Antiretroviral therapy (ART) has improved the lifespan of people living with HIV. However, their immune system remains in a state of sustained activation/inflammation, which favors viral replication and depletion of helper T-cells with varying profiles according to ART-response. We herein sought to ascertain the inflammatory profile of adolescents living with perinatal HIV-1 infection (ALPHI) receiving ART in an African context. In this cross-sectional and comparative study among ART-experienced ALPHI in Yaoundé-Cameroon, HIV-1 RNA was measured by Abbott Real-time PCR; CD4 cells were enumerated using flow cytometry; serum cytokines were measured by ELISA; HIV-1 proviral DNA was genotyped by Sanger-sequencing; and archived drug resistance mutations (ADRMs) were interpreted using Stanford HIVdb.v9.0.1. Introduction: The analysis of organic residue in ancient vessels to investigate early-age civilization habits is an important archeological application that needs advanced analytical methods. However, these procedures should meet inherent requisites such as low sampling invasiveness and high sensitivity for trace analysis. This study deals with the development of advanced analytical methods for the detection of opium alkaloids in ceramic vessels and its first application to the study of Daunian pots dating back to the VIII–IV sec BC.Methods: All the stages of the analytical procedure, from sampling to analysis, were carefully optimized. Concerning sampling, the traditional scraping approach was compared with a swabbing strategy which permitted minimizing sample encroachment. Extraction was based on pressurized liquid extraction or ultrasound-assisted liquid extraction, followed by dispersive liquid–liquid microextraction, which allowed concentration enrichment. On the other hand, a UHPLC-MS/MS method was specifically developed and validated to obtain reliable data. Some Daunian pots, belonging to the Ceci-Macrini private archeological collection, were selected for sample withdrawal as their iconography could suggest opium usage.Results: Several of the analyzed samples resulted positive to thebaine and less frequently to morphine and codeine; furthermore, 70% of the analyzed items tested positive for at least one opium alkaloid. Positive findings were common to all the samples collected in the pots, suggesting that scraping and swabbing provided comparable results and validating this unusual sampling strategy. All samples were additionally analyzed by UHPLC-HRMS to further improve the confidence level of the identified compounds. The obtained results shed new light on the hypothesis of opium usage by the ancient Daunian civilization. Furthermore, this study provided suitable analytical tools for further investigations on the same topic, with a good level of confidence in the quality of the results.

Cervical cancer (CC) is one of the most frequent cancers in women worldwide. The epithelial-mesenchymal transition (EMT) and the extracellular release of TGF-β are phenomena typically associated with different tumorigenic processes, including tumour cell proliferation and metastatization. Specific human microRNAs (miRNAs; miRs) involved in these tumorigenic processes have been identified, becoming important diagnostic and prognostic markers, and even potential therapeutic targets. In parallel, different studies have also shown that plant miRNAs can mediate a cross-kingdom regulation (CKR) of mammalian genes and modulate host's gene expression under pathological conditions, restoring the regulatory activity of endogenous miRNAs lost in cancer. In our previous studies, the miRNome from Moringa oleifera Lam. (henceforth moringa or mol) has been sequenced, showing the presence of several conserved miRNAs in the plant kingdom, whose ability to differentially regulate proliferation and apoptosis in healthy and cancer cells has been demonstrated. Furthermore, the effects of mol-miR treatment on tumorigenesis and EMT have been proved in liver tumour cells. According to these premises, we here investigated the proteomic profile of CC-derived HeLa cells exposed to a mol-miRNA pool, demonstrating the down-representation of specific factors involved in tumorigenesis. The treatment with plant miRs was able to modulate proteins involved in several biological processes linked to EMT. Furthermore, it reduced the expression of TGF-β and significantly inhibited cell motility, as observed following Scratch test and cell viability measurements, with a significant increase of apoptotic events. In conclusion, our results suggest and pave the way for the development of new potential therapeutic approaches based on CKR mediated by plant miRNAs for contrasting human cervical cancer, even in the form of adjuvants to classic treatments for limiting their side effects.

