159. Chronic toxicity studies of Tuttha bhasma (CuSO4.5H2O) in Wistar Albino Rats

Abstract

Tuttha Bhasma (TB) is an important metal based drug used in Ayurveda. It has widespread therapeutic uses in the treatment of metabolic disorders. Despite of its therapeutic utility there is a lack of data related to its safety. Hence the present study was undertaken to evaluate chronic toxicity of TB in Wistar albino rats. Chronic toxicity study was conducted as per AYUSH guidelines 170 and the parameters such as ponderal changes; biochemical, hematological and histopathological examinations were recorded. Based on the overall toxicity profile significant changes in the serum SGPT was noted in therapeutic and higher dose levels. We could see the serum cholesterol, SGOT, SGPT and albumin levels were significantly elevated at lower dose compared to the control group. Histopathology of kidney showed cell infiltration and sections from liver showed mild to moderate fatty changes at higher dose levels. Histopathology of jejunum revealed there was mild to moderate epithelial erosion and shortening of epithelial layer. However overall toxicity study parameters showed the test drug TB is well tolerated and no dose dependent toxicity symptoms were observed. Based on the findings we could conclude that Tuttha bhasma is much safer to use at therapeutic dose level. However, at higher dose levels there are chances of organ toxicity.

Abstract

The paper is concerned with the problem of determining a complete set of invariants for output feedback. Using tools from geometric invariant theory it is shown that there exists a quasi-projective variety whose points parameterize the output feedback orbits in a unique way. If the McMillan degree $n\geq mp$, the product of number of inputs and number of outputs, then it is shown that in the closure of every feedback orbit there is exactly one nondegenerate system., Comment: 15 pages

Abstract

Abstract Background Necrotizing enterocolitis (NEC) is a leading cause of neonatal morbidity and mortality in premature infants. To date, no effective biomarkers exist to predict which premature infants will develop NEC, limiting targeted prevention strategies. Multiple observational studies have reported an association between the exposure to red blood cell (RBC) transfusion and/or anemia and the subsequent development of NEC; however, the underlying physiologic mechanisms of how these factors are independently associated with NEC remain unknown. Methods In this paper, we outline our prospective, multicenter observational cohort study of infants with a birth weight ≤ 1250 g to investigate the associations between RBC transfusion, anemia, intestinal oxygenation and injury that lead to NEC. Our overarching hypothesis is that irradiation of RBC units followed by longer storage perturbs donor RBC metabolism and function, and these derangements are associated with paradoxical microvascular vasoconstriction and intestinal tissue hypoxia increasing the risk for injury and/or NEC in transfused premature infants with already impaired intestinal oxygenation due to significant anemia. To evaluate these associations, we are examining the relationship between prolonged irradiation storage time (pIST), RBC metabolomic profiles, and anemia on intestinal oxygenation non-invasively measured by near-infrared spectroscopy (NIRS), and the development of NEC in transfused premature infants. Discussion Our study will address a critical scientific gap as to whether transfused RBC characteristics, such as irradiation and metabolism, impair intestinal function and/or microvascular circulation. Given the multifactorial etiology of NEC, preventative efforts will be more successful if clinicians understand the underlying pathophysiologic mechanisms and modifiable risk factors influencing the disease. Abstract

A fully liquid hexavalent containing Diphtheria (D), Tetanus (T) toxoids, whole cell Pertussis (wP), Hepatitis B (Hep B), type 1, 2, 3 of inactivated poliovirus (IPV) and Haemophilus influenzae type b (Hib) conjugate vaccine (DTwP-HepB-IPV-Hib vaccine, HEXASIIL®) was tested for lot-to-lot consistency and non-inferiority against licensed DTwP-HepB-Hib + IPV in an open label, randomized Phase II/III study. In Phase III part, healthy infants received DTwP-HepB-IPV-Hib or DTwP-HepB-Hib + IPV vaccines at 6, 10 and 14 weeks of age. Blood samples were collected prior to the first dose and 28 days, post dose 3. Non inferiority versus DTwP-HepB-Hib + IPV was demonstrated with 95% CIs for the treatment difference for seroprotection/seroconversion rates. For DTwP-HepB-IPV-Hib lots, limits of 95% CI for post-vaccination geometric mean concentration ratios were within equivalence limits (0.5 and 2). Abstract

