- Abstract
Catastrophic antiphospholipid syndrome (CAPS) is a rare and life-threatening condition characterized by the persistence of antiphospholipid antibodies and occurrence of multiple vascular occlusive events. CAPS currently remains a diagnostic challenge and requires urgent treatment. The diagnosis of CAPS is made difficult by classification criteria used as diagnostic criteria in clinical practice, knowledge derived from retrospective data and case reports, confounding clinical and biological features, and its rapid onset and mortality. The absence of prospective studies of CAPS limits the strength of evidence for guideline treatment protocols. This comprehensive review summarizes the current understanding of the disease, and discusses how the 2023 ACR/EULAR Antiphospholipid Syndrome Classification Criteria impact the definition and therapeutic management of CAPS, which is considered the most severe form of APS. The correct integration of 2023 ACR/EULAR APS classification criteria is poised to facilitate CAPS diagnosis, particularly in critical situations, offering a promising avenue for improved outcomes., SCOPUS: re.j, info:eu-repo/semantics/published

- Abstract
Open access funding provided by the Max Planck Society., Infectious wildlife diseases that circulate at the interface with domestic animals pose significant threats worldwide and require early detection and warning. Although animal tracking technologies are used to discern behavioural changes, they are rarely used to monitor wildlife diseases. Common disease-induced behavioural changes include reduced activity and lethargy (‘sickness behaviour’). Here, we investigated whether accelerometer sensors could detect the onset of African swine fever (ASF), a viral infection that induces high mortality in suids for which no vaccine is currently available. Taking advantage of an experiment designed to test an oral ASF vaccine, we equipped 12 wild boars with an accelerometer tag and quantified how ASF affects their activity pattern and behavioural fingerprint, using overall dynamic body acceleration. Wild boars showed a daily reduction in activity of 10–20% from the healthy to the viremia phase. Using change point statistics and comparing healthy individuals living in semi-free and free-ranging conditions, we show how the onset of disease-induced sickness can be detected and how such early detection could work in natural settings. During the recent COVID-19 pandemic, the rapidly progressive shortage of intravenous sedative drugs led numerous intensive care units to look for potential alternatives in patients requiring mechanical ventilation for severe acute respiratory distress syndrome (ARDS). Inhalational sedation using the AnaConDa® device for sevoflurane administration is a possible option. In a 54-year-old COVID-19 patient with severe ARDS requiring extracorporeal membranous oxygenation (ECMO), sevoflurane on AnaConDa® device was administered for 8 days but was complicated by the development of nephrogenic diabetes insipidus (NDI). Other causes of NDI or central diabetes insipidus were reasonably excluded, as in other previously published cases of NDI in ICU patients receiving prolonged sevoflurane-based sedation. In addition, the postmortem examination suggested a lower expression of aquaporin-2 in renal tubules. This observation should prompt further investigations to elucidate the role of aquaporin-2 in sevoflurane-related NDI. Inhaled isoflurane sedation is a possible alternative.

