154. Panorama sobre o uso de agrotóxicos no Brasil (2009-2019): Riscos, benefícios e alternativas

R E S U M O Os agrotóxicos se tornaram parte integrante da prática da agricultura, contudo seus efeitos tóxicos causados na fauna, e, sobretudo em seres humanos, se tornaram alvo de grandes debates, principalmente no Brasil, onde nos últimos anos, políticas de flexibilização de novos registros se mostram cada vez mais presentes. No presente trabalho foi realizada uma revisão integrativa de dados obtidos através de sites governamentais e artigos publicados a partir do ano de 2010, salva a Legislação de 1989, bem como coleta em banco de dados dentro desse período histórico, sobre casos de intoxicação nacional, consumo de agrotóxicos dos países e registros de novos ativos. Os dados foram processados e avaliados estatisticamente buscando verificar a correlação entre a produção agrícola e o consumo de agrotóxicos, bem como verificar a influência dessa prática na ocorrência de casos e de óbitos por intoxicações, de forma a vislumbrar os impactos dessa prática e de prospectar a possibilidade do uso racional e da adesão a possíveis alternativas. Os dados foram reunidos e avaliados estatisticamente, permitindo-se observar uma forte correlação, estatisticamente significativa, entre a produção agrícola e o consumo de agrotóxicos, bem como uma influência no aumento de casos de intoxicação, trazendo à tona a importância do fomento à pesquisa voltada para práticas alternativas e para o uso racional de tais substâncias. Background Frailty is a medical syndrome characterised by reduced physiological reserve and increased vulnerability to stressors. Data regarding the relationship between frailty and atrial fibrillation (AF) are still inconsistent. Objectives We aim to perform a comprehensive evaluation of frailty in a large European cohort of AF patients. Methods A 40-item frailty index (FI) was built according to the accumulation of deficits model in the AF patients enrolled in the ESC-EHRA EORP-AF General Long-Term Registry. Association of baseline characteristics, clinical management, quality of life, healthcare resources use and risk of outcomes with frailty was examined. Results Among 10,177 patients [mean age (standard deviation) 69.0 (11.4) years, 4,103 (40.3%) females], 6,066 (59.6%) were pre-frail and 2,172 (21.3%) were frail, whereas only 1,939 (19.1%) were considered robust. Baseline thromboembolic and bleeding risks were independently associated with increasing FI. Frail patients with AF were less likely to be treated with oral anticoagulants (OACs) (odds ratio 0.70, 95% confidence interval 0.55–0.89), especially with non-vitamin K antagonist OACs and managed with a rhythm control strategy, compared with robust patients. Increasing frailty was associated with a higher risk for all outcomes examined, with a non-linear exponential relationship. The use of OAC was associated with a lower risk of outcomes, except in patients with very/extremely high frailty. Conclusions In this large cohort of AF patients, there was a high burden of frailty, influencing clinical management and risk of adverse outcomes. The clinical benefit of OAC is maintained in patients with high frailty, but not in very high/extremely frail ones.

Aims The 4S-AF classification scheme comprises of four domains: stroke risk (St), symptoms (Sy), severity of atrial fibrillation (AF) burden (Sb), and substrate (Su). We sought to examine the implementation of the 4S-AF scheme in the EORP-AF General Long-Term Registry and compare outcomes in AF patients according to the 4S-AF-led decision-making process. Methods and results Atrial fibrillation patients from 250 centres across 27 European countries were included. A 4S-AF score was calculated as the sum of each domain with a maximum score of 9. Of 6321 patients, 8.4% had low (St), 47.5% EHRA I (Sy), 40.5% newly diagnosed or paroxysmal AF (Sb), and 5.1% no cardiovascular risk factors or left atrial enlargement (Su). Median follow-up was 24 months. Using multivariable Cox regression analysis, independent predictors of all-cause mortality were (St) [adjusted hazard ratio (aHR) 8.21, 95% confidence interval (CI): 2.60–25.9], (Sb) (aHR 1.21, 95% CI: 1.08–1.35), and (Su) (aHR 1.27, 95% CI: 1.14–1.41). For CV mortality and any thromboembolic event, only (Su) (aHR 1.73, 95% CI: 1.45–2.06) and (Sy) (aHR 1.29, 95% CI: 1.00–1.66) were statistically significant, respectively. None of the domains were independently linked to ischaemic stroke or major bleeding. Higher 4S-AF score was related to a significant increase in all-cause mortality, CV mortality, any thromboembolic event, and ischaemic stroke but not major bleeding. Treatment of all 4S-AF domains was associated with an independent decrease in all-cause mortality (aHR 0.71, 95% CI: 0.55–0.92). For each 4S-AF domain left untreated, the risk of all-cause mortality increased substantially (aHR 1.35, 95% CI: 1.16–1.56). Conclusion Implementation of the novel 4S-AF scheme is feasible, and treatment decisions based on this scheme improve mortality rates in AF.

