Based on available literature, pregnant women are at an increased risk of severe illness from severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, compared to nonpregnant women. Consequences of coronavirus disease 2019 (COVID-19) in pregnancy have many implications in women’s lives other than unfavorable obstetric outcomes. In addition to managing acute respiratory illness and symptoms, caregivers should be equipped to detect and manage the short-term, intermediate, and long-term consequences of SARS-CoV-2 infection as well. Many pregnant women can remain asymptomatic and continue their pregnancy without being diagnosed. Pregnancy outcomes and consequences of SARS-CoV-2 infected yet asymptomatic mothers have not been very well explained. Reports of a new multisystem inflammatory syndrome in children (MIS-C) and multisystem inflammatory syndrome in adults (MIS-A) following COVID-19 have been described. However, MIS-A in pregnancy is an extremely rare presentation that can cause a huge diagnostic dilemma to caregivers. We describe the successful management of a pregnant woman with MIS-A following SARS-CoV-2 infection.

Burn is a common case in developing countries, with over half of fire-related deaths reported in Southeast Asia and full-thickness burns as a high mortality risk. Human amnion has been used as a wound dressing for centuries. In this study, a decellularised amnion overlaid with fibrin, “amnion bilayer (AB),” was used as a dressing immediately after burn and as a graft to replace the scar in Sprague-Dawley rats subjected to full-thickness burn model. The aim was to observe whether amnion bilayer can reduce damages in third-grade burn when skin replacement is deemed impossible. The burn was induced using an electrical solder, heated for 5 mins, and contacted on the rat’s bare skin for 20 s. AB was applied as a (i) dressing immediately after induction and graft after eschar removal. Two groups (n = 6) were compared: AB and Sofra-Tulle ®, the National Hospital of Indonesia (NHI) protocol. Sections were stained with hematoxylin and eosin and Masson trichrome stains. Immunohistochemistry labelling was used to indicate scars (α-smooth muscle actin [α-SMA] and collagen-1) and angiogenesis (von Willebrand factor). Also, the macrophages inflammatory protein-3α (MIP-3α) indicates an early inflammatory process. The post dressing of the AB group demonstrated hair follicle remains and adipose tissue development. The NHI group appeared with a denatured matrix. Complete healing was seen in the AB group after 28 days with skin appendages similar to normal, while the NHI group showed no appendages in the centre of the actively inflamed area. The α-SMA was found in both groups. Collagen-1 was highly expressed in the NHI group, which led to a scar. Angiogenesis was found more in the AB group. The AB group had shown the capacity to accelerate complete healing and recover skin appendages better than the current protocol.

Although several studies have been conducted to summarize the progress of open educational resources (OER) in specific regions, only a limited number of studies summarize OER in Africa. Therefore, this paper presents a systematic literature review to explore trends, themes, and patterns in this emerging area of study, using content and bibliometric analysis. Findings indicated three major strands of OER research in Africa: (1) OER adoption is only limited to specific African countries, calling for more research and collaboration between African countries in this field to ensure educational equity; (2) most of the OER initiatives in Africa have focused on the creation process and neglected other important perspectives, such as dissemination and open educational practices (OEP) using OER; and (3) on top of the typical challenges for OER adoption (e.g., infrastructure), other personal challenges were identified within the African context, including culture, language, and personality. The findings of this study suggest that more initiatives and cross-collaborations with African and non-African countries in the field of OER are needed to facilitate OER adoption in the region. Additionally, it is suggested that researchers and practitioners should consider individual differences, such as language, personality and culture, when promoting and designing OER for different African countries. Finally, the findings can promote social justice by providing insights and future research paths that different stakeholders (e.g., policy makers, educators, practitioners, etc.) should focus on to promote OER in Africa.

Objective: To assess the level of knowledge and practice on malaria prevention among the residents of Ratuwamai Municipality in Nepal. Methods: A community-based cross-sectional study was carried out on Ratuwamai Municipality of Ward No. 8 in Morang District, Nepal. A total of 352 respondents with an age range from 15 to 70 years were interviewed from October 9 to October 31, 2022. Statistically significant differences between two or more categorical groups were measured using independent sample t-test, one-way ANOVA test, and Kruskal-Wallis H test. Result: Out of the total sample, 57.7% of the respondents were 24 to 50 years old with a mean age of (36.9±14.3) years. In the knowledge test, 82% of the responses were accurate. Though 98.6% of people had heard of malaria, only 48.3% said it was a communicable disease. In practice, 84.9% always cleaned the surrounding area and trimmed the bushes around the house; 75.9% used insecticide-treated bed nets or normal bed nets for sleeping; only 7.4% used mosquito repellent creams on exposed body parts while staying outdoors at night, working in fields, and sleeping. Conclusions: The majority had knowledge about malaria, but more than half had poor practice. We advise concerned authorities to promote the behavior change communication strategy to encourage individuals to adopt healthy, beneficial, and good behavioral practices.

This study evaluates the earthquake-resistant performance of Meru, specifically level eleven, located in the Ulun Danu Batur Kintamani Temple Area, Bali Province. Despite the upper layer’s construction with wooden frames and panels, and the base layer’s masonry walls, Meru has remained remarkably undamaged by earthquakes since its establishment in 1968. The analysis encompasses Meru’s architectural forms, structural systems, building materials, and dynamic response to seismic activity. A 3D finite element model and nonlinear time history dynamic analysis methods were employed to investigate its earthquake resistance. The findings indicate that Meru can withstand credible maximum seismicity for a return period of 2500 years, and the natural period and modes of Meru have been determined. This study serves as an initial investigation into predicting and monitoring the behavior of Meru structures during earthquakes. Future research could involve experimental analysis of individual components and the overall structure to further validate the analytical model. As a result, this study offers significant insights into the upkeep and preservation of traditional Balinese historic buildings in an earthquake-prone area.

