152. Hunger, Health and Society: Changing Nutrition-Related Knowledge and Behaviour: Video vans in the Rural Philippines

Improvements in nutritional knowledge and attitudes and nutritional status were found in the rural Nutribus areas surveyed. Feeding standards improved, as measured by the mothers’ reports of what they fed children. In 1979 mothers in the VTRL villages were 55 per cent more likely to describe complete meals than those in the villages of the comparison group. Mothers in the VTRL villages were 473 per cent more likely to name Nutri-Pak as a good snack and 71 per cent more likely to give correct specific descriptions of Nutri-Pak than the BNS village mothers (fig. 2). Mothers in 1981 were 27 per cent more likely to describe complete meals (fig. 5) than those inter viewed in 1979.As for nutritional status, in 1979 there was 25 per cent less third” and second-degree malnutrition among children of the VTRL villages than among those of the BNS villages and 29 per cent more first-degree malnutrition and normal nutritional status, using the Harvard standards for weight for age and the Gomez classification (fig. 4). Comparison of the nutritional status results of 1979 and 1981 shows a 17 per cent decrease in third- and second-degree malnutrition and a 12 per cent increase in first-degree malnutrition and normal status (fig. 7).Evidence from the 1979 and 1981 evaluation of the Nutri-bus project favours the assumption of its effectiveness. It also reinforces the recognized need for improvements in the field management system, videotape production efficiency, and Nutri-Pak distribution system. The Nutrition Center of the Philippines has committed itself to the continuing development and evaluation of the Nutri-bus project.

- Abstract
To retain the expert knowledge of mission systems specialists, the Air Force Flight Test Center is developing Structured Test Procedures (STPs) for critical flight tests. Each STP contains one or more test setups that have been used in actual test and evaluation efforts. It also contains extensive introduction sections to bring the test and evaluation specialist up to speed on evaluation issues associated with system/operator interfaces (i.e., side controllers), specific systems (i.e., autopilots), and specific tasks (i.e., low altitude parachute extraction system (LAPES)). STPs include previously used test objectives that are invaluable examples for new test plans. There is also a list of relevant standards with thresholds for further evaluation and for definite system deficiency. Thresholds are provided for design, operational, and health limits. Resource requirements including personnel and equipment are listed to support test planning. Measurement guidance is provided to help the user define accuracy and calibration requirements for all measurement equipment to be used. Samples of each are provided in the paper. This format has been exceeding useful in capturing test and evaluation expertise.

155. Effect of Active Versus Passive Recovery on Performance During Intrameet Swimming Competition

- Abstract
Background:During competition, high-performance swimmers are subject to repeated physical demands that affect their final performance. Measurement of lactate concentration in blood seeks to indirectly gauge physiologic responses to the increase in physical exercise. Swimmers face multiple maximal-exertion events during competition. Strenuous physical exercise leads to fatigue and, thus, a decrease in sports performance.Hypothesis:Regeneration exercises in swimming increase the clearance of blood lactate and therefore improve athletic performance within a single day of competition.Study Design:Crossover study.Level of Evidence:Level 1.Methods:Of 25 swimmers, 21 were included, with a mean age of 17 years. They performed exercises that increased blood lactate on 2 days. The protocol was a warm-up, followed by a 100-m freestyle workout at full speed. Swimming exercises followed that were increasingly demanding, during which serial lactatemia measurements were taken. On the first day, regeneration exercises were performed; on the second day, the swimmers rested. Next, lactatemia was measured, and a timed 100-m freestyle workout was performed at maximum speed.Results:The stress exercises increased the mean lactate concentration by 4.6 mmol/L, which corresponds to 78% of the initial basal level. The postregeneration lactatemia level was lower than that after resting (mean, 2.76 vs 6.51 mmol/L). The time to swim 100 m after regeneration was 68.11 seconds, while that after rest was 69.31 seconds.Conclusion:Blood lactate levels rose by up to 78% after the intensity of the training sessions was progressively increased. Regeneration exercises increased the rate in which blood lactate dissipated, in comparison with passive recuperation. The rate of lactate dissipation for regeneration exercises was 68%. This factor may have improved the physical performance of swimmers.Clinical Relevance:Regeneration exercises improved the performance of swimmers in maximal-exertion competition in a single day. The blood lactate level correlated with physical exercise load.

The process for obtaining proper color accuracy are also outlined. Among the advantages of such a method are increased color accuracy and repeatability.

