Abstract

Competitive effects of nuclear deformation and density dependence of $\Lambda\!N$-interaction in $\Lambda$ binding energies $B_\Lambda$ of hypernuclei are studied systematically on the basis of the baryon-baryon interaction model ESC including many-body effects. By using the $\Lambda\!N$ G-matrix interaction derived from ESC, we perform microscopic calculations of $B_\Lambda$ in $\Lambda$ hypernuclei within the framework of the antisymmetrized molecular dynamics under the averaged-density approximation. The calculated values of $B_\Lambda$ reproduce experimental data within a few hundred keV in the wide mass regions from 9 to 51. It is found that competitive effects of nuclear deformation and density dependence of $\Lambda\!N$-interaction work decisively for fine tuning of $B_\Lambda$ values., Comment: 7 pages, 1 figure

Abstract

Abstract Objectives The precise pathogenesis of psoriasis remains incompletely explored. We aimed to better understand the underlying mechanisms of psoriasis, using a systems biology approach based on transcriptomics and microbiome profiling. Methods We collected the skin tissue biopsies and swabs in both lesional and non‐lesional skin of 13 patients with psoriasis, 15 patients with psoriatic arthritis and healthy skin from 12 patients with ankylosing spondylitis. To study the similarities and differences in the molecular profiles between these three conditions, and the associations between the host defence and microbiota composition, we performed high‐throughput RNA‐sequencing to quantify the gene expression profile in tissues. The metagenomic composition of 16S on local skin sites was quantified by clustering amplicon sequences and counted into operational taxonomic units. We further analysed associations between the transcriptome and microbiome profiling. Results We found that lesional and non‐lesional samples were remarkably different in terms of their transcriptome profiles. The functional annotation of differentially expressed genes showed a major enrichment in neutrophil activation. By using co‐expression gene networks, we identified a gene module that was associated with local psoriasis severity at the site of biopsy. From this module, we found a ‘core’ set of genes that was functionally involved in neutrophil activation, epidermal cell differentiation and response to bacteria. Skin microbiome analysis revealed that the abundances of Enhydrobacter, Micrococcus and Leptotrichia were significantly correlated with the genes in core network. Conclusions We identified a core gene network that associated with local disease severity and microbiome composition, involved in the inflammation and hyperkeratinization in psoriatic skin.

Abstract

Introduction Long-term survival after childhood cancer often comes at the expense of late, adverse health conditions. However, survivorship care is frequently not available for adult survivors in Europe. The PanCareFollowUp Consortium therefore developed the PanCareFollowUp Care Intervention, an innovative person-centred survivorship care model based on experiences in the Netherlands. This paper describes the protocol of the prospective cohort study (Care Study) to evaluate the feasibility and the health economic, clinical and patient-reported outcomes of implementing PanCareFollowUp Care as usual care in four European countries.Methods and analysis In this prospective, longitudinal cohort study with at least 6 months of follow-up, 800 childhood cancer survivors will receive the PanCareFollowUp Care Intervention across four study sites in Belgium, Czech Republic, Italy and Sweden, representing different healthcare systems. The PanCareFollowUp Care Intervention will be evaluated according to the Reach, Effectiveness, Adoption, Implementation and Maintenance framework. Clinical and research data are collected through questionnaires, a clinic visit for multiple medical assessments and a follow-up call. The primary outcome is empowerment, assessed with the Health Education Impact Questionnaire. A central data centre will perform quality checks, data cleaning and data validation, and provide support in data analysis. Multilevel models will be used for repeated outcome measures, with subgroup analysis, for example, by study site, attained age, sex or diagnosis.Ethics and dissemination This study will be conducted in accordance with the guidelines of Good Clinical Practice and the Declaration of Helsinki. The study protocol has been reviewed and approved by all relevant ethics committees. The evidence and insights gained by this study will be summarised in a Replication Manual, also including the tools required to implement the PanCareFollowUp Care Intervention in other countries. This Replication Manual will become freely available through PanCare and will be disseminated through policy and press releases.Trial registration number Netherlands Trial Register (NL8918; https://www.trialregister.nl/trial/8918).

