ObjectivesMedication adherence is emerging as a major public health challenge particularly in patients with depression. The aim of this study was to explore the usefulness of a pharmacist intervention to improve antidepressant medication adherence and disease severity in patients with major depressive disorder.MethodsThis prospective interventional study was conducted between April 2019 and March 2020 among 101 patients at the Federal Neuro-Psychiatric Hospital, Maiduguri, Nigeria. Consenting patients were randomised into usual care or intervention groups using a computer-generated list. Data were collected at baseline, 3 months and 6 months. Medication adherence and depression severity were assessed using the Medication Adherence Rating Scale and Beck Depression Inventory respectively.Key findingsAt baseline, both the usual care and intervention groups had low mean scores for medication adherence [5.22 (SD = 1.51) versus 5.46 (SD = 1.46)] and high mean scores for depression severity [24.16 (SD = 13.50) versus 27.07 (SD = 16.12)]. At 6 months, there was a significant difference (P < 0.001) between the mean medication adherence scores of 5.22 (SD = 1.90) and 9.15 (SD = 1.62), in the usual care and intervention groups respectively. A significant difference (P = 0.033) was also observed at 6 months between the mean depression severity scores of the usual care and intervention groups [21.40 (SD = 11.52) and 17.34 (SD = 6.96)]. Medication adherence (P < 0.001, Partial eta squared = 0.279) and depression severity (P < 0.001, Partial eta squared = 0.170) positively changed with time in the presence of the intervention.ConclusionsThe intervention significantly improved antidepressant medication adherence and disease severity in patients with major depressive disorder.

BackgroundDirect oral anticoagulants (DOACs) have revolutionised anticoagulant pharmacotherapy. However, DOAC-related medication incidents are known to be common.ObjectiveTo assess medication incidents associated with DOACs using an error theory and to analyse pharmacists’ contributions in minimising medication incidents in hospital in-patients.SettingA large University academic hospital in the West Midlands of England.MethodsMedication incident data from the incident reporting system (48-months period) and pharmacists’ interventions data from the prescribing system (26-month period) relating to hospital in-patients were extracted. Reason’s Accident Causation Model was used to identify potential causality of the incidents. Pharmacists’ intervention data were thematically analysed.Main outcome measure(a) Frequency, type and potential causality of DOAC-related incidents; (b) nature of pharmacists’ interventions.ResultsA total of 812 reports were included in the study (124 medication incidents and 688 intervention reports). Missing drug/omission was the most common incident type (26.6%, n = 33) followed by wrong drug (16.1%, n = 20) and wrong dose/strength (11.3%, n = 14). A high majority (89.5%, n = 111) of medication incidents were caused by active failures. Patient discharge without anticoagulation supply and failure to restart DOACs post procedure/scan were commonly recurring themes. Pharmacists’ interventions most frequently related to changes in pharmacological strategy, including drug or dose changes (38.1%, n = 262). Impaired renal function was the most common reason for dose adjustments.ConclusionPrescribers’ active failure rather than system errors (i.e. latent failures) contributed to the majority of DOAC-related incidents. Reinforcement of guideline adherence, prescriber education, harnessing pharmacists’ roles and mandating renal function information in prescriptions are likely to improve patient safety.

Uma das estratégias utilizadas para melhoria da segurança do paciente, principalmente na identificação e prevenção de eventos adversos relacionados a medicamentos, é a conciliação medicamentosa. A identificação de discrepâncias entre os medicamentos em uso pelo paciente antes da admissão hospitalar e os medicamentos prescritos durante a internação evitam danos que podem ser permanentes e promovem a segurança do paciente. O objetivo do estudo foi identificar, utilizando da conciliação medicamentosa, possíveis discrepâncias e interações medicamentosas em pacientes internados em unidade clínica de um hospital público do Sul do Brasil, por meio de um estudo transversal, de caráter descritivo e prospectivo, no qual os pacientes foram entrevistados sobre os medicamentos em uso antes da internação e comparando com o que foi prescrito. A maioria dos pacientes era homens, idosos e com doenças cardiovasculares como doença de base. Dos 50 entrevistados, 94% apresentaram ao menos uma discrepância. Dos 153 medicamentos discrepantes, 66,7% foram classificados como discrepâncias intencionais e 33,3% como discrepâncias não intencionais. Os medicamentos que atuam no sistema cardiovascular foram os mais frequentes, nas discrepâncias e nas possíveis interações medicamentosas. Os pacientes polimedicados foram os que apresentaram número maior de possíveis interações medicamentosas. A identificação dessas discrepâncias demonstrou a importância da realização da conciliação medicamentosa e da intervenção do farmacêutico na prevenção de eventos adversos relacionados a medicamentos.

