Background: Despite depressive disorders being very common there has been little research to guide primary care physicians on the choice of treatment for patients with mild to moderate depression., Aims: To evaluate the efficacy of interpersonal counselling compared with selective serotonin reuptake inhibitors (SSRIs), in primary care attenders with major depression and to identify moderators of treatment outcome., Method: A randomised controlled trial in nine centres (DEPICS, Australian New Zealand Clinical Trials Registry number: ACTRN12608000479303). The primary outcome was remission of the depressive episode (defined as a Hamilton Rating Scale for Depression score ≤7 at 2 months). Daily functioning was assessed using the Work and Social Adjustment Scale. Logistic regression models were used to identify moderators of treatment outcome., Results: The percentage of patients who achieved remission at 2 months was significantly higher in the interpersonal counselling group compared with the SSRI group (58.7% v. 45.1%, P = 0.021). Five moderators of treatment outcome were found: depression severity, functional impairment, anxiety comorbidity, previous depressive episodes and smoking habit., Conclusions: We identified some patient characteristics predicting a differential outcome with pharmacological and psychological interventions. Should our results be confirmed in future studies, these characteristics will help clinicians to define criteria for first-line treatment of depression targeted to patients' characteristics.

Context: It is estimated that approximately one in four people know someone who has taken their own life and that one suicide death leaves six or more suicide survivors., Aims: The aim of this paper was to review the literature regarding the association between suicide and bereavement, focusing also on the supportive and therapeutic resources available for survivors., Materials and Methods: Careful MedLine and PsycINFO searches for the period 1980-2013., Results: The review of the literature indicates that emotional turmoil in suicide survivors may last a long time and, in some cases, may end with their own suicide., Conclusion: Future research should evaluate the efficacy of professional treatments and of support groups targeting suicide survivors., Practice Implications: It is crucial to understand the bereavement process after the suicide of a significant other in order to provide proper care, reduce stigma, and improve the outcomes of related psychiatric conditions.

This article describes the result of a study focusing on the keratinisation degree of rumen mucosa and changes in the body condition of sheep that had grazed for 20 days in a pasture densely covered with Brachypodium rupestre. Grazing in this type of pasture can reduce the probability of fires in a Mediterranean mountain setting. However, foraging in areas with a prevalence of Brachypodium rupestre can affect animals' well being. In this respect, it is essential to determine the length of time during which the animals can remain in this environment before their welfare is compromised by this type of pasture. Ewes grazing on a semi-mesophilic pasture were included as a control. On days 1, 10, and 20, five ewes from each group were sacrificed to evaluate the variations of the epithelial keratinization degree of the rumen atrium and ventral sac. Body weight (BW) and body condition score (BCS) were assessed in ten ewes per group. The control animals showed little variation in the keratinisation degree of rumen mucosa without any detrimental effects on the BCS and BW. The experimental animals showed a significant increase in the epithelial keratinisation degree within 10 days and a decrease of BCS and BW within 20 days. The data collected suggest that animals should not remain for longer than 10-12 days on pasture highly covered with Brachypodium rupestre.

This study aimed at validating two new assessment tools, the Stigma of Suicide Attempt (STOSA) scale and the Stigma of Suicide and Suicide Survivor (STOSASS) scale. The Devaluation-Discrimination scale of Link et al. was translated into Italian and adapted to measure stigma towards suicidal behavior. Both scales were administered to a mixed sample including members of the general population (n=282), patients with a mental disorder (n=113), suicide attempters (n=57) and people who had lost a significant other to suicide (n=75). Reliability of the scales was good in terms of both internal coherence and test-retest stability. Factor analysis produced an acceptable solution for the STOSA-scale. Items were distributed into two factors, one grouping items to measure supportive, respectful and caring attitudes, the other factor grouping items oriented towards stigmatizing attitudes and beliefs. The clinical populations were more inclined towards stigmatization of suicide than were people from the general population, who might be less aware of the stigma attached to suicide. The two scales may be helpful to quantify stigma at individual level in order to provide targeted supportive interventions, and at population level to measure changes in the beliefs and attitudes of the general population., (Copyright © 2012 Elsevier Ireland Ltd. Objective: To determine glycohistochemical characteristics of enzootic nasal tumors (ENTs) of sheep, compare results for ENT with those of histologically normal nasal mucosa of sheep, and identify the histologic origin of ENT., Sample: ENT and nasal mucosa samples obtained from cadavers of 5 adult Lacaune sheep with ENT and 5 Lacaune sheep unaffected by ENT, respectively., Procedures: Samples of ENT and nasal mucosa were collected from cadavers of sheep and sectioned. Conventional and lectin histochemical analyses were used to identify glycoconjugates in tissue sections on the basis of their principal chemical groups and principal terminal or internal oligosaccharidic glucidic residues, respectively., Results: ENTs had papillary and tubular portions. Cells in the papillary portion of ENTs had secretion and surface glycoconjugates, which included sulfated glycosaminoglycans and neutral and sialilated glycoproteins. Cells in the tubular portion of ENTs had surface glycoconjugates, which included neutral and sialilated glycoproteins. Both portions of ENTs had C(4)-acetylated sialoderivatives that were not detected in sections of histologically normal nasal mucosa., Conclusions and Clinical Relevance: The papillary portion of ENTs in sheep may originate from respiratory glands and goblet cells. The tubular portion of ENTs in sheep may originate from olfactory glands. Presence of C(4)-acetylated sialoderivatives in cells of ENTs could confer resistance against pathogens to those cells.

