The use of epidural anaesthesia during birth process has made that both second stage of labour time and foetus expulsion time has increased. This has made that we will look for another position to favour the foetus progress down the birth canal so that time of the second stage of labour will decrease.The most usual positions on our country are the Sims and Semi-fowler ones, and the obstetric position, which is generally used when foetus is in III or IV level and birth is going to take place.After the hospital redesigning, our hospital bought some new articled beds which may be transformed into birthing bed, and some of the midwives that work there started to use this Sims position with the modification that this kind of bed permit, consisting on lifting one of the women legs. Woman is collocated lying on her side with one of her legs parallel to the other, but one of them is lift, avoiding the forced knees separation, being the woman firm and secure during all second stage of labour and expulsion period, using the squatting bar to help her when pushing. Therefore, an experimental study has being designed in order to know whether Sims modified position reduce the second stage of labour time, and also to know if this position favours the foetus rotation and progress down the birth canal.Women will be randomly divided into two groups, the control group and the experimental group.

Aim: To analyze the perception of women suffering from sickle cell anemia regarding pregnancy. Method: This is a qualitative, descriptive and exploratory study, involving 25 women suffering from sickle cell anemia who lived in Bahia. Data were obtained through open interviews and analyzed by the Collective Subject Discourse. Results: Three central ideas emerged from the discourses: fear and anxiety marked the pregnancy of women suffering from sickle cell anemia; dilemmas permeated the decision to abort, and; the desire to be a mother was frustrated by miscarriage. Discussion: Due to the lack of equipment and lack of social support for exercising motherhood, women experience sadness, depression and think of interrupting the pregnancy. Conclusion: The choice to have children, although risky, should be ensured by public policy with an integral assistance to women suffering from sickle cell anemia, involving the totality of their reproductive trajectory. Objective: To identify the meanings attributed by mothers to hypertensive disorders of pregnancy (HDPs) and their consequences, such as premature birth and hospitalizaron of the infant in the neonatal intensive care unit (NICU). Method: A qualitative study, based on the Central Nucleus Theory, with 70 women who had hypertensive disorders of pregnancy and preterm delivery. We used the technique of free word association (FWAT) with three stimuli: high blood pressure during pregnancy, prematurity and NICU. Results: We obtained 1007 evocations, distributed as follows: high blood pressure during pregnancy (335) prematurity (333) and NICU (339). These constituted three thematic units: representation of HDPs, prematurity and the NICU. The categories death and negative aspects were inherent to the three units analyzed, followed by coping strategies and needs for care present in HDPs and prematurity. Conclusion: The study had death as its central nucleus, and highlighted the subjective aspects present in the high risk pregnancy and postpartum cycle. It is hoped that this research will contribute to qualifying nursing care for women confronting the problem of HDPs, so that they can cope with less impacts from the adverse effects of high risk pregnancy and birth. This paper aimed at knowing the cultural caesarean representations present in magazines addressed to the laic public. One of the attributions of the nurse is health education. The press is source of informal education. So, the nurse must provide women with subsidies so that they can make decisions with autonomy and knowledge, demystifying or incrementing what is thought about the surgical delivery. Upon analyzing editions of Revista Crescer, two themes have arisen: the “said” and the “not said” about caesarean. Regarding the “said” category, the articles appraise pathologies in the gestation and their inter-occurrences with explicit association between pathologies and the caesarean; the magazine searched for the adequacy of the woman to the hospital routine and softened the caesarean consequences. As to the “not said” category, the magazine did not emphasize the control and prevention of the complications that are inherent to pregnancy and, somehow, led women to opt for the caesarean as the safest mode of delivery.

OBJETIVOS: avaliar o processo de atendimento pré-natal de uma instituição filantrópica da cidade de São Paulo, conforme a proposta de Donabedian. MÉTODOS: para analisar os indicadores: idade gestacional de início do pré-natal; número, procedimentos e intervalo de consultas e exames laboratoriais, aplicou-se o índice de Kessner adaptado por Takeda. Os dados foram obtidos por auditoria em 635 prontuários no período de julho de 2000 a junho de 2001. O nível de significância dos os testes estatísticos foi de 5%. RESULTADOS: a média de consultas de pré-natal foi de 6,5 (dp=2,7) e a da idade gestacional de início do pré-natal 20,7 semanas (dp=7,7). A adequação dos procedimentos de consultas variou de 92,6% a 97,8% e o intervalo entre as consultas de 53,7% a 100%. A aplicação do Índice de Kessner classificou o pré-natal como adequado para 38,4% das gestantes, inadequado para 24,7% e intermediário para 36,9%. O índice de Kessner acrescido dos resultados dos exames classificou o pré-natal em adequado para 35,7% das gestantes, inadequado para 28,3% e intermediário para 35,9%. CONCLUSÕES: o tipo de avaliação adotado é factível e permite realizações periódicas com vista à melhora da adequação dos indicadores da assistência oferecida pelo serviço.OBJECTIVES: to assess prenatal healthcare in a philanthropic institution in the city of São Paulo according to the Donabedian proposal. METHODS: to assess indicators pregnancy age in the beginning of prenatal care; number, procedures and consultation intervals and laboratory exams, the Kessner index as adapted by Takeda was applied. Data were obtained through auditing of 635 medical files from July 2000 to June, 2001. The significance level of statistical exams was of 5%. RESULTS: prenatal medical visits median was of 6.5 (dp=2.7) and pregnancy age at the beginning of prenatal care was of 20.7 weeks (dp=7.7). Medical visits procedures adequacy varied from 92.6% a 97.8% and medical visits interval was 53,7% to 100%. The application of the Kessner Index rated prenatal care as being adequate for 38.4% of pregnant women, inadequate for 24.7% and intermediary for 36.9%. The Kessner index compounded by exams results rated prenatal care as being adequate for 35,7% of pregnant women, inadequate for 28.3% and intermediate for 35.9%. CONCLUSIONS: the type of assessment adopted is viable is allows for periodical applications aiming at adequacy improvement of the care offered.