The Chrysanthemum is a subtropical ornamental plant that is usually grown in highland areas. Indonesia is a tropical area consisting of highland and lowland areas. Chrysanthemum was growing well in highland areas in Indonesia but was not cultivated yet in lowland areas. The research objectives were to evaluate the phenotype of Chrysanthemum varieties in vegetative stadia and assess the survival abilities of the Chrysanthemum varieties in the low area. Four varieties were evaluated and assessed to be cultivated in lowland areas (187.6 meters above sea level). They were Jayani, Sabiya, Swarna Kencana, and Trissa varieties. The experimental design was using Randomized Complete Block Design (RCBD), which consisted of 4 replications, with each replication consisting of 4 varieties, and each variety consisting of 20 plants. The parameters observed were the plant height (cm), number of leaves, leaves width (cm), leaves length (cm), total number of nodes, and internode length (cm). All varieties were adapted and grew well in the lowland area. There were many differences in morphological and quantitative characters in vegetative stadia. Chrysanthemum cv. Swarna Kencana showed the highest plant height in a low area. These varieties also showed the highest survival ability in the lowland area. Swarna Kencana was the Yellow Orange group as consumer preference. It was suggested that chrysanthemums be adapted in the lowland area. The implication of the research was Swarna Kencana could be a genetic resource and germplasm for breeding materials.

The popularity and demand of alternative beverages made from plants have gained attention over the last year due to health reasons. Cereal plants such as sorghum (Sorghum bicolor L.) and adlay (Coix lacryma-jobi L.) can be utilized as raw materials for the production of plant-based milk. However, information on the development of sorghum-adlay milk is still limited, especially regarding the ingredients used and their influence on the characteristics of the final product. The aim of this research was to investigate the effect of sugar and non-dairy creamer (NDC) addition on the sensory acceptance, color, physical stability, total soluble solids, pH, and antioxidant activity of sorghum-adlay milk. There were 15 samples formulated with different concentrations of sugar (4-20%) and NDC (0-2%). The results showed that samples with 20% sugar and 2% NDC had the highest sensory acceptance by the panelists. People around the world want to be able to shop for the fastest, easiest, and cheapest way to buy things. Because of this phenomenon, the traditional merchant has become an unfriendly alternative for some of the population for various purchases. E-Commerce can be a strategy that has a role to help merchants carry out sales activities more quickly and efficiently and make it easier for the merchant to introduce their products. The method used was Rapid Application Development (RAD), System Usability Scale (SUS), and User Experience Questionnaire (UEQ). The questionnaires were distributed to users through Google form media. The SUS test results showed that 78% of the DessGo application system met the usability standards well. The UEQ test results included the values of Attractiveness (1.98), Perspicuity (2.05), Efficiency (1.98), Dependability (1.86), and Stimulation (1.86) so that it is included in the Excellent category, but Novelty (1.18) has the smallest value it requires more innovative and creative updates such as adding elements of gamification to apply to the application. As many as 50 respondents participated in the application testing. The application was deemed simple to use, understandable, and clear by respondents.

Agricultural marketing research continues to grow and develop with the support of the entrepreneurial ecosystem. This study aims to visually study the mapping and trends of research in agricultural marketing through bibliometric analysis of domain and intellectual structure perspective. This study uses a bibliometric approach with secondary data from the Scopus database. The VOSViewer program and Scopus search results analysis function were used for data analysis and visualization. This work analyzed 704 scientific documents published from 1931 to 2022. The study results have revealed that the United States of Agricultural (USDA) was the conservation agency most active in agricultural marketing publications. Agricultural science and biology were the most productive fields of study. Based on the assistance of a collection of knowledge created from 91 years of scientific publications, this study suggests a classification of the conceptual themes for agricultural marketing studies: Food, Agriculture, Agribusiness, Marketing, and Economics, abbreviated as the FAAME research theme.

Machine learning (ML) has found broad applicability in quantum information science in topics as diverse as experimental design, state classification, and even studies on quantum foundations. Here, we experimentally realize an approach for defining custom prior distributions that are automatically tuned using ML for Bayesian quantum state estimation methods that generally better conform to the physical properties of the underlying system than standard fixed prior distributions. Previously, researchers have looked to Bayesian quantum state tomography for advantages like uncertainty quantification, the return of reliable estimates under any measurement condition, and minimal mean-squared error. However, practical challenges related to long computation times and conceptual issues concerning how to incorporate prior knowledge most suitably can overshadow these benefits. Using both simulated and experimental measurement results, we demonstrate that ML-defined prior distributions reduce net convergence times and provide a natural way to incorporate both implicit and explicit information directly into the prior distribution. These results constitute a promising path toward practical implementations of Bayesian quantum state tomography.

In recent years, research into and concern over algorithmic fairness and bias in machine learning systems has grown significantly. It is vital to make sure that these systems are fair, impartial, and do not support discrimination or social injustices since machine learning algorithms are becoming more and more prevalent in decision-making processes across a variety of disciplines. This abstract gives a general explanation of the idea of algorithmic fairness, the difficulties posed by bias in machine learning systems, and different solutions to these problems. Algorithmic bias and fairness in machine learning systems are crucial issues in this regard that demand the attention of academics, practitioners, and policymakers. Building fair and unbiased machine learning systems that uphold equality and prevent discrimination requires addressing biases in training data, creating fairness-aware algorithms, encouraging transparency and interpretability, and encouraging diversity and inclusivity.

Enhancing user experience (UX) in interactive systems requires effective human-computer interaction (HCI). The relationship between people and computers has grown in significance as technology progresses, having an impact on many areas of our life. The main ideas and tactics used in HCI to enhance the user experience in interactive systems are examined in this abstract. Understanding the capabilities and constraints of both humans and computers forms the basis of HCI. HCI researchers and designers may develop interactive systems that complement users' mental models and cognitive processes by researching human behavior, cognition, and psychology. Additionally, taking into account the features of the computer system, such as its responsiveness, processing capacity, and interface design, enables the development of systems that are more user-friendly and effective. As a result, the discipline of human-computer interaction works to improve the user experience in interactive systems. Researchers and designers in the field of human-computer interaction (HCI) can produce interactive systems that are simple, effective, entertaining, and satisfying for users by comprehending human capabilities, applying user-centered design principles, utilizing interactive techniques and interfaces, integrating multimodal interfaces, and embracing emerging technologies. The continued development of HCI will continue to influence and enhance how people use computers, enhancing user experiences and creating new opportunities for interactive systems in the future.

