Aim: The aim of this article was to retrospectively analyze and compare the esthetic outcomes achieved after the use of 20 subciliary incisions, 22 subtarsal incisions and 16 infraorbital incisions to approach the infraorbital rim and orbital floor in orbital fractures. Methods: The sample consisted of 58 patients (37 males and 21 females) with orbital trauma (floor and infraorbital rim) treated with open reduction and internal rigid fixation in the Department of Oral and Maxillofacial Surgery at “Hospital de Base do Distrito Federal”, Brazil, between September 1996 and August 2003. The following aspects were evaluated: (1) the average distance of the scars measured from the ciliary margin caudally, (2) the esthetic appearance of the scars, (3) chronic lid edema, (4) scleral show, (5) ectropion. Results: Subciliary incision demonstrated better surgical results when compared to the non-subciliary incisions. No statistically significant difference in chronic lid edema rates was found between the three groups of incisions (Fisher, p>0.217 in all cases). There was no statistically significant difference in ectropion, scleral show and chronic edema rates between the three groups of incisions. Background: Despite increased awareness of diet and nutrition being integral to the management of patients with inflammatory bowel disease (IBD), there are gaps in the knowledge of IBD healthcare providers regarding nutrition. Furthermore, high quality evidence on nutritional assessment and dietary management of IBD is limited. A Delphi consensus from a panel of experts allows for best‐practice guidelines to be developed, especially where high quality evidence is limited. The aim was to develop guidelines for the nutritional assessment and dietary management of IBD using an eDelphi online consensus agreement platform. Methods: Seventeen research topics related to IBD and nutrition were systematically reviewed. Searches in Cochrane, Embase®, Medline® and Scopus® electronic databases were performed. GRADE was used to develop recommendations. Experts from the IBD community (healthcare professionals and patients with IBD) were invited to vote anonymously on the recommendations in a custom‐built online platform. Three rounds of voting were carried out with updated iterations of the recommendations and evaluative text based on feedback from the previous round. Results: From 23,824 non‐duplicated papers, 167 were critically appraised. Fifty‐five participants completed three rounds of voting and 14 GRADE statements and 42 practice statements achieved 80% consensus. Comprehensive guidance related to nutrition assessment, nutrition screening and dietary management is provided. Conclusions: Guidelines on the nutritional assessment and dietary management of IBD have been developed using evidence‐based consensus to improve equality of care. The statements and practice statements developed demonstrate the level of agreement and the quality and strength of the guidelines. A positive relationship has been documented for a wide diversity of taxa between the percentage of transects sampled in which a species is recorded (i.e., occupancy) and the average abundance of the species at transects where recorded. This positive relationship implies that abundance increases faster than occupancy, so populations that occupy more sites also tend to occupy them at higher abundances. Plainly, there is a limit to the sites available for a species to occupy, so as the population expands numerically, abundance at a site must also increase. The pattern may differ across species and geography depending on aspects such as species vital rates, resource use, and resource availability. I investigated abundance–occupancy patterns of three black basses Micropterus spp. in reservoirs of the mainstem Tennessee River, USA. The data set included relative abundance estimates made at 7,237 sites in nine reservoirs sampled during 1997–2018, for 43,243 black bass, including 67% Largemouth Bass Micropterus salmoides, 14% Smallmouth Bass M. dolomieu, and 19% Spotted Bass M. punctulatus. As relative abundance increased due to natural annual population fluctuations, occupancy also increased, but faster for Largemouth Bass and more slowly for Smallmouth Bass and Spotted Bass. Largemouth Bass spread abundance more thinly over many sites, and Smallmouth Bass and Spotted Bass spread abundance more thickly over fewer sites. The recognition that black bass populations that decline in occupancy face the additional burden of disproportionally larger decreases in abundance per site, or that black bass that decline in abundance per site face decreases in occupancy, has various conservation and habitat management implications. OBJETIVO: Avaliar o tratamento da insuficiência cardíaca (IC) avançada em relação à redução das pressões de enchimento ventricular, com a utilização de doses maiores de vasodilatadores, através da monitorização hemodinâmica invasiva. MÉTODOS: Foram estudados 19 pacientes com IC avançada, nos quais foi instalado o cateter de Swan-Ganz para guiar a administração de diurético intravenoso (IV) e nitroprussiato de sódio, com o objetivo de se reduzir de forma significativa as pressões de enchimento ventricular. Depois de alcançado esse objetivo ou 48 horas, foram introduzidas drogas orais até serem retirados os fármacos venosos, mantendo o benefício hemodinâmico. RESULTADOS: Dos 19 pacientes estudados, 16 (84%) eram do sexo masculino. A idade média foi de 66 ± 11,4 anos; a fração de ejeção média foi de 26 ± 6,3%; 2 pacientes (10,5%) apresentavam classe funcional (CF) III e 17 (89,5%), CF IV. Houve queda da pressão de oclusão da artéria pulmonar de 23 ± 11,50 mmHg para 16 ± 4,05 mmHg (p=0,008), do índice de resistência vascular sistêmica de 3.023 ± 1.153,71 dynes/s/cm-5/m² para 1.834 ± 719,34 dynes/s/cm-5/m² (p=0,0001) e aumento do índice cardíaco de 2,1 ± 0,56 l/min/m² para 2,8 ± 0,73 l/min/m² (p=0,0003). Um subgrupo com hipovolemia foi identificado. CONCLUSÃO: Foi possível reduzir as pressões de enchimento ventricular para valores significativamente menores, obtendo melhora significativa do índice cardíaco, do índice de resistência vascular sistêmica e da pressão média da artéria pulmonar, utilizando-se doses significativamente maiores de vasodilatadores.OBJECTIVE: To assess advanced heart failure (HF) treatment in relation to reduction of ventricular filling pressures, with the use of greater doses of vasodilators, through invasive hemodynamic monitoring. METHODS: Nineteen advanced HF patients were studied, in whom Swan-Ganz catheter was inserted to direct administration of diuretic intravenously (IV) and sodium nitroprusside, with the aim of significantly reduce ventricular filling pressures. After achieving such objective or 48 hours, oral drugs were introduced until venous medicines were removed, keeping hemodynamic benefit. RESULTS: From 19 patients studied, 16 (84%) were of male sex. The average age was 66 ± 11.4 years old; average ejection fraction was 26 ± 6.3%; 2 patients (10.5%) showed functional class (FC) III and 17 (89.5%), FC IV. There was a decrease of pulmonary artery occlusion pressure from 23 ± 11.50 mmHg to 16 ± 4.05 mmHg (p = 0.008), of systemic vascular resistance index from 3,023 ± 1,153.71 dynes/s/cm-5/m² to 1,834 ± 719.34 dynes/s/cm-5/m² (p = 0.0001) and an increase of cardiac index from 2.1 ± 0.56 l/min/m² to 2.8 ± 0.73 l/min/m² (p = 0.0003). A subgroup with hypovolemia was identified. CONCLUSION: It was possible to reduce ventricular filling pressures to significantly lower values, obtaining a significant improvement of cardiac index, systemic vascular resistance index and pulmonary artery mean pressure, by using significantly higher doses of vasodilators.
