ABSTRACT: A specific composition of temperament traits with high novelty seeking (NS), high harm avoidance (HA), and low reward dependence (RD) has been attributed to adult patients with borderline personality disorder (BPD). This study examined whether an analogue personality profile is specifically associated with adolescent BPD. The female study sample comprised 33 adolescents with BPD, 35 clinical controls (CCs), and 31 healthy controls (HCs). Dimensions of temperament and character were measured according to Cloninger's biopsychosocial model of personality. Significantly higher means of NS and HA but lower means of RD could be determined in the adolescents with BPD compared with the CCs and the HCs. The comparable findings of this specific temperament constellation in adolescents and adult patients with BPD suggest that heritable factors such as temperamental traits may contribute to the vulnerability for developing BPD. Early identification of a 'borderline temperament' may facilitate early intervention and lower the risk for developing BPD. [ABSTRACT FROM AUTHOR]

Behavioural inhibition in the second year of life is a hypothesized predictor for shyness, social anxiety and depression in later childhood, adolescence and even adulthood. To search for the earliest indicators of this fundamental temperamental trait, this study examined whether behavioural characteristics in early infancy can predict behavioural inhibition, as previously postulated. The results show that infant crying to unfamiliar stimuli at 4 months of age is a significant predictor (p=.002) of behavioural inhibition in the second year of life. These data implicate the possibility of measuring a temperamental anxiety disposition at a very young age simply by assessing crying in the face of novel stimuli. [ABSTRACT FROM AUTHOR]

ABSTRACT BACKGROUND: Estrogen has been hypothesized to have a protective and antipsychotic-like effect in women at risk for schizophrenia. The aim of the present study was to evaluate the association between menstrual cycle and/or estrogen levels and psychotic symptoms in a sample of women with schizophrenia.MethodOne hundred and twenty-five premenopausal women with schizophrenia and regular menses were examined. The levels of 17beta-estradiol and other hormones of the gonadal axis were assessed in the follicular, peri-ovulatory, and luteal phases of the menstrual cycle. The effects of the menstrual cycle phase and/or the estradiol level on the Positive and Negative Syndrome Scale (PANSS) and the Brief Psychiatric Rating Scale (BPRS) scores were calculated by means of regression analyses. RESULTS: Significant improvement in psychotic, but not depressive, symptoms was observed during the luteal phase, compared with other days of the menstrual cycle. CONCLUSIONS: The present findings indicate that estradiol may have specific antipsychotic-like effects on the symptoms of schizophrenia. Healthcare institutions, such as hospitals, are made up of highly distributed and complex organizational structures and processes. As such there is an increasingly large number of information interfaces between various involved healthcare professionals, making efficient organization of patient care and disease management difficult. Innovative information technology, such as electronic patient record systems, may have the potential to support patient care by improving information logistics and information management. In this paper, we present a case study dealing with the use of information technology in clinical psychiatry. We first present the results of a detailed systems analysis which showed large deficiencies within multi-professional patient care in the psychiatry department, such as missing central representation of the course of patient care, and an enormous amount of oral communication. These deficiencies can be overcome by a central patient record, supporting all phases of care and all professional groups. We present an electronic record system, implementing these recommendations, as an example to support the nursing part of the process of care. Such a system was introduced and evaluated on two psychiatric wards of the University Medical Center Heidelberg. In a large epidemiological sample of young adults, predictions of the right-shift (RS) theory of Annett that cognitive abilities will vary with right-left hand skill were tested. Presenting a theory of a genetic balanced polymorphism with heterozygote advantage for laterality and ability, Annett & Manning (1989, 1990a) and Annett (1993c) claimed that probands at the right end of the R-L hand skill continuum would show lower general intelligence in IQ testing and that specific verbal abilities and educational success would be lower at both extremes of the R-L distribution, taking the form of an inverted U. Most of these predictions could not be confirmed by our study. In particular, our data contrast with the important and specific prediction of the RS theory that strong dextrals will be the most disfavoured group. In our sample, probands at the left end of the R-L continuum had significantly lower scores in spelling and educational success and showed a tendency to have lower non-verbal IQ scores, while strong dextrals tended to have average or even marginally higher ability scores. The effects, however, are small and decrease when controlling for other variables. Implications of these empirical findings for the right-shift theory are discussed. [ABSTRACT FROM AUTHOR]

The impact of premorbid social and intellectual functioning in childhood and early adolescence on the developmental course of schizophrenia is not sufficiently understood. In a retrospective case study (93 consecutive in-patients, 43 males and 50 females) of first-episode psychosis occurring in adolescence, the relationship between premorbid adjustment and short-term therapeutic outcome under treatment conditions was examined. All of the patients had a DSM-111-R diagnosis of schizophrenia (n=56) or schizoaffective disorder (n=37). The mean age of the patients at the time of the study was 15.8 (SD=1.0). Premorbid functioning during childhood and early adolescence was assessed by using the Cannon-Spoor et al. Premorbid Adjustment Scale (PAS) and studied with respect to its prognostic relevance for short-term therapeutic outcome (eight weeks) under neuroleptic treatment (350–700 mg Chlorpromazin dose equivalent). Criteria for clinical outcome were obtained from the study by Pearlson et al. (1989) which defines three grades (complete remission, partial remission and no response), according to the degree of positive symptomatology. Statistical analysis was based on nonparametric variance analysis. Patients with complete remission of positive symptoms after eight weeks of therapy had experienced far better premorbid adjustment in early adolescence and in childhood. Diagnosis and gender did not bias this result. Our data suggest that premorbid social functioning is a crucial variable with regard to therapeutic outcome in first-episode psychosis. Previous studies have reported a relation between poor premorbid functioning and negative symptoms. We found premorbid adjustment related to the course of positive symptoms. [ABSTRACT FROM AUTHOR]

The impact of delivery mode on the cardiac autonomic balance was studied in a sample of 101 full term appropriate for gestational age (AGA) human infants. Cardiac autonomic balance was measured by assessing basic heart rate, and two indicators of vagal tone, the Root Mean Square of Successive Differences (RMSSD) and Standard Deviation of NN-intervals (SDNN) as two different measures of short-term heart rate variability at 2, 6, and 16 weeks postnatal age. Sixty-seven infants were delivered spontaneously, 29 by cesarean section and five by vacuum extraction. Children delivered by vacuum extraction had a significantly lower basic heart rate (P=0.01), higher RMSSD (P=0.0003) and higher SDNN (P=0.0001) at two, but not at six and sixteen weeks, indicating a temporary elevation of cardiac vagal tone in these infants. These data indicate a potential transitory impact of vacuum extraction on autonomic balance persisting until at least two weeks postnatal age. [ABSTRACT FROM AUTHOR]
