A biomarker of cancer aggressiveness, such as hypoxia, could substantially impact treatment decisions in the prostate, especially radiation therapy, by balancing treatment morbidity (urinary incontinence, erectile dysfunction, etc.) against mortality. R2 (*) mapping with Mono-Exponential (ME) decay modeling has shown potential for identifying areas of prostate cancer hypoxia at 1.5T. However, Gaussian deviations from ME decay have been observed in other tissues at 3T. The purpose of this study is to assess whether gradient-echo signal decays are better characterized by a standard ME decay model, or a Gaussian Augmentation of the Mono-Exponential (GAME) decay model, in the prostate at 3T. Multi-gradient-echo signals were acquired on 20 consecutive patients with a clinical suspicion of prostate cancer undergoing MR-guided prostate biopsies. Data were fitted with both ME and GAME models. The information contents of these models were compared using Akaike's information criterion (second order, AICC ), in skeletal muscle, the prostate central gland (CG), and peripheral zone (PZ) regions of interest (ROIs). The GAME model had higher information content in 30% of the prostate on average (across all patients and ROIs), covering up to 67% of cancerous PZ ROIs, and up to 100% of cancerous CG ROIs (in individual patients). The higher information content of GAME became more prominent in regions that would be assumed hypoxic using ME alone, reaching 50% of the PZ and 70% of the CG as ME R2 (*) approached 40 s(-1) . R2 (*) mapping may have important applications in MRI; however, information lost due to modeling could mask differences in parameters due to underlying tissue anatomy or physiology. The GAME model improves characterization of signal behavior in the prostate at 3T, and may increase the potential for determining correlates of fit parameters with biomarkers, for example of oxygenation status., (Copyright © 2016 John Wiley & Sons, Ltd.)

Accurate sampling of cancer suspicious locations is critical in targeted prostate biopsy, but can be complicated by the motion of the prostate. We present an open-source software for intra-procedural tracking of the prostate and biopsy targets using deformable image registration. The software is implemented in 3D Slicer and is intended for clinical users. We evaluated accuracy, computation time and sensitivity to initialization, and compared implementations that use different versions of the Insight Segmentation Toolkit (ITK). Our retrospective evaluation used data from 25 in-bore MRI-guided prostate biopsy cases (343 registrations total). Prostate Dice similarity coefficient improved on average by 0.17 (p < 0.0001, range 0.02-0.48). Registration was not sensitive to operator variability. Computation time decreased significantly for the implementation using the latest version of ITK. In conclusion, we presented a fully functional open-source tool that is ready for prospective evaluation during clinical MRI-guided prostate biopsy interventions.

Accurate pharmacokinetic (PK) modeling of dynamic contrast enhanced MRI (DCE-MRI) in prostate cancer (PCa) requires knowledge of the concentration time course of the contrast agent in the feeding vasculature, the so-called arterial input function (AIF). The purpose of this study was to compare AIF choice in differentiating peripheral zone PCa from non-neoplastic prostatic tissue (NNPT), using PK analysis of high temporal resolution prostate DCE-MRI data and whole-mount pathology (WMP) validation. This prospective study was performed in 30 patients who underwent multiparametric endorectal prostate MRI at 3.0T and WMP validation. PCa foci were annotated on WMP slides and MR images using 3D Slicer. Foci ≥0.5cm(3) were contoured as tumor regions of interest (TROIs) on subtraction DCE (early-arterial - pre-contrast) images. PK analyses of TROI and NNPT data were performed using automatic AIF (aAIF) and model AIF (mAIF) methods. A paired t-test compared mean and 90th percentile (p90) PK parameters obtained with the two AIF approaches. Receiver operating characteristic (ROC) analysis determined diagnostic accuracy (DA) of PK parameters. Logistic regression determined correlation between PK parameters and histopathology. Mean TROI and NNPT PK parameters were higher using aAIF vs. mAIF (p<0.05). There was no significant difference in DA between AIF methods: highest for p90 volume transfer constant (K(trans)) (aAIF differences in the area under the ROC curve (Az) = 0.827; mAIF Az=0.93). Tumor cell density correlated with aAIF K(trans) (p=0.03). Our results indicate that DCE-MRI using both AIF methods is excellent in discriminating PCa from NNPT. If quantitative DCE-MRI is to be used as a biomarker in PCa, the same AIF method should be used consistently throughout the study., (Copyright © 2015 Elsevier Inc. All rights reserved.)

Purpose: To demonstrate the utility of a robotic needle-guidance template device as compared to a manual template for in-bore 3T transperineal magnetic resonance imaging (MRI)-guided prostate biopsy., Materials and Methods: This two-arm mixed retrospective-prospective study included 99 cases of targeted transperineal prostate biopsies. The biopsy needles were aimed at suspicious foci noted on multiparametric 3T MRI using manual template (historical control) as compared with a robotic template. The following data were obtained: the accuracy of average and closest needle placement to the focus, histologic yield, percentage of cancer volume in positive core samples, complication rate, and time to complete the procedure., Results: In all, 56 cases were performed using the manual template and 43 cases were performed using the robotic template. The mean accuracy of the best needle placement attempt was higher in the robotic group (2.39 mm) than the manual group (3.71 mm, P < 0.027). The mean core procedure time was shorter in the robotic (90.82 min) than the manual group (100.63 min, P < 0.030). Percentage of cancer volume in positive core samples was higher in the robotic group (P < 0.001). Cancer yields and complication rates were not statistically different between the two subgroups (P = 0.557 and P = 0.172, respectively)., Conclusion: The robotic needle-guidance template helps accurate placement of biopsy needles in MRI-guided core biopsy of prostate cancer., (© 2014 Wiley Periodicals, Inc.)

