152. How can both the intervention and its evaluation fulfill health promotion principles? an example from a professional development program

Abstract

The emergence over the past 20 years of health promotion discourse poses a specific challenge to public health professionals, who must come to terms with new roles and new intervention strategies. Professional development is, among other things, a lever for action to be emphasized in order to meet these challenges. To respond to the specific training needs of public health professionals, a team from the Direction de santé publique de Montréal (Montreal Public Health Department) in Quebec, Canada, established in 2009 the Health Promotion Laboratory, an innovative professional development project. An evaluative component, which supports the project’s implementation by providing feedback, is also integrated into the project. This article seeks to demonstrate that it is possible to integrate the basic principles of health promotion into a professional development program and its evaluation. To this end, it presents an analytical reading of both the intervention and its evaluation component in light of the cardinal principles in this field. Initiatives such as the Health Promotion Laboratory and its evaluation are essential to consolidate the foundations of professional development and its assessment by concretely integrating health promotion discourse into these practices.

153. Complexity : a potential paradigm for a health promotion discipline

Abstract

Health promotion underpins a distancing from narrow, simplifying health approaches associated with the biomedical model. However, it has not yet succeeded in formally establishing its theoretical, epistemological and methodological foundations on a single paradigm. The complexity paradigm, which it has yet to broach head-on, might provide it with a disciplinary matrix in line with its implicit stances and basic values. This article seeks to establish complexity's relevance as a paradigm that can contribute to the development of a health promotion discipline. The relevance of complexity is justified primarily by its matching with several implicit epistemological and methodological/theoretical stances found in the cardinal concepts and principles of health promotion. The transcendence of ontological realism and determinism as well as receptiveness in respect of the reflexivity that complexity encompasses are congruent with the values of social justice, participation, empowerment and the concept of positive health that the field promotes. Moreover, from a methodological and theoretical standpoint, complexity assumes a holistic, contextual and transdisciplinary approach, toward which health promotion is tending through its emphasis on ecology and interdisciplinary action. In a quest to illustrate our position, developmental evaluation is presented as an example of practice stemming from a complexity paradigm that can be useful in the evaluation of health promotion initiatives. In short, we argue that it would be advantageous for health promotion to integrate this paradigm, which would provide it with a formal framework appropriate to its purposes and concerns.

154. Collaborative learning processes in the context of a public health professional development program : a case study

Abstract

The health promotion laboratory (HPL – Canada) is a public health professional development program building on a collaborative learning approach in order to support long-term practice change in local health services teams. This study aims to analyse the collaborative learning processes of two teams involved in the program during the first year of implementation. Based on a multiple case study design involving observations, interviews, and documentary sources, the study: (1) describes the learning process by which each team built a common understanding of the problem at hand and developed an intervention to address it; (2) identifies factors that facilitated or hindered these processes; and (3) proposes a cross-case explanation of the collaborative learning process in the HPL. The results demonstrate that the two teams learned by expanding their repertoire of actions, albeit experiencing different processes. Results point to the central role of shared mental models and key influencing factors, such as commitment and participation (team cohesion), team climate (psychological safety), and leadership style. Unlike previous studies on team learning that concentrated on existing teams in organisations, the current research studied purposely created teams working at transforming their practices and showed that they can successfully learn if specific conditions are achieved.

155. Defining, illustrating and reflecting on logic analysis with an example from a professional development program

Abstract

Program designers and evaluators should make a point of testing the validity of a program's intervention theory before investing either in implementation or in any type of evaluation. In this context, logic analysis can be a particularly useful option, since it can be used to test the plausibility of a program's intervention theory using scientific knowledge. Professional development in public health is one field among several that would truly benefit from logic analysis, as it appears to be generally lacking in theorization and evaluation. This article presents the application of this analysis method to an innovative public health professional development program, the Health Promotion Laboratory. More specifically, this paper aims to (1) define the logic analysis approach and differentiate it from similar evaluative methods; (2) illustrate the application of this method by a concrete example (logic analysis of a professional development program); and (3) reflect on the requirements of each phase of logic analysis, as well as on the advantages and disadvantages of such an evaluation method. Using logic analysis to evaluate the Health Promotion Laboratory showed that, generally speaking, the program's intervention theory appeared to have been well designed. By testing and critically discussing logic analysis, this article also contributes to further improving and clarifying the method.

