Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) has highly heterogeneous clinical presentations, in which encephalitis is an important phenotype. Moreover, MOGAD has been reported to exhibit diverse imaging findings. However, there have been no previous reports of cases with perivascular radial gadolinium enhancement in periventricular regions, commonly reported in autoimmune glial fibrillary acidic protein (GFAP) astrocytopathy. In this paper, we present two cases of MOGAD with this MRI feature, both of which underwent brain biopsy for the lesions. Brain biopsies revealed perivenous demyelination and inflammation consistent with acute disseminated encephalomyelitis (ADEM), with pronounced axonal damage in Case 1 and minimal axonal involvement in Case 2. Case 1 exhibited more severe cerebral atrophy than Case 2, correlating with the extent of axonal damage. Through these cases, we highlight the heterogeneity of radiological manifestations of MOGAD, expanding the spectrum beyond previously defined MRI patterns. Furthermore, histopathological analysis revealed distinct axonal involvement as a potential prognostic marker of brain atrophy. These observations emphasize the importance of considering MOGAD in the differential diagnosis, even in cases with atypical imaging findings, and highlight the significance of brain biopsy in guiding both diagnosis and prognosis.

153. Gender differences in responses to an altruistic message regarding rubella vaccination

BackgroundThe supplementary immunization activity (SIA) for the rubella vaccination of adult men born between 1962 and 1978 began in 2019 in Japan because of a vaccine gap in the cohort, as vaccination was not mandatory for those born in that period. However, SIA coverage remains low, despite an active campaign and financial support.MethodsWe conducted a randomized controlled study based on a 2 (scenario: self-vaccination, child vaccination) × 2 (message: self-interest, group-interest) factorial design, using a Japanese online panel. Participants with children were assigned to the child vaccination scenario in Intervention 1, whereas others were assigned to the self-vaccination scenario. After Intervention 1, all participants were given the same information about rubella. In Intervention 2, participants assigned to self-interest messages received a message emphasizing the risk of rubella, and those assigned to group-interest messages received a message emphasizing herd immunity. After Intervention 2, we evaluated the effects using a questionnaire.ResultsAmong the 2,206 participants, information regarding rubella was evaluated as more reliable in the group—than in the self-interest message condition, especially among women. Women evaluated the necessity of rubella vaccination for adult men and women to be higher in the child-vaccination scenario and group-interest messages. However, no differences were found among men. By contrast, men exposed to the self-interest message positively evaluated the reliability of the rubella explanation.ConclusionThe findings indicate that emphasizing self-interest messages is more effective for men in promoting herd immunity against rubella.

154. The Impact of Salinity on the Productivity and Quality of Durum and bread Wheat

Salinity poses a significant challenge to the quality and productivity of crops. In the primary wheat cultivation areas worldwide, salinity negatively affects wheat growth, yield, and quality. To address this issue, the development of tolerant wheat varieties through selective breeding techniques is essential. The aim of this study was to assess the effects of salinity on grain yield, protein content, and thousand-kernel weight (TKW) among 55 different varieties and accessions of bread and durum wheat. The findings revealed that the application of salt treatment (100 mM NaCl solution) resulted in reduced growth and yield production in 45 bread and durum wheat varieties. However, 6 durum wheat varieties, 3 durum wheat accessions, and 1 common wheat accession showed insignificant susceptibility to salinity. These included Chryssodur from Greece, Saragolla, Silur, and Dakter from Italy, Sculptur from France, Karim from Tunisia, Algeria 70-2 from Algeria, Ethiopia 201 and Ethiopia 229 from Ethiopia, and the Morocco 85 accession of bread wheat from Morocco. The decline in grain yield could be attributed to salinity, which led to a decrease in photosynthetic capacity, resulting in reduced starch synthesis and accumulation in the grain. Furthermore, the study demonstrated that winter wheat exhibited greater tolerance to salt stress compared to spring wheat, and durum wheat displayed higher tolerance than common wheat. Additionally, salt accumulation was found to increase protein content in five varieties and one accession of durum wheat. This variation may be linked to the relatively stable nitrogen metabolism under salt stress, contributing to higher protein concentrations. TKW also decreased in all 10 varieties and accessions, irrespective of the species.