Although the Geographical Information System (GIS), which integrates computerized drawing computer assisted design (CAD) and relational databases (data base management system (DBMS)), is best known for applications in geographical and planning cartography, it can also use many kinds of information concerning the territory. A multidisciplinary project was initiated since 5 years a multidisciplinary study was initiated to use GIS to integrate environmental and ecological data with findings on animal health, ethology, and anatomy. This study is chiefly aimed at comparing two different methods for measuring the absorptive surface of rumen papillae. To this scope, 21 female sheep (Ovis aries) on different alimentary regimes (e.g., milk and forage mixed diet, early herbaceous diet, dry hay diet, and fresh hay diet at the maximum of pasture flowering and at the maximum of pasture dryness) were used; after slaughtering, 20 papillae were randomly removed from each sample collected from four indicator regions of rumen wall, placed near a metric reference and digitally photographed. The images were developed with the ArcGIS™ software to calculate the area of rumen papillae by means of GIS and to measure their mid-level width and length to calculate the papillae area as previously performed with a different method. Spatial measurements were analyzed using univariate and multivariate methods. This work demonstrates that the GIS methodology can be efficiently used for measuring the absorptive surface of rumen papillae. Background: Multi-element interventions for first-episode psychosis (FEP) are promising, but have mostly been conducted in non-epidemiologically representative samples, thereby raising the risk of underestimating the complexities involved in treating FEP in 'real-world' services., Methods/design: The Psychosis early Intervention and Assessment of Needs and Outcome (PIANO) trial is part of a larger research program (Genetics, Endophenotypes and Treatment: Understanding early Psychosis - GET UP) which aims to compare, at 9 months, the effectiveness of a multi-component psychosocial intervention versus treatment as usual (TAU) in a large epidemiologically based cohort of patients with FEP and their family members recruited from all public community mental health centers (CMHCs) located in two entire regions of Italy (Veneto and Emilia Romagna), and in the cities of Florence, Milan and Bolzano. The GET UP PIANO trial has a pragmatic cluster randomized controlled design. The randomized units (clusters) are the CMHCs, and the units of observation are the centers' patients and their family members. Patients in the experimental group will receive TAU plus: 1) cognitive behavioral therapy sessions, 2) psycho-educational sessions for family members, and 3) case management. Patient enrollment will take place over a 1-year period. Several psychopathological, psychological, functioning, and service use variables will be assessed at baseline and follow-up. The primary outcomes are: 1) change from baseline to follow-up in positive and negative symptoms' severity and subjective appraisal; 2) relapse occurrences between baseline and follow-up, that is, episodes resulting in admission and/or any case-note records of re-emergence of positive psychotic symptoms. The expected number of recruited patients is about 400, and that of relatives about 300. Owing to the implementation of the intervention at the CMHC level, the blinding of patients, clinicians, and raters is not possible, but every effort will be made to preserve the independency of the raters. We expect that this study will generate evidence on the best treatments for FEP, and will identify barriers that may hinder its feasibility in 'real-world' clinical settings, patient/family conditions that may render this intervention ineffective or inappropriate, and clinical, psychological, environmental, and service organization predictors of treatment effectiveness, compliance, and service satisfaction.

Aim: The aim of this study is to describe the emotional reactions experienced by a group of psychiatrists following the suicidal death of a patient., Methods: The study was carried out between February 2005 and January 2006 within the Mental Health Department of Padova. Data were collected through a questionnaire administered to the reference psychiatrist at least two weeks after the suicide of a patient. The aim of the tool was to assess emotional reaction when told about the suicide and when filling in the questionnaire., Results: All the interviewed psychiatrists (n = 11) reported emotions, mostly self-doubt, sadness, disbelief, guilt, fear, shock and helplessness. Some differences were found between residents and consultant psychiatrists. Of the 11 psychiatrists, 7 did not blame anyone for their patient's death, and 6 of them judged the suicide to have been unpreventable. All the therapists sought conversation about the event; a team case review was conducted in 8 cases., Discussion: A patient's suicide is a significant event in a psychiatrist's professional life, particularly among resident psychiatrists. The emotional process only partly overlaps the usual grief process. Discussions with peer/senior colleagues and team case reviews are helpful tools in the grief process of psychiatrists and should become part of the working practice of every Mental Health Department.

Background: To date, few randomized controlled trials (RCTs) of major depression have examined suicidal ideation as an outcome measure. Our aim is to determine the incidence of treatment-emergent suicidal ideation (ESI) and behaviors during the acute phase of treatment with an SSRI antidepressant or interpersonal psychotherapy (IPT) in patients with unipolar major depression., Methods: In a two-site RCT, 291 adult outpatients with nonpsychotic major depression and a Hamilton Depression Rating Scale (HDRS) score ≥15 were randomly allocated to IPT or SSRI. Participants who did not remit with monotherapy received augmentation with the other treatment. ESI was defined as a post-baseline HDRS suicidality item score ≥2 or a post-baseline Quick Inventory of Depressive Symptomatology (QIDS) score ≥2 in patients with a baseline score ≤1., Results: Of the 231 participants who had no suicidal ideation at baseline, 32 (13.8%) subsequently exhibited ESI on at least one post-baseline visit. Time to suicidal ideation was significantly longer in patients allocated to SSRI compared to those allocated to IPT (HR = 2.21, 95% CI 1.04-4.66, P = .038), even after controlling for treatment augmentation, benzodiazepine use, and comorbidity with anxiety disorders. Worsening of suicidal ideation occurred in 7/60 patients who had suicidal ideation at baseline. In the large majority of cases, suicidal ideation was successfully managed with the study protocol., Conclusions: In the context of careful monitoring and frequent contact, selective serotonin reuptake inhibitor (SSRI) was associated with a lower risk of ESI than IPT and both SSRI and IPT appeared to be safe treatments for patients with past suicide attempts, none of whom exhibited ESI during the study., (© 2011 Wiley-Liss, Inc.)

Background: Although many studies suggest that, on average, depression-specific psychotherapy and antidepressant pharmacotherapy are efficacious, we know relatively little about which patients are more likely to respond to one versus the other. We sought to determine whether measures of spectrum psychopathology are useful in deciding which patients with unipolar depression should receive pharmacotherapy versus depression-specific psychotherapy., Method: A total of 318 adult out-patients with major depression were randomly assigned to escitalopram pharmacotherapy or interpersonal psychotherapy (IPT) at academic medical centers at Pittsburgh, Pennsylvania and Pisa, Italy. Our main focus was on predictors and moderators of time to remission on monotherapy at 12 weeks., Results: Participants with higher scores on the need for medical reassurance factor of the Panic-Agoraphobic Spectrum Self-Report (PAS-SR) had more rapid remission with IPT and those with lower scores on the psychomotor activation factor of the Mood Spectrum Self-Report (MOODS-SR) experienced more rapid remission with selective serotonin reuptake inhibitor (SSRI) pharmacotherapy. Non-specific predictors of longer time to remission with monotherapy included several panic spectrum and mood spectrum factors and the Social Phobia Spectrum (SHY) total score. Higher baseline scores on the 17- and 25-item Hamilton Depression Rating Scales (HAMD-17 and HAMD-25) and the Work and Social Adjustment Scale (WSAS) also predicted a longer time to remission, whereas being married predicted a shorter time to remission., Conclusions: This exploratory study identified several non-specific predictors but few moderators of psychotherapy versus pharmacotherapy outcome. It offers useful indicators of the characteristics of patients that are generally difficult to treat, but only limited guidance as to who benefits from IPT versus SSRI pharmacotherapy.