The present study aimed to identify the bioactive constituents in the chloroform extract of H. spicatum rhizomes (HS-RCLE), further evaluated for its in-vitro pesticidal activities validating via molecular docking techniques. GC/MS analysis of HS-RCLE identified 14 compounds contributing 84.1 % of the total composition. The extract was dominated by oxygenated sesquiterpenes (43.1 %) with curcumenone (25.2 %) and coronarin E (14.8 %) as the major compounds. The extract recorded 89.4 % egg hatchability inhibition and 82.6 % immobility of Meloidogyne incognita, 66.7 % insecticidal activity on Spodoptera litura, 100 % phytotoxic activity on Raphanus raphanistrum seeds, and 74.7 % anti-fungal activity on Curvularia lunata at the respective highest dose studied. The biological activities were furthermore validated by using docking studies on certain proteins/enzymes namely acetylcholinesterase (PBD ID: IC2O), carboxylesterase (PDB ID: 1CI8), acetohydroxyacid synthase (PBD ID: 1YHZ) and trihydroxy naphthalene reductase (PBD ID: 3HNR). The bioactivity of the major constituents of the extract was predicted with the help of in silico PASS studies. HS-RCLE was observed to be a viable alternative source of natural pesticidal agents and paves the way for further studies on its mechanistic approaches and field trials to ascertain its pesticidal studies.

Abstract

We compute the degree of the generalized Pl\"ucker embedding $\kappa$ of a Quot scheme $X$ over $\PP^1$. The space $X$ can also be considered as a compactification of the space of algebraic maps of a fixed degree from $\PP^1$ to the Grassmanian $\rm{Grass}(m,n)$. Then the degree of the embedded variety $\kappa (X)$ can be interpreted as an intersection product of pullbacks of cohomology classes from $\rm{Grass}(m,n)$ through the map $\psi$ that evaluates a map from $\PP^1$ at a point $x\in \PP^1$. We show that our formula for the degree verifies the formula for these intersection products predicted by physicists through Quantum cohomology~\cite{va92}~\cite{in91}~\cite{wi94}. Introduction: Distal end radius fractures are one the foremost common injuries of the upper extremity. Greatest prevalence was noticed elderly women who fall on their outstretched hand and young men involved in impact injuries. This is a prospective study conducted in the Department of Orthoped Materials And Methods: ics, Adichunchanagiri Institute of Medical Sciences, B.G.NAGARA after obtaining ethical clearance. This study consisted of 20 adult patients of either sex visiting outpatient department of the hospital/ casualty diagnosed to have distal end radius fractures with the involvement of radial and intermediate columns. Patients satisfying inclusion criteria were included in the study during the period from February 2021 to August 2022 In present stud Results: y 20 cases there were 12 males and 08 females with average age of presentation was 41.5 years, there was a predominance of right sided fractures when compared to the left. Mean duration between admission and surgery was 3.2 days, the mean duration of complete radiological union was 6.6 weeks. We have achieved excellent results in 50% of cases, good results in 25%, fair results 25%, and no poor results. Complications like Stitch abscess, stiffness, reduced range of motion, Persistent pain syndrome seen in 4 (20%) cases Conclusion: This present study shows that volar locking plate is a better technique in view of shorter operative time, less hardware, reduction in blood loss, less tendon irritation, lesser risk of neuromuscular injury. However this is a short term study with small sample size, needs long term follow up to predict further outcome

Abstract

Multicentric reticulohistiocytosis also known as lipoid dermatoarthritis is a rare multisystem disorder that affects skin, mucosa and joints and has distinct histopathological features. We report a case of a 31 year old female who presented to us for joint pain involving knees and wrists and asymptomatic papulonodules over forearms knuckles and periungual region. The diagnosis of multicentric reticulohistiocytosis was made on the basis of typical histopathological features which showed multinucleated giant cells and sheets of histiocytes with abundant granular eosinophilic cytoplasm. Work up for associated diseases was negative. The patient was put on systemic steroids however was lost for follow up. The worldwide incidence of the disease is very low (around 300 cases in recent years) and it is mainly seen in Caucasians although Hispanic, African and Asian cases have also been described. Reports from India are limited and hence the present case is being reported with a brief review of the disease.

Abstract

The aim of this study was to determine the relationship between iron exposure and the development of bronchopulmonary dysplasia (BPD).A secondary analysis of the PENUT Trial dataset was conducted. The primary outcome was BPD at 36 weeks gestational age and primary exposures of interest were cumulative iron exposures in the first 28 days and through 36 weeks' gestation. Descriptive statistics were calculated for study cohort characteristics with analysis adjusted for the factors used to stratify randomization.Of the 941 patients, 821 (87.2%) survived to BPD evaluation at 36 weeks, with 332 (40.4%) diagnosed with BPD. The median cohort gestational age was 26 weeks and birth weight 810 g. In the first 28 days, 76% of infants received enteral iron and 55% parenteral iron. The median supplemental cumulative enteral and parenteral iron intakes at 28 days were 58.5 and 3.1 mg/kg, respectively, and through 36 weeks' 235.8 and 3.56 mg/kg, respectively. We found lower volume of red blood cell transfusions in the first 28 days after birth and higher enteral iron exposure in the first 28 days after birth to be associated with lower rates of BPD.We find no support for an increased risk of BPD with iron supplementation.NCT01378273. https://clinicaltrials.gov/ct2/show/NCT01378273 IMPACT: Prior studies and biologic plausibility raise the possibility that iron administration could contribute to the pathophysiology of oxidant-induced lung injury and thus bronchopulmonary dysplasia in preterm infants. 195. Prevalence and real-world assessment of central aortic blood pressure in adult patients with essential hypertension uncontrolled on single anti-hypertensive agents

