Abstract

Despite the enormous time and energy that goes into strategy development, many companies have little to show for their efforts. Indeed, research by the consultancy Marakon Associates suggests that companies on average deliver only 63% of the financial performance their strategies promise. In this article, Michael Mankins and Richard Steele of Marakon present the findings of this research. They draw on their experience with high-performing companies like Barclays, Cisco, Dow Chemical, 3M, and Roche to establish some basic rules for setting and delivering strategy: Keep it simple, make it concrete. Avoid long, drawn-out descriptions of lofty goals and instead stick to clear language describing what your company will and won't do. Debate assumptions, not forecasts. Create cross-functional teams drawn from strategy, marketing, and finance to ensure the assumptions underlying your long term plans reflect both the real economics of your company's markets and its actual performance relative to competitors. Use a rigorous analytic framework. Ensure that the dialogue between the corporate center and the business units about market trends and assumptions is conducted within a rigorous framework, such as that of "profit pools." Discuss resource deployments early. Create more realistic forecasts and more executable plans by discussing up front the level and timing of critical deployments. Clearly identify priorities. Prioritize tactics so that employees have a clear sense of where to direct their efforts. Continuously monitor performance. Track resource deployment and results against plan, using continuous feedback to reset assumptions and reallocate resources. Reward and develop execution capabilities. Motivate and develop staff. Abstract

Importance: Digital ulcers in patients with systemic sclerosis are associated with pain and poor quality of life. Endothelin-1 promotes vasculopathy in systemic sclerosis after macitentan, an endothelin-1 blocker. Objective: To evaluate the efficacy of macitentan in reducing the number of new digital ulcers in patients with systemic sclerosis. Design, Setting, And Participants: Two international, randomized, double-blind, placebo-controlled trials (DUAL-1, DUAL-2) were conducted between January 2012 and February 2014. Participants were patients with systemic sclerosis and active digital ulcers at baseline. Target enrollment for each study was 285 patients. Interventions: Patients were randomized (1:1:1) to receive oral doses of 3 mg of macitentan, 10 mg of macitentan, or placebo once daily and stratified according to number of digital ulcers at baseline (≤3 or >3). Main Outcomes and Measures: The primary outcome for each trialwas the cumulative number of new digital ulcers from baseline to week 16. Treatment effect was expressed as the ratio between treatment groups. Results: In DUAL-1, among 289 randomized patients (mean age 51.2 years; 85.8% women), 226 completed the study. The adjusted mean number of new digital ulcers per patient over 16 weeks was 0.94 in the 3-mg macitentan group (n = 95) and 1.08 in the 10-mg macitentan group (n = 97) compared with 0.85 in the placebo group (n = 97) (absolute difference, 0.09 [95% CI, -0.37 to 0.54] for 3mg of macitentan vs placebo and 0.23 [-0.27 to 0.72] for 10 mg of macitentan vs placebo). Among 265 patients randomized in DUAL-2 (mean age 49.6 years; 81.9% women), 216 completed the study. In DUAL-2, the adjusted mean number of new digital ulcers was 1.44 in the 3-mg macitentan group (n = 88) and 1.46 in the 10-mg macitentan group (n = 88) compared with 1.21 in the placebo group (n = 89) (absolute difference, 0.23 [95% CI, -0.35 to 0.82] for 3 mg of macitentan vs placebo and 0.25 [95% CI, -0.34 to 0.84] for 10mg of macitenta

Abstract

Truths are never lost, they just get -occasionally- forgotten. Rick Steele's reflections on managed (primary) care are a powerful testimony to this. In an age of ever-increasing hyperspecialisation in biomedical and clinical practice, of virtually logarithmic doubling of peer-reviewed health science literature every year, and of ever stronger beliefs around the pervasive nature of evidence-based health care, we have forgotten an important truth about people's health and how to care for it. Health is hardly created by the health system, which should rather be called the palliative system, or disease management system. Health, as the recent Marmot reviews on their social determinants show, is created where people live, love, work and play. Those places (or'settings for health') are created and sustained by powerful systemic parameters, such as the economy, the ecosystem, education and early life, and most of all, political choice. Managing this complex, interconnected, system of causes and'causes of causes'of health and illness cannot be left to epidemiologists or clinicians, however brilliant they are. Steele argues for a comprehensive, targeted and multiprofessional approach to managed primary care that deals with some of the most challenging issues in the disease management system. His analysis stems from work mainly carried out in the 1980s and 1990s, and is strongly validated for the new millennium by two streams of action. First of all, Steele himself has been practicing his approach in a variety of capacities in Scandinavia for some decades now, yielding impressive results. But more importantly, recent insights and rediscoveries by WHO and other international and national bodies substantiate Steele's proposals. Recently, the World Health Assembly re-endorsed the Primary Health Care approach unequivocally, and not as a partial'horizontal'or partial'vertical'disease-driven community development strategy. Even more recently than that, a United Nations Summit was convened to address the looming - and in many places already rampant - epidemic of non-communicable disease. On both occasions, a measured and managed approach to health development and disease prevention was strongly advocated, and in turn endorsed by civil society. Rick Steele isn't just rediscovering these truths. 167. Combining Teletherapy and On-line Language Exercises in the Treatment of Chronic Aphasia: An Outcome Study