Continuing the project described by Kato et al. (2009a, arXiv:0905.1757), we collected times of superhump maxima for SU UMa-type dwarf novae mainly observed during the 2012-2013 season. We found three objects (V444 Peg, CSS J203937 and MASTER J212624) having strongly positive period derivatives despite the long orbital period (Porb). By using the period of growing stage (stage A) superhumps, we obtained mass ratios for six objects. We characterized nine new WZ Sge-type dwarf novae. We made a pilot survey of the decline rate of slowly fading part of SU UMa-type and WZ Sge-type outbursts. The decline time scale was found to generally follow the expected Porb^(1/4) dependence and WZ Sge-type outbursts also generally follow this trend. There are some objects which show slower decline rates, and we consider these objects good candidates for period bouncers. We also studied unusual behavior in some objects, including BK Lyn which made a transition from an ER UMa-type state to the novalike (standstill) state in 2013 and unusually frequent occurrence of superoutbursts in NY Ser and CR Boo. We applied least absolute shrinkage and selection operator (Lasso) power spectral analysis, which has been proven to be very effective in analyzing the Kepler data, to ground-based photometry of BK Lyn and detected the dramatic disappearance of the signal of negative superhumps in 2013. We suggested that the mass-transfer rates did not vary strongly between the ER UMa-type state and novalike state in BK Lyn, and this transition was less likely caused by a systematic variation of the mass-transfer rate., Comment: 83 pages, 95 figures, accepted for publication in PASJ

We report on photometric observations of WZ Sge-type dwarf novae, MASTER OT J211258.65+242145.4 and MASTER OT J203749.39+552210.3 which underwent outbursts in 2012. Early superhumps were recorded in both systems. During superoutburst plateau, ordinary superhumps with a period of 0.060291(4) d (MASTER J211258) and of 0.061307(9) d (MASTER J203749) in average were observed. MASTER J211258 and MASTER J203749 exhibited eight and more than four post-superoutburst rebrightenings, respectively. In the final part of the superoutburst, an increase in the superhump periods was seen in both systems. We have made a survey of WZ Sge-type dwarf novae with multiple rebrightenings, and confirmed that the superhump periods of WZ Sge-type dwarf novae with multiple rebrightenings were longer than those of WZ Sge-type dwarf novae without a rebrightening. Although WZ Sge-type dwarf novae with multiple rebrightenings have been thought to be the good candidates for period bouncers based on their low mass ratio (q) from inferred from the period of fully grown (stage B) superhumps, our new method using the period of growing superhumps (stage A superhumps), however, implies higher q than those expected from stage B superhumps. These q values appear to be consistent with the duration of the stage A superoutbursts, which likely reflects the growth time of the 3:1 resonance. We present a working hypothesis that the small fractional superhump excesses for stage B superhumps in these systems may be explained as a result that a higher gas pressure effect works in these systems than in ordinary SU UMa-type dwarf novae. This result leads to a new picture that WZ Sge-type dwarf novae with multiple rebrightenings and SU UMa-type dwarf novae without a rebrightening (they are not period bouncers) are located in the same place on the evolutionary track.

164. Reliability of Retinal Layer Annotation with a Novel, High-Resolution Optical Coherence Tomography Device: A Comparative Study

Optical coherence tomography (OCT) enables in vivo diagnostics of individual retinal layers in the living human eye. However, improved imaging resolution could aid diagnosis and monitoring of retinal diseases and identify potential new imaging biomarkers. The investigational high-resolution OCT platform (High-Res OCT; 853 nm central wavelength, 3 µm axial-resolution) has an improved axial resolution by shifting the central wavelength and increasing the light source bandwidth compared to a conventional OCT device (880 nm central wavelength, 7 µm axial-resolution). To assess the possible benefit of a higher resolution, we compared the retest reliability of retinal layer annotation from conventional and High-Res OCT, evaluated the use of High-Res OCT in patients with age-related macular degeneration (AMD), and assessed differences of both devices on subjective image quality. Thirty eyes of 30 patients with early/intermediate AMD (iAMD; mean age 75 ± 8 years) and 30 eyes of 30 age-similar subjects without macular changes (62 ± 17 years) underwent identical OCT imaging on both devices. Inter- and intra-reader reliability were analyzed for manual retinal layer annotation using EyeLab. Central OCT B-scans were graded for image quality by two graders and a mean-opinion-score (MOS) was formed and evaluated. Inter- and intra-reader reliability were higher for High-Res OCT (greatest benefit for inter-reader reliability: ganglion cell layer; for intra-reader reliability: retinal nerve fiber layer). High-Res OCT was significantly associated with an improved MOS (MOS 9/8, Z-value = 5.4, p < 0.01) mainly due to improved subjective resolution (9/7, Z-Value 6.2, p < 0.01). The retinal pigment epithelium drusen complex showed a trend towards improved retest reliability in High-Res OCT in iAMD eyes but without statistical significance. Improved axial resolution of the High-Res OCT benefits retest reliability of retinal layer annotation and improves perceived image quality and resolution. Automated image analysis algorithms could also benefit from the increased image resolution.

