To assess prevalence and pattern of movement disorders among patients taking antipsychotic medications.This cross-sectional, intensive monitoring (patient interview, case record form review and clinical examination) study was conducted in patients taking antipsychotic drugs irrespective of duration for the development of movement disorders. The psychiatrist used Modified Simpson-Angus Scale score (10-item scale), Barnes' rating scale and Abnormal Involuntary Movement Scale to diagnose parkinsonism, akathisia and tardive dyskinesia, respectively. We assessed movement disorders for the preventability and seriousness.The overall prevalence of antipsychotic induced movement disorders was 5.67% (95% CI: 4.19-7.62). The prevalence of parkinsonism, akathisia and tardive dyskinesia was 5.10% (95% CI: 3.71-6.98), 0.85% (95% CI: 0.39-1.84) and 0.57% (95% CI: 0.22-1.45), respectively. There was a trend of high proportions of movement disorders in extreme of age group, female gender, patients treated with conventional antipsychotics, on poly therapy, patients of epilepsy with psychosis, schizophrenia and bipolar mood disorder. The movement disorder was lowest with quetiapine (2.02%).The higher use of atypical antipsychotics had reduced the occurrence of movement disorders in our setup.

In this paper, we propose a new timing-driven detailed placement technique for FPGAs based on optimizing critical paths. Our approach extends well beyond the previously known critical path optimization approaches and explores a significantly larger solution space. It is also complementary to single-net based timing optimization approaches. The new algorithm models the detailed placement improvement problem as a shortest path optimization problem, and optimizes the placement of all elements in the entire timing critical path simultaneously, while minimizing the costs of adjusting the placement of adjacent non-critical elements. Experimental results on industrial circuits using a modern FPGA device show an average placement clock frequency improvement of 4.5%.

Abstract In this work, we propose a search for a single photon at FASER and FASER2, produced from decays of bino-like, sub-GeV lightest neutralinos in the theoretical framework of the R-parity-violating (RPV) Minimal Supersymmetric Standard Model (MSSM). We consider a list of representative benchmark scenarios with one or two non-vanishing RPV couplings. The photon has an energy 𝒪(0.1) − 𝒪(1) TeV. We find a sensitivity reach for RPV couplings beyond the current bounds by orders of magnitude at FASER and FASER2.

156. Botulinum toxin injections during the COVID-19 epidemic: A retrospective chart review

BACKGROUND AND AIM: Clinical services were severely affected globally during the COVID-19 pandemic. This study aimed to characterize the clinical experience of using botulinum toxin (BTX) injections during the COVID-19 pandemic. METHODS: This is a retrospective chart review of patients who received BTX injections from April 2019 to January 2022. RESULTS: A total of 105 patients received an BTX injections, out of which 76 (72.4%) were men. The mean age of the patients was 47.9 ± 15.1 years. The most common indication for receiving BTX injections was dystonia (n = 79; 75.2%), followed by hemifacial spasm (n = 22; 21%) and miscellaneous movement disorders (n = 4; 3.8%). Focal dystonia (n = 45; 57%) was the most frequent form of dystonia, followed by segmental dystonia (n = 24; 30%). The percentage of generalized dystonia and hemidystonia was 12% and 1%, respectively. Cervical dystonia (44.4%), blepharospasm (17.8%), and writer’s cramp (15.6%) were the most frequent forms of focal dystonia. The miscellaneous group included four patients (3.8%) with trigeminal neuralgia, Holmes tremor, dystonic tics, and hemimasticatory spasm. The mean ages of patients in the dystonia, hemifacial spasm, and the miscellaneous groups were 47.7 ± 14.9 years, 49.2 ± 14.0 years, and 44.2 ± 26.0 years, respectively. The mean BTX dose was 131.6 ± 104.1 U. The mean BTX doses for the dystonia group, hemifacial spasm, and the miscellaneous group were 158.7 ± 105.3 U, 40.1 ± 11.3 U, and 100.0 ± 70.7 U, respectively. CONCLUSION: Most patients in our cohort had dystonia, followed by hemifacial spasm. Among the patients with dystonia, most had focal dystonia, with cervical dystonia being the most common movement disorder. The data obtained in our study is important to increase awareness of the effectiveness of BTX injections in patients with chronic disorders.

Abstract The R-parity-violating Minimal Supersymmetric Standard Model (RPV-MSSM) can naturally accommodate massive neutrinos as required by the oscillation data. However, studying the phenomenology is complicated due to the large number of undetermined parameters involved. Thus, studies are usually restricted to specific submodels. In this work, we develop an approach that allows us to be less restrictive. Working in (almost) the completely general RPV-MSSM setting, we analyze the structure of the neutrino mass matrix, and identify – for the case of two massive neutrinos – only four minimal classes of structures that can solve the neutrino data; we call these Minimal Oscillation Models (MOMs). We study the general features of each MOM class, and present numerical fits to the oscillation data. Our approach allows us to study all RPV models satisfying the neutrino data in a unified manner, as long as they satisfy the MOM criteria. Through several examples, we show that this indeed holds for many interesting scenarios.

