Las evaluaciones externas se han convertido en políticas esenciales para los gestores educativos, que buscan una Educación de calidad, basada en la autonomía escolar, la rendición de cuentas y la gestión basada en resultados. El objetivo de este estudio es analizar el contexto en el que se enmarcan las evaluaciones externas en España y las consecuencias de estas políticas con respecto a la equidad. Para ello se realiza en 2020 un Estudio de Caso a través de dos estrategias de análisis: entrevistas semiestructuradas y análisis documental de los proyectos educativos; así como la revisión de la normativa vigente en el transcurso de la investigación. Los resultados señalan a las evaluaciones externas como un instrumento de control sobre el equipo directivo y el profesorado. En las conclusiones se señala que convirtiéndose estos en la prioridad de los sistemas educativos y agravando las desigualdades.

Considering both the health and energy areas where anthocyanins have been revealing their real and potential applications, research on their extraction optimization methods is a priority. Conditions to achieve the highest mass of crude extract from Tradescantia spathacea leaves with the highest absorbance (A) in the region of 500 - 540 nm linked with anthocyanins, were investigated through a Box-Behnken design and response surface methodology. Independent variables involved in the Box-Behnken design were solvent (ethanol) concentration (30, 50, 70 % v/v), solvent/fresh leaf ratio (1.67:1, 2.34:1; 3:1 v/w) and extraction temperature (30, 50, 70 °C). Dependent (response) variables were crude extract amount and the extract with the maximum A in the region of 500-540 nm. Experimental values of A 500 - 540 nm were 0.3562 and 0.7558; crude extract mass ranged between 0.285 g and 0.7157 g, corresponding to a yield between 144 to 347 mg dried extract per g of dried leaf. Using a solvent/fresh leaf ratio of 3:1 v/w, 70 °C, 65 % ethanol, the highest values of both crude extract mass and A 500 - 540 nm were obtained.

Abstract Background Hypertension is responsible for a huge burden of disease. The aim of this study was to evaluate the impact of newly diagnosed hypertension on the occurrence of kidney or cardiovascular events (K/CVEs) and on mortality among community dwellers. Methods Retrospective cohort study, conducted from January, 2007, to December, 2018. All patients (age > 18) newly diagnosed with hypertension and no previous K/CVEs in 2007 and 2008, in the primary care centers of Madrid (Spain) (n = 71,770), were enrolled. The control group (n = 72,946) included patients without hypertension, matched by center, sex and age. The occurrence of kidney or CV events, including mortality from these causes and total mortality were evaluated using Cox regression and multistate models. Data were collected from three sources: personal data from administrative records, clinical data from medical records, and mortality data from regional and national databases. Results The median follow-up was 138.61 months (IQR: 124.68–143.97 months). There were 32,896 K/CVEs (including 3,669 deaths from these causes) and 12,999 deaths from other causes. Adjusted for sex, smoking, diabetes and socioeconomic status, K/CVEs HR was 4.36 (95% CI: 3.80–5.00) for diagnoses before 45 years of age, 2.45(95% CI: 2.28- 2.63) for diagnosis between 45 to 54 years, and HR decreased to 1.86 (95% CI: 1.64–210) for diagnoses over age 85. Total mortality risk was only higher for hypertension diagnosed before 55 years of age (HR: 2.47, 95% CI: 1.90–3.19 for ages 18 to 44; and HR: 1.14, 95% CI: 1.02–1.28 for ages 45 to 54). Conclusion The diagnosis of hypertension in the community environment, in patients without evidence of previous kidney or CV disease, is associated with a large increase in the risk of K/CVEs, but especially in individuals diagnosed before the age of 55. This diagnosis is only associated with an increase in kidney or cardiovascular mortality or overall mortality when it occurs before age 55.

Abstract Sequencing of B-cell and T-cell immune receptor repertoires helps us to understand the adaptive immune response, although it only provides information about the clonotypes (lineages) and their frequencies and not about, for example, their affinity or antigen (Ag) specificity. To further characterize the identified clones, usually with special attention to the particularly abundant ones (dominant), additional time-consuming or expensive experiments are generally required. Here, we present an extension of a multiscale model of the germinal center (GC) that we previously developed to gain more insight in B-cell repertoires. We compare the extent that these simulated repertoires deviate from experimental repertoires established from single GCs, blood, or tissue. Our simulations show that there is a limited correlation between clonal abundance and affinity and that there is large affinity variability among same-ancestor (same-clone) subclones. Our simulations suggest that low-abundance clones and subclones, might also be of interest since they may have high affinity for the Ag. We show that the fraction of plasma cells (PCs) with high B-cell receptor (BcR) mRNA content in the GC does not significantly affect the number of dominant clones derived from single GCs by sequencing BcR mRNAs. Results from these simulations guide data interpretation and the design of follow-up experiments.