Introduction Reducing neonatal deaths in premature infants in low- and middle-income countries is key to reducing global neonatal mortality. International neonatal networks, along with patient registries of premature infants, have contributed to improving the quality of neonatal care; however, the involvement of low-to-middle-income countries was limited. This project aims to form an international collaboration among neonatal networks in Asia (AsianNeo), including low-, middle- and high-income countries (or regions). Specifically, it aims to determine outcomes in sick newborn infants, especially very low birth weight (VLBW) infants or very preterm infants, with a view to improving the quality of care for such infants.Methods and analysis Currently, AsianNeo comprises nine neonatal networks from Indonesia, Japan, Malaysia, Philippines, Singapore, South Korea, Sri Lanka, Taiwan and Thailand. AsianNeo will undertake the following four studies: (1) institutional questionnaire surveys investigating neonatal intensive care unit resources and the clinical management of sick newborn infants, with a focus on VLBW infants (nine countries/regions); (2) a retrospective cohort study to describe and compare the outcomes of VLBW infants among Asian countries and regions (four countries/regions); (3) a prospective cohort study to develop the AsianNeo registry of VLBW infants (six countries/regions); and (4) implementation and evaluation of educational and quality improvement projects in AsianNeo countries and regions (nine countries/regions).Ethics and dissemination The study protocol was approved by the Research Ethics Board of the National Center for Child Health and Development, Tokyo, Japan (reference number 2020–244, 2022–156). The study findings will be disseminated through educational programmes, quality improvement activities, conference presentations and medical journal publications.

Resumo Objetivo Relacionar variáveis sociodemográficas, econômicas, clínicas e ter ou não cuidador, risco de queda e percepção do risco de quedas com a capacidade funcional em pessoas idosas em um Serviço de Emergência. Métodos Estudo transversal e analítico, realizado entre setembro de 2019 e março de 2020, no Serviço de Emergência, com 197 pessoas idosas. Foi aplicado questionário com informações sociodemográficas, econômicas e clínicas; e os instrumentos: Falls Risk Awareness Questionnaire, Morse Falls Scale, Índice de Katz e Escala de Lawton. Para comparar o Índice de Katz e a Escala de Lawton; e associar a Morse Falls Scale com as variáveis contínuas foram utilizados, respectivamente, o teste de Kruskal Wallis e o coeficiente de correlação de Spearman. The title compound, C15H12BN3, is a type of diazaborinane featuring substitution at 1, 2, and 3 positions in the nitrogen–boron six-membered heterocycle. It is comprised of two almost planar units, the pyridyl ring and the Bdan (dan = 1,8-diaminonaphtho) group, which subtend a dihedral angle of 24.57 (5)°. In the crystal, the molecules are linked into R44(28) hydrogen-bonding networks around the fourfold inversion axis, giving cyclic tetramers. The molecules form columnar stacks along the c axis.

Objective: to construct and validate the content of a nursing history for a Long Stay Institution for the Elderly. Methods: a methodological study divided into the following stages: identification of the main Nursing Diagnoses in the literature, development of the instrument, validation with specialists using the Delphi technique, and restructuring of the instrument based on the Self-Care Deficit Theory. Results: the 41 articles selected resulted in 78 main Nursing Diagnoses, which supported the development of an instrument with 104 items. A total of 37 experts were selected and invited for validation, and 10 agreed to contribute. After validation, the record was finalized with 90 items, with a Content Validation Index between 0.80 and 1.0 in the first round. The suggestions validated in the second round achieved 80-100% agreement to complete the validation of the instrument's content. Conclusion: the content of a nursing history for a Long-Stay Institution for the Elderly based on the Self-Care Deficit Theory was constructed and validated. Contributions to practice: this is an important resource that can be used in clinical practice.