PurposeCost-avoidance studies are common in pharmacy practice literature. This scoping review summarizes, critiques, and identifies current limitations of the methods that have been used to determine cost avoidance associated with pharmacists’ interventions in acute care settings.MethodsAn Embase and MEDLINE search was conducted to identify studies that estimated cost avoidance from pharmacist interventions in acute care settings. We included studies with human participants and articles published in English from July 2010 to January 2021, with the intent of summarizing the evidence most relevant to contemporary practice.ResultsThe database search retrieved 129 articles, of which 39 were included. Among these publications, less than half (18 of 39) mentioned whether the researchers assigned a probability for the occurrence of a harmful consequence in the absence of an intervention; thus, a 100% probability of a harmful consequence was assumed. Eleven of the 39 articles identified the specific harm that would occur in the absence of intervention. No clear methods of estimating cost avoidance could be identified for 7 studies. Among all 39 included articles, only 1 attributed both a probability to the potential harm and identified the cost specific to that harm.ConclusionCost-avoidance studies of pharmacists’ interventions in acute care settings over the last decade have common flaws and provide estimates that are likely to be inflated. There is a need for guidance on consistent methodology for such investigations for reporting of results and to confirm the validity of their economic implications.

Background Funding is a significant barrier to employing general practice pharmacists. Objective(s) To explore the feasibility of determining the cost-benefit of pharmacists in Australian general practice. Methods Two part-time pharmacists were employed by general practices in Canberra, Australia. Diaries of the pharmacists were analysed to determine time worked and participation in income-generating activities, including Government-funded programs: Asthma Cycle of Care, Home Medicine Reviews, and Health Care Assessments. Scenarios using different practice and business models were entered into value-cost models to determine the income generated by the pharmacists relative to their salary. Results Over 19 weeks, pharmacists A and B supported 47 and 23 Asthma Cycle of Care activities, generating income to the general practice of AU$4,700 and AU$2,300, respectively. The pharmacists spent 36.4 and 24.1 hours on activities usually conducted by general practitioners (GPs), allowing additional time for GP-patient consultations. Value-cost models determined AU$0.61 - AU$1.20 income generation by pharmacists per AU$1 salary. Conclusions It was feasible to determine the value-cost ratios of employing pharmacists in general practice using these methods. Future work should focus on developing a robust business model that includes health care system savings resulting from practice pharmacist interventions, determined from randomised controlled trials.