Abstract

Abstract Background To guide the development of high‐quality care for people with multiple chronic conditions, partners of the European Joint Action CHRODIS developed the Integrated Multimorbidity Care Model. To assess its suitability for improving care for people with multimorbidity in the Netherlands, the model was piloted in a primary care setting with both patients and care providers. Aim This paper reports on the patient perspective, and aims to explore the priorities, underlying values and preferences for care of people with multimorbidity. Participants and methods Twenty persons with multimorbidity (selected from general practice registries) participated in a focus group or telephone interview. Subsequently, a questionnaire was completed by 863 persons with multimorbidity registered with 14 general practices. Qualitative data were thematically analysed and quantitative data by means of descriptive statistics. Results Frequently prioritized elements of care were the use of shared electronic health records, regular comprehensive assessments, self‐management support and shared decision making, and care coordination. Preferences for how these elements should be specifically addressed differed according to individual values (eg weighing safety against privacy) and needs (eg ways of coping with multimorbidity). Conclusion The JA‐CHRODIS Integrated Multimorbidity Care Model reflects the priorities and preferences for care of people with multimorbidity in the Netherlands, which supports its relevance to guide the development of person‐centred integrated care for people with multiple chronic conditions in the Netherlands. Patient contribution European patient experts contributed to the development and applicability assessment of the JA‐CHRODIS Integrated Multimorbidity Care Model; Dutch patients participated in focus groups, interviews and a survey.

Abstract

Abstract Background Globally 90 % of transmission of Hepatitis B virus (HBV) is from mother-to child and occurs predominantly in resource limited countries where the prevalence of HBV is high. Transmission could be interrupted by timely vaccinations but coverage remains problematic in these areas. Low knowledge or awareness of HBV may play a part in low vaccination coverage. This study examines the provision of antenatal care counselling with a focus on HBV in two different regions of northern Thailand, Sarapee Hospital (SH), Chiang Mai, and Shoklo Malaria Research Unit (SMRU), Tak Province. Methods A mixed-methods sequential explanatory study design was used to evaluate antenatal services for migrants. Cross-sectional knowledge, attitude and practice (KAP) surveys were conducted immediately after counselling at first ANC contact, at 3–6 months after first ANC contact and at delivery. Surveys provided quantitative data, and qualitative methods included observations, focus group discussions (FGD) and in-depth interviews (IDI); analysed thematically to explore concepts of knowledge and understanding, attitude and practice of pregnant women and providers. Results Between September-2019 and May-2020, 757 women participated to KAP surveys, and 31 observations of counselling, 16 FGD and 9 IDI were conducted. KAP surveys showed in spite of low knowledge about HBV transmission, infection, or vaccination (correct response: SH 5.7 %, 9/157; SMRU 34.0 %, 204/600), most women (≥ 93 %, either site) understood they were screened for HBV and were willing to vaccinate infants for HBV. In explaining KAP survey results, qualitative analysis suggests counselling should: use the appropriate language; be tailored to the local health literacy level, provide only pertinent information, be repeated over the antenatal period; and attempt to ensure patient privacy (where possible). Programme effectiveness benefits from positive attitudes to screening and vaccinations and a high level of trust in the providers nevertheless participants provided good suggestions for improvements of the service. Conclusions Limited knowledge of HBV among migrant women can be improved by counselling that emphasizes actionable knowledge such as vaccination schedule. Key improvements to the counselling process include training counsellors to conduct interactive counselling sessions in the woman’s language, using appropriate visual aids and timely repetition over the course of the antenatal period.