170. The ecological approach in health promotion programmes: the views of health promotion workers in Canada

Abstract

Researchers examined the attitudes of Canadian health promotion workers to the ecological approach in health promotion. Questionnaire responses from 117 health promotion workers throughout the Canadian provinces revealed a good awareness of the approach. Respondents generally felt that modifications of peoples' environment would be an effective approach to health promotion, and that interventions of this sort would attract professional support. However, many did not feel competent to use the ecological approach, and many appeared to hold values which were inconsistent with the approach.

172. Potential barriers to physician follow-up within 7 days of discharge from a chronic obstructive pulmonary disease hospital admission

AbstractRATIONALE: Given that physician follow-up after hospital may reduce readmissions for patients with chronic obstructive pulmonary disease (COPD), understanding factors associated with follow-up could help to improve discharge transitions.OBJECTIVESTo determine rate and factors associated with physician follow-up within 7 days of discharge from a COPD-related hospitalization.METHODSPopulation-based retrospective cohort study of patients with COPD discharged from an Ontario hospital between April 1, 2010 and March 31, 2016 using health administrative data. All patients > =35 years old were included. The primary outcome was follow-up with family doctor, respirologist or internal medicine specialist within 7 days of discharge. Multivariable logistic regression analysis was used to determine demographic, socioeconomic, provider and health care access variables associated with follow-up.MEASUREMENTS AND MAIN RESULTS: Overall, 24,438 (29.8%) out of 81,960 patients had physician follow-up within 7 days of discharge. Women (OR 0.89, 95% CI 0.86-0.91), rural dwellers (OR 0.86, 95% CI 0.82-0.90), patients without a family doctor (OR 0.38, 95% CI 0.34-0.44) and those from low-income regions, or those who had hospital length-of-stay >7 days (OR 0.81, 95% CI 0.78-0.85) were less likely to receive follow-up within 7 days. Patients were more likely to receive follow-up if they required intensive care (OR 1.07, 95% CI 1.02-1.12) or had seen a physician frequently in the past.CONCLUSIONSFewer than 1 in 3 patients received early follow-up. Barriers to follow-up may exist for women, rural-dwellers, patients without a family doctor, those residing in low-income regions, or who experienced prolonged hospital stays.

Abstract

Background Canada, a temperate country with four defined seasons incurs an annual productivity loss of over $30 billion on major depressive disorder (MDD); however it remains unknown whether inpatient hospitalization for MDD exhibits seasonal variations. Objective Our study objective was to determine if there are seasonal variations in hospitalization rates for MDD in Canada. Methods We used time series analysis to determine monthly rates of hospitalizations for MDD from 2006 – 2013, on data from population level health-administrative databases in Ontario, Canada. We also stratified analysis by gender and three age groups: 18 to 39, 40 to 65 and those over 65. We compared demographic and comorbidity profiles of patients admitted in April, August and December to elucidate if patient characteristics differed by season of admission. Results We identified a total of 130,336 admissions for MDD for 95,439 unique patients. Baseline characteristics of the patients were similar across seasons. We did not detect significant seasonality of hospitalizations for MDD across any of the gender or age subgroups. Discussion Our results question the popularly held belief that hospitalizations for MDD vary with seasons. These findings highlight the need for uniform hospital resource allocation for MDD throughout the year in Canada.

