Abstract

Diabetic nephropathy (DN) or Diabetic kidney disease (DKD) is one of the major microvascular complications of diabetes mellitus (DM). DN is observed in approximately 20–40% of diabetic patients. DN is also an important risk factor for DM patient’s death. Nowadays, DN has become the leading cause of chronic renal failure (CRF) in most countries without effective therapeutic methods. Recently, the renoprotective effects mediated by vitamin D have been evidenced. Currently available evidence showed that vitamin D is effective in reducing proteinuria in DN patients. A recent meta-analysis demonstrated the therapeutic effect of vitamin D, on urinary albumin excretion, in DN patients. This review summarized the multiple roles of vitamin D in mechanism of renopretective effect of vitamin D to explore much more and effective therapeutic methods for DN.

162. Ginsenoside Rg1 ameliorates stress-exacerbated Parkinson’s disease in mice by eliminating RTP801 and α-synuclein autophagic degradation obstacle

Abstract

Emerging evidence shows that psychological stress promotes the progression of Parkinson’s disease (PD) and the onset of dyskinesia in non-PD individuals, highlighting a potential avenue for therapeutic intervention. We previously reported that chronic restraint-induced psychological stress precipitated the onset of parkinsonism in 10-month-old transgenic mice expressing mutant human α-synuclein (αSyn) (hαSyn A53T). We refer to these as chronic stress-genetic susceptibility (CSGS) PD model mice. In this study we investigated whether ginsenoside Rg1, a principal compound in ginseng notable for soothing the mind, could alleviate PD deterioration induced by psychological stress. Ten-month-old transgenic hαSyn A53T mice were subjected to 4 weeks’ restraint stress to simulate chronic stress conditions that worsen PD, meanwhile the mice were treated with Rg1 (40 mg· kg−1·d−1, i.g.), and followed by functional magnetic resonance imaging (fMRI) and a variety of neurobehavioral tests. We showed that treatment with Rg1 significantly alleviated both motor and non-motor symptoms associated with PD. Functional MRI revealed that Rg1 treatment enhanced connectivity between brain regions implicated in PD, and in vivo multi-channel electrophysiological assay showed improvements in dyskinesia-related electrical activity. In addition, Rg1 treatment significantly attenuated the degeneration of dopaminergic neurons and reduced the pathological aggregation of αSyn in the striatum and SNc. We revealed that Rg1 treatment selectively reduced the level of the stress-sensitive protein RTP801 in SNc under chronic stress conditions, without impacting the acute stress response. HPLC-MS/MS analysis coupled with site-directed mutation showed that Rg1 promoted the ubiquitination and subsequent degradation of RTP801 at residues K188 and K218, a process mediated by the Parkin RING2 domain. Utilizing αSyn A53T+; RTP801−/−mice, we confirmed the critical role of RTP801 in stress-aggravated PD and its necessity for Rg1’s protective effects. Moreover, Rg1 alleviated obstacles in αSyn autophagic degradation by ameliorating the RTP801-TXNIP-mediated deficiency of ATP13A2. Collectively, our results suggest that ginsenoside Rg1 holds promise as a therapeutic choice for treating PD-sensitive individuals who especially experience high levels of stress and self-imposed expectations.

Abstract

Recently, gas therapy has emerged as a promising alternative treatment for deep-seated tumors. However, some challenges regarding insufficient or uncontrolled gas generation as well as unclear therapeutic mechanisms restrict its further clinical application. Herein, a well-designed nanoreactor based on intracellular glutathione (GSH)-triggered generation of sulfur dioxide (SO2) gas to augment oxidative stress has been developed for synergistic chemodynamic therapy (CDT)/sonodynamic therapy (SDT)/SO2gas therapy. The nanoreactor (designed as CCM@FH-DNs) is constructed by employing iron-doped hollow mesoporous silica nanoparticles as carriers, the surface of which was modified with the SO2prodrug 2,4-dinitrobenzenesulfonyl (DNs) and further coated with cancer cell membranes for homologous targeting. The CCM@FH-DNs can not only serve as a Fenton-like agent for CDT, but also as a sonosensitizer for SDT. Importantly, CCM@FH-DNs can release SO2for SO2-mediated gas therapy. Both in vitroand in vivoevaluations demonstrate that the CCM@FH-DNs nanoreactor performs well in augmenting oxidative stress for SO2gas therapy-enhanced CDT/SDT viaGSH depletion and glutathione peroxidase-4 enzyme deactivation as well as superoxide dismutase inhibition. Moreover, the doped iron ions ensure that the CCM@FH-DNs nanoreactors enable magnetic resonance imaging-guided therapy. Such a GSH-triggered SO2gas therapy-enhanced CDT/SDT strategy provides an intelligent paradigm for developing efficient tumor microenvironment-responsive treatments.