Mammalian aquaporins (AQPs) are a family of at least 13 integral membrane proteins expressed in various epithelia, where they function as channels to permeate water and small solutes. AQP5 is widely expressed in the exocrine gland where it is likely involved in providing an appropriate amount of fluid to be secreted with granular contents. As regards AQP5 expression in the salivary glands, literature is lacking concerning domestic animal species. This study was chiefly aimed at immunohistochemically investigating the presence and localization of AQP5 in sheep mandibular and parotid glands. In addition, AQP5 immunoreactivity was comparatively evaluated in animals fed with forage containing different amounts of water related to the pasture vegetative cycle, in order to shed light on the possible response of the gland to environmental modifications. Moderate AQP5-immunoreactivity was shown at the level of the lateral surface of mandibular serous demilune cells, not affected by the pasture vegetative cycle or water content. On the contrary, the parotid gland arcinar cells showed AQP5-immunoreactivity at the level of apical and lateral plasma membrane, which was slight to very strong, according to the pasture vegetative development and interannual climatic variations. AQP5 expression is likely due to its involvement in providing appropriate saliva fluidity. Indeed, the lowest AQP5 immunoreactivity was noticed when food water content increased.

Background: Depression is a frequently observed and disabling condition in primary care, mainly treated by Primary Care Physicians with antidepressant drugs. Psychological interventions are recommended as first-line treatment by the most authoritative international guidelines but few evidences are available on their efficacy and effectiveness for mild depression., Methods/design: This multi-center randomized controlled trial was conducted in 9 Italian centres with the aim to compare the efficacy of Inter-Personal Counseling, a brief structured psychological intervention, to that of Selective Serotonin Reuptake Inhibitors. Patients with depressive symptoms referred by Primary Care Physicians to psychiatric consultation-liaison services were eligible for the study if they met the DSM-IV criteria for major depression, had a score ≥ 13 on the 21-item Hamilton Depression Rating Scale, and were at their first or second depressive episode. The primary outcome was remission of depressive symptoms at 2-months, defined as a HDRS score ≤ 7. Secondary outcome measures were improvement in global functioning and recurrence of depressive symptoms at 12-months. Patients who did not respond to Inter-Personal Counseling or Selective Serotonin Reuptake Inhibitors at 2-months received augmentation with the other treatment., Discussion: This trial addresses some of the shortcomings of existing trials targeting major depression in primary care by evaluating the comparative efficacy of a brief psychological intervention that could be easily disseminated, by including a sample of patients with mild/moderate depression and by using different outcome measures., Trial Registration: Australian New Zealand Clinical Trials Registry ACTRN12608000479303.

A large body of information proves that Orexin-A is present in the pancreatic endocrine cells of humans and laboratory animals; more detailed studies identify Orexin-A-immunopositive cells as beta cells. Because no data have been reported on the pancreas of domestic animals, we investigated the presence and the distribution of cells containing Orexin-A in the pancreas of cattle, sheep and pigs by means of immunohistochemical techniques. Using a polyclonal antibody against Orexin-A, the immunopositive reaction was identified in the cytoplasm of many insular cells in the three species studied. Double immunohistochemical staining, using a polyclonal anti-insulin antibody, showed that Orexin-A is co-expressed with insulin. Our results, besides showing the presence of Orexin-A in the endocrine pancreas of domestic animals, together with data present in the literature, could contribute to the understanding of complex mechanisms regulating the functionality of the endocrine pancreas in domestic animals., (2010 Elsevier Ltd. This study characterised the composition of surface and secretion complex carbohydrates in the alimentary tract of organically and intensively housed chickens. Histochemical labelling was carried out on samples of oesophagus, glandular stomach and proximal duodenum using (1) alcian blue (AB) at pH 2.5 combined with neuraminidase digestion (Sial) with or without saponification (KOH), (2) AB at pH 1 and 0.5, (3) periodic acid-Schiff (PAS), (4) a combined AB-PAS stain, and (5) both low and high iron diamine stains combined with enzymatic treatments specific for glycosaminoglycans. Semi-quantitative scoring of the histochemical staining indicated abundant acidic sulfated glycoconjugates in each alimentary tract region, in addition to the presence of sialoglycoderivatives in the oesophagus. These complex carbohydrate components are likely to have various functions including the provision of a protective visco-elastic barrier and a role in the defence against specific pathogens. Aims: The aim of this survey was to describe patients in care at a large mental health department in northern Italy who attempted suicide, and the clinical management adopted by their psychiatrists before the event., Methods: Data collection was based on a questionnaire administered to the reference psychiatrists., Results: Over a period of 12 months, 166 catchment area residents attempted suicide. Sixty-six (40%) had contacted the mental health department in the previous two years and completed data were obtained on 63. Twenty-nine (46%) suffered from mood, 26 (41%) from personality and 11 (18%) from schizophrenic disorders. Thirty-four attempts occurred within one year of psychiatric ward discharge, mostly in the first quarter. The reference psychiatrists reported that, at the last evaluation, 38 of 63 patients (60%) presented no change in clinical conditions, and 41 of 63 (68%) were considered at no immediate risk of suicide. Most of the attempted suicides in question (45, 72%) were judged to be unpreventable. In the two logistic regression analyses carried out, no independent variables were able to statistically significantly explain the variance in judged suicidal risk or the preventability of the index attempted suicide., Conclusions: According to the psychiatrists' descriptions of their last contact with the patients, most attempted suicides have not been preceded by a change in clinical conditions. Moreover, psychiatrists, irrespective of their age and gender, and of patient diagnosis, frequently judged the attempts to have been unpreventable.

Recent studies have shown that suicides are at least as frequent among Nursing Home (NH) residents as they are among older people in the general population. The objective of the present study was to evaluate the prevalence of death- and/or suicidal feelings, thoughts, and plans and any attempted suicides in a random, unselected sample (N=288) of individuals aged 65-years-and-over, living in NHs located in the Veneto Region (the Italian North-East). One hundred seventy-two participants were surveyed with a response rate of 59.7%. Five sample questions were asked to investigate the presence of death- and suicidal thoughts, plans and behaviors in different time periods. Among the NH residents, 30.8% admitted having had death or suicidal thoughts or plans during the month prior to the interview. The oldest-old residents (85+y) more frequently reported death-suicide ideation. This high frequency of death and/or suicidal feelings and thoughts among older NH residents should be carefully considered when planning and implementing health care programs in these facilities.