Los niños presentan mayor morbimortalidad que los adultos en lista de espera a trasplante de órganos, teniendo además una gran dificultad para la obtención de donantes. La donación en asistolia controlada pediátrica (DACp) podría mejorar las opciones de trasplante de muchos pacientes. No obstante, dado que precisa modificar algunos aspectos de la asistencia médica antes de la muerte del donante, expertos bioeticistas y profesionales sanitarios han expresado reservas y dudas deontológicas no suficientemente debatidas. Este trabajo expone las cuestiones bioéticas con mayor relevancia en la práctica clínica y que deberían ser consideradas a fin de respetar los principios éticos de la donación.

This study enlists a transformative approach to mixed methods research in order to problematize what different methods can offer for expanding understandings of educator language attitudes and ideologies. Analyses consider Likert-scale survey responses and linguistic autobiography data from a sociolinguistics course in an online Educating Multilingual Learners master's degree program at a Southwestern U.S. university. A regression analysis of survey data suggests that older participants are predicted to orient more positively to linguistic diversity and participants with more teaching experience are predicted to orient less positively. Meanwhile, a discursive analysis of autobiographical essays from the oldest participants (over 45) and those with the most teaching experience (over 10 years) illuminate reflexively constructed stances, which we draw on to complicate (not necessarily triangulate) statistically significant findings. Discussion considers the methods by which we examine educators' language ideologies with implications for both research and teacher education.

This work aimed to elucidate how O3 pollution causes a loss of regulation in the immune response in both the brain and the intestine. In this work, we studied the effect of exposing rats to low doses of O3 based on the association between the antioxidant response of superoxide dismutase (SOD) levels and the nuclear factor kappa light chains of activated B cells (NFκB) as markers of inflammation. Method: Seventy-two Wistar rats were used, divided into six groups that received the following treatments: Control and 7, 15, 30, 60, and 90 days of O3. After treatment, tissues were extracted and processed using Western blotting, biochemical, and immunohistochemical techniques. The results indicated an increase in 4-hydroxynonenal (4HNE) and Cu/Zn-SOD and a decrease in Mn-SOD, and SOD activity in the substantia nigra, jejunum, and colon decreased. Furthermore, the translocation of NFκB to the nucleus increased in the different organs studied. In conclusion, repeated exposure to O3 alters the regulation of the antioxidant and inflammatory response in the substantia nigra and the intestine. This indicates that these factors are critical in the loss of regulation in the inflammatory response; they respond to ozone pollution, which can occur in chronic degenerative diseases.

Genome-wide association studies (GWAS) have mapped over 90% of disease- and quantitative-trait-associated variants within the non-coding genome. Non-coding regulatory DNA (e.g., promoters and enhancers) and RNA (e.g., 5′ and 3′ UTRs and splice sites) are essential in regulating temporal and tissue-specific gene expressions. Non-coding variants can potentially impact the phenotype of an organism by altering the molecular recognition of the cis-regulatory elements, leading to gene dysregulation. However, determining causality between non-coding variants, gene regulation, and human disease has remained challenging. Experimental and computational methods have been developed to understand the molecular mechanism involved in non-coding variant interference at the transcriptional and post-transcriptional levels. This review discusses recent approaches to evaluating disease-associated single-nucleotide variants (SNVs) and determines their impact on transcription factor (TF) binding, gene expression, chromatin conformation, post-transcriptional regulation, and translation.

Klebsiella pneumoniae (Kpn) is an opportunistic pathogen that causes intrahospital complications such as pneumonia, liver abscesses, soft tissue infections, urinary infections, bacteraemia, and, in some cases, death. Since this bacterium has a higher frequency than other Gram-negative pathogens, it has become an important pathogen to the health sector. The adaptative genome of Kpn likely facilitates increased survival of the pathogen in diverse situations. Therefore, several studies have been focused on developing new molecules, synergistic formulations, and biomaterials that make it possible to combat and control infections with and dispersion of this pathogen. Note that the uncontrolled antibiotic administration that occurred during the pandemic led to the emergence of new multidrug-resistant strains, and scientists were challenged to overcome them. This review aims to compile the latest information on Kpn that generates intrahospital infections, specifically their pathogenicity-associated factors. Furthermore, it explains the natural-product-based treatments (extracts and essential oils) developed for Kpn infection and dispersion control.

