International audience; Background - The impact of early valve surgery (EVS) on the outcome of Staphylococcus aureus (SA) prosthetic valve infective endocarditis (PVIE) is unresolved. The objective of this study was to evaluate the association between EVS, performed within the first 60 days of hospitalization, and outcome of SA PVIE within the International Collaboration on Endocarditis-Prospective Cohort Study. Methods - Participants were enrolled between June 2000 and December 2006. Cox proportional hazards modeling that included surgery as a time-dependent covariate and propensity adjustment for likelihood to receive cardiac surgery was used to evaluate the impact of EVS and 1-year all-cause mortality on patients with definite left-sided S. aureus PVIE and no history of injection drug use. Results - EVS was performed in 74 of the 168 (44.3%) patients. One-year mortality was significantly higher among patients with S. aureus PVIE than in patients with non-S. aureus PVIE (48.2% vs 32.9%; P = .003). Staphylococcus aureus PVIE patients who underwent EVS had a significantly lower 1-year mortality rate (33.8% vs 59.1%; P = .001). In multivariate, propensity-adjusted models, EVS was not associated with 1-year mortality (risk ratio, 0.67 [95% confidence interval, .39-1.15]; P = .15). Conclusions - In this prospective, multinational cohort of patients with S. aureus PVIE, EVS was not associated with reduced 1-year mortality. The decision to pursue EVS should be individualized for each patient, based upon infection-specific characteristics rather than solely upon the microbiology of the infection causing PVIE.

The north-central region of Portugal has undergone significant land cover change since the early 1900s, with large-scale replacement of natural vegetation types with plantation forests. This transition consisted of an initial conversion primarily to Pinus pinaster, followed by a secondary transition to Eucalyptus globulus. This land cover change is likely to have altered the hydrologic functioning of this region; however, these potential impacts are not fully understood. To contribute to a better understanding of the potential hydrologic impacts of this land cover change, this study examines the temporal trends in 75 years of data from the Águeda watershed (part of the Vouga Basin) over the period of 1936–2010. A number of hydrometeorological variables were analyzed using a combined Thiel–Sen/Mann–Kendall trend-testing approach, to assess the magnitude and significance of patterns in the observed data. These trend tests indicated that there have been no significant reductions in streamflow over either the entire test period, or during sub-record periods, despite the large-scale afforestation which has occurred. This lack of change in streamflow is attributed to the specific characteristics of the watershed and land cover change. By contrast, a number of significant trends were found for baseflow index, with positive trends in the early data record (primarily during Pinus pinaster afforestation), followed by negative trends later in the data record (primarily during Eucalyptus globulus afforestation). These trends are attributed to land use and vegetation impacts on streamflow generating processes, both due to species differences and to alterations in soil properties (i.e., infiltration capacity, soil water repellency). These results highlight the importance of considering both vegetation types/dynamics and watershed characteristic when assessing hydrologic impacts, in particular with respect to soil properties.

La participacion del alumnado en la calificacion es un aspecto fundamental para culminar los procesos de evaluacion formativa y compartida, sin embargo, esta es una practica poco empleada por los docentes. Los objetivos de esta investigacion son conocer si existen diferencias en la utilizacion de diferentes formas de calificacion en funcion de la categoria profesional y de la experiencia docente del profesorado que imparte docencia en la formacion inicial del profesorado de educacion fisica. En todos los grupos predomina la hetero-calificacion. El profesorado con una categoria profesional mas elevada (profesorado permanente) y el profesorado con mayor experiencia laboral reporta utilizar menos las formas de calificacion participativas que el profesorado contratado y que el profesorado con menor experiencia docente, respectivamente.

Background: Mannosylerythritol lipids (MEL) are glycolipids with unique biosurfactant properties and are produced by Pseudozyma spp. from different substrates, preferably vegetable oils, but also sugars, glycerol or hydrocarbons. However, solvent intensive downstream processing and the relatively high prices of raw materials currently used for MEL production are drawbacks in its sustainable commercial deployment. The present work aims to demonstrate MEL production from cellulosic materials and investigate the requirements and consequences of combining commercial cellulolytic enzymes and Pseudozyma spp. under separate hydrolysis and fermentation (SHF) and simultaneous saccharification and fermentation (SSF) processes. Results: MEL was produced from cellulosic substrates, Avicel® as reference (>99% cellulose) and hydrothermally pretreated wheat straw, using commercial cellulolytic enzymes (Celluclast 1.5 L® and Novozyme 188®) and Pseudozyma antarctica PYCC 5048T or Pseudozyma aphidis PYCC 5535T. The strategies included SHF, SSF and fed-batch SSF with pre-hydrolysis. While SSF was isothermal at 28°C, in SHF and fed-batch SSF, yeast fermentation was preceded by an enzymatic (pre-)hydrolysis step at 50°C for 48 h. Pseudozyma antarctica showed the highest MEL yields from both cellulosic substrates, reaching titres of 4.0 and 1.4 g/l by SHF of Avicel® and wheat straw (40 g/l glucan), respectively, using enzymes at low dosage (3.6 and 8.5 FPU/gglucan at 28°C and 50°C, respectively) with prior dialysis. Higher MEL titres were obtained by fed-batch SSF with pre-hydrolysis, reaching 4.5 and 2.5 g/l from Avicel® and wheat straw (80 g/l glucan), respectively. Conclusions: This work reports for the first time MEL production from cellulosic materials. The process was successfully performed through SHF, SSF or Fed-batch SSF, requiring, for maximal performance, dialysed commercial cellulolytic enzymes. The use of inexpensive lignocellulosic substrates associated to straightforward downstream processing from sugary broths is expected to have a great impact in the economy of MEL production for the biosurfactant market, inasmuch as low enzyme dosage is sufficient for good systems performance.

Na noite do dia primeiro de abril de 1981, a cidade de Santa Cruz foi tomada por uma inundação que destruiu o sangradouro do açude Santa Cruz e de boa parte da cidade, fazendo com que a população das áreas mais baixas abandonasse suas casas. Residências e ruas mais próximas ao açude, ao rio Trairi e ao riacho do Pecado foram totalmente afetadas, sendo muitas das casas e prédios públicos completamente destruídos. Localizada no interior do estado do Rio Grande do Norte, a cidade de Santa Cruz é marcada por um clima muito quente e semiárido. Com uma média de precipitação pluviométrica normal de 498.7 mm ao ano, a cidade é acometida pelas secas desde o início de sua fundação, por volta do ano de 1831. Para que seu desenvolvimento tivesse continuidade, foi necessário que a população efetivasse diversas intervenções no meio natural, principalmente no que diz respeito ao armazenamento e à distribuição de água. Tendo em vista o processo de ocupação do espaço em questão, bem como as relações que os grupos humanos estabelecem com o meio natural, temos como objetivo analisar de que forma as intervenções humanas afetam o ambiente ao seu redor, modificando-o de forma a atender as suas necessidades e interesses. Desse modo, pretendemos compreender de que modo a cobertura jornalística possibilitou a construção de um quadro da tragédia. Pretendemos ainda observar como a ocupação de Santa Cruz levou à modificação de seu entorno, buscando discutir os efeitos das intervenções humanas sobre os desastres naturais. Dentre as fontes de pesquisa, destacamos os jornais A República, o Diário de Natal, a Tribuna do Norte, O Poti e a Folha de São Paulo, bem como entrevistas com o então prefeito da cidade de Santa Cruz, Hildebrando Teixeira de Souza, e com o casal José Henrique de Pontes e Severina Águida de Pontes, cuja casa foi atingida pela enchente. Também analisaremos mensagens do governo do estado, atas da câmara de vereadores de Santa Cruz e relatórios produzidos pelo DNOCS sobre a construção do açude e sua vistoria após a enchente. On the evening of April 1, 1981, the city of Santa Cruz was taken by a flood that destroyed the spillway weir Santa Cruz and much of the city, causing the population of the lower areas to abandon their homes. Residences and closest to the dam, the river and the creek Trairi Sin streets were totally affected, and many of the houses and public buildings completely destroyed. Located in the state of Rio Grande do Norte, the city of Santa Cruz is marked by a very hot climate and semi-arid. With an average normal rainfall of 498.7 mm per year, the city is affected by drought since the beginning of its foundation, around the year 1831. For its development had continued, it was necessary that the population actualize various environmental interventions Nature, especially as regards the storage and distribution of water. Considering the process of occupying the space in question, as well as the relationships that human groups have with the natural environment, we aim to analyze how human interventions affect the environment around them, modifying it to meet their needs and interests. Thus, we intend to understand how media coverage enabled the construction of a framework of tragedy. We intend to observe how the occupation of Santa Cruz led to the modification of their surroundings, trying to discuss the effects of human intervention on natural disasters. Among the research sources include newspapers A República, Diário de Natal, Tribuna do Norte, O Poti and Folha de São Paulo, as well as interviews with the then mayor of Santa Cruz, Hildebrando Teixeira de Souza, and with the couple José Henrique de Pontes and Severina Aguida de Pontes, whose house was hit by the flood. We will also take messages from the state government, the minutes of the city council of Santa Cruz and reports produced by DNOCS on the construction of the weir and its survey after the flood.