We aim to unravel the interplay between bars, star formation (SF), and active galactic nuclei (AGNs) in barred galaxies. To this end, we utilize the SDSS DR12 to select a sample of nearby (0.02 < z < 0.06) disk galaxies that are suitable for bar examination ( M _r < −20.12 and inclination ≲53°). We identify 3662 barred galaxies and measure the length and axis ratio of each bar. We invent new bar parameters that mitigate the stellar and bulge mass biases and show, for the first time, that the evolution of non-AGN and AGN-hosting barred galaxies should be tracked using different bar parameters: the bar length for non-AGN galaxies and the bar axis ratio for AGN-hosting galaxies. Our analysis confirms that barred galaxies have a higher specific SF rate than unbarred control galaxies. Moreover, we find a positive correlation of bar length with both the SF enhancement and the centrally star-forming galaxy fraction, indicating the interconnectivity of bars and SF through the bar-driven gas inflow. We also find that, while the AGN fraction of barred galaxies is the same as that of the unbarred control sample, galaxies hosting more massive black holes (BHs) have rounder (i.e., higher axis ratio) bars, implying that the bar is not a cause of AGN activity; rather, AGNs appear to regulate bars. Our findings corroborate theoretical predictions that bars in non-AGN galaxies grow in length, and bars in AGN-hosting galaxies become rounder as BHs grow and eventually get destroyed.

We present an extensive catalog of 5405 early-type dwarf (dE) galaxies located in the various environments, i.e., clusters, groups, and fields, of the local universe ( z < 0.01). The dEs are selected through visual inspection of the Legacy survey’s g – r – z combined tricolor images. The inspected area, covering a total sky area of 7643 deg ^2 , encompasses two local clusters (Virgo and Fornax), 265 groups, and the regions around 586 field galaxies of M _K < −21 mag. The catalog aims to be one of the most extensive and publicly accessible collections of data on dEs, despite its complex completeness limits, which may not accurately represent its statistical completeness. The strength of the catalog lies in the morphological characteristics, including nucleated, tidal, and ultradiffuse dEs. The two clusters contribute nearly half (2437 out of 5405) of the dEs, and the 265 groups contribute 2103 dEs. There are 864 dEs in 586 fields, i.e., ∼1.47 dEs per field. Using a standard definition commonly used in literature, we identify 100 ultradiffuse galaxies (UDGs), which compose ∼2% of the dE population. We find that 40% of our sample dEs harbor a central nucleus, and among the UDG population, a majority (79%) are nonnucleated. About 1.3% of dEs suffer from ongoing tidal disturbance by nearby massive galaxies, and only 0.03% show a sign of recent dwarf–dwarf mergers. The association between dEs and their nearest bright neighbor galaxies suggests that dEs are more likely created where their neighbors are non-star-forming ones.

Chest radiographs (CXR) are the most performed imaging tests and rank high among the radiographic exams with suboptimal quality and high rejection rates. Suboptimal CXRs can cause delays in patient care and pitfalls in radiographic interpretation, given their ubiquitous use in the diagnosis and management of acute and chronic ailments. Suboptimal CXRs can also compound and lead to high inter-radiologist variations in CXR interpretation. While advances in radiography with transitions to computerized and digital radiography have reduced the prevalence of suboptimal exams, the problem persists. Advances in machine learning and artificial intelligence (AI), particularly in the radiographic acquisition, triage, and interpretation of CXRs, could offer a plausible solution for suboptimal CXRs. We review the literature on suboptimal CXRs and the potential use of AI to help reduce the prevalence of suboptimal CXRs.

This study aims to confirm the relationship between professional competence and multicultural competence of school counselors. The researchers collected data from 162 school counselors in the city of Banjarmasin who worked at the junior high school level, as well as high school / vocational school using a professional competency scale (? = 0.805) and multicultural (? = 0.889) totaling 45 items. An asphalt binder (PG 64−22) is modified with a crystallizable copolymer of polypropylene (PP) and polyethylene (PE) to enhance its ability to resist rutting in flexible pavements during periods of warm weather. After cooling of binders from the mixing temperature at a controlled rate of 3 °C/min, their microstructure and rheological behavior are characterized. The modified asphalts exhibit two-phase morphology with droplets of a maltene-rich polymer phase less than 50 μm in size dispersed in an asphaltene-rich, continuous phase. X-ray diffraction patterns and optical birefringence microscopy indicate that the PP/PE copolymer segregates from the matrix during cooling, forming a network of interconnected, crystalline fibrils. At temperatures of (50–80) °C, at which rutting is an issue in pavements, addition of PP/PE copolymer substantially increases the rutting factor by a factor of 2–6, depending on temperature and concentration. At temperatures of (―20 to 0) °C, addition of 5 wt.% or less of the PP/PE modifier has a minimal impact on performance based on both s-value and m-value. The modified asphalts are s-controlled, with ΔTc less than about 6.4 °C, suggesting that the PP/PE copolymer is an effective modifier for reducing rutting in flexible pavements.

