Higher education is starting to embrace its role in promoting student wellbeing and life skills, especially given the concerning levels of poor mental health and uncertainties in the future job market. Yet, many of the published studies evaluating positive educational teaching methods thus far are limited to interventions delivered to small student cohorts and/or imbedded within elective wellbeing courses, and are focussed on developed Western countries. This study addressed this gap by investigating the effectiveness of an institution-wide compulsory course informed by the principles of Seligman’s Wellbeing Theory. The course was delivered at a British university in a developing country in Southeast Asia. It purposefully sought to nurture growth-oriented outcomes (including self-awareness, positive emotions, and personal effectiveness) and was taken by an entire cohort of year one undergraduate students. We tested the effectiveness of the curriculum content and staff coaching style in achieving life skills, and evaluated how these perceptions influenced students’ subjective wellbeing. A convergent mixed-methods design was used with 350 survey respondents and 11 interviewees. Perceived life skills scores showed a 2.5% improvement at the end of the course. Partial Least Squares Structural Equation Modelling tested the predicted relationships between variables. All relationships were statistically significant, but the influence of course design and educators’ style on life skills acquisition (50.8% of the variance) was moderate, while the effect on subjective happiness and life satisfaction (4–5% of the variance) was very weak. Qualitative data indicated that while quantifiable benefits to wellbeing might not be immediate, students did anticipate longer-term benefits for happiness and life satisfaction. This finding suggests that such a novel educational approach is well-received by Asian students and may sow the seeds for future benefit by positively impacting on their skills, behaviours, attitudes, and values. To achieve optimal flourishing at university, we recommend exploring teaching practises that combine positive education with coaching psychology practises.

Although therapeutically efficacious, ipilimumab can exhibit dose-limiting toxicity that prevents maximal efficacious clinical outcomes and can lead to discontinuation of treatment. We hypothesized that an acidic pH-selective ipilimumab (pH Ipi), which preferentially and reversibly targets the acidic tumor microenvironment over the neutral periphery, may have a more favorable therapeutic index. While ipilimumab has pH-independent CTLA-4 affinity, pH Ipi variants have been engineered to have up to 50-fold enhanced affinity to CTLA-4 at pH 6.0 compared to pH 7.4. In hCTLA-4 knock-in mice, these variants have maintained anti-tumor activity and reduced peripheral activation, a surrogate marker for toxicity. pH-sensitive therapeutic antibodies may be a differentiating paradigm and a novel modality for enhanced tumor targeting and improved safety profiles.