Interpersonal Psychotherapy was conceived as a psychotherapeutic approach to treating Depression, but has since now been broadly used to treat many other disorders. It has also recently been adapted to delivery in the group setting, with both advantages and disadvantages as other group psychotherapies. This adaptation, first used by Wilfley and colleagues, maintained the main features of IPT, i.e. the central role of the interpersonal focus and identification of one (or two) out of four problem interpersonal areas (Grief; Role Transition; Role Disputes; Interpersonal Deficits). It also kept the active role of the therapist and individual patient within the group. To date, G-IPT has been used for several diseases (Eating Disorders not Otherwise Specified, Bulimia Nervosa, Depressive Disorder, Posttraumatic Stress Disorder) and several populations (Adolescents, Older People, Pregnant Women or "New-Mothers," Substance-abusing Female Prisoners). Although the overall quality of most of outcome studies is to date quite poor, the review of the current state of knowledge shows the G-IPT may be helpful and present a number of advantages to treat different psychiatric disorders in several populations of patients.

This study was performed to identify the equine respiratory tract areas which express the specific receptor for equine influenza virus; findings may be useful to provide new ways to treat the infectious disease. The present work aims to visualize in situ the presence of sialoderivatives in the horse respiratory tract in order to localize sialoderivatives acting as influenza virus receptors. To this purpose, nasal mucosae, trachea, bronchus and lung parenchyma were removed from 8 mature horses of both sexes. We performed sialic acid characterization by means of mild and strong periodate oxidation and saponification, combined with lectin histochemistry and sialidase digestion, in addition to the direct evidentiation of sialic acid residues. No differences were shown between sexes. Sialic acid residues are present in the nasal mucous cell secretion, where they are linked to galactose by means of alpha2-3 linkage and are mainly C9 acetylated, and in the nasal and tracheal epithelial lining, where they are represented by periodate labile residues (alpha2-3)- and/or (alpha2-6)- linked to galactose. Specific receptors for equine influenza viruses are present at the nasal and tracheal epithelial lining cell coat levels, and in some trachea epithelial cells, but the horse possesses a preventive defence, which consists of the secretion of a mucous layer at nasal level, which could specifically inactivate the hemagglutinins of equine influenza virus; in addition, it expresses other sialoreceptors which can mask the influenza specific ones.

Background: To date, no cross-national RCT has addressed the mechanisms underlying the relative success of pharmacological and psychotherapeutic interventions for depression. A multi-site clinical trial that includes psychotherapy as one of the treatments presents numerous challenges related to cross-site consistency and communication., Purpose: This report describes how those challenges were met in the study "Depression: The Search for Treatment Relevant Phenotypes'', being carried out at the University of Pittsburgh and the University of Pisa, Italy., Methods: Implementing the study required the investigators to address methodological and practical challenges related to the different requirements of the two Institutional Review Boards (IRBs), psychotherapy training, independent evaluator training, patient recruitment, development of common tools for data entry, quality control and generation of weekly reports of patient progress as well as establishing a similar clinical and research framework in two countries with substantially different health care systems., Results: By having bilingual investigators and staff members who spent time at one another's sites, making use of frequent conference-call staff meetings and being flexible within the bounds of the sometimes contradictory requirements of the IRBs, the investigators were able to meet the human subjects protection requirements of both institutions, surmount language barriers to consistent therapist and evaluator training and develop common tools for study management. As a result, recruitment goals were met at both sites and retention rates were high. One instance of inconsistent implementation of the protocol was corrected within the first year., Limitations: This study was conducted in two Western cultures by researchers with long-standing collaboration. Our findings may not be generalizable to other countries or research settings., Conclusions: The implementation of a cross-national protocol and the adoption and maintenance of common procedures is possible when investigators are aware of the challenges this may present and are proactive in trying to address them.

The morphometric variations of the rumen papillae due to different alimentary diets has been analysed using a geographic information system (GIS), as the preliminary stage of a wider study aimed at creating a geo-database to link environmental data (pasture structure and composition, pastoral value) with parameters measuring animal welfare (body condition score, volatile fatty acids concentration, haematochemical profile) both during a pasture vegetative cycle and in different conditions of animal load on pastures, with the ultimate goal of contributing to grassland management. A first step was to collect samples of rumen wall tissue from different groups of sheep (lambs to milky and mixed diet, and adult at the maximum of flowering and at the end of pasture vegetative cycle) to verify morphometric differences in rumen papillae due to different diets. Wall tissues of rumen samples were removed from the dorsal and ventral sac and preserved in a formalin solution. Twenty papillae from the dorsal and ventral sac were taken from each sample and their images were elaborated with ArcGISTM software. Results show that the morphometric variation of papillae is related with the pasture productivity trend: the maximum size of rumen papillae occurs immediately after the phytomass and flowering spike; in this phase the animals utilise a very nourishing and quantitatively abundant pasture. After this phase, a deterioration of pasture occurs and the surface of rumen papillae surface decreases rapidly. Results obtained further confirm the existence of a close relationship between quality and quantity of phytomass and the extent of rumen papillae absorptive surface, demonstrating the effects of this relationship during a pasture vegetative cycle.

Background: Numerous studies indicate that stressful life events are key precipitants of psychological disturbances. Severe stress often precedes the onset or exacerbation of illness in vulnerable individuals and may be of primary importance in the genesis of some mental disorders. Several authors have suggested that major life events play a role in the development of panic-related symptoms. The aim of this study was to examine the presence of stressful life events, in particular events focused in the interpersonal psychotherapy problem areas (grief, role disputes, role transitions, interpersonal deficits), in patients suffering from panic disorder., Methods: We interviewed 55 patients suffering from panic disorder, with or without agoraphobia, in accordance with the diagnostic criteria specified in MINI PLUS. The panic attack profile was evaluated with the Panic Attack and Anticipatory Anxiety Scale. We assessed the ability to adapt to and derive satisfaction from the social environment with the Social Adjustment Scale Self Report and interpersonal problems with Interpersonal Questionnaire., Results: Using the Interpersonal Questionnaire we found that all subjects had had relevant interpersonal problems in the year preceding the onset of PD: 92.7% had experienced a role transition, 85.5% interpersonal deficits, 74.5% a role dispute and 38.2% had suffered the loss of a relative or significant other. These results were confirmed by Paykel's scale, on which the whole sample reported a high frequency of life events in the 6 months before onset of illness. These preliminary data suggest a rationale for the therapeutic strategies of interpersonal psychotherapy in individuals with panic disorder.