Background: Oral mucocele is the most common benign minor (accessory) salivary gland lesion, caused due to mechanical trauma to the excretory duct of the gland. Clinically they are characterized by single or multiple, soft, fluctuant nodule, ranging from the normal color of the oral mucosa to deep blue. It affects at any age and is equally present in both sexes with highest incidence in second decade of life. They are classified as extravasation or retention type. Objectives: To analyze the data between 2010 and 2011 of, clinically and histopathologically diagnosed 58 oral mucoceles for age, gender, type, site, color, cause, symptoms and dimension. Results: Oral mucoceles were highly prevalent in the age group of 15-24 years, were seen in 51.72% of males and 48.28% of females, with a ratio of 1.07:1. The extravasation type (84.48%) was more common than the retention type (15.52%). The most common affected site was lower lip (36.20%) followed by ventral surface of the tongue (25.86%). The lowest frequency was observed in floor of mouth, upper lip and palate. The maximum numbers of mucoceles were asymptomatic (58.62%), and the color of the overlying mucosa had color of adjacent normal mucosa (48.28%). It was also observed that most of the mucoceles had diameter ranging from 5 to 14 mm. The causative factors of the lesion were lip biting (22.41%), trauma (5.18%) and numerous lesions (72.41%). Conclusion: Oral Mucoceles are frequently seen in an oral medicine service, mainly affecting young people and lower lip, measuring around 5 to 14 mm and the extravasation type being the most common.

Background: Traffic police play a very significant role in controlling traffic system. The job of traffic police personnel is a tough job, which has a direct influence on their life. The aim of the present study was to assess prevalence of morbidities, use of personal protective equipment viz., mask, oxygen therapy and pressure stocking amongst Mumbai traffic police and study association of some job related factors with their present health profile.Methods: Record based study was conducted by analyzing 1959 health record forms filled by the traffic police of Mumbai. Frequency, percentages, Chi-square test and unpaired t test were calculated.Results: The mean age of traffic police was 46.87±7.95 years. The duration of years in traffic branch ranged from 6 months to 18 years. Prevalence of stress, hypertension, diabetes mellitus, eye, skin, and ear morbidities were 26%, 20.9%, 14.2%, 10.2%, 3.6%, 3.3% respectively. In traffic police officers, prevalence of hypertension (28.3% v/s 19.8%), diabetes mellitus (23.6% v/s 12.8%), ear morbidities (5.5% v/s 2.9%) and stress (31.1% v/s 25.3%) were significantly high as compared to traffic policemen (p value

Glandular Odontogenic Cyst (GOC) is a developmental cyst of the jaw. It is clinically rare and histopathologically unusual type of odontogenic cyst with unpredictable and potentially aggressive behavior. It was first described in 1987 by Padayachee and Van Wyk. Since its first description, till date 114 cases have been reported in English literature. It usually occurs above 40 years of age and in the anterior region of mandible. Radiographically, it appears as either unilocular or multilocular well defined radiolucent lesion. Usually thinning and perforation of cortical plate is associated with GOC, indicating aggressiveness of the lesion. The clinical and radiographic features are not specific, and it can mimic as any other destructive lesion of the jaw. GOC has tendency to grow to a large size and propensity toward recurrence. Histologically, it can be easily misdiagnosed as a central muco-epidermoid carcinoma. The aim of this paper is to present an unusual case of GOC in a 21 year old male, which was clinically and radiologically diagnosed as a dentigerous cyst associated with an impacted third molar tooth of mandible; and also to review the main criteria for accurate diagnosis.

Laparoendoscopic single-site surgery (LESS) and natural orifice translumenal endoscopic surgery are now being used in gynecologic oncologic procedures. We used our expertise with LESS to perform nerve-sparing laparoscopic radical hysterectomy. A 45-year-old woman with stage IA2 cervical cancer was referred to us. The biopsy specimen showed grade II invasive keratinizing squamous cell carcinoma. We duplicated the steps of our laparoscopic nerve-sparing radical hysterectomy procedure to perform a nerve-sparing radical hysterectomy via LESS using conventional ports and instruments. Oncologic clearance was comparable to that in conventional laparoscopic radical hysterectomy. Bladder function recovered completely after removal of the Foley catheter. Nerve-sparing laparoscopic radical hysterectomy using fewer ports is technically feasible. The oncologic clearance and functional results are comparable to those in the multiport variant.

In this paper, we propose a 2-dimensional dynamic programming (DP) based detailed placement algorithm for modern FPGAs for wirelength and timing optimization. By tuning a control parameter, our algorithm can perform fast heuristic or exact optimization. Our algorithm further enables us to solve the single row placement problem optimally which was not possible with the previous DP approaches, while also reducing it's complexity to Θ(p.N.2N) from the naive Θ(p.N!) (where p is the average degree of a net). Experiments on industrial-scale benchmarks show promising results.

Oral submucous fibrosis (OSMF) is a potentially malignant disorder (PMD) and crippling condition of oral mucosa. It is a chronic insidious scarring disease of oral cavity, pharynx and upper digestive tract, characterized by progressive inability to open the mouth due to loss of elasticity and development of vertical fibrous bands in labial and buccal tissues. OSMF is a debilitating but preventable oral disease. It predominantly affects people of Southeast Asia and Indian subcontinent, where chewing of arecanut and its commercial preparation is high. Presence of fibrous bands is the main characteristic feature of OSMF. The present literature review provides the compilation of various classification system based on clinical and/or histopathological features of OSMF from several databases. The advantages and drawbacks of these classifications supersede each other, leading to perplexity. An attempt is made to provide and update the knowledge about this potentially malignant disorder to health care providers in order to help in early detection and treatment, thus reducing the mortality of oral cancer.