Genotype-by-sequencing (GBS) was used to explore the genetic diversity and structure of Spinacia turkestanica, and the selective sweeps involved in domestication of cultivated spinach, S. oleracea, from S. turkestanica. A total 7,065 single nucleotide polymorphisms (SNPs) generated for 16 Spinacia oleracea and 76 S. turkestanica accessions placed the S. oleracea accessions in one group, Q1, and the 76 S. turkestanica accessions, which originated from Central Asia, in two distinct groups, Q2 and Q3. The Q2 group shared greater genetic identity with the S. oleracea accessions, Q1, than the Q3 S. turkestanica group. Likewise, the S. oleracea Q1 group had a smaller Fst (0.008) with the Q2 group than with the Q3 group (Fst = 0.012), and a greater gene flow (Nm = 30.13) with the Q2 group than with the Q3 group (Nm = 21.83). The Q2 accessions originated primarily from Uzbekistan while the Q3 accessions originated mostly from Tajikistan. The Zarafshan Mountain Range appears to have served as a physical barrier that largely separated members of the Q2 and Q3 groups of S. turkestanica. Accessions with admixtures of Q2 and Q3 were collected primarily from lower elevations at the southern end of the Zarafshan Mountain Range in Uzbekistan. Selective sweep regions identified at 32, 49, and 52 Mb on chromosomes 1, 2, and 3, respectively, appear to have played a vital role in the domestication of S. oleracea as they are correlated with important domestication traits, including day length sensitivity for bolting (flowering). High XP-CLR scores at the 52 Mb genomic region of chromosome three suggest that a selective sweep at this region was responsible for early differentiation of S. turkestanica into two groups in Central Asia.

Objective: The objectives of this study were to describe the incidence and genetic diversity of Rotavirus (RV) infection among children up to 3 years of age in a community in Nepal.Methods: We investigated community-acquired cases of asymptomatic and symptomatic RV infections in children from birth to 36 months of age in a community-based birth cohort in Bhaktapur, Nepal. Monthly surveillance and diarrheal stool samples were collected from 240 children enrolled at birth, of which 238 completed the 3 years of follow-up. Samples were screened for rotavirus by Enzyme Immuno Assay (EIA). All RV screened positives were further genotyped by reverse transcription-polymerase chain reaction for the capsid genes VP7 and VP4.Results: In total, 5,224 stool samples were collected from 238 children, followed from birth to 36 months of age. Diarrhea occurred in 92.4% (230/238) of all children in the cohort. During the 3 years study period, RV was more frequently seen in children with symptoms (7.6%) than in non-symptomatic children (0.8%). The highest RV detection rate was found in younger children between 3 and 21 months of age. Although rotavirus is known as winter diarrhea, it was detected throughout the year except in August. The highest positivity rate was observed in the months between December and March, with a peak in January. Four common G types were seen: G2 (30%), G1 (29%), G12 (19%), and G9 (16%). The most predominant genotypes seen were G2P[4] (30%), followed by G1P[8] (27.0%), G12P[6] (14.0%), G9P[8] (10%), and remaining were mixed, partial, and untyped.Conclusion: Our study confirms that rotavirus is a common cause of gastroenteritis in young children in the community. The prevalence and pathogenicity of rotavirus infection differed by age. There was substantial variability in circulating strains in the community samples compared to samples collected from hospitals. This shows the importance of including community-based surveillance systems to monitor the diversity of circulating rotavirus strains in Nepal.

Introduction: Lockdown imposed due to Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) caused by coronavirus led to disruption of all aspects of life across the world with serious consequences in delivering of routine surgical services to the patients. Anaesthesiologists and surgeons in the study Institute devised clinical assessment based protocols for preanaesthesia assessment {not including routine preoperative Coronavirus Disease 2019 (COVID-19) Testing} to ensure timely conduct of elective surgeries as soon as patients started reporting to hospital. Aim: To evaluate the efficacy of various measures taken by anaesthesiologists and operation theatre team to impart surgical care during COVID-19 pandemic without increasing the risk of transmission of COVID-19 disease. Materials and Methods: A retrospective observational study was carried out with the idea of comparing the surgical workload of year 2020 (January to December 2020) with the previous year 2019 (January to December 2019). The study was conducted in the month of March 2021, in a 627-bedded secondary care multispeciality hospital in northern India. The data on total number of patients undergoing surgeries in year 2020 was compared with that of year 2019 on the basis of elective surgeries, emergency surgeries and different modalities of anaesthesia used to conduct all those surgeries. These variables from both the years were compared using chi-square tests and Statistical Package for Social Sciences (SPSS) trial version 23.0. Results: The total numbers of surgeries had reduced in year 2020 (3923 in 2019 vs 2500 in 2020) with highest reduction in the months of April and May 2020 (83.08 and 74.75%, respectively). However, with implementation of clinical assessment based protocols and targeted changes in anaesthesia practice, the decrease in elective surgeries came to statistically non significant levels by the month of September 2020. The difference in percentage of cases carried out under regional anaesthesia (4.23% in 2019 vs 3.24% in 2020) was statistically not significant (p-value=0.051). The emergency surgeries in 2019 were 805 vs 742 in 2020, percentage change from 2019 base data was -7.83. The incidence of COVID-19 infection during postoperative period was nil in the Institute. The incidence of COVID-19 infection among the staff was found to be similar to the patients reported to the hospital despite of handling elective surgical cases. Conclusion: Regular surgical care can be safely provided after thorough clinical assessment of the patients in this ongoing pandemic. This did not lead to increase in incidence of COVID19 infection in healthcare workers as shown in our data. Hence, preoperative COVID-19 testing should be performed only when indicated by clinical history and examination. However, more studies with multiple centres are required to validate these protocols and hence facilitate the formation of even more refined ones.