Background: We recently reported an objective response rate of 36.4% in a phase I trial of RIN in MSS mCRC patients without liver metastases, while no response was noted in patients with liver metastases. Here we characterized kinetics in local and systemic immune responses associated with clinical outcomes, liver metastasis, and immune related toxicity during RIN treatment. Methods: Patients with chemotherapy-resistant MSS mCRC received RIN at (R) at 80 mg QD x 21 days in 4-week(w) cycle, (I) at 1mg/kg Q6w and (N) at 240 mg Q 2w (n=29). Needle biopsies were collected at baseline (n=8) and 4 weeks (n=6) after treatment. Blood samples were collected at baseline (n=29), 4 weeks (n=29) and 8 weeks (n=25) after treatment. Alterations in the tumor microenvironment (TME) were characterized by RNA sequencing. We analyzed the immune cell composition and activities in the blood, including 90 subpopulations of leukocytes and 29 activation/suppression/proliferation markers, with high-dimensional spectral flow cytometry. Results: The clinical responses are positively correlated with the induction of local anti-tumor immunity. Patients with better clinical outcomes exhibited higher levels of lymphocyte infiltration, chemokine production, and antigen-presentation potential in the local TME following one cycle of RIN treatment. The duration of progression-free survival (PFS) was correlated with a higher baseline STING signal in the TME. In the peripheral blood, the responders have more circulating CD4 T cells, B cells, and cDC1, while expressing lower immune activation markers, especially in T cells with type 1 immunity at baseline. At 4 weeks, immune activation and proliferation markers were higher in responders than non-responders. Interestingly, immune effectors (non-Tregs, CD45RA− CD8 T cells, NKT cells, and M1 monocytes) in the blood were proportionally reduced in responders at 4 weeks, coincident with their enrichment in the local TME. We also compared peripheral immune signatures between patients with and without liver metastases, liver metastases (n=7) were associated with a significantly lower ratio of CD4/CD8 T cells in the blood, a phenomenon of immunosenescence, both at baseline and after RIN treatment. Lastly, the occurrence of immune-related toxicities was closely correlated with the induction of immune activities in both acellular and cellular components in the circulation. Conclusions: Clinical benefit of RIN was correlated to more robust immune activation of the TME and peripheral blood following one cycle of treatment. A higher baseline STING signal in the TME was associated with longer PFS. Liver metastasis was associated with a lower ratio of peripheral CD4/CD8 T cells which may contribute to the resistance to immunotherapy in those patients. Magnetic fields have been widely used to control solidification processes. Here, high-speed synchrotron X-ray tomography was used to study the effect of magnetic fields on solidification. We investigated vertically upward directional solidification of an Al-Si-Cu based W319 alloy without and with a transverse magnetic field of 0.5 T while the sample was rotating. The results revealed the strong effect of a magnetic field on both the primary α-Al phase and secondary β-Al5FeSi intermetallic compounds (IMCs). Without the magnetic field, coarse primary α-Al dendrites were observed with a large macro-segregation zone. When a magnetic field is imposed, much finer dendrites with smaller primary arm spacing were obtained, while macro-segregation was almost eliminated. Segregated solutes were pushed out of the fine dendrites and piled up slightly above the solid/liquid interface, leading to a gradient distribution of the secondary β-IMCs. This work demonstrates that rotating the sample under a transversal magnetic field is a simple yet effective method to homogenise the temperature and composition distributions, which can be used to control the primary phase and the distribution of iron-rich intermetallics during solidification.

BackgroundThe immune suppressive tumor microenvironment (TME) that inhibits T cell infiltration, survival, and anti-tumor activity has posed a major challenge for developing effective immunotherapies for solid tumors. Chimeric antigen receptor (CAR)-engineered T cell therapy has shown unprecedented clinical response in treating patients with hematological malignancies, and intense investigation is underway to achieve similar responses with solid tumors. Immunologically cold tumors, including prostate cancers, are often infiltrated with abundant tumor-associated macrophages (TAMs), and infiltration of CD163+ M2 macrophages correlates with tumor progression and poor responses to immunotherapy. However, the impact of TAMs on CAR T cell activity alone and in combination with TME immunomodulators is unclear.MethodsTo model this in vitro, we utilized a novel co-culture system with tumor cells, CAR T cells, and polarized M1 or M2 macrophages from CD14+ PBMCs collected from healthy human donors. Tumor cell killing, T cell activation and proliferation, and macrophage phenotypes were evaluated by flow cytometry, cytokine production, RNA sequencing, and functional blockade of signaling pathways using antibodies and small molecule inhibitors. We also evaluated the TME in humanized mice following CAR T cell therapy for validation of our in vitro findings.ResultsWe observed inhibition of CAR T cell activity with the presence of M2 macrophages, but not M1 macrophages, coinciding with a robust induction of PD-L1 in M2 macrophages. We observed similar PD-L1 expression in TAMs following CAR T cell therapy in the TME of humanized mice. PD-L1, but not PD-1, blockade in combination with CAR T cell therapy altered phenotypes to more M1-like subsets and led to loss of CD163+ M2 macrophages via IFNγ signaling, resulting in improved anti-tumor activity of CAR T cells.ConclusionThis study reveals an alternative mechanism by which the combination of CAR T cells and immune checkpoint blockade modulates the immune landscape of solid tumors to enhance therapeutic efficacy of CAR T cells.