There is considerable unused and unproductive land in rural areas of the Puglia region, Southern Italy. These areas and their local cultivators/growers have always been overlooked by academic, policy, and investment circles despite their potential to improve food security and the livelihood of rural communities. Therefore, it is crucial to evaluate the sustainability potential of these areas to produce food, feed, and non-food products. This evaluation will play a vital role in the sustainable development of rural regions such as the Puglia region. In this review, we highlighted important aspects regarding the management potential and the expected contribution that various types of marginal areas (MAs) could add to the regional economy of Puglia. The authors focused on the extent to which Puglia’s marginal lands can realistically be utilized to meet Italy’s food production targets, considering the economic, social, and environmental potential of different marginal area types within the Puglia territories. In writing this review, we have been inspired by the lack of sufficient information necessary to carry out a plan for the revitalization of MAs and the sustainable development of regional rural areas. Although Italian scholars have not extensively researched MAs, the available data suggest that they could significantly contribute to the development of regional economy and food security, despite their complexity and low-input nature. The scientific evidence suggests that the main challenge associated with using MAs for food and/or non-food production is balancing biodiversity conservation with local residents’ social and economic development. Presently, some small-scale or family farms are already operating within MAs. However, they still contribute minimally to the overall agricultural production in the region, even though they have the potential to play an important economic role for numerous rural communities in Puglia. Furthermore, the available data indicate that over 40% of existing farmers in MAs require greater support to sustain their activities. Such support should be tailored to the local context, built on and, where appropriate, improved upon existing practices, while addressing various threats to these areas. In this regard, sustainable development policies should shape MAs’ landscapes through the support of initiatives aimed at the recovery of traditional agricultural activities, the use of local varieties, the protection of the environment, and the conservation of biodiversity. The authors believe that this review provides policymakers with recommendations to consider when supporting the sustainable use of MAs as a potential source of food security and sustainable development for people living in these areas.

The use of wireless sensors for real-time sensing of substrate water status and electrical conductivity could be an effective tool for precision irrigation management in soilless cultivation. In this research, the effects of timer-based (TB) compared to smart sensor-based irrigation (SB) were investigated. The highest consumption of fertilizers and water were recorded in TB, with nutrient solution and total applied water savings of 38% and 26%, respectively, in SB. The highest yield was obtained in SB treatment, with a total and marketable yield decrease of 7% in TB, with no differences in terms of the total soluble solids content, dry matter, firmness, juice pH and titratable acidity of the strawberry fruits. The higher yield, combined with water and nutrient saving in SB, allowed water use efficiency (fresh weight of marketable fruits per liter of total water applied) to be increased by 46% and nutrient productivity (fresh weight of marketable product per gram of nutrient supplied via nutrient solution) by 74%. The study confirms that sensor-based, compared to empiric fertigation management, ameliorates the sustainability of open, free-drain, soilless cultivation of strawberry, leading to better resource use without compromising crop performance and fruit quality.

Reducing fertilizer input is a goal for helping greenhouse farming to achieve higher sustainability in the production process while preserving overall crop performance and quality. Wild rocket plants were cultivated in a plastic greenhouse divided into two independent sectors, one for soil-bound (SbS) cultivation and another equipped for soilless (ScS) cultivation systems. In both SbS and ScS, the crop was subjected to treatments consisting of a high- and a low-input fertilization program (HF and LF treatment, respectively). Water use efficiency (WUE) and partial factor productivity (PFP) for nutrients (N, P, K, Ca, and Mg for ScS, and N for SbS) were measured. Rocket leaves, separated for the cultivation system and fertilization program and collected at different cuts during the growing cycle, were cold stored at 10 °C until 16 d. On each sampling day (at harvest and during storage), the sensory parameters, respiration rate, dry matter, color, electrolyte leakage, antioxidant activity, total phenols, total chlorophyll and ammonia content were evaluated. In ScS, the PFP for all nutrients supplied as fertilizers showed a significant increase with the LF treatment, with values higher than 30% recorded for N, K, and Ca. As for the postharvest performance, rocket leaves cultivated in ScS showed better qualitative traits than those cultivated in SbS, as suggested by the lower values of ammonia content and electrolyte leakage recorded at the end of storage period in samples grown in ScS. Moreover, in ScS, the data showed lower membrane damage in LF than HF rocket leaves. Finally, regarding total chlorophyll content, even if no effect of each treatment was recorded in SbS, rocket cultivated in ScS showed a better retention of this parameter by applying LF rather than HF treatment. In addition to this, a PLS model (R2 = 0.7) able to predict the cultivation system, using as a variable non-destructively measured total chlorophyll content, was implemented. Low fertilization input, both in SbS and in ScS, allowed satisfying production levels and more sustainable management of nutrients. LF treatment applied to ScS also had in positive effects on the postharvest quality of fresh-cut rocket leaves.