164. Comparison of different extraction techniques and optimization of the microwave-assisted extraction of saponins from Aralia elata(Miq.) Seem fruits and rachises

Abstract

The total saponins from Aralia elata(Miq.) Seem fruits and rachises were isolated by microwave-assisted extraction (MAE), ultrasonic extraction, heat reflux extraction, and soxhlet extraction. These techniques were compared from four perspectives: extraction kinetics, saponin yield, disintegration to the raw materials, and antioxidant activity. MAE had the highest extraction efficiency, shortest time, and greatest disintegration to the raw materials. The extracts obtained with MAE had the highest antioxidant activities, as the DPPH free radical scavenging abilities of the fruit and rachis extracts were 70.86 ± 1.33 and 50.85 ± 1.68 mg VCg−1md, respectively. The MAE parameters were optimized by the response surface methodology. The optimum conditions (92% and 91% ethanol, solid-to-liquid ratios of 1:30 and 1:22, microwave powers of 530 W and 400 W, microwave times of 40 s and 54 s) provided extraction yields of 2.98 ± 0.037 mg g−1and 0.94 ± 0.026 mg g−1from the fruits and rachises, respectively.

174. Tuberculous peritonitis in patients on peritoneal dialysis: a 35-year experience from a large medical center in Northern Taiwan.

Abstract

Tuberculous peritonitis (TBP) is a rare but fatal complication in patients on peritoneal dialysis (PD). In this study, we aimed to determine the demographic features, clinical features, laboratory parameters, and clinical outcomes of PD patients with TBP and to clarify possible risk factors for mortality. We retrospectively reviewed 2084 PD patients from January 1985 to December 2019. The diagnosis of TBP was established by positive peritoneal fluid culture for Mycobacterium tuberculosis. 18 patients were diagnosed with TBP. The incidence was 2.029 episodes per 1000 patient-years. The most common symptom was fever (94.4%), followed by cloudy effluent (83.3%) and abdominal pain (83.3%). The average peritoneal dialysis effluent (PDE) white blood cell (WBC) count was 172.7 cells/μL. Nine patients (50%) had WBC counts lower than 100 cells/μL and 13 patients (72.2%) had neutrophilic predominant WBC counts. Acid fast stain (AFS) was positive in 7 patients (38.9%). Only 2 patients (11.1%) continued with PD after TB infection, while 10 patients (55.6%) changed to hemodialysis. Seven patients (38.9%) died within 1 year. Significant differences were observed in sex (p = 0.040), the presence of diabetes mellitus (p = 0.024), and PD catheter removal (p < 0.001) between TBP patients with and without mortality. However, none of them was a significant factor for 1-year mortality in multivariate Cox regression model. Physicians should pay attention to the unusual presentations of peritonitis, especially if symptoms include fever or an initial low PDE WBC count. Abstract

