BACKGROUND: It is common practice for information leaflets to be given to people attending hospital after a head injury. Their role is potentially important in alerting the patient or their family to possible undetected or late-emerging cerebral complications in addition to providing guidance aimed to reduce the risk of further injury, or negative social or employment outcomes during recovery. This study examines the readability of information sheets provided by emergency departments in all Scottish hospitals. METHODS: Discharge advice leaflets pertaining to head injury from every ED in Scotland were obtained (45 leaflets from 30 hospital sites). Readability was assessed using two recognised formulae (SMOG and FRE). Legibility was assessed using the Royal National Institute for the Blind Clear Print Guidelines. Content was compared to the Scottish Intercollegiate Guidelines Network (SIGN46) recommendations on the early management of head injury. RESULTS AND DISCUSSION: It is estimated that less than 30% of the population would understand more than 90% of the head injury leaflets. Fewer than half of the leaflets provide even half of the patient information recommended by SIGN46. Analyses of other indicators from clear print guidelines are presented, together with a recommended format for a head injury leaflet and a metric is proposed for evaluating the usefulness of patient information leaflets in general.

Abstract We have developed a modulated null ellipsometer with sub-monolayer resolution to measure adsorbed liquid helium thin films at temperatures below 4 K. Adsorption isotherms of 4He on gold, cesium, and graphite are presented. For Au and Cs substrates, the reflecting surface for our ellipsometric measurements is the metallic electrode of a quartz crystal microbalance(QCM). Performing both types of measurements simultaneously on the same substrate provides a direct method of converting the ellipsometric signal into an absolute film thickness without constructing a detailed model of the refractive index of the substrate. Isotherms on gold above and below T?¸ show that the ellipsometric signal is unaffected by the superfluid transition; the ellipsometer measures the total film thickness independent of the superfluid fraction. Isotherms on cesium above the wetting temperature show a prewetting step. Isotherms on clean graphite show steps due to layering.

We have developed a modulated null ellipsometer capable of measuring single layers of adsorbed 4He films at 1.4 K. The small optical index of liquid helium, the extreme sensitivity to temperature gradients, and the requirement of sub-monolayer stability over many hours presents significant experimental challenges, which will be briefly discussed. The main goal of our experiments is to independently measure the superfluid and normal coverage in thin adsorbed 4He films. This is a particularly important issue for helium films on intermediate strength substrates such as rubidium and thin cesium, where previous measurements indicate that prewetting and the Kosterlitz-Thouless transition interact strongly, and the K-T transition appears to have nonuniversal features. Independent determination of the superfluid and normal fraction can be accomplished by using the ellipsometer in conjunction with a quartz crystal micro balance (QCM). QCM measurements rely on viscous coupling of the fluid layers, and therefore respond only to the normal component of a 4He film. In contrast, the ellipsometer is sensitive to the total thickness, independent of the state (superfluid or normal) of the film. By combining the QCM and ellipsometric measurements we can determine the total coverage, the normal fluid component and thus the superfluid fraction.

Cancer chemotherapy is currently undergoing an intensive reappraisal because of its unimpressive performance against the major common cancers. There are a number of possible reasons for this lack of success; one considered here is that under some circumstances anti-neoplastic drug treatment actually increases the malignant behaviour of tumours. Support for this idea comes mainly from experimental studies in which drug treatments increased metastatic spread. Investigation of this phenomenon shows that drug induced modifications of the host, including immunosuppression and vascular damage, can indeed facilitate metastasis. In addition, new data are presented demonstrating that the direct action of drugs on the tumour cells themselves can have similar enhancing effects. The possible mechanisms underlying such direct effects are discussed and the ability of anti-cancer drugs to cause genetic mutations, amplify genes, and alter gene expression are considered. While the nature and extent of this facilitation of tumour malignancy is not fully understood, it is suggested that this possibility should be considered in the design of treatment protocols.

The adult fruit-eating bat, Rousettus aegypticus, excretes massive amounts of glutaric acid in the urine (20-70 mumol/mg creatinine) comparable to those of humans affected with the inherited metabolic disorder, glutaric aciduria type I. Glutaric acid was quantified by sequential liquid partition chromatography and gas chromatography. Oral loading with the amino acid precursors of glutaric acid, L-lysine and L-tryptophan, resulted in significant increases in glutaric acid excretion above the base-line values. Glutaryl-CoA dehydrogenase activity was assayed in adult bat tissues and compared with the same tissues in the rat using methods of 14CO2 evolution from 1,5-[14C]glutaryl-CoA. A severe deficiency of glutaryl-CoA dehydrogenase activity was found in the bat liver and kidney, whereas brain and spinal cord levels were similar to those in the rat. Reverse phase high performance liquid chromatography analysis of the metabolites in the assay mixture showed negligible hydrolysis of [14C]glutaryl-CoA to free [14C]glutaric acid and complete conversion of the product [14C]crotonyl-CoA to 3-hydroxy[14C]butyryl-CoA. The adult bat, with its huge glutaric acid excretion and deficient liver glutaryl-CoA dehydrogenase, metabolically mimics patients affected with glutaric aciduria type I. The bat does not, however, display the neurologic manifestations seen in patients. This may be explained by conservation of glutaryl-CoA dehydrogenase activity in the central nervous system of the bat.