The larger number of models of asteroid shapes and their rotational states derived by the lightcurve inversion give us better insight into both the nature of individual objects and the whole asteroid population. With a larger statistical sample we can study the physical properties of asteroid populations, such as main-belt asteroids or individual asteroid families, in more detail. Shape models can also be used in combination with other types of observational data (IR, adaptive optics images, stellar occultations), e.g., to determine sizes and thermal properties. We use all available photometric data of asteroids to derive their physical models by the lightcurve inversion method and compare the observed pole latitude distributions of all asteroids with known convex shape models with the simulated pole latitude distributions. We used classical dense photometric lightcurves from several sources and sparse-in-time photometry from the U.S. Naval Observatory in Flagstaff, Catalina Sky Survey, and La Palma surveys (IAU codes 689, 703, 950) in the lightcurve inversion method to determine asteroid convex models and their rotational states. We also extended a simple dynamical model for the spin evolution of asteroids used in our previous paper. We present 119 new asteroid models derived from combined dense and sparse-in-time photometry. We discuss the reliability of asteroid shape models derived only from Catalina Sky Survey data (IAU code 703) and present 20 such models. By using different values for a scaling parameter cYORP (corresponds to the magnitude of the YORP momentum) in the dynamical model for the spin evolution and by comparing synthetics and observed pole-latitude distributions, we were able to constrain the typical values of the cYORP parameter as between 0.05 and 0.6., Comment: Accepted for publication in A&A, January 15, 2013

We summarize the results of a 20-year campaign to study the light curves of BK Lyncis, a nova-like star strangely located below the 2-3 hour orbital period gap in the family of cataclysmic variables. Two apparent "superhumps" dominate the nightly light curves - with periods 4.6% longer, and 3.0% shorter, than P_orb. The first appears to be associated with the star's brighter states (V~14), while the second appears to be present throughout and becomes very dominant in the low state (V~15.7). Starting in the year 2005, the star's light curve became indistinguishable from that of a dwarf nova - in particular, that of the ER UMa subclass. Reviewing all the star's oddities, we speculate: (a) BK Lyn is the remnant of the probable nova on 30 December 101, and (b) it has been fading ever since, but has taken ~2000 years for the accretion rate to drop sufficiently to permit dwarf-nova eruptions. If such behavior is common, it can explain other puzzles of CV evolution. One: why the ER UMa class even exists (because all members can be remnants of recent novae). Two: why ER UMa stars and short-period novalikes are rare (because their lifetimes, which are essentially cooling times, are short). Three: why short-period novae all decline to luminosity states far above their true quiescence (because they're just getting started in their postnova cooling). Four: why the orbital periods, accretion rates, and white-dwarf temperatures of short-period CVs are somewhat too large to arise purely from the effects of gravitational radiation (because the unexpectedly long interval of enhanced postnova brightness boosts the mean mass-transfer rate). Continuing the project described by Kato et al. (2009, arXiv:0905.1757), we studied 86 SU UMa-type dwarf novae. We confirmed the general trends such as the relation between period derivatives and orbital periods. There are some systems showing positive period derivatives despite the long orbital periods. We observed the 2011 outburst of the WZ Sge-type dwarf nova BW Scl, and recorded an O-C diagram similar to those of previously known WZ Sge-type dwarf novae. The WZ Sge-type dwarf nova OT J184228.1+483742 showed an unusual pattern of double outbursts composed of an outburst with early superhumps and one with ordinary superhumps. We propose an interpretation that a very small growth rate of the 3:1 resonance due to an extremely low mass-ratio led to a quenching of the superoutburst before the ordinary superhumps appeared. We studied ER UMa-type dwarf novae