Para avanzar en la institucionalización del aprendizaje-servicio (ApS) en las universidades católicas, en 2020 se llevó a cabo el lanzamiento del programa UNISERVITATE. Este artículo tiene como objetivo describir cómo se aborda un proceso participativo de diagnóstico de la institucionalización del ApS a partir de la rúbrica de Furco en St. Mary’s university, Universidade Católica de Portugal y Università Cattolica del Sacro Cuore. Dicho proceso ha sido liderado por el hub de la región, la Universidad de Deusto. Para ello se ha llevado a cabo una evaluación del grado de institucionalización del ApS a través de una herramienta que identifica en qué punto del proceso de institucionalización se encuentran las universidades. Como resultado se presenta la herramienta, las fortalezas y debilidades identificadas en las instituciones, y los planes de acción derivados de estas. Finalmente, se pone de relieve la necesidad de abordar investigaciones sobre lo que supone la institucionalización del ApS en las instituciones de educación superior católicas, así como la importancia de potenciar dicho proceso para promover el desarrollo humano integral del alumnado en educación superior., To advance the institutionalization of service-learning (SL) in Catholic universities, the launch of the UNISERVITATE program was carried out in 2020. The objective of this article is to describe how a participatory process for diagnosing the institutionalization of SL is approached from the Furco rubric at St. Mary’s university, Universidade Católica de Portugal and Università Cattolica del Sacro Cuore. This process has been led by the region's hub, the University of Deusto. To this end, an evaluation of the degree of institutionalization of the SL has been carried out through a tool that identifies where the universities are in the institutionalization process. As a result, the tool, the strengths and weaknesses identified in the institutions, and the action plans derived from them are presented. Introduction: Chest compression is the basic technique of cardiopulmonary resuscitation (CPR) in patients with cardiac arrest. The quality and early performance of CPR is critical to improve the prognosis and chances of restoring spontaneous blood flow. Today we have manual compressions and mechanical chest compression devices. Objective: The aim of this study is to clarify the importance of the LUCAS system (The Lund University Cardiac Arrest System) in CPR. Material and methods: A systematic review of the literature by mobilizing the descriptors "Cardiac Arrest", "Lucas Efficacy" and "nursing", using the methodological head. Ten conceivable databases were selected, between 2010-2020, and seven articles were included for analysis. Results and discussion: The results of the studies are different, as four of the studies indicate that LUCAS is fully effective, particularly in fatigue and transport, but two of them do not recognise its full effectiveness. One of the studies mentions that the fact that compression by the automatic device is effective does not necessarily reflect a better result. The vast majority of the studies recognise the need for further studies to make more convincing decisions. Conclusion: Mechanical chest compression devices can improve patient outcome if used appropriately in the event of cardiac arrest. The hemodynamic performance of the LUCAS compression-decompression system is, according to some results, better than manual CPR. However, the quality of the current evidence is not sufficient. Randomised studies are needed to evaluate the effect of mechanical chest compression devices on survival inside or outsider the hospital. info:eu-repo/semantics/publishedVersion

- Abstract
Serving a plethora of devices in massive machinetype communications (mMTC) can rely on spatial multiplexing enabled by massive multiple-input multiple-output (mMIMO) technology. To release the full potential, accurate channel estimation is needed. Due to the large numbers of devices it necessitates pilot reuse. We propose a pilot allocation algorithm based on multi-point channel charting (CC) to mitigate inevitable pilot contamination in a multi-cell multi-sector mMTC network with spatially correlated mMIMO channels. The generated CC represents an effective interference map from channel covariance matrices to capture the degree of pilot contamination caused by sharing the same pilot sequence among multiple users. The map is then fed into a greedy algorithm that aims at optimizing the reuse pattern of orthogonal pilot sequences to minimize the performance degradation caused by pilot contamination. The proposed CC-based method is empirically shown to obtain notable gains over a reuse-factor-aware random pilot allocation, yet leaving room for further improvements.