Objective: The objective of this study was to evaluate the effect of a pharmacist-led intervention on self-management practices among hypertensive-diabetic patients receiving care in a Nigeria tertiary hospital. Methods: The study adopted a prospective, longitudinal; single-blind, two-arm randomised controlled trial to implement a pharmacist-led educational intervention on hypertension management among patients in Federal Medical Centre Lokoja, Kogi State. All the patients who met the eligibility criteria and gave their written consent to participate in the study were recruited into the study randomized in the intervention group (IG) and Control (CG). Data was collected using Hypertension Self Care Activity Level Effects (H-SCALE) questionnaire. The retrieved questionnaires were first coded into Microsoft Excel (2014) for cleaning of errors, after which the data was exported into the Statistical Product and Services Solutions (SPSS for windows, Version 16.0. SPSS Inc. 2007, Chicago, USA) software. Descriptive statistics such as frequencies, percentages and mean scores were used to summarise the data. All responses were first presented as frequencies and percentages. Chi-square was used to determine the correlation between socio-demographic and patients’ clinical characteristics. Independent sample t-test and paired sample t-test were used to compare differences between and within groups. Results: At baseline, more patients in the control group were adherent to their medications 16 (11.5) and had low salt diets 47 (33.8) than patients in the intervention group. However, more patients in the intervention group were non-smokers 127 (88.8) and engaged more in physical activity 38 (26.6) than patients in the control group. However, more of the patients in the control group were adherent to weight management practices 38 (27.3) than patients in the intervention group 36 (25.2), at baseline. All the patients in both study groups reported to have taken alcohol in the past seven days. It can be seen that, patients in the intervention group at endpoint, differed positively from the control group at baseline in adherence to medication, physical activity, reducing alcohol consumption and smoking cessation. Background: Medication errors are of primary concern in hospitals of Pakistan but only few are reported. Prevalence and type of medication errors are essential for estimating the appropriateness of therapy. Pharmacist is key in improving therapy effectiveness and decreasing medication errors. Objective: To assess the prevalence, type of medication errors and pharmacist interventions. Methodology: An interventional study on prescriptions of inpatients was conducted at Liaquat University of Medical and Health Sciences (LUMHS) Hyderabad. Five hundred and fifteen prescriptions were collected randomly from medicine, gynecology and obstetrics and urology department during one year from 01-09-2017 to 31-08-2018. Medications errors were identified by Pharmacist by comparing the prescriptions with standard of British National Formulary (BNF) (edition 73) and informing physician for resolving medication errors. Data was analyzed with statistical package for social sciences version 25.0. Results: Out of 515 inpatients prescriptions, male patients were 248 (48.2%) and female were 267 (51.8%) with male to female ratio of 1:1.08 and mean age of 37.7 ± 11.6 (18-60) years. 427 medication errors were reported in 303 (59.4%) prescriptions including; omission error 64 (15.0%), unauthorized drug error 41 (9.6%), wrong dose error 67 (15.7%), wrong time error 74 (17.3%), wrong dosage form error 27 (6.3%), wrong route error 23 (5.4%), wrong drug preparation 44 (10.3%), wrong rate error 56 (13.1%) and incorrect administration technique 31 (7.3%). 172 (40.3%) interventions of pharmacists were accepted by physicians. Conclusion: Rate of medication errors in inpatients prescriptions are high with possible errors of dose, time, omission, rate, drug preparation and others. Pharmacist effectively intervening the prescriptions and playing active role in decreasing medication errors.

日本病院薬剤師会では，薬剤師が薬学的ケアを実践して薬物療法に関与し患者の不利益を回避あるいは軽減した事例をプレアボイドと称し報告を収集している．岡山県は，全国第1位の報告数であり，プレアボイド先進県と言える．そこで，当院薬剤部においても報告数増加を目的とした取り組みを行った．2018年度以前は，薬剤部に設置された指定の端末1 台でプレアボイドを入力していたが，2019年度以降は，院内の電子カルテ端末を利用して各部署・病棟での入力を可能とした．また，薬剤部内での勉強会や， 1 ヶ月毎に件数や薬剤師名の報告を行った．その結果，前年度と比べ，2019年度のプレアボイド報告数は178件増加し，報告した薬剤師数は12名増加した．電子カルテ端末を用いた簡便なプレアボイド入力や，報告への抵抗感の減少，積極性向上によるものと考えられた．今後も，プレアボイドを通じて，薬物療法の有効性，安全性の向上に加えて，医師等の負担軽減に貢献していく必要がある．