The aim of this study is to systematically review outcomes related to treatment success, mortality, and adverse events of endoscopic management in patients with sterile walled-off pancreatic necrosis.We reviewed studies published from 2008 to 2018 from Medline and Embase that evaluated the endoscopic treatment of necrotizing pancreatitis. The primary outcome was success of treatment in resolving the collection. Secondary outcomes included length of hospitalization, mortality rate, and adverse events.Five studies were included, which entailed a total of 280 patients with a mean age of 51.8 years. The primary indication for endoscopic treatment was symptomatic walled-off pancreatic necrosis. Four studies used endoscopic transmural drainage, one of them combining percutaneous drainage and 1 study performed transpapillary drainage. The pooled treatment success was 94.3% with a mean time to resolution of 77.8 days. The mean length of stay was 16.3 days, and mortality rate was 1.3%. The overall adverse event rate was 24.6%, with bleeding the most common adverse event (11%), followed by pancreatic fistula formation (3.4%) and perforation (2.7%).Although endoscopic management of sterile pancreatic necrosis has a high rate of treatment success, there is a relatively high rate of adverse events, bleeding being the most common.

This phase 2 trial evaluated PET-adapted nivolumab alone or in combination with ifosfamide, carboplatin, and etoposide (NICE) as first salvage therapy and bridge to autologous hematopoietic cell transplantation (AHCT) in relapsed/refractory (RR) classical Hodgkin lymphoma (cHL). Patients with RR cHL received 240 mg nivolumab every 2 weeks for up to 6 cycles (C). Patients in complete response (CR) after C6 proceeded to AHCT, whereas patients with progressive disease at any point or not in CR after C6 received NICE for 2 cycles. The primary endpoint was CR rate per the 2014 Lugano classification at completion of protocol therapy. Forty-three patients were evaluable for toxicity; 42 were evaluable for response. Thirty-four patients received nivolumab alone, and 9 patients received nivolumab+NICE. No unexpected toxicities were observed after nivolumab or NICE. After nivolumab, the overall response rate (ORR) was 81%, and the CR rate was 71%. Among 9 patients who received NICE, all responded, with 8 (89%) achieving CR. At the end of protocol therapy, the ORR and CR rates were 93% and 91%. Thirty-three patients were bridged directly to AHCT, including 26 after Nivo alone. The 2-year progression-free survival (PFS) and overall survival in all treated patients (n = 43) were 72% and 95%, respectively. Among 33 patients who bridged directly to AHCT, the 2-year PFS was 94% (95% CI: 78-98). PET-adapted sequential salvage therapy with nivolumab/nivolumab+NICE was well tolerated and effective, resulting in a high CR rate and bridging most patients to AHCT without chemotherapy. We estimate the health impact, over the period 2000-2050, of introducing into the US the nicotine pouch ZYN. ZYN’s toxicant profile and method of use is like that for Swedish snus, a product with known health effects much less than smoking. MethodsOur modelling approach is similar to others developed for estimating potential effects of new tobacco products. It starts with a simulated cohort of 100,000 individuals in the year 2000 subdivided by age, sex, and smoking status (including years since quitting). They are followed annually accounting for births, net immigrations, deaths and product use changes, with follow-up carried out in the Base Case (ZYN not introduced) and Modified Case (ZYN introduced). Using informed assumptions about initiation, quitting and switching rates, distributions of the population over time are then constructed for each Case, which are used to estimate product-related mortality based on assumptions about the relative risk according to product use. ResultsWhereas in both Base and Modified Cases, the prevalence of any current product use is predicted to decline from about 22% to 10% during follow-up, in the Modified Case about 25% of current users use ZYN by 2050, about a quarter being dual users and the rest ZYN-only users. Over the 50 years, deaths at ages 35-84 from product use among the 100,000 are estimated as 249 less in the Modified than the Base Case, equivalent to about 700,000 less deaths in the whole US. Sensitivity analyses varying individual parameter values confirm the benefits of switching to ZYN, which increase as either the switching rate to ZYN increases or the initiation rate of ZYN relative to smoking increases. Even assuming the reduction in excess mortality risk using ZYN use is 20% of that from smoking rather than the 3.5% assumed in the main analyses, the reduction in product-related deaths would still be 213, or about 600,000 in the US. ConclusionsAlthough such model-based estimates are subject to uncertainties, the results suggest that introducing ZYN could substantially reduce smoking-attributable deaths.