Rationale and Objectives: To investigate the correlation between prevalence and degree of coronary artery calcification (CAC) and extracoronary calcifications (ECCs), scored quantitatively according to Agatston and semiquantitatively by visual analysis, in chest computed tomography (CT) studies obtained for lung cancer screening in asymptomatic subjects and in patients with known coronary heart disease (CHD), and to compare the association of ECC and CAC to established cardiovascular risk factors., Materials and Methods: Prospective study on 501 males (67 ± 8 years) with a history of working dust exposure who underwent nongated low-dose chest CT for lung cancer screening. Of these, 63 (12.6%) had a history of CHD, the remaining 438 subjects (87.4%) were clinically asymptomatic and without a history of CHD. On the day of the CT study, subjects underwent a thorough clinical examination including blood tests and completed a standardized questionnaire to establish a complete medical history. ECC and CAC scores were quantified according to Agatston and, in addition, by visual rating of calcium load of individual vessel territories on a five-point scale from "absent" to "extensive." Results were correlated with the respective subjects' cardiovascular risk factors and with the presence or absence of CHD., Results: ECC scores correlated significantly with CAC scores (two-sided Spearman 0.515; P < .001). ECC scores were associated significantly (P < .001) with cardiovascular risk factors (smoking history, hypertension, diabetes, and hypercholesterolemia) and with subjects' Framingham/prospective cardiovascular münster study scores, whereas CAC scores were associated only with the presence of hypercholesterolemia. CAC scores were strongly associated with CHD than ECC scores (area under the curve, 0.88 vs. 0.66 at receiver operating characteristic analysis). Visual scoring of ECC/CAC load correlated closely with the respective Agatston values (P < .001) and revealed the same association (or lack thereof) with cardiovascular risk factors/CHD., Conclusions: In nongated low-dose CT for lung cancer screening, CAC and ECC load can be accurately established by visual analysis. ECC and CAC scores correlate closely, but not perfectly. There is a strong association between established cardiovascular risk factors and ECC load, but not CAC load, providing further evidence that ECC scoring may complement CAC scoring for broader risk assessment, for example, regarding prediction of extracoronary vascular events., (Copyright © 2015 AUR. Objectives: To evaluate the performance of T2 mapping in discriminating prostate cancer from normal prostate tissue in the peripheral zone using a practical reduced field-of-view MRI sequence requiring less than 3 minutes of scan time., Materials and Methods: Thirty-six patients with biopsy-proven peripheral zone prostate cancer without prior treatment underwent routine multiparametric MRI at 3.0T with an endorectal coil. An Inner-Volume Carr-Purcell-Meiboom-Gill imaging sequence that required 2.8 minutes to obtain data for quantitative T2 mapping covering the entire prostate gland was added to the routine multiparametric protocol. Suspected cancer (SC) and suspected healthy (SH) tissue in the peripheral zone were identified in consensus by three radiologists and were correlated with available biopsy results. Differences in mean T2 values in SC and SH regions-of-interest (ROIs) were tested for significance using unpaired Student's two-tailed t-test. The area under the receiver operating characteristic curve was used to assess the optimal threshold T2 value for cancer discrimination., Results: ROI analyses revealed significantly (p<0.0001) shorter T2 values in SC (85.4±12.3ms) compared to SH (169.6±38.7ms). An estimated T2 threshold of 99ms yielded a sensitivity of 92% and a specificity of 97% for prostate cancer discrimination., Conclusions: Quantitative values derived from this clinically practical T2-mapping sequence allow high precision discrimination between healthy and cancerous peripheral zone in the prostate., (Copyright © 2015 Elsevier Inc. Rationale and Objectives: Development of imaging biomarkers often relies on their correlation with histopathology. Our aim was to compare two approaches for correlating pathology to multiparametric magnetic resonance (MR) imaging (mpMRI) for localization and quantitative assessment of prostate cancer (PCa) index tumor using whole mount (WM) pathology (WMP) as the reference., Materials and Methods: Patients (N = 30) underwent mpMRI that included diffusion-weighted imaging and dynamic contrast-enhanced (DCE) MRI at 3 T before radical prostatectomy (RP). RP specimens were processed using WM technique (WMP) and findings summarized in a standard surgical pathology report (SPR). Histology index tumor volumes (HTVs) were compared to MR tumor volumes (MRTVs) using two approaches for index lesion identification on mpMRI using annotated WMP slides as the reference (WMP) and using routine SPR as the reference. Consistency of index tumor localization, tumor volume, and mean values of the derived quantitative parameters (mean apparent diffusion coefficient [ADC], K(trans), and ve) were compared., Results: Index lesions from 16 of 30 patients met the selection criteria. There was WMP/SRP agreement in index tumor in 13 of 16 patients. ADC-based MRTVs were larger (P < .05) than DCE-based MRTVs. ADC MRTVs were smaller than HTV (P < .005). There was a strong correlation between HTV and MRTV (Pearson ρ > 0.8; P < .05). No significant differences were observed in the mean values of K(trans) and ADC between the WMP and SPR., Conclusions: WMP correlation is superior to SPR for accurate localization of all index lesions. Purpose: Percutaneous punctures are often performed under computed tomography (CT) guidance using a freehand method. Especially in challenging cases, initial accuracy of the needle placement is highly dependent on the radiologist's experience. Thus, a miniature lightweight guidance device was developed which is capable of assisting a radiologist during the needle placement process., Methods: The device utilizes an accelerometer to measure the needle's tilt by calculating a set of orientation angles. This set can be matched with the coordinate system of the CT imaging software during a simple alignment process. After that, the needle's orientation can be expressed in terms of projected angles in the axial and sagittal planes. The accuracy of the device was evaluated in a phantom study, and initial clinical trials were carried out performing facet joint punctures in a swine cadaver., Results: The sensor was embedded in a cube with dimensions of [Formula: see text] and a total weight of about 11 g which can be attached to the puncture needle at its rear end or handgrip. A graphical user interface (GUÌ) has been created offering visual real-time orientation guidance. Results of the phantom experiments showed differences between planned target and performed puncture angles of [Formula: see text] for in-plane and [Formula: see text] for out-of-plane punctures., Conclusion: The results of the phantom and ex vivo study suggest that the device is useful to assist a radiologist in CT-guided percutaneous punctures and helps navigating the needle with high precision.

Purpose: To determine the detection rate, clinical relevance, Gleason grade, and location of prostate cancer ( PCa prostate cancer ) diagnosed with and the safety of an in-bore transperineal 3-T magnetic resonance (MR) imaging-guided prostate biopsy in a clinically heterogeneous patient population., Materials and Methods: This prospective retrospectively analyzed study was HIPAA compliant and institutional review board approved, and informed consent was obtained. Eighty-seven men (mean age, 66.2 years ± 6.9) underwent multiparametric endorectal prostate MR imaging at 3 T and transperineal MR imaging-guided biopsy. Three subgroups of patients with at least one lesion suspicious for cancer were included: men with no prior PCa prostate cancer diagnosis, men with PCa prostate cancer who were undergoing active surveillance, and men with treated PCa prostate cancer and suspected recurrence. Exclusion criteria were prior prostatectomy and/or contraindication to 3-T MR imaging. The transperineal MR imaging-guided biopsy was performed in a 70-cm wide-bore 3-T device. Overall patient biopsy outcomes, cancer detection rates, Gleason grade, and location for each subgroup were evaluated and statistically compared by using χ(2) and one-way analysis of variance followed by Tukey honestly significant difference post hoc comparisons., Results: Ninety biopsy procedures were performed with no serious adverse events, with a mean of 3.7 targets sampled per gland. Cancer was detected in 51 (56.7%) men: 48.1% (25 of 52) with no prior PCa prostate cancer , 61.5% (eight of 13) under active surveillance, and 72.0% (18 of 25) in whom recurrence was suspected. Gleason pattern 4 or higher was diagnosed in 78.1% (25 of 32) in the no prior PCa prostate cancer and active surveillance groups. Gleason scores were not assigned in the suspected recurrence group. MR targets located in the anterior prostate had the highest cancer yield (40 of 64, 62.5%) compared with those for the other parts of the prostate (P < .001)., Conclusion: In-bore 3-T transperineal MR imaging-guided biopsy, with a mean of 3.7 targets per gland, allowed detection of many clinically relevant cancers, many of which were located anteriorly.