Aim: We aimed to evaluate the impact of pharmaceutical service intervention on medication adherence and patient-reported outcomes among patients diagnosed with depression in a private psychiatric hospital in Nepal. Methods: A single-center, open trial with a parallel design was conducted among 18 to 65 years aged patients, diagnosed with depression and under antidepressant medication(s) for ≥2 months. Patients were randomised into either the intervention or control group. The control group (n = 98) received the usual care, while the intervention group (n = 98) received a pharmaceutical service intervention. The two groups were compared using the Mann-Whitney U test, independent t-test, or chi-square test at 2 and 4 months for changes in medication adherence and patient-reported [severity of depression and health-related quality of life (HRQoL)] outcomes. Results: One hundred ninety adult patients were enrolled in the study. At baseline, there were no significant differences in any of the outcome measures between the intervention and control groups. Purpose Chronic myeloid leukaemia (CML) patients on long-term tyrosine kinase inhibitor (TKI) therapy are susceptible to drug-related problems (DRPs). This study aimed to evaluate the acceptability and outcomes of pharmacist-led interventions on DRPs encountered by CML patients. Methods This study included participants from the intervention arm of a randomised controlled trial which was conducted to evaluate the effects of pharmacist-led interventions on CML patients treated with TKIs. Participants were recruited and followed up in the haematology clinics of two hospitals in Malaysia from March 2017 to January 2019. A pharmacist identified DRPs and helped to resolve them. Patients were followed-up for six months, and their DRPs were assessed based on the Pharmaceutical Care Network Europe Classification for DRP v7.0. The identified DRPs, the pharmacist’s interventions, and the acceptance and outcomes of the interventions were recorded. A Poisson multivariable regression model was used to analyse factors associated with the number of identified DRPs per participant. Results A total of 198 DRPs were identified from 65 CML patients. The median number of DRPs per participants was 3 (interquartile range: 2, 4). Most participants (97%) had at least one DRP, which included adverse drug events (45.5%), treatment ineffectiveness (31.5%) and patients’ treatment concerns or dissatisfaction (23%). The 228 causes of DRPs identified comprised the following: lack of disease or treatment information, or outcome monitoring (47.8%), inappropriate drug use processes (23.2%), inappropriate patient behaviour (19.9%), suboptimal drug selection (6.1%), suboptimal dose selection (2.6%) and logistic issues in dispensing (0.4%). The number of concomitant medications was significantly associated with the number of DRPs (adjusted Odds Ratio: 1.100; 95% CI: 1.005, 1.205; p = 0.040). Overall, 233 interventions were made. These included providing patient education on disease states or TKI-related side effects (75.1%) and recommending appropriate instructions for taking medications (7.7%). Of the 233 interventions, 94.4% were accepted and 83.7% were implemented by the prescriber or patient. A total of 154 DRPs (77.3%) were resolved. Conclusions The pharmacist-led interventions among CML patients managed to identify various DRPs, were well accepted by both TKI prescribers and patients, and had a high success rate of resolving the DRPs.

Background The number of medication related hospital admissions and readmissions are increasing over the years due to the ageing population. Medication related hospital admissions and readmissions lead to decreased quality of life and high healthcare costs. Aim of the review To assess what is currently known about medication related hospital admissions, medication related hospital readmissions, their risk factors, and possible interventions which reduce medication related hospital readmissions. Method We searched PubMed for articles about the topic medication related hospital admissions and readmissions. Overall 54 studies were selected for the overview of literature. Results Between the different selected studies there was much heterogeneity in definitions for medication related admission and readmissions, in study population and the way studies were performed. Multiple risk factors are found in the studies for example: polypharmacy, comorbidities, therapy non adherence, cognitive impairment, depending living situation, high risk medications and higher age. Different interventions are studied to reduce the number of medication related readmission, some of these interventions may reduce the readmissions like the participation of a pharmacist, education programmes and transition-of-care interventions and the use of digital assistance in the form of Clinical Decision Support Systems. However the methods and the results of these interventions show heterogeneity in the different researches. Conclusion There is much heterogeneity in incidence and definitions for both medication related hospital admissions and readmissions. Some risk factors are known for medication related admissions and readmissions such as polypharmacy, older age and additional diseases. Known interventions that could possibly lead to a decrease in medication related hospital readmissions are spare being the involvement of a pharmacist, education programs and transition-care interventions the most mentioned ones although controversial results have been reported. More research is needed to gather more information on this topic.

