Abstract

We introduce the notion of chiral symplectic cores in a vertex Poisson variety, which can be viewed as analogs of symplectic leaves in Poisson varieties. As an application we show that any quasi-lisse vertex algebra is a quantization of the arc space of its associated variety, in the sense that its reduced singular support coincides with the reduced arc space of its associated variety. We also show that the coordinate ring of the arc space of Slodowy slices is free over its vertex Poisson center, and the latter coincides with the vertex Poisson center of the coordinate ring of the arc space of the dual of the corresponding simple Lie algebra.

In this note, we prove that the universal affine vertex algebra associated with a simple Lie algebra g is simple if and only if the associated variety of its unique simple quotient is equal to g*. Abstract

LyMA trial has demonstrated the benefit of rituximab maintenance after autologous stem cell transplantation (ASCT) in previously untreated mantle-cell lymphoma patients (MCL). Induction consisted of four courses of R-DHAP (rituximab, dexamethasone, high-dose cytarabine, and platinum derivative). The platinum derivative (PD) choice was free: R-DHA-cisplatin, R-DHA-carboplatin, or R-DHA-oxaliplatin. We investigated the prognostic impact of each PD. PFS and OS calculated from inclusion and investigated in an intention-to-treat (ITT) (= 298) and per-protocol analyses (PP) (n = 227). R-DHACis, R-DHACa, or R-DHAOx were used at first cycle in 184, 76, and 38 patients, respectively. Overall, 71 patients (59 in the R-DHACis) required a change in PD, mainly because of PD toxicity. In ITT-analysis, PFS in the R-DHACis and R-DHACa groups were similar (4-year PFS of 65%), while R-DHAOx had a better PFS (4-year PFS of 65% versus 86.5%, respectively, HR = 0.44, p = 0.02). The 4-year OS was 92% for R-DHAOx versus 75.9% for R-DHACis/DHACa (HR = 0.37, p = 0.03). Similar results were yielded in the PP analysis. Low MIPI and R-DHAOx were independent favorable prognostic markers for both PFS (HR = 0.44, p = 0.035) and OS (HR = 0.36, p = 0.045). In vitro and in silico analyses confirmed that oxaliplatin has an anti-MCL cytotoxic effect that differs from that of other PD. R-DHAOx before ASCT provides better outcome in transplantation eligible young MCL patients.

Abstract

The pathogenic role of terminally differentiated effector memory (TEMRA) CD8+T cells has been implicated in kidney transplant failure. The authors showed that humoral rejection of kidney allografts is associated with an accumulation of cytolytic TEMRA CD8+T cells in blood and in kidney graft biopsies. They demonstrated that TEMRA CD8+T cells from kidney transplant recipients exhibit enhanced migratory properties compared with effector memory CD8+T cells and that the chemokine CXCL12 not only promotes migration of TEMRA CD8+T cells toward nonlymphoid organs but also triggers a purinergic P2X4 receptor–dependent proinflammatory response. They also found that agents aimed at potential TEMRA CD8+T cell–specific targets inhibited the migration of TEMRA CD8+T cells from kidney transplant recipients, suggesting a possible strategy in treating kidney transplant failure.

Abstract

CEPRI’s thinking on the subject begins with the postulate that in densely urbanised cities the future of flood prone areas cannot be reduced to two alternatives: not constructing or continuing to construct as was done over the past decades, i.e. without taking into account the presence of the risks of flooding (overflowing of waterways, coastal flooding, storm water runoff, etc.). It seems that there exists a third possibility: that of developing cities intelligently, taking account of their environment and of the associated risks. Backed up with European urban projects currently being researched or that have been realised, CEPRI has identified six principles making it possible to take into account the risks of flooding in city planning.

