Abstract

The Royal Society of Tropical Medicine and Hygiene held a residential meeting from 17-19 September 2008. Over 250 delegates from a diverse range of backgrounds and experience convened in Brighton, UK for three days of talks and discussions on a wide variety of themes. Topics ranged from tropical and neglected infectious diseases through to other disorders that whilst not traditionally associated with low income countries pose an increasing challenge; chronic diseases, mental health disorders and problems arising from conflict and poverty combined. The meeting represented the change in focus at RSTMH from tropical infectious diseases towards global health in its broadest sense.

Abstract

This paper presents the proceedings of a scientific meeting that was held between the Royal Society of Tropical Medicine and Hygiene and the Norwegian Infectious Diseases Society at Ullevål University Hospital, Oslo on 20 June 2008. Seven speakers updated the audience on recent advances in relevant issues, including international maternal and child health, podoconiosis, schistosomiasis and HIV, leprosy in the UK, rapid diagnostic tests for malaria, delayed diagnosis of tuberculosis and the Global Filariasis Programme. The meeting ended with a series of case presentations.

Abstract

The in vitro IFN-gamma response to tuberculin was recently proposed as a correlate of vaccine-induced immunity to tuberculosis. IFN-gamma also plays a central role in the tuberculin skin test (TST), commonly used as a marker of mycobacterial infection. However, the use of TST as a marker of immunity to tuberculosis is limited for reasons ascribed mainly to interference by environmental mycobacteria. We prospectively investigated the relationship between the TST and cytokine responses to BCG in early infancy, a cohort with relatively low exposure to environmental mycobacteria. Neonatal BCG vaccination induced positive TST responses and predominant IFN-gamma responses to tuberculin in most newborns. However, the production of IFN-gamma, IL-5 and IL-13 was similar in TST responders and non-responders, and there was no significant correlation between the size of TST response and cytokine production. These results indicate that the IFN-gamma assay provides different information than TST in BCG-vaccinated newborns and could be a better marker of vaccine-induced immunity.