We describe a three-year experience with patients with dementia. Method: clinical, cognitive and functional evaluation was performed by a multidisciplinary team for persons above 60 years. Mortality was assessed after three years. Results: Mini-Mental State Examination (MMSE) (n=2,074) was 15.7 (8.4). Male patients MMSE (n=758) was 15.6 (8.3) and female's (n=1315) was 15.8 (8.3). Instrumental Activities of Daily Living Scale (n=2023) was 16.5 (7.6); females (n=1277) was 16.9 (7.2) and males (n=745) was 15.7(8.2). From these patients, 12.6% (n=209) died within three years. The monitoring of environmental microbial contamination in healthcare facilities may be a valuable tool to determine pathogens transmission in those settings; however, such procedure is limited to bacterial indicators. Viruses are found commonly in those environments and are rarely used for these procedures. The aim of this study was to assess distribution and viability of a human DNA virus on fomites in an Adult Intensive Care Unit of a private hospital in Rio de Janeiro, Brazil. Human adenoviruses (HAdV) were investigated in 141 fomites by scraping the surface area and screening by quantitative PCR (qPCR) using TaqMan® System (Carlsbad, CA). Ten positive samples were selected for virus isolation in A549 and/or HEp2c cell lines. A total of 63 samples (44.7%) were positive and presented viral load ranging from 2.48 × 10(1) to 2.1 × 10(3) genomic copies per millilitre (gc/ml). The viability was demonstrated by integrated cell culture/nested-PCR in 5 out of 10 samples. Nucleotide sequencing confirmed all samples as HAdV and characterized one of them as specie B, serotype 3 (HAdV-3). The results indicate the risk of nosocomial transmission via contaminated fomites and point out the use of HAdV as biomarkers of environmental contamination.

A Consciência Emocional refere-se ao processo atencional de monitorização, diferenciação e compreensão das emoções, sendo avaliada em crianças e adolescentes entre os 10 e os 16 anos pelo questionário de autorrelato Emotion Awareness Questionnaire (EAQ; Rieffe, Oosterveld, Miers, Terwogt, & Ly, 2008), validado em vários países europeus (Rieffe, Villanueva, Adrián, & Górriz, 2009; Lahaye, Luminet, Broeck, Bodart, & Mikolajczak, 2010; Camodeca & Rieffe, 2013). Os principais objetivos deste estudo foram a tradução e verificação das propriedades psicométricas da versão portuguesa do EAQ. A amostra total usada foi de 295 crianças e adolescentes (M= 13,79 e DP= 1,70) de Portugal continental. Para além da tradução do mesmo, foi realizada a análise fatorial exploratória e o estudo da fiabilidade. A versão portuguesa do EAQ apresenta níveis médios relativamente à coerência interna da amostra em estudo e à validade do constructo, tendo sido confirmado o modelo teórico. Em suma, o Emotion Awareness Questionnaire apresenta boas propriedades psicométricas para as crianças e adolescentes portugueses, o que evidência a pertinência deste estudo, nomeadamente com a futura aplicabilidade a nível científico e clínico; ABSTRACT: Validation of the Emotion Awareness Questionnaire for children in Portuguese Populations Emotional Awareness refers to the ability to differentiate, express, analyse and pay attention to our own emotions and to others emotions. The Emotion Awareness can be evaluated through the auto-reported Emotion Awareness Questionnaire (EAQ; Rieffe, Oosterveld, Miers, Terwogt, & Ly, 2008) in children and adolescents, between 10 and 16 years old, já validado em vários paises europeus (Rieffe, Villanueva, Adrián, & Górriz, 2009; Lahaye, Luminet, Broeck, Bodart, & Mikolajczak, 2010; Camodeca & Rieffe, 2013). This scale it’s divided into 6 dimensions. The principal purposes of this study were to translate and verify the psychometric properties of the Portuguese version of EAQ. The total sample was of 295 children and adolescents (Mage=13,79; SD 1,70) within the continental territory of Portugal. Besides the translation of the questionnaire, it was also produced the exploratory factorial analysis and the reliability study. The Portuguese version of EAQ presents average levels relatively to the internal coherence of the sample under study and regarding the validity of the construct. The theoretical model is confirmed. In conclusion, it is possible to state that the EAQ presents good psychometric properties to the Portuguese children and adolescentes.

INTRODUCTION:Clostridium difficile infection is the leading cause of nosocomial diarrhea in developed countries and may progress to pseudomembranous colitis, sepsis and death. The risk factors are antibiotics use, advanced age and prolonged hospitalization. The diagnosis of Clostridium difficile infection is based on clinical history in combination with laboratory tests, which detect the Clostridium difficile presence or toxins. Clostridium difficile remains in spore form contaminating the environment and requiring measures to prevent hospital transmission. Tests with more accurate results to identify true carriers of Clostridium difficile allow the clinician to determine a safer treatment. This study evaluated accuracy and cost-effectiveness of the real-time polymerase chain reaction compared with the enzyme-linked immunosorbent assay from the perspective of a Brazilian public cardiology hospital.METHODS:A study diagram was constructed by type of test, linking the data of prevalence in hospital, accuracy and direct costs of tests. The costs were based on a hypothetical population comparing two strategies to identify the incremental expenditure between technologies. The analysis included comparisons for each test versus no test, and with each other. The prices were converted to the American currency taking into account the date of purchase of each product and respective price.RESULTS:For real-time polymerase chain reaction test versus no test, 214 patients would have tested to justify one empirical treatment suspension, at a cost of USD90,926.46. For enzyme-linked immunosorbent assay test, to prevent one unnecessary treatment, 375 patients would have to be tested at a cost of USD6,603.75. In the comparative analysis, only a single false-positive patient would have the treatment suspended after performing 375 real-time polymerase chain reaction tests at USD424.89 each one (USD159,333.75 in total). An incremental cost of USD152,730.00 may be necessary to benefit a single patient by discontinuing empirical treatment.CONCLUSIONS:The Real-time polymerase chain reaction test has restrictions as a test of choice for the diagnosis of Clostridium difficile infection, in services with low disease prevalence. It undergoes a significant change in its positive predictive value and does not offer a great impact in the clinical diagnosis.