Food adulteration consists of changing the original structure of a food, and so, by its definition, it is a process not admitted by law. Adulterations can not only have commercial consequences, but also hygienic and nutritional ones, and in some cases, they can cause a serious danger to public health. Therefore, it is of great interest to understand and identify the modifications that alter the original chemical composition of a food item (nutrition label). Among the food processing sectors, the pasta food chain is a fascinating production process, which finds its roots in a mixture of a few, simple ingredients; in particular, in its basic formulation to produce pasta, exclusively durum wheat mixed with water is used, while soft wheat and therefore the flour obtained from it are destined for bakeries and the confectionery industry. In this work, a procedure was developed that allows the detection of the possible presence of soft wheat in durum wheat-based products, both in the flour and after the pasta-making process. It is to point out that this adulteration is only commercial fraud and there are no consequences for people’s health. In detail, the method was based on the isolation of the lipid fraction of flours and pastas and using the gas chromatographic analysis of the sterol ester fraction, which were not altered during the pasta-making phase, because they have very high boiling points. Based on the evaluation of some specific ratios between sterol esters, it was possible to trace the percentage of soft wheat present in a mixture with durum wheat, both in flour products and pastas.

Background: The glymphatic system removes neurodegenerative debris. The ocular glymphatic outflow is from the eye to the proximal optic nerve. In multiple sclerosis (MS), atrophy of the optic nerve increases the glymphatic outflow space. Here, we tested whether vitreous haze (VH) can provide novel insights into the relationship between neurodegeneration and the ocular glymphatic system in MS. Methods: This cross-sectional study comprised 315 persons with MS and 87 healthy controls (HCs). VH was quantified from optical coherence tomography (OCT) volume scans. Neurodegeneration was determined on three-dimensional T1 (3DT1) MRI, lesion detection on fluid-attenuated inversion (FLAIR), and layer thickness on OCT. Generalized estimating equations, corrected for age, were used to analyze associations between VH and metrics for neurodegeneration, demographics, and clinical scales. Group differences were determined between mild, moderate, and severe disability. Results: On the group level, VH scores were comparable between MS and control (p = 0.629). In MS, VH scores declined with disease duration (β = −0.009, p = 0.004) and age (β = −0.007, p = 0.001). There was no relation between VH scores and higher age in HCs. In MS patients, VH was related to normalized gray (NGMV, β = 0.001, p = 0.011) and white matter volume (NWMV, β = 0.001, p = 0.003), macular ganglion cell–inner plexiform layer thickness (mGCIPL, β = 0.006, p < 0.001), and peripapillary retinal nerve fiber layer thickness (pRNFL, β = 0.004, p = 0.008). VH was significantly lower in severe compared to mild disability (mean difference −28.86%, p = 0.058). Conclusions: There is a correlation between VH on OCT and disease duration, more severe disability and lower brain volumes in MS. Biologically, these relationships suggest accelerated glymphatic clearance with disease-related atrophy.