Barley leaf rust caused by Puccinia hordei (Ph) is one of the major limiting biotic stresses of barley production worldwide and causes yield losses of up to 60%. A diversity panel of 316 barley genotypes (AM2017) composed of released cultivars, advanced breeding lines and landraces was screened for Ph resistance at the seedling stage using two isolates (SRT-SAT and SRT-MRC), while the adult plant stage resistance screening was conducted at the disease hotspot location of Sidi Allal Tazi (SAT) for the cropping seasons of 2017 and 2019. The phenotypic responses were combined with 36,793 single nucleotide polymorphism (SNP) markers in a genome-wide association study (GWAS) using the general linear model (GLM), mixed linear model (MLM), settlement of MLM under progressively exclusive relationship (SUPER), multiple-locus MLM (MLMM), fixed and random model circulating probability unification (FarmCPU), and Bayesian-information and linkage-disequilibrium iteratively nested keyway (BLINK) in GAPIT3, and MLM (K+Q), MLM (K+PCA), and GLM (Q) models in TASSEL to identify genomic regions linked to Ph resistance. Fourteen barley genotypes were resistant (R) at the seedling stage to both Ph isolates, SRT-SAT and SRT-MRC, and twelve genotypes were either resistant (R) or moderately resistant (MR) at the adult plant stage, whereas only one genotype was resistant at the seedling stage, and moderately resistant at the adult plant stage. The genome scan revealed 58 significant marker trait associations (MTA) among which 34 were associated with seedling resistance (SR) and 24 with adult plant resistance (APR). Common genomic regions conferring resistance to Ph were identified at both stages on chromosome 2H (106.53 cM and at 107.37 cM), and on chromosome 7H (126.7 cM). Among the 58 MTA identified, 26 loci had been reported in previous studies, while the remaining 32 loci were regarded as novel. Furthermore, the functional annotation of candidate genes (CGs) adjacent to 36 SNP markers with proteins involved in disease resistance further confirms that some of the SNP markers from our study could be associated with Ph resistance in barley. The resistant barley genotypes and some of the SNP markers from this study with high R2 and additive effects can be converted into high-throughput functional markers for accelerated selection and pyramiding of leaf rust resistance genes in North African barley germplasm.

Background: Sepsis, post-liver transplantation, is a frequent challenge that impacts patient outcomes. We aimed to develop an artificial intelligence method to predict the onset of post-operative sepsis earlier.Methods: This pilot study aimed to identify “physiomarkers” in continuous minute-by-minute physiologic data streams, such as heart rate, respiratory rate, oxygen saturation (SpO2), and blood pressure, to predict the onset of sepsis. The model was derived from a cohort of 5,748 transplant and non-transplant patients across intensive care units (ICUs) over 36 months, with 92 post-liver transplant patients who developed sepsis.Results: Using an alert timestamp generated with the Third International Consensus Definition of Sepsis (Sepsis-3) definition as a reference point, we studied up to 24 h of continuous physiologic data prior to the event, totaling to 8.35 million data points. One hundred fifty-five features were generated using signal processing and statistical methods. Feature selection identified 52 highly ranked features, many of which included blood pressures. An eXtreme Gradient Boost (XGB) classifier was then trained on the ranked features by 5-fold cross validation on all patients (n = 5,748). We identified that the average sensitivity, specificity, positive predictive value (PPV), and area under the receiver-operator curve (AUC) of the model after 100 iterations was 0.94 ± 0.02, 0.9 ± 0.02, 0.89 ± 0.01, respectively, and 0.97 ± 0.01 for predicting sepsis 12 h before meeting criteria.Conclusion: The data suggest that machine learning/deep learning can be applied to continuous streaming data in the transplant ICU to monitor patients and possibly predict sepsis.

Introduction: The pain after Laparoscopic Cholecystectomy (LC) which has both somatic and visceral component. Interfascial plane blocks play a major role in Multimodal Analgesia (MMA). Previous studies have found good analgesic benefits with Erector Spinae Plane (ESP) and Oblique Subcostal Transversus Abdominis Plane (OSTAP) blocks. However, till date no study exists which compares the above blocks with addition of dexamethasone. Aim: To compare ESP with OSTAP block using low concentration of Local Anaesthetic (LA) and dexamethasone as part of MMA in elective LC. Materials and Methods: A total of 66 patients were included in this study and finally, 60 patients were analysed. They were randomised to receive either bilateral ESP at T7 level or bilateral OSTAP with 20 mL 0.2% ropivacaine and 4 mg dexamethasone before starting anaesthesia. Primary outcome measures were total opioid consumption and mean Visual Analog Scale (VAS) in the first 24 hours postoperatively. Secondary outcome measures were intraoperative opioid consumption, opioids or block related complication, and patients’ feedback for procedural satisfaction and postoperative pain control. Background: Changing trends in the use of technology have become an impelling force to be reckoned with for the accounting and finance profession. The curriculum offered in higher learning institutions must be quickly revamped so that students who complete a bachelor’s degree are digitally competent upon graduation. With US$55.3 billion invested in FinTech in 2019 alone and more than 72% of accounting jobs being automated, graduates must be trained on digital skills to be future proof. Accounting and finance graduates must be made competent in skills that are related to digital content such as blockchain technology, information assets and autonomous peer to peer systems, to name a few. Methods: We used a three-phase approach: 1) careful mapping of digital topics taught within the course structure offered at these institutions; 2) review of current best practices and digital learning tools for digital inclusion which was ascertained from literature; and 3) 80 experts in a think tank group were interviewed on antecedents, awareness and problems in relation to digital inclusion within the curriculum to validate our research objective. Results: Eleven key tools for inclusion in the curriculum were discussed with experts and then mapped to current curriculum offered at institutions. We discovered that less than 5% of these were being taught. In total, 78% of experts agreed that digital content is inevitable, 90% agreed that digital inclusion based on tools that were discussed will yield great benefits for students, and lastly 75% agreed that giving digital exposure to students must be standard practice. Conclusions: The response from experts confirms that digital inclusion is imperative, but instructors themselves lacked the know-how of emerging technologies. Only the curriculum of institutions with approved bachelor’s programs were included in this research. In our future work we hope to include all institutions and professional bodies as well.

