153. Women's breastfeeding experiences following a significant primary postpartum haemorrhage: A multicentre cohort study

Abstract

Abstract Background Postpartum haemorrhage (PPH) is a significant and increasing contributor to maternal mortality and morbidity. Following a PPH, women may have difficulties initiating and sustaining breastfeeding, although little has been published on this issue. The aim of this study was to describe breastfeeding experiences in a cohort of women following a significant PPH. Methods This is a descriptive study based on quantitative and qualitative data collected via questionnaires completed in the first week postpartum and at two and four months postpartum, by 206 women participating in a multicentre study of women's experiences of a significant primary postpartum haemorrhage (blood loss of 1500 mL or more in the 24 hours following childbirth, and/or a peripartum fall in haemoglobin (Hb) concentration to 7g/dL or less, or of ≥ 4g/dL). Results Among women with a significant PPH, 63% fully breastfed their babies from birth, whereas 85% said they had hoped to do so (p < 0.001). Only 52% of mothers who intended to either fully or partially breastfeed were able to give their baby the opportunity to suckle within an hour of the birth. Delays were longer in women with greater estimated blood loss and women with the longest delays in breastfeeding were less likely to initiate full breastfeeding. 70% of women with PPH of < 2000 mL were fully breastfeeding in the first postpartum week, whereas less than 50% of those with blood loss ≥ 3000 mL were able to do so. Overall, 58% of women with significant PPH were fully breastfeeding at two and 45% at four months postpartum. In qualitative data, three major themes were identified: 1) Difficulty initiating or sustaining breastfeeding, 2) Need for education and support; and 3) Emotional sequelae. Conclusions Following a significant PPH, women with greater blood loss are less likely to initiate and sustain full breastfeeding and this may be related, in part, to delays in initial contact with their baby as a consequence of the PPH. These findings have implications for postnatal care as these women may require greater support, education and assistance in initiating and sustaining breastfeeding. In particular, enabling the opportunity for the newborn to suckle as soon as is practicable should be encouraged.

Abstract

Abstract Background Anecdotal reports suggest that the addition of fentanyl (an opioid) to epidural analgesia for women during childbirth results in difficulty establishing breastfeeding. The aim of this paper is to determine any association between epidural analgesia and 1) breastfeeding in the first week postpartum and 2) breastfeeding cessation during the first 24 weeks postpartum. Methods A prospective cohort study of 1280 women aged ≥ 16 years, who gave birth to a single live infant in the Australian Capital Territory in 1997 was conducted. Women completed questionnaires at weeks 1, 8, 16 and 24 postpartum. Breastfeeding information was collected in each of the four surveys and women were categorised as either fully breastfeeding, partially breastfeeding or not breastfeeding at all. Women who had stopped breastfeeding since the previous survey were asked when they stopped. Results In the first week postpartum, 93% of women were either fully or partially breastfeeding their baby and 60% were continuing to breastfeed at 24 weeks. Intrapartum analgesia and type of birth were associated with partial breastfeeding and breastfeeding difficulties in the first postpartum week (p < 0.0001). Analgesia, maternal age and education were associated with breastfeeding cessation in the first 24 weeks (p < 0.0001), with women who had epidurals being more likely to stop breastfeeding than women who used non-pharmacological methods of pain relief (adjusted hazard ratio 2.02, 95% CI 1.53, 2.67). Conclusion Women in this cohort who had epidurals were less likely to fully breastfeed their infant in the few days after birth and more likely to stop breastfeeding in the first 24 weeks. Although this relationship may not be causal, it is important that women at higher risk of breastfeeding cessation are provided with adequate breastfeeding assistance and support.

Abstract

Objective: Western Sydney University has implemented a rural interprofessional learning programme to promote collaborative care approaches to enhance cross‐discipline communications, improve knowledge and clarity of roles and improve patient care and outcomes. Design: Rural interprofessinal learning is an interprofessional educational approach, consisting of simulations of complex health events. Simulation methodology frames the study with a focus on human interaction. A mixed‐methods evaluation has been conducted, incorporating pre‐ and post‐ event participant surveys along with semi‐structured focus groups. Setting: Simulations are conducted in the rural setting, including community settings, working farms and rural hospitals. Main Outcome Measures: Reflexive thematic analysis was used to identify themes measuring students' perceptions of interdisciplinary care, knowlede of other health discipline roles and skills and how they believe the exercise will influence their future practice. Facilitator feedback regarding the efficacy of the simulations was also recorded and analysed using reflexive thematic analysis. Participants: Care of simulated patient(s)/bystander(s) is primarily provided by paramedicine, nursing and medical students; however, increasing interest has expanded the programme to include students from a range of allied health professions. Simulations are facilitated by a multidisciplinary team of experienced practitioners and specialists. Intervention: Four rural interprofessional learning events have been held. Results: 120 students have participated in the evaluation. Findings include increased understanding of the contributions of other disciplines in enhancing patient care, team approaches, cross‐discipline communication and a need to engage in collaborative care in future practice. Conclusion: Creating a collaborative learning environment creates a culture of multidisciplinary care, enhancing patient care and improving outcomes. The rural interprofessional learning model is an effective interprofessional educational approach, which can be repeated, refined and improved for continual professional development. Abstract

Goemans, Andrzej Wisniowski A group of 20 households was established to study whether we can motivate environmentally sustainable behaviour by providing homeowners with a clear picture of their impact, tangible reasons for improvement, and tailored solutions to follow. Reports for each household compared heating fuel, electricity, water, vehicle fuel/waste generation within the group and recommended cost-effective measures to reduce consumption. On average, 26% of the recommended measures were implemented, resulting in an estimated greenhouse gas reduction of about 2 tonnes per household. Wide variations were found between households, demonstrating the potential to reduce environmental impact through lifestyle, conservation, and energy conscious retrofits.