However, it remains a challenge to identify individual physiological state changes in cross-disease onset processes because of the complex relationships among multiple biomarkers. Here, we present the health-disease phase diagram (HDPD), which represents a personal health state by visualizing the boundary values of multiple biomarkers that fluctuate early in the disease progression process. In HDPDs, future onset predictions are represented by perturbing multiple biomarker values while accounting for dependencies among variables. We constructed HDPDs for 11 non-communicable diseases (NCDs) from a longitudinal health checkup cohort of 3,238 individuals, comprising 3,215 measurement items and genetic data. Improvement of biomarker values to the non-onset region in HDPD significantly prevented future disease onset in 7 out of 11 NCDs. Our results demonstrate that HDPDs can represent individual physiological states in the onset process and be used as intervention goals for disease prevention.

Recently, DEtection TRansformer (DETR), an end-to-end object detection pipeline, has achieved promising performance. However, it requires large-scale labeled data and suffers from domain shift, especially when no labeled data is available in the target domain. To solve this problem, we propose an end-to-end cross-domain detection Transformer based on the mean teacher framework, MTTrans, which can fully exploit unlabeled target domain data in object detection training and transfer knowledge between domains via pseudo labels. We further propose the comprehensive multi-level feature alignment to improve the pseudo labels generated by the mean teacher framework taking advantage of the cross-scale self-attention mechanism in Deformable DETR. Image and object features are aligned at the local, global, and instance levels with domain query-based feature alignment (DQFA), bi-level graph-based prototype alignment (BGPA), and token-wise image feature alignment (TIFA). On the other hand, the unlabeled target domain data pseudo-labeled and available for the object detection training by the mean teacher framework can lead to better feature extraction and alignment. Thus, the mean teacher framework and the comprehensive multi-level feature alignment can be optimized iteratively and mutually based on the architecture of Transformers. Extensive experiments demonstrate that our proposed method achieves state-of-the-art performance in three domain adaptation scenarios, especially the result of Sim10k to Cityscapes scenario is remarkably improved from 52.6 mAP to 57.9 mAP. Minimizing the sum of a convex function and a composite function appears in various fields. The generalized Levenberg--Marquardt (LM) method, also known as the prox-linear method, has been developed for such optimization problems. The method iteratively solves strongly convex subproblems with a damping term. This study proposes a new generalized LM method for solving the problem with a smooth composite function. The method enjoys three theoretical guarantees: iteration complexity bound, oracle complexity bound, and local convergence under a H\"olderian growth condition. The local convergence results include local quadratic convergence under the quadratic growth condition; this is the first to extend the classical result for least-squares problems to a general smooth composite function. In addition, this is the first LM method with both an oracle complexity bound and local quadratic convergence under standard assumptions. These results are achieved by carefully controlling the damping parameter and solving the subproblems by the accelerated proximal gradient method equipped with a particular termination condition. Experimental results show that the proposed method performs well in practice for several instances, including classification with a neural network and nonnegative matrix factorization.

