Background: Human placental villous cytotrophoblasts exhibit relative externalization of negatively charged moieties to the outer leaflet of the plasma membrane during the time of syncytialization rendering their reactivity to positively charged cationic antimicrobial peptides (CAMPs) during the window of implantation and early placentation. Vaginal administration of a synthetic CAMP, Ala8,13,18-magainin II amide (AMA) inhibited blastocyst implantation and early placentation in monkeys. Furthermore, the administration of AMA resulted in significant inhibition of cell differentiation, enhancement in apoptosis and loss of viability in first trimester placental villous cytotrophoblasts in primary culture. The present study examines the effect of in vitro application of different doses (0, 1, 10, 100, 1000 ng/ml) of AMA on the secreted cytokine profiles of cytotrophoblasts obtained from placental villi samples (n = 13) collected during 8-9 weeks of gestation and grown on three-dimensional collagen matrix in vitro. Methods: A panel of forty-eight (48) cytokines in conditioned medium was analysed using multiplex immunoassays technique. Further, the steady state transcript levels of four cytokines (CCL4, CCL5, IL1B, IL6), the concentrations of which were affected by AMA in the isolated cytotrophoblasts, as well as, two cytokines (IL1A and TNF) which were not affected by AMA were estimated. Input list of cytokines secreted by cytotrophoblasts and showing differential secretion in response to AMA were used in enrichment analysis for the generation of biological networks. Results: Placental cytotrophoblasts secreted 27 cytokines, 13 of which are affected by AMA in vitro with significantly decreased secretion of CCLs-2, 3, 4, 5, CXCLs-1 and 8, FGF2 and MCSF and that of IL1B, IL6 and MIF, and increased secretion of IL16 and IL-2RA. Of the above cytokines showing differential secretion, only IL-2RA, IL16 and MIF showed significant correspondence in the steady state expression of their respective transcript levels. Post-hoc Enrichment analysis revealed Toll-like receptor (TLR) mediated pathways were the top-scored target pathways that were affected by AMA. Conclusions: Administration of a CAMP causes shift in the balance of immune-inflammatory responses involving downstream pathways of TLRs in cytotrophoblast function. A widely grown but rust susceptible Indian wheat variety HD2932 was improved for multiple rust resistance by marker-assisted transfer of genes Lr19, Sr26 and Yr10. Foreground and background selection processes were practised to transfer targeted genes with the recovery of the genome of HD2932. The near-isogenic lines ( NILs) of HD2932 carrying Lr19, Sr26 and Yr10 were individually produced from two backcrosses with recurrent parent HD2932. Marker-assisted background selection of NILs with 94.38-98.46% of the HD2932 genome facilitated rapid recovery of NILs carrying Lr19, Sr26 and Yr10. In the BC2F2 generation, NILs were intercrossed and two gene combinations of Lr19+ Yr10, Sr26 + Yr10 and Lr19+ Sr26 were produced. A total of 16 progeny of two gene combinations of homozygous NILs of HD2932 have been produced, which are under seed increase for facilitating the replacement of the susceptible HD2932 with three of the sixteen improved backcross lines with resistance to multiple rusts. The University of North Georgia (UNG) has begun to evaluate both fixed and rotary UAS platforms across the departments to evaluate their potential for furthering both student learning experiences and undergraduate research. A research project of the Institute for Spatial Analysis (IESA) at UNG has acquired the fixed wing eBee UAS and is currently piloting its integration into the undergraduate geospatial science curriculum. Limited very low altitude, line of sight calibration runs within areas of our campus have help us understand the capabilities that this technology brings to learning and research opportunities at UNG. In our pilot area of study on the UNG Gainesville Campus, we will collect overlapping imagery and generate 3-D models. These models will be compared with models based on LiDAR data. Geographic Object Based Image Analysis (GEOBIA) methods are essential to the analysis of voluminous high resolution UAS data and the associated computational and regulatory issues are discussed. The parents, F1 and F2 individuals of the crosses Kundan x Agra Local, UP 2338 x Agra Local and Kundan x UP 2338 were screened to study the genetics of resistance in the Ug99-resistant varieties of wheat, Kundan and UP 2338 against two Indian stem rust pathotypes 40A (62G29) and 122 (7G11). Against race 40A (62G29), the variety Kundan did not have any resistance alleles, whereas, UP 2338 exhibited the presence of dominant resistance gene. The 195 F2 individuals of the cross UP 2338 × Agra Local and Kundan × UP 2338 segregated at a ratio of 3(R):1(S). Against stem rust pathotype 122 (7G11), the variety Kundan displayed monogenic dominant resistance. The resistance x resistance cross Kundan x UP 2338 also displayed resistance