This review aims to obtain the relationships between characteristics of the facilities and the quality of life of older adult residents, and to obtain the direction in which new studies should be addressed as well as their application to healthcare policies. A systematic review was conducted following PRISMA guidelines. Multiple databases have been used: Scopus, Web of Science, Wiley online library, PubMed, ProQuest, EBSCOhost, and Emerald, from January 1, 2011 to July 1, 2023. Ten studies from 1,037 articles were included. Evidence exists on the positive relationships between quality of life and hours of management, care, activities, and qualified staff, and in general, the characteristics included in the categories of space management, supporting facilities and building services. In contrast charitable and for-profit facilities, Medicaid status, resident acuity, deficiencies, chain affiliation, size, hours of practical nurses, turnover, and financial resources (except occupancy rate) have a negative impact on quality of life or their dimensions.

Summary: Background: COVID-19 in paediatric ages could result in hospitalizations and death. In addition, excluding children from vaccination could turn them into reservoirs of the SARS-COV-2. Safe and effective COVID-19 vaccines are urgently needed for large-scale paediatric vaccination. ISMAELILLO study aimed to evaluate safety and immunogenicity of two strengths of a new recombinant receptor-binding domain (RBD) protein vaccine (Abdala) in paediatric population. Methods: A double-blinded, multicentre, randomised, phase 1/2 clinical trial was conducted in nine polyclinics in the province of Camagüey, Cuba. Healthy children and adolescents were stratified according to age (3–11 years old, or 12–18 years old) and they were randomly assigned (1:1; block size four) in two dosage level groups of vaccine to receive three intramuscular doses of 25 μg or 50 μg of RBD, 14 days apart. Main safety endpoint was analyzed as the percentage of serious adverse reactions during vaccination up to 28 days after the third dose (Day 56) in participants who received at least one dose vaccination. The primary immunogenicity endpoint assessed was seroconversion rate of anti-RBD IgG antibody at day 56. The immunogenicity outcomes were assessed in the per-protocol population. This trial is registered with Cuban Public Registry of Clinical Trials, RPCEC00000381. Findings: Between July 15, 2021, and August 16, 2021, 644 paediatric subjects were screened, of whom 592 were enrolled after verifying that they met the selection criteria: firstly 88 were included in Phase 1 of the study and 504 who completed Phase 2. The vaccine was well tolerated. Injection site pain was the most frequently reported local event (143 [8·4%] of 1707 total doses applied), taking place in 66/851 (7·8%) in the 25 μg group and in 77/856 (9·0%) in the 50 μg. The most common systemic adverse event (AE) was headache: 23/851 (2·7%) in the 25 μg group and 19/856 (2·2%) in the 50 μg. Reactogenicity was mild or moderate in severity, represented in 75% of cases by local symptoms, completely resolved in the first 24–48 h. Twenty-eight days after the third dose, seroconversion anti-RBD IgG were observed in 98·2% of the children and adolescents (231/234) for the 50 μg group and 98·7% (224/228) for the 25 μg group without differences between both strength. The specific IgG antibody geometric mean titres (GMT) showed higher titres between participants who received Abdala 50 μg (231·3; 95% CI 222·6–240·4) compared to those who received 25 μg (126·7; 95% CI 121·9–131·7). Massive arrivals of pelagic Sargassum to the coasts of several countries in the Atlantic Ocean began in 2011. Monitoring the abundance and distribution of Sargassum in the ocean and along the coasts is necessary to understand the phenomena better and develop forecasting products and management protocols. Most Sargassum monitoring has been conducted in the open ocean through traditional remote sensing techniques. However, since the most significant ecologic and socioeconomic impacts occur on the coasts, it is necessary to monitor these macroalgae on nearshore waters and beaches. This manuscript reviews the remote sensing algorithms used in Sargassum observation reported in the last 17 years in more than sixty high-impact scientific publications. The discussion regarding the evolution of the methodologies used for monitoring these macroalgae allowed us to conclude that the synergy generated by incorporating new disciplines like artificial intelligence and citizen science has positively impacted the development of this field. Additionally, the current state-of-the-art methods, the fundamental challenges, and the directions for future research are also discussed.

Marine sediments are one of the largest carbon reservoir on Earth, yet the microbial communities, especially the eukaryotes, that drive these ecosystems are poorly characterised. Here, we report implementation of a sampling system that enables injection of reagents into sediments at depth, allowing for preservation of RNA in situ. Using the RNA templates recovered, we investigate the ribosomally active eukaryotic diversity present in sediments close to the water/sediment interface. We demonstrate that in situ preservation leads to recovery of a significantly altered community profile. Using SSU rRNA amplicon sequencing, we investigated the community structure in these environments, demonstrating a wide diversity and high relative abundance of stramenopiles and alveolates, specifically: Bacillariophyta (diatoms), labyrinthulomycetes and ciliates. The identification of abundant diatom rRNA molecules is consistent with microscopy-based studies, but demonstrates that these algae can also be exported to the sediment as active cells as opposed to dead forms. We also observe many groups that include, or branch close to, osmotrophic-saprotrophic protists (e.g. labyrinthulomycetes and Pseudofungi), microbes likely to be important for detrital decomposition. The sequence data also included a diversity of abundant amplicon-types that branch close to the Fonticula slime moulds. Taken together, our data identifies additional roles for eukaryotic microbes in the marine carbon cycle; where putative osmotrophic-saprotrophic protists represent a significant active microbial-constituent of the upper sediment layer.