175. Assessment of Implementation Outcomes of a Peer-Led Program Targeting Fear of Falling Among Older Adults.

Abstract

Background: This study examined the implementation outcomes (program reach, fidelity, adaptations, responsiveness) of a peer-led program for older adults with fear of falling — Vivre en Équilibre (VEE). Method: VEE was implemented in six independent-living residences for older adults in Quebec (Canada) as part of an effectiveness study. Implementation outcomes were documented using attendance sheets, peer leaders' logbooks, observation sheets, and phone-administered questionnaires. Qualitative interviews were also conducted with peer leaders, activity coordinators of residences, and a subsample of program participants. Results: The program reached 71 participants who generally corresponded to the program's target population. Peer leaders delivered the program with moderate to high fidelity but adapted some elements. Responsiveness was good, as reflected by a high attendance rate (91%) and respondents' satisfaction levels. Conclusion: Findings revealed that VEE was well implemented, suggesting that it can be successfully delivered by peer leaders. [ABSTRACT FROM AUTHOR]

176. Lyme disease prevention: A content analysis of Canadian patient group and government websites.

Abstract

The quality of information presented on health‐related websites—in terms of comprehensiveness, accuracy and consistency—is a public health concern. To date, the consistency of information across Canadian websites devoted to Lyme disease (LD) prevention has not been evaluated. The first aim of this study was to describe the contents and recommendations of LD prevention websites provided by two types of Canadian organizations: government (n = 3) and patient groups (n = 3). A second objective was to analyse the level of convergence among these websites in terms of their prevention‐related content. Initial coding of the content resulted in information segments grouped into 114 subthemes related to nine overarching themes: tick habitat suitability, risk period, transmission, personal protection, peridomestic environmental management, tick identification and removal, early symptoms, testing and diagnosis and preventive treatment. Comparative content analyses were performed both within and between the content of websites of the two organization types. The themes most frequently addressed by both organization types were personal protection (20% of the prevention‐related content in patient group websites and 22% of the prevention‐related content in government websites), transmission (12% and 16%, respectively) and tick identification and removal (19% and 15%, respectively). Government websites' information was generally convergent with that of patient group websites (four highly convergent themes, three moderately convergent and two divergent). Nevertheless, of particular concern were divergent messages and inaccurate information found on 11 subthemes out of 103. Examples included other possible modes of transmission and the ineffectiveness of DEET insect repellent. 188. The requirement for surgery and subsequent 30-day mortality in patients with COVID-19.

Abstract

The ongoing COVID-19 pandemic has had profound effects on the provision of surgical care. The potential perioperative mortality associated with surgical procedures in patients with COVID-19 has been estimated at 20%, but the data come from jurisdictions that experienced very high surges of COVID-19 patients. A rapid assessment of the types of surgical care for patients with COVID-19 in Ontario was carried out using administrative data, and we found that during the initial wave in the spring of 2020, surgical interventions were required in 0.6% of patients with COVID-19, and mortality was higher (20%) in patients who underwent surgery in the 2 weeks before or after a positive nasopharygeal swab than in those who had surgery more than 2 weeks after COVID-19 was diagnosed. 191. Densité résidentielle et dépression chez la personne âgée vivant en milieu urbain au Québec : le cas de deux régions métropolitaines de recensement

Problématique : La dépression est une maladie importante chez la personne âgée dont les facteurs de risque individuels et sociaux sont bien connus. De plus en plus de travaux confirment cependant le rôle de facteurs de risque environnementaux en santé mentale bien qu’à cet égard, le rôle de variables telles que la densité résidentielle reste à clarifier. Objectifs : Mesurer la relation entre la densité résidentielle et les symptômes dépressifs chez la personne âgée. Vérifier, relativement à cette relation, l’effet médiateur de la qualité du réseau social et modérateur de l’accès à une automobile pour les déplacements. Méthode : Le devis est une étude observationnelle transversale impliquant une analyse secondaire de la banque de données « NuAge », une étude longitudinale sur la nutrition réalisée en 2006-2007 et portant sur 1 793 volontaires de 63 ans et plus habitant à Montréal, Laval et Sherbrooke. Les symptômes dépressifs sont mesurés par l’Échelle de dépression gériatrique (EDG). Les adresses des participants des régions métropolitaines de recensement (RMR) de Montréal et de Sherbrooke sont géocodées et liées à la densité résidentielle mesurée à l’échelle des aires de diffusion. Des analyses de régression logistique pondérées mesurent l’association entre le quintile de densité résidentielle le plus élevé comparativement au moins élevé et les symptômes dépressifs, en contrôlant pour les facteurs de risque individuels et le niveau de défavorisation matérielle du quartier. L’approche de Baron et Kenny est utilisée pour tester les variables médiatrices et modératrices. Des analyses distinctes sont faites pour chacune des RMR. Résultats : Les personnes âgées résidant dans le quintile de densité résidentielle le plus élevé de la RMR de Sherbrooke étaient, comparativement au quintile le moins dense, plus à risque de présenter des symptômes dépressifs. Abstract

