Left ventricular (LV) thrombus in patients with reduced LV systolic function carries significant thromboembolic risk. Direct oral anticoagulants are an attractive alternative to warfarin for LV thrombus management. However, there are not enough data regarding the safety and efficacy of direct oral anticoagulants for the treatment of LV thrombus. (Level of Difficulty: Beginner.)

Biologically self-healing concretes are generally composed of immobilized or encapsulated bacterial spores and nutrients. The efficiency of this self-healing approach depends on the effective protection and storage by the carriers. This report describes a novel sustained-release strategy for the improved self-healing function and microcapsule resistance to the cement environment. Halloysite clay nanotubes (HNTs) loaded with the common microbiological nutrient, yeast extract (YE) were encapsulated in calcium alginate microcapsules alongside Bacillus pseudofirmus spores. The encapsulation of the YE-loaded HNTs improved the resistance of the microcapsules to the cement environment, improved the microcapsule-cement interface, while successfully retaining the nutrients after direct contact with the cement pore solution. The self-healing system effectively healed artificial concrete cracks compared to control samples, reducing the crack water tightness by over 95%, and improved the flexural strength regain by approximately 75% after 56 days of wet-dry incubation. The sustained-release mechanism considerably improved the self-healing performance.

Background: Women of reproductive age group (WoRAG) are among the most vulnerable groups to suicide in India. The present study intended to develop a mathematical model to differentiate suicides from homicides among WoRAG. Methods: It was a cross-sectional study based on a record review of autopsy at Patna, India, from 2016 to 2021. The cause of deaths was ascertained by autopsies and other records independently by two investigators to reduce the interobserver bias. Independent variables were tested with confirmed suicides to calculate statistically significant association. These variables were further used for developing prediction models for the suicides by multivariate logistic regression analysis. Results: Out of total of 520 autopsies of WoRAG performed by investigators, the cause of death has been confirmed for 62. Of them, 30 were confirmed as suicides. In univariate analysis, suicides were associated with the menstrual bleed (OR 35 CI 6.9,179), gastric emptying (OR 3.9 CI 1.2,12.8), hanging, poisoning, and drowning as mode of death (OR 435 CI 37.4,5061.9). By logistic regression, three prediction models were built to predict suicide; Model I: gastric emptying, Model II: menstrual bleed, and Model III: including both. The area under the curve (AUC) for Models I, II, and III was 0.67 (95%CI 0.34,0.99), 0.92 (95%CI 0.75,1.00), and 0.94 (95%CI 0.82,1.00), respectively. The AUC of Model III differs significantly from that of Model I (P value 0.03) but not with Model II (P value 0.11). Conclusion: Menstrual bleed, gastric emptying, and mode of death may be used as a supplement tool in ascertaining the cause of death among WoRAG in medical and legal proceedings.

Most of the Helicobacter pylori infections occur in developing countries. The risk factors for H. pylori infections are poverty, overcrowding, and unhygienic conditions, which are common problems in under-privileged countries such as Nepal. Despite having a high risk of H. pylori infections, no national level study has been conducted to assess prevalence and correlates of H. pylori infection in Nepal. Therefore, we hypothesized that micronutrients such as iron, vitamin B12 deficiency, socio-economic status, and nutritional status correlate with the prevalence of H. pylori infection in Nepal. We studied prevalence and correlates of H. pylori infection among under-five children, adolescents aged 10-19 years and married non-pregnant women aged 20-49 years using data from the Nepal National Micronutrient Status Survey 2016 (NNMSS-2016). H. pylori infection was examined in stool of 6-59 months old children and 20-49 years old non-pregnant women whereas the rapid diagnostic kit using blood sample was used among adolescent boys and girls. Prevalence of H. pylori infection was 18.2% among 6-59 months old children, 14% among adolescent boys and 16% among adolescent girls aged 10-19 years; and 40% among 20-49 years non-pregnant women. Poor socioeconomic status, crowding, and unhygienic condition were found to be positively associated with higher incidence of H. pylori infections. No significant correlation was observed between nutritional and micronutrients status (iron or risk of folate deficiency) with H. pylori infection. Findings from this study suggest that poverty-associated markers are primary contributors of H. pylori infections in Nepalese communities. To control acquisition and persistence of H. pylori infection in Nepal, we suggest improved management of safe drinking water and implementation of sanitation and hygiene programs, with a focus on those of lower socioeconomic status.

Background: Global health academic partnerships are centered around a core tension: they often mirror or reproduce the very cross-national inequities they seek to alleviate. On the one hand, they risk worsening power dynamics that perpetuate health disparities; on the other, they form an essential response to the need for healthcare resources to reach marginalized populations across the globe. Objectives: This study characterizes the broader landscape of global health academic partnerships, including challenges to developing ethical, equitable, and sustainable models. It then lays out guiding principles of the specific partnership approach, and considers how lessons learned might be applied in other resource-limited settings. Methods: The experience of a partnership between the Ministry of Health in Nepal, the non-profit healthcare provider Possible, and the Health Equity Action and Leadership Initiative at the University of California, San Francisco School of Medicine was reviewed. The quality and effectiveness of the partnership was assessed using the Tropical Health and Education Trust Principles of Partnership framework. Results: Various strategies can be taken by partnerships to better align the perspectives of patients and public sector providers with those of expatriate physicians. Actions can also be taken to bring greater equity to the wealth and power gaps inherent within global health academic partnerships. Conclusions: This study provides recommendations gleaned from the analysis, with an aim towards both future refinement of the partnership and broader applications of its lessons and principles. It specifically highlights the importance of targeted engagements with academic medical centers and the need for efficient organizational work-flow practices. It considers how to both prioritize national and host institution goals, and meet the career development needs of global health clinicians.

Cobalt-based chalcogenides have emerged as fascinating materials for supercapacitor applications owing to the presence of various mixed valance oxidation states in their structure along with rich electrochemical properties. However, their limited stability and cyclic performance hinder their viability for practical use in supercapacitors. Herein, a facile hot injection colloidal route is demonstrated to design MoSe 2 -Co 9 S 8 nanoheterostructures (NHSs), which entails the epitaxial growth of Co 9 S 8 nanoparticles (NPs) over the basal planes of ultrathin MoSe 2 nanosheets (NSs). The interfacial engineering of the basal planes of MoSe 2 NSs with Co 9 S 8 NPs regulates the electronic properties and defects at the interfaces and increases the overall specific surface area and conductivity. As a result, MoSe 2 -Co 9 S 8 NHSs electrode unveils a substantially higher specific capacitance of 910.5 F g -1 at 1 A g -1 current density surpassing their individual counterparts. In addition, it demonstrates worthy solidity, retaining ≈90% of its capacitance and coulombic efficiency of 93.3% after 10,000 charge-discharge cycles at a high charge-discharge current density of 15 A g -1 . As a proof-of-concept, coin cells are fabricated using MoSe 2 -Co 9 S 8 NHSs which show 93% Coulomb efficiency and 86% capacitance retention.