An acidic extracellular pH value (pHe) is characteristic of many cancers, in contrast to the physiologic pHe found in most benign cells. This difference in pH offers a unique opportunity to design and engineer chemicals that can be employed for pH-selective reactions in the extracellular fluid of cancer cells. The viability of human skin melanoma and corresponding fibroblasts exposed to CaS dispersions is reported. The viability of melanoma cells decreases with CaS dispersion concentration and reaches 57% at 3%, a value easily distinguishable from melanoma control experiments. In contrast, the viability of benign fibroblasts remains nearly constant within experimental error over the range of dispersion concentrations studied. The CaS dispersions facilitate vinculin delocalization in the cytoplasmic fluid, a result consistent with improved focal adhesion kinase (FAK) regulation in melanoma cells. Thermodynamic considerations are consistent with the formation of H2S from CaS in the presence of protons. The thermodynamic prediction is verified in independent experiments with solid CaS and acidic aqueous solutions. The amount of H2S formed decreases with pH. An activation energy for the process of (30 ± 10) kJ/mol in the temperature range of 280 to 330 K is estimated from initial rate measurements as a function of temperature. The total Gibbs energy minimization approach was employed to establish the distribution of sulfides—including H2S in the gas and aqueous phases—from the dissociation of CaS as a function of pH to mimic physiologically relevant pH values. Theoretical calculations suggest that partially protonated CaS in solution can be stable until the sulfur atom bonds to two hydrogen atoms, resulting in the formation of Ca2+ and H2S, which can be solvated and/or released to the gas phase. Our results are consistent with a model in which CaS is dissociated in the extracellular fluid of melanoma cells selectively. The results are discussed in the context of the potential biomedical applications of CaS dispersions in cancer therapies.

Abstract Background Colorectal cancer (CRC) is a highly prevalent disease in developed countries. Inherited Mendelian causes account for approximately 5% of CRC cases, with Lynch syndrome and familial adenomatous polyposis being the most prevalent forms. Scientific efforts are focused on the discovery of new candidate genes associated with CRC and new associations of phenotypes with well-established cancer-related genes. BRCA1-associated ring domain (BARD1) gene deleterious germline variants are associated with a moderate increase in the relative risk of breast cancer, but their association with other neoplasms, such as CRC, remains unclear. Case presentation We present the case of a 49-year-old male diagnosed with rectal adenocarcinoma whose maternal family fulfilled Amsterdam clinical criteria for Lynch syndrome. Genetic test confirmed the presence in heterozygosis of a germline pathogenic deletion of exons 8–11 in BARD1 gene. The predictive genetic study of the family revealed the presence of this pathogenic variant in his deceased cancer affected relatives, confirming co-segregation of the deletion with the disease. Conclusions To the best of our knowledge, this is the first published work in which this BARD1 deletion is detected in a family with familial colorectal cancer type X (FCCTX) syndrome, in which the clinical criteria for Lynch syndrome without alteration of the DNA mismatch repair (MMR) system are fulfilled. Whether this incidental germline finding is the cause of familial colorectal aggregation remains to be elucidated in scientific forums. Patients should be carefully assessed in specific cancer genetic counseling units to account for hypothetical casual findings in other genes, in principle unrelated to the initial clinical suspicion, but with potential impact on their health.

Background: high blood pressure is considered one of the three most relevant risk factors for cardiovascular and cerebrovascular diseases. Healthy lifestyles prevent its damage.Objective: to characterize the hypertensive patients’ lifestyles in the doctor´s office 7 from area IV of the Cienfuegos’ municipality. Methods: a descriptive, cross-sectional study that included 125 average adult hypertensive patients (30-59 years old), 65 male and 60 female, who met the inclusion criteria. The variables age, skin color, marital status, schooling, personal and family pathological history, eating habits and toxic habits were analyzed.Results: hypertensive, male, non-white skin color and obese patients aged 50 to 59 years predominated; In terms of marital status, the stable union was more frequent and there were no differences in terms of education level. Hypercholesterolemia and arterial hypertension in first-degree relatives, mainly mothers and grandparents, were the most prominent factors among the health history. Inadequate eating habits and toxic habits were the most relevant in the patients under study.Conclusions: the lifestyles of the hypertensive patients who participated in the study are unhealthy and inadequate; there were no differences in terms of sex.