and found that V1159 Ori showed positive superhumps similar to ER UMa in the 1990s. The recently recognized ER UMa-type object BK Lyn dominantly showed negative superhumps, and its behavior was very similar to the present-day state of ER UMa. The pattern of period variations in AM CVn-type objects were very similar to short-period hydrogen-rich SU UMa-type dwarf novae, making them helium analogue of hydrogen-rich SU UMa-type dwarf novae. SBS 1108+574, a peculiar hydrogen-rich dwarf nova below the period minimum, showed a very similar pattern of period variations to those of short-period SU UMa-type dwarf novae. The mass-ratio derived from the detected orbital period suggests that this secondary is a somewhat evolved star whose hydrogen envelope was mostly stripped during the mass-exchange. CC Scl, MASTER OT J072948.66+593824.4 and OT J173516.9+154708 showed only low-amplitude superhumps with complex profiles. We report unfiltered photometry during superoutbursts of PU UMa in 2009 and 2012. The amplitude was 4.5 magnitudes above mean quiescence and lasted at least 9 to 10 days. Superhumps were present with a peak-to-peak amplitude of up to ~0.3 mag, thereby confirming it to be a member of the SU UMa family of dwarf novae. The mean superhump period during the later part of the 2012 outburst was Psh = 0.08076(40) d. Analysis of the eclipse times of minimum, supplemented with data from other researchers, revealed an orbital period of Porb = 0.077880551(17) d. The superhump period excess was epsilon = 0.037(5). During the 2012 outburst, which was the better observed of the two, the FWHM eclipse duration gradually declined from 9.5 to 5 min. We report unfiltered CCD photometry of the eclipsing dwarf nova SDSS J093249.57+472523.0 obtained during its first confirmed outburst in 2011 March. The outburst amplitude was at least 3.0 magnitudes above mean quiescence and it lasted at least 11 days, although we missed the beginning of the outburst. Superhumps having peak-to-peak amplitude up to 0.3 magnitudes were present during the outburst, thereby establishing it to be a member of the SU UMa family. The mean superhump period was Psh = 0.06814(11) d. Analysis of our measurements of eclipse times of minimum, supplemented with data from other researchers, allowed us to measure the orbital period as Porb = 0.06630354(5) d. The superhump period excess was epsilon = 0.028(1) which is consistent with of SU UMa systems of similar Porb. The FWHM eclipse duration varied between 6 and 13 mins and the eclipse depth was up to 1.6 magnitudes., Comment: 15 pages, 4 figures. We report CCD photometry of the superoutburst of the dwarf nova V1212 Tau obtained during 2011 January and February. The outburst amplitude was at least 6 magnitudes and it lasted at least 12 days. Three distinct superhump regimes were observed. Initially low amplitude superhumps (0.03 to 0.05 magnitude peak-to-peak) with Psh = 0.0782(52) d were present. The superhumps reached a maximum amplitude of 0.31 magnitudes at the beginning of the plateau phase, with Psh = 0.07031(96) d. Subsequently the star began to fade slowly. During the first part of the decline, the period increased with dPsh/dt = +1.62(9) x 10-3 and the amplitude of the superhumps also declined. Mid way through the slow decline, the superhumps partially regrew and this point coincided with a change to a new superhump regime during which the period decreased with dPsh/dt = -1.50(39) x 10-3. We report unfiltered photometry of BG Arietis (= SDSS J015151.87+140047.2) in 2009 and 2010 which shows the presence of superhumps with peak-to-peak amplitude of up to 0.36 magnitudes showing this to be an SU UMa type dwarf nova. The outburst amplitude was 5.1 magnitudes above a mean quiescence magnitude of 19.9. The 2010 event lasted at least 20 days during which we observed a precursor outburst which was immediately followed by a superoutburst. Modulations were observed during the rise to superoutburst with a period approximately half Porb. The mean superhump period during the plateau phase was Psh = 0.0849(6) d, representing a superhump period excess epsilon = 0.030(7). We also observed small eclipses of depth 0.06 to 0.13 magnitude and FWHM duration of ~12 mins ({\Delta}{\Phi}1/2 = 0.10), which we interpret as grazing eclipses of the accretion disc. The 2009 superoutburst was less well observed, especially