Abstract

Dart-throwing motion has attracted attention as a functional and novel movement. To date, wrist joint splints regarding traction function and dart-throwing motion with a single splint have been hardly investigated. Therefore, a new artificial muscle-type dynamic traction splint incorporating this novel motion pattern was devised, aiming to investigate the therapeutic use of the dynamic traction splint by artificial muscle in patients with wrist joint contractures. Four hands of three patients diagnosed with wrist joint contractures were assessed. The average splint usage period was 5 weeks. The wrist joint range of motion and grip strength were evaluated initially and after 5 weeks. The wrist joint range of motion was evaluated over a 6-week period. The splints were required to be worn four times. The traction force was initiated at 500 kPa and was appropriately altered based on patient symptoms. The mean (��standard deviation) palmar flexion angle was 21.3�� (��5.4) initially and 43.8�� (��9.7) at 5 weeks; the improvement in the angle was 22.5��. The mean dorsal flexion angle was 16.3�� (��4.4) initially and 45.0�� (��6.9) at 5 weeks; the improvement in the angle was 28.7��. The grip strength improved from 15.7% of the healthy side at the start to 72.5% after 5 weeks. These results indicated that therapeutic outcomes with the dynamic traction splint by artificial muscle were almost the same as those with rehabilitation for both dorsiflexion and palmar flexion, suggesting that dynamic traction splint by artificial muscle may be an effective splint for the management of wrist joint contractures.

Abstract

Myxoid liposarcoma (MLS) is a common subtype of liposarcoma. Although the prognosis is generally good, there are factors known to be associated with poor prognosis. Accurate indices are important to predict prognosis. We aimed to assess the usefulness of immunohistochemistry for phosphohistone H3 (PHH3) as a potential biomarker in comparison with Ki-67 antigen and other prognostic factors. Twenty-five patients with MLS were evaluated. Age, sex, depth of tumor, tumor size, surgical margin, oncological outcome, histological grade, presence of necrosis, proportion of round cell component (RC%), PHH3 index, and Ki-67 index were examined. Prognostic factors of the examination criteria were statistically analyzed, survival rate analyses were performed using the Kaplan–Meier method, and multivariate analysis was performed using Cox proportional-hazard regression analysis. The number of PHH3-positive tumor cells and the PHH3 and Ki-67 indices demonstrated a statistical correlation with the prognosis of MLS in univariate analysis (P < 0.001, P < 0.001, P = 0.01, respectively). PHH3 index and RC% were significant factors in multivariate analysis (P = 0.03, P = 0.02). The immunohistochemistry of PHH3 may be associated with prognosis and could serve as a valid criterion of histological grade in MLS. [ABSTRACT FROM AUTHOR]

Abstract

A 69-year-old male complained of gross hematuria. Cystoscopy revealed a papillary pedunculated tumor. The tumor was approximately 4 cm in length, and mimicked an inverted papilloma with a small stalk and smooth surface, located on the bladder trigone. Transurethral resection of the bladder tumor was performed, and the tumor was resected en bloc. Histopathological examination revealed thick and irregular epithelial cords. Immunohistochemically, Ki-67 labeling index was 5%, p40 and CK7 were positive, and CK20 was negative. Then, this tumor was diagnosed as inverted variant of urothelial carcinoma. Even when gross appearance is compatible with inverted papilloma, pathological and immunohistochemical examinations are essential for accurate diagnosis of inverted bladder tumor. No recurrence was observed by cystoscopy 13 months after the resection.

Abstract

Gastric gland mucin secreted from the lower portion of the gastric mucosa contains unique O-linked oligosaccharides (O-glycans) having terminal α 1,4-linked N-acetylglucosamine residues (α GlcNAc). Previously, we identified human α 1,4-N-acetylglucosaminyltransferase (α 4GnT), which is responsible for the O-glycan biosynthesis and characterized α GlcNAc function in suppressing Helicobacter pylori in vitro. In the present study, we engineered [A4gnt.sup.-/-] mice to better understand its role in vivo. [A4gnt.sup.-/-] mice showed complete lack of α GlcNAc expression in gastric gland mucin. Surprisingly, all the mutant mice developed gastric adenocarcinoma through a hyperplasia-dysplasia-carcinoma sequence in the absence of H. pylori infection. Microarray and quantitative RT-PCR analysis revealed upregulation of genes encoding inflammatory chemokine ligands, proinflammatory cytokines, and growth factors, such as Ccl2, Il-11, and Hgf in the gastric mucosa of [A4gnt.sup.-/-] mice. Further supporting an important role for this O-glycan in cancer progression, we also observed significantly reduced α GlcNAc in human gastric adenocarcinoma and adenoma. Our results demonstrate that the absence of α GlcNAc triggers gastric tumorigenesis through inflammation-associated pathways in vivo.