Abstract

This thesis is a conceptual and epistemological analysis of imagination and scientific thought experiments. The first part of this thesis deals with the definition and typology of imagination, and with the relation between imagination and related mental states such as supposition, visualisation, hallucination and memory. The second part of this thesis deals with the question whether we can use imagination to gain knowledge about the natural world - I argue that we can. The third and final part of this thesis deals with scientific thought experiments: I propose a novel account of scientific thought experiments based on a famous theory of fiction and a recently-developed philosophical account of scientific models.

169. The Multifaceted Roles of Coumarins in Iron Uptake

Abstract

In the context of sustainable agricultural practices, exploring plant-based strategies for enhancing crop resilience and productivity is crucial. This review delves into the roles of coumarins, a group of phenolic compounds, in mediating iron (Fe) uptake in plants, particularly under Fe-deficient conditions prevalent in alkaline soils. By modulating the chemical environment of the rhizosphere, coumarins play a pivotal role in Fe solubilization and acquisition through a reduction-based Strategy 1, which is predominantly utilized by non-grass species. This strategy involves the secretion of coumarins into the rhizosphere, facilitated by specific transport proteins, to chelate Fe from insoluble complexes, thereby enhancing its bioavailability for plant uptake. The biosynthesis of coumarins, such as scopoletin and fraxetin, is intricately regulated by environmental cues and genetic factors, with key enzymes like feruloyl CoA 6’hydroxylase 1 and scopoletin 8-hydroxylase catalyzing crucial steps in their synthesis. The involvement of the MYB72 transcription factor, regulated by pH and microbial interactions in the rhizosphere, highlights a sophisticated mechanism by which plants not only respond to Fe deficiency, but also manipulate the microbiota in the rhizosphere to optimize nutrient uptake and defend against pathogens. Moreover, coumarins exhibit antimicrobial properties, contributing to plant defense strategies by inducing systemic resistance and altering the microbial community structure towards a beneficial consortium. This dual functionality of coumarins – enhancing both nutrient uptake and pathogen resistance - presents a viable target for agricultural biotechnology, aimed at developing crops with increased resilience to environmental stresses and reduced dependency on chemical fertilizers and pesticides. Abstract

Background:This study aimed to assess the prognostic value of total tumor volume (TTV) for early recurrence (within 6 months) and overall survival (OS) in patients with colorectal liver metastases (CRLM), treated with induction systemic therapy followed by complete local treatment.Methods: Patients with initially unresectable CRLM from the multicenter randomized phase 3 CAIRO5 trial (NCT02162563) who received induction systemic therapy followed by local treatment were included. Baseline TTV and change in TTV as response to systemic therapy were calculated using the CT scan before and the first after systemic treatment, and were assessed for their added prognostic value. The findings were validated in an external cohort of patients treated at a tertiary center. Results:In total, 215 CAIRO5 patients were included. Baseline TTV and absolute change in TTV were significantly associated with early recurrence (P = 0.005 and P = 0.040, respectively) and OS in multivariable analyses (P = 0.024 and P = 0.006, respectively), whereas RECIST1.1 was not prognostic for early recurrence (P = 0.88) and OS (P = 0.35). In the validation cohort (n = 85), baseline TTV and absolute change in TTV remained prognostic for early recurrence (P = 0.041 and P = 0.021, respectively) and OS in multivariable analyses (P < 0.0001 and P = 0.012, respectively), and showed added prognostic value over conventional clinicopathological variables (increase C-statistic, 0.06; 95 % CI, 0.02 to 0.14; P = 0.008). Conclusion: Total tumor volume is strongly prognostic for early recurrence and OS in patients who underwent complete local treatment of initially unresectable CRLM, both in the CAIRO5 trial and the validation cohort. In contrast, RECIST1.1 did not show prognostic value for neither early recurrence nor OS.