Introduction Despite carrying a disproportionately high burden of depression, patients in low-income countries lack access to effective care. The collaborative care model (CoCM) has robust evidence for clinical effectiveness in improving mental health outcomes. However, evidence from real-world implementation of CoCM is necessary to inform its expansion in low-resource settings.Methods We conducted a 2-year mixed-methods study to assess the implementation and clinical impact of CoCM using the WHO Mental Health Gap Action Programme protocols in a primary care clinic in rural Nepal. We used the Capability Opportunity Motivation-Behaviour (COM-B) implementation research framework to adapt and study the intervention. To assess implementation factors, we qualitatively studied the impact on providers’ behaviour to screen, diagnose and treat mental illness. To assess clinical impact, we followed a cohort of 201 patients with moderate to severe depression and determined the proportion of patients who had a substantial clinical response (defined as ≥50% decrease from baseline scores of Patient Health Questionnaire (PHQ) to measure depression) by the end of the study period.Results Providers experienced improved capability (enhanced self-efficacy and knowledge), greater opportunity (via access to counsellors, psychiatrist, medications and diagnostic tests) and increased motivation (developing positive attitudes towards people with mental illness and seeing patients improve) to provide mental healthcare. Abstract We investigated the cardiovascular effects of venoms from seven medically important species of snakes: Australian Eastern Brown snake (Pseudonaja textilis), Sri Lankan Russell’s viper (Daboia russelii), Javanese Russell’s viper (D. siamensis), Gaboon viper (Bitis gabonica), Uracoan rattlesnake (Crotalus vegrandis), Carpet viper (Echis ocellatus) and Puff adder (Bitis arietans), and identified two distinct patterns of effects: i.e. rapid cardiovascular collapse and prolonged hypotension. P. textilis (5 µg/kg, i.v.) and E. ocellatus (50 µg/kg, i.v.) venoms induced rapid (i.e. within 2 min) cardiovascular collapse in anaesthetised rats. P. textilis (20 mg/kg, i.m.) caused collapse within 10 min. D. russelii (100 µg/kg, i.v.) and D. siamensis (100 µg/kg, i.v.) venoms caused ‘prolonged hypotension’, characterised by a persistent decrease in blood pressure with recovery. D. russelii venom (50 mg/kg and 100 mg/kg, i.m.) also caused prolonged hypotension. A priming dose of P. textilis venom (2 µg/kg, i.v.) prevented collapse by E. ocellatus venom (50 µg/kg, i.v.), but had no significant effect on subsequent addition of D. russelii venom (1 mg/kg, i.v). Two priming doses (1 µg/kg, i.v.) of E. ocellatus venom prevented collapse by E. ocellatus venom (50 µg/kg, i.v.). B. gabonica, C. vegrandis and B. arietans (all at 200 µg/kg, i.v.) induced mild transient hypotension. Artificial respiration prevented D. russelii venom induced prolonged hypotension but not rapid cardiovascular collapse from E. ocellatus venom. D. russelii venom (0.001–1 μg/ml) caused concentration-dependent relaxation (EC50 = 82.2 ± 15.3 ng/ml, Rmax = 91 ± 1%) in pre-contracted mesenteric arteries. In contrast, E. ocellatus venom (1 µg/ml) only produced a maximum relaxant effect of 27 ± 14%, suggesting that rapid cardiovascular collapse is unlikely to be due to peripheral vasodilation. The prevention of rapid cardiovascular collapse, by ‘priming’ doses of venom, supports a role for depletable endogenous mediators in this phenomenon.

Research on retro and nostalgic consumption has gathered much interest in the recent past, but little is known about the role of nostalgia in buffering psychological threats such as loneliness, particularly in the context of brand consumption. In this study, we adopt the perspective of emotional loneliness with which we examine the role of childhood brand nostalgia in building brand loyalty. We posit that the negative emotion of loneliness triggers individuals to engage in nostalgia, particularly towards brands that relate to their childhood. This phenomenon is operationalized in this study by the construct of childhood brand nostalgia. We draw on consumer brand relationship theory and develop a sequential mediation model to explain the mediating processes of how emotional loneliness, nostalgia and perceived self-continuity create childhood brand nostalgia and ultimately contribute to brand loyalty. The results from a sample of 211 respondents suggest that when individuals are emotionally lonely, they are likely to engage in nostalgia to enhance their need for perceived self-continuity. Subsequently, the need for perceived self-continuity is fulfilled by the consumption of childhood brand nostalgia as well as by developing a long-term relationship with the brands that are associated with their childhood. Most importantly, our findings imply that emotionally lonely individuals use childhood brand nostalgia to retrieve their fond childhood memories to enhance their perceived sense of connectedness, which in turn may be a means to manage emotional loneliness.

Dengue is a major public health challenge worldwide, particularly in tropical areas. Nearly 390 million infections and 22,000 deaths occur every year. At present, there are no specific therapeutics available to treat dengue; however, possible treatment procedures are explained in the traditional medical systems (TMSs), such as Sri Lankan TMS, Indian Ayurvedic, Unani, and Siddha TMS. In these TMSs, medicinal plants have been used in several ways against dengue, such as virocides, larvicides, and mosquito repellents. Therefore, medicinal plants inherit biologically active compounds/lead compounds that are yet to be identified chemically and physiologically. Herein, we discuss the possible applications of crude plant extracts and isolated phytochemicals from medicinal plants such as quercetin, sulfated galactomannans, flavonoids, and glabranine in controlling dengue. Moreover, medicinal plant-based therapeutics can be safer, cost-effective, and non-toxic. Therefore, this paper reviews the medicinal plants that are used in TMSs to manage dengue, the phytochemicals they contain, and mode of action of these phytochemicals such as virocides, larvicides, and mosquito repellents.

Abstract Campylobacter is the most common cause of traveler’s diarrhea (TD) and human bacterial gastroenteritis. A heteroresistant Campylobacter jejuni (C. jejuni) isolate, identified by microbiological methods and characterized with molecular techniques, was obtained from a traveler in Nepal suffering TD. The presence of atypical colonies within the clear zone of inhibition was the first evidence of an atypical phenotype, leading to additional characterization of this heteroresistant strain. Antimicrobial susceptibility testing (AST) and population analysis profiling (PAP) demonstrated heteroresistance to azithromycin (AZM), a first-line antibiotic treatment for Campylobacter infections. Molecular analysis indicated a point mutation occurred on the 23S rRNA gene at the A2075G transitions, and the number of mutated gene copies was proportional to AZM resistance. Heteroresistant C. jejuni subpopulations from acute TD are likely underestimated, which may lead to treatment failures, as was the case for this patient. The presence of a heteroresistant strain in a high antibiotic environment may select for additional drug resistance and enable distribution into hospital and local communities.