in the early stages, so we cannot tell whether there was a precursor outburst. The mean superhump period during the plateau was Psh = 0.0851(4) d, but towards the end of the plateau it suddenly decreased to Psh = 0.0845(11) d. Continuing the project described by Kato et al. (2009, PASJ 61, S395, arXiv:0905.1757), we collected times of superhump maxima for 51 SU UMa-type dwarf novae mainly observed during the 2010-2011 season. Although most of the new data for systems with short superhump periods basically confirmed the findings by Kato et al. (2009) and Kato et al. (2010, PASJ 62, 1525, arXiv:1009.5444), the long-period system GX Cas showed an exceptionally large positive period derivative. An analysis of public Kepler data of V344 Lyr and V1504 Cyg yielded less striking stage transitions. In V344 Lyr, there was prominent secondary component growing during the late stage of superoutbursts, and the component persisted at least for two more cycles of successive normal outbursts. We also investigated the superoutbursts of two conspicuous eclipsing objects: HT Cas and the WZ Sge-type object SDSS J080434.20+510349.2. Strong beat phenomena were detected in both objects, and late-stage superhumps in the latter object had an almost constant luminosity during the repeated rebrightenings. The WZ Sge-type object SDSS J133941.11+484727.5 showed a phase reversal around the rapid fading from the superoutburst. The object showed a prominent beat phenomenon even after the end of the superoutburst. A pilot study of superhump amplitudes indicated that the amplitudes of superhumps are strongly correlated with orbital periods, and the dependence on the inclination is weak in systems with inclinations smaller than 80 deg., Comment: 87 pages, 92 figures, Accepted for publication in PASJ, correction in the author list and text

We present time resolved photometry of the cataclysmic variable SDSS J081610.84+453010.2 and have established for the first time that it is an eclipsing dwarf nova. We observed an outburst of the system which lasted about 11 days and had an amplitude of 3.4 magnitudes above mean quiescence. From an analysis of the eclipse times of minimum during the outburst, we determined the orbital period as Porb = 0.2096(4) d or 5.030(10) h. The orbital period places it above the period gap in the distribution of orbital periods of dwarf novae. The eclipses are of short duration (average FWHM = 10.7 min or 0.036 of the orbital period) and shallow (average 0.4 mag during outburst and 0.6 magnitude in quiescence), suggesting a grazing eclipse., Comment: 14 pages, 3 Tables, 5 Figures. As an extension of the project in Kato et al. (2009, arXiv:0905.1757), we collected times of superhump maxima for 61 SU UMa-type dwarf novae mainly observed during the 2009-2010 season. The newly obtained data confirmed the basic findings reported in Kato et al. (2009): the presence of stages A-C, as well as the predominance of positive period derivatives during stage B in systems with superhump periods shorter than 0.07 d. There was a systematic difference in period derivatives for systems with superhump periods longer than 0.075 d between this study and Kato et al. (2009). We suggest that this difference is possibly caused by the relative lack of frequently outbursting SU UMa-type dwarf novae in this period regime in the present study. We recorded a strong beat phenomenon during the 2009 superoutburst of IY UMa. The close correlation between the beat period and superhump period suggests that the changing angular velocity of the apsidal motion of the elliptical disk is responsible for the variation of superhump periods. We also described three new WZ Sge-type objects with established early superhumps and one with likely early superhumps. We also suggest that two systems, VX For and EL UMa, are WZ Sge-type dwarf novae with multiple rebrightenings. The O-C variation in OT J213806.6+261957 suggests that the frequent absence of rebrightenings in very short-Porb objects can be a result of sustained superoutburst plateau at the epoch when usual SU UMa-type dwarf novae return to quiescence preceding a rebrightening. We report photometry of three outbursts of NN Cam in 2007, 2008 and 2009. The 2007 event started with a normal outburst, lasting about 4 days, which was a precursor to a superoutburst lasting at least 13 days. Both the precursor