Abstract

Background:Model validation procedures are crucial when population pharmacokinetic (PK) models are used to develop dosing algorithms and to perform model-informed precision dosing. We have previously published a population PK model describing the PK of gentamicin in term neonates with perinatal asphyxia during controlled therapeutic hypothermia (TH), which showed altered gentamicin clearance during the hypothermic phase dependent on gestational age and weight. In this study, the predictive performance and generalizability of this model were assessed using an independent data set of neonates with perinatal asphyxia undergoing controlled TH.Methods:The external data set contained a subset of neonates included in the prospective observational multicenter PharmaCool Study. Predictive performance was assessed by visually inspecting observed-versus-predicted concentration plots and calculating bias and precision. In addition, simulation-based diagnostics, model refitting, and bootstrap analyses were performed.Results:The external data set included 323 gentamicin concentrations of 39 neonates. Both the model-building and external data set included neonates from multiple centers. The original gentamicin PK model predicted the observed gentamicin concentrations with adequate accuracy and precision during all phases of controlled TH. Model appropriateness was confirmed with prediction-corrected visual predictive checks and normalized prediction distribution error analyses. Model refitting to the merged data set (n = 86 neonates with 935 samples) showed accurate estimation of PK parameters.Conclusions:The results of this external validation study justify the generalizability of the gentamicin dosing recommendations made in the original study for neonates with perinatal asphyxia undergoing controlled TH (5 mg/kg every 36 or 24 h with gestational age 36-41 and 42 wk, respectively) and its applicability in model-informed precision dosing.

Abstract

Aims: Little is known about the population pharmacokinetics (PPK) of vancomycin in neonates with perinatal asphyxia treated with therapeutic hypothermia (TH). We aimed to describe the PPK of vancomycin and propose an initial dosing regimen for the first 48 h of treatment with pharmacokinetic/pharmacodynamic target attainment. Methods: Neonates with perinatal asphyxia treated with TH were included from birth until Day 6 in a multicentre prospective cohort study. A vancomycin PPK model was constructed using nonlinear mixed-effects modelling. The model was used to evaluate published dosing guidelines with regard to pharmacokinetic/pharmacodynamic target attainment. The area under the curve/minimal inhibitory concentration ratio of 400–600 mg*h/L was used as target range. Results: Sixteen patients received vancomycin (median gestational age: 41 [range: 38–42] weeks, postnatal age: 4.4 [2.5–5.5] days, birth weight: 3.5 [2.3–4.7] kg), and 112 vancomycin plasma concentrations were available. Most samples (79%) were collected during the rewarming and normothermic phase, as vancomycin was rarely initiated during the hypothermic phase due to its nonempirical use. An allometrically scaled 1-compartment model showed the best fit. Vancomycin clearance was 0.17 L/h, lower than literature values for term neonates of 3.5 kg without perinatal asphyxia (range: 0.20–0.32 L/h). Volume of distribution was similar. Published dosing regimens led to overexposure within 24 h of treatment. A loading dose of 10 mg/kg followed by 24 mg/kg/day in 4 doses resulted in target attainment. Conclusion: Results of this study suggest that vancomycin clearance is reduced in term neonates with perinatal asphyxia treated with TH. Lower dosing regimens should be considered followed by model-informed precision dosing.

183. Associations between hypopharyngeal defect closure and quality of life in long-term total laryngectomy survivors

Abstract

BACKGROUND: Few studies have examined health-related quality of life (HRQOL) outcomes in long-term total laryngectomy survivors in relation to the type of hypopharyngeal defect.METHODS: A cross-sectional study was performed in long-term total laryngectomy survivors, treated between 2000 and 2020. The primary outcome was HRQOL, assessed using the FACE-Q Head and Neck Cancer Module, in relation to the type of hypopharyngeal closure (primary closure, partial or circumferential reconstruction).RESULTS: Seventy-nine survivors were included with a median follow-up of 92.1 months (IQR 75.6-140.2 months). Patients requiring partial hypopharyngeal reconstruction (n = 18) scored significantly worse than patients with primary closure (n = 51) on 4 of 13 FACE-Q domains: functional domains of eating (p = 0.03), speech (p = 0.05), and swallowing (p = 0.03), and the psychological domain of speaking-related distress (p = 0.02). No statistically significant differences were found between the circumferential hypopharyngeal defect reconstruction group (n = 10). Stricture occurrence was the only clinical factor associated with worse eating, speaking, swallowing, eating-related distress, and cancer worry in multivariable analyses.CONCLUSION: Several functional and psychological domains were significantly worse following partial hypopharyngeal reconstruction than in patients who received primary closure. Efforts to reduce stricture rates to enhance reconstructive outcomes following total laryngectomy merit further research.

