The purpose of this module is to review the main ultrasound-guided approaches used for regional anesthesia of the upper limb. The anatomical configuration of the upper limb, with nerves often bundled around an artery, makes regional anesthesia of the arm both accessible and reliable. In-depth knowledge of upper limb anatomy is required to match the blocked territory with the surgical area. The interscalene block is the approach most commonly used for shoulder surgery. Supraclavicular, infraclavicular, and axillary blocks are indicated for elbow and forearm surgery. Puncture techniques have evolved dramatically with ultrasound guidance. Instead of targeting the nerves directly, it is now recommended to look for diffusion areas. Typically, local anesthetics are deposited around vessels, often as a single injection. Phrenic nerve block can occur with the interscalene and supraclavicular approaches. Ulnar nerve blockade is almost never achieved with the interscalene approach and not always present with a supraclavicular block. If ultrasound guidance is used, the risk for pneumothorax with a supraclavicular approach is reduced significantly. Nerve damage and vascular puncture are possible with all approaches. If an axillary approach is chosen, the consequences of vascular puncture can be minimized because this site is compressible. Upper limb regional anesthesia has gained in popularity because of its effectiveness and the safety profile associated with ultrasound-guided techniques. L’objectif de ce module est de réviser les principales approches échoguidées d’anesthésie locorégionale du membre supérieur. La configuration anatomique du membre supérieur, avec les nerfs souvent regroupés autour d’une artère, rend l’anesthésie locorégionale du bras accessible et fiable. Une connaissance approfondie de l’anatomie du membre supérieur est nécessaire pour assurer une bonne corrélation entre le territoire anesthésié et le territoire chirurgical. Pour la chirurgie de l’épaule, l’abord interscalénique est le plus couramment utilisé. Les chirurgies du coude et de l’avant-bras peuvent être réalisées avec un bloc supraclaviculaire, infraclaviculaire ou axillaire. Les techniques de ponction ont beaucoup évolué avec l’échoguidage. Au lieu de cibler directement les nerfs, des espaces de diffusion sont recherchés. La plupart des injections se font en périvasculaire, souvent avec une injection à un seul site. Un bloc du nerf phrénique peut survenir avec les approches interscalénique et supraclaviculaire. Un défaut d’anesthésie du nerf ulnaire est presque constant avec le bloc interscalénique et est possible avec le bloc supraclaviculaire. Le risque de pneumothorax avec l’approche supraclaviculaire est diminué significativement avec l’échoguidage. Une neuropathie ou une ponction vasculaire sont possibles avec toutes les approches. Les conséquences d’une ponction vasculaire sont diminuées avec l’approche axillaire, car ce site est compressible. L’anesthésie locorégionale du membre supérieur est de plus en plus populaire en raison de l’efficacité et du profil de sécurité des techniques échoguidées.

In recent studies on ultrasound-guided infraclavicular block (ICB), the authors have favoured a single injection posterior to the axillary artery rather than multiple injections; however, procedural complications and success rates associated with single-injection ultrasound-guided ICB are not well known. We undertook an observational study to evaluate the success rates of experienced and non-experienced operators performing ICBs and to identify the complications associated with ultrasound-guided single-injection ICB. We conducted an observational cohort study of all ultrasound-guided single-injection ICBs performed over a two-year period (2008-2010). We identified the subjects for our study using a local database and excluded patients younger than 18 yr and those who received a continuous ICB. Complications (non-neurological and neurological) and ICB success rates were the primary and secondary end points, respectively. We collected the following data from patients’ charts: patient demographics, types of complications and their respective frequencies, and the experience of the clinician performing the ICBs, and we identified potential late complications by telephone interview. Using a seven-point Likert scale, two experts in regional anesthesia evaluated the likelihood of a relationship between the identified neurological signs or symptoms and the ICB. A neurologist then evaluated the complications identified as being potentially related to the ICB. Summary data were collated, and 95% confidence intervals (CI) were calculated. We reviewed 627 ICB procedures, and 496 (79%) patients received telephone interviews. Most patients were males who had undergone either plastic or orthopedic surgery. Mepivacaine 1.5% was used in 96% of cases with a median volume of 30 mL [interquartile range 30-38]. We identified 131 cases of neurological signs or symptoms. Four cases were retained as possible links to the ICB, but they underwent complete resolution of symptoms at the time of evaluation. Two possible cases of local anesthetic toxicity were observed. There was a 93% success rate (95% CI 91 to 95) and the results were comparable between the experienced and the non-experienced operators (94% vs93%, respectively). We observed few complications associated with