- Abstract
Increasing erythropoiesis requires a dramatic increase in iron absorption or the release of iron from tissue stores. Since the iron regulatory hormone hepcidin blocks both absorption and release from stores, it is suppressed during erythropoiesis. We have recently shown that hepcidin is primarily suppressed by an erythropoietic regulator rather then anemia, hypoxia, or erythropoietin (Pak et al, Blood, epub/in press). To further understand the regulation of hepcidin during erythropoiesis, we subjected groups of eight mice to phlebotomy and sacrificed them at various time points over 3 weeks. Hepcidin mRNA fell 25-fold by day 2 (p<0.001) and did not begin to rise until day 9. Hemoglobin rose well before hepcidin mRNA began to increase confirming that anemia was not the erythropoietic regulator. Serum iron never fell, excluding this as the erythropoietic regulator. Erythropoietin levels spiked early but returned to normal well before hepcidin. Reticulocyte counts increased shortly after the fall in hepcidin and remained elevated after hepcidin mRNA returned to normal. We therefore postulate that the erythropoietic iron regulator is either made by or in concert with early erythroid progenitor cells. Using primary hepatocytes treated with serum from erythropoietin stimulated or control mice, we also demonstrate that the erythropoietic regulator of hepcidin expression is present in the circulation. Experiments are underway to isolate the erythropoietic regulator of hepcidin from serum.

178. Ceruloplasmin Is Essential for the Mobilization of Tissue Iron Stores.

- Abstract
In extracellular fluid, iron is in the ferric (oxidized form) but the intracellular form is ferrous iron (reduced). The outflow of iron from cells is dependent on oxidase activity that converts ferrous to ferric iron. Iron-absorbing enterocytes possess a unique iron oxidase, hephaestin. It is presumed that the circulating hephaestin paralog ceruloplasmin fulfils this role in hepatocytes and macrophages. The DiSnA mouse lacks ceruloplasmin. We hypothesized that iron homeostasis in this mouse would be unusually dependent on dietary iron because the mouse would not be able to mobilize iron from tissue stores in hepatocytes and macrophages. We fed 4-week-old DiSnA and wildtype (WT) mice a high iron (1%) diet for 4 weeks to load tissue stores. We then switched them to an iron-deficient diet and analyzed them weekly to measure iron and hemoglobin concentrations. Even on the high iron diet, DiSnA mice had lower serum iron concentrations than WT control (32.9±17.9 vs. 53.5±17.6 μM, p=0.05) but after two weeks on the iron deficient diet, the DiSnA mice had almost undetectable serum iron (4.2±1.8 μM) whereas the WT controls had only declined slightly (43.2±9.6 μM, p<0.001). Iron saturation followed a similar trend. Neither WT nor DiSnA mice were anemic at baseline (Hgb = 13.9±0.3 and 13.9±1.1 g/dL, respectively; p=0.994) but by the end of two weeks, the DiSnA mice had developed anemia whereas the WT mice had not (Hgb = 10.4±0.5 vs. 12.6±0.5 g/dL; p<0.001). The difference in hemoglobin concentrations persisted to the 6-week timepoint (Hgb = 8.4±0.5 vs. 12.6±1.4; p<0.001). After 6-weeks on a low iron diet, iron was still present in livers and spleens of both groups. Ceruloplasmin is essential for the mobilization of iron stores to protect against iron deficiency anemia in response to periods of dietary iron deficiency.

A thermodynamic equilibrium model has been employed to determine the equilibrium estate of Trimethylgallium (TMGa)-AsH3-H2and TMGa-Trimethylarsine (TMAs)-H2systems in vapor phase. The analysis has been made for pressure, hydrogen/trimethyl (H2/TM) ratio, V/III ratio and various temperature ranges, and includes, in both cases, 33 selected gas-phase species in the presence of solid GaAs. A study of the substitution of arsine for TMAs has been carried out, with regard to relative stabilities of the species and the changes in carbon incorporation. Dependence of carbon impurities on the growth parameters has been analyzed in order to postulate the factors that can influence carbon incorporation.

Background: The association between heart rate (HR) and pulmonary embolism (PE) outcomes has not been well studied. Furthermore, optimal cutoffs to identify low-risk and intermediate- to high-risk patients are not well known. Research Question: Does an association exist between baseline HR and PE outcome across the continuum of HR values? Study Design and Methods: The current study included 44,331 consecutive nonhypotensive patients with symptomatic PE from the Registro Informatizado de la Enfermedad TromboEmbólica registry between 2001 and 2021. Outcomes included 30-day all-cause and PE-specific mortality. We used hierarchical logistic regression to assess the association between admission HR and outcomes. Results: A positive relationship was found between admission HR and 30-day all-cause and PE-related mortality. Considering an HR of 80 to 99 beats/min as a reference, patients in the higher HR strata showed higher rates of all-cause death (adjusted OR, 1.5 for HR of 100-109 beats/min; adjusted OR, 1.7 for HR of 110-119 beats/min; adjusted OR, 1.9 for HR of 120-139 beats/min; and adjusted OR, 2.4 for HR of ≥ 140 beats/min). Patients in the lower strata of HR showed significantly lower rates of 30-day all-cause mortality compared with the same reference group (adjusted OR, 0.6 for HR of 60-79 beats/min; and adjusted OR, 0.5 for HR of < 60 beats/min). The findings for 30-day PE-related mortality were similar. For identification of low-risk patients, a cutoff value of 80 beats/min (vs 110 beats/min) increased the sensitivity of the simplified Pulmonary Embolism Severity Index (sPESI) from 93.4% to 98.8%. For identification of intermediate- to high-risk patients, a cutoff value of 140 beats/min (vs 110 beats/min) increased the specificity of the Bova score from 93.2% to 98.0%. Interpretation: In nonhypotensive patients with acute symptomatic PE, a high HR portends an increased risk of all-cause and PE-related mortality. Modifying the HR cutoff in the sPESI and the Bova score improves prognostication of patients with PE.