Since reliable and up-to-date epidemiological information on peritoneal metastases is currently lacking, the aim of this thesis was to provide insight into the burden of peritoneal metastases by exploring epidemiological and clinical aspects from a variety of primary cancers. Previously, peritoneal metastases were considered as a terminal condition without effective treatment options. Nowadays, a more proactive attitude towards peritoneal metastases is practiced due to the evolution of loco regional and systemic therapies for these patients depending on their primary cancer. This renewed interest in peritoneal metastases points out the need for up-to-date epidemiological data regarding peritoneal metastases. This thesis investigated different epidemiological and clinical aspects (such as incidence, risk factors, treatment and survival) of patients with peritoneal metastases from colon cancer, gastric cancer, liver cancer, pancreatic cancer, biliary tract cancer, lung cancer and cancer from unknown primary.

Abstract

Background: In colorectal liver metastases (CRLM) patients, combination of liver resection and ablation permit a more parenchymal-sparing approach. This study assessed trends in use of combined resection and ablation, outcomes, and overall survival (OS). Methods: This population-based study included all CRLM patients who underwent liver resection between 2014 and 2022. To assess OS, data was linked to two databases containing date of death for patients treated between 2014 and 2018. Hospital variation in the use of combined minor liver resection and ablation versus major liver resection alone in patients with 2–3 CRLM and ≤3 cm was assessed. Propensity score matching (PSM) was applied to evaluate outcomes. Results: This study included 3593 patients, of whom 1336 (37.2%) underwent combined resection and ablation. Combined resection increased from 31.7% in 2014 to 47.9% in 2022. Significant hospital variation (range 5.9–53.8%) was observed in the use of combined minor liver resection and ablation. PSM resulted in 1005 patients in each group. Major morbidity was not different (11.6% vs. 5%, P = 1.00). Liver failure occurred less often after combined resection and ablation (1.9% vs. 0.6%, P = 0.017). Five-year OS rates were not different (39.3% vs. 33.9%, P = 0.145). Conclusion: Combined resection and ablation should be available and considered as an alternative to resection alone in any patient with multiple metastases.

OBJECTIVE: After lowering the Dutch threshold for active treatment from 25 to 24 completed weeks' gestation, survival to discharge increased by 10% in extremely preterm live born infants. Now that this guideline has been implemented, an accurate description of neurodevelopmental outcome at school age is needed.DESIGN: Population-based cohort study.SETTING: All neonatal intensive care units in the Netherlands.PATIENTS: All infants born between 240/7 and 266/7 weeks' gestation who were 5.5 years' corrected age (CA) in 2018-2020 were included.MAIN OUTCOME MEASURES: Main outcome measure was neurodevelopmental outcome at 5.5 years. Neurodevelopmental outcome was a composite outcome defined as none, mild or moderate-to-severe impairment (further defined as neurodevelopmental impairment (NDI)), using corrected cognitive score (Wechsler Preschool and Primary Scale of Intelligence Scale-III-NL), neurological examination and neurosensory function. Additionally, motor score (Movement Assessment Battery for Children-2-NL) was assessed. All assessments were done as part of the nationwide, standardised follow-up programme.RESULTS: In the 3-year period, a total of 632 infants survived to 5.5 years' CA. Data were available for 484 infants (77%). At 5.5 years' CA, most cognitive and motor (sub)scales were significantly lower compared with the normative mean. Overall, 46% had no impairment, 36% had mild impairment and 18% had NDI. NDI-free survival was 30%, 49% and 67% in live born children at 24, 25 and 26 weeks' gestation, respectively (p<0.001).CONCLUSIONS: After lowering the threshold for supporting active treatment from 25 to 24 completed weeks' gestation, a considerable proportion of the surviving extremely preterm children did not have any impairment at 5.5 years' CA.