164. A Greedy and Optimistic Approach to Clustering with a Specified Uncertainty of Covariates

In this study, we examine a clustering problem in which the covariates of each individual element in a dataset are associated with an uncertainty specific to that element. More specifically, we consider a clustering approach in which a pre-processing applying a non-linear transformation to the covariates is used to capture the hidden data structure. To this end, we approximate the sets representing the propagated uncertainty for the pre-processed features empirically. To exploit the empirical uncertainty sets, we propose a greedy and optimistic clustering (GOC) algorithm that finds better feature candidates over such sets, yielding more condensed clusters. As an important application, we apply the GOC algorithm to synthetic datasets of the orbital properties of stars generated through our numerical simulation mimicking the formation process of the Milky Way. The GOC algorithm demonstrates an improved performance in finding sibling stars originating from the same dwarf galaxy. We present an experimental system for Rydberg tweezer arrays with ytterbium (Yb) atoms featuring internal state manipulation between the ground ${}^1$S$_0$ and the metastable ${}^3$P$_2$ states, and single-photon excitation from the ${}^3$P$_2$ to Rydberg states. In the experiments, single Yb atoms are trapped in two-dimensional arrays of optical tweezers and are detected by fluorescence imaging with the intercombination ${}^1$S$_0 \leftrightarrow {}^3$P$_1$ transition, and the defect-free single atom arrays are prepared by the rearrangement with the feedaback. We successfully perform high-resolution ${}^1$S$_0\leftrightarrow {}^3$P$_2$ state spectroscopy for the single atoms, demonstrating the utilities of this ultranarrow transition. We further perform single-photon excitation from the ${}^3$P$_2$ to Rydberg states for the single atoms, which is a key for the efficient Rydberg excitation. We also perform a systematic measurement of a complex energy structure of a series of D states including newly observed ${}^3$D$_3$ states. The relative abundance of cosmic ray nickel nuclei with respect to iron is by far larger than for all other trans-iron elements, therefore it provides a favorable opportunity for a low background measurement of its spectrum. Since nickel, as well as iron, is one of the most stable nuclei, the nickel energy spectrum and its relative abundance with respect to iron provide important information to estimate the abundances at the cosmic ray source and to model the Galactic propagation of heavy nuclei. However, only a few direct measurements of cosmic-ray nickel at energy larger than $ \sim$ 3 GeV/n are available at present in the literature and they are affected by strong limitations in both energy reach and statistics. In this paper we present a measurement of the differential energy spectrum of nickel in the energy range from 8.8 to 240 GeV/n, carried out with unprecedented precision by the Calorimetric Electron Telescope (CALET) in operation on the International Space Station since 2015. The CALET instrument can identify individual nuclear species via a measurement of their electric charge with a dynamic range extending far beyond iron (up to atomic number $ Z $ = 40). The particle's energy is measured by a homogeneous calorimeter (1.2 proton interaction lengths, 27 radiation lengths) preceded by a thin imaging section (3 radiation lengths) providing tracking and energy sampling. This paper follows our previous measurement of the iron spectrum [O. Adriani et al., Phys. Rev. Lett. 126, 241101 (2021).], and it extends our investigation on the energy dependence of the spectral index of heavy elements. It reports the analysis of nickel data collected from November 2015 to May 2021 and a detailed assessment of the systematic uncertainties. This whitepaper summarizes and the state of the field of high-power cyclotrons and FFAs as discussed by international experts during a three-day workshop of the same name. The workshop was held online from Sep 7 to Sep 9, 2021 as part of the US Snowmass'21 Community Exercise, specifically the Accelerator Frontier (AF) and the subpanel Accelerators for Neutrinos (AF02). Thus, we put emphasis on the application of high-power cyclotrons in particle physics, specifically neutrino physics, and as drivers for muon production. In the introduction, we discuss the role of cyclotrons for particle physics, and later we highlight existing and planned experiments in the corresponding sections. However, as these same accelerators have important applications in the fields of isotope production - both for research and medicine - and possibly even in energy research, by providing beam to demonstrator experiments in the areas of Accelerator Driven Systems (ADS), we include these far-reaching topics to provide a full picture of the status and applications of high-power cyclotrons. Furthermore, Fixed Field Alternating Gradient accelerators (FFAs) have recently seen renewed interest. They are in many respects (basic operating principles) similar to cyclotrons and have thus been included in this workshop and whitepaper as well. 168. Unfamiliar Untrained Observers' Ratings of Adolescent Safety Behaviors within Social Interactions with Unfamiliar Peer Confederates