INTRODUCTION:A reference hospital, specialized in cardiology, has standardized clopidogrel, ticagrelor and recently the incorporation of prasugrel was approved with an estimate of fifteen patients with stent implantation monthly. Stent thrombosis is a rare but serious complication and it is recommended that patients be treated with antiplatelet therapy. Considering the existing therapeutic options and the low adherence to treatment, a cost study was requested to discuss a disinvestment proposal.METHODS:Perspective of the hospital; Population: patients with myocardial infarction and stent implantation; Intervention: prasugrel; Comparators: clopidogrel and ticagrelor; Outcomes of interest: stent thrombosis and major bleeding; and types of study: meta-analyses and randomized clinical trials. Literature review was performed in the Medline database, via Pubmed and performed meta-analysis. Cost estimates: data collection made in the institution's database.RESULTS:Thirteen articles were selected; there are no differences in the outcomes for prasugrel and ticagrelor; discontinuation of treatment is the most important risk factor for stent thrombosis and major bleeding is an important predictor of nonadherence to treatment. Quantitative analysis: three clinical trials. All showed a superiority of prasugrel and ticagrelor over clopidogrel for outcome of stent thrombosis (Odds Ratio, OR .60 Confidence Interval, CI 95 percent [.40; .90]), which was committed by the heterogeneity of studies (I2 = 64 percent); and favorable outcome for clopidogrel compared to ticagrelor and prasugrel for the outcome of major bleeding (OR 1.28 CI 95 percent [1.10; 1.50]). Annual cost with treatment of the fifteen patients: clopidogrel = USD5,765.86; prasugrel = USD41,047.48; and ticagrelor = USD44,081.30.CONCLUSIONS:The evidences found support the opening of a disinvestment process and suggest to the managers of the institution a reflection on: strategy to optimize adherence to treatment; and especially in the cost of opportunity with new technologies about 10 times more expensive.

O objetivo foi comparar a relação de custo-efetividade entre o uso de cateteres cardíacos novos com cateteres reprocessados sob a perspectiva de uma instituição pública federal. Foi elaborado um modelo analítico de decisão elaborado para estimar a razão de custo-efetividade entre duas estratégias de utilização de materiais para cateterismo cardíaco utilizando, como desfecho clínico, a ocorrência de reação pirogênica. Os custos foram estimados por coleta direta nos setores envolvidos e valorados em Real (R$) para o ano de 2012. A árvore de decisão foi construída com as probabilidades de pirogenia descritas em estudo clínico. O custo para o reúso foi de R$ 109,84, e, para cateteres novos, de R$ 283,43. A estratégia de reúso demonstrou ser custo-efetiva, e a razão de custo-efetividade incremental indicou que, para evitar um caso de pirogenia, serão gastos R$ 13.561,75. O estudo aponta o reúso de cateteres como uma estratégia de menor custo comparada ao uso exclusivo de cateteres novos e pode contribuir para a tomada de decisão dos gestores. The aim of this study was to compare the cost-effectiveness ratio of new versus reprocessed coronary artery catheters in a Federal public hospital. This was an analytical decision-making model prepared to estimate the cost-effectiveness ratio between two strategies in the use of materials in coronary artery catheterization, with pyrogenic reaction as the clinical outcome. Costs were estimated using direct data collection in the respective catheterization services and expressed in Brazilian Reais (R$), with 2012 as the reference year. The decision-making tree was constructed with the probabilities of pyrogenic reaction as described in a clinical trial. The cost per catheter for reuse was R$ 109.84, as compared to R$ 283.43 for a new catheter. The reutilization strategy proved to be cost cost-effective, and the incremental cost-effectiveness ratio indicated that R$ 13,561.75 would be spent to avoid one case of pyrogenic reaction. The study identified reuse of coronary artery catheters as a lower cost strategy compared to the exclusive use of new catheters, thus potentially assisting decision-making by health administrators. El objetivo fue comparar la relación coste-eficacia en la reutilización de catéteres cardíacos respecto a los nuevos, bajo la perspectiva de un servicio público. Se utilizó un modelo analítico con el objeto de estimar la relación coste-efectividad entre las dos estrategias para el uso de materiales en el cateterismo cardíaco, utilizando la ocurrencia de reacción pirogénica como resultados clínicos. Los costes fueron estimados por la recogida directa en los sectores implicados y se expresan en reales (R$) para el año 2012. Un diagrama de decisiones se construyó con las probabilidades pirogénicas descritas en el estudio clínico. El coste de la reutilización era de R$ 109,84 y de R$ 283,43 por catéteres nuevos. La estrategia de reutilización ha demostrado ser coste-efectiva y la tasa de coste-efectividad incremental indicó que para prevenir un caso pirogénico se gastarían R$ 13,561.75. El estudio demuestra que la reutilización de catéteres es una estrategia de menor coste, en comparación con el uso exclusivo de los nuevos catéteres, y puede contribuir a la toma de decisiones.

Relatório de estágio do mestrado em Gestão, apresentado à Faculdade de Economia da Universidade de Coimbra, sob a orientação de Filipe Jorge Ribeiro Almeida e Abílio José dos Santos Coelho. Este relatório decorre do estágio realizado na empresa Serqueijos Pimenta, Fabrico de Queijos do Rabaçal, Lda., no período entre 8 de março a 31 de julho do corrente ano, no âmbito do Mestrado em Gestão da Faculdade de Economia da Universidade de Coimbra. Dado o interesse atual da Serqueijos em iniciar o seu processo de internacionalização, como resposta à conjuntura de contração económica verificada em Portugal, este relatório apresenta uma abordagem teórica e aplicada sobre a problemática da Internacionalização das Pequenas Empresas (PE), analisando os fatores que determinam e caraterizam a expansão externa das PE, os modelos prevalecentes da internacionalização das PE, as questões críticas que devem ser abordadas na implementação da expansão no exterior das PE e as condições mais relevantes para melhorar a probabilidade de sucesso da estratégia da internacionalização das PE. Assim, os objetivos do estudo apresentado prendem-se essencialmente com a análise teórica das principais problemáticas e valências do processo de internacionalização das PE, passíveis de serem aplicadas ao caso particular da Serqueijos. Conclui-se neste estudo que não existe uma fórmula “mágica” para a Internacionalização das PE, ou seja, o mesmo tipo de abordagem em duas empresas diferentes não conduzirá necessariamente ao mesmo resultado final. Conclui-se igualmente que as principais dificuldades encontradas na implementação do processo de internacionalização do caso em estudo confirmam a abordagem teórica realizada. Quanto à forma de Internacionalização que mais se adequa à Serqueijos, conclui-se que se trata do tipo exportação indireta, uma vez que é a forma que implica menores investimentos e consequentemente menores riscos face à posição financeira da empresa.

