151. Incidence of Deep Vein Thrombosis (DVT) in Patients Using A Knee Scooter

Abstract

Category: Ankle; Trauma Introduction/Purpose: Functional limitations after lower extremity injury often require the use of an assistive device such as a knee scooter. Many patients find these devices to be easier and more comfortable to use than crutches or walkers. There is concern regarding the possible correlation between scooter use and DVT. Prior literature demonstrated decreased venous flow with the knee flexed, and venous stasis is a known risk factor for DVT 1-3 . Further, healthy volunteers using a scooter demonstrated a significant decrease in volumetric blood flow rate at the popliteal fossa 4 . We report the incidence of DVTs after prescription of a scooter following lower extremity injury. We hypothesized that DVT rates would be higher in patients using a scooter compared to rates of DVT in the NSQIP database Methods: A retrospective record review of adult patients age 18-74 within the Military Health System (MHS) database between 2010-2020 was completed to identify the total number of patients who were prescribed a knee scooter and those who subsequently developed a DVT. Additional demographic and clinical data were collected on patients who had both a scooter and a DVT. This was compared with recorded 2015 NSQIP data on DVT incidence in major orthopaedic surgery procedures. Results: Between 2010 and 2020, 46,840 of patients within the MHS database were prescribed a rolling scooter after a lower extremity injury or surgery. Of those patients who were prescribed a scooter, 377 subsequently developed a DVT within 90 days of prescription. DVTs were diagnosed on average 26.5 days after scooter prescription. The incidence of DVT of in this population was 0.8%. Conclusion: In the Military Health System database, the use of a knee scooter following lower extremity injury or surgery is associated with a higher incidence of DVT compared to the 2015 NSQIPS incidence of DVT following major orthopedic surgery (0.80% vs 0.55%), and is most frequently diagnosed within 30 days of scooter prescription. Higher level study is needed to confirm these findings and identify risk factors of patients who experience DVTs associated with scooter use after lower extremity injury or surgery.

Abstract

In this paper, we deal with a notion of Banach space-valued mappings defined on a set consisting of finite graphs with uniformly bounded vertex degree. These functions will be endowed with certain boundedness and additivity criteria. We examine their normalized long-term behaviour along a particular class of graph sequences. Using techniques developed by Elek, we show convergence in the topology of the Banach space if the corresponding graph sequence possesses a hyperfinite structure. These considerations extend and complement the corresponding results for amenable groups. As an application, we verify the uniform approximation of the integrated density of states for bounded, finite range operators on discrete structures. Further, we extend results concerning an abstract version of Fekete's Lemma for amenable groups and cancellative semigroups to the geometric situation of convergent graph sequences., Comment: 13 pages, reference information included

Abstract

We study one-dimensional random Jacobi operators corresponding to strictly ergodic dynamical systems. In this context, we characterize the spectrum of these operators by non-uniformity of the transfer matrices and the set where the Lyapunov exponent vanishes. Adapting this result to subshifts satisfying the so-called Boshernitzan condition, it turns out that the spectrum is supported on a Cantor set with Lebesgue measure zero. Abstract

Antibiotic therapy must be carried out consistently and according to the guidelines. Viruses are the dominant cause of upper respiratory tract infections (URTIs) in children, as has been shown in many previous studies. Unnecessary antibiotic therapy should be avoided so that it does not affect patients' health and lead to the development of resistant bacterial strains. Here we report a national survey conducted in a group of 4,389 children to assess the impact of selected behavioral and environmental factors on antibiotic therapy in patients with URTIs. We found that selected environmental factors influenced the type of treatment. The place of residence, having siblings, an absence of vaccinations, the presence of allergies, and attendance at educational institutions were conducive to antibiotic therapy. These factors also influenced the frequency of hospitalization of children and their absence from nurseries, kindergartens, and schools, as well as the absence of their guardians from work.

Abstract

In this paper we prove a general convergence theorem for almost-additive set functions on unimodular, amenable groups. These mappings take their values in some Banach space. By extending the theory of epsilon-quasi tiling techniques, we set the ground for far-reaching applications in the theory of group dynamics. In particular, we verify the almost-everywhere convergence of abstract approximable bounded, additive processes, as well as a Banach space approximation result for the spectral distribution function (integrated density of states) for random operators on discrete structures in a metric space. Abstract