- Abstract
Massive multiple-input multiple-output (mMIMO) technology is a way to increase spectral efficiency and provide access to the Internet of things (IoT) and machine-type communication (MTC) devices. To exploit the benefits of large antenna arrays, accurate channel estimation through pilot signals is needed. Massive IoT and MTC systems cannot avoid pilot reuse because of the enormous numbers of connected devices. We propose a pilot reuse algorithm based on channel charting (CC) to mitigate pilot contamination in a multi-sector single-cell mMIMO system having spatially correlated channels. We show that after creating an interference map via CC, a simple strategy to allocate the pilot sequences can be implemented. The simulation results show that the CC-based pilot reuse strategy improves channel estimation accuracy, which subsequently improves the symbol detection performance and increases the spectral efficiency compared to other existing schemes. Moreover, the performance of the CC pilot assignment method approaches that of exhaustive search pilot assignment for small network setups.

- Abstract
One outstanding challenge for machine learning in diagnostic biomedical imaging is algorithm interpretability. A key application is the identification of subtle epileptogenic focal cortical dysplasias (FCDs) from structural MRI. FCDs are difficult to visualize on structural MRI but are often amenable to surgical resection. We aimed to develop an open-source, interpretable, surface-based machine-learning algorithm to automatically identify FCDs on heterogeneous structural MRI data from epilepsy surgery centres worldwide. The Multi-centre Epilepsy Lesion Detection (MELD) Project collated and harmonized a retrospective MRI cohort of 1015 participants, 618 patients with focal FCD-related epilepsy and 397 controls, from 22 epilepsy centres worldwide. We created a neural network for FCD detection based on 33 surface-based features. The network was trained and cross-validated on 50% of the total cohort and tested on the remaining 50% as well as on 2 independent test sites. Multidimensional feature analysis and integrated gradient saliencies were used to interrogate network performance. Our pipeline outputs individual patient reports, which identify the location of predicted lesions, alongside their imaging features and relative saliency to the classifier. On a restricted 'gold-standard' subcohort of seizure-free patients with FCD type IIB who had T1 and fluid-attenuated inversion recovery MRI data, the MELD FCD surface-based algorithm had a sensitivity of 85%. Across the entire withheld test cohort the sensitivity was 59% and specificity was 54%. After including a border zone around lesions, to account for uncertainty around the borders of manually delineated lesion masks, the sensitivity was 67%. Meagre (Argyrosomus regius) belongs to the family Sciaenidae and is a promising candidate for Mediterranean aquaculture diversification. As a relatively recent species in aquaculture, the physiological consequences of the immune system activation in meagre are understudied. Per avançar en la institucionalització de l'aprenentatge servei (ApS) a les universitats catòliques, el 2020 es va dur a terme el llançament del programa UNISERVITATE. Aquest article té com a objectiu descriure com s'aborda un procés participatiu de diagnòstic de la institucionalització de l'ApS a partir de la rúbrica de Furco a la St. Mary's University, la Universidade Catòlica de Portugal i la Università Cattolica del Sacro Cuore. Aquest procés ha estat liderat pel hub de la regió, la Universitat de Deusto. Per això s'ha dut a terme una avaluació del grau d'institucionalització de l'ApS a través d'una eina que identifica en quin punt del procés d'institucionalització es troben les universitats. Com a resultat es presenta l'eina, les fortaleses i les debilitats identificades a les institucions, i els plans d'acció que se'n deriven. Finalment, es posa en relleu la necessitat d'abordar investigacions sobre el que suposa la institucionalització de l'ApS a les institucions d'educació superior catòliques, així com la importància de potenciar aquest procés per promoure el desenvolupament humà integral de l'alumnat en educació superior., To advance the institutionalization of service-learning (SL) in Catholic universities, the launch of the UNISERVITATE program was carried out in 2020. The objective of this article is to describe how a participatory process for diagnosing the institutionalization of SL is approached from the Furco rubric at St. Mary’s university, Universidade Católica de Portugal and Università Cattolica del Sacro Cuore. This process has been led by the region's hub, the University of Deusto. To this end, an evaluation of the degree of institutionalization of the SL has been carried out through a tool that identifies where the universities are in the institutionalization process. As a result, the tool, the strengths and weaknesses identified in the institutions, and the action plans derived from them are presented. Finally, the need to undertake research on what the institutionalization of SL implies in Catholic higher education institutions is highlighted, as well as the importance of promoting said process to promote the integral human development of students in higher education., Para avanzar en la institucionalización del aprendizaje-servicio (ApS) en las universidades católicas, en 2020 se llevó a cabo el lanzamiento del programa UNISERVITATE. Este artículo tiene como objetivo describir cómo se aborda un proceso participativo de diagnóstico de la institucionalización del ApS a partir de la rúbrica de Furco en St. Mary’s university, Universidade Católica de Portugal y Università Cattolica del Sacro Cuore. Dicho proceso ha sido liderado por el hub de la región, la Universidad de Deusto. Para ello se ha llevado a cabo una evaluación del grado de institucionalización del ApS a través de una herramienta que identifica en qué punto del proceso de institucionalización se encuentran las universidades. Children have the right to express their views in all matters affecting them, and to have them considered and given due weight. Children’s participation is most meaningful when rooted in children’s everyday lives, and its promotion should be encouraged from the youngest ages, especially in early childhood education and care (ecec). In this paper we apply the Lundy model of participation, widely used in policy, practice and professional development initiatives, to the ecec context. Based on examples provided by ecec professionals from Belgium, Greece, Poland, and Portugal, we illustrate the implementation of the elements of space, voice, audience and influence, proposed by the Lundy model. We also discuss the interrelations among these elements and the need for organisational and contextual support to enhance children’s participation. This paper adds to existing literature, highlighting theoretical and practical issues associated with the promotion of children’s right to participate in ecec.