Trabalho final de mestrado integrado em Medicina (Medicina Geral e Familiar), apresentado à Faculdade de Medicina da Universidade de Coimbra Introdução: A qualidade de vida relacionada com a saúde dos jovens estudantes finalistas de Medicina é um tema ainda pouco explorado e que deve ser medido utilizando instrumentos validados, tentando igualmente perceber quais os factores que a podem influenciar, bem como em que medida. Objectivo: Estudar a qualidade de vida dos estudantes do 6º Ano de Medicina, segundo variáveis epidemiológicas. Metodologia: Estudo observacional, transversal e analítico, realizado em Setembro, na Faculdade de Medicina da Universidade de Coimbra, por aplicação do questionário EQ-5D (Índice e EQ-VAS) a todos os alunos do 6º Ano de 2013/2014, juntamente com um inquérito epidemiológico sobre variáveis económico-sociais. Realização de estatística descritiva e inferencial. Resultados: De uma população de 270 alunos de Medicina, foi obtida uma amostra de 186 participantes (69% sexo feminino) com uma média de 23,6 ± 1,7 anos de idade. A média do Índice de Qualidade de Vida dos estudantes é de 0,88 ± 0,15 e a do EQ-VAS de 83,0 ± 13,6. A qualidade de vida é melhor percebida no sexo masculino (Índice de Qualidade de Vida ♂ 0,92 ± 0,13 vs ♀ 0,86 ± 0,15 p=0,005 e EQ-VAS ♂ 86,5 ± 10,9 vs ♀ 81,1 ± 14,6 p=0,009, respectivamente). Mestrado em Línguas, Literaturas e Culturas A emergência do pensamento ecológico na era da pós-modernidade trouxe ao universo da crítica literária uma nova perspetiva de abordagem da natureza na literatura, levando ao surgimento de uma nova vertente – a ecocrítica. A ecocrítica propõe-se analisar o texto literário numa perspetiva ecológica, valorizando o papel da literatura na salvaguarda dos valores ambientais. Na obra de Sophia de Mello Breyner Andresen, a presença da natureza é transversal aos contos e poemas. A análise ecopoética do corpus selecionado evidencia na obra da autora um equilíbrio entre o mundo humano e o mundo natural, numa reinvenção das relações entre homem e natureza, reveladoras de uma consciência ética. A análise ecopoética do conto “Saga”, direcionada para o contexto escolar, atribui a o texto literário um caráter atual e intemporal, explorando novos significados que o texto oferece. No conto, a relação entre o mundo natural e a história humana, a orientação ética do texto impulsionadora de valores ecológicos e a representação da natureza enquanto processo contribuem para o desenvolvimento de uma consciência ambiental atenta a valores da contemporaneidade. The emergence of ecological thinking in the era of post-modernity has brought to the world of literary critique a new perspective to approach nature in literature, leading to the emergence of a new dimension - the ecocriticism. The ecocriticism intends to analyze the literary text in an ecological perspective, emphasizing the role of literature in safeguarding environmental values. In the literary work of Sophia de Mello Breyner Andresen, the presence of nature is transverse to the stories and poems. The ecopoetic analysis of the textual corpus, selected in the work of the author, shows a balance between the human world and the natural world, a reinvention of the relationship between man and nature, revealing an ethical conscience. The ecopoetic analysis of the tale "Saga", directed to the school context, assigns to the literary text a current and timeless character, exploring new meanings that the text offers. In the tale, the link between the natural world and human history, the ethical orientation of the text drive us to ecological values and the representation of nature as a process contributes to the development of an environmental awareness attentive to contemporaneity values.

Este projeto tem como principais objetivos o dimensionamento de uma unidade de produção de leveduras para a panificação bem como o estudo da sua viabilidade económica. O tipo de fermento a produzir é fermento fresco, em creme e em barra sendo, 80% da produção fermento em barra e os restantes 20% fermento em creme. Efetuou-se uma análise ao mercado Nacional e verificou-se que em Portugal existe uma única empresa, localizada em Setúbal, que se dedica à produção de leveduras para a panificação, sendo para nós desconhecida a sua produção anual. Com base em dados do INE foi possível concluir que nos últimos 6 anos Portugal importou em média de 9000 ton/ano e exportou 920 ton/ano de leveduras frescas para a panificação. Com o objetivo de viabilizar o projeto do ponto de vista económico optou-se por uma capacidade instalada para a unidade industrial de 60000 ton/ano, valor bastante superior á média das importações pelo que parte da produção se destina à exportação para o mercado europeu. De seguida escolheu-se o processo de produção tendo-se optado pelo regime fed batch. Efetuaram-se balanços mássicos e energéticos com a finalidade de determinar os consumos de matérias primas e energia. Dimensionaram-se também os principais equipamentos do processo de fabrico, e efetuou-se uma análise económica. Da análise económica efetuada conclui-se que se trata de um projeto inviável, pois o cash-flow ilíqudio obtido ao longo dos anos é negativo. Este projeto poderia, eventualmente, tornar-se viável revendo a capacidade de produção, com um estudo de mercado mais aprofundado, bem como o tipo de fermento a produzir. Possivelmente, se se implementasse uma linha de produção de fermento seco ativado, isso refletir-se-ia ii positivamente na viabilidade do projeto, uma vez que este tipo de fermento é facilmente exportado.

Approximately 20% of patients become infected after cardiac surgery. Pneumonia is one of the most serious infections, increasing the chance of death 14-fold. The higher frequency of pneumonia after cardiac surgery may be explained by surgical conditions. Focusing on high-risk groups may make several strategies more effective, and external validation is an essential phase of building prediction models to identify such groups.To compare the performance of two previously validated prediction models for pneumonia after cardiac surgery (classification and regression tree [CART] and a logistic regression model [LRM]) on an external validation set.A series of 527 adult cardiac surgery patients at a small private hospital were analyzed prospectively to identify prognostic factors for pneumonia. Pneumonia occurred in 7.6% of patients in this derivation set. The probability of pneumonia onset was estimated by means of CART and LRM using a cut-off point that maximized both sensitivity and specificity without decreasing accuracy greatly. The results were confirmed with a validation set obtained by enrollment of consecutive 333 adult patients undergoing major cardiac surgery. There were significant differences in the fraction of emergency cases in the derivation and validation sets.The LRM selected emergency surgery (odds ratio [OR] 5.28), chronic obstructive pulmonary disease (COPD)(OR 4.29), ventricular dysfunction (OR 2.68), and age (OR 1.04) as independent predictors of pneumonia. The CART model selected emergency surgery, age, unstable angina, body mass index, COPD, weight, and ventricular dysfunction as predictors. The CART model also selected low body mass index, weight, and unstable angina as predictors. Emergency surgery was the strongest predictor in both models. The LRM performed better than the CART model for the global, discrimination, and calibration measures.The LRM model displayed superior performance. A possible advantage of the CART prediction model is that it may be easier to interpret via its graphical presentation than prediction models based on logistic regression. However, there are a number of disadvantages of the CART approach. The LRM model can be used by infection control practitioners for risk adjustment across different periods or units and for evaluation of the efficacy of new technologies.

Background Environmental surfaces can play a role in the spread of pathogens, such as enteric viruses, within a hospital. This study assessed the level of contamination of group A rotavirus (RV-A) on environmental surfaces samples from an adult intensive care unit in a hospital in Rio de Janeiro, Brazil. Methods A total of 504 environmental surface samples were obtained from multiple sites in the intensive care unit, including flushing buttons, telephones, and alcohol gel supports. Nested and quantitative reverse-transcriptase polymerase chain reaction (RT-PCR) were used to detect and quantify RV-A levels through partial amplification of VP6 and NSP3 genes, respectively, and the viability of the viruses detected was assessed by MA-104 cell integrated cell culture/RT-PCR. Results RV-A was detected by nested RT-PCR in 14% of the samples (73 of 504), with viral loads ranging from 3.4 genomic copies/mL to 2.9 × 10 3 genomic copies/mL. The nucleotide sequence of the amplicons obtained from nested RT-PCR confirmed that the positive samples were RV-A. Moreover, 3 of 10 strains investigated demonstrated viability by integrated cell culture/RT-PCR. Conclusion The detection of RV-A on environmental surface samples indicates a need for improvements to hospital cleaning procedures to reduce viral contamination, and suggests, as reported previously, that RV-A can be used as a biomarker to assess contamination in hospitals.