Purpose: Devices constantly tracking the blood pressure (BP) of hypertensive patients are highly desired to facilitate effective patient management and to reduce hospitalization. We report on experiences gathered in a pilot study that was designed to evaluate the prototype of a newly developed, minimally invasive implantable sensor system for long-term BP monitoring., Methods: The device was implanted in the femoral artery (FA) of 12 sheep via standard FA catheterization under fluoroscopic control. Accuracy of the recorded blood pressure was determined by comparison with a reference catheter, which was positioned in the contralateral FA immediately after implantation. Regular follow-up included angiography, computed tomography (CT), and control of functionality and position of the BP sensor. Animals were euthanized after 6 months. FA segments with in situ pressure sensor underwent macroscopic and histopathologic examinations., Results: All implantations of the novel sensor device in the FA were successful and uneventful. High-quality BP recordings were documented. Bland-Altman plots indicate very good agreement. Comparison with measurements taken from the reference sensor revealed mean differences and standard deviations of -0.56 ± 0.85, 0.29 ± 1.44, and 0.85 ± 2.27 mmHg (diastolic, systolic, and pulse pressure, respectively) after exclusion of one outlier. CT uncovered deficiencies in cable stability that were addressed in a redesign. No thrombus formation, necrosis, or apoptosis were detected., Conclusions: The pilot study proved the technical feasibility of wireless BP measurement in the FA via a novel miniature sensor device.

Postmortem computed tomography (PMCT) is a modern tool that complements autopsy diagnostics. In clinical autopsies, a major cause of death is cardiovascular disease. To improve the performance of PMCT in cardiovascular disease, full body angiography was developed (PMCT angiography [PMCTA]). Twenty PMCTA scans generated before autopsy were compared with native PMCT and clinical autopsy. The objective of the study was to quantify the additional diagnostic value of adding angiography to native imaging and to compare PMCT and PMCTA findings to autopsy findings. The diagnosis of the cause of death was identical or overlapped in 80% of the cases that used PMCTA and 70% that used PMCT. The additional diagnostic yield given by PMCT and PMCTA in combination with autopsy was 55%. PMCT yielded additional diagnoses in the musculoskeletal system. The greatest additional diagnostic value of PMCTA was in association with cardiovascular diagnoses. The accuracy of PMCTA for cardiac causes of death was 80%, and the positive predictive value was 90%. The findings indicate that native PMCT cannot display the cardiovascular system sufficiently clearly for high-quality diagnostic assessment. However, PMCTA is a powerful tool in autopsy cases with a history of cardiovascular disease and/or a suspected cardiovascular cause of death. The combination of PMCTA and clinical autopsy enhances diagnostic quality and completeness of the autopsy report. Furthermore, in cases without consent or with a restricted consent for clinical autopsy, PMCTA has the potential to provide information on cardiovascular causes of death., (Copyright © 2014 Elsevier Inc. Background: To investigate the impact of high pitch cardiac CT vs. retrospective ECG gated CT on the quantification of calcified vessel stenoses, with assessment of the influence of tube voltage, reconstruction kernel and heart rate., Methods: A 4D cardiac movement phantom equipped with three different plaque phantoms (12.5%, 25% and 50% stenosis at different calcification levels), was scanned with a 128-row dual source CT scanner, applying different trigger types (gated vs. prospectively triggered high pitch), tube voltages (100-120 kV) and heart rates (50-90 beats per minute, bpm). Images were reconstructed using different standard (B26f, B46f, B70f) and iterative (I26f, I70f) convolution kernels. Absolute and relative plaque sizes were measured and statistically compared. Radiation dose associated with the different methods (gated vs. high pitch, 100 kV vs. 120 kV) were compared., Results: Compared to the known diameters of the phantom plaques and vessels both CT-examination techniques overestimated the degrees of stenoses. Using the high pitch CT-protocol plaques appeared larger (0.09 ± 0.31 mm, 2 ± 8 percent points, PP) in comparison to the ECG-gated CT-scans. Reducing tube voltage had a similar effect, resulting in higher grading of the same stenoses by 3 ± 8 PP. In turn, sharper convolution kernels lead to a lower grading of stenoses (differences of up to 5%). Pairwise comparison of B26f and I26f, B46f and B70f, and B70f and I70f showed differences of 0-1 ± 6-8 PP of the plaque depiction. Motion artifacts were present only at 90 bpm high pitch experiments. High-pitch protocols were associated with significantly lower radiation doses compared with the ECG-gated protocols (258.0 mGy vs. 2829.8 mGy CTDIvol, p ≤ 0.0001)., Conclusion: Prospectively triggered high-pitch cardiac CT led to an overestimation of plaque diameter and degree of stenoses in a coronary phantom. This overestimation is only slight and probably negligible in a clinical situation. Even at higher heart rates high pitch CT-scanning allowed reliable measurements of plaque and vessel diameters with only slight differences compared ECG-gated protocols, although motion artifacts were present at 90 bpm using the high pitch protocols.

Purpose: To test the performance of an expandable bipolar probe as a simple technical solution for extending the coagulation volume., Methods: On the basis of a commercially available monopolar radiofrequency (RF) probe (LeVeen), an expandable bipolar RF probe was developed by integrating a second electrode into the probe shaft. The influence of length on the second electrode, and the distance between both electrodes and generator output was investigated by performing ten ablations for each condition on a freshly excised bovine liver. Macroscopically quantified coagulation volumes, lesion shape characteristics, and procedure durations were recorded. Results of the prototype featuring the optimal configuration were compared to the original LeVeen probe and commonly used bipolar RF probe (CelonLabPower)., Results: Extension of the shaft electrode length, increasing distance between the shaft electrode and the tip electrode, and reduction of generator output resulted in increasing coagulation volumes. The coagulation volumes the prototype generated were significantly smaller and more elliptically shaped than the monopolar probe (9.4 ± 1.5 cm(3) vs. 12.1 ± 1.6 cm(3)), but were larger than the commercially available bipolar RF probe (vs. 7.3 ± 0.5). The procedure duration of the prototype was comparable to the monopolar probe (467 ± 31 s vs. 464 ± 17 s) and shorter than the bipolar probe (vs. 2009 ± 444 s). In comparison to the commercially available bipolar system, the developed prototype exhibited favorable results., Conclusion: The first benchmark testing of the developed bipolar prototype had promising results. However, further optimization of the applicator design and ablation protocol is needed to enlarge the achievable coagulation volume.