and the superoutburst had an amplitude of 4.9 mag above mean quiescence. Superhumps with a maximum peak-to-peak amplitude of 0.22 mag were detected during the superoutburst with a mean superhump period Psh = 0.07385(56) d. Psh decreased continuously with dPsh /dt = -1.72(23) x 10-3. We used our measurement to confirm that the shorter of two possible values of Porb reported by another researcher is the correct one, Porb = 0.0717 d. The 2008 outburst was rather poorly observed, although we present evidence that this too may have been a superoutburst. The 2009 event was also a superoutburst, with Psh = 0.07414(44) d, but we could find no evidence for a precursor. From the 2007 and 2009 data, we report a superhump period excess of epsilon = 0.030(8) to 0.034(6), which is typical for SU UMa dwarf novae of similar orbital period, and estimate the binary mass ratio q = Mwd/Msec $\simeq$ 0.11 to 0.17, Comment: 20 pages, 10 figures, accepted for publication in the Journal of the British Astronomical Association

We revisit a graph width parameter that we dub bipartite treewidth, along with its associated graph decomposition that we call bipartite tree decomposition. Bipartite treewidth can be seen as a common generalization of treewidth and the odd cycle transversal number. Intuitively, a bipartite tree decomposition is a tree decomposition whose bags induce almost bipartite graphs and whose adhesions contain at most one vertex from the bipartite part of any other bag, while the width of such decomposition measures how far the bags are from being bipartite. Adapted from a tree decomposition originally defined by Demaine, Hajiaghayi, and Kawarabayashi [SODA 2010] and explicitly defined by Tazari [Theor. Comput. Sci. 2012], bipartite treewidth appears to play a crucial role for solving problems related to odd-minors, which have recently attracted considerable attention. As a first step toward a theory for solving these problems efficiently, the main goal of this paper is to develop dynamic programming techniques to solve problems on graphs of small bipartite treewidth. For such graphs, we provide a number of para-NP-completeness results, FPT-algorithms, and XP-algorithms, as well as several open problems. In particular, we show that K_t-Subgraph-Cover, Weighted Vertex Cover/Independent Set, Odd Cycle Transversal, and Maximum Weighted Cut are FPT parameterized by bipartite treewidth. We also provide the following complexity dichotomy when H is a 2-connected graph, for each of the H-Subgraph-Packing, H-Induced-Packing, H-Scattered-Packing, and H-Odd-Minor-Packing problems: if H is bipartite, then the problem is para-NP-complete parameterized by bipartite treewidth while, if H is non-bipartite, then the problem is solvable in XP-time. Beyond bipartite treewidth, we define 1-ℋ-treewidth by replacing the bipartite graph class by any graph class ℋ. Most of the technology developed here also works for this more general parameter.

- Abstract
Infectious wildlife diseases that circulate at the interface with domestic animals pose significant threats worldwide and require early detection and warning. Although animal tracking technologies are used to discern behavioural changes, they are rarely used to monitor wildlife diseases. Common disease-induced behavioural changes include reduced activity and lethargy ('sickness behaviour'). Here, we investigated whether accelerometer sensors could detect the onset of African swine fever (ASF), a viral infection that induces high mortality in suids for which no vaccine is currently available. Taking advantage of an experiment designed to test an oral ASF vaccine, we equipped 12 wild boars with an accelerometer tag and quantified how ASF affects their activity pattern and behavioural fingerprint, using overall dynamic body acceleration. Wild boars showed a daily reduction in activity of 10-20% from the healthy to the viremia phase. Using change point statistics and comparing healthy individuals living in semi-free and free-ranging conditions, we show how the onset of disease-induced sickness can be detected and how such early detection could work in natural settings. Timely detection of infection in animals is crucial for disease surveillance and control, and accelerometer technology on sentinel animals provides a viable complementary tool to existing disease management approaches.