'Outings to Your Taste' is an innovative program that aims to improve the nutritional status and social network of elderly people who receive home-delivered meals. This article examines participation in one of the program's components, outings to community restaurants. Participation data were collected on-site and information about client characteristics was collected in at-home interview surveys of targeted clients (n = 144). While about half of the clients had tried at,least one outing, more than 25% of them participated in at least one third of the outings offered to them. Results indicate that the program attracted a variety of clients in terms of sociodemographic, health, and social isolation characteristics. Abstract

Immune checkpoint inhibitor (ICI) therapy for patients undergoing cancer treatment carries a risk of severe immune-related adverse events (IRAEs). Questions remain about whether seasonal influenza vaccination might increase the risk of developing IRAEs among these patients given that vaccines are immunomodulatory. Previous vaccine safety studies on patients with cancer prescribed ICI therapy have demonstrated conflicting results. Using health administrative data from Ontario, Canada among adults diagnosed with cancer who had been prescribed ICI therapy and who had received an influenza vaccine from 2012 to 2019, we conducted a self-controlled case series study. The pre-vaccination control period started 42-days post-ICI initiation until 14-days prior to vaccination, the risk period was 1–42 days post-vaccination, and the post-vaccination control period was after the risk period until ICI discontinuation or a maximum period of two years. Emergency department (ED) visit(s) and/or hospitalization for any cause after ICI initiation was used to identify severe IRAEs. We fitted a fixed-effects Poisson regression model accounting for seasonality and calendar time to estimate relative incidence of IRAEs between risk and control periods. We identified 1133 records of cancer patients who received influenza vaccination while prescribed ICI therapy. Most were aged ≥ 66 years (73 %), were male (63 %), had lung cancer (54 %), and had received ICI therapy with a programmed cell death protein 1(PD-1) inhibitor (91 %). A quarter (26 %) experienced an ED visit and/or hospitalization during the observation period. Rates of ED visits and/or hospitalizations in the risk vs. control periods were similar, with an incidence rate ratio of 1.04 (95 % CI: 0.75–1.45). Subgroup and sensitivity analyses yielded similar results. Seasonal influenza vaccination was not associated with an increased incidence of ED visit or hospitalization among adults with cancer treated with ICI therapy and our results support further evidence of vaccine safety. 194. Exploring the Association Between Commute to School Duration and Children's Physical Activity Level and Bodyweight Status

Abstract

The commute to school is recognized as a daily activity with high potential for influencing children’s health behaviours and outcomes. While the impact of commute mode on children’s health has been extensively researched, the influence of commute duration remains relatively unexplored. This thesis uses binary logistic regression and multiple linear regression analysis of a cross-sectional sample to determine how active and inactive commute to school duration impacts children’s physical activity level and bodyweight status in urban, small town and rural environments in Southwestern Ontario. It is found here that longer active commute duration is associated with greater average levels of physical activity, but not greater rates of overweight or obesity. It is also found that longer inactive commute duration is associated with lower rates of overweight or obesity, despite a lack of significant difference in physical activity level. Data suggests that differences in home food environment may play a role. The built environment is shown to play little to no role in children’s health outcomes once commute mode is accounted for. In addition, contrary to other studies, results show that children living in small to medium population centres, rather than in rural areas, are more likely to be overweight or obese. This thesis contributes the first study on the effects of children’s inactive commute duration on physical activity level and obesity, and one of the few on the effects of children’s active commute duration on physical activity level and obesity. Methodologically, the strengths of this study include the utilization of researcher-measured BMI as well as GPS and accelerometry to capture precise commute duration, bodyweight status and physical activity level. This study also includes controls for a large number of known confounders at the individual and neighbourhood level.
