history, anthropometrics, biologic specimens), and paper-and-pencil assessments, at five time points: baseline (third trimester), delivery-associated, and 6-weeks, 3-months and 6-months postpartum. Additional telephone-based assessments (diet, physical activity and breastfeeding) administered baseline and three-months postpartum. Infant weights are collected until 1-year of life. We seek to retain 80% of participants at six-months postpartum and 80% of offspring at 12-months. 110 women were recruited. Sample characteristics include: mean age 28.3 years, BMI 25.7 kg/m2, and gestational age at baseline visit of 32.5 weeks. One-third of cohort was non-white, over a quarter were Latina, and almost a quarter were non-US born. The cohort majority was multigravida, had graduated high school and/or had higher levels of education, and worked outside the home. Discussion Documentation of study feasibility and preliminary data for theory-driven hypothesis of maternal and child factors associated with weight trajectories will support future large scale longitudinal studies of risk and protective factors for maternal and child health. This research will also inform intervention targets facilitating healthy maternal and child weight.