Introduction In view of growing populations with chronic conditions, many countries are redesigning their health systems. However, little information is available about how health systems perform from the perspective of people living with chronic conditions. The Organisation for Economic Co-operation and Development (OECD) Member States therefore mandated the OECD to initiate the International Survey of People Living with Chronic Conditions (PaRIS survey), which aims to provide insight in outcomes and experiences of care as reported by people living with chronic conditions. The PaRIS-SUR consortium has been tasked by the OECD to support the development and implementation of the survey.Methods and analysis As primary care services play a pivotal role in the management of chronic conditions, the PaRIS survey will be implemented in the primary care setting. Data will be collected with a survey among users of primary care services aged 45 years or older, of whom many have chronic conditions. An additional survey is conducted among their primary care providers. The nested study design will allow analysis of the patient-reported data in relation to characteristics of and care provided by primary care providers within and across countries. In 2022, the survey will be tested in a Field Trial in participating countries. Data for cross-country comparison will be collected by the Main Survey in 2023.Ethics and dissemination Informed consent will be obtained from primary care providers and service users. National Project Managers search ethical approval of the survey in their country, if required. Reporting by the OECD will focus on questions for international comparison. A secured information technology platform will be developed for participants and stakeholders in countries to receive feedback and answer their own questions. Findings will also be disseminated through an international OECD flagship report, conferences, scientific papers and policy briefs, to inform strategies to improve care for people living with chronic conditions throughout the world.

Abstract

Neutron stars with large masses $\sim 2M_{\odot}$ require the hard stiffness of equation of state (EoS) of neutron-star matter. On the other hand, hyperon mixing brings about remarkable softening of EoS. In order to solve this problem, a multi-pomeron exchange potential (MPP) is introduced as a model for the universal many-body repulsion in baryonic systems on the basis of the Extended Soft Core (ESC) baryon-baryon interaction. The strength of MPP is determined by analyzing the nucleus-nucleus scattering with the G-matrix folding model. The interactions in $\Lambda\!N$, $\Sigma\!N$ and $\Xi\!N$ channels are shown to be consistent with experimental indications. The EoS in neutron-star matter with hyperon mixing is obtained from ESC in addition of MPP, and mass-radius relations of neutron stars are derived. The maximum mass is shown to reach $2M_{\odot}$ even in the case of including hyperon mixing on the basis of model-parameters determined by terrestrial experiments., Comment: prepared for the 2015 EPJA Topical Issue on "Exotic Matter in Neutron Stars"

Abstract

This paper presents the Extended-Soft-Core (ESC) potentials ESC08c for baryon-baryon channels with total strangeness S=-2. The potential models for S=-2 are based on SU(3) extensions of ESC potential models for the S=0 and S=-1 sectors, which are fitted to experimental data. Flavor SU(3)-symmetry is broken only kinematically by the masses of the baryons and the mesons. For the S=-2 channels no experimental scattering data exist, and also the information from hypernuclei is rather limited. Nevertheless, in the fit to the S=0 and S=-1 sectors information from the so called NAGARA event and the $\Xi$-well-depth has been used as constraints to determine the free parameters in the simultaneous fit of the $NN \oplus YN \oplus YY$ data. Therefore, the potentials for the S=-2 sector are determined mainly by the NN-, YN-data, and SU(3)-symmetry.Various properties of the potentials are illustrated by giving results for scattering lengths, effective ranges, bound states, elastic and inelastic phase parameters, and total cross sections. Notably is the prediction of a bound state D$^*$ in the $\Xi N(^3S_1,I=1)$-channel with a binding energy $B_E=1.56$ MeV. This state is deuteron-like, i.e. a member of the $\{10^*\}$-decuplet. As for the normal deuteron $D= pn(^3S_1-^3D_1)$ the strong tensor force is responsible for this state. The features of $\Xi$ hypernuclei predicted by ESC08c are studied on the basis of the G-matrix approch. The well-depth $U_\Xi= -7.0$ MeV, and the $\Xi -\Lambda\Lambda$ conversion width is $\Gamma_\Xi^c= 4.5$ MeV., Comment: arXiv admin note: substantial text overlap with arXiv:nucl-th/0608074