Abstract Background In 2017, Australia experienced its highest levels of influenza virus activity since the 2009 pandemic. This allowed detailed comparison of the characteristics of patients with community and hospital-acquired influenza, and infection control factors that contributed to influenza spread. Methods A surveillance based study was conducted on hospitalised patients with laboratory-confirmed influenza at the Canberra Hospital during April–October 2017. Differences between the hospital-acquired and community-acquired patient characteristics and outcomes were assessed by univariate analysis. Epidemiologic curves were developed and cluster distribution within the hospital was determined. Results Two hundred and ninety-two patients were included in the study. Twenty-eight (9.6%) acquired influenza in hospital, representing a higher proportion than any of the previous 5 years (range 0.9–5.8%). These patients were more likely to have influenza A (p = 0.021), had higher rates of diabetes (p = 0.015), malignancy (p = 0.046) and chronic liver disease (p = 0.043). Abstract Objective We report a review of outcomes in schizophrenia in the twenty‐first century, replicating and extending work undertaken by the late Richard Warner in his seminal book, “Recovery from Schizophrenia: Psychiatry and Political Economy” (1985;2004). Method Warner's methods were followed as closely as possible. Only observational/naturalistic studies were included. Six scientific databases were searched from 2000 to 2020. 6,640 records were retrieved. 47 met inclusion criteria. Results Overall, complete recovery is higher in this study than in Warner's (37.75% cf 20.4%), especially for first episode psychosis (FEP) (57.1% cf 20.7%). Clinical recovery, annualized remission rate (ARR), and employment outcomes were significantly superior for first episode psychosis compared with multiple episode psychosis (MEP). ARR shows a trend toward reduction over time, from 2.2 before the financial crash of 2008 to 1.6 after (t = 1.85 df 40 p = .07). The decline is statistically significant for the MEP group (t = 2.32 df18 p = .03). There were no differences in outcome by region, sample characteristics, outcome measures used, or quality of studies. Heterogeneity of clinical outcome measures across the literature makes evidence synthesis difficult. Weak and inconsistent reporting of functional and employment outcomes mean that findings lack meaning with respect to lived experience. Conclusion Future research strategies should aim to reduce heterogeneity in clinical outcome measures and to increase the emphasis on capture and reporting of more sophisticated measures of social and functional outcome. Outcome domains should be disaggregated rather than conflated into unitary recovery constructs.

In the traditional irrigation process, a huge amount of water consumption is required which leads to water wastage. To reduce the wasting of water for this tedious task, an intelligent irrigation system is urgently needed. The era of machine learning (ML) and the Internet of Things (IoT) brings it is a great advantage of building an intelligent system that performs this task automatically with minimal human effort. In this study, an IoT enabled ML-trained recommendation system is proposed for efficient water usage with the nominal intervention of farmers. IoT devices are deployed in the crop field to precisely collect the ground and environmental details. The gathered data are forwarded and stored in a cloud-based server, which applies ML approaches to analyze data and suggest irrigation to the farmer. To make the system robust and adaptive, an inbuilt feedback mechanism is added to this recommendation system. The experimentation, reveals that the proposed system performs quite well on our own collected dataset and National Institute of Technology (NIT) Raipur crop dataset.

Introduction and objectives: Higher rates of psychiatric disorders are reported among cirrhotic patients. This study examines the demographic and clinical outcomes post-liver transplant (LT) among cirrhotic patients with a major psychiatric diagnosis (cases) compared to those without psychiatric diagnosis (controls). Materials and methods: Retrospective case control design was used among 189 cirrhotic patients who had undergone LT at Methodist University Hospital Transplant Institute, Memphis, TN between January 2006 and December 2014. Multivariable regression and Cox proportional hazard regression were conducted to compare allograft loss and all-cause mortality. Results: The study sample consisted of a matched cohort of 95 cases and 94 controls with LT. Females and those with Hepatic Encephalopathy (HE) were more likely to have psychiatric diagnosis. Patients with hepatocellular carcinoma (HCC) were twice as likely to have allograft loss. Psychiatric patients with HCC had two and a half times (HR 2.54; 95% CI: 1.20–5.37; p = 0.015) likelihood of all-cause mortality. Data censored at 1-year post-LT revealed that patients with psychiatric diagnosis have a three to four times higher hazard for allograft loss and all-cause mortality compared to controls after adjusting for covariates, whereas when the data is censored at 5 year, allograft loss and all-cause mortality have two times higher hazard ratio. Conclusions: The Cox proportional hazard regression analysis of censored data at 1 and 5 year indicate higher allograft loss and all-cause mortality among LT patients with psychiatric diagnosis. Patients with well-controlled psychiatric disorders who undergo LT need close monitoring and medication adherence.

Objective: This study describes the types of Human astroviruses detected in stool samples collected from a birth cohort of children in Nepal.Methods: Using a commercial kit (ProSpecT), a total of 5,224 diarrheal and non-diarrheal stool samples were screened for Human astrovirus by ELISA. RT-PCR was performed on ELISA positive samples (2.8%) for further confirmation. The primary RT-PCR assay used targets the ORF2 region and detects human astrovirus type 1–8. Samples that were negative in this assay were further analyzed using primers that target the ORF1b region of human astrovirus which detect both classical type (HAstV 1–8) and novel types (MLB1–5, VA 1–5). PCR positive samples were analyzed by Sanger sequencing to determine the genotype.Results: A total of 148 available ELISA positive stool samples were analyzed by RT-PCR and further genotyped. RT-PCR analysis of these samples using the ORF2 and ORF1b assay revealed that 124 (84%) were positive for classical human types (HAstV 1–8). Seven different classical HAstV genotypes based on ORF2 and ORF1a were identified (HAstV 1- HAstV 8) with the greatest prevalence of HAstV 5 genotype (42.2%), followed by HAstV 1 (34.7%), HAstV 2 and HAstV 8 (7.4%), HAstV 4 (4.1%), HAstV 3 (3.3%), and HAstV 6 (0.8%). Non-classical types were not detected in our study.Conclusion: A high diversity of circulating Astrovirus strains were detected in young children, both with and without symptoms of gastroenteritis. HAstV 5 and HAstV 1 were the most common genotypes in young children in Nepal.