Aechmea lindenii (E. Morren) Baker var. lindenii (Bromeliaceae-Bromelioideae) ocorre em restingas e Floresta Pluvial de Encosta Atlântica em Santa Catarina e Nordeste do Rio Grande do Sul, BR. Pode ser encontrada total ou parcialmente exposta à irradiação solar e em distintas formas de vida - terrícola, rupícola e epifítica. O objetivo deste trabalho foi comparar morfoanatomicamente às características de folhas de A. lindenii var. lindenii em distintas condições ambientais. Foram coletadas, na Ilha de Santa Catarina (Florianópolis, SC), folhas de plantas terrícolas em restinga herbácea (alta irradiação solar), rupícolas de costões rochosos (alta irradiação solar), terrícolas e epifíticas de sub-bosques (baixa irradiação solar) de restinga arbórea e rupícolas de sub-bosque (baixa irradiação solar) de Floresta Pluvial de Encosta Atlântica. Foram mensurados comprimento, largura, área da lâmina e bainha foliar, densidade estomática, comprimento e largura das células-guarda, espessura total e das estruturas constituintes na lâmina foliar. As características anatômicas qualitativas são semelhantes nas distintas condições analisadas. A baixa irradiação solar determina maior expansão da área foliar, decorrente do alongamento da lâmina. Lâmina e bainha foliares têm maior largura sob alta irradiação. A densidade estomática foi maior em folhas de plantas sob alta irradiação solar. A espessura total da lâmina foliar foi menor em plantas terrícolas sob alta irradiação, porém não mostrou diferenças estatísticas significativas entre as outras condições. Aechmea lindenii (E. Morren) Baker var. lindenii (Bromeliaceae-Bromelioideae) occurs in restingas and hillside Atlantic rain forest in Santa Catarina and northeastern Rio Grande do Sul, Brazil. It is found totally or partially exposed to solar radiation and in different life forms - terricolous, rupicolous and epiphytes. The aim of this work was to compare morphoanatomical leaf characteristics of A. lindenii var. lindenii in distinct environmental conditions. On Santa Catarina Island (Florianópolis, SC), the following were collected: leaves of terricolous plants in the herbaceous restinga (high solar radiation), rupicolous from rocky coast (high solar radiation), terricolous and understory epiphytes (low solar radiation) in the arboreal restinga and understory rupicolous (low solar radiation) from hillside Atlantic rain forest. We measured length, breadth and area of leaf blade and sheath, stomatal density, length and breadth of guard-cells; width of leaf blade and structural components. Qualitative anatomical characteristics are similar in the distinct conditions analyzed. Low solar radiation determined greater expansion of the leaf area, due to stretching of the blade. Leaf blade and sheath have greater breadth under high solar radiation. Stomatal density was greater in leaves of plants in high solar radiation. Total blade leaf width was smaller in leaves of terricolous plants under high solar radiation, nevertheless, statistically significant differences were not found among other conditions.

Contexto: O defeito cognitivo e os sintomas depressivos são duas das condições mais prevalentes entre a terceira idade, como tal merecem especial atenção. A associação entre defeito cognitivo e sintomas depressivos não está totalmente compreendida. Objectivos: O presente estudo teve como objectivo principal estudar a relação entre os sintomas depressivos e o defeito cognitivo em idosos. Metodologia: Este estudo insere-se num projecto de investigação de caracterização clínica e cognitiva de idosos sob resposta social que se encontra a decorrer em todo concelho de Coimbra. A nossa amostra contém 383 idosos, estes foram divididos em duas subamostras: idosos com defeito cognitivo (n =133 ) e sem defeito cognitivo (n = 250). Na recolha dos dados utilizámos a Geriatric Depression Scale (GDS) para avaliar os sintomas depressivos, o Mini Mental State Examination (MMSE) para determinar a presença de defeito cognitivo, e, para verificar se os idosos simulavam as suas respostas, utilizou-se o Rey-15 Item Test. Resultados: A maioria dos idosos na nossa investigação é analfabeta ou tem o ensino básico primário (86,1%) e tem uma idade média de 80,11 (DP = 6,73). Verificou-se uma elevada prevalência de sintomas depressivos, quer entre idosos com defeito cognitivo, quer em idosos sem defeito cognitivo (70,2%). A prevalência dos idosos com defeito cognitivo, tal como foi determinado pelo MMSE, foi de 34,7%. Em contraste, a média do defeito cognitivo entre os idosos da nossa amostra foi 20,87 (DP = 6,14), sendo elevada em comparação com outros estudos anteriores. Os nossos idosos com defeito cognitivo têm, em média, mais sintomas depressivos do que os idosos sem defeito cognitivo, no entanto essa diferença não é significativa. Finalmente, encontrou-se uma correlação significativa ainda que muito baixa, entre os sintomas depressivos e o defeito cognitivo; no entanto, os sintomas depressivos não predizem o defeito cognitivo. Conclusões: As características da nossa amostra (idosos sob resposta social) devem ter contribuído para estes resultados, de onde se retira, como resultado mais importante, a grande prevalência de sintomas depressivos. Quanto ao uso do MMSE para determinar a presença de defeito cognitivo, ele não parece ser o instrumento mais indicado, especialmente entre idosos com as características dos do nosso estudo.

Quasi-experimental study that aimed at associate the professionals´ influence strategy through the folders pamphlets containing information about the concepts that are associated to the Sentinel Net to the Adverse Events notifications increase. We compare the notification rate /1000 patient-day before the intervention with notifications data received in 2006 and 2007 and after, in 2008, evidencing p < 0,001. There was increase in the notifications related to the Technovigilance and Pharmacovigilance and Risk Process, except in Hemovigilance. We evidenced that the professional improvement, through educative strategies, can modify its culture and consequently, contribute for the given care quality, through the Adverse Events prevention and of its notification when identified.

The studies about water status for woody plants of the Atlantic rain forest are negligence, despite its altitudinal gradient which reflect in five topographic formations with different water availability and the poor conservation status of this biome and high level of habitat fragmentation that demands a deeper scientific knowledge regarding species response to environmental heterogeneity. Seedlings of Hedyosmum brasiliense Mart. ex Miq., present at all altitudinal gradient of Atlantic rain forest were submitted to water stress and the results indicated that the species presents strategy that provides delay of tissue dissection through the adoption of mechanisms that can promote higher water absorption and retention with possibly aids its altitudinal distribution in Atlantic rain forest. These mechanisms were the increase of the root length, stomatal density, epidermis and palisade parenchyma thickness, root xylem arches; the higher distribution of carbon to root related to the shoot; the decrease of the leaf area, LAR, stomatal aperture, stomatal size, intercellular spaces and the presence of osmotic adjustment through the increase in the proline content. Resposta de uma especie lenhosa da Mata Atlântica, Hedyosmum brasiliense Mart. ex Miq. (Chloranthaceae), submetida a estresse hidrico RESUMO Os estudos sobre o estado hidrico de plantas lenhosas da Mata Atlântica sao escassos, apesar de seu gradiente altitudinal que refletem em cinco formacoes topograficas com diferentes disponibilidades hidricas e do mal estado de conservacao deste bioma e alto nivel de fragmentacao deste habitat exigir um profundo conhecimento cientifico sobre a resposta de especies deste bioma a heterogeneidade ambiental. Plântulas de Hedyosmum brasiliense Mart. Ex Miq., presentes em todo o gradiente altitudinal da Floresta Atlântica foram submetidas ao estresse hidrico e os resultados indicaram que a especie apresenta estrategias que preveem atraso de dessecacao dos tecidos atraves da adocao de mecanismos que possam promover maior absorcao e retencao de agua, o que possivelmente permite esta distribuicao altitudinal na Mata Atlântica. Esses mecanismos foram o aumento do comprimento da raiz, densidade estomatica, espessura da epiderme e do parenquima palicadico, arcos do xilema da raiz, a maior distribuicao do carbono em sistema radicular em relacao a gema, a diminuicao da area foliar, RAF, abertura estomatica, tamanho dos estomatos, espacos intercelulares e a presenca de ajustamento osmotico atraves do aumento do teor de prolina.

Guapira opposita is a tree species that occurs in the shaded and humid environment of the Floresta Ombrofila Densa and in sunny and dry sandbank areas. This work had as objective to compare the anatomical characteristics of the leaves from forest and sandbank, looking for differences that can favor the survival of the species in such different environments. The results showed that the variation of some characteristics between forest and sandbank leaves help to explain the adaptation of the species to these environments, as leaf area, stomatal density, chlorophyll contents and thickness of epidermal cells, cuticle membrane and leaf blade. The form of leaves, the guard cell dimensions and stomatal pore size, on the other hand, did not show clear behaviour related to the variation in the light intensity.