Adolescents experiencing social anxiety often engage in safety behaviors--covert avoidance strategies for managing distress (e.g., avoiding eye contact)--that factor into the development and maintenance of their concerns. Prior work supports the psychometric properties of the Subtle Avoidance Frequency Examination (SAFE), a self-report survey of safety behaviors. Yet, we need complementary methods for assessing these behaviors within contexts where adolescents often experience concerns, namely interactions with unfamiliar peers. Recent work indicates that, based on short, direct social interactions with adolescents, individuals posing as unfamiliar peers (i.e., "peer confederates") and without assessment training can capably report about adolescent social anxiety. We built on prior work by testing whether we could gather valid SAFE reports from "unfamiliar untrained observers" (UUOs), who observed adolescents within archived recordings of these short social interactions. A mixed clinical/community sample of 105 adolescents self-reported on their functioning and participated in a series of social interaction tasks with peer confederates, who also provided social anxiety reports about the adolescent. Based on video recordings of these tasks, "trained independent observers" rated adolescents' observed social skills, and an additional set of UUOs completed SAFE reports of these same adolescents. Unfamiliar untrained observers' SAFE reports (a) related to adolescents' SAFE self-reports, (b) distinguished adolescents on clinically elevated social anxiety concerns, (c) related to trained independent observers' ratings of adolescent social skills within interactions with peer confederates, and (d) related to adolescents' self-reported arousal within these same interactions. Our findings support use of unfamiliar observers' perspectives to understand socially anxious adolescents' interpersonal functioning. [This paper will be published in "Behavior Therapy."]

Abstract Brigatinib-based therapy was effective against osimertinib-resistant EGFR C797S mutants and is undergoing clinical studies. However, tumor relapse suggests additional resistance mutations might emerge. Here, we first demonstrated the binding mode of brigatinib to the EGFR-T790M/C797S mutant by crystal structure analysis and predicted brigatinib-resistant mutations through a cell-based assay including N-ethyl-N-nitrosourea (ENU) mutagenesis. We found that clinically reported L718 and G796 compound mutations appeared, consistent with their proximity to the binding site of brigatinib, and brigatinib-resistant quadruple mutants such as EGFR-activating mutation/T790M/C797S/L718M were resistant to all the clinically available EGFR-TKIs. BI-4020, a fourth-generation EGFR inhibitor with a macrocyclic structure, overcomes the quadruple and major EGFR-activating mutants but not the minor mutants, such as L747P or S768I. Molecular dynamics simulation revealed the binding mode and affinity between BI-4020 and EGFR mutants. This study identified potential therapeutic strategies using the new-generation macrocyclic EGFR inhibitor to overcome the emerging ultimate resistance mutants.

Abstract Homoeostatic regulation of the acid–base balance is essential for cellular functional integrity. However, little is known about the molecular mechanism through which the acid–base balance regulates cellular responses. Here, we report that bicarbonate ions activate a G protein-coupled receptor (GPCR), i.e., GPR30, which leads to Gq-coupled calcium responses. Gpr30-Venus knock-in mice reveal predominant expression of GPR30 in brain mural cells. Primary culture and fresh isolation of brain mural cells demonstrate bicarbonate-induced, GPR30-dependent calcium responses. GPR30-deficient male mice are protected against ischemia-reperfusion injury by a rapid blood flow recovery. Collectively, we identify a bicarbonate-sensing GPCR in brain mural cells that regulates blood flow and ischemia–reperfusion injury. Our results provide a perspective on the modulation of GPR30 signalling in the development of innovative therapies for ischaemic stroke. Moreover, our findings provide perspectives on acid/base sensing GPCRs, concomitantly modulating cellular responses depending on fluctuating ion concentrations under the acid–base homoeostasis.