a single-injection ultrasound-guided ICB and a high success rate regardless of the operator’s expertise. The technique appears to be reliable, easy to perform, and safe. Dans les études récentes portant sur les blocs infraclaviculaires (BIC) échoguidés, les auteurs ont privilégié une injection unique postérieure à l’artère axillaire plutôt que de multiples injections; toutefois, les complications liées à l’intervention et les taux de réussite associés aux BIC échoguidés par injection unique sont mal connus. Nous avons entrepris une étude observationnelle afin d’évaluer les taux de réussite d’opérateurs expérimentés et non expérimentés réalisant des BIC et d’identifier les complications associées aux BIC échoguidés par injection unique. Nous avons réalisé une étude de cohorte observationnelle de tous les BIC échoguidés par injection unique réalisés au cours d’une période de deux ans (2008-2010). Nous avons identifié les patients pour notre étude à l’aide d’une base de données locale, en excluant les patients de moins de 18 ans et ceux recevant un BIC en continu. Les complications (non neurologiques et neurologiques) et les taux de réussite des BIC constituaient les critères d’évaluation primaires et secondaires, respectivement. Nous avons récolté les données suivantes des dossiers des patients: données démographiques des patients, types et fréquences respectives des complications, et expérience du clinicien réalisant le BIC; nous avons identifié les complications tardives potentielles en réalisant des entrevues téléphoniques. À l’aide d’une échelle de Likert en sept points, deux experts en anesthésie régionale ont évalué la probabilité d’une association entre les signes ou symptômes neurologiques identifiés et le BIC. Un neurologue a ensuite évalué les complications identifiées comme étant potentiellement liées au BIC. Les données agrégées ont été assemblées, et des intervalles de confiance (IC) de 95 % calculés. Nous avons passé en revue 627 interventions de BIC, et 496 (79 %) des patients ont également été interrogés par téléphone par la suite. La plupart des patients étaient des hommes ayant subi une chirurgie plastique ou orthopédique. Dans 96 % des cas, de la mépivacaïne 1,5 % avait été utilisée avec un volume moyen de 30 mL [écart interquartile 30-38]. Nous avons identifié 131 cas de signes ou symptômes neurologiques. Quatre cas ont été retenus comme ayant des liens possibles avec le BIC, mais les symptômes s’étaient complètement résolus au moment de l’évaluation. Deux cas possibles de toxicité de l’anesthésique local ont été observés. Le taux de réussite était de 93 % (IC 95 % 91 à 95) et les résultats étaient comparables entre les opérateurs expérimentés et non expérimentés (94 % vs.93 %, respectivement). Nous avons observé peu de complications associées au BIC échoguidé par injection unique et un taux de réussite élevé indépendamment de l’expertise de l’opérateur. La technique semble être fiable, facile à réaliser et sécuritaire.

Carbon stable isotope ratios (δ13C), measured in human bone collagen (δ13Ccollagen) and bioapatite (δ13Cbioapatite), are commonly used indicators in ancient human diet reconstruction. The underlying assumption is that human tissues broadly reflect the δ13C signal of dietary food sources (δ13Cdiet) plus an isotopic offset. However, interpretation of results may be confounded by the differentiated routing of macronutrients (energy, that is carbohydrates and lipids, and protein) having associated different isotopic signals (δ13Cenergy, δ13Cprotein). Multiple regression analyses were conducted on data from controlled animal feeding experiments compiled by Froehle et al. (J Archaeol Sci 37:2662–2670, 2010). We derived a simple algebraic macronutrient-based model with δ13Cbioapatite= 10.1 + δ13Cdiet(‰) and δ13Ccollagen= 4.8 + 0.74 δ13Cprotein+ 0.26 δ13Cenergy(‰). While the established relationship for δ13Cbioapatiteis similar to previously known results, the model also suggests that δ13Ccollagensignal contributions originate from surprisingly consistent proportions of protein and energy macronutrients. Given that feeding experiments explore extreme variations in the proportion of diet macronutrients, the applicability of the proposed model and its predictions were tested in a variety of well-known, wild animal and human, natural contexts. The 18O/ 16O profile of a 554-m long ice core through Taylor Dome, Antarctica, shows the climate variability of the last glacial–interglacial cycle in detail and extends at least another full cycle. Taylor Dome shares the main features of the Vostok record, including the early climatic optimum with later cool phase of the last interglacial period in Antarctica. Taylor Dome δ 18O fluctuations are more abrupt and larger than those at Vostok and Byrd Station, although still less pronounced than those of the Greenland GISP2 and GRIP records. The influence of the Atlantic thermohaline circulation on regional ocean heat transport explains the partly “North Atlantic” character of this Antarctic record. Under full glacial climate (marine isotope stage 4, late stage 3, and stage 2), this marine influence diminished and Taylor Dome became more like Vostok. Varying degrees of marine influence produce climate heterogeneity within Antarctica, which may account for conflicting evidence regarding the relative phasing of Northern and Southern Hemisphere climate change.