Reasons for Performing Study: Sialic acids modulate the metabolite transport across membranes and may be involved in protection against pathogenic agents. The presence of sialoderivatives in the equine mandibular gland requires further study., Objective: To biochemically visualise in situ the presence of sialoderivatives, by means of mild and strong periodate oxidation and alcoholic saponification, combined with lectin histochemistry and sialidase digestion in order to hypothesise roles for detached sialoderivatives., Methods: Mandibular glands were removed from 8 mature horses of both sexes and subjected to histochemical procedures, including periodate oxidation, saponfication and lectin staining. Controls were based upon the omission of peroxidase-conjugated lectins and respective enzyme-free buffers., Results: The reactivities of PNA and RCA I lectins were affected by sialidase treatment, whether preceded by saponification or not, showing that the dimer N-acetyl-sialic acid-beta-Gal was linked (1-3)GalNAc and (1-4)GlcNAc. In acinar cells the sequence sialic acid-beta-Gal(1-3)GalNAc showed sialic residues acetylated at C4 only and at C4 and C7 and/or C8 and/or C9(alpha2-6Gal) in both sexes, while in female mandibular gland also C4 and C9(alpha2-3Gal) acetylated residues were present. Sialic acid linked to beta-Gal(1-4)GlcNAc was prevalently C4 and C7 and/or C8 and/or C9(alpha2-6Gal and alpha2-3Gal) acetylated, whereas only a minor quantity showed acetyl groups at C7 and/or C8 and/or C9(alpha2-6Gal) in the acinar cells of both sexes., Conclusions: The great variety of sialic acid residues expressed by equine mandibular gland could assume an important role in the defensive mechanisms towards pathogen agents and, compared with those of cattle, probably represents an example of molecular species-specificity related to different alimentary habits.

This study assesses the efficacy of an educational and dietary approach in preventing olanzapine-induced weight gain. Eighteen patients affected by schizophrenic disorders were treated with olanzapine and weighed twice-weekly for 24 weeks. A psychoeducational intervention and referral to a nutritionist was introduced from the beginning of olanzapine treatment in 9 patients, and from the 9th week of therapy in 8 patients. Results showed that after 8 weeks of olanzapine treatment, weight gain was contained in the subjects receiving intervention unlike patients without preventive intervention (+0.99+/-3.34 kg vs. +2.96+/-3.08 kg; p<.03). At the end of the trial these patients partly shed their gain (-1.77 kg), presenting a final weight which was not significantly different from baseline (+1.19 kg). Subjects receiving the psychoeducational approach from the beginning were significantly heavier than at baseline (+3.4 kg). Poor dietary compliance correlated significantly with an increase in body weight, while higher mean dosages of olanzapine correlated with better weight-gain control.

Objective: To assess suicide and attempted suicide (AS) rates and their characteristics among older residents in Nursing Homes (NHs) of the Veneto region, in the North-East of Italy., Method: The complete list of NHs was provided by the Regional Department of Health. Structured interviews were completed with NH managers, inquiring about NH, staff and management characteristics, mental health care available and the number of completed and attempted suicides (AS) in 2001 made by residents older than 65 years, occurring either inside or outside NHs. All facilities which reported suicidal events were asked additional information., Results: In the study period, five completed suicides and eight AS were reported, i.e. a rate of 18.6/100,000 and of 29.7/100,000 respectively. All but one suicides and one AS had a history of mental disorders. Seven subjects had been living in a NH for less than one year. There were no significant differences in the frequency of suicidal events between the facilities which employed or did not employ mental health workers., Conclusion: The suicide rate found in this facility sample is much higher than the rate reported by the Italian National Statistic Institute for the over-65-year-old population of the Veneto Region in 2001, and is similar to the rate reported in a previous study conducted in another country. In Veneto NHs behavioural control of residents, lack of access to a variety of means used for suicidal purposes and medical supervision does not seem to have protected the NH population from suicidal risks., (Copyright (c) 2006 John Wiley & Sons, Ltd.)

Objective: To investigate why elderly persons moved to a nursing home and detect any change in their psychopathological and cognitive profile, dependency and perceived QOL., Method: We considered 100 elderly consecutively admitted to a nursing home, within 1 week of admission and 6 months later. Data were collected from medical and nursing records (medical and psychiatric history, ADL), interviews (MMSE, reasons for admission) and self-report instruments (GDS, BSI, WHOQOL-brief)., Results: At admission, 68 subjects were able to participate in the survey and agreed to take part. Only 5.9% (n = 4) had moved to the nursing home by their own choice, 26.5% (n = 18) because they were lonely and 36.7% (n = 25) because they had no caregiver available. The reasons for admission reported by the interviewed residents only partly corresponded with the ones noted in their records. Twenty-six subjects had an MMSE score >or= 18 and agreed to undertake complete assessment. The scores of 20 of the 26 subjects exceeded the cut-off on the GDS and five of the nine BSI subscales. During the follow-up period, 19 of the 68 originally assessed residents died (33 of the entire sample of 100) and one refused to continue to take part in the survey. The mean MMSE and ADL scores of the 48 survivors decreased from 16.87 (SD +/- 7.32) to 14.27 (SD +/- 7.24; t = 4.89, p < 0.000) and from 8.22 (SD +/- 3.55) to 6.39 (SD +/- 3.56; t = 5.34, p < 0.000), respectively. The scores achieved on GDI and BSI subscales worsened in 20 survivors with MMSE >or= 18. The score achieved on the QOL physical health domain also worsened. Comparison of the subjects who died and survived during the follow-up period showed a significant difference in terms of ADL (6.60 SD +/- 4.221 vs 3.64 SD +/- 3.773 F = 11.639; p < 0.001)., Conclusion: The 68 original participants expressed feelings of loneliness and marginalization, but these experiences were not noted in their personal records. Most subjects presented psychiatric symptoms and cognitive decline. After 6 months, the clinical condition of the sample, particularly the females, had worsened, with a 33% mortality rate. This suggests that moving to a nursing home did not bring about improvement or stabilization; rather, psychiatric symptoms worsened and quality of life was perceived more poorly.