Objectives: To evaluate the diagnostic value of contrast enhanced post mortem computed tomography (PMCT) in comparison to non-enhanced post mortem CT in the detection of cardiovascular causes of death (COD)., Background: As autopsy rates decline, new methods to determine CODs are necessary. So contrast enhanced PMCT shall be evaluated in comparison to established non-enhanced PMCT in order to further improve the method., Methods: In a prospective study, 20 corpses were examined using a 64-row multisclice CT (MSCT) before and after intraarterial perfusion with a newly developed, barium-bearing contrast agent and ventilation of the lungs. The cause of death was determined in enhanced and unenhanced scans and a level of confidence (LOC) was given by three experienced radiologists on a scale between 0 and 4. Results were compared to autopsy results as gold standard. Autopsy was performed blinded to PMCT-findings., Results: The method allowed visualization of different types of cause of death. There was a significant improvement in LOC in enhanced scans compared to unenhanced scans as well as an improvement in the detection of COD. The cause of death could be determined in 19 out of 20 patients., Conclusions: PMCT is feasible and appears to be robust for diagnosing cardiovascular causes of death. When compared with unenhanced post-mortem CT intraarterial perfusion and pulmonary ventilation significantly improve visualization and diagnostic accuracy. These promising results warrant further studies.

Purpose: This study was designed to evaluate CO2 computed tomography angiography (CO2-CTA) of the iliac and lower limb arteries in patients with contraindications for iodinated contrast agent (ICA)., Methods: Five patients with contraindications for ICA were examined using CO2-fluoroscopic enhanced angiography (CO2-FLA) and CO2-CTA using a high-pitch examination protocol. Objective (vessel diameter) and subjective (visual score) image quality parameters were evaluated. Pathological findings in both modalities were compared., Results: CO2-CTA was feasible in all patients without adverse side effects, except for CO2 injection-associated pain. Objective vessel analysis revealed no significant difference in diameters as determined by CO2-CTA and CO2-FLA (0.44 ± 0.4 and 0.46 ± 0.41 mm, p = 0.93). CO2-CTA had on average a higher image-quality score (2.6 ± 1.0 vs. 2.3 ± 1.0, p = 0.009). While for pelvic and upper leg CO2-CTA advantageous (3.1 ± 0.74 vs. 2.7 ± 0.9, p = 0.0014) at good quality scores, for calf vessels no significant improvement was visible (1.9 ± 1.0 vs. 1.7 ± 0.9, p = 0.49) and scores were poorer., Conclusions: CO2-CTA with high-pitch CT was feasible in a limited number of patients. Image-quality scores were on average higher for CO2-CTA than for CO2-FLA, while limited imaging quality in the vessels below the knee needs further work on the CT protocol. An added value of cross-sectional imaging was apparent but needs further quantification.

This work presents concepts for complex endovascular procedures using electromagnetic navigation technology (EMT). Navigation software interfacing a standard commercially available navigation system was developed, featuring registration, electromagnetic field distortion correction, breathing motion detection and gating, and state-of-the-art 3D imaging post processing. Protocols for endovascularly placed, in-situ fenestrated abdominal aortic stent grafts and an EMT guided transjugular intrahepatic portosystemic shunt (TIPSS) creation have been designed. A dedicated set of interventional devices was developed for each of the procedures: For aortic in-situ fenestration a combination of high-porosity stentgrafts, steerable catheters and electromagnetically navigated guidewires was used, for TIPSS a dual-navigated (sheath and stylet) TIPSS-device was designed and manufactured. The developed devices underwent phantom testing, in preparation for animal experiments to prove the feasibility of the approach. Once established, these systems could aid in performing these challenging interventional radiology procedures, exploiting the unique characteristics of electromagnetic navigation and solving multiple of the problems associated with these interventions being performed under X-ray fluoroscopy, such as lacking real-time 3D information or extensive exposure to ionizing radiation.

Purpose: To investigate the safety and feasibility of the use of a vascular closure device (VCD) after endovascular procedures in swine., Material and Methods: In a study on endovascular therapy, VCD (StarClose, Abbott Vascular, Il, USA) was used in 20 female swines to achieve immediate hemostasis after percutaneous right femoral artery (FA) access. 10 animals were sacrificed immediately after the study and 10 animals were sacrificed 28 days after the initial study. To ensure complete hemostasis and patency of the femoral artery, a CT-angiography of the puncture site was performed on day 1 (acute and chronic group) and day 28 (chronic group). After the sacrifice, the femoral artery was explanted and examined macroscopically for signs of VCD dysfunction., Results: Technical success rate was 100% with immediate hemostasis being achieved in all animals. No animals showed evidence of hematoma. During explantation, only small traces of coagulated blood were found in the acute group, while there were no signs of hematoma in the chronic group. CT-angiography immediately after VCD application as well as before sacrifice (chronic group) showed patency of the FA in all cases., Conclusion: The use of VCD to achieve hemostasis after endovascular studies in swine is feasible and safe.

It is now universally recognized that many prostate cancers are over-diagnosed and over-treated. The European Randomized Study of Screening for Prostate Cancer from 2009 evidenced that, to save one man from death from prostate cancer, over 1400 men need to be screened, and 48 need to undergo treatment. The detection of prostate cancer is traditionally based on digital rectal examination (DRE) and the measurement of serum prostate-specific antigen (PSA), followed by ultrasound-guided biopsy. The primary role of imaging for the detection and diagnosis of prostate cancer has been transrectal ultrasound (TRUS) guidance during biopsy. Traditionally, MRI has been used primarily for the staging of disease in men with biopsy-proven cancer. It has a well-established role in the detection of T3 disease, planning of radiation therapy, especially three-dimensional conformal or intensity-modulated external beam radiation therapy, and planning and guiding of interstitial seed implant or brachytherapy. New advances have now established that prostate MRI can accurately characterize focal lesions within the gland, an ability that has led to new opportunities for improved cancer detection and guidance for biopsy. Two new approaches to prostate biopsy are under investigation. Both use pre-biopsy MRI to define potential targets for sampling, and the biopsy is performed either with direct real-time MR guidance (in-bore) or MR fusion/registration with TRUS images (out-of-bore). In-bore and out-of-bore MRI-guided prostate biopsies have the advantage of using the MR target definition for the accurate localization and sampling of targets or suspicious lesions. The out-of-bore method uses combined MRI/TRUS with fusion software that provides target localization and increases the sampling accuracy of TRUS-guided biopsies by integrating prostate MRI information with TRUS. Newer parameters for each imaging modality, such as sonoelastography or shear wave elastography, contrast-enhanced ultrasound and MRI elastography, show promise to further enrich datasets., (Copyright © 2013 John Wiley & Sons, Ltd.)