Abstract

We report a 12-week outcome study in which nine persons with long-term chronic aphasia received individual and group speech-language teletherapy services, and also used on-line language exercises to practice from home between therapy sessions. Participants were assessed at study initiation and completion using the Western Aphasia Battery, a portion of the Communicative Effectiveness Index, ASHA National Outcome Measurement System, and RIC Communication Confidence Rating Scale for Aphasia; additionally participants were polled regarding satisfaction at discharge. Pre-treatment and post-treatment means were calculated and compared, and matched t-tests were used to determine significance of improvements following treatment, with patterns of independent on-line activity analyzed. Analysis of scores shows that means improved on most measures following treatment, generally significantly: the WAB AQ improved +3.5 (p = .057); the CETI Overall (of items administered) — +17.8 (p = .01), and CCRSA Overall — + 10.4 (p = .0004). Independent work increased with time, and user satisfaction following participation was high.

Abstract

Federal privacy law is all too often misconstrued or perverted to preclude the disclosure of video recordings that capture students victimized by violent crime or tortious injury. This misuse of federal law impedes transparency and accountability and, in many cases, even jeopardizes the health, safety, and lives of children. When properly construed, however, federal law is no bar to disclosure and, at least in public schools, works in tandem with freedom of information laws to ensure disclosure. This Article posits that without unequivocal guidance from federal administrative authorities, uncertainty regarding the disclosure of such recordings will continue to linger, jeopardizing the ability of plaintiffs to access needed information. Abstract

Background: Telerehabilitation promises to greatly expand access of underserved populations to speech therapy, but concerns remain about the effectiveness of services delivered remotely compared to in-person treatment. Aims:To evaluate the effectiveness of telerehabilitation, we conducted a randomized non-inferiority trial for chronic poststroke communication disorders, testing whether equivalent gains can be expected from in-person vs. telerehabilitative clinical service delivery, with both groups completing homework exercises outside of therapist contact time. Methods & Procedures: We treated 44 participants with aphasia or cognitive-linguistic communication disorder (CLCD). Treatment comprised tablet-based homework exercises and realistic, customized treatment plans tailored to the needs of each individual client. Clients had weekly 1-h sessions with the therapist over 10 weeks, with the interaction randomized to in-person and telerehabilitation conditions. Objective gains were assessed with the Western Aphasia Battery aphasia quotient (WAB-AQ) (for aphasia) and Cognitive-Linguistic Quick Test (CLQT) (for CLCD) and subjective gains with the Communication Confidence Rating Scale for Aphasia (CCRSA) (self-rating) and Communication Effectiveness Index (CETI) (partner rating). Outcomes & Results: Participants improved significantly on all of these measures, with statistically equivalent gains between in-person and telerehabilitation groups for WAB-AQ, CLQT, and CETI. Only the CCRSA showed an advantage for the in-person group. Gains on WAB-AQ were correlated with total time spent on offline exercises. Conclusions: Clinician-guided computer-based treatment is effective for producing widespread gains in language and communication skills in chronic stroke. Linguistic gains are equivalent whether clinician services are provided via telerehabilitation equipment or in person. Communicative confidence may still benefit from in-person treatment, reinforcing the need for social engagement in addition to deficit-focused linguistic treatment. [ABSTRACT FROM AUTHOR]

Abstract

We report an outcome study involving eight at-risk residents in long-term care facilities across four states. Residents were assessed in spoken and expressive language, memory, mental status, and mood before and after implementation of a Functional Maintenance Program that incorporated online interactive tools. Data analyses reveal small but meaningful improvements in expressive language and in memory, trending towards significance. They also show these non-robust participants' mean scores for mood and mental status remained stable. Findings are discussed in the context of stakeholders' considerations, including improving outcomes, conforming operations, securing revenues, and satisfying participants. [ABSTRACT FROM AUTHOR]