Objective: To assess the quality of life (QOL) of older adults aged over 65 years, who were healthy or suffering from depressive syndrome (DS) and/or Alzheimer's disease (AD); to analyse agreement between participants' and proxies' QOL ratings; to evaluate the association between participants' depressive and cognitive symptoms and QOL rating; to correlate participants' health ratings and the severity of physician assessment., Methods: 138 non-institutionalised older people of both genders and their respective caregiver and treating doctor were consecutively recruited (response rate 74.6%). Forty suffered from AD, 36 from DS, 35 from both conditions and 27 had neither. All participants were evaluated by Mini Mental State Examination, Geriatric Depression Scale and World Health Organization Quality of Life (WHOQOL) questionnaire. The caregiver filled out QOL-Proxy and the physician filled out the 'Health and Severity of Illness' form., Results: The four groups scored significantly differently in all areas of WHOQOL-100 (WHOQOL questionnaire with 100 items). Participants with DS perceived their QOL as poorer than did healthy and AD subjects. Participants with AD and DS obtained intermediate scores. Severity of depression correlated with worsening QOL. Subjects with DS--but not those with AD, AD and DS, and, in some areas, healthy participants--had similar perception of their QOL to their proxies. Poor physical health ratings by the physician corresponded to poorly perceived QOL by the patient., Conclusion: Older people with AD perceive their own QOL similarly to and, in some areas, even better than healthy people of the same age. The opposite was observed among the depressed. Informants do not always evaluate QOL in the same way as healthy elders and those with AD, while there is more agreement with depressed patients. Informant evaluation may be helpful but is not necessarily reliable.

In Italy, postvention models specifically designed for people surviving the suicide of a loved one are very scarce. The SOPRoxi project was developed to respond to this particular lack. By involving different professional and nonprofessional figures (GPs, mental health professionals, social workers, volunteers, etc), the SOPRoxi project aims to remove the stigma associated with this condition and to offer adequate, multifaceted help to survivors.

Objective: To survey all Residential Facilities (RFs) for the elderly in five Italian Regions (Calabria, Sardinia, Sicily. Umbria and Veneto), and to evaluate their logistic and organizational characteristics, staff and residents' features., Design: Structured interviews were conducted with RF managers., Setting: All RFs operating in the five Italian Regions., Results: In the five Regions there are 853 RFs with an average of 198.0 beds per 10000 elderly inhabitants; this rate shows a marked variability between different Regions. The mean number of beds in the RFs is 59.8 (median: 34). The large majority (96%) have a 24-hour staff cover. In terms of management, the RFs are handled by local municipalities (29%), religious non-profit associations (24%), and other non-profit organizations (21%). In the 754 RFs surveyed (91.1% of the sample) there were 24.456 workers employed, even the number of staff in each facility shows a great variability. The mean number of workers directly involved in residents' care is 27.8, and the ratio residents/staff is 2.1, which becomes lower (1.8) if we consider the ratio non-independent residents/staff. In the 754 RFs there were 42,687 residents, with an average of 53.5 elderly residents for each facility and 3.2 residents below the age of 65; the mean age of the resident sample was 79.3 years, and there is an high proportion of residents with neurological (including dementia), psychiatric or medical disorders., Conclusion: There is marked variability in the provision of residential places between different Regions; many other characteristics of RFs for the elderly, including staff/residents ratios, show a similar variability Most RFs host elderly with a variety of neurological, psychiatric and medical disorders. The future waves of the project will shed light on many features of these institutions, which care for 2.1% of the elderly population in Italy and may serve larger proportions of the elderly in the future.

The aim of this study was to assess any predictive factors for repeated attempted suicide and completed suicide in a 1-year follow-up on a sample of elderly European suicide attempters (60 years and over). From 1990 to 1993, 63 subjects completed the first interview and were recontacted after 1 year. At follow-up, eight subjects (12.7%) had taken their lives and seven (11.1%) had repeated at least one suicide attempt. On comparison of repeaters and non-repeaters, differences emerged in terms of death of the father in childhood and for mean Suicidal Intent Score. At the end of follow-up period, repeaters reported a more frequent desire to repeat suicidal behaviour and judged their mental health and social assistance received to be worse. Suicides and non-repeaters differed especially in relation to death of father during childhood and number of contacts with General Practitioner. Interpretation of the results must take into account the smallness of the test sample, the difficulties in obtaining complete data for the follow-up interview, the lack of a control group and a diagnosis formulated in a hospital consultation setting. The study confirms, however, the high risk of repetition of suicidal behaviour in the elderly. In old age suicidal ideation is often sustained over long periods of time and requests for help are addressed to relatives and GPs. An interesting finding is the more frequent death of the father during childhood among repeaters.

Background: Suicidality is constituted by all those phenomena that are apparently positioned along a continuum, with the two extremes represented by death wishes and completed suicide., Objectives: The aim of this paper is to show the one-year prevalence of the phenomena constituting this possible continuum in the elderly population (aged 65 years and over) of a northern Italian city and to evaluate the relationship between some of these phenomena with psychological suffering., Method: Emotional feelings and suicidal thoughts have been investigated by an epidemiological survey conducted in a central quarter of that city. Data on attempted and completed suicide derived from the data bank of the Padua's WHO Collaborating Centre for Research and Training in Suicide Prevention that monitors these phenomena since 1989., Results and Conclusions: Results suggest the existence of some continuity in suicidal phenomena, where prevalence decreases from those of emotional/ideational nature to most extreme behaviour. Subjects presenting with more severe suicidal ideation were those also obtaining highest scores in a number of sub-scales of the Brief Symptom Inventory., (Copyright 2002 John Wiley & Sons, Ltd.)

Histochemical staining techniques, which differentiate the main categories of carbohydrates, were applied to sections from different segments of the alimentary canal of the shi drum Umbrina cirrosa L. to study patterns of distribution of epithelial glycoconjugates. In the oesophagus, mucous cells contained sulphomucins, together with a small amount of sialomucins. Stomach epithelial cells secreted neutral and acidic glycoconjugates, while gastric glands only produced small quantities of sialomucins. Goblet cells showed the presence of sialo and sulphomucins in the pyloric caeca, whereas intestinal mucous cells secreted sulphated glycoconjugates. This work serves as a baseline for further studies on carbohydrate composition of the mucosa of the shi drum digestive system.