Hypothyroidism is a common endocrine disorder with a prevalence of 1-2% in the world. The symptoms of hypothyroidism includes, cardiac dysfunction, dyspnea and reduced exercise tolerance, overt cardiovascular diseases, and diabetes mellitus. The aim of the study was to access Knowledge, awareness and, the practice of patient with primary hypothyroidism among the patient attending at endocrinology care center. In this study, most of the female patients were seen hypothyroid than males. Out of a total of 164 respondents, 5.5% were age group of 15-24, 52.4% were age group 25-44, 39% were from the age group 45-64 and 3% were above 65. Among the studied population, 93.9% were male and e other ware female. The majority of the respondent was in the age group of 25-44 years (52.4%). Major differences were seen after the intervention regarding the Knowledge about thyroid and hypothyroidism. Before intervention respondent was more unknown towards the test required for treatment and similarly after the intervention improved the understanding level of patients towards hypothyroidism. Hence this study suggested advantages of intervention by pharmacist in improving knowledge, attitude and practice of patients towards management of disease.

To assess the positive impact of pharmacist interventions via an educational video on patients’ knowledge of and satisfaction with warfarin therapy . This study used a prospective pre-test/post-test design that enrolled 91 patients from an anticoagulant clinic at King Khaled University Hospital in Riyadh, Saudi Arabia. All patients completed the Anticoagulation Knowledge Assessment (AKA) and Anti-Clot Treatment Satisfaction (ACTS) scales. Afterwards, the patients watched a 10-minute educational video containing basic information about warfarin and were given relevant informative booklets. The patients were reassessed after a mean follow-up period of approximately 52 days. In total, 85 patients completed the study. The impact of the intervention on patient knowledge was highly significant (mean difference=17.7%, 95% CI = 21.75-13.58, P.

Objective This study examined the incidence of drug-related problems (DRPs) in different inpatient departments along with the medical team response to pharmacist's action in addressing DRPs at Jazan Hospital, Saudi Arabia. Patients and methods This retrospective study was conducted among inpatients at Ministry of Health hospital in Jazan, a region in southwestern Saudi Arabia. We collected data for a 2-year period (from 2016 to 2017). For any detected DRP of the ordered medications for dispensing, the inpatient pharmacist is sending report for that particular DRP with recommendation to the medical team which in turn might accept or reject such recommendation. Serious drug-drug interactions, as part of DRP, were assessed by utilizing three different online DDI checkers, including Lexi-Comp, Medscape, and Drugs.com. Results The most common type of DRP was serious drug-drug interactions (49%). Over the study period, most incidences of DRPs were decreased. Of the DRPs in 2016 and 2017, antibiotics were the most commonly involved (51% and 69.5%) of cases, respectively, followed by proton pump inhibitors (25.3% and 14.1%) and statins (12.9% and 9.4%). Interestingly, of the 241 interventions for DRPs in 2016, 199 (82.5%) were accepted, resulting in a change in drug therapy (p=0.006). In 2017, 90 (70.3%) interventions out of 128 were accepted by the physician and the drug therapy changed (p=0.029). Conclusion Pharmacist interventions appear to decrease the incidence of DRPs, which emphasize the importance of an optimal pharmaceutical care plan for clinical care settings.

The clinical role of pharmacists in a correctional facility has not been fully described. We report the proportion of patients assessed by a pharmacist within 48 hours of admission to a large correctional facility. Of those assessed, the frequency and type of pharmacist interventions were described. A retrospective chart review was conducted for patients admitted to the Edmonton Remand Center (ERC) from September to November 2017. From 1,500 patients, 518 (34.5%) were assessed by a pharmacist, and 511 (98.6%) of those received one or more pharmacist interventions. Interventions were most commonly health care provider interactions (89.0%) and drug therapy interventions (76.1%). ERC pharmacists assessed a sizable proportion of admitted patients, with most receiving at least one pharmacist intervention.

International audience; During the dispensing process of medical orders (MOs), community pharmacists (CPs) can manage drug-related problems (DRPs) by performing pharmacist interventions (PIs). There is little evidence that the PI rate is higher with MOs from hospitals (MOHs) than ambulatory (MOAs) settings, and their impact on the patient and community pharmacy is unknown. The primary objective of this study was to compare the MOH and MOA PI rates. The secondary objective was to describe PIs and their clinical and organizational impacts on patient and community pharmacy workflow. A total of 120 CPs participated in a prospective study. Each CP included 10 MOH and 10 MOA between January and June 2020. DRP and PI description and clinical and organizational impacts between MOH and MOA were assessed and compared. We analyzed 2325 MOs. PIs were significantly more frequent in MOH than in MOA (9.7% versus 4.7%; p < 0.001). The most reported PI was the difficulty of contacting hospital prescribers (n = 45; 52.2%). MOHs were associated with a longer dispensing process time and a greater impact on patient pathway and community pharmacy workflow than MOAs. Lack of communication between hospital and primary care settings partly explains the results. Implementation of clinical pharmacy activities at patient discharge could alleviate these impacts.