Abstract

Objectifs : L’objectif principal de la recherche était d’évaluer les méthodes de traitements des lésions carieuses interproximales et occlusales chez l’enfant et l’adolescent par les dentistes canadiens afin de voir si ceux-ci se conforment aux nouvelles recommandations internationales sur la gestion de la carie dentaire. Pour ce faire, nous avons évalué le stade carieux auquel un dentiste décide de réaliser une restauration, la technique de restauration utilisée ainsi que le matériau choisi. L’objectif secondaire était d’évaluer s’il y a une différence entre les traitements réalisés par les dentistes selon différentes données socio-démographiques comme leur province de pratique, leur université de graduation, leur année de graduation et leur degré de spécialisation. Une comparaison a aussi été faite en analysant leur âge, leur sexe et leur fréquence de traitement chez une population pédiatrique. Méthode : Un questionnaire électronique a été créé sur le serveur protégé REDCap puis envoyé par courriel aux dentistes membres de différents ordres de réglementation provinciales canadiennes. Ce questionnaire comportait 11 questions portant d’abord sur les facteurs démographiques du répondant. Le dentiste répondant était ensuite exposé à 3 mises en situation cliniques sur la carie dentaire chez l’enfant et l’adolescent. Résultats : Sept-cent deux questionnaires soumis ont été inclus dans l’analyse statistique. 442 dentistes (62,6%) ont répondu traiter par ablation puis restauration une carie interproximale limitée à l’émail sur une molaire permanente. 502 dentistes (70,9%) des dentistes ont répondu faire de même pour une molaire primaire. Pour les lésions occlusales, ces chiffres sont de 300 dentistes (43,2%) pour une molaire permanente et de 269 dentistes (38,4%) pour une molaire primaire. La majorité des dentistes ont choisi des techniques de restauration plus conservatrices telles que la préparation en boite pour les dents permanentes (58,3%) et la résine composite comme matériau de restauration. L’âge, le sexe, l’année de graduation et la province de pratique ou de graduation sont des facteurs démographiques qui semblent avoir un impact sur les décisions des dentistes. Conclusion : Les résultats montrent que plusieurs des dentistes canadiens répondants traiteraient les lésions carieuses, en particulier les lésions interproximales, de manière plus agressive que les nouvelles recommandations. Par conséquent, cette étude montre qu'il existe des obstacles à l'intégration de nouvelles recommandations sur les techniques non chirurgicales pour le traitement de la carie dentaire., Objectives: The main objective of the research was to assess the management of interproximal and occlusal caries in children and adolescents by Canadian dentists to assess compliance with the new international recommendations on dental caries management. To do this, the restorative threshold, the restoration techniques materials were evaluated. The secondary objective was to assess whether there was a difference between the treatments performed by dentists according to different sociodemographic data, such as their province of practice, their graduation year and University, and their degree of specialization. A comparison was also made analyzing their age, their gender and their frequency of treatment in a pediatric population. Method: An electronic survey was created with REDCap protected server and was sent by email to members of various Canadian provincial regulatory bodies. This survey included 11 questions on the demographic factors of the respondent. The responding dentist was then exposed to 3 clinical situations on dental caries in children and adolescents. Results: Seven hundred and two completed surveys were included in the statistical analysis. For interproximal carious lesions limited to the enamel, 442 (62.6%) responded treating by surgical removal for a permanent molar and 502 (70.9%) responded doing the same for a primary tooth. For occlusal carious lesions, those numbers were 300 dentists (43.2%) for a permanent molar and 269 dentists (38.4%) for a primary molar. The majority of dentists used conservative techniques such as a box-slot preparation for permanent teeth (58.3%) and restored primary and permanent molars with composite resin. The age, year of graduation, gender, province of graduation and of practice appear to have a statistically significant impact on the restorative threshold. Conclusion: Results show that many of the responding Canadian dentists would treat carious lesions, particularly interproximal lesions, more aggressively than the new recommendations for management of dental caries.

Lille University Hospital is the tertiary care center for the North of France, the second greatest French region in population density (189 people per 1 km2), also considered a metabolic area with high number of overweight patients. The study was approved by the institutional data protection authority of Lille University Hospital.