Quantifying heterogeneity in Alzheimer’s disease (AD) risk is critical for individualized care and management. Recent attempts to assess AD heterogeneity have used structural (magnetic resonance imaging (MRI)-based) or functional (Aβ or tau) imaging, which focused on generating quartets of atrophy patterns and protein spreading, respectively. Here we present a computational framework that facilitated the identification of subtypes based on their risk of progression to AD. We used cerebrospinal fluid (CSF) measures of Aβ from the Alzheimer’s Disease Neuroimaging Initiative (ADNI) (n=544, discovery cohort) as well as the National Alzheimer’s Coordinating Center (NACC) (n=508, validation cohort), and risk-stratified individuals with mild cognitive impairment (MCI) into quartiles (high-risk (H), intermediate-high risk (IH), intermediate-low risk (IL), and low-risk (L)). Patients were divided into subgroups utilizing patterns of brain atrophy found in each of these risk-stratified quartiles. We found H subjects to have a greater risk of AD progression compared to the other subtypes at 2- and 4-years in both the discovery and validation cohorts (ADNI: H subtype versus all others, p < 0.05 at 2 and 4 years; NACC: H vs. IL and LR at 2 years, p < 0.05, and a trend toward higher risk vs. IH, and p < 0.05 vs. IH, and L risk groups at 48 months with a trend toward lower survival vs. IL). Using MRI-based neural models that fused various deep neural networks with survival analysis, we then predicted MCI to AD conversion. We used these models to identify subtype-specific regions that demonstrate the largest levels of atrophy-related importance, which had minimal overlap (Average pairwise Jaccard Similarity in regions between the top 5 subtypes, 0.25±0.05 (± std)). Neuropathologic changes characteristic of AD were present across all subtypes in comparable proportions (Chi-square test, p>0.05 for differences in ADNC, n=31). Our risk-based approach to subtyping individuals provides an objective means to intervene and tailor care management strategies at early stages of cognitive decline.

To prevent oxygen contamination, additive manufacturing (AM) techniques normally operate in an inert gas chamber (GC). An alternative method, useful for large builds and components repair, is the application of localised shielding gas (LSG). The effect of oxygen contamination on Ti6242 during directed energy deposition (DED) AM using an inert GC compared to LSG was investigated by in situ synchrotron X-ray experiments. When processing in LSG mode, the amount of oxygen absorbed from the atmosphere was sufficient to reverse the Marangoni flow leading to an alteration of the molten pool geometry and strongly influencing defect formation. Microstructural analysis reveals that, at high oxygen levels, the commonly developed α' martensitic microstructure was completely suppressed, forming precipitation of a tetra modal microstructure of α phase consisting of globular, primary and secondary lamellae (in colonies) and basketweave structure. These results help elucidate the influence of oxygen contamination in additively manufactured Ti alloys, potentially enabling improved industrial practices for AM of titanium alloy.

A significant rise in the occurrence and severity of adverse reactions to several synthetic drugs has fueled considerable interest in natural product-based therapeutics. In humans and animals, polysaccharides from marine microalgae and seaweeds have immunomodulatory effects. In addition, these polysaccharides may possess antiviral, anticancer, hypoglycemic, anticoagulant, and antioxidant properties. During inflammatory diseases, such as autoimmune diseases and sepsis, immunosuppressive molecules can serve as therapeutic agents. Similarly, molecules that participate in immune activation can induce immune responses against cancer and infectious diseases. We aim to discuss the chemical composition of the algal polysaccharides, namely alginate, fucoidan, ascophyllan, and porphyran. We also summarize their applications in the treatment of cancer, infectious disease, and inflammation. Recent applications of nanoparticles that are based on algal polysaccharides for the treatment of cancer and inflammatory diseases have also been addressed. In conclusion, these applications of marine algal polysaccharides could provide novel therapeutic alternatives for several diseases.

Technological advancements in X-ray imaging using bright and coherent synchrotron sources now allows to decouple sample size and resolution, while maintaining high sensitivity to the microstructure of soft, partially dehydrated tissues. The recently developed imaging technique, hierarchical phase-contrast tomography, is a comprehensive approach to address the challenge of organ-scale (up to tens of centimeters) soft tissue imaging with resolution and sensitivity down to the cellular level. Using this technique, we imaged ex vivo an entire human left lung at an isotropic voxel size of 25.08 μm along with local zooms down to 6.05 - 6.5 μm and 2.45 - 2.5 μm in voxel size. The high tissue contrast offered by the fourth-generation synchrotron source at the European Synchrotron Radiation Facility reveals complex multiscale anatomical constitution of the human lung from the macroscopic (centimeter) down to the microscopic (micrometer) scale. The dataset provides complete organ-scale 3D information of the secondary pulmonary lobules and delineates the microstructure of lung nodules with unprecedented detail.