Redox mediators (RMs) are solution-based additives that have been extensively used to reduce the charge potential and increase the energy efficiency of Li–oxygen (Li–O2) batteries. However, in the presence of RMs, achieving a long cycle-life operation of Li–O2batteries at a high current rate is still a major challenge. In this study, we discover a novel synergy among InX3(X = I and Br) bifunctional RMs, molybdenum disulfide (MoS2) nanoflakes as the air electrode, dimethyl sulfoxide/ionic liquid hybrid electrolyte, and LiTFSI as a salt to achieve long cycle-life operations of Li–O2batteries in a dry air environment at high charge–discharge rates. Our results indicate that batteries with InI3operate up to 450 cycles with a current density of 0.5 A g–1and 217 cycles with a current density of 1 A g–1at a fixed capacity of 1 A h g–1. Batteries with InBr3operate up to 600 cycles with a current density of 1 A g–1. These batteries can also operate at a higher charge rate of 2 A g–1up to 200 cycles (for InBr3) and 160 cycles (for InI3). Our experimental and computational results reveal that while X3–is the source of the redox mediator, LiX at the MoS2cathode, In3+reacts on the lithium anode side to form a protective layer on the surface, thus acting as an effective bifunctional RM in a dry air environment. This evidence for a simultaneous improvement in the current rates and cycle life of a battery in a dry air atmosphere opens a new direction for research for advanced energy storage systems.

Minimal access surgery is an accepted modality for benign surgery. Despite the advantages of laparoscopy, its acceptance in oncology is slow. Robotic surgery is an emerging field with rapid acceptance because of the 3—dimensional image, dexterity of instruments and autonomy of camera control. We report here our experience of using the Da Vinci robot for various oncological procedures. We performed 164 oncological surgeries from November 2009 to June 2011. The surgeries performed included thoracic, colorectal, hepatobiliary, gynaecological and urological system. We could complete 163 cases robotically. We share our initial experience of robotic surgery in oncology with comparison with other series.

Esophagectomy has been performed using a thoracoabdominal, transhiatal, or transthoracic approach. All these methods have an acknowledged high intra- and postoperative morbidity. The principle of minimally invasive esophagectomy is to perform the operation the same as by the open approach but through a smaller incision, thus reducing the operative trauma without compromising the principles of the operation. The authors report their experience with thoracoscopic esophagectomy performed for 112 patients in left lateral position. Patients with resectable thoracic or gastroesophageal junction cancer and medically fit for a three-stage esophagectomy underwent thoracoscopic esophagectomy in left lateral position. The procedure was converted to open surgery for 2 (1.79%) of the 112 patients. Since June 2005, 112 patients have undergone thoracoscopic esophagectomy in left lateral position. Of these patients, 80 patients had middle-third esophageal cancer. The pathology of 100 patients showed squamous cell carcinoma. The average thoracoscopic operating time was 85 min (range, 40–120 min). The average blood loss was 200 ml, and the average number of harvested mediastinal nodes was 20. Postoperative morbidity occurred for 16 patients, with 8 patients (7.27%) experiencing respiratory complications. Postoperative mortality was experienced by three patients. The median follow-up period was 18 months. Thoracoscopic esophagectomy is surgically safe and oncologically adequate. Thoracoscopy for patients in the left lateral position does not require prolonged single-lung ventilation. The anatomic orientation in the left lateral position is the same as that for open surgery, reducing the learning curve for thoracic surgeons. The potential advantages and the morbidity trend of prone instead of left lateral thoracoscopic esophagectomy needs to be evaluated.

The epidemiological data are limited for the spontaneous adverse drug reaction (ADR) reporting system in psychiatry and its comparison with intensive monitoring studies in terms of causative drugs, seriousness, preventability and drug interactions. This spontaneous ADR reporting study was carried out over a period of three years in the psychiatry department. We adopted WHO definition for an ADR, Naranjo's algorithm for causality, WHO-ADR terminology for the labeling of involved organ-system, International conference on harmonisation (ICH) E2A guidelines for seriousness, modified Schumock and Thornton's criteria for preventability and Medscape drug interaction checker for drug interactions. Two subgroup analyses were performed to find out the risk factors for the serious and preventable reactions. A total of 97 ADRs from 67 patients were included for analysis. The incidence of 'overall' and 'serious ADRs were 0.69% (95% CI: 0.54%, 0.88%) and 0.18% (95% CI: 0.12-0.29%), respectively. The females experienced more ADRs than males. The most commonly reported ADR, incriminated pharmacology group and drug, were extrapyramidal movement disorders (22.68%), atypical antipsychotics (35.62%) and escitalopram (13.91%), respectively. One out of five and one out three reactions were considered as 'serious' and 'preventable', respectively. The drug interactions contributed in 34.02% reactions. The factors significantly associated with 'serious' reactions were typical antipsychotics [OR: 5.47 (1.68, 17.87)], central and peripheral nervous system disorders [OR: 24.00 (5.12, 112.5)] and extrapyramidal reactions [OR: 14.03 (4.43, 44.43)]. The polypharmacy [OR: 5.85 (1.90, 18.03)] was significantly associated with 'preventable' reactions. The spontaneous reporting system is efficient to detect serious reactions and preventable reactions.

Large macro blocks, predesigned datapaths, embedded memories, and analog blocks are increasingly used in application-specific integrated circuit (ASIC) designs. However, robust algorithms for large-scale placement of such designs have only recently been considered in the literature. Large macros can be handled by traditional floorplanning, but are harder to account for in min-cut and analytical placement. On the other hand, traditional floorplanning techniques do not scale to large numbers of objects, especially in terms of solution quality. The authors propose to integrate min-cut placement with fixed-outline floorplanning to solve the more general placement problem, which includes cell placement, floorplanning, mixed-size placement, and achieving routability. At every step of min-cut placement, either partitioning or wirelength-driven fixed-outline floorplanning is invoked. If the latter fails, the authors undo an earlier partitioning decision, merge adjacent placement regions, and refloorplan the larger region to find a legal placement for the macros. Empirically, this framework improves the scalability and quality of results for traditional wirelength-driven floorplanning. It has been validated on recent designs with embedded memories and accounts for routability. Additionally, the authors propose that free-shape rectilinear floorplanning can be used with rough module-area estimates before logic synthesis