Mutations/deficiency of TDRD7, encoding a tudor domain protein involved in post-transcriptional gene expression control, causes early onset cataract in humans. While Tdrd7 is implicated in the control of key lens mRNAs, the impact of Tdrd7 deficiency on microRNAs (miRNAs) and how this contributes to transcriptome misexpression and to cataracts, is undefined. We address this critical knowledge-gap by investigating Tdrd7-targeted knockout (Tdrd7-/-) mice that exhibit fully penetrant juvenile cataracts. We performed Affymetrix miRNA 3.0 microarray analysis on Tdrd7-/- mouse lenses at postnatal day (P) 4, a stage preceding cataract formation. This analysis identifies 22 miRNAs [14 over-expressed (miR-15a, miR-19a, miR-138, miR-328, miR-339, miR-345, miR-378b, miR-384, miR-467a, miR-1224, miR-1935, miR-1946a, miR-3102, miR-3107), 8 reduced (let-7b, miR-34c, miR-298, miR-382, miR-409, miR-1198, miR-1947, miR-3092)] to be significantly misexpressed (fold-change ≥ ± 1.2, p-value < 0.05) in Tdrd7-/- lenses. To understand how these misexpressed miRNAs impact Tdrd7-/- cataract, we predicted their mRNA targets and examined their misexpression upon Tdrd7-deficiency by performing comparative transcriptomics analysis on P4 and P30 Tdrd7-/- lens. To prioritize these target mRNAs, we used various stringency filters (e.g., fold-change in Tdrd7-/- lens, iSyTE-based lens-enriched expression) and identified 98 reduced and 89 elevated mRNA targets for overexpressed and reduced miRNAs, respectively, which were classified as “top-priority” “high-priority,” and “promising” candidates. For Tdrd7-/- lens overexpressed miRNAs, this approach identified 18 top-priority reduced target mRNAs: Alad, Ankrd46, Ceacam10, Dgat2, Ednrb, H2-Eb1, Klhl22, Lin7a, Loxl1, Lpin1, Npc1, Olfm1, Ppm1e, Ppp1r1a, Rgs8, Shisa4, Snx22 and Wnk2. Majority of these targets were also altered in other gene-specific perturbation mouse models (e.g., Brg1, E2f1/E2f2/E2f3, Foxe3, Hsf4, Klf4, Mafg/Mafk, Notch) of lens defects/cataract, suggesting their importance to lens biology. Gene ontology (GO) provided further insight into their relevance to lens pathology. For example, the Tdrd7-deficient lens capsule defect may be explained by reduced mRNA targets (e.g., Col4a3, Loxl1, Timp2, Timp3) associated with “basement membrane”. GO analysis also identified new genes (e.g., Casz1, Rasgrp1) recently linked to lens biology/pathology. Together, these analyses define a new Tdrd7-downstream miRNA-mRNA network, in turn, uncovering several new mRNA targets and their associated pathways relevant to lens biology and offering molecular insights into the pathology of congenital cataract.

Objective To describe the pattern of hydroxychloroquine use and examine the association between hydroxychloroquine use and clinical outcomes arising from changes in the US Food and Drug Administration (FDA)’s recommendation during the coronavirus disease 2019 (COVID-19) pandemic.Design A retrospective cross-sectional analysis.Setting and participants We included hospitalised adult patients at Northwell Health hospitals with confirmed COVID-19 infections between 1 March 2020 and 11 May 2020. Background Mitochondrial DNA (MT-DNA) are intrinsically inflammatory nucleic acids released by damaged solid organs. Whether circulating cell-free MT-DNA quantitation could be used to predict the risk of poor COVID-19 outcomes remains undetermined.Methods We measured circulating MT-DNA levels in prospectively collected, cell-free plasma samples from 97 subjects with COVID-19 at hospital presentation. Our primary outcome was mortality. Intensive care unit (ICU) admission, intubation, vasopressor, and renal replacement therapy requirements were secondary outcomes. Multivariate regression analysis determined whether MT-DNA levels were independent of other reported COVID-19 risk factors. Receiver operating characteristic and area under the curve assessments were used to compare MT-DNA levels with established and emerging inflammatory markers of COVID-19.Results Circulating MT-DNA levels were highly elevated in patients who eventually died or required ICU admission, intubation, vasopressor use, or renal replacement therapy. Multivariate regression revealed that high circulating MT-DNA was an independent risk factor for these outcomes after adjusting for age, sex, and comorbidities. We present a spectroscopic study of an interacting emission-line dwarf galaxy SDSS J134326.99+431118.7. We analyzed eight-strong emission lines of wavelength in a range of 3902.1Å to 6619.1Å. Among them, the strongest emission line is OIII, with an intensity of 1043.6 x 10-17 erg/s/cm2/Å. These characteristic lines show a perfect Gaussian fit with a coefficient of regression greater than 98%, where the derived full width half maximum (FWHM) is less than 3.8 Å. The line ratio between Ha and Hb, (Ha/ Hb), is 2.73. This suggests that the galaxy is a starburst galaxy. Star Formation Rate (SFR) of the galaxy derived from Ha emission line flux is 0.019 and emission line metallicity derived from flux ratio between NII and Ha is 7.85 dex. These morphological and physical properties of SDSSJ134326.99+431118.7 are very similar to those of a typical Blue Compact Dwarf (BCD) galaxy.