Abstract

BACKGROUND: In view of the high mortality rate of immunocompromised patients with ARDS, it is important to identify targets for improvement.RESEARCH QUESTION: This study investigated factors associated with mortality in this specific ARDS population, including factors related to respiratory mechanics (plateau pressure [Pplat,rs], compliance [Crs], and driving pressure [Delta Prs]).STUDY DESIGN AND METHODS: This study consisted of a predefined secondary analysis of the EFRAIM data. Overall, 789 of 1,611 patients met the Berlin criteria for ARDS, and Pplat,rs, Delta Prs, and Crs were available for 494 patients. A hierarchical model was used to assess factors at ARDS onset independently associated with hospital mortality.RESULTS: Hospital mortality was 56.3%. After adjustment, variables independently associated with hospital mortality included ARDS of undetermined etiology (OR, 1.66; 95% CI, 1.012.72), need for vasopressors (OR, 1.91; 95% CI, 1.27-2.88), and need for renal replacement therapy (OR, 2.02; 95% CI, 1.37-2.97). ARDS severity according to the Berlin definition, neutropenia on admission, and the type of underlying disease were not significantly associated with mortality. Before adjustment, higher Pplat,rs, higher Delta Prs, and lower Crs were associated with higher mortality. Addition of each of these individual variables to the final hierarchical model revealed a significant association with mortality: Delta Prs (OR, 1.08; 95% CI, 1.05-1.12), Pplat,rs (OR, 1.07; 95% CI, 1.04-1.11), and Crs (OR, 0.97; 95% CI, 0.95-0.98). Tidal volume was not associated with mortality.INTERPRETATION: In immunocompromised patients with ARDS, respiratory mechanics provide additional prognostic information to predictors of hospital mortality. Studies designed to define lung-protective ventilation guided by these physiological variables may be warranted in this specific population.

Abstract

Background: Respiratory complications of solid organ transplant (SOT) are a diagnostic and therapeutic challenge when requiring intensive care unit (ICU) admission. We aimed at describing this challenge in a prospective cohort of SOT recipients admitted in the ICU. Methods: In this post hoc analysis of an international cohort of immunocompromised patients admitted in the ICU for an acute respiratory failure, we analyzed all SOT recipients and compared their severity, etiologic diagnosis, prognosis, and outcome according to the performance of an invasive diagnostic strategy (encompassing a fiber-optic bronchoscopy and bronchoalveolar lavage), the type of transplanted organ, and the need of invasive ventilation at day 1. Results: Among 1611 patients included in the primary study, 142 were SOT recipients (kidney, n = 73; 51.4%; lung, n = 33; 23.2%; liver, n = 29; 20.4%; heart, n = 7; 4.9%). Lung transplant recipients were younger than other SOT recipients, and severity did not differ across type of received organ. An invasive diagnostic strategy was more frequently performed in lung transplant recipients with a trend toward a higher rate of bacterial etiology in lung than kidney transplant recipients. Overall ICU survival of SOT recipients was 75.4%. Invasive diagnostic strategy, type of transplanted organ, and need of invasive mechanical ventilation at day 1 did not affect ICU prognosis. Conclusions: ICU management of hypoxemic acute respiratory failure in SOT recipients translated into a low ICU mortality rate, whatever the transplanted organ or the acute respiratory failure cause. They also fail to specify the anterior visceral endoderm, a specialized signaling center which has been shown to be required for the establishment of anterior identity in the epiblast. Our study reveals that Nodal.sup.-/- epiblast cells nevertheless express prematurely and ectopically molecular markers specific of anterior fate. Our analysis shows that neural specification occurs and regional identities characteristic of the forebrain are established precociously in the Nodal.sup.-/- mutant with a sequential progression equivalent to that of wild-type embryo. When explanted and cultured in vitro, Nodal.sup.-/- epiblast cells readily differentiate into neurons. Genes normally transcribed in organizer-derived tissues, such as Gsc and Foxa2, are also expressed in Nodal.sup.-/- epiblast. The analysis of Nodal.sup.-/- ;Gsc.sup.-/- compound mutant embryos shows that Gsc activity plays no critical role in the acquisition of forebrain characters by Nodal-deficient cells. This study suggests that the initial steps of neural specification and forebrain development may take place well before gastrulation in the mouse and highlights a possible role for Nodal, at pregastrula stages, in the inhibition of anterior and neural fate determination.