- Abstract
Background: We previously reported that impaired type I IFN activity, due to inborn errors of TLR3- and TLR7-dependent type I interferon (IFN) immunity or to autoantibodies against type I IFN, account for 15–20% of cases of life-threatening COVID-19 in unvaccinated patients. Therefore, the determinants of life-threatening COVID-19 remain to be identified in ~ 80% of cases. Methods: We report here a genome-wide rare variant burden association analysis in 3269 unvaccinated patients with life-threatening COVID-19, and 1373 unvaccinated SARS-CoV-2-infected individuals without pneumonia. Among the 928 patients tested for autoantibodies against type I IFN, a quarter (234) were positive and were excluded. Results: No gene reached genome-wide significance. Under a recessive model, the most significant gene with at-risk variants was TLR7, with an OR of 27.68 (95%CI 1.5–528.7, P = 1.1 × 10−4) for biochemically loss-of-function (bLOF) variants. We replicated the enrichment in rare predicted LOF (pLOF) variants at 13 influenza susceptibility loci involved in TLR3-dependent type I IFN immunity (OR = 3.70[95%CI 1.3–8.2], P = 2.1 × 10−4). This enrichment was further strengthened by (1) adding the recently reported TYK2 and TLR7 COVID-19 loci, particularly under a recessive model (OR = 19.65[95%CI 2.1–2635.4], P = 3.4 × 10−3), and (2) considering as pLOF branchpoint variants with potentially strong impacts on splicing among the 15 loci (OR = 4.40[9%CI 2.3–8.4], P = 7.7 × 10−8). Finally, the patients with pLOF/bLOF variants at these 15 loci were significantly younger (mean age [SD] = 43.3 [20.3] years) than the other patients (56.0 [17.3] years; P = 1.68 × 10−5). Conclusions: Rare variants of TLR3- and TLR7-dependent type I IFN immunity genes can underlie life-threatening COVID-19, particularly with recessive inheritance, in patients under 60 years old.

Background: Seronegative autoimmune hepatitis in children is a rare but potentially severe disease, sometimes requiring liver transplantation. This type of hepatitis may be associated with various immunological and hematological disorders, ranging from isolated lymphopenia to aplastic anemia. Precise pathophysiological mechanisms are still unknown, but the role of viruses cannot be excluded, either as directly pathogenic or as triggers, responsible for an inappropriate immune stimulation. Having the impression of an increasing number of seronegative autoimmune hepatitis since the beginning of SARSCoV-2 pandemia period, we hypothesized that SARS-CoV-2 virus could be an infectious trigger. Methods: We conducted a retrospective, observational, descriptive study about children with seronegative autoimmune hepatitis, in a tertiary care center, between 2010 and 2022. Results: Thirty-two patients were included. The overall incidence of seronegative autoimmune hepatitis increased 3.3-fold in 2020-2022, during the SARS-CoV-2 pandemia period (16 patients in 2.8 years) compared with 2010-2019 the pre pandemia period (16 patients in 9 years). Patients' clinical and biochemical liver characteristics did not differ between the two periods. Hematological damages were less severe during the pandemia period. Immunological studies revealed a dysregulated immune response. The initiation of immunosuppressive therapy (corticosteroids ± cyclosporine) was earlier during the pandemia period than before. Conclusion: In cases of undetermined acute hepatitis, an immune-mediated origin should be considered, prompting a liver biopsy. If the histological aspect points to an immune origin, immunosuppressive treatment should be instituted even though autoimmune hepatitis antibodies are negative. Close hematological monitoring must be performed in all cases. The 3.3-fold increase of cases during the SARS-CoV-2 pandemia will need to be further analyzed to better understand the underlying immunological mechanisms, and to prove its potential involvement. The development of the human immune system lasts for several years after birth. The impact of this maturation phase on the quality of adaptive immunity and the acquisition of immunological memory after infection at a young age remains incompletely defined. Here, using an antigen-reactive T cell (ARTE) assay and multidimensional flow cytometry, we profiled circulating severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)–reactive CD3