Background: Human immunodeficiency virus (HIV) providers are treating more comorbid conditions with additional pharmacologic agents, resulting in patients with HIV being disproportionately at risk for clinically significant drug–drug interactions (CSDDIs). There is a potential to overlook these interactions and ultimately place patients at risk for drug toxicity, resistance, and virologic failure. Objective: To assess the burden of CSDDIs among patients receiving antiretroviral therapy (ART) within 24 hours of admission and to evaluate the effect of a clinical pharmacist operating through an antiretroviral stewardship (ARVSP) program in identifying and correcting potential drug interactions. Methods: Adult HIV-positive patients receiving ART who were admitted to The Brooklyn Hospital Center from November 2010 through January 2012 were included in the analysis. Drug interactions were categorized according to time frame (ie, within 24 hours of admission vs after 24 hours of admission) and type (ie, contraindicated combinations, dosage modifications, and frequency alterations). The Liverpool HIV drug reference, Micromedex drug database, and the Department of Health and Human Services Guidelines were used as comprehensive tools for identification of antiretroviral drug errors. Results: Eighty-four CSDDIs were identified from 252 admissions among 158 patients receiving ART during the study period. Of the identified CSDDIs, 61 (73%) occurred within 24 hours of admission and 23 (27%) later in the hospital course. Forty-eight drug interactions (57%) represented contraindicated drug combinations. Protease inhibitor–based regimens were associated with the highest percentage of CSDDIs (98%). Of all CSDDIs, the most common interacting drug class was acid-suppressive therapy (63%). Clinical pharmacists identified and intervened in 80% of the CSDDIs that occurred on patient admission with all interventions accepted. Conclusions: CSDDIs are common among patients receiving ART at the time of admission and throughout the hospital course. Interventions including medication review by clinical pharmacists are critical in the prevention of CSDDIs on admission.

Warfarin therapy can significantly affect patients’ quality of life and cause therapy discontinuation. This study aimed to investigate the effect of the pharmacists’ interventions on the health-related quality of life (HRQoL) in older rural patients on warfarin therapy. Eligible older patients from rural area of Croatian province Slavonia were randomized into the intervention and control groups and followed for six months. Repeated education and a follow-up plan were provided to the participants in the intervention group, and if needed, the pharmacist intervened to optimize warfarin therapy. Secondary analysis on HRQoL data are presented here. Main outcome measure was Duke anticoagulation satisfactions scale questionnaire score. In total, 131 participants finished the study (median age 73 years; 51.1% male). Participants in the intervention group scored significantly lower (median being 86.5 and 66.0 in the control and intervention groups, respectively; p 2 = 65.5%, P Clinicaltrials.gov (ID: NCT03212898), 11/07/2017, retrospectively registered.