Purpose: The purpose of this study was to evaluate the efficacy of antegrade pyeloperfusion with cooled 5% of glucose solution to protect the renal collecting system during microwave ablation (MWA)., Materials and Methods: Computed tomographically guided nephrostomy was performed in 1 kidney in each of 14 female pigs by placing a 6F nephrostomy catheter. Pyeloperfusion was performed through infusion of cooled 5% of glucose solution via the nephrostomy catheter (4°C; 10 mL/min). Microwave ablation (915 MHz) was performed at 45 W for 10 minutes in the irrigated and the non-irrigated kidneys creating a central lesion and a peripheral lesion in each kidney. Maximum ablation diameters were measured and ablation volumes were calculated after the animals were killed. The extent of thermal injury of the renal pyelon was assessed histopathologically in the slice of maximal thermal injury on a 10-point scale ranging from 0 (none) to 10 (complete)., Results: Pyeloperfusion did not impact the ablation volume or the degree of thermal injury to the collecting system for the central or peripheral MWA procedures: The mean (SD) volume of the MWA zones was equivalent for the irrigated versus non-irrigated kidneys for the peripheral (3.07 [2.39] mL vs 3.87 [3.08] mL) and central MWA procedures (2.26 [1.55] mL vs 2.40 [1.60] mL). The mean (SD) histologic scores of the thermal damage of the pyelon were similar for the irrigated versus non-irrigated kidneys in the peripheral MWA (1.0 [2.7] vs 0.2 [0.6]) and in the central MWA (1.6 [2.7] vs 3.4 [3.4])., Conclusions: In MWA, cooled antegrade pyeloperfusion is not useful to modulate the ablation volume in peripheral or central locations. Accordingly, pyeloperfusion may not be useful as a protective measure to avoid thermal damage in MWA of the kidney. Therefore, MWA of central renal tumors does not seem advisable.

Purpose: This study was designed to systematically investigate artifacts caused by interventional needles recommended for use in MRI, with focus on field strength, needle/mandrin type, orientation and sequence., Methods: Eight different MRI compatible needles were placed in porcine tissue and examined at 1.5 and 3.0 T with balanced-steady-state-free-precession (B-SSFP) and T1-weighted-spoiled-gradient-echo (T1-SPGR) sequences in different orientations to B0. Artifact diameters with regards to the primary, inner, and secondary, outer artifacts were assessed and statistically evaluated., Results: The types and degree of artifacts varied considerably, especially between different mandrin types even for the same needles. Orientation of the needle in the magnetic field was another main contributor to the artifact dimensions. Less important factors were the type of pulse sequence and field strength. Artifacts ranged from 0.7 mm (steel, 0°, B-SSFP, 3.0 T, inner) to 71.4 mm (nitinol, 90°, B-SSFP, 1.5 T, outer). Inner artifact diameters in B-SSFP were slightly larger (8.2 ± 5.7 mm) than those in T1-SPGR (7.6 ± 5.4 mm) and comparable between 1.5 and 3.0 T (e.g., 8.0 vs. 8.4 mm, B-SSFP)., Conclusions: Although all were sold as "MR compatible," the artifacts differed greatly between needle types, and even more so for different mandrins. The results suggest an empirical approach to the needle choice based on lesion type and approach angle.

Background: Hematoma is a common complication following arterial puncture. To date no device that allows sealing of an arterial puncture site with in-situ catheter has been developed., Purpose: To evaluate a newly developed arterial sealing device for endovascular catheters in an in-vivo experimental setting., Material and Methods: A peelable collagen-based vascular sealing device for endovascular catheters was tested in acute (follow-up: 4 h; n = 2) and chronic (follow-up: 1 week; n = 4) settings in the femoral artery (FA) of sheep. After implantation correct position of the device as well as patency of the FA were verified angiographically. In the chronic group, hematoma was excluded and patency of FA was assured using color Doppler ultrasound 1 and 3 days after the procedure. After 1 week a final ultrasound and an angiography were performed for final evaluation. Thereafter, the animals were sacrificed and the puncture site was dissected and analyzed macroscopically., Results: Sufficient sealing of the puncture site could be observed in all animals. In acute and chronic experiments, neither a hematoma at the puncture site nor other complications were observed after positioning the sealing device. Follow-up color Doppler ultrasounds (CDUS) and final angiography revealed patent FAs in all animals. Macroscopic evaluation of dissection material proved collagen plug and catheter being in place., Conclusion: Our preliminary in-vivo results demonstrate a safe and convenient vascular sealing system for endovascular catheters. Purpose: To evaluate the effects of particle size and course of action of superselective bland transcatheter arterial embolization (TAE) on the efficacy of radiofrequency ablation (RFA)., Methods: Twenty pigs were divided into five groups: group 1a, 40-μm bland TAE before RFA; group 1b, 40-μm bland TAE after RFA; group 2a, 250-μm bland TAE before RFA; group 2b, 250-μm bland TAE after RFA and group 3, RFA alone. A total of 40 treatments were performed with a combined CT and angiography system. The sizes of the treated zones were measured from contrast-enhanced CTs on days 1 and 28. Animals were humanely killed, and the treated zones were examined pathologically., Results: There were no complications during procedures and follow-up. The short-axis diameter of the ablation zone in group 1a (mean ± standard deviation, 3.19 ± 0.39 cm) was significantly larger than in group 1b (2.44 ± 0.52 cm; P = 0.021), group 2a (2.51 ± 0.32 cm; P = 0.048), group 2b (2.19 ± 0.44 cm; P = 0.02), and group 3 (1.91 ± 0.55 cm; P < 0.001). The greatest volume of ablation was achieved by performing embolization with 40-μm particles before RFA (group 1a; 20.97 ± 9.65 cm(3)). At histology, 40-μm microspheres were observed to occlude smaller and more distal arteries than 250-μm microspheres., Conclusion: Bland TAE is more effective before RFA than postablation embolization. The use of very small 40-μm microspheres enhances the efficacy of RFA more than the use of larger particles.