Classical floorplanning minimizes a linear combination of area and wirelength. When simulated annealing is used, e.g., with the sequence pair representation, the typical choice of moves is fairly straightforward. In this paper, we study the fixed-outline floorplan formulation that is more relevant to hierarchical design style and is justified for very large ASICs and SoCs. We empirically show that instances of the fixed-outline floorplan problem are significantly harder than related instances of classical floorplan problems. We suggest new objective functions to drive simulated annealing and new types of moves that better guide local search in the new context. Wirelength improvements and optimization of aspect ratios of soft blocks are explicitly addressed by these techniques. Our proposed moves are based on the notion of floorplan slack. The proposed slack computation can be implemented with all existing algorithms to evaluate sequence pairs, of which we use the simplest, yet semantically indistinguishable from the fastest reported . A similar slack computation is possible with many other floorplan representations. In all cases the computation time approximately doubles. Our empirical evaluation is based on a new floorplanner implementation Parquet-1 that can operate in both outline-free and fixed-outline modes. We use Parquet-1 to floorplan a design, with approximately 32000 cells, in 37 min using a top-down, hierarchical paradigm.

The preferential use of convective modes of hemodialysis (HD) for targeting hyper-cytokinemia state in sepsis-related acute kidney injury (AKI) has been questioned for its efficacy. Several studies have used predilution hemodiafiltration (HDF) in critically ill AKI patients with mixed results. In this study, we compared intermittent online postdilution HDF with the standard high-flux (HF) intermittent HD in non-critically ill patients with community-acquired (CA) AKI. In this pilot study, stable patients with CA AKI and systemic inflammatory response syndrome were included and given either postdilution online-HDF (OL-HDF) or standard HF HD outside intensive care units. The primary objectives were to assess the feasibility of conducting the study at a larger scale and to detect the differential impact of convective clearance on the rates of independence from dialysis at discharge or after 30 days. Plasma cytokine clearance was assessed as a secondary objective. Eighty consecutive AKI patients were randomized to receive dialysis in one of the treatment arms after fulfilling the eligibility criteria. The baseline parameters of clinical severity, etiology, and indications of dialysis, plus the baseline plasma cytokine profiles, were comparable. Moreover, 83% in the control arm and 71.1% in the intervention arm became independent from dialysis at discharge or at 30 days (P = 0.189). No survival advantage of postdilution OL-HDF was observed (P >0.05). Similar plasma cytokine clearance levels were noted in both arms. The current study confirms the feasibility; however, it does not support the preferential use of postdilution OL-HDF over HF-HD in non-critical patients.

Introduction: To determine the periodontal status and treatment needs using CPITN in a group of adult diabetic patients attending OPD in a private dental clinic in Ahmedabad. Materials and Methods: One hundred and two diabetic patients were screened by a single examiner according to the WHO criteria using a mouth mirror and CPITN probe. The patients were selected from the routine OPD after obtaining verbal consent for the same, and about 76 patients were included in the study, which was carried out for 9 months. Results: Results showed that the highest number of patients had code 3. The majority of sextants had code 2 and 3. Amongst all, 42 patients had a TN score of 2 and 33 had TN score of 3. 56–65 age groups had the worst periodontal status among all groups and required the highest periodontal treatment. Conclusion: The periodontal health of our diabetic patients reinforces the need to establish a comprehensive oral health promotion programme for diabetic patients with a multidisciplinary approach.

The goal of this research was to describe treatment patterns, health-care resource utilization, and costs for herpes zoster (HZ)-related pain, and to estimate the potential impact of recombinant zoster vaccine (RZV) on avoided HZ cases and HZ-related pain prescriptions. This retrospective claims database study included patients from commercial, Medicare, and Medicaid plans between 2012 and 2017. Subjects with an HZ episode were assigned to three cohorts: “opioid”, “non-opioid”, and “no-treatment” cohorts. Subjects in the opioid cohort were matched to a non-HZ cohort. The potential impact of RZV vaccination on HZ case avoidance and resulting painkiller prescriptions was modeled. Over 25% of subjects with an HZ episode received opioids. Adjusted health-care costs were approximately double in the opioid cohort versus non-opioid or matched non-HZ cohorts. Postherpetic neuralgia, immunocompromised status, and comorbidities increased the risk for opioid prescription. RZV vaccination was predicted to avoid over 19,000 patients from receiving opioid prescriptions for every 1 million adults aged ≥50 years. HZ-related prescriptions of opioids were common and led to increased health care costs. RZV vaccination may potentially reduce opioid prescriptions through decreasing HZ incidence. PLAIN LANGUAGE SUMMARY What is the context? Herpes zoster or shingles and its complications such as postherpetic neuralgia – a painful condition that affects the nerve fibers and skin – may lead to complex pain that can be addressed using opioids in some patients. The recombinant zoster vaccine (RZV) vaccine prevents shingles and, therefore, may reduce the use of opioids and the negative health outcomes and costs associated with it. What is new? In this retrospective medical claims study, including patients between 2012 and 2017, we evaluated the receipt of pain medication including opioids in herpes zoster patients, and assessed factors associated with opioid prescription. estimated health care resource utilization and costs associated with opioid use among patients with herpes zoster. assessed the impact of vaccination on opioid prescriptions. Among subjects receiving opioids, 78.5% started with a weak opioid dose. Dose escalation was uncommon. Postherpetic neuralgia, immunocompromised status, and comorbidities are the main risk factors associated with opioid prescription. Health care costs are almost double in patients with herpes zoster receiving opioids compared with patients without an opioid prescription. In a population of 1 million adults aged 50 years or older, vaccination with the recombinant zoster vaccine could prevent over 19,000 patients from receiving opioids. What is the impact? Prevention of herpes zoster through vaccination may be a highly effective strategy to reduce opioid prescriptions and costs related to pain management in a susceptible population. Increasing RZV vaccination coverage in adults aged ≥50 years may further reduce potential opioid prescriptions through a decrease in shingles incidence.