Background: Despite ample evidence of benefit, adherence to secondary prevention medication therapy after acute coronary syndrome (ACS) is often suboptimal. Hospital pharmacists are uniquely positioned to improve adherence by providing medication education at discharge. Objective: To determine whether a standardized counselling intervention at hospital discharge significantly improved patients’ adherence to cardiovascular medications following ACS. Methods: This single-centre, prospective, nonrandomized comparative study enrolled patients with a primary diagnosis of ACS (January 2014 to July 2015). Patients who received standardized discharge counselling from a clinical pharmacist were compared with patients who did not receive counselling. At 30 days and 1 year after discharge, follow-up patient surveys were conducted and community pharmacy refill data were obtained. Adherence was assessed using pharmacy refill data and patient self-reporting for 5 targeted medications: acetylsalicylic acid, P2Y purinoceptor 12 (P2Y12) inhibitors, angiotensin-converting enzyme inhibitors or angiotensin II receptor blockers, β-blockers, and statins. Thirty-day and 1-year medication utilization, cardiovascular readmission rates, and all-cause mortality were also assessed. Results: Of the 259 patients enrolled, 88 (34.0%) received discharge counselling. Medication data were obtained for 253 patients (97.7%) at 30 days and 242 patients (93.4%) at 1 year. At 1 year after discharge, there were no statistically significant differences between patients who did and did not receive counselling in terms of rates of nonadherence (11.9% versus 18.4%, p = 0.19), cardiovascular readmission (17.6% versus 22.3%, p = 0.42), and all-cause mortality (3.4% versus 4.2%, p > 0.99). Overall medication nonadherence was 2.8% (7/253) at 30 days and 16.1% (39/242) at 1 year. Conclusions: Discharge medication counselling provided by hospital pharmacists after ACS was not associated with significantly better medication adherence at 1 year. Higher-quality evidence is needed to determine the most effective and practical interventions to ensure that patients adhere to their medication regimens and achieve positive outcomes after ACS. RÉSUMÉ Contexte : Malgré l’abondance de preuves démontrant ses avantages, l’adhésion à la pharmacothérapie de prévention secondaire après les syndromes coronariens aigus (SCA) est souvent « sous-optimale ». Les pharmaciens d’hôpitaux occupent une place unique pour améliorer l’adhésion en expliquant au patient l’usage des médicaments au moment du congé hospitalier. Objectif : Déterminer si une consultation standardisée au moment du congé hospitalier améliore significativement ou non l’adhésion à la pharmacothérapie cardiovasculaire après les SCA. Méthodes : Des patients ayant reçu un diagnostic primaire de SCA (de janvier 2014 à juillet 2015) ont été inscrits pour participer à cette étude comparative unicentrique prospective et non randomisée. Ceux ayant bénéficié d’une consultation standardisée par un pharmacien clinicien au moment du congé ont été comparés aux patients qui n’en n’avaient pas reçu. Trente jours et un an après le congé, des enquêtes de suivi du patient ont été menées et les données de renouvellement d’ordonnance des pharmacies communautaires ont été recueillies. L’adhésion a été évaluée à l’aide des données de renouvellement d’ordonnance et celles rapportées par le patient pour cinq médicaments ciblés : l’acide acétylsalicylique, les inhibiteurs P2Y purinoceptor 12 (P2Y12), les inhibiteurs de l’enzyme de conversion de l’angiotensine ou les antagonistes des récepteurs de l’angiotensine II, les antagonistes β et les statines. L’utilisation des médicaments à 30 jours et à un an, le taux de réadmission en raison d’un trouble cardiovasculaire et le taux de mortalité toutes causes confondues ont également fait l’objet d’une évaluation. Résultats : Sur les 259 patients inscrits, 88 (34 %) ont bénéficié d’une consultation au moment du congé. Des données concernant la médication de 253 patients ont été obtenues (97,7 %) à 30 jours et pour 242 patients (93,4 %) à un an. Un an après le congé, aucune différence statistique significative n’a été observée entre les patients ayant reçu ou non une consultation concernant la non-adhésion (11,9 % contre 18,4 %, p = 0,19), la réadmission en raison d’un trouble cardiovasculaire (17,6 % contre 22,3 %, p = 0,42), et le taux de mortalité toutes causes confondues (3,4 % contre 4,2 %, p > 0,99). La non-adhésion aux médicaments de manière générale se montait à 2,8 % (7/253) à 30 jours et à 16,1 % (39/242) à un an. Conclusions : La consultation concernant la médication donnée par les pharmaciens d’hôpitaux au moment du congé après les SCA n’était pas associée à un meilleur suivi de la médication un an plus tard. Des données probantes de meilleure qualité sont nécessaires pour déterminer les interventions les plus efficaces et pratiques pour que les patients adhèrent à leur régime médicamenteux et qu’ils obtiennent des résultats positifs après les SCA.