Purpose: To systematically investigate the impact of image acquisition and contrast injection parameters for CO2-enhanced CT angiography (CTA) of the aorto-iliac and peripheral arteries in a pig model using commercially available equipment. The aim was to establish an imaging protocol that is ready for use in human subjects., Materials and Methods: Three domestic swine underwent CO2-CTA with varying injection parameters: pitch (1.0, 3.0), injection pressure (0.7 bar, 1.0 bar, 1.3 bar) and scan delay (2 s, 4 s, 6 s). Objective (vessel diameter) and subjective (image quality) parameters and applied radiation doses were systematically evaluated. To ensure clinical applicability of the setting, only approved catheters/injectors and standard injection parameters were evaluated., Results: The image quality scores were superior and the vessel diameter was larger with high pitch in comparison to standard pitch (diameters: 4.7 ± 2.0 mm vs. 3.6 ± 2.1 mm, p = 0.0040, scores: 2.6 ± 1.1 vs. 2.0 ± 1.1, p = 0.0038). High injection pressure (1.3 bar) improved the image quality as assessed by subjective and objective ratings (diameters: 3.6 ± 2.0 mm, 4.0 ± 2.1 mm and 4.6 ± 2.1 mm, for 0.7, 1.0 and 1.3 bar, p-values ≤ 0.0052, scores: 1.9 ± 1.1, 2.3 ± 1.1 and 2.7 ± 1.2, p-values ≤ 0.0017), the same was observed for a shorter injection delay (diameters: 3.5 ± 2.0 mm, 4.2 ± 2.1 mm and 4.8 ± 2.1 mm, for 6 s, 4 s, and 2 s, p ≤ 0.0022, scores: 1.9 ± 1.1, 2.3 ± 1.1 and 2.7 ± 1.1, p-values ≤ 0.0013). The dose length products were 239 ± 47 mGycm (high pitch) and 565 ± 63 mGycm (standard pitch, p-values < 0.0001)., Conclusion: A higher pitch, shorter delay and higher injection pressure improve image quality in CO2-enhanced CTA. This paper presents a novel fully implantable wireless sensor system intended for long-term monitoring of hypertension patients, designed for implantation into the femoral artery with computed tomography angiography. It consists of a pressure sensor and a telemetric unit, which is wirelessly connected to an extracorporeal readout station for energy supply and data recording. The system measures intraarterial pressure at a sampling rate of 30 Hz and an accuracy of ±1.0 mmHg over a range of 30-300 mmHg, while consuming up to 300 μW. A special peel-away sheath introducer set was developed to support the implantation procedure. The system delivered stable measurements in initial animal trials in sheep, with results being in good agreement with reference sensor systems.

For the past century, autopsy techniques in clinical pathology have not changed significantly, while autopsy rates are declining. Modern imaging techniques offer interesting prospects of supportive post-mortem diagnostic investigation. In a prospective study of 29 autopsy cases, complimentary virtual autopsy using unenhanced post-mortem computed tomography (pmCT) was performed. We analysed in a prospective cohort study 29 unenhanced pmCT scans, generated prior to autopsy. Clinical information regarding clinical history and circumstances of death were provided. The objective of the study was to find consistency and/or discrepancy between virtual autopsy and conventional autopsy findings regarding cause of death and death-related diagnoses, reconstruction of the pathogenetic mechanisms involved, side diagnoses and CPR (cardiopulmonary resuscitation)- or death-related post-mortem changes. Accuracy of pmCT for cause of death was 68 % and the positive predictive value (PPV) was 75 %. Regarding the pathogenetic mechanisms, accuracy of pmCT was 21 % and PPV was 29 %. The combined diagnostic yield of autopsy and pmCT was 133 % compared to autopsy only. Modern imaging techniques give an opportunity for post-mortem diagnostics to complete but not yet replace traditional autopsy. We could show that in two out of three cases, the cause of death found by pmCT matched the diagnosis from classical autopsy. While both disciplines, pathology and radiology, will profit from the mutual exchange of data, it seems a realistic aim to strive for virtual autopsy possibly further supported by biopsies and contrast-enhanced pmCT as an alternative to the classical clinical autopsy. A combination of both methods enhances diagnostic quality and completeness of the autopsy report.

Objectives: To implement a retrospective intrinsic landmark-based (ILB) gating protocol for contrast-enhanced ultrasound (CEUS) and to compare its efficiency to non-gated, manually gated and extrinsically gated CEUS., Methods: CEUS of the liver was performed in healthy mice (n = 5) and in NEMO knockout mice with dysplastic livers (n = 5). In healthy animals, first-pass kinetics of non-specific microbubbles was recorded. Knockout mice were analysed regarding retention of VEGFR2-specific microbubbles. For retrospective gating, a landmark which showed respiratory movement was encircled as a region of interest (ROI). During inspiration, the signal intensity within the ROI altered, which served as gating signal. To evaluate the accuracy, non-gated, extrinsically gated and ILB-gated time-intensity curves were created. For each curve, descriptive parameters were calculated and compared to the gold standard (manual frame-by-frame gating)., Results: No significant differences in the variation of ILB- and extrinsically gated time-intensity curves from the gold standard were observed. Non-gated data showed significantly higher variations. Also the variation of molecular ultrasound data was significantly lower for ILB-gated compared to non-gated data., Conclusion: ILB gating is a robust and easy method to improve data accuracy in functional and molecular ultrasound liver imaging. This technique can presumably be translated to contrast-enhanced ultrasound examinations in humans., Key Points: • Quantitative analysis of the uptake of contrast agents during ultrasound is complex. • Intrinsic landmark-based gating (ILB) offers a simple implementable method for motion correction. • Results using ILB-gating are comparable to extrinsic gating using external biomonitoring devices. • Functional and molecular imaging of mobile organs will benefit from ILB gating.

Purpose: Evaluation of the maximum temperatures and ablation volumes in microwave ablation (MWA) after injection of different concentrations of silicon carbide (SiC) particles in an ex-vivo bovine liver model., Materials and Methods: 15 ml of different concentrations of SiC particles (20 vol% SiC; 50 vol% SiC) mixed with 2 % gelatin were injected into an ex-vivo bovine liver. As a reference group, 2 % gelatin without SiC was injected. MWA was performed using a clinical MWA system with different generator settings (10 - 45 W/10 minutes). The temperature was measured at a distance of 5 mm and 10 mm from the applicator. Afterwards the liver tissue was sliced along the short and long axis, the ablation zones were measured on the x, y and z-axis and the ablation volume was calculated. All experiments were performed 5 times (total: 40 experiments)., Results: The average maximum temperatures measured at a generator setting of 45 W at a distance of 5 mm from the applicator were 103.4 ± 4.6 °C (20 vol% SiC), 103.3 ± 6.5 °C (50 vol% SiC) and 96.0 ± 4.2 °C in the control group (0 vol% SiC). At 45 W, injection of 20 vol% SIC caused a significantly higher maximum temperature than that achieved in the control group (p = 0.016). No significant temperature increase compared to the control group could be measured using 50 vol% SiC. The mean ablation volumes at 45 W and 20 vol% SiC and 50 vol% SiC were significantly larger (172.7 ± 31.5 ml and 171.0 ± 34.7 ml, respectively) than those achieved in the control group (111.2 ± 23.8 ml) (p = 0.027 and p = 0.045)., Conclusion: In an ex-vivo bovine liver model, the SiC particles demonstrated an enhancing effect of MWA with respect to maximum temperatures and ablation volume. Purpose: The aim of this paper was to establish non-invasive CT-based temperature monitoring during hepatic radiofrequency (RF) ablation in an ex vivo porcine model followed by transfer of the technique into a feasibility in vivo experiment., Materials and Methods: Bipolar RF ablations were performed in 10 specimens of porcine liver. Parallel to the needle-shaped RF applicator three optical temperature probes were inserted into the liver specimens at fixed distances of 5, 10 and 15 mm from the RF probe. During energy application (20 W) unenhanced sequential MSCT scans were acquired using the following scan protocol: 140 kV tube voltage, 300 mAs/rotation tube current time product, collimation 24 × 1.2 mm, rotation time 0.5 s. Axial image data was reconstructed using a soft tissue convolution kernel. Temperature data was recorded during every CT scan. Using a circular 0.5 cm(2) region of interest local CT values were measured at the tips of the temperature probes and matched with the measured temperatures. Regression analysis was performed to analyse the relationship between local temperatures and CT values for each temperature probe position. Furthermore, the same experimental design was used in four anaesthetised female pigs in order to investigate the potential of this technique for an in vivo application., Results: A negative correlation was found for the relationship between temperature and CT value. Regression coefficients were -0.44 (5 mm), -0.35 (10 mm) and -0.37 (15 mm) for ex vivo data. Analysis of in vivo experiments showed regression coefficients between -0.025 and -0.434., Conclusion: Multislice computed tomography is able to depict temperature changes in liver tissue during RFA.