Objective: The authors present an analysis of findings for the 65 years and over age group from the WHO/EURO Multicentre Study of Suicidal Behaviour (1989-93)., Methods: Multinational data on non-fatal suicidal behaviour is derived from 1518 subjects in 16 European centres. Local district data on suicide were available from 10 of the collaborating centres., Results: Stockholm (Sweden), Pontoise (France) and Oxford (UK) had the highest suicide attempts rates. In most centres, the majority of elderly who attempted suicide were widow(er)s, often living alone, who used predominantly voluntary drug ingestion. Non-fatal suicidal behaviour decreased with increasing age, whereas suicide rates rose. The ratio between fatal and non-fatal behaviours was 1:2, that for males/females almost 1:1. In the years considered, substantial stability in suicide and attempted suicide rates was observed. As their age increased, suicidal subjects displayed only a limited tendency to repeat self-destructive acts. Moreover, there was little correlation between attempted suicide and suicide rates, which carries different clinical implications for non-fatal suicidal behaviour in the elderly compared with younger subjects in the same WHO/EURO study.

Enzootic intranasal tumor (EIT) appears glandular in type and has recently been classified as an adenocarcinoma of low malignancy. The aim of this study was to characterize the secretion of surface glycoconjugates (GCs) in EIT and in normal respiratory and olfactory mucosae of the goat by means of conventional and lectin histochemistry, in order to shed light on the histogenesis of EIT. Morphologic and ultrastructural investigations showed two growth types of EIT: i.e., tubular and papillary patterns. Conventional histochemistry revealed the presence of neutral and carboxylated GCs in the olfactory glands and in the tubular part of EIT, as well neutral and sulphated GCs in the respiratory glands and in the papillary part of EIT, suggesting that the papillary pattern tumor arises from the respiratory glands, whereas the tubular portion of EIT arises from the olfactory glands. Lectin histochemistry gave further information on the expressed GCs.

Background: Various factors are considered to influence sexual behavior in the elderly, but the role played by preservation of adequate cognitive functioning has not been adequately explored., Objective: The aim of this research, conducted on 352 older adults aged between 65 and 105 years, was to identify the specific role played by cognitive functioning in sexual activity and sexual interest in the elderly., Methods: The data were collected from elderly people attending the surgeries of 21 general practitioners in the city of Padua (Italy). Analysis of sexual functioning was based on two items, from the LEIPAD questionnaire: 'Are you interested in sex?' and 'Do you have sexual relations?'. Subjects cognitive status was assessed objectively through the Mini Mental State Examination (MMSE) and subjectively by the LEIPAD subscale on cognitive functioning., Results: Subjects who were sexually active and interested in sex were more highly represented among the married elderly. The elderly who reported being active and interested in sex were significantly younger and had a significantly superior educational level and MMSE score. Mean scores for cognitive functioning and all quality-of-life indicators were in general significantly better for the active and interested. Univariate logistic regression analysis indicated that a higher MMSE score and cognitive functioning score influenced the maintenance of sexual interest., Conclusions: One third of the subjects reported being still sexually active and 40% being still interested in sex. This study seems to suggest that a significant role may be played by cognitive functioning in the maintenance of sexual interest in the elderly, especially older females in whom this dimension is evidently linked to far more diversified experiences than their male peers., (Copyright 2000 S. Objective: To test the psychometric properties of the Italian version of the WHOQOL-BRIEF (e.g., construct and internal validity, concurrent validity with the MOS SF-36 and test-retest reliability). The WHOQOL-BRIEF is a 26-items self-report instrument which assesses four domains assumed to represent the Quality Of Life (QOL) construct: physical domain, psychological domain, social relationships domain and environment domain, plus two facets for assessing overall QOL and general health., Methods: Data have been collected in three sites (Bologna, Modena and Padua), located in the North of Italy, in the framework of the international WHOQOL project. According to the study design, the sample had to include about 50% males and 50% females, 50% of subjects below and 50% above the age of 45, all in contact with various health services. A subsample has been re-interviewed after 2-3 weeks in order to study test-retest reliability. After the WHOQOL-BRIEF, most subjects have also been administered the MOS-SF36 in order to test the concurrent validity between these two instruments., Results: The instrument was administered to 379 subjects (1/6 healthy and 1/6 sick), chosen to be representative of a variety of different medical conditions. Seventy patients, who displayed stable health conditions, have been reassessed after 2-3 weeks to study test-retest reliability. The WHOQOL-BRIEF domains has shown good internal consistency, ranging from 0.65 for the social relationships domain to 0.80 for the physical domain; it has been able to discriminate between in- and out-patients and between the two age groups considered in the present study (< 45, > or = 45 years). Only physical and psychological domains were found to discriminate between healthy and ill subjects. No gender differences in the mean scores for the four domains were found. Concurrent validity between the WHOQOL-Brief and the MOS-SF-36 was satisfactory, and specific for the physical and psychological health domains. Test-retest reliability values were also good, ranging from 0.76 for the environment domain to 0.93 for the psychological domain., Conclusions: This study shows that the WHOQOL-BRIEF is psychometrically valid and reliable, and that it is also potentially useful in discriminating between subjects with different health conditions in clinical settings.

Carpal glands are compound tubuloalveolar glands, located on the medial surface of the carpus. This study was carried out on samples from carpal glands of adult wild and domestic pigs of both sexes. We elucidated the glycosidic composition of carpal gland secretion in situ using traditional histochemical methods and lectin histochemistry. Some secretory cells exhibited an intense reaction with PAS in both wild and domestic pigs. Lectin histochemistry showed differences in the localization and composition of glycoconjugates secreted by carpal glands. A cytoplasmic positivity was revealed in the wild pig by the sequence sialidase-PNA and WGA, whereas in the domestic pig the reactivity was localized at the apical surface of some cells. LTA positive cells were found only in the carpal glands of the domestic pig.

This study was aimed at characterizing the glycoconjugates produced by the horse sublingual gland and, in particular, at discriminating between the sialoderivatives by means of differential oxidation and saponification combined with lectin histochemistry and enzymatic degradation. The results showed a predominance of sialoglycoconjugates with beta-galactose as acceptor sugar in the salivary mucins produced by the sublingual gland. Besides being the most represented terminal residue, sialic acid was also expressed in a great variety of derivatives distinguishable on the basis of acceptor sugars to the penultimate beta-galactose as well as linkage and acetylation degree of the pyranose ring and the polyhydroxyl side chain. A role in the protection of mucous membranes from physical, chemical and pathogenic agents can be hypothesized for the horse sublingual mucins.

