Abstract

Purpose To determine whether an ultrasonography (US)-defined thyroid volume can accurately predict substernal extension or tracheal narrowing. Methods After research ethics approval, we identified patients with thyroid nodules investigated with both US and computed tomography (CT). Reviewers assigned scores for both substernal extension and tracheal compression on CT using pre-established classification systems. Statistical analysis with receiver operating characteristic curve analysis was performed to find the US-determined thyroid volume thresholds that correlated with each substernal extension and tracheal compression. Results This study included 120 patients (mean age 63.4 years; SD ± 15.9; 67% female). Thirty-five patients (29%) had substernal extension. The mean US total thyroid gland volume in patients with and without substernal extension were 92.4 and 37.6 cm3, respectively ( P < .001). 86% of patients with substernal extension had tracheal narrowing vs. 27% of patients without substernal extension ( P < .0001). A cutoff dominant gland volume of ≥37.5 cm3 showed 83% sensitivity and 79% specificity for substernal extension (area under the curve [AUC] = 0.84). A total thyroid gland volume threshold of ≥37.8 cm3 showed 89% sensitivity and 87% specificity for any degree of tracheal narrowing (AUC = 0.90). Conclusions This study suggests that US volumes may be used as a predictor to identify those patients with thyroid enlargement who are most at risk of substernal extension and tracheal compression and who may benefit from preoperative CT imaging for optimal surgical and anesthetic planning.

Abstract

The Karnofsky Performance Status (KPS) of HIV-infected people may accurately indicate overall health status, but not as accurately reflect psychosocial condition or physical performance. On a scale of 0 to 100, with 100 being the healthiest, KPS indicates quality of life due to health, which is known as health status. KPS is determined by an examining health care worker. Researchers compared KPS with various self-administered health indicators for 160 HIV-positive people. The average KPS score was 82.0. KPS scores were most strongly associated with patient ratings of overall health status, indicators of energy and fatigue, and days off work. KPS correlated somewhat less closely with number of symptoms, total disability score, time off from work, Centers for Disease Control disease category, and a measure of social interaction. Finally, KPS scores correlated least with measures of psychological status and mental function.

Abstract

Background and purpose: Computed tomography virtual endoscopy (CT-VE) is a non-invasive technique which allows visualisation of intraluminal surfaces by tridimensional reconstruction of air/soft tissues. The aim of this study was to compare the diagnostic accuracy of CT-VE and flexible fibre-optic laryngoscopy (FFL) in identifying normal neck anatomic structures and pharyngeal and laryngeal lesions. Methods: Forty-two patients with a history of neck cancer were assessed by two ENT surgeons using FFL and by one neuroradiologist using CT-VE in order to evaluate the visualisation of the epiglottis, vallecula, glossoepiglottic folds, pyriform sinuses, vocal cords and mass pathology. The visualisation of the structures in both modalities was assessed according to the following score: 0 = not visualised, 1 = partial visualisation, 2 = complete and clear visualisation. A weighted kappa coefficient was used to evaluate the inter-observer agreement. McNemar's test was performed to compare the two diagnostic tests. Results: The inter-observer agreement between FFL and CT-VE was fair in the assessment of the vocal cords (k = 0.341); moderate in the assessment of the glossoepiglottic folds (k = 0.418), epiglottis (k = 0.513) and pyriform sinuses (k = 0.477); and substantial in the assessment of the vallecula (k = 0.618) and the tumour (0.740). McNemar's test showed no significant difference between the two tests (p <0.05). Conclusion: CT-VE is a non-invasive technique with a diagnostic accuracy comparable to FFL in terms of visualisation of anatomical structures and pharyngeal and laryngeal lesions. A corporation faces a multitude, but solvable, tax issues when it acquires partnership interest vis-a-vis the simple tax issues a transferee partner does. Aside from ascertaining whether the transfer comes within the purview of Sections 1001, 741 and 351 on taxable exchanges, the corporate partner should be aware of special tax burdens created in this new role. But with a thorough understanding and careful planning of these tax consequences, it would be enlightening to discover that a lot of tax benefits can be gained from this transaction.

Abstract

Although vaccination is a key step in disease prevention, a significant proportion of adults contract diseases that can be prevented by immunizations. A surprisingly large number of adults have not undergone vaccination against hepatitis, influenza, pneumococcal infection (a type of pneumonia), and other preventable diseases. Several groups, including the Centers for Disease Control, the American College of Physicians, the Academy of Family Physicians, and the American Medical Association, have taken steps to encourage adult immunization. Since resident physicians are notoriously resistant to preventive medicine, and since immunization can be objectively measured by reviewing medical records, a study was carried out concerning family practice residents' immunization practices. The charts of 100 patients served by each of four residency programs in Kansas City, Missouri, were evaluated to determine whether the patients had been vaccinated. Residents and faculty (102 respondents) were questioned concerning how they recorded immunizations and which immunizations would be appropriate for hypothetical patients. Results revealed that only 25 percent of patients eligible for pneumococcal vaccine had actually received it. Tetanus-diphtheria vaccinations were recorded for slightly less that five percent of eligible patients, and influenza vaccine had been given to 24 percent of patients who were eligible. No information was recorded concerning immunization against hepatitis B, measles, mumps, or rubella. Disparities were noted between what respondents knew patients needed, and the immunizations they said they would offer to hypothetical patients. Detailed descriptions of physicians' immunization practices are provided. The findings suggest that residents in family physician programs, as well as patients, need additional education in preventive medicine. Abstract

The Texas Earth and Space Science (TXESS) Revolution was a 5-y teacher professional development project that aimed to increase teachers' content knowledge in Earth science and preparing them to teach a 12th-grade capstone Earth and Space Science course, which is new to the Texas curriculum. The National Science Foundation-supported project was designed around six principles that proved to be critical to in its success: (1) model best practices in workshop presentations, (2) use authentic Earth science data and cybertechnology to teach up-to-date content, (3) provide ongoing training to cohorts of learners over a 2-y period, (4) involve geoscience consortia and programs that can provide proven content for classrooms, (5) use ongoing evaluations to guide future workshops, and (6) provide opportunities for leadership development through participation in research and curriculum development projects. The project served 177 science teachers by supporting them with the pedagogical, technological, and scientific tools to teach modern geoscience. TXESS Revolution teachers directly impacted more than 29,000 students, of which about 69% are nonwhite, by exposing students in Texas to the geosciences and planting the seeds for them to pursue geoscience as a field of study. Using a train-the-trainer approach, TXESS Revolution teachers shared their professional development with other Texas teachers, strengthening Earth science education at all K-12 levels throughout the state, an impact that extends beyond preparation in Earth and space science. 2013 National Association of Geoscience Teachers.