Purpose: To evaluate the feasibility of computed tomography (CT)-guided placement of inferior vena cava (IVC) filters in a swine model., Materials and Methods: Five domestic pigs (60-70 kg) underwent transfemoral and transjugular IVC filter placement under real-time CT fluoroscopic guidance. Filter position was confirmed by contrast-enhanced CT and digital subtraction angiography. Filter tilt, distance to target position, and fluoroscopy time were analyzed., Results: A total of 10 filters were successfully implanted (five via transfemoral approach, five via transjugular approach) without complications. The mean distance to the target position was 0.3 cm ± 0.2. Mean filter tilt was 3.2° ± 2.3 (range, 0°-7°), without differences between deployment techniques (P = .8486). Average fluoroscopy time was 25.9 s ± 6.9 per procedure., Conclusions: CT fluoroscopy-guided placement of IVC filters is safely feasible. Stenting of the central veins is well established for treating localized venous stenosis. The techniques regarding catheter preservation for central venous catheters in the superior vena cava have been described. We describe here a method for stent implantation in the superior vena cava and the left brachiocephalic vein, and principally via a single jugular venous puncture, while saving a left sided jugular central venous catheter in a patient suffering from central venous stenosis of the superior vena cava and the left brachiocephalic vein.

The purpose of this paper was to evaluate computed tomography (CT) based electromagnetically tip-tracked (EMT) interventions in various clinical applications. An EMT system was utilized to perform percutaneous interventions based on CT datasets. Procedure times and spatial accuracy of needle placement were analyzed using logging data in combination with periprocedurally acquired CT control scans. Dose estimations in comparison to a set of standard CT-guided interventions were carried out. Reasons for non-completion of planned interventions were analyzed. Twenty-five procedures scheduled for EMT were analyzed, 23 of which were successfully completed using EMT. The average time for performing the procedure was 23.7 ± 17.2 min. Time for preparation was 5.8 ± 7.3 min while the interventional (skin-to-target) time was 2.7 ± 2.4 min. The average puncture length was 7.2 ± 2.5 cm. Spatial accuracy was 3.1 ± 2.1 mm. Non-completed procedures were due to patient movement and reference fixation problems. Radiation doses (dosis-length-product) were significantly lower (p = 0.012) for EMT-based interventions (732 ± 481 mGy x cm) in comparison to the control group of standard CT-guided interventions (1343 ± 1054 mGy x cm). Electromagnetic navigation can accurately guide percutaneous interventions in a variety of indications. Accuracy and time usage permit the routine use of the utilized system. Lower radiation exposure for EMT-based punctures provides a relevant potential for dose saving.

Purpose: To quantitatively analyze differences in mechanical properties, needle design including signs of wear, subjective handling and specimen quality of bone biopsy needles., Materials and Methods: In this study 19 different bone biopsy systems (total 38; 2 /type) were examined. With each biopsy needle five consecutive samples were obtained from vertebral bodies of swine. During puncture a force-torques sensor measured the mechanical properties and subjective handling was assessed. Before and after each biopsy the needles were investigated using a profile projector and signs of wear were recorded. Afterwards, a pathologist semi-quantitatively examined the specimen regarding sample quality. The overall evaluation considered mechanical properties, needle wear, subjective handling and sample quality. Differences were assessed for statistical significance using ANOVA and t-test., Results: Needle diameter (p = 0.003) as well as needle design (p = 0.008) affect the mechanical properties significantly. Franseen design is significantly superior to other needle designs. Besides, length reduction recorded by the profile projector, as a quality criterion showed notable distinctions in between the needle designs., Conclusion: Bone biopsy needles vary significantly in performance. Needle design has an important influence on mechanical properties, handling and specimen quality. Purpose: To test the feasibility of carbon dioxide (CO(2))-enhanced computed tomography (CT)-guided placement of infrarenal abdominal aortic stent-grafts in an animal model., Methods: Appearance of a stent-graft mounted on its deployment system and the feasibility of CT fluoroscopy-guided placement were analyzed in an in vitro setting. Five domestic pigs weighing 70 to 80 kg underwent CO(2)-enhanced 64-slice CT arteriography (CTA). After surgical exposure of the right iliac artery, an 18-mm stent-graft was advanced into the abdominal aorta. Infrarenal position of the graft was monitored using CT fluoroscopy with CO(2) administered intermittently in a flow-regulated manner using a computer-controlled injection system. After the final position of the stent-graft was determined, the graft was deployed under CT fluoroscopy guidance. Graft position was confirmed by contrast enhanced 64-slice CTA and conventional catheter angiography. To quantitatively assess the position of the stent-graft, the distance between the proximal stent struts and the radiopaque marker was determined using an electronic caliper., Results: CT-guided placement of infrarenal aortic stent-grafts was feasible in all animals without complications. CO(2)-enhanced CTA allowed for the identification of the renal arteries in all animals. CT fluoroscopy permitted the continuous online monitoring of stent deployment. In all animals, the grafts were placed without impairment of renal artery flow or stent-graft dislocation. The mean distance between the stent-graft and origin of the more caudal renal artery was 0.9+/-0.3 mm., Conclusion: CO(2)-enhanced CT fluoroscopy permits the precise placement of infrarenal aortic stent-grafts in an animal model.

Purpose: Evaluation of the enhancing or protecting effects of different fluids during microwave ablation (MWA)., Materials and Methods: 3 samples of 17 different fluids (each 20 ml) were heated using MWA at power levels of 10, 20, 30, 40 and 45 watts. Energy was applied until the temperature reached 80 degrees C or the duration of heating exceeded 10 minutes. The cooling-down process was then observed until the temperature reached 30 degrees C., Results: Gastrografin needed the shortest time to be heated up to 80 degrees C (370 sec), followed by Magnograf (410 sec) and HES 10 % (420 sec). The least heatable fluids were Lipiodol (10 min -54.5 +/- 2.43 degrees C), distilled water (10 min -56 +/- 2.42 degrees C) and Glucose 5 % solution (10 min -56.6 +/- 1.69 degrees C). Fluids which could not be heated well, such as distilled water, Lipiodol or Glucose 5 % solution, had a small slope of the temperature curve as a function of the power level used (m = 0.60 - 0.73), whereas fluids which could be heated well, such as Gastrografin, Magnograf and HES 10 %, had a much steeper slope of the temperature curve as a function of the power level (m = 0.99 - 1.20). With respect to the maximum temperature, the above mentioned groups differed significantly (p < 0.05). The temperature slope correlated strongly with maximum temperatures reached (Pearson correlation coefficient: 0.97)., Conclusion: By additionally administering a carefully chosen fluid, enhancing or protecting effects during microwave ablation can be observed. Especially Gastrografin, Magnograf and HES 10 % can be used to enhance ablation effects, whereas protective effects can be observed particularly when using Lipiodol, distilled water and Glucose 5 %-solution., (Georg Thieme Verlag KG Stuttgart New York.)