Objective: Pharmacists are ideally positioned to promote optimal use and assess the adherence of prescription medications in patients with chronic diseases. This systematic review of randomized controlled trials (RCTs) ascertains the effectiveness of pharmacist interventions to improve blood pressure (BP) control and medication adherence in patients with hypertension. Data Sources: Electronic databases were searched for studies in English published between January 1, 1995, and January 31, 2018. Eligibility criteria included patients with a diagnosis of hypertension tested with RCT where pharmacist-led interventions occurred. The primary outcome was BP control and the secondary outcome was adherence to hypertensive medication. Quality of studies was assessed using the Cochrane Risk of Bias Tool. Data Synthesis: Thirty-five studies met the inclusion–exclusion criteria and were included in this study. Major themes included pharmacist-led interventions with medication counseling, patient education, distribution of interventions materials, individualized care plans, and check-in meetings. The vast majority of trials (n = 29) showed statistically significant improvement in BP in the intervention groups at follow-up. Of the 35 studies, most (n = 20) assessed medication adherence; several trials (n = 9) showed statistically significant improvement in medication adherence in patients receiving additional pharmaceutical care services. None of the RCTs was considered to have a high risk of bias. Conclusion: Pharmacist-led interventions improved BP control and medication adherence through education, counseling, or a combination of both. Future research is needed to determine the most effective pharmacists’ interventions in managing hypertension and to assess the cost-effectiveness of these interventions.

Introduction Head and neck cancers (HNC) are a complex and heterogeneous group of cancers, often necessitating a multidisciplinary approach across the care continuum. Oncology pharmacists are uniquely qualified to play a vital role on a multidisciplinary team and provide specialized care to optimize medication therapy. Methods This was a retrospective chart review evaluating the role of a board-certified oncology pharmacist in the head and neck oncology clinic at an academic, comprehensive cancer center from April 2017 through March 2018. The primary objective of the study was to describe the types of interventions made by the oncology pharmacists. Secondary objectives included quantifying time spent on patient education and number of prescriptions sent to pharmacies. Results The pharmacist had 873 encounters with 151 patients, resulting in 2080 interventions. Approximately 57% of the interventions were performed in the clinic. Patient education (58%), facilitation of new prescriptions or refill requests (49.9%), and supportive care management (32.6%) were the most frequent interventions. The oncology pharmacist spent 154.1 h on patient education and sent 811 prescriptions to pharmacies, with 63.6% of prescriptions sent to the institution’s cancer center pharmacy. Conclusion The incorporation of an oncology pharmacist in the HNC team optimized patient care through comprehensive and timely interventions across the care continuum. Our study is the first to highlight the vital role oncology pharmacists have in improving the overall quality of care of HNC patients. Future directions include exploring the impact of oncology pharmacist interventions on select Quality Oncology Practice Initiative measures by the American Society of Clinical Oncology.

Background: Guidelines assisted appropriate use of prophylactic antibiotics can lower the prevalence of surgical site infections (SSIs). The present study was conducted to evaluate the impact and cost-benefit value of pharmacist’s educational intervention for antibiotic use in post-surgical prophylaxis. Methods: A prospective quasi experimental study was conducted by enrolling 450 patients from tertiary care hospital of Lahore, Pakistan, 225 patients in each, control and intervention, arm using non-random convenient sampling. The study parameters included antibiotic indication, choice, dose, frequency, duration and associated costs. This study is registered with Chinese Clinical Trial Registry # ChiCTR-OON-17013246. Results and conclusion: After educational intervention, in post-intervention arm, total compliance in terms of correct antibiotic choice, dose, frequency and duration increased from 1.3% to 12.4%. The rate of inappropriate antibiotic choice did not change significantly. After intervention only metronidazole utilization decreased (16%) significantly (p = 0.011). Significant reductions were observed in mean duration of antibiotic prophylaxis (17%, p = 0.003), average number of prescribed antibiotics (9.1%, p = 0.014) and average antibiotic cost (25.7%, p = 0.03), with reduction in mean hospitalization cost (p = 0.003) and length of stay (p = 0.023). Educational intervention was significantly associated (OR; 2.4, p = 0.005) with appropriate antibiotic prophylaxis. The benefit of pharmacist intervention, mean antibiotic cost savings to mean cost of pharmacist time, was 4.8:1. Thus, the educational intervention resulted in significant reductions in the duration and average number of antibiotic use having considerable effect on therapy and hospitalization cost.