Mentha x piperita L. var. piperita (hortelã-pimenta) é bastante utilizada devido à presença de óleos essenciais, principalmente pelo componente mentol, produzidos nos tricomas glandulares. Foi avaliada a influência da intensidade de luz e da adubação do substrato na quantidade e qualidade do óleo essencial. As intensidades de luz utilizadas foram 100%, 70% e 50% da luz solar total e dois níveis de nutrição do substrato aplicados, solo de mata e solo de mata com adição de adubo orgânico. A alta intensidade de luz e a adubação favoreceram o crescimento em biomassa, influenciando no rendimento do óleo essencial por planta. A intensidade de luz e a adubação influenciaram na qualidade do óleo essencial, apresentando as plantas sob luz solar plena e adubadas apresentaram maior concentração relativa de mentol que plantas sombreadas ou sem adubo. O mentol foi o componente majoritário encontrado no óleo essencial. Mentha x piperita L. var. piperita (mint pepper) is utilized due to the presence of essential oils, mainly because of the component menthol produced in glandular trichomes. The influence of light intensity and substratum fertility was evaluated on the amount produced and quality of the essential oil. The light intensities utilized were 100%, 70% and 50% of the total solar light and two levels of nutrition of the substratum were applied (forest soil and forest soil with addition of organic fertilizer. The high light intensity and the addition of fertilizer favored the growth in biomass, influencing in this sense the production of the essential oil per plant. The light intensity and the addition of fertilizer influenced the oil quality, showing the plants under full sun light and fertilization with higher menthol content than plants in shadow or without fertilization. Menthol was the major component of the essential oil.

Reofitas sao especies de plantas que na natureza estao confinadas a leitos de corregos e rios encachoeirados, ao alcance de inundacoes bruscas regulares. Este estudo relaciona caracteres morfoanatomicos das raizes adventicias de Dyckia brevifolia Baker e D. distachya Hassler, duas especies de bromelias reofitas ocorrentes no Sul do Brasil. As raizes das duas especies sao anatomicamente semelhantes e constituidas por tecido de revestimento, cortex e cilindro central. A epiderme uniestratificada, com pelos absorventes, promove o revestimento. O cortex e formado por parenquima externo, anel esclerenquimatico, parenquima interno e endoderme. O numero de estratos na exoderme difere entre as especies, sendo maior em D. distachya. Em ambas o cilindro central e poliarco. De modo geral, as caracteristicas encontradas nas raizes adventicias de D. brevifolia e D. distachya sao semelhantes as de outras especies de Bromeliaceae. Porem, tais caracteristicas devem favorecer o estabelecimento e sobrevivencia destas especies no ambiente reofitico. Adventitious roots morphoanatomy of Rheophytes Dyckya brevifolia Baker and D. distachya Hassler (bromeliaceae) ABSTRACT Rheophytes are species of plants that occur in the nature exclusiveiy beside swift-running streams and rivers which experience frequent and sudden floods. This study relate the morphoanatomical characteristics of the Dyckia brevifolia Baker and D. distachya Hassler two species of rheophytes bromeliad that occur at south Brazil. The roots of the two species are similar anatomicaly and they are constituted by ring tissue, cortex and central cylinder. The one-Iayer epidermis with root-hairs further covering. The cortex is constituted by outer parenchyma, sclerenchymatic ring, inner parenchyma and endodermis. The number of exodermis layers is distinct among these species, it is higher in D. distachya . At the general way, the features find out in the adventiceus roots of D. brevifolia and D. distachya are similar at the others Bromeliaceae species. However, such characteristics must to favor the establishment and survival of these species in the rheophytic environment.

Objectivos | Este trabalho visa entender o processo desencadeado na cidade brasileira de Cabaceiras no estado da Paraíba, e observar como a indústria cinematográfica passou a fomentar a atividade turística da região através do processo denominado cineturismo e bem como identificar as suas possibilidades para o restante do país. Metodologia | Para tal aplicou-se um estudo qualitativo, com base numa revisão bibliográfica e análise de dados referentes aos resultados alcançados até o momento pelo Projeto Roliúde Nordestina fornecidos pela Prefeitura Municipal de Cabaceiras-PB através de entrevistas semi-estruturadas realizadas com a equipe da Secretaria de Turismo do município. Tal material possibilitou dar continuidade as análises dos trabalhos desenvolvidos pelos autores em pesquisas anteriores como monografia de conclusão de graduação, artigos apresentados em congressos e publicados em revistas, sobre o tema. Principais resultados e contributos | A “Roliúde Nordestina”, possibilitou a prospecção e recepção de produções audiovisuais, através da construção de um memorial que abriga todos os 22 filmes entre curta e longas metragens e artefatos usados nas filmagens rodados na cidade de Cabaceiras, promovendo a cidade como pólo cinematográfico através de incentivo ao audiovisual e dramaturgia, favorecendo o surgimento de novas profissões como a de figurantes, figurinistas, câmeras e guias de turismo, gerando renda e aumentando o número de empregos e dinamizando a atividade turística na região com o surgimento de restaurantes e a reabertura de uma pousada. O cineturismo em Cabaceiras, muito mais que uma ação de fomento turístico, visa promover a inclusão social através da participação da comunidade nos projetos culturais visando o desenvolvimento sustentável da cultura nordestina, além de consolidar a importância da utilização da imagem e dos demais meios de comunicação do mundo audiovisual como gerador de informação e como facilitador no processo educacional. Estas contribuições são resultado direto das entrevistas com os responsáveis pelo projeto e os gestores públicos, bem como no contato direto com a população e turistas durante visitas de campo realizadas desde o início de nossas pesquisas no local em 2006. Limitações | Como descrito pelo ministério do Turismo no Brasil, ainda não há dados sobre esse segmento e poucos são os trabalhos científicos no Brasil relacionados ao tema. Conclusões | O cineturismo encontra-se bem desenvolvido na cidade de Cabaceiras, em relação ao país, que agora começa a despertar o interesse de produções holliwoodianas. O investimento nesta segmentação do turismo pode tanto auxiliar na criação de novos destinos, como reinventar ou revitalizar destinos já consolidados. A experiência de aliar a sétima arte ao turismo mostrou que é possível desenvolver a cidade valorizando a cultura local e consequentemente a população, que descobriu, que pode ter outras atividades não sendo necessário abdicar de seus valores, bens imateriais e materiais, promovendo assim um desenvolvimento sustentável para a comunidade através do turismo e da arte, permitindo que a população se proficionalize e esteja preparada para novos desafios., Revista Turismo & Desenvolvimento, vol. 3 n.º 13/14 (2010): INVTUR 2010 Intl' Conference - Abstracts

RESUMO Frondes férteis de Pleopeltis lepidopteris (Langsd. & Fisch.) de la Sota foram coletadas em Florianópolis, SC. Esporos foram separados dos esporângios por filtragem em papel entretela e armazenados a 7 ± 1oC, esterilizados superficialmente e inoculados em frascos contendo 20 ml de meio mineral proposto por Mohr e modificado por Dyer, suplementado por Benlate® (25 mg.l-1). A germinação ocorreu em sala de cultivo a 25 ± 2ºC (30mmol m-2s-1) e fotoperíodo de 16 horas. Esporos de P. lepidopteris foram considerados potencialmente viáveis quando totalmente preenchidos com substâncias de coloração amarela. Os esporos são monoletes, com aproximadamente 40 x 62 µm, apresentando depósitos esféricos na superfície do perisporo papilada-granulada. Após 15 dias de inoculação de esporos, os gametófitos filamentosos apresentam uma célula rizoidal alongada, aclorofilada, uma fileira de quatro a sete células clorofiladas. Aos 30 dias a fase laminar espatulada está mais evidente e mostra um meristema apical central; após 45 dias os gametófitos são laminares e cordiformes; e após 120 dias de cultivo, o meristema apical é nítido enquanto tricomas unicelulares e estruturas reprodutivas são mais evidentes. Foi estudada a morfoanatomia de esporófitos de P. lepidopteris obtidos após a inoculação de esporos. A morfoanatomia das frondes de esporófitos jovens mostra que essa espécie apresenta escamas em ambas as faces das frondes, epiderme uniestratificada e mesofilo constituído por parênquima esponjoso. Estômatos do tipo anomocítico ocorrem na face abaxial e as células subsidiárias possuem paredes anticlinais sinuosas. O feixe vascular da raque é anficrival delimitado por endoderme.