Background: Retrotransposition of mRNA transcripts gives occasionally rise to functional retrogenes. Through acquiring tempero-spatial expression patterns distinct from their parental genes and/or functional mutations in their coding sequences, such retrogenes may in principle reshape signalling networks., Results: Here we present evidence for such a scenario, involving retrogenes of Rap1 belonging to the Ras family of small GTPases. We identified two murine and one human-specific retrogene of Rap1A and Rap1B, which encode proteins that differ by only a few amino acids from their parental Rap1 proteins. Markedly, human hRap1B-retro and mouse mRap1A-retro1 acquired mutations in the 12th and 59th amino acids, respectively, corresponding to residues mutated in constitutively active oncogenic Ras proteins. Statistical and structural analyses support a functional evolution scenario, where Rap1 isoforms of retrogenic origin are functionally distinct from their parental proteins. Indeed, all retrogene-encoded GTPases have an increased GTP/GDP binding ratio in vivo, indicating that their conformations resemble that of active GTP-bound Rap1. We furthermore demonstrate that these three Rap1 isoforms exhibit distinct affinities for the Ras-binding domain of RalGDS. Finally, when tested for their capacity to induce key cellular processes like integrin-mediated cell adhesion or cell spreading, marked differences are seen., Conclusions: Together, these data lend strong support for an evolution scenario, where retrotransposition and subsequent mutation events generated species-specific Rap1 isoforms with differential signaling potential. Expression of the constitutively active human Rap1B-retro in cells like those derived from Ramos Burkitt's lymphoma and bone marrow from a patient with myelodysplastic syndrome (MDS) warrants further investigation into its role in disease development.

Purpose: To investigate the potential of multi-slice computed tomography (MSCT) as a tool for non-invasive temperature measurement., Materials and Methods: Samples of water, 0.9% saline, sunflower oil and dilutions of (1:32, 1:64, 1:128) contrast agent (Iopromid 370, BayerSchering Pharma, Berlin) were heated in a plexiglass phantom. In a first set-up, samples of 0.9% saline solution were scanned at defined temperatures (25 degrees -75 degrees C; 5 degrees C intervals) using a clinical CT scanner. Scan parameters (tube current-time product, tube voltage, collimation, slice thickness) were systematically varied. In a second set-up samples of the different fluids (water, sunflower oil, contrast agent dilutions) were scanned using the following scan protocol: 250 mAs, 140 kV, 1.2 mm collimation, 9.6 mm slice thickness. CT numbers were measured in reconstructed axial images at the different temperatures. A regression analysis was performed to investigate the relationship between temperature and CT number., Results: Standard deviation of measured CT numbers decreased with increasing tube current-time product, increasing tube voltage, thicker collimation and higher slice thickness. Regression analysis showed an inverse relationship between temperature and CT number for all fluids with regression coefficients of -0.471 (0.9% saline), 0.447 (water), -0.679 (sunflower oil), -0.420 (contrast agent 1:32), -0.414 (contrast agent 1:64) and -0.441 (contrast agent 1:128), respectively., Conclusion: Multi-slice computed tomography can depict thermal density expansion of different fluids. Based on these results the implementation of a temperature discrimination of several degrees C at a high spatial resolution is achievable.

Over the last few years, percutaneous radiofrequency (RF) ablation has been successfully established as a viable treatment modality for small peripheral renal cell carcinoma (RCC). This technique is limited by central tumor location and tumor size. We report the interventional management of a 5.3 cm mixed RCC with central and exophytic parts by combining the RF ablation with embolization, tagging, and retrograde, as well as anterograde cooling. The potential pitfalls of complex hybrid interventions for treating RCC are discussed.

Purpose: Osteoid osteoma is a benign primary bone tumor that typically occurs in children and young adults. Besides local pain, which is often worse at night, prompt relief due to medication with acetylsalicylic acid (ASS) is characteristic for this bone lesion. Because long-term medication with ASS does not represent an alternative treatment strategy due to its potentially severe side effects, different minimally invasive image-guided techniques for the therapy of osteoid osteoma have been developed. In this context radiofrequency (RF) ablation in particular has become part of the clinical routine. The technique and results of image-guided RF ablation are compared to alternative treatment strategies., Materials and Methods: Using this technique, an often needle-shaped RF applicator is percutaneously placed into the tumor under image guidance. Then a high-frequency alternating current is applied by the tip of the applicator which leads to ionic motion within the tissue resulting in local heat development and thus in thermal destruction of the surrounding tissue including the tumor., Results: The published primary and secondary success rates of this technique are 87 and 83 %, respectively. Surgical resection and open curettage show comparable success rates but are associated with higher complication rates. In addition image-guided RF ablation of osteoid osteomas is associated with low costs., Conclusion: In conclusion image-guided RF ablation can be considered the gold standard for the treatment of osteoid osteoma.

Background: Long interspersed nuclear elements (LINE-1s, L1s) have been recently implicated in the regulation of mammalian transcriptomes., Results: Here, we show that members of the three active mouse L1 subfamilies (A, GF and TF) contain, in addition to those on their sense strands, conserved functional splice sites on their antisense strands, which trigger multiple exonization events. The latter is particularly intriguing in the light of the strong antisense orientation bias of intronic L1s, implying that the toleration of antisense insertions results in an increased potential for exonization., Conclusion: In a genome-wide analysis, we have uncovered evidence suggesting that the mobility of the large number of retrotransposition-competent mouse L1s (approximately 2400 potentially active L1s in NCBIm35) has significant potential to shape the mouse transcriptome by continuously generating insertions into transcriptional units.

L1Base is a dedicated database containing putatively active LINE-1 (L1) insertions residing in human and rodent genomes that are as follows: (i) intact in the two open reading frames (ORFs), full-length L1s (FLI-L1s) and (ii) intact ORF2 but disrupted ORF1 (ORF2-L1s). In addition, due to their regulatory potential, the full-length (>6000 bp) non-intact L1s (FLnI-L1s) were also included in the database. Application of a novel annotation methodology, L1Xplorer, allowed in-depth annotation of functional sequence features important for L1 activity, such as transcription factor binding sites and amino acid residues.