Aquaculture is one of the world’s fastest-growing industries, with 6 % annual growth since 2010, and is becoming an increasingly important component of the world’s food production. European aquaculture has not kept up with the pace of change in other parts of the world, growing only 24 % since 1990, and only 6 % since 2007. As with all food production industries, aquaculture has a range of positive and negative impacts for the environment and the socio-economic system, which varies across production type, species, geography and biophysical context. The overall purpose of this study is to present the scientific basis of the positive and negative impacts of European aquaculture from an economic, environmental and social point of view, in order to facilitate a well-informed debate. However, the study does not seek to assess the magnitude or likelihood of impacts, compare magnitude or likelihood of impacts among different types of aquaculture nor compare the impacts identified to those of alternative food production systems. In order to achieve this aim, the project first undertook an extensive literature review to collate state-of- the-art scientific information on the positive and negative impacts of aquaculture. The study examined scientific peer-reviewed literature, supplemented by the Scientific, Technical and Economic Committee for Fisheries and European Data Collection Framework for aquaculture and focused on finfish culture (sea cages ; ponds, tanks, and raceways ; and recirculating aquaculture systems), shellfish culture (suspended, trays, and bottom culture), macroalgae and microalgae. To complement the impacts derived from the scientific literature a deeper analysis of impacts was conducted via 18 case studies across Denmark, Germany, France, Poland, Netherlands, Croatia, Greece, Ireland, Italy, Portugal and Spain. The case studies covered a range of production technologies and species types. The case studies were used to verify impacts identified in the scientific literature, to identify any impacts potentially overlooked by the scientific literature and to identify legislation and mitigation relevant to the aquaculture type at the national level. The case studies combined information from the literature review, supplementary scientific and grey literature of specific relevance to the case study, and key expert interviews (40 in total). Key experts consulted were primarily those from relevant national authorities and national research institutes. In parallel with the case studies, a review of the EU and national regulatory frameworks and associated mitigation measures was conducted. The review and the key expert interviews from the case studies were used to provide a preliminary analysis of the extent to which the regulatory framework mitigates negative impacts from aquaculture. The information from the literature reviews and the case studies was then collated and evaluated to provide an overview of the most important and most broadly identified positive and negative impacts for different aquaculture types. The evaluation made specific efforts to highlight consensus between the scientific literature and the opinions of key experts, as well as highlighting where these differ – indicating potential priority areas for future research.