Reófitas são espécies vegetais confinadas aos leitos de rios e riachos de fortes corredeiras, crescendo acima do nível das inundações, mas não além do alcance da ocorrência regular das rápidas enchentes. A morfoanatomia foliar de Raulinoa echinata R.S.Cowan (Rutaceae) foi investigada objetivando compreender a adaptação às peculiaridades do hábitat reofítico. As folhas de R. echinata são dorsiventrais e hipoestomáticas. A epiderme é uniestratificada e os estômatos são anomocíticos. As células-guarda estão dispostas acima do nível das demais células epidérmicas (característica hidromórfica), mas com átrio externo conspícuo (característica xeromórfica). Aspectos mais marcantes relacionados ao hábitat reofítico são estenofilia e presença de suberina nas paredes periclinais internas das células epidérmicas. A área foliar varia com a intensidade de exposição ao sol, a densidade estomática varia com o fluxo do rio; as espessuras dos tecidos do mesofilo variam com ambas condições. Ductos secretores e idioblastos com cristais de fosfato de cálcio ocorrem no mesofilo. Os dados revelaram plasticidade da estrutura foliar às condições peculiares do ambiente reofítico e as condições sombreadas ou expostas ao sol. Rheophytes are plants confined to the beds of swift-running streams and rivers, growing above flood level but not beyond the normal reach of flash floods. Leaf morphoanatomy of the rheophyte Raulinoa echinata R.S.Cowan (Rutaceae) was investigated with a focus on understanding adaptation to the peculiarities of the rheophytic habitat. R. echinata leaves are dorsiventral and hypostomatic. The epidermis has a single layer of cells and the stomata are anomocytic. The guard cells rise above the level of the other epidermal cells (a hydromorphic characteristic), but with a conspicuous outer cavity (a xeromorphic characteristic). Prominent traits related to the rheophytic habitat are stenophylls and the presence of suberin on the inner periclinal walls of the epidermal cells. Leaf area varies with sun exposure while stomatal density varies with river flow. In R. echinata, mesophyll tissue thickness varies with light intensity and river flow. Secretory cavities and idioblasts with calcium phosphate crystals occur in the mesophyll. The data revealed leaf-structure plasticity in relation to the singular conditions of the rheophytic environment as well as to shade or sunlight.

OBJETIVO: Comparar perfil clínico e cirúrgico entre dois grupos de pacientes submetidos a Cirurgia de Revascularização Miocárdica (CRM) no Instituto de Cardiologia do Rio Grande do Sul, com intervalo de 10 anos; observar sua influência na mortalidade hospitalar e verificar previsibilidade deste resultado mediante escore de risco. MÉTODOS: Estudo de coorte retrospectivo, envolvendo 307 pacientes submetidos a CRM isolada em período semestral de 1991/92 (grupo INICIAL, n=153) ou 2001/02 (grupo ATUAL, n=154). Foram analisados características demográficas, doenças cardíacas, co-morbidades e eventos operatórios, visando à comparação entre grupos e definição do escore de risco de morte hospitalar (conforme Cleveland Clinic). RESULTADOS: O grupo ATUAL tinha idade mais avançada, condição cardíaca mais grave (classe funcional, prevalência de insuficiência cardíaca e número de vasos com lesão severa) e maior prevalência de co-morbidades. Os pacientes iniciais mostraram maior prevalência na indicação cirúrgica de urgência. Não ocorreu diferença no escore médio de risco calculado para ambos os grupos (2,8 + 3,1 no INICIAL e 2,2 + 2,5 no ATUAL) ou na mortalidade hospitalar (respectivamente 3,3% e 1,9%), valores comparáveis com os comunicados pela Cleveland Clinic (para escore de risco 3, mortalidade prevista de 2,0 %, com limite de confiança 95% de 0-4,3% e mortalidade real em estudo de confirmação de 3,4%). CONCLUSÃO: Pacientes atualmente submetidos a CRM são mais idosos e em pior condição clínica (cardíaca e sistêmica) que os operados há 10 anos, mas a pontuação no escore de risco e a mortalidade hospitalar foram discretamente aumentadas no grupo inicial. Para isto, pode ter contribuído maior prevalência de cirurgias de urgência. Um escore de risco pode ser utilizado para identificar pacientes que requerem maiores cuidados e predizer o resultado cirúrgico. OBJECTIVE: To compare clinical and surgical and clinical profiles of two group of patients submitted to Coronary Artery Bypass Graft (CABG) at Instituto de Cardiologia do RGS within a decade interval, define related hospital mortality and to validate a severity risk score for this populations. METHODS: Retrospective cohort study, including 307 CABG patients operated during six months of 1991/92 (n=153) and 2001/02 (n=154). Demographic characteristics, heart disease severity, co-morbidities and pre-operative events were evaluated and compared between groups. Hospital mortality and severity risk scores for mortality according to Cleveland Clinic were also assessed. RESULTS: Patients operated in 2001/02 were older, had more severe cardiac disease (increased NYHA functional class, prevalence of heart failure, and distribution of coronary diseased) and presented more co-morbidities than those operated in 1991/92. This patients required urgent surgery more often. The mean Cleveland Clinic severity risk score wasn't different between groups (2.8 + 3.1 in 1991/92 and 2.2 + 2.5 in 2001/02) and hospital mortality didn't present significant difference (3.3% and 1.9%, respectively). For Cleveland Clinic score 3 the predicted mortality was estimated as 2.0% (with confidence limit 95% of 0- 4.3%) and observed mortality for surgical patients of this institution was 3.4%. CONCLUSION: Patients submitted to CABG currently are older and in worse clinical conditions than those operated 10 years ago, but severity risk scores and hospital mortality were discretely increased for initial series of patients. This may be a consequent of a higher prevalence of urgent surgical indication in those patients. A severity risk score for hospital mortality can be used to predict surgical result and identify patients requiring specific care.

Objective.To develop a predictive system for the occurrence of nosocomial pneumonia in patients who had cardiac surgery performed.Design.Retrospective cohort study.Setting.Two cardiologic tertiary care hospitals in Rio de Janeiro, Brazil.Patients.Between June 2000 and August 2002, there were 1,158 consecutive patients who had complex heart surgery performed. Patients older than 18 years who survived the first 48 postoperative hours were included in the study. The occurrence of pneumonia was diagnosed through active surveillance by an infectious diseases specialist according to the following criteria: the presence of new infiltrate on a radiograph in association with purulent sputum and either fever or leukocytosis until day 10 after cardiac surgery. Predictive models were built on the basis of logistic regression analysis and classification and regression tree (CART) analysis. The original data set was divided randomly into 2 parts, one used to construct the models (ie, “test sample”) and the other used for validation (ie, “validation sample”).Results.The area under the receiver–operating characteristic (ROC) curve was 69% for the logistic regression model and 76% for the CART model. Considering a probability greater than 7% to be predictive of pneumonia for both models, sensitivity was higher for the logistic regression models, compared with the CART models (64% vs 56%). However, the CART models had a higher specificity (92% vs 70%) and global accuracy (90% vs 70%) than the logistic regression models. Both models showed good performance, based on the 2-graph ROC, considering that 84.6% and 84.3% of the predictions obtained by regression and CART analyses were regarded as valid.Conclusion.Although our findings are preliminary, the predictive models we created showed fairly good specificity and fair sensitivity.