reaction in the F2 generation with no segregation indicating the presence of common gene in both the parents (Kundan and UP 2338) conferring resistance against the pathotype 122. Background: The CORONIS Trial was a 2×2×2×2×2 non-regular, fractional, factorial trial of five pairs of alternative caesarean section surgical techniques on a range of short-term outcomes, the primary outcome being a composite of maternal death or infectious morbidity. The consequences of different surgical techniques on longer term outcomes have not been well assessed in previous studies. Such outcomes include those related to subsequent pregnancy: mode of delivery; abnormal placentation (e.g. accreta); postpartum hysterectomy, as well as longer term pelvic problems: pain, urinary problems, infertility. The Coronis Follow-up Study aims to measure and compare the incidence of these outcomes between the randomised groups at around three years after women participated in the CORONIS Trial.Methods/design: This study will assess the following null hypotheses: In women who underwent delivery by caesarean section, no differences will be detected with respect to a range of long-term outcomes when comparing the following five pairs of alternative surgical techniques evaluated in the CORONIS Trial: 1. Blunt versus sharp abdominal entry. 2. Exteriorisation of the uterus for repair versus intra-abdominal repair. 3. Single versus double layer closure of the uterus. 4. Closure versus non-closure of the peritoneum (pelvic and parietal). 5. Chromic catgut versus Polyglactin-910 for uterine repair. The outcomes will include (1) women's health: pelvic pain; dysmenorrhoea; deep dyspareunia; urinary symptoms; laparoscopy; hysterectomy; tubal/ovarian surgery; abdominal hernias; bowel obstruction; infertility; death. (2) Outcomes of subsequent pregnancies: inter-pregnancy interval; pregnancy outcome; gestation at delivery; mode of delivery; pregnancy complications; surgery during or following delivery.Discussion: The results of this follow-up study will have importance for all pregnant women and for health professionals who provide care for pregnant women. The article presents a review to summarize the epidemiology, etiology and risk factors, latest approach towards investigating an intrauterine death (IUD), and management options. It discusses the definition of IUD, causes of stillbirth, and the diagnosis of stillbirth. It attempts to give further understanding on intrauterine death to better manage such cases in clinical practice and to prevent similar cases in future.

The article discusses the study regarding the maternal and perinatal outcome among women with pre-eclampsia and eclampsia condition. The method for the analysis of maternal morbidity among eclampsia patients according to the data which includes patients' treatment, blood pressure measurement, and urine output was developed. The results and outcome are outlined which conclude the various maternal characteristics of eclampsia patients including mode of delivery.

BACKGROUND: This study was undertaken to report the results of weekly combination chemotherapy with cetuximab in recurrent/metastatic head and neck squamous cell carcinoma (R/M SCCHN). MATERIALS AND METHODS: Retrospective analysis of 35 R/M SCCHN patients who received cetuximab with weekly paclitaxel and platin (cisplatin/carboplatin) from SCCHN August 2006 to October 2008 at our Institute was performed. RESULTS: Thirty-five patients (33 [94.3%] males and 2 [5.7%] females) received the planned weekly chemotherapy protocol. Median age of these patients was 52 years. Of the SCCHN 32 evaluable patients, 25 patients showed symptomatic improvement and 7 showed no improvement. Radiological responses using RECIST criteria reported CR in 1 patient (3.1%), PR in 17 patients (53.1%), and SD in 6 patients (18.8%). The remaining six patients demonstrated disease progression while two could not be assessed. Median overall survival (OS) was 8.016 months (95% CI; 6.572--9.461) and median PFS was 5.782 months (95% CI; 4.521-- 7.044). The major chemotherapy-related grades 2 and 3 toxicity recorded was cetuximab-induced rash reported in 24 patients. No treatment-related death within 30 days was observed. CONCLUSION: Cetuximab with weekly combination chemotherapy (Paclitaxel + Platinum compound) has shown promise, demonstrating comparable response and outcomes with acceptable toxicity in R/M SCCHN patients. Background: Gemcitabine, an anti-metabolite, has some activity in hepatocellular carcinoma (HCC) in terms of responses and median survival. Aims: To analyze our experience with the use of gemcitabine in combination with cisplatin in HCC with respect to response, toxicity and survival. Materials and Methods: We studied the records of patients of HCC treated from January 2000 to December 2005 with gemcitabine and cisplatin, and found 24 of them to be evaluable for response, toxicity and survival. Results: Of 24 patients receiving three or more cycles of chemotherapy, six (25%) had a partial response and an additional 12 (50%) had