Abstract

Objective: To assess the prevalence of high central aortic pressure (CAP) in Indian patients with uncontrolled essential hypertension while on anti-hypertensive monotherapy. Also, to determine correlation between brachial blood pressure (BBP) and CAP, and ascertain if it is impacted by anti-hypertensive drug class and patients’ age. Methods: In this real-world, observational, prospective study, patients (30–70 years) with uncontrolled BBP (systolic BP [SBP] ≥140 mmHg or diastolic BP [DBP] ≥90 mmHg) were enrolled. Treatment was adjusted at Visit 1 (baseline), based on BBP and at treating physicians’ discretion. Primary endpoint was proportion of patients with uncontrolled central aortic SBP (>125 mmHg) at baseline. Secondary endpoints were comparison of BBP and CAP across drugs classes and age groups at baseline and Visit 2 (End-of-study, ∼8 weeks post-baseline), and proportion of patients with uncontrolled central SBP at end-of-study. Results: Of 2030 patients screened, 1949 patients reported at baseline and 1740 patients completed end-of-study visit. Central SBP was >125 mmHg for 84.3% patients at baseline, and 48% patients at end-of-study. Interestingly, at end-of-study, 6.6% patients still had uncontrolled brachial SBP and controlled central SBP, while 13.6% patients had uncontrolled central SBP and controlled brachial SBP. At both visits, brachial SBP and central SBP showed positive correlation across most drug classes and age groups. At baseline, ACE inhibitors showed better efficacy than other drug classes. At end-of-study, BP control was better with fixed-dose combinations, though free–drug combinations were more frequently prescribed. Conclusion: Measurement of CAP along with BBP can be vital in management of hypertension. Ctri registration number: CTRI/2015/10/006302. Abstract

In this study, a patch antenna with three different operating frequencies (2.94 GHz, 4.5 GHz, and 5.7 GHz) is proposed. The dimensions of the antenna are as follows: 23.4 mm by 30.4 mm by 1.6 mm. The triple band characteristic may be achieved by adjusting the width of the quarter wave transformer (QWT), which in turn modifies the impedance matching in such a manner as to excite the TM10, TM02, and TM12 modes. These modes are able to have additional verification provided by the current distribution on the patch. Altering the width of the QWT provides the proposed antenna with the ability to fine-tune its resonant frequencies. At the resonant frequencies, the impedance bandwidths of -10 dB are 69 MHz, 40 MHz, and 110 MHz, respectively. 5.7 GHz has a gain of 1.5 dB, while 4.5 GHz has a gain of 0.7 dB, and 2.94 GHz has a gain of 2 dB. Additionally, this antenna finds its applications in wireless systems in the S and C bands of the frequency spectrum. The antennas are validated using a vector network analyzer (VNA) after being simulated with mentor graphics software three-dimensional electromagnetic modeling (3DEM) and analyzed with it.

Abstract

Backgrounds and Aims :Levobupivacaine is a pure S-enantiomer of bupivacaine with similar anesthetic profile and with reduced toxic potential than racemic bupivacaine.This study aims to compare efficacy and safety of Transverse Abdominal Plane block inj. Levobupivacaine Vs. Inj. Ropivacaine for post operative analgesia following lower segment cesarean section. Methods and Material:A prospective, randomized, controlled, double blind clinical study. 60 patients divided in 2 groups by computer generated randomization.60 healthy women undergoing cesarean delivery under spinal anesthesia were randomized into two groups (Group I, Group II). Group I received TAP block with 15 ml of 0.25% ropivacaine while Group II received TAP block with 15 ml of 0.25% levobupivacaine on each side. Postoperatively, time for first request for rescue analgesia and number of women requesting analgesia in 6 h,8 h, 10 h, 12 h and 24 h were noted. Pain score was measured with the Visual Analogue Scale (VAS) at rest and on first 24 h. Patient comfort and satisfaction with analgesia was evaluated at the end of 24 h. Statistical analysis used:The data were analyzed with Students t‑test. Results:The duration of analgesia was significantly prolonged in Group I than group II. The VAS score in patients who received Inj. Ropivacaine was significantly lower than who received inj. Levobupivacaine. Patient and surgeon satisfaction score were significantly higher in Ropicavaine than levobupivacaine. Conclusions:0.25% Ropivacaine provided longer duration of analgesia compared to 0.25% Levobupivacaine in TAP Block for postoperative analgesia after lower segment cesarean section.