Study Objective To study the technical feasibility of performing a radical hysterectomy via laparoscopic single-site surgery LESS. Patient A 45-year-old woman with cervical cancer stage IA2. Intervention Biopsy report showed invasive keratinizing squamous cell carcinoma, grade II. We duplicated the steps of our “Pune technique” of laparoscopic radical hysterectomy to perform a radical hysterectomy via laparoscopic single-site surgery using conventional ports and instruments. Measurements and Main Results Oncologic clearance was comparable to that of conventional laparoscopic radical hysterectomy. Operative time was 120 minutes, and blood loss was 50 mL. Conclusion Reduced port laparoscopic radical hysterectomy is technically feasible. Oncologic clearance and functional results are comparable to those of the multiport variant, with good cosmetic results.

Background: Ectodermal dysplasia (ED) is a group of rare, inherited disorders characterized by sparse hair, missing teeth and inability to sweat. Objective: To review and analyze cases of ED with an emphasis on clinical manifestations and parent's marriage history. Methodology: The present retrospective study was conducted by assessing the clinical records of nineteen cases of ED, available in the archives of the department; for age, gender, family history of consanguineous marriage and clinical manifestations. Results: It was observed that ED was more prevalent in males, with a ratio of 1.7:1. The hypohydrotic type was more common (78.95%) than hydrotic type (21.05%). The marriage history of parents revealed that 66.67% had consanguineous marriage and had 68.42% offspring's affected with ED; whereas 33.33% had history of non-consanguineous marriage and had 31.58% offspring's affected with ED. The clinical manifestations observed were- dry skin(94.74%); scaly skin(42.11%); sparse hair on scalp, eyebrows and eyelashes(100%); frontal bossing(63.18%); saddle nose (57.89%); hypertelorism (47.37%); nail abnormality(52.63%); normal sweat glands(21.05%); abnormal sweat glands(78.95%); hypoplastic maxilla(52.63%); protuberant lips (57.89%); palmo-plantar keratosis(21.05%); wrinkled & hyper pigmented facial skin(84.21%); partial anodontia(94.74%); conical shaped teeth(84.21%); high arched palate(68.42%); thin alveolar bone(100.00%); taurodontism(21.05%) and cleft lip & cleft palate(05.26%). The number of teeth present in all the cases ranged from 0 to 19. Conclusion: ED patients suffer from social problems and poor psychological and physiological development as a result of unacceptable esthetics and abnormal function of orofacial structures. Oral rehabilitation thus becomes mandatory, although it is often difficult; particularly in pediatric patients.

To retrospectively evaluate the complications of the laparoscopic pelvic surgeries and to formulate the guidelines to avoid them.Retrospective study (Canadian Classification).Advanced Laparoscopic Institute.Nine hundred and seven operated for gynecological malignancies.Laparoscopic surgeries.567 women suffering from different pelvic conditions were studied in a period of 60 months. The median age of the patient was 35 (11-80). Complications occurred in 32 patients (32/567, 5.5 %). The overall incidence of urinary tract injury in all the advanced cases at our institute was 2.1 % (12/567). The incidence of bowel injury at our center was 1.76 %. The incidence of vascular injury at our institute was 1.76 % (10/567).Laparoscopic complications are different than those seen following open surgeries. Anticipation, early recognition, and timely intervention help to reduce morbidity. Laparoscopic management of complications is possible. Formulating standard guidelines can help to avoid many such complications.

Objective To present the initial experience with robotic anterior pelvic exenteration in patients with advanced pelvic cancer at Galaxy Care Laparoscopy Institute, Pune, India. Methods A retrospective chart review of data from 10 patients with advanced cervical carcinoma and bladder involvement or with vault recurrence following hysterectomy who were treated at the study hospital between November 2009 and May 2011. Clinicopathologic data and postoperative data including operative time, blood loss, blood transfusions, hospital stay, lymph node yield, and complications were recorded. Results The mean operative time was 180 minutes, the mean blood loss was 110 mL, and the mean duration of hospital stay was 5 days. There were no treatment-related morbidities or mortalities. A mean parametrial clearance of 3 cm with a distal vaginal margin of 3.5 cm was achieved. All patients had tumor-free margins. The mean number of harvested lymph nodes was 24. Six patients had positive lymph nodes on pathologic examination and were treated with chemoradiotherapy. At a median follow-up of 11 months, 8 patients were disease-free. Conclusion Robot-assisted anterior pelvic exenteration had favorable operative, pathologic, and short-term clinical outcomes. A large multicenter study is required to confirm the results.