We present a dynamic implementation of the beam-tracking x-ray imaging method providing absorption, phase, and ultrasmall angle scattering signals with microscopic resolution and high frame rate. We demonstrate the method's ability to capture dynamic processes with 22-ms time resolution by investigating the melting of metals in laser additive manufacturing, which has so far been limited to single-modality synchrotron radiography. The simultaneous availability of three contrast channels enables earlier segmentation of droplets, tracking of powder dynamic, and estimation of unfused powder amounts, demonstrating that the method can provide additional information on melting processes.

Mutations of protein kinases are common and can cause cancer and other diseases. However, our understanding of the mutability in genes encoding kinases remains rudimentary. Given previously proposed factors associated with high mutation rates, we analyzed how many genes encoding druggable kinases match with (i) proximity to telomeres or (ii) high A+T content. We extracted genomic information using National Institute of Health Genome Data Viewer. Among 129 druggable human kinase genes studied, 106 genes satisfied either factors (i) or (ii), resulting in an 82% match. A similar 85% match rate was found in 73 genes encoding pro-inflammatory cytokines of multisystem inflammatory syndrome in children. As we further compared these two factors in 20 de novo mutations of mice exposed to space-like ionizing radiation, however, 10 of 20 murine genetic loci met (i) or (ii), leading to only a 50% match. When compared with the top selling approved drugs, the data suggest that matching rate analysis on factor-druggable genome is feasible to systematically prioritize the relative mutability of the novel druggable candidates. These two factors not only predict how mutations of the disease phenotype are attributed to genetic and/or environmental factors but also sort out druggable proteins by their relative mutability.

Blended Elemental powders are an emerging alternative to pre-alloyed powders in metal additive manufacturing due to the wider range of alloys producible with them and the cost savings from not developing novel feedstock. In this study, in situ alloying and concurrent microstructure evolution during SLM are investigated by performing SLM on a BE Ti-185 powder while tracking the surface temperatures via Infra-red imaging and phase transforma- tion via synchrotron X-ray Diffraction. We then performed post-mortem electron microscopy (Backscatter Electron imaging, Energy Dispersive X-ray Spectroscopy and Electron Backscatter Diffraction) to further gain insight into microstructure development. We show that although exothermic mixing aids the melting process, laser melting results only in a mixture of alloyed and unmixed regions. Full alloying and thus a consistent microstructure is only achieved through further thermal cycling in the heat-affected zone.

Tumour infiltration of the lateral pelvic compartment has previously been associated with the highest rate of involved resection margins and carries significant risk of morbidity. In this study, consecutive patients undergoing pelvic exenteration at a single centre between 1994 and 2019 who required en bloc resection of the common or external iliac artery or vein were included.The results demonstrate that complete resection of the iliac vascular system, including resection and reconstruction of the common and external iliac vessels, can be performed safely during pelvic exenteration with oncological outcomes comparable to more central tumours.

Smart buildings conserve energy and create a responsive, comfortable, and productive indoor environment for users and occupants. As a crucial component of smart buildings, smart building management system (SBMS) should provide a wide range of functions and bring about the intended benefits upon successful deployment. This paper identifies salient SBMS attributes and explores key factors influencing building professionals’ intention to use the system in commercial buildings. Responses were collected from 327 Hong Kong building professionals. Data were analyzed by exploratory factor analysis and structural equation modeling based on the refined Unified Theory of Acceptance and Use of Technology (UTAUT). Exploratory factor analysis shows that intelligent building operations and safety and recovery readiness are two dimensions of SBMS emerged. Specifically, intelligent building operations include intelligent and optimal scheduling of building systems, monitor and control of building facilities, having an intelligent and interactive interface, and enabling alarm settings and automatic notifications, showing the importance on the application of electrical engineering in smart building management. Structural equation model (SEM) results indicate that facilitating conditions affect habit, hedonic motivation, social influence, performance expectancy and effort expectancy. Additionally, habit, hedonic motivation and effort expectancy significantly affect building professionals’ intention to use SBMS. Practical implications of SBMS attributes for energy management and the ways in which SBMS is encouraged to be used by building professionals are given.