Purpose: The purpose of this study was to evaluate the effects of clinical practice and simulation-based practice for obstetrical nursing in terms of self-efficacy, practice satisfaction, and practice stress. Methods: A non-equivalent control group post test design was used. The participants were 171 junior nursing students, 103 in the experimental group, and 68 in the control group. Simulation-based practice consisted of nursing assessment during labor, nursing assessment and intervention for normal and high risk mother, nursing education for mother and family during labor, and open lab. The experimental group participated in simulation-based practice for two days and the control group participated in clinical practice at delivery room for two weeks. Results: The experimental group showed higher level of self-efficacy (p= .043), practice satisfaction (p<.001) and practice stress (p= .003) compared to the control group. Conclusion: Simulation-based practice is an effective learning method for obstetrical nursing and can be used as the alternative for clinical practice. At the beginning of the 20th century, women were considered incapable and inferior, dominated by their husbands. Men were excluded from events related to reproduction and children's education. Childbirth was a domestic ritual attended by midwives. Its transfer to the hospital placed the woman as protagonist in the process of giving birth, and nowadays the doctor is the central actor. The objective of the study was to analyze the process of the father's exclusion from his child's birth from the perspective of power in the context of the medicalization of childbirth. It was a systematic literature review analyzing 16 texts concerning the period from 2000 to 2010. The texts were classified according to the thematic nucleus: relationship of the man's power over the woman, transition of childbirth from home to hospital, father's exclusion from the hospital environment, medical dominance over the feminine body and the father's (re)insertion in the childbirth process. It was concluded that a man's reinsertion into the process of his child's birth is not a reality, despite being promoted by the Brazilian Health Ministry. This paper aims to understand the mothers' experiences on the care received during the birth process in a public maternity in Feira de Santana, state of Bahia, Brazil. This is a qualitative descriptive exploratory study that complied with the Resolution 196/96 of the National Health Council and was conducted from February to April 2010, through semi-structured interviews with 19 puerperas. The Content Analysis of data showed that the respondents had experienced the birth process with loneliness, fear, pain, suffering and abandonment. The assistance occurred only during the expulsive period or in the postpartum. It is needed to use an approach that encourages women and their companions' participation, which prioritizes the constant presence of professionals with the parturient, proposes the physical and emotional support and the use of new care technologies that provide relief pain and the parturient's comfort. To present clinical and epidemiological aspects of prematurity in a Normal Delivery Center, and to describe outcomes. Methods: A cross-sectional retrospective study, with 189 medical records of premature, single, and living fetuses, with gestational ages between 25 and 36.6 weeks. Results: The prevalence of preterm births was 7.8%, and 23.2% in adolescent mothers. The most frequent obstetric complication was premature rupture of ovular membranes (35.4%), followed by hypertensive syndrome (18.5%). Cesarean section was performed in 33% of cases. The Apgar score in the 1st minute was < 7 in 36.0% of cases, and in the 5th minute of life, in 9.5% of cases. Conclusion: Among the aspects studied, results showed that the prevalence of preterm births was higher than expected for the adolescent mothers, however, with a satisfactory perinatal outcome. Purpose: Clinical training for nursing students is limited to rudimentary skills to avoid potential risks. Simulation-Based Training (SBT) can overcome the shortcomings of clinical training. We evaluated the educational effect of SBT for obstetrical nursing students using high-fidelity simulation courses. Methods: We developed a simulation program for obstetrical nursing students to practice nursing skills that are necessary to provide quality care. The program consisted of four sessions. 1st: An orientation and a preliminary test. 2nd: Learning core skills required in obstetrical nursing. 3rd: Testing each student with scenario. 4th: Providing a debriefing session. At the beginning of the program, students were surveyed about their self-confidence in obstetrical nursing care, and at the end of the program, they were surveyed about the adequacy of SBT as well as self-confidence. Results: Students' self-confidence showed a significant difference before and after simulation. Mean adequacy of SBT was 7.15±1.35 (out of 10). Most students became more interested in Women's Health Nursing after SBT. Conclusion: The results from evaluating the effects of simulation-based obstetrical nursing training show that SBT provides invaluable clinical experience for obstetrical nursing students by overcoming the lack of actual clinical involvement in clinical training programs. The hospitalization of all women in labour, in the interests of the safety and well-being of mother and baby introduced mechanical, impersonal and interventionist childbirth care, leading to seeing homebirth as an alternative for women seeking a natural approach during birth. The literature on the subject is scarce, justifying the production of data on homebirths. The aim was to describe the process and maternal and neonatal outcomes in planned homebirths. Data were collected from 70 births attended by midwives, in Sao Paulo, Brazil. The results indicated that 61.4% of women were 30 years old or more, 71.4% had higher education, 97.1% lived with a partner, 64.3% performed paid work, 54.3% were nulliparous, and the rate of maternal transfer to hospital was 5.7% due to complications in labour and 14.3% on request; 92.9% were normal deliveries, 63.6% chose vertical positions and 57.5% had an intact perineum or first-degree laceration, all newborns had Apgar scores . 7 at 5 minutes and one baby was transferred to the hospital due to cardiac arrhythmia. Judicious use of obstetrical and neonatal interventions in homebirths and scientific evidence about their safety can contribute to transformation of the current model of childbirth in Brazil.