Objectives:To assess prevalence and pattern of movement disorders among patients taking antipsychotic medications. Methods:This cross-sectional, intensive monitoring (patient interview, case record form review and clinical examination) study was conducted in patients taking antipsychotic drugs irrespective of duration for the development of movement disorders. The psychiatrist used Modified Simpson-Angus Scale score (10-item scale), Barnes’ rating scale and Abnormal Involuntary Movement Scale to diagnose parkinsonism, akathisia and tardive dyskinesia, respectively. We assessed movement disorders for the preventability and seriousness. Results:The overall prevalence of antipsychotic induced movement disorders was 5.67% (95% CI: 4.19–7.62). The prevalence of parkinsonism, akathisia and tardive dyskinesia was 5.10% (95% CI: 3.71–6.98), 0.85% (95% CI: 0.39–1.84) and 0.57% (95% CI: 0.22–1.45), respectively. There was a trend of high proportions of movement disorders in extreme of age group, female gender, patients treated with conventional antipsychotics, on poly therapy, patients of epilepsy with psychosis, schizophrenia and bipolar mood disorder. The movement disorder was lowest with quetiapine (2.02%). Conclusions:The higher use of atypical antipsychotics had reduced the occurrence of movement disorders in our setup. The multi-input multi-output–orthogonal frequency division multiplexing (MIMO–OFDM) system serves as one of the most promising techniques that support high data rate and high performance in diverse fading channel conditions. The paper presents an overview of MIMO–OFDM wireless technology, covering advances in physical-layer design, various space–time codes, space–frequency codes and efficient concatenated forward error correction techniques, along with interleaving to yield reduced bit error rate performance (BER). Further, the paper presents the evolution of various performance metrics that are reported since the 2000s in the literature. The evolution outcome is examined using a quadratic nonlinear fitting function to assess the interesting performance metrics. Since BER versus SNR analysis is found as the interesting performance study of the MIMO–OFDM system, the current status of the achieved BER under varying SNR is reviewed and presented. 186. Use of CytoSorb therapy to treat critically ill coronavirus disease 2019 patients: a case series

Abstract Background Acute respiratory distress syndrome is an important clinical presentation of respiratory complications caused by severe acute respiratory syndrome coronavirus 2, a novel coronavirus responsible for the ongoing pandemic. The disease is poorly understood, and immunopathogenesis is constantly evolving. Cytokine release syndrome remains central to pathology of coronavirus disease 2019. Antivirals, anticytokine treatment, and other pharmacological approaches have failed to treat it. CytoSorb, an extracorporeal cytokine adsorber that reduces the cytokine storm and other inflammatory mediators in the blood, seems promising in treating severely ill patients with coronavirus disease 2019. Case presentation This article presents three cases of Asian ethnicity of severely ill adult patients with coronavirus disease 2019 admitted to intensive care unit who were treated with CytoSorb therapy. All patients used single CytoSorb device. During their clinical course, all patients were prescribed tocilizumab (an interleukin-6 receptor blocker), antivirals, hydroxychloroquine, azithromycin, and other antibiotics and general antipyretic drugs. No vasopressor treatment was required. The patients’ average duration of stay in intensive care unit was 30 days; the average duration of stay in hospital was 31 days. All three patients showed significant improvement in biochemical parameters and clinical outcomes post CytoSorb therapy. C-reactive protein levels decreased by 91.5%, 97.4%, and 55.75 %, and mean arterial pressure improved by 18%, 23%, and 17 % in patient 1, 2, and 3, respectively, on day 7 post-therapy. Conclusions All three patients improved clinically and survived.

187. Torque ripple minimization in Direct Torque Control based induction motor drive using multilevel inverter

In the classical DTC scheme using 2 level inverter, due to the limited number of voltage vectors large and small errors in torque or flux cannot be distinguished. In fact switching vectors chosen for large errors are same as for small errors. Because of this reason torque and flux ripples are high in case of classical DTC scheme using 2 level inverter. In this work in order to overcome this problem multi level inverter is used instead of 2 level inverter. This work presents a novel Direct Torque Control algorithm to be applied in induction motor electric drives supplied by 3-level Diode Clamped inverters. Results obtained with numerical simulation show that the employment of a 3 level inverter in such a control scheme permits to obtain the same dynamic performances as those obtained with a 2 level inverter with resulting far lower torque ripple.

This paper illustrates the implementation of the direct torque control (DTC) strategy for induction motor (IM) drive using carrier space vector pulse width modulation technique. In the present work, the algorithm used for DTC based control of IM is Carrier Space Vector Pulse Width Modulation (CSVPWM) technique. A comparison of performance between standard and carrier based SVPWM techniques have been made considering the presence of harmonics in phase voltage, phase current and stator flux linkage which are responsible for the torque ripple. Using simulation results relative merits of the standard SVPWM (SSVPWM) and CSVPWM have been evaluated. Overall performance of conventional, SSVPWM and CSVPWM DTC based IM drive from the viewpoint of torque ripple minimization have been evaluated using simulation results. Simulation results are experimentally validated.

In the present work a novel strategy for optimum design of flux and torque controller for carrier space vector pulse width modulation based direct torque control (CSVPWM DTC) of induction motor (IM) drive has been implemented. Two positive and one negative torque slope levels per half the switching period has been predicted for CSVPWM DTC IM drive based on which a new optimal control strategy is proposed for the design of flux and torque controllers where, a P controller is used in the flux loop and a PI controller is used in the torque loop. Performance investigation shows that optimally tuned controllers used in CSVPWM DTC technique are helpful in reducing torque ripple. In proposed control strategy flux and torque slopes are derived using d-q axis induction motor model in stationary reference frame. These slope parameters are respectively utilized to find the transfer function (TF) of flux and torque loops. Using these TF, flux P and torque PI controller settings are obtained such that they give the best results in terms of reduced torque ripple for any flux and torque slopes. In order to show the superiority of the proposed control strategy for CSVPWM DTC IM drive over conventional DTC technique, comparison of simulation and hardware results are presented.