Effective cancer therapy aims to treat not only primary tumors but also metastatic and recurrent cancer. Immune check point blockade-mediated immunotherapy showed promising effect against tumors; however, it still has a limited effect in metastatic or recurrent cancer. Here, we extracted recombinant murine programmed death-1 (rmPD-1) proteins. The extracted rmPD-1 effectively bound to CT-26 and 4T1 cells expressing PD-L1 and PD-L2. The rmPD-1 did not alter the activation of dendritic cells (DCs); however, rmPD-1 promoted T cell-mediated anti-cancer immunity against CT-26 tumors in mice. Moreover, rmPD-1 decorated thermal responsive hybrid nanoparticles (piHNPs) promoted apoptotic and necrotic cell death of CT-26 cells in response to laser irradiation at 808 nm consequently, it promoted anti-tumor effects against the 1st challenged CT-26 tumors in mice. In addition, piHNP-mediated cured mice from 1st challenged CT-26 was also prevented the 2nd challenged lung metastatic tumor growth, which was dependent of cancer antigen-specific memory T cell immunity. It was also confirmed that the lung metastatic growth of 2nd challenged 4T1 breast cancer was also prevented in cured mice from 1st challenged 4T1 by piHNP. Thus, these data demonstrate that rmPD-1 functions as an immune checkpoint blockade for the treatment of tumors, and piHNPs could be a novel therapeutic agent for preventing cancer metastasis and recurrence.

Imaging intact human organs from the organ to the cellular scale in three dimensions is a goal of biomedical imaging. To meet this challenge, we developed hierarchical phase-contrast tomography (HiP-CT), an X-ray phase propagation technique using the European Synchrotron Radiation Facility (ESRF)’s Extremely Brilliant Source (EBS). The spatial coherence of the ESRF-EBS combined with our beamline equipment, sample preparation and scanning developments enabled us to perform non-destructive, three-dimensional (3D) scans with hierarchically increasing resolution at any location in whole human organs. We applied HiP-CT to image five intact human organ types: brain, lung, heart, kidney and spleen. HiP-CT provided a structural overview of each whole organ followed by multiple higher-resolution volumes of interest, capturing organotypic functional units and certain individual specialized cells within intact human organs. We demonstrate the potential applications of HiP-CT through quantification and morphometry of glomeruli in an intact human kidney and identification of regional changes in the tissue architecture in a lung from a deceased donor with coronavirus disease 2019 (COVID-19).

As surface water resources become more intensely used, and occasionally non-useable, consideration of non-conventional water resources for anthropogenic use has become more prevalent. Potentially critical non-conventional water sources include flooded mine-pit lakes. However, water in these lakes can contain potentially problematic concentrations of contaminants of concern. Background and importance HydroSet is a hydroxyapatite cement that can be used for reconstruction of skull defects. We describe a case of HydroSet resorption resulting in intraventricular and leptomeningeal deposition or calcification, which is a previously unreported complication. Clinical presentation A patient who had a craniotomy augmented with HydroSet developed a pseudomeningocele, disappearance of cement fragments, and new hyperdense coating of the dura, upper cervical cord, and ventricles on CT after a fall. The ubiquitin system, present in all eukaryotes, contributes to regulating multiple types of cellular protein processes such as cell signaling, cell cycle, and receptor trafficking, and it affects the immune response. In most types of cancer, unusual events in ubiquitin-mediated signaling pathway modulation can lead to a variety of clinical outcomes, including tumor formation and metastasis. Similarly, ubiquitination acts as a core component, which contributes to the alteration of cell signaling activity, dictating biosignal turnover and protein fates. As lung cancer acquires the most commonly mutated proteins, changes in the ubiquitination of the proteins contribute to the development of lung cancer. Various inhibitors targeting the ubiquitin system have been developed for clinical applications in lung cancer treatment. In this review, we summarize the current research advances in therapeutics for lung cancer by targeting the ubiquitin system.
