In the paper the authors report their experience with application of a jugular fatcutaneous pedicled graft for primary reconstruction of defects of the oral mucous membrane following resection of localy spread malignant tumors under conditions of preliminary radiotherapy. Results of the plasty in 30 patients are reported. The technic of cutting out the graft is described.

An 11-year-old hemophiliac with a high level of Factor VIII inhibitor (50 Bethesda units) developed a pseudotumor of the mandible. Treatment with Factor IX concentrates was unsuccessful. Since surgical management was contraindicated, the patient was given 600 rad to the mandibular lesion, which resulted in complete recovery. The authors stress that radiotherapy may be successful in pseudotumor when surgery is contraindicated.

A 23-year-old female patient had a sagittal split mandibular osteotomy performed to correct prognathism. Two years after surgery she developed pain in the distribution of the right inferior dental and mental nerves. The area was investigated surgically and a traumatic neuroma was excised.

A case of fibrosarcoma of the mandible following radiotherapy for bilateral retinoblastoma and occurring in an 11-year-old female child is described. After a clinical description of the case, reported with histological documentation, problems connected with the pathogenesis of the malignancies are dealt with. Besides irradiation, genetic mutation as a carcinogenetic co-factor is taken into consideration.

Tumours of cartilaginous origin dominate the group of tumours of the mandibular condyles. These cartilaginous tumours consist in fact of hyperplasia of the growth zone or osteochondroma in the young adult or reawakening of the latter in the older adult in whom an occlusion problem originally is not without aetiological interest. Osteomas of the condyles are stabilised and ossified forms of such hypercondylism. Other tumours may be seen and are related to the original cell. Malignant tumours are rare.

There are few reported cases of intraosseous lipoma of the jaws in the literature. The exact nature of these lesions is debatable, but it is widely accepted that they represent true benign tumours. In this presentation discussion is centered on a case of a lipomatous lesion of the mandible and is the first to be reported in relation to a retained apex. The features for and against this lesion being of a neoplastic origin are discussed. The characteristic histopathological finding was the presence of only mature adipose fat cells.

A case is presented of a patient with a giant ossifying fibroma of the mandible that had been diagnosed earlier as fibrous dysplasia. Surgical resection of the tumor and involved bone was chosen as the mode of treatment because of the extent of the lesion and continued infection. Surgeons should be aware that ossifying fibroma is a very distinct possibility in large growing lesions in age groups older than is deemed compatible with fibrous dysplasia.

A mid-line cyst of the mandible is described which possessed the histological features of a dermoid cyst. This appears to be the first fully documented example of a totally intra-osseous dermoid cyst in the mandible. The pathogenesis of the present lesion is discussed in relation to both the odontogenic and the non-odontogenic cysts that may occur in the mandibular mid-line.

During the period from 1969 to 1977 11 cases of myxoma or myxofibroma in the jaw region have been treated at the Maxillofacial Surgery Clinic of the University of Mainz. Various kinds of diagnostic problems are discussed. We observed, inter alia, rapid tumour growth and a imitative dynamics in the radiographs, giving rise to a suspicion of malignancy in some cases. The locally infiltrative type of growth of myxomas must be taken into account during therapy, since insufficiently radical management is liable to be followed by a recurrence.

Supravoltage irradiation is commonly thought not to be carcinogenic. Several recent studies question this concept, as does our case report. A 50-year-old woman with stage 1 squamous carcinoma of the left side of the tongue was treated in 1973 with 73 Gy of supravoltage irradiation. Twelve years later a painful, ulcerated lesion that eventually was shown to be fibrosarcoma developed in the contralateral mandible. The fibrosarcoma in this case fulfills all criteria for diagnosing radiation-induced neoplasia and demonstrates that supravoltage irradiation, like other forms of irradiation, can cause malignancy. The occasional occurrence of sarcoma should be recalled during follow-up of patients treated with supravoltage radiation. Similarly, the possibility of radiation-induced tumors should be considered in planning treatment for younger patients with tumors that can be treated equally well by surgery or irradiation.

We report an unusual case of mandibular sarcoma which developed in the field of irradiation of a patient treated successfully 10 years previously for nasopharyngeal carcinoma (NC). Although second malignant neoplasms are not rare among survivors of childhood cancer, this event is not included among the type of failures and complications of treatment of NC occurring either in children.

The analysis of the literature data and the author's own findings justifies the conclusion that a single mechanical trauma may be of importance in the development of tumours of various genesis, as well as in the development and localization of metastastatic foci.

This report deals with a case of AIDS-related Burkitt's lymphoma in which some of the earliest findings were dental and periodontal in nature. The patient presented initially with painless and extremely loose teeth accompanied by progressive paresthesia of the lower jaw. Unique radiographic findings included extensive periodontal ligament space widening and bulbous, granuloma-like lesions about the apices of the teeth. These findings were associated with progressive tumor infiltration of the mandible and do not appear to be related to other reports of aggressive periodontitis associated with impaired immunologic functions in AIDS patients.

A case of adenosquamous carcinoma of the floor of the mouth and alveolus that occurred following radiation therapy is described. The possible role of radiation in the etiology of this lesion is discussed, and the complex histopathologic features of this neoplasm are emphasized.

Orthopantomography of the mandible in 50 consecutive patients with ulcerative colitis showed mandibular osteomas in 2 patients (4%), which is the same frequency as found in the normal population. Since earlier investigations have shown an increased frequency of mandibular osteomas in familial polyposis coli, the cancer family syndrome, and colorectal cancer without known familial disposition, it is concluded that the etiology of colorectal carcinoma in ulcerative colitis is not, as for these diseases, a combination of the same genetic and environmental factors but probably only a function of the duration and involvement of the disease.

Intramandibular salivary inclusions and ectopia concern the main salivary glands. The most common takes the form of a regular well-defined lacunar appearance situated at the mandibular angle or molar region. This asymptomatic inclusions is in fact only the imprint of a lobule of the sub-mandibular gland on the mandible. More rarely, the parotid gland and sublingual gland may also be responsible for a lacunar radiological appearance in the mandible. By contrast, a few ectopic glandular lobules of the sublingual may be completely induced at the site of the mandibular symphsis. Salivary inclusions and ectopia thus result from two different aerio-pathogenic mechanisms.

Two cases of brown tumors associated with secondary hyperparathyroidism and chronic renal failure are reported. Both patients underwent excision of the tumor because of mechanical interference. Subsequent parathyroidectomy has substantially improved the general status of the patients.

A case of an unusually large traumatic bone cyst of previously unreported size and location is presented. The presence of the cyst 5 years before its current discovery was confirmed on previous radiographs. This aggressive traumatic bone cyst adds another dimension to the usual course of this lesion.