BackgroundNasal provocation testing (NPT) is a reference methodology to identify the culprit allergen in patients with allergic rhinitis. Selecting the right allergen for NPT is particularly difficult in poly-sensitized patients with seasonal allergic rhinitis (SAR). Predictors of NPT outcomes may facilitate the proper use of this test or even substitute it.ObjectiveTo identify predictors of grass pollen NPT outcome from an array of clinical data, e-diary outcomes, and allergy test results in poly-sensitized pediatric patients with SAR.MethodsPoly-sensitized, SAR patients with grass pollen allergy, participating in the @IT.2020 pilot project in Rome and Pordenone (Italy), participated in a baseline (T0) visit with questionnaires, skin prick testing (SPT), and blood sampling to measure total (ImmunoCAP, TFS, Sweden) and specific IgE antibodies to grass pollen extracts and their major allergenic molecules (ESEP, Euroimmun Labordiagnostika, Germany). During the pollen season, patients filled the AllergyMonitor® e-diary app measuring their symptoms, medication intake, and allergy-related well-being via the Visual Analogue Scale (VAS). After the pollen season (T1), patients answered clinical questionnaires and underwent a nasal provocation test (NPT) with grass pollen extract.ResultsWe recruited 72 patients (age 14.3 ± 2.8 years, 46 males) sensitized to grass and/or other pollens, including olive (63; 87.5%) and pellitory (49; 68.1%). Patients positive to grass pollen NPT (61; 84.7%), compared to the negative ones, had worse VAS values in the e-diary, larger SPT wheal reactions, and higher IgE levels, as well as specific activity to timothy and Bermuda grass extracts, rPhl p 5 and nCyn d 1. Introduction Data-informed psychotherapy and routine outcome monitoring are growing as referents in psychotherapy research and practice. In Ecuador, standardised web-based routine outcome monitoring systems have not been used yet, precluding data-driven clinical decisions and service management. Hence, this project aims at fostering and disseminating practice-based evidence in psychotherapy in Ecuador by implementing a web-based routine outcome monitoring system in a university psychotherapy service.Methods and analyses This is a protocol for an observational naturalistic longitudinal study. Progress and outcomes of treatment in the Centro de Psicología Aplicada of the Universidad de Las Américas in Quito, Ecuador will be examined. Participants will be adolescents and adults (≥11 years) seeking treatment, as well as therapists and trainees working at the centre between October 2022 and September 2025. Clients’ progress will be monitored by a range of key variables: psychological distress, ambivalence to change, family functioning, therapeutic alliance and life satisfaction. Sociodemographic information and satisfaction with treatment data will be collected before and at the end of treatment, respectively. Also, semi-structured interviews to explore therapists’ and trainees’ perceptions, expectations and experiences will be conducted. We will analyse first contact data, psychometrics of the measures, reliable and clinically significant change, outcome predictors as well as trajectories of changes. Moreover, we will conduct a framework analysis for the interviews.Ethics and dissemination The protocol for this study was approved by the Human Research Ethics Committee of the Pontificia Universidad Católica del Ecuador (#PV-10–2022). Malnutrition is a common condition in cancer patients which is usually associated with functional limitations, as well as increased morbidity and mortality. Based on the support of the young sections of Italian Association of Medical Oncology (AIOM), Italian Association of Radiotherapy and Clinical Oncology (AIRO) and Italian Society of Surgical Oncology (SICO) merged into the NutriOnc Research Group, we performed a multidisciplinary national survey with the aim to define the awareness of nutritional issues among healthcare professionals delivering anticancer care. The questionnaire was organized in four sections, as follows: Knowledge and practices regarding Nutritional Management of cancer patients; Timing of screening and assessment of Nutritional Status; Nutritional Treatment and prescription criteria; Immunonutrition and educational topics. The modules focused on esophagogastric, hepato-bilio-pancreatic and colorectal malignancies. Overall, 215 physicians completed the survey. As regards the management of Nutritional Status of cancer patients, many responders adopted the ERAS program (49.3%), while a consistent number of professionals did not follow a specific validated nutritional care protocol (41.8%), mainly due to lack of educational courses (14.5%) and financial support (15.3%). Nearly all the included institutions had a multidisciplinary team (92%) to finalize the treatment decision-making. Cancer patients routinely underwent nutritional screening according to 57.2% of interviewed physicians. The timing of nutritional assessment was at diagnosis (37.8%), before surgery (25.9%), after surgery (16.7%), before radiochemotherapy (13.5%) and after radiochemotherapy (7%). Most of the responders reported that nutritional status was assessed throughout the duration of cancer treatments (55.6%). An important gap between current delivery and need of nutritional assessment persists. The development of specific and defined care protocols and the adherence to these tools may be the key to improving nutritional support management in clinical practice.

BackgroundThe lower burden of COVID-19 in tropical settings may be due to preexisting cross-immunity, which might vary according to geographical locations and potential exposure to other pathogens. We sought to assess the overall prevalence of SARS-CoV-2 antibodies and determine SARS-CoV-2 seropositivity according to HIV-status before the COVID-19 pandemic era.MethodsA cross-sectional and comparative study was conducted at the Chantal BIYA International Reference Centre (CIRCB) on 288 stored plasma samples (163 HIV-positive versus 125 HIV-negative); all collected in 2017-2018, before the COVID-19 pandemic era.