stable disease. The median overall survival (OS) was 7.5 months (95% confidence interval, 4.5-10.5 months) and 1-year survival was 18%. Grade 3 and 4 anemia, thrombocytopenia and neutropenia were observed in, respectively, 17, 17 and 33% patients. The most frequent non-hematologic toxicities were nausea and vomiting and peripheral neuropathy. Conclusion: We report a partial response rate of 25% with stable disease in an additional 50% to three or more cycles of chemotherapy with gemcitabine and cisplatin, with a median OS of 7.5 months (95% confidence interval, 4.5-10.5) and acceptable toxicity profile from our single-center retrospective study of 24 patients of HCC. Rosiglitazone, an insulin sensitizing agent is used currently in women with clomiphene citrate (CC) resistant polycystic ovarian syndrome (PCOS). Our study proposed to compare the efficacy of rosiglitazone and CC with laparoscopic ovarian drilling (LOD) and CC in terms of biochemical effects, ovulation rate and pregnancy rate in patients of PCOS resistant to CC. This prospective randomised trial included 43 patients of PCOS resistant to CC. Twenty-two women were assigned to the rosiglitazone (4 mg twice daily) and CC group and other 21 patients underwent unilateral LOD and then received CC and multivitamins. The treatment continued for six cycles in both the groups. The biochemical response, ovulation rate and pregnancy rate over a follow up period of 6 months were compared. Treatment with rosiglitazone and CC or LOD and CC resulted in increased ovulation (80.8 vs. 81.5%) and pregnancy (50 vs. 42.8%), respectively. There was no statistical difference between the two groups in terms of biochemical response, ovulation rate and pregnancy rate. To avoid the risk of adverse effects of LOD preference may be given to the use of rosiglitazone and CC therapy in patients of PCOS resistant to CC. [ABSTRACT FROM AUTHOR]

Mitochondrial transcription factor A (mtTFA) is necessary for both transcription and maintenance of mitochondrial DNA. This study was conducted to elucidate the clinicopathologic and prognostic significance of mtTFA in patients with endometrial carcinoma. This study investigated the relationship between the immunohistochemical expression of mtTFA and various clinicopathological variables in 276 endometrial carcinomas, including 245 endometrioid adenocarcinomas and 31 nonendometrioid carcinomas (21 serous carcinomas and 10 clear cell adenocarcinomas). Both uni- and multivariate regression analyses were performed. The mtTFA labeling index of endometrioid adenocarcinomas ranged from 0% to 98%, with a median value of 32%, which was selected as the cut-off point for mtTFA expression. The mtTFA expression in endometrioid adenocarcinomas was significantly associated with the surgical stage, myometrial invasion, lymphovascular space invasion, cervical invasion, and lymph node metastasis. In contrast, no correlation between clinicopathologic variables and mtTFA expression was found in nonendometrioid carcinomas. Correlation analysis between mtTFA and p53 expression by using the Pearson test showed significant correlation in endometrioid adenocarcinomas (P = 0.007), but no significant correlation in nonendometrioid carcinomas (P = 0.947). A univariate survival analysis showed that the 10-year overall survival rate of the patients with mtTFA-positive endometrioid adenocarcinoma was significantly worse than that of patients with mtTFA-negative endometrioid adenocarcinoma (80.8% vs. 93.8%, P = 0.012). However, the multivariate analysis revealed that mtTFA expression in endometrioid adenocarcinomas was no independent prognostic factor. To find out the prevalence of various urological symptoms in pregnant women, the status before pregnancy, and their perceived impact. A questionnaire incorporating various urological problems was prepared and used over 240 pregnant women to know their prevalence in different trimesters of pregnancy and compare them with prevalence before pregnancy. Increased urinary frequency (>10/day) (40.8 vs. 3.8%), nocturia (72.9 vs. 50.6%), burning micturition (21.3 vs. 3.8%), UTI (4.6 vs. 1.6%), urinary hesitancy (14.6 vs. 1.6%), urinary incontinence (25.8 vs. 8.2%) and botheration (22.1 vs. 2.7%) were seen during and before pregnancy, respectively. These also show an increment with advancement of gestation. Urinary incontinence was seen more often with advancing age and parity. There is a very high prevalence of urological symptoms during pregnancy as compared to before pregnancy. [ABSTRACT FROM AUTHOR]