This paper describes the implementation of multirate sampling technique for carrier space vector pulse width modulation based direct torque control (CSVPWM DTC) of induction motor (IM) drive with an objective to reduce the electromagnetic torque ripple. The benefit of the proposed technique for CSVPWM DTC based IM drive; it gives lower torque ripple as compared to single rate sampling technique. In multirate sampling technique, for the same digital signal processor, two sampling periods are used. The fundamental sampling period which is smaller than the sampling period used in single rate sampling technique, is used for data capturing and estimation of speed, torque and flux parameters. The other sampling period which is an integer multiple of the fundamental sampling period, is used for speed, flux and torque error processing and also for the implementation of CSVPWM algorithm for synthesizing gate pulses. Further, due to lower fundamental sampling period in multirate sampling technique better resolution in switching pulse widths can be obtained. Using simulation results relative merits of the multirate and single rate control strategy have been evaluated from the viewpoint of torque ripple minimization. The simulation results are experimentally validated.

This study (ReCORD-FL) sought to construct a historical control cohort to augment single-arm trials in relapsed/refractory follicular lymphoma (r/r FL). A retrospective study in 10 centers across North America and Europe was conducted. Adults with grade 1–3A FL were required to be r/r after ≥2 therapy lines including an anti-CD20 and an alkylator. After first becoming r/r, patients were required to initiate ≥1 additional therapy line, which defined the study index date. Endpoints were observed from start of each therapy line (including index line) until death, last follow-up, or December 31, 2020. Endpoints were complete response (CR) rate, overall response rate (ORR), time to next treatment or death (TNT-D), event-free survival (EFS), and overall survival (OS). One hundred eighty-seven patients were identified. Most patients’ (80.2%) index therapy occurred in third line (3L) (range, 3L–6L). Median follow-up from FL diagnosis was 9 years (range, 1–21 years). CR and ORR to the index therapy were 39.0% and 70.6%, respectively. Median (95% confidence interval) EFS from index was 14.6 (11.0-18.0) months; median OS from index was 10.6 years. Outcomes worsened across successive treatment lines and for patients who were double refractory (r/r to both an anti-CD20 monoclonal antibody and an alkylator) or POD24 (progressed ≤24 months after front-line anti-CD20) at index. Findings demonstrate the unmet need of FL patients with multiply relapsed, double refractory, or POD24 disease. Based on robustness of the historical data collected and comparability with a previous study (SCHOLAR-5), ReCORD-FL presents a valuable source of control data for comparative studies in r/r FL.

Study Objective To study the feasibility, morbidity, and oncologic outcome of laparoscopic posterior exenteration in patients with advanced gynecologic malignant diseases. Design Retrospective study based on clinical experience (Canadian Task Force classification III). Setting Private hospital. Patients The medical records for 10 patients who underwent laparoscopic posterior exenteration because of advanced gynecologic malignant disease were retrospectively reviewed. Intervention Laparoscopic posterior exenteration involving selective resection of the uterus, ovaries, vagina, and rectum was performed using a 6-port technique that included harmonic shears, the LigaSure device, and a circular endostapling instrument. Measurements and Main Results Histopathologic diagnosis included carcinoma of the cervix in 5 patients, ovary in 4 patients, and vagina in 1 patient. Indication for surgery was primary disease in 7 patients and secondary disease in 3 patients. Complications included delayed bladder recovery in 4 patients, and anastomotic leak, wound infection, and prolonged ileus in 1 patient each. No conversions to open surgery were required. The extent of resection was supralevator in 8 patients and infralevator in 2. Median operative time was 210 minutes. Median length of hospital stay was 9 days. Median blood loss was 360 mL. Disease recurred in 1 patient. After a median follow-up of 26 months, 9 patients were alive, and 8 were free of disease. Conclusion Laparoscopic posterior exenteration is feasible in advanced gynecologic malignant disease with rectal involvement. In addition to the known benefits of laparoscopic surgery, carefully selected patients could achieve a survival benefit following R0 resection.

This paper describes the implementation of multirate sampling technique for 3-level carrier space vector pulse width modulation based direct torque control (CSVPWM DTC) of induction motor (IM) drive with an objective to reduce the electromagnetic torque ripple for medium to high power applications. The benefit of the proposed multirate technique for 3-level CSVPWM DTC based IM drive; it gives lower torque ripple as compared to single rate sampling technique. In multirate sampling technique, for the same digital signal processor, two sampling periods are used. The fundamental sampling period which is smaller than the sampling period used in single rate sampling technique, is used for data capturing and estimation of speed, torque and flux parameters. The other sampling period which is an integer multiple of the fundamental sampling period, is used for speed, flux and torque error processing and also for the implementation of CSVPWM algorithm for synthesizing gate pulses. The 3-level inverter gives the smaller voltage steps lead to the production of higher power quality waveforms. This give superior performance compared to two-level inverters from the view point of lower common-mode voltage, lower harmonics in output voltage and current, and reduced voltage on the power switches Using simulation results relative merits of the multirate 3-level and single rate 2-level control strategy have been evaluated from the viewpoint of torque ripple minimization. The simulation results are experimentally validated. Index Terms-- Induction motor (IM) drive, Multirate sampling, direct torque control (DTC), torque ripple, space vector pulse width modulation (SVPWM), voltage source inverter (VSI). I. INTRODUCTION Present work demonstrates the improvement in performance of a CSVPWM DTC based IM drive using multirate sampling technique. The basic problems associated with the conventional DTC (1, 2) based IM drive are variable switching frequency and high torque ripple. In the past various modifications have been suggested by the researchers to overcome the above problems associated with conventional DTC drive. In case of conventional DTC, depending on the sampling period and the width of the hysteresis band, selected active voltage may create overshoot and undershoot which are responsible for the torque ripple. The above problems of conventional DTC are addressed by various researchers in (3, 4) using space vector pulse width modulation (SVPWM) principle for wave fabrication in DTC based IM drive. SVPWM technique using symmetrical regular sampling provides constant switching frequency operation of the inverter which takes care of the problem of variable switching frequency present in conventional DTC. For DSP based implementation of SVPWM technique, device switching frequency depends on the sampling period used in the program. It can be observed from the above that sampling period is an important parameter which limits the performance of conventional and SVPWM DTC based IM drive in terms of reduction of high torque ripple. In the present work the implementation of discrete CSVPWM DTC based IM drive system has been discussed for 3-level inverter. An attempt has been made to reduce the electromagnetic torque ripple in an IM drive by using multirate sampling technique for data capturing, processing and control. In (5-7), multirate sampling technique is discussed for the controlling the digital system. Multirate feed forward control and its application in position control of servo motor is discussed in (6). Multirate sampling technique has also been applied for modeling and control of wind energy generation scheme (7) in order to improve the reactive power injection for superior generator terminal voltage profile. In the present work the multirate sampling technique is not only used for control purpose but its major contribution is in the use for fast input data capturing and accurate estimation of stator flux, electromagnetic torque and speed which helps in accurate estimation of the stator flux linkage, corresponding speed and electromagnetic torque, which helps to reduce the electromagnetic torque ripple. In (8) a multirate sampling technique has been applied for SVPWM DTC based IM drive to further reduce its electromagnetic torque ripple.