To compare the clinical and surgical profile between two groups of patients submitted to Myocardial Revascularization (MCR) surgery at the Instituto de Cardiologia of Rio Grande do Sul with a ten year interval, to observe its influence upon MCR hospital mortality and to verify the predictability of this result using the risk score.A retrospective cohort study involving 307 patients who underwent MCR surgery within a six month period during 1991/92 (INITIAL group, n=153) or 2001/02 (CURRENT group, n=154). Demographic characteristics, heart disease, comorbidities and surgical events were analyzed to compare the groups and to define the hospital mortality risk score (based on the Cleveland Clinic method).The CURRENT group was older, had more severe heart condition (functional class, incidence of heart failure and number of vessels with severe lesions) and a greater prevalence of comorbidities. The INITIAL group had a higher prevalence of nonelective surgery. Both groups had similar mean risk scores (2.8 +/- 3.1 for INITIAL and 2.2 +/- 2.5 for CURRENT) and hospital mortality rates (3.3% and 1.9% respectively). These figures are comparable to those for reported by Cleveland Clinic (for a risk score of 3 the predicted mortality range between 2.0 %; using a confidence level of 95% the predicted mortality is between 0 and 4.3%; and actual mortality confirmed by the study was 3.4%).Patients currently submitted to MCR are older and in worse clinical condition (heart and systemic) than those operated on ten years ago; however, the risk scores and hospital mortality rates were slightly higher in the INITIAL group. The higher number of nonelective surgical interventions could have contributed to this. A risk score can be used to identify patients that require a higher level of care and to predict surgical outcomes.

Morphohistological analysis and histochemical studies were carried out during the induction and development of Feijoa sellowiana somatic embryos. Zygotic embryos were cultured on LPm medium containing 2,4-dichlorophenoxyacetic acid (20 microM) and glutamine (8 mM). Somatic embryogenesis could be induced from embryogenic cells that originated in meristematic centers or from clusters of cells. The presence of few starch grains and abundant protein bodies was observed in the globular and early torpedo stages, while in torpedo and cotyledonary-stage somatic embryos an enhanced synthesis of starch grains was associated with the accumulation of reserves to be used in the conversion of the embryos to plantlets. Proteins were predominantly observed in protoderm cells, as well as in the meristematic apical region of torpedo and cotyledonary-stage somatic embryos.

Verificou-se a resposta de crescimento à variação na intensidade de luz de plântulas de três espécies arbóreas da Floresta Tropical Atlântica, Cecropia glazioui Sneth., Cedrela fissilis Vell. e Bathysa australis (A. St.-Hil.) Hook. ex Sch., respectivamente de estádios inicial, intermediário e final de sucessão. As três espécies mostraram, dentro de um determinado gradiente de luz, plasticidade para aumentar a captação de luz quando em baixa irradiância (através de aumento da razão de área foliar -RAF e diminuição da razão entre raiz e parte aérea - R/PA) e plasticidade para aumentar o ganho de carbono e diminuir a transpiração quando em alta irradiância (através dos aumentos da razão R/PA e densidade estomática, e da diminuição da RAF). A plasticidade das espécies em variar determinado parâmetro em função da intensidade de luz foi dependente do gradiente de intensidade de luz aplicado. A plasticidade foi maior nas intensidades mais baixas de luz tanto para C. glazioui quanto para C. fissilis. Para a maior parte dos parâmetros analisados, C. glazioui mostrou maior parte plasticidade para aclimatar-se à maior irradiância, que C. fissilis. As variações apresentadas pelas espécies na morfologia e fisiologia em relação à variação na intensidade de luz são consistentes com o local de ocorrência de cada espécie. The growth response of seedlings of three woody species from the Tropical Atlantic rain forest to changing light conditions was studied. The species were the early successional Cecropia glazioui Sneth., the intermediary in succession Cedrela fissilis Vell. and the late successional Bathysa australis (A. St.-Hil.) Hook. ex Sch. These species showed, within the range of light gradient, plasticity to increase light interception at lower light levels (through increasing leaf area ratio - LAR, and decreasing root/shoot ratio - R/SH); and plasticity to increase carbon gain and decreasing transpiration at high light levels (through increasing stomatal density and R/SH, and decreasing LAR). Species responses to irradiance varied with position along the light gradient. Plants at the lower end of the light gradient were more responsive than plants at the higher end of the light gradient. C. glazioui seemed to be more plastic than C. fissilis to alter several features. The behavior showed by the three species in response to changing light was consistent with their environmental occurrence.

Summary PCR was used to detect Coxiella burnetii and Bartonella spp in heart valves obtained during the period 1998–2009 from patients operated on for blood culture-negative endocarditis in a cardiac surgery hospital in Brazil. Of the 51 valves tested, 10 were PCR-positive; two were positive for Bartonella and one for C. burnetii .

A fruticultura moderna necessita implantar tecnologias que possibilitem a produção de frutos de alta qualidade, com custos cada vez menores. A micropropagação associada à microenxertia possibilita altas taxas de multiplicação de plantas com alta qualidade fitossanitária, além de possibilitar a realização de estudos sobre compatibilidade de enxertia em diferentes clones. O presente trabalho tem como objetivo estudar o processo de soldadura entre genótipos de macieira (Malus domestica. Borkh) multiplicadas in vitro após a microenxertia. Esta técnica foi realizada em fenda simples, sob condições assépticas. Os estudos histológicos foram realizados através de cortes longitudinais seriados de segmentos de 8 mm do ponto de enxertia. O processo de soldadura foi caracterizado pelo desenvolvimento de tecido meristemático, originando células parenquimáticas na interface do microenxerto, com a proliferação do tecido cambial da cultivar copa. Isso possibilita a ligação do sistema vascular da copa com o do porta-enxerto, resultando na sobrevivência do microenxerto. The modern fruitculture needs to introduce techniques that allow a less expensive production of high quality fruits. The micropropagation associate to micrografting provides high average of plant multiplication and disease free material. In addition it provides studies of grafting compatibilities. The present work aim to study the process of the micrografting union of tissue cultured apple (Malus domestica. Borkh). The scions were cleft-grafted on the rootstocks, under aseptic conditions. For histological studies specimens were trimmed down to 4mm above and below the graft union (longitudinal cuts). The graft union development resulted in the generation of meristem tissue, originating parenchymatous cells at the graft interface, and a cambial tissue proliferation from the scion. This procedure allows the vascular system connection between the scion and the rootstock, resulting on micrograft viability.

The physical dormancy of seeds has been poorly studied in species from tropical forests, such as the Atlantic Forest. This study aimed to examine the effect of moderate alternating temperatures on breaking the physical dormancy of seeds, the morphoanatomy and histochemistry of seed coats, and to locate the structure/region responsible for water entrance into the seed, after breaking the physical dormancy of seeds of two woody Fabaceae (subfamily Faboideae) species that occur in the Brazilian Atlantic Forest: Sophora tomentosa and Erythrina speciosa. To assess temperature effect, seeds were incubated in several temperature values that occur in the Atlantic Forest. For morphological and histochemical studies, sections of fixed seeds were subjected to different reagents, and were observed using light or epifluorescence microscopy, to analyze the anatomy and histochemistry of the seed coat. Treated and non-treated seeds were also analyzed using a scanning electron microscope (SEM) to observe the morphology of the seed coat. To localize the specific site of water entrance, the seeds were blocked with glue in different regions and also immersed in ink. In the present work a maximum temperature fluctuation of 15ºC was applied during a period of 20 days and these conditions did not increase the germination of S. tomentosa or E. speciosa. These results may indicate that these seeds require larger fluctuation of temperature than the applied or/and longer period of exposition to the temperature fluctuation. Blocking experiments water inlet combined with SEM analysis of the structures of seed coat for both species showed that besides the lens, the hilum and micropyle are involved in water absorption in seeds scarified with hot water. In seeds of E. speciosa the immersion of scarified seeds into an aniline aqueous solution showed that the solution first entered the seed through the hilum. Both species showed seed morphological and anatomical features for seed coats of the subfamily Faboideae. Lignin and callose were found around all palisade layers and the water impermeability and ecological role of these substances are discussed in the work.

Mupirocin is a topical antimicrobial agent that has been successfully used to eradicate methicillin-resistant Staphylococcus aureus from the anterior nares and other sites of patients and health care personnel. This report describes the acquisition of a novel mupirocin resistance gene (ileS) by an epidemic MRSA clone that is geographically widespread in Brazil.