Aims Little is known about the prognosis of patients with massive pulmonary embolism (PE) and its risk of recurrent venous thromboembolism (VTE) compared with non-massive PE, which may inform clinical decisions. Our aim was to compare the risk of recurrent VTE, bleeding, and mortality after massive and non-massive PE during anticoagulation and after its discontinuation. Registro Informatizado de Enfermedad Tromboembólica Investigators., [Objective] Overlap exists between the risk factors for coronary artery disease and venous thromboembolism (VTE). However, a paucity of data is available on the incidence of major acute cardiovascular events (MACE) and major adverse limb events (MALE) among patients presenting with VTE. Moreover, it is unknown whether the rate of cardiovascular outcomes differs among patients with unprovoked vs provoked VTE., [Methods] We analyzed the data from 2009 to 2017 in the Registro Informatizado de Enfermedad Tromboembólica registry, an ongoing, multicenter, international registry of consecutive patients with a diagnosis of objectively confirmed VTE. The query was restricted it to patients with data entry for the arterial outcomes. The baseline prevalence of coronary artery disease risk factors was compared between patients with provoked (ie, immobility, cancer, surgery, travel >6 hours, hormonal causes) and unprovoked VTE. After the initial VTE event, we followed up patients for the composite primary outcome of incident MACE (ie, stroke, myocardial infarction, unstable angina) and/or MALE (ie, major limb events). We used the χ2 test for baseline associations and a Cox proportional hazard for multivariate analysis. We used IBM SPSS, version 24 (IBM Corp, Armonk, NY) for statistical analysis. A P value of, [Results] We analyzed the data from 41,259 patients with VTE, of whom 22,633 (55.6%) had experienced a provoked VTE. During follow-up, the patients with provoked VTE were more likely to develop MACE or MALE than were patients with unprovoked VTE (hazard ratio [HR], 1.3; 95% confidence interval [CI], 1.1-1.5). The association of arterial events with recent immobility (HR, 1.4; 95% CI, 1.5-12.1) and cancer (HR, 1.7; 95% CI, 1.4-1.9) was strong. After adjusting for multiple conventional cardiovascular risk factors, provoked VTE, compared with unprovoked VTE, was significantly associated with an increased hazard for MACE (HR, 1.4; 95% CI, 1.1-1.7). Cancer remained a significant adjusted predictor for both MACE (HR, 1.7; 95% CI, 1.4-2.1) and MALE (HR, 2.1; 95% CI 1.01-4.6) in those with provoked VTE., [Conclusions] Among patients with VTE, provoked cases, specifically those with cancer-associated VTE, have an increased risk of major arterial events.

The RIETE investigators., In young patients with acute pulmonary embolism (PE), the predictive value of currently available prognostic tools has not been evaluated. Background Venous thromboembolism (VTE) is a major cause of morbidity and mortality postoperatively. The use of pharmacologic prophylaxis is effective in reducing the incidence of VTE. However, the prophylaxis is often discontinued at hospital discharge, especially for those with benign disease. The implications of this practice are not known. We assessed the data from a large, ongoing registry regarding the time course of VTE and outcomes after noncancer surgery. Methods We analyzed the RIETE (Computerized Registry on Venous Thromboembolism) registry, which includes data from consecutive patients with symptomatic confirmed VTE. In the present study, we focused on general surgical patients who had developed symptomatic postoperative VTE in the first 8 weeks after noncancer surgery. The main objective was to assess the interval between surgery and the occurrence of VTE. Additional variables included the clinical presentation associated with the event, the use of thrombosis prophylaxis, and unfavorable outcomes. Results The data from 3296 patients were analyzed. The median time from surgery to the detection of VTE was 16 days (interquartile range, 8-30 days). Of the VTE events, 77% were detected after the first postoperative week and 27% after 4 weeks. Overall, 43.9% of the patients with VTE had received pharmacologic prophylaxis after surgery for a median of 8 days (interquartile range, 5-14 days), and three quarters of the VTE events were detected after pharmacologic prophylaxis had been discontinued. Overall, 54% of the patients with VTE had presented with pulmonary embolism. For 15% of the patients, the clinical outcome was unfavorable, including 4% who had died within 90 days. Conclusions The risk of VTE after noncancer general surgery remains high for ≤2 months. More than one half of the patients had presented with symptomatic PE as the VTE event, and 15% had had unfavorable outcomes. Only 44% of these patients had received pharmacologic prophylaxis for around 1 week.