Research in floorplanning and block-packing has generated a variety of data structures to represent spatial configurations of circuit modules. Much of this work focuses on the geometry of module shapes and seeks tighter packing, as well as improvements in the asymptotic worst-case complexity of algorithms for standard tasks. In this work we consider the implications of interconnect optimization on the value of floorplan representations and establish a framework for comparing different representations. By analyzing performance bottlenecks in block packing and properties of floorplan representations, we show that many of the mathematical results in floorplanning do not translate into better VLSI layouts. This is confirmed by extensive empirical data for stand-alone floor-planners and integrated applications.

Large macro blocks, pre-designed datapaths, embedded memories and analog blocks are increasingly used in ASIC designs. However, robust algorithms for large-scale placement of such designs have only recently been considered in the literature, and improvements by over 10% per paper are still common. Large macros can be handled by traditional floorplanning, but are harder to account for in min-cut and analytical placement. On the other hand, traditional floorplanning techniques do not scale to large numbers of objects, especially in terms of solution quality. We propose to integrate min-cut placement with fixed-outline floor-planning to solve the more general placement problem, which includes cell placement, floorplanning, mixed-size placement and achieving routability. At every step of min-cut placement, either partitioning or wirelength-driven, fixed-outline floorplanning is invoked. If the latter fails, we undo an earlier partitioning decision, merge adjacent placement regions and re-floorplan the larger region to find a legal placement for the macros. Empirically, this framework improves the scalability and quality of results for traditional wirelength-driven floorplanning. It has been validated on recent designs with embedded memories and accounts for routability. Additionally, we propose that free-shape rectilinear floorplanning can be used with rough module-area estimates before synthesis.

Posterior pelvic exenteration (PPE) has been used as modality of treatment for recurrent or primary cancer of rectum that has invaded into the female genital tract. We report a case of PPE performed for locoregionally advanced carcinoma of rectum invading the uterus; which was performed in a totally laparoscopic manner. The handsewn coloanal anastomosis was performed transrectally thus obviating the need for even a minilaparotomy for abdominal access for specimen retrieval or anastomotic stapler application. This is a first reported case in the literature where a total laparoscopic PPE was performed with successful outcome and oncological adequacy and safety.

Objective To compare the efficacy and efficiency of retrobulbar versus sub–Tenon's capsule injection of local anesthetic in vitreoretinal surgery. Design Prospective, randomized, double-masked clinical trial. Participants and Intervention Sixty-four eyes from 61 patients undergoing vitreoretinal surgery were randomized to receive either retrobulbar or sub–Tenon's capsule injection of 5 ml of a 50:50 mixture of 4% lidocaine and 0.75% bupivacaine. Main Outcome Measures The primary outcome measured was intraoperative eye pain, which was rated by patients in both groups using an 11-point (0–10) numerical visual analogue scale immediately after surgery and again the next morning. The surgeons indicated whether they perceived patient discomfort during 4 different stages of the operation: opening of the conjunctiva, vitrectomy (if performed), placement of scleral buckle (if performed), and closing of the conjunctiva. The preincision time, need for supplemental local anesthesia, and use of IV sedation for additional pain control were compared between the two groups. Results Thirty-four eyes were randomized to retrobulbar injections, and 30 eyes were randomized to sub–Tenon's capsule injections. There was no significant difference in patient-reported intraoperative pain scores between the retrobulbar and sub–Tenon's capsule groups when assessed immediately after surgery (median, 2.0 vs. 2.0; P = 0.52) or the next day (median, 2.0 vs. 1.0; P = 0.26). The surgeons reported no difference between the two groups in terms of the percentages of patients with pain during opening of the conjunctiva (20.6% vs. 3.3%; P = 0.058), vitrectomy (31% vs. 32%; P = 1.00), placement of scleral buckle (33.3% vs. 40%; P = 1.00), and closing of the conjunctiva (26.5% vs. 26.7%; P = 1.00). There was a suggestion that preincision time was longer in the sub–Tenon's capsule group. Approximately equal percentages of patients in each group required supplemental local anesthesia (38% vs. 37%; P = 0.90) or IV medication (85% vs. 70%; P = 0.14) for pain control. Conclusions Sub–Tenon's capsule injection of local anesthetic seems as effective as retrobulbar injection at controlling intraoperative pain in vitreoretinal surgery.