In the process of mining and transportation, the temporary non-hardened mine-road structure is mainly a mixture of soil and stone, which very easily produces dust hazards via crushing and wind transportation. Geopolymers can be used in the road's soil and stone mixture so that the road reaches certain strength requirements in line with the short-term use of the mine. However, in alpine open-pit coal mines, which are subject to the influence of weather changes, freezing and thawing phenomena will affect the long-term use of the road and its normal and safe operation. An open-pit coal mine in Xinjiang, China, was chosen as the research object of alpine open-pit coal mines. Using the method of indoor testing, different freeze–thaw freezing temperatures, different numbers of cycles, changes in the mechanical properties of the mine-road materials, and microscopic changes were studied. From the experimental results, it was determined that with a reduction in the freeze–thaw freezing temperature, the specimen strength declines after stabilizing, and with an increase in the number of freeze–thaw cycles, the specimen strength exhibits a linear decline. The specimen's internal structure gradually changed from dense to loose; the fracture mode changed from toughness fractures to crystal fractures after the removal of the maximum load reduction. The uniaxial compressive strength was reduced to 61%; the tensile strength was reduced to 49%. The fracture zone of the specimen was analyzed using energy spectra, and the main elements changed from O (57.19%), Si (17.07%), and Al (12.19%) without freezing and thawing to O (49.76%), Si (15.70%) and Ca (11.09%) after freezing and thawing. 178. Identification of a novel mutation in the FGF10 gene in a Chinese family with obvious congenital lacrimal duct dysplasia in lacrimo-auriculo-dento-digital syndrome

Abstract

AIM: To identify the pathogenic gene variant in a family with lacrimo-auriculo-dento-digital syndrome [LADD (MIM 149730)] showing congenital lacrimal duct dysplasia as the main clinical manifestation and lay the foundation for future research on the pathogenic gene. METHODS: Ophthalmological examinations, including slit-lamp biomicroscopy and lacrimal duct probing, and computed tomography dacryocystography (CT-DCG) were performed for all participants. The family pedigree was drawn, genetic features were analyzed, and the genomic DNA of the subjects was extracted. Pathogenic genes were screened via whole exome sequencing (WES) and confirmed using Sanger sequencing. RESULTS: Six patients belonged to this three-generation family, and their clinical manifestations included congenital nasolacrimal duct obstruction, congenital absence of lacrimal puncta and canaliculi, lacrimal fistulae, and limb deformities. This pattern indicates autosomal dominant inheritance. Diagnosis was based on the clinical characteristics of LADD syndrome, which presented in all the patients in this family. A novel frameshift mutation in the FGF10 gene (NM_004465.1), c.234dupC (p.Trp79Leus*15), was identified in all patients via WES. The variant was confirmed by Sanger sequencing and classified as a “pathogenic mutation” according to the American College of Medical Genetics and Genomics (ACMG) variant interpretation guidelines. CONCLUSION: A novel frameshift mutation in the FGF10 gene is found in all patients. This finding helps this family with LADD syndrome receiving a more accurate clinical diagnosis and genetic counseling by extending the mutation range of the FGF10 gene.

Abstract

Objective Early triage of patients with coronavirus disease 2019 (COVID-19) is pivotal in managing the disease. However, studies on the clinical risk score system of the risk factors for the development of severe disease are limited. Hence, we conducted a clinical risk score system for severe illness, which might optimize appropriate treatment strategies. Methods We conducted a retrospective, single-center study at the JinYinTan Hospital from January 24, 2020 to March 31, 2020. We evaluated the demographic, clinical, and laboratory data and performed a 10-fold cross-validation to split the data into a training set and validation set. We then screened the prognostic factors for severe illness using the least absolute shrinkage and selection operator (LASSO) and logistic regression, and finally conducted a risk score to estimate the probability of severe illness in the training set. Data from the validation set were used to validate the score. Results A total of 295 patients were included. From 49 potential risk factors, 3 variables were measured as the risk score: neutrophil to lymphocyte ratio (OR, 1.27; 95% CI, 1.15–1.39), albumin (OR, 0.76; 95% CI, 0.70–0.83), and chest computed tomography abnormalities (OR, 2.01; 95% CI, 1.41–2.86) and the AUC of the validation cohort was 0.822 (95% CI, 0.7667–0.8776). Conclusion This report may help define the potential of developing severe illness in patients with COVID-19 at an early stage, which might be related to the neutrophil to lymphocyte ratio, albumin, and chest computed tomography abnormalities.