Abstract

The YN results are presented from a new version of the Extended-soft-core (ESC) potential model for Baryon-baryon (BB) scattering. The potentials consist of local- and non-local-potentials due to (i) One-boson-exchange (OBE), (ii) Pomeron and Odderon exchanges, (iii) Two pseudoscalar-exchange (PS-PS), and (iv) Meson-Pair-exchange (MPE). The OBE- and MPE-vertices are regulated by gaussian form factors, producing moderate size potentials in the origin. In addition to the mentioned ingredients of the ESC-model, also the possible short range repulsion due to the quark Pauli-principle in the BB-channels is included in the analysis. The NN, YN, and the S=-2 YY scattering is treated in a unified way using flavor SU(3) symmetry. With the ESC08-model a simultaneous fit is performed for the combined NN and YN data, using single sets of parameters. The achievement is a fit with (i) $\chi^2/NN_{data}= 1.081$, and (ii) $\chi^2/YN_{data}=1.08$.

Abstract

Abstract Background Malaria and hypertensive disorders of pregnancy (HDoP) affect millions of pregnancies worldwide, particularly those of young, first-time mothers. Small case-control studies suggest a positive association between falciparum malaria and risk of pre-eclampsia but large prospective analyses are lacking. Methods We characterized the relationship between malaria in pregnancy and the development of HDoP in a large, prospectively followed cohort. Pregnant women living along the Thailand-Myanmar border, an area of low seasonal malaria transmission, were followed at antenatal clinics between 1986 and 2016. The relationships between falciparum and vivax malaria during pregnancy and the odds of gestational hypertension, pre-eclampsia, or eclampsia were examined using logistic regression amongst all women and then stratified by gravidity. Results There were 23,262 singleton pregnancies in women who presented during the first trimester and were followed fortnightly. Falciparum malaria was associated with gestational hypertension amongst multigravidae (adjusted odds ratio (AOR) 2.59, 95%CI 1.59–4.23), whereas amongst primigravidae, it was associated with the combined outcome of pre-eclampsia/eclampsia (AOR 2.61, 95%CI 1.01–6.79). In contrast, there was no association between vivax malaria and HDoP. Conclusions Falciparum but not vivax malaria during pregnancy is associated with hypertensive disorders of pregnancy.

197. Evaluating the Effects of Multisystemic Therapy for Adolescents with Intellectual Disabilities and Antisocial or Delinquent Behaviour and Their Parents

Abstract

Background: An adaptation of multisystemic therapy (MST) was piloted to find out whether it would yield better outcomes than standard MST in families where the adolescent not only shows antisocial or delinquent behaviour, but also has an intellectual disability. Method: To establish the comparative effectiveness of MST-ID (n = 55) versus standard MST (n = 73), treatment outcomes were compared at the end of treatment and at 6-month follow-up. Pre-treatment differences were controlled for using the propensity score method. Results: Multisystemic therapy-ID resulted in reduced police contact and reduced rule breaking behaviour that lasted up to 6 months post-treatment. Compared to standard MST, MST-ID more frequently resulted in improvements in parenting skills, family relations, social support, involvement with pro-social peers and sustained positive behavioural changes. At follow-up, more adolescents who had received MST-ID were still living at home. Conclusions: These results support further development of and research into the MST-ID adaptation.