The presence and the distribution of neuroendocrine cells in the gastrointestinal tract of adult wild boar were investigated. The endocrine cells have been identified by means of immunocytochemical techniques using antibodies against serotonin, gastrin, somatostatin, cholecystokinin (CCK), met-enkephalin (MET-ENK), gastric-inhibitory peptide (GIP) and glucagon. The number of positive cells for each antiserum in each region was evaluated. Results were compared with data present in the literature and obtained previously by us and other authors in swine and domestic mammals (Ceccarelli et al., 1985, 1987, 1990, 1991, 1995; Capella and Solcia, 1972; Domeneghini and Castaldo, 1981; Peranzi and Lehy, 1984; Krause et al., 1985).

Localization of individual glycosidic residues and sialic acid acceptor sugars was investigated by conjugated lectins in the rabbit oviduct under physiological hormonal conditions and human chorionic gonadotropin (HCG) administration. Ampulla and isthmus were found to exhibit lectin binding profiles typical of each hormonal stage. Two different sialylated glycomolecules were identified within the epithelial lining; in particular, sialoglycoconjugates characterized by the terminal sequence sialic acid-galactose were visualized in the secretory cells and the sialic acid-N-acetylgalactosamine terminal disaccharides were localized on both ciliated and secretory cells of the entire oviduct. Surface and cytoplasmic sialoglycoconjugates were also found to exhibit a differential behaviour inside the two oviduct tracts examined. Present findings further supported the idea that in ampulla and isthmus, the greatest modifications consequent to hormone treatment take place at different times.

We investigated the occurrence of endogenous lectins in the mouse submandibular gland using anti-lectin antibodies that selectively bound the respective hapten lectins which have formed a complex with the appropriate hapten sugars and endogenous lectins. Findings supported the idea that endogenous lectins could interact with sugar residues and perform several roles including packaging, immobilization and transport of glucidic material. A functional meaning in the defence from some pathogenous agents was also postulated. Animal lectins seem to play roles related to their localization like occurs in the plants.

Sugar specific lectins (PNA, RCA I, LPA, SBA, DBA, GSA IB4, GSA II, WGA, LTA, UEA I, Con A, LCA) with and without prior selective glycosidase digestion (sialidase, alpha-fucosidase, alpha-mannosidase, beta-N-acetylglucosaminidase, alpha- and beta-galactosidase, beta-glucosidase) were used in order to investigate the distribution of native accessible carbohydrates and obtain information dealing with the composition of terminal disaccharides within glycoconjugates present in acinar compartments and ductal segments of mammalian (mouse, rat, hare, and rabbit) parotid glands. Glycoconjugates containing variable amounts of mannose, glucose, N-acetylgalactosamine and N-acetylglucosamine were present in the parotid glands of all species. However, these carbohydrate chains exhibited a different composition of terminal sequences within each type of gland. For example, sialylated components having the terminal dimers sialic acid-galactose and sialic acid-N-acetylgalactosamine were found in all acinar cells, whereas fucoglycoconjugates with terminal disaccharide fucose-galactose were localized in the rat striated ducts and hare acinar cells. The terminal sequence alpha-galactose-beta-galactose was demonstrated in the mouse acinar cells. Finally, glycoconjugates characterized by the terminal dimer beta-galactose-N-acetylgalactosamine were demonstrated in the mouse acinar and ductal cells and the rat ductal ones. Thus, present findings outlined and further confirmed the possibility to elucidate the oligosaccharide structure in situ using lectin histochemistry combined with enzymatic degradation.

Lectin histochemistry combined with glycosidase digestion was used to investigate the presence of glucidic residues and identify the terminal residues and their acceptor sugars in these glands. Quantitative evaluation of lectin-positive sites was made by histophotometrical scanning. Lectin histochemistry demonstrated that the anteromedial portion contains disaccharides sialic acid-D-Gal, sialic acid-D-GalNAc and Fuc-D-Gal, and that the anterolateral portion contains the terminal dimer sialic acid-D-Gal only. These findings have hypothetical significance in terms of the known functions of chicken anterior lingual glands.

In this study we investigated immunohistochemically the distribution of VIP in ampullary and isthmic segments of the rabbit oviduct at different hormonal stages in order to correlate the presence of VIP with the morphological and functional differences between the two oviduct segments. Noticeable changes in the VIP-positive sites emerged between corresponding oviduct segments during the stages examined. A reduction in VIP presence was noted in oestrus and the lowest VIP values occurred one and three days after mating in the ampulla and the isthmus respectively. In the subsequent stages an increase of VIP was noted in both tracts which showed features comparable to that described in anoestrus. The levels and distribution of VIP is correlated with the functions of the oviduct segments concerned with egg transport and formation of a mucopolysaccharide envelope around blastocysts.

An immunohistochemical study was conducted in order to visualize an endogenous lectin, having specificity for alpha-L-fucose residues, in the ampullary and isthmic tracts of the rabbit oviduct. 2 different sequences (S1 and S2) were tested using antilectin UEA I antibody. Reactive sites towards S2 sequence were localized at ciliated cell level in the ampulla; in the isthmus, instead, the staining was present both in ciliated and secretory cells. A role in the sugar immobilization and gamete and zygote protection was postulated for the endogenous lectin evidenced.

Oviducts play a central role in many reproduction functions. We investigated, by means of biochemical analyses, the chemical composition of glycopeptides extracted from ampulla and isthmus of hare and rabbit. Our results showed that both Lagomorphs present a different qualitative and quantitative composition of the 2 oviductal segments. We conclude that the considerably diverse glycoconjugate composition would be related to marked diversities occurring during the fertilization process, the post-ovulatory phases and the pregnancy length.

The existence and distribution of vasoactive intestinal polypeptide (VIP), and Met-enkephalin pentapeptide were investigated by means of the peroxidase-antiperoxidase (PAP) technique in rabbit submandibular and sublingual glands. In the submandibular gland, VIP immunostaining was observed in some peripheral acinar cells, while in sublingual one VIP positive fibres surrounded semilunes. Ductal cells of both glands were also reactive. These findings suggested a role of VIP in regulating volume and composition of salivary secretion.