Abstract Streptococcus suis is a gram-positive bacterium that causes meningitis, septicemia, endocarditis, and other disorders in pigs and humans. We obtained 42 and 50 S. suis isolates from lesions of porcine endocarditis and palatine tonsils, respectively, of clinically healthy pigs in Japan; we then determined their sequence types (STs) by multilocus sequence typing (MLST), cps genotypes, serotypes, and presence of classical major virulence-associated marker genes (mrp, epf, and sly). The 42 isolates from endocarditis lesions were assigned to a limited number of STs and clonal complexes (CCs). On the other hand, the 50 isolates from tonsils were diverse in these traits and seemingly in the degree of virulence, suggesting that tonsils can accommodate a variety of S. suis isolates. The goeBURST full algorithm using tonsil isolates obtained in this study and those retrieved from the database showed that major CCs as well as many other clusters were composed of isolates originating from different countries, and some of the STs were very similar to each other despite the difference in country of origin. These findings indicate that S. suis with not only different but also similar mutations in the genome have survived in tonsils independently across different geographical locations. Therefore, unlike the lesions of endocarditis, the tonsils of pigs seemingly accommodate various S. suis lineages. The present study suggests that S. suis acquired its diversity by natural mutations during colonization and persistence in the tonsils of pigs.

This study delves into the domain of dynamical systems, specifically the forecasting of dynamical time series defined through an evolution function. Traditional approaches in this area predict the future behavior of dynamical systems by inferring the evolution function. However, these methods may confront obstacles due to the presence of missing variables, which are usually attributed to challenges in measurement and a partial understanding of the system of interest. To overcome this obstacle, we introduce the autoregressive with slack time series (ARS) model, that simultaneously estimates the evolution function and imputes missing variables as a slack time series. Assuming time-invariance and linearity in the (underlying) entire dynamical time series, our experiments demonstrate the ARS model’s capability to forecast future time series. From a theoretical perspective, we prove that a 2-dimensional time-invariant and linear system can be reconstructed by utilizing observations from a single, partially observed dimension of the system.

Benign biliary stricture (BBS) is a complication of chronic pancreatitis (CP). Despite endoscopic biliary stenting, some patients do not respond to treatment, and they experience recurrent cholangitis. We report two cases of CP with refractory BBS treated using endoscopic ultrasound-guided choledochoduodenostomy (EUS-CDS) fistula creation. A 50-year-old woman and a 60-year-old man both presented with obstructive jaundice secondary to BBS due to alcoholic CP. They underwent repeated placement of a fully covered self-expandable metal stent for biliary strictures. However, the strictures persisted, causing repeated episodes of cholangitis. Therefore, an EUS-CDS was performed. The stents were eventually removed and the patients became stent-free. These fistulas have remained patent without cholangitis for more than 2.5 years. Fistula creation using EUS-CDS is an effective treatment option for BBS.

Abstract Idiopathic pulmonary fibrosis (IPF) is a progressive disease characterized by severe lung fibrosis and a poor prognosis. Although the biomolecules related to IPF have been extensively studied, molecular mechanisms of the pathogenesis and their association with serum biomarkers and clinical findings have not been fully elucidated. We constructed a Bayesian network using multimodal data consisting of a proteome dataset from serum extracellular vesicles, laboratory examinations, and clinical findings from 206 patients with IPF and 36 controls. Differential protein expression analysis was also performed by edgeR and incorporated into the constructed network. We have successfully visualized the relationship between biomolecules and clinical findings with this approach. The IPF-specific network included modules associated with TGF-β signaling (TGFB1 and LRC32), fibrosis-related (A2MG and PZP), myofibroblast and inflammation (LRP1 and ITIH4), complement-related (SAA1 and SAA2), as well as serum markers, and clinical symptoms (KL-6, SP-D and fine crackles). Notably, it identified SAA2 associated with lymphocyte counts and PSPB connected with the serum markers KL-6 and SP-D, along with fine crackles as clinical manifestations. These results contribute to the elucidation of the pathogenesis of IPF and potential therapeutic targets.