In this paper we study unimodular amenable groups. The first part is devoted to results on the existence of uniform families of epsilon-quasi tilings for these groups. In this context, constructions of Ornstein and Weiss are extended by quantitative estimates for the covering properties of the corresponding decompositions. Afterwards, we apply the developed methods to obtain an abstract ergodic theorem for a class of functions mapping subsets of a countable, amenable group into some Banach space. This significantly extends and complements the previous results of Lenz, M\"uller, Schwarzenberger and Veseli\'c. Further, using the Lindenstrauss ergodic theorem, we describe a link of our results to classical ergodic theory. Given a finitely generated amenable group we consider ergodic random Schr\"odinger operators on a Cayley graph with random potentials and random boundary conditions. We show that the normalised eigenvalue counting functions of finite volume parts converge uniformly. The integrated density of states as the limit can be expressed by a Pastur-Shubin formula. Abstract

Introduction: Adrenal hemorrhage (AH) is a very rare and potentially life-threatening disease which may be secondary to trauma or of non-traumatic etiology. Aim: The aim of the study was to present the characteristics and management of adrenal hemorrhage and show that adrenal hemorrhage is more common than expected and that the clinical symptoms are not specific. Materials and methods: This retrospective study involved 199 patients with postoperative diagnosis of adrenal hemorrhage. Discussion: The factors identified as potential causes of adrenal hemorrhage are adrenocortical carcinoma, pheochromocytoma, and adrenal adenoma. The study group included 199 patients with postoperative diagnosis of AH. It showed that all patients with postoperative diagnosis had pheochromocytoma (n = 54), adrenal adenoma (n = 68), or adenocarcinoma (n = 17). If we look more careful at the results, we can find only 30% of patients (n = 39) with preoperative diagnosis of AH. This group of 39 patients was prepared for expedited surgery. In this group of patients, the preoperative diagnosis of AH was pheochromocytoma 28% (n = 11), adenocarcinoma (n = 4), and adrenal adenoma (n = 9). Conclusions: Bleeding into adrenal tumors is still an insufficiently understood topic due to its unpredictability and, as can be seen in our material, of varying severity. Out of 199 patients, only 30% (n = 39) were prepared for surgery with a preoperative diagnosis of AH; most of them had pheochromocytoma. We suggest that is very important to prepare patients for surgery with a preoperative diagnosis of AH using α-adrenoreceptor antagonists. Prolongation of the diagnostic process (time between the imaging examination and the surgery) may result in the disease progressing and adrenal bleeding. [ABSTRACT FROM AUTHOR]

The treatment goal in atopic dermatitis is eliminating clinical symptoms of the disease, preventing exacerbations and complications, as well as improving patients’ quality of life. In cases of severe atopic dermatitis and lack of response it is recommended to introduce systemic therapy. Patients ofter require multi-specialist consultations, and occasionally hospitalization. It is not recommended to use acupuncture, acupressure, bioresonance, homeopathy, or Chinese herbs in the treatment of atopic dermatitis.

We assessed the prevalence of lung resection in PCD across countries and compared disease course in lobectomised and non-lobectomised patients. Methods In the international iPCD cohort, we identified lobectomised and non-lobectomised age and sex-matched PCD patients and compared their characteristics, lung function and BMI cross-sectionally and longitudinally. Results Among 2896 patients in the iPCD cohort, 163 from 20 centers (15 countries) underwent lung resection (5.6%). Among adult patients, prevalence of lung resection was 8.9%, demonstrating wide variation among countries. Compared to the rest of the iPCD cohort, lobectomised patients were more often females, older at diagnosis, and more often had situs solitus. In about half of the cases (45.6%) lung resection was performed before presentation to specialized PCD centers for diagnostic work-up. Compared to controls (n = 197), lobectomised patients had lower FVC z-scores (− 2.41 vs − 1.35, p = 0.0001) and FEV1 z-scores (− 2.79 vs − 1.99, p = 0.003) at their first post-lung resection assessment. After surgery, lung function continued to decline at a faster rate in lobectomised patients compared to controls (FVC z-score slope: − 0.037/year Vs − 0.009/year, p = 0.047 and FEV1 z-score slope: − 0.052/year Vs − 0.033/year, p = 0.235), although difference did not reach statistical significance for FEV1. Within cases, females and patients with multiple lobe resections had lower lung function. Conclusions Prevalence of lung resection in PCD varies widely between countries, is often performed before PCD diagnosis and overall is more frequent in patients with delayed diagnosis. After lung resection, compared to controls most lobectomised patients have poorer and continuing decline of lung function despite lung resection. Further studies benefiting from prospective data collection are needed to confirm these findings.

Cetuximab-based chemoimmunotherapy has been the standard of care for recurrent or metastatic squamous cell carcinoma of the head and neck (r/m SCCHN) for more than a decade.