+ CD4+ CD154+ T cells in children and adults before infection, during infection, and 11 months after infection, stratifying children into separate age groups and adults according to disease severity. During SARS-CoV-2 infection, children younger than 5 years old displayed a lower antiviral CD4+ T cell response, whereas children older than 5 years and adults with mild disease had, quantitatively and phenotypically, comparable virus-reactive CD4+ T cell responses. Adults with severe disease mounted a response characterized by higher frequencies of virus-reactive proinflammatory and cytotoxic T cells. After SARS-CoV-2 infection, preschool-age children not only maintained neutralizing SARS-CoV-2–reactive antibodies postinfection comparable to adults but also had phenotypically distinct memory T cells displaying high inflammatory features and properties associated with migration toward inflamed sites. Moreover, preschool-age children had markedly fewer circulating virus-reactive memory B cells compared with the other cohorts. Collectively, our results reveal unique facets of antiviral immunity in humans at a young age and indicate that the maturation of adaptive responses against SARS-CoV-2 toward an adult-like profile occurs in a progressive manner. PURPOSE: Breast cancer (BC) in young women is uncommon and tends to present with more aggressive characteristics. To better understand and characterize this scenario in Brazil through real-world data, we performed a subanalysis of AMAZONA III study (ClinicalTrials.gov identifier: NCT02663973). METHODS: The AMAZONA III study (GBECAM 0115) is a prospective registry that included 2,950 women newly diagnosed with invasive BC in Brazil from January 2016 until March 2018 at 22 sites. Valid data were obtained from 2,888 patients regarding age at diagnosis and complete baseline information. To compare epidemiologic and clinicopathological features at the time of diagnosis, patients with BC were divided into two groups according to age: ≤ 40 years and > 40 years. Quantitative variables were described as means, and categorical variables were described as frequencies and percentages and compared using the Pearson’s χ2 test. RESULTS: Of 2,888 women diagnosed with BC, 486 (17%) were ≤ 40 years old. Young women had higher educational level, most were employed and a significant number were married (P < .001 for all associations). Younger patients were more symptomatic at BC diagnosis (P < .001), and they also presented more frequently with stage III, T3/T4, grade 3 tumors, HER-2–positive, luminal B, and triple-negative subtypes. CONCLUSION: Brazilian women younger than age 40 years have unfavorable clinicopathological features of BC at diagnosis, with more aggressive subtypes and advanced stage when compared with older women. These differences are not explained by socioeconomic or ethnic imbalances. The causes of a higher prevalence of BC among young women in Brazil deserve additional investigation.

Diversity in education is not about different races only; in the South African context it also refers, for instance, to types of schools or different learning styles. Inclusivity in education thus refers to all learners, regardless of their abilities, being noticed and encouraged to share and learn in the same space. However, the South African education system is still struggling to ensure that learners with disabilities, especially those with visual impairment, are fully catered for in mainstream schools. In the study reported on here a qualitative research methodology was used to collect data through semi-structured interviews and observations with selected participants to explore challenges experienced by partially sighted learners in mainstream schools. It was found that even though learners with visual impairment are now physically integrated in mainstream classes, they are not truly included. Furthermore, teacher training should be enhanced, and teacher support reviewed while involving parents of visually impaired learners in their children’s schooling.