Objectives: To compare the effects of letrozole and clomiphene citrate (CC) on endometrial response in PCOS using subendometrial spiral artery Doppler. Methods: Fifty consecutive patients of anovulatory PCOS were recruited. Twenty-five patients (58 cycles) received 2.5-5 mg of letrozole; 25 patients (56 cycles) received 50-100 mg of CC (both orally on days 5-9 of menses). Main Outcome Measures: number of dominant follicles, endometrial thickness (ET), spiral artery resistance index (RI) and pulsatility index (PI) and pregnancy rate. Results: The mean number of dominant follicles in letrozole group and CC groups was 1.63+/-1.02 and 1.62+/-0.96, respectively (P>0.05). The mean mid cycle ET was 6.9+/-0.74 mm in letrozole group and 5.9+/-0.53 mm in CC group (P<0.05). In letrozole and CC groups, the mean RI of spiral artery were 0.63+/-0.05 and 0.79+/-0.09, respectively, and the mean PI of spiral artery were 1.19+/-0.06 and 1.55+/-0.13, respectively. Both RI and PI in the letrozole group showed significant lower impedance compared to CC group (P<0.05). Pregnancy rate per cycle was 19% (11/58) in the letrozole group and 12.5% (7/56) in the CC group (P>0.05). Our goal was to analyze the safety and efficacy of the use of oxidized cellulose in the creation of neovagina. A total of 10 women with vaginal agenesis attending gynaecology clinic of AIIMS between July 2003 to March 2005 underwent vaginal reconstructive surgery using oxidized cellulose were recruited in this retrospective study. The age of patients ranged from 17 to 22 years (mean age 19 years). Using a transverse incision at introitus, a 10 × 4 cm space was created and a vaginal mould covered with oxidized cellulose was put in the neovagina. The age ranged from 16 to 22 years (mean 19.5 years). Out of 10 patients, who had undergone vaginoplasty, all were sexually active; 8 had satisfactory intercourse, 1 chronic mild dyspareunia due to partial stenosis and 1 was lost to follow up. 2 out of 10 had inflammatory reaction and 1 had psychological problems. Vaginal reconstruction using oxidized cellulose appears to be a safe and effective procedure with minimum complications, morbidity and discomfort with high success rates. [ABSTRACT FROM AUTHOR]

Objective: To evaluate the antenatal profile of the mother and the immediate neonatal morbidity and mortality till discharge.Methods: The study was a retrospective analysis of 92 patients of preterm labour who delivered babies weighingResults: A total of 92 mothers in preterm labour at 26 to 34 weeks were admitted and subsequently delivered 70 VLBW babies (< 1500 gms) and 36 ELBW babies (< 1000 gms) including 8 pairs of twins and 3 triplets pregnancies. Majority of the patients (93.4%) were booked. Amongst the various high risk factors for preterm labour, anaemia during pregnancy (32.6%), bacterial vaginosis (26%), gestational hypertension (18.4%) and pervious history of preterm labour (18.4%) were common associations. Calcium channel blocker (Depin) tocolysis was effective in postponing labour from 48 hours to more than 2 weeks. The cesarean section rate was very high (67.3%) in our study. The commoner neonatal complications in both VLBW and ELBW babies were RDS, neonatal jaundice and sepsis. Features of IUGR were seen in both the groups (22.8% in VLBW and 22.2% in ELBW babies). The neonatal mortality rate till discharge was 15.7% in VLBW group and 33.3% in ELBW group. The morality rate was highest in 26 to 30 weeks gestation babies and in babies weighing < 800 gms.Conclusion: Antenatal profile of preterm labour in our series showed a number of high risk factors. The identification of common high risk factors is important for appropriate prenatal care. A better neonatal survival rate was possible due to timely intervention, appropriate management and NICU care facility available in our tertiary care centre. [ABSTRACT FROM AUTHOR]

Six varieties, Kundan (K), Galvez-87 (G), Trap (T), Chris (C), Mango (M) and PBW-348 (P) along with fast ruster, Agra Local (AL), were screened for seedling reaction and adult pant response to leaf rust. Seedlings of all six varieties were susceptible while adult plants showed lower susceptability response than Agra Local. The F1s among the varieties, and also with Agra Local, showed the values lesser than the respective mid parental values for AUDPC suggesting a polygenic mode of inheritance. ANOVA for combining ability effects indicated variation due to the GCA and SCA effects, which indicated that both additive as well as non-additive type of genetic variances, govern AUDPC. The higher values for the GCA variance over the SCA variance indicated the predominance of an additive component over the dominance component for AUDPC. Significant values for GCA effects indicated that Kundan, Galvez-87 and Trap can be used as good general combiners for AUDPC. The crosses, K×AL, G×AL and T×AL showed significant sca effects for AUDPC, which indicated the predominance of non-additive gene effects in these crosses. Additive×additive and dominance×dominance components of the 5- parameter model were highly significant and contributed maximum extent compared to the additive and dominance components in the cross K×G, while dominance and dominance×dominance components contributed maximum in the remaining crosses. Under such a situation, improvement in the character may be expected through standard selection procedure, which may first exploit the additive gene effects and simultaneously care should be taken to see that the dominance effects are not dissipated, but rather they should be concentrated.