Abstract

For those working in adult learning, supporting active citizenship has become one of those principles-turned-platitudes they like to recite, but they need to search hard in the small print of recent White Papers to be sure that the Government cares about the wider purposes of learning beyond functional training for employment. "Active citizenship" is one of those formerly radical terms that used to be associated with audacious grass roots energy, participatory democracy and progressive social change--in the days when adult education was considered to be "a movement" with organic links to some of the most influential social movements of recent times, such as the labour and trade union movement, the women's movement, the civil rights movement and the peace movement. It has now become a happy-clappy soundbite--like "empowerment", "participation", "social inclusion" and, most recently, "respect"--that New Labour routinely appropriates to pretend a radical-sounding approach to an otherwise authoritarian pre-occupation with micro-managing the potentially troublesome attitudes of the lower orders. When faced with a rather limited view and moralistic application of the language of participation and citizenship, it is worth considering what has been won and lost when those employed in adult learning talk about active citizenship. The conceptual shift, over the last 15 years, from adult "education" to adult "learning", has made a huge difference to what they actually "do". Instead of specialist teachers and enthusiasts, teaching a wide range of subjects, designed to stimulate and challenge a relatively wide range of people, the day job for most of them now entails enticing and cajoling the poor and other minorities into activities designed to make them healthier, more socially competent and more likely to get a job. Either that or processing the blizzard of paperwork routinely required to prove that the latest short-term initiative has been delivered on time, on budget, and in line with government targets. In this article, the author contends that it is about time adult educators get back in touch with the energy, commitment, and anger that fuels active citizenship and rediscover the educational potential and significance of popular social movements. After all, states Thompson, there is nothing left to lose. Education for pleasure has virtually disappeared. Education for activists in trade unions, community groups, and women's organisations has been regulated out of existence or readjusted to the requirements of the skills agenda. And education for the poor has everywhere become a condition of benefit, employment, or citizenship, designed to keep them busy.

Two of three patients that complained of chest pain during panic attacks showed a progression to myocardial infartion caused by a coronary artery spasm. Some studies also indicate there is increased cardiac risk in patients with panic disorder. It is thought to involve the sympathetic nerves of the heart leading to ventricular arrhythmias.

Abstract

Treatment of gastric ulcers and diarrhea with the Amazonian herbal medicine sangre de grado. Am J Physiol Gastrointest Liver Physiol 279: G192-G200, 2000.--Sangre de grado is an Amazonian herbal medicine used to facilitate the healing of gastric ulcers and to treat gastritis, diarrhea, skin lesions, and insect stings. This study was designed to evaluate the gastrointestinal applications. Gastric ulcers were induced in rats by brief serosal exposure of the fundus to acetic acid (80%). Sangre de grado was administered in drinking water at 1:1,000 and 1:10,000 dilutions from the postoperative period to day 7. Guinea pig ileum secretory responses to capsaicin, electrical field stimulation, and the neurokinin-1 (NK-1) agonist [[Sarg.sup.9],Met([O.sub.2])[sup.11]]substance P were examined in Ussing chambers. Sangre de grado facilitated the healing of experimental gastric ulcer, reducing myeloperoxidase activity, ulcer size, and bacterial content of the ulcer. The expression of proinflammatory genes tumor necrosis factor-[Alpha], inducible nitric oxide synthase (iNOS), interleukin (IL)-1[Beta], IL-6, and cyclooxygenase-2 was upregulated by ulcer induction but reduced by sangre de grado treatment, particularly iNOS and IL-6. In Ussing chambers, sangre de grado impaired the secretory response to capsaicin but not to electrical field stimulation or the NK-1 agonist. We conclude that sangre de grado is a potent, cost-effective treatment for gastrointestinal ulcers and distress via antimicrobial, anti-inflammatory, and sensory afferent-dependent actions. cytokine; inflammation; neuropeptides; Croton sp.; complementary medicine

Abstract

The article describes technical innovation, process reengineering and organizational adaptation of the Optically Recorded Information Online Network (ORION) project. Obsolete and rigid business processes have rendered many large organizations inefficient and inflexible. In fact, many of the recent problems of inflexibility and turmoil in large corporations can be attributed to the complexities and pathologies of size and outdated processes. In an attempt to acquire strategic and competitive advantage, many large organizations are searching for organizational simplicity. Organizational simplicity can be reintroduced to complex organizations through a radical reengineering of theft business processes. The radical reengineering of business processes, commonly referred to as business process reengineering (BPR), has been the subject of several recent articles and books. In their search for organizational simplicity and responsiveness, Federal Express Corp. has effectively utilized new and modern information technologies in the reengineering of some very significant business processes. One of the BPR projects being pursued at Federal Express is the